Abstract

COVID-19 induces acute and persistent neurological symptoms in mild and severe cases. Proposed concomitant mechanisms include direct viral infection and strain, coagulopathy, hypoxia, and neuroinflammation. However, underlying molecular alterations associated with multiple neurological outcomes in both mild and severe cases are majorly unexplored. To illuminate possible mechanisms leading to COVID-19 neurological disease, we retrospectively investigated in detail a cohort of 35 COVID-19 mild and severe hospitalized patients presenting neurological alterations subject to clinically indicated cerebrospinal fluid (CSF) sampling. Clinical and neurological investigation, brain imaging, viral sequencing, and cerebrospinal CSF analyses were carried out. We found that COVID-19 patients presented heterogeneous neurological symptoms dissociated from lung burden. Nasal swab viral sequencing revealed a dominant strain at the time of the study, and we could not detect traces of SARS-CoV-2's spike protein in patients' CSF by multiple reaction monitoring analysis. Patients presented ubiquitous systemic hyper-inflammation and broad alterations in CSF proteomics related to inflammation, innate immunity, and hemostasis, irrespective of COVID-19 severity or neuroimaging alterations. Elevated CSF interleukin-6 (IL6) correlated with disease severity (sex-, age-, and comorbidity-adjusted mean Severe 24.5 pg/ml, 95% confidence interval (CI) 9.62–62.23 vs. Mild 3.91 pg/mL CI 1.5–10.3 patients, p = 0.019). CSF tumor necrosis factor-alpha (TNFα) and IL6 levels were higher in patients presenting pronounced neuroimaging alterations compared to those who did not (sex-, age-, and comorbidity-adjusted mean TNFα Pronounced 3.4, CI 2.4–4.4 vs. Non-Pronounced 2.0, CI 1.4–2.5, p = 0.022; IL6 Pronounced 33.11, CI 8.89–123.31 vs Non-Pronounced 6.22, CI 2.9–13.34, p = 0.046). Collectively, our findings put neuroinflammation as a possible driver of COVID-19 acute neurological disease in mild and severe cases.

Abstract

Indirect dark matter detection methods are used to observe the products of dark matter annihilations or decays originating from astrophysical objects where large amounts of dark matter are thought to accumulate. With neutrino telescopes, an excess of neutrinos is searched for in nearby dark matter reservoirs, such as the Sun and the Galactic Centre, which could potentially produce a sizeable flux of Standard Model particles. The KM3NeT infrastructure, currently under construction, comprises the ARCA and ORCA undersea \v{C}erenkov neutrino detectors located at two different sites in the Mediterranean Sea, offshore of Italy and France, respectively. The two detector configurations are optimised for the detection of neutrinos of different energies, enabling the search for dark matter particles with masses ranging from a few GeV/c$^2$ to hundreds of TeV/c$^2$. In this work, searches for dark matter annihilations in the Galactic Centre and the Sun with data samples taken with the first configurations of both detectors are presented. No significant excess over the expected background was found in either of the two analyses. Limits on the velocity-averaged self-annihilation cross section of dark matter particles are computed for five different primary annihilation channels in the Galactic Centre. For the Sun, limits on the spin-dependent and spin-independent scattering cross sections of dark matter with nucleons are given for three annihilation channels.

Abstract

The KM3NeT Collaboration has tackled a common challenge faced by the astroparticle physics community, namely adapting the experiment-specific simulation software to work with the CORSIKA air shower simulation output. The proposed solution is an extension of the open-source code gSeaGen, allowing for the transport of muons generated by CORSIKA to a detector of any size at an arbitrary depth. The gSeaGen code was not only extended in terms of functionalities but also underwent a thorough redesign of the muon propagation routine, resulting in a more accurate and efficient simulation. Abstract

Neutrinos described as an open quantum system may interact with the environment which introduces stochastic perturbations to their quantum phase. This mechanism leads to a loss of coherence along the propagation of the neutrino $-$ a phenomenon commonly referred to as decoherence $-$ and ultimately, to a modification of the oscillation probabilities. Fluctuations in space-time, as envisaged by various theories of quantum gravity, are a potential candidate for a decoherence-inducing environment. Consequently, the search for decoherence provides a rare opportunity to investigate quantum gravitational effects which are usually beyond the reach of current experiments. In this work, quantum decoherence effects are searched for in neutrino data collected by the KM3NeT/ORCA detector from January 2020 to November 2021. The analysis focuses on atmospheric neutrinos within the energy range of a few GeV to $100\,\mathrm{GeV}$. Adopting the open quantum system framework, decoherence is described in a phenomenological manner with the strength of the effect given by the parameters $\Gamma_{21}$ and $\Gamma_{31}$. Following previous studies, a dependence of the type $\Gamma_{ij} \propto (E/E_0)^n$ on the neutrino energy is assumed and the cases $n = -2,-1$ are explored. No significant deviation with respect to the standard oscillation hypothesis is observed. Therefore, $90\,\%$ CL upper limits are estimated as $\Gamma_{21} < 4.6\cdot 10^{-21}\,$GeV and $\Gamma_{31} < 8.4\cdot 10^{-21}\,$GeV for $n = -2$, and $\Gamma_{21} < 1.9\cdot 10^{-22}\,$GeV and $\Gamma_{31} < 2.7\cdot 10^{-22}\,$GeV for $n = -1$