Abstract

Introduction: Inflammation and platelet activation play a pivotal role in the pathogenesis of acute coronary syndrome (ACS). Mounting evidence indicates that microparticles (MP) are potent pro-coagulant molecules modulating inflammatory processes. Aims: The aim of my study was to investigate the intracoronary and systemic microparticle expression in patients with symptomatic coronary heart disease (CHD) and their relationship with markers of inflammation, platelet activation and microvascular dysfunction (MvD). Methods: Forty eight patients with CHD (37 with ACS [23 patients with ST segment myocardial infarction (STEMI) and 14 with non-ST segment elevation myocardial infarction (NSTEMI)] and 11 with stable angina (SA) treated with percutaneous coronary intervention (PCI) were recruited. Blood samples were aspirated sequentially from the right atrium (RA) and the coronary (CO) artery (distal to the culprit lesion). AnnexinV+ MP (AnV+MP) from platelet poor plasma were measured using fluorescent monoclonal antibodies and flow cytometry. Markers of inflammation (hs-CRP, IL-6, TNF-α, serum amyloid antigen (SAA) and platelet activation (soluble p-selectin and platelet monocyte aggregates (PMA))) were measured using ELISA. MvD was assessed by measuring the index of microvascular resistance (IMR) and coronary wedge pressure (Pw). Results: The main novel findings of this study are the demonstration of: 1) differences between MP levels in patients with varying severity of CHD i.e. MP levels were overall higher in those with ACS (more specifically the highest MP levels were observed in the STEMI patients) compared with SA; 2. differential local and systemic AnV+ MP expression in human coronary artery disease; 3. differential local and systemic expression of inflammatory markers in ACS patients; 4. differential local and systemic expression of markers of platelet activation in ACS patients; 5. positive correlation between AnV+MP and markers of inflammation, platelet activation and myocardial necrosis in ACS patients both in the CO and RA; and 6. positive correlation between invasive markers of microvascular dysfunction and PMA in human coronary artery disease. Conclusions: High levels of AnV+MP occur in the coronary artery of patients with ACS. Levels of AnV+MP correlate with severity of the ischaemic lesion since higher MPs were detected in the STEMI versus the NSTEMI and SA groups. Markers of platelet activation and inflammation in ACS patients, both at the site of the culprit lesion and in the systemic circulation, strongly correlated with total and cell specific AnV+MP. The interaction between activated platelets and monocytes with endothelial cells and the subsequent formation of AnV+MP and PMA during ACS would be compatible with a direct pathogenic link between inflammatory and prothrombotic pathways in the pathogenesis of ACS and myocardial necrosis. This may suggest a novel role for MPs as effectors of the inflammatory response and cellular injury in ACS. Our observations also support the hypothesis that PMA formation may be important determinants of platelet activation, inflammation and microvascular dysfunction in coronary artery disease. Whether these MP merely reflect the severity of the ischaemic lesion, and/or are active participants in the pathogenesis of ACS now warrants further study, since these could be important, novel therapeutic targets.

14. Indexed oxygen delivery during pediatric cardiopulmonary bypass is a modifiable risk factor for postoperative acute kidney injury

Abstract

Background: Acute kidney injury after pediatric cardiac surgery is a common complication with few established modifiable risk factors. We sought to characterize whether indexed oxygen delivery during cardiopulmonary bypass was associated with postoperative acute kidney injury in a large pediatric cohort. Methods: This was a retrospective analysis of patients under 1 year old undergoing cardiac surgery with cardiopulmonary bypass between January 1, 2013, and January 1, 2020. Receiver operating characteristic curves across values ranging from 260 to 400 mL/min/m2 were used to identify the indexed oxygen delivery most significantly associated with acute kidney injury risk. Results: We included 980 patients with acute kidney injury occurring in 212 (21.2%). After adjusting for covariates associated with acute kidney injury, an indexed oxygen delivery threshold of 340 mL/min/m2 predicted acute kidney injury in STAT 4 and 5 neonates (area under the curve = 0.66, 95% CI = 0.60 – 0.72, sensitivity = 56.1%, specificity = 69.4%). An indexed oxygen delivery threshold of 400 mL/min/m2 predicted acute kidney injury in STAT 1-3 infants (area under the curve = 0.65, 95% CI = 0.58 – 0.72, sensitivity = 52.6%, specificity = 74.6%). Conclusion: Indexed oxygen delivery during cardiopulmonary bypass is a modifiable variable independently associated with postoperative acute kidney injury in specific pediatric populations. Strategies aimed at maintaining oxygen delivery greater than 340 mL/min/m2 in complex neonates and greater than 400 mL/min/m2 in infants may reduce the occurrence of postoperative acute kidney injury in the pediatric population.

Abstract

Since 1996 we have used a new procedure--the free tracheal autograft technique--to repair long segment congenital tracheal stenosis (LSCTS). The purpose of this review is to examine the intermediate term results of that technique. Between January 1996 and February 1999, 8 infants underwent repair of LSCTS using a free tracheal autograft. Age ranged from 10 days to 12 months (mean age = 5.2 months). The number of complete tracheal rings ranged from 6 to 20 (mean # rings = 15). On cardiopulmonary bypass the trachea was incised anteriorly through the area of stenosis. The midportion of the stenotic trachea was excised and an end-to-end anastomosis was made posteriorly. The excised tracheal segment was trimmed and then sutured in place anteriorly as a free autograft. In 5 patients the autograft was not long enough and the upper portion of the tracheal opening was patched with pericardium. There was one early death 26 days post-op in a 6 month old that had simultaneous repair of Tetralogy of Fallot and required ECMO for 7 days post-op for cardiac failure. The other 7 children are all alive and well at 2-40 months post-op (mean follow-up = 26 months). Two children have tracheostomies at 2 and 32 months post-op. All children have had serial postoperative bronchoscopic exams. Current bronchoscopy shows a widely patent tracheal lumen in all survivors. Abstract

This review discusses the historical aspects, current state of the art, and potential future advances in the areas of nomenclature and databases for congenital heart disease. Five areas will be reviewed: (1) common language = nomenclature, (2) mechanism of data collection (database or registry) with an established uniform core data set, (3) mechanism of evaluating case complexity, (4) mechanism to ensure and verify data completeness and accuracy, and (5) collaboration between medical subspecialties. During the 1990s, both the Society of Thoracic Surgeons (STS) and the European Association for Cardiothoracic Surgery (EACTS) created congenital heart surgery outcomes databases. Beginning in 1998, the EACTS and STS collaborated in the work of the International Congenital Heart Surgery Nomenclature and Database Project. By 2000, a common congenital heart surgery nomenclature, along with a common core minimal data set, were adopted by the EACTS and the STS and published in the Annals of Thoracic Surgery. In 2000, the International Nomenclature Committee for Pediatric and Congenital Heart Disease was established; this committee eventually evolved into the International Society for Nomenclature of Paediatric and Congenital Heart Disease (ISNPCHD). The working component of ISNPCHD is the International Working Group for Mapping and Coding of Nomenclatures for Paediatric and Congenital Heart Disease, also known as the Nomenclature Working Group (NWG). By 2005, the NWG cross-mapped the EACTS-STS nomenclature with the European Paediatric Cardiac Code of the Association for European Paediatric Cardiology and created the International Paediatric and Congenital Cardiac Code (IPCCC) (www.IPCCC.NET). This common nomenclature (IPCCC), and the common minimum database data set created by the International Congenital Heart Surgery Nomenclature and Database Project, are now utilized by both EACTS and STS; since 1998, this nomenclature and database have been used by both the STS and EACTS to analyze outcomes of more than 75,000 patients. Two major multi-institutional efforts have attempted to measure case complexity; the Risk Adjustment in Congenital Heart Surgery-1 and the Aristotle Complexity Score. Efforts to unify these two scoring systems are in their early stages but are encouraging. Collaborative efforts involving the EACTS and STS are under way to develop mechanisms to verify data completeness and accuracy. Further collaborative efforts are also ongoing between pediatric and congenital heart surgeons and other subspecialties, including pediatric cardiac anesthesiologists (via the Congenital Cardiac Anesthesia Society), pediatric cardiac intensivists (via the Pediatric Cardiac Intensive Care Society), and pediatric cardiologists (via the Joint Council on Congenital Heart Disease). Clearly, methods of congenital heart disease outcomes analysis continue to evolve, with continued advances in five areas: nomenclature, database, complexity adjustment, data verification, and subspecialty collaboration.

31. Captopril reduces graft coronary artery disease in a rat heterotopic transplant model

Abstract

Transplant coronary artery disease (CAD) is characterized by severe myointimal proliferation causing vascular stenosis. Nontransplant vascular injury models have shown that angiotensin converting enzyme (ACE) inhibitors reduce myointimal proliferation and preserve lumen integrity. We examined the effect of the ACE inhibitor captopril on graft CAD in a Lewis to F344 rat heterotopic cardiac transplantation model.Twenty-five control rats (group 1) were observed without captopril administration after heterotopic cardiac transplantation, and the other 19 rats (group 2) were administered captopril (50 mg/kg per day in drinking water) after heart transplantation. Graft survival 3 months after transplantation was significantly (P.02) higher in group 2 (18 of 19, 95%) than that in group 1 (16 of 25, 64%). Cellular rejection grades of the heart allografts were significantly higher in group 1 than those in group 2 both at 3 months (grades, 2.5 +/- 0.4 vs 1.3 +/- 0.7; P.01) and 6 months (grades, 2.4 +/- 0.9 vs 0.8 +/- 0.5; P.05) after transplantation. The grades of graft CAD (vascular rejection) were also significantly higher in group 1 than those in group 2 both at 3 months (grades, 2.3 +/- 0.7 vs 0.9 +/- 0.9; P.05) and 6 months (grades, 3.0 +/- 0.9 vs 0.9 +/- 0.3; P.01) after transplantation. The cardiac allographs in group 2 showed minimal intimal proliferation, intact elastic laminae, and reduced smooth muscle cell proliferation.These results suggest that the ACE inhibitor captopril may be effective in prevention of accelerated graft CAD.
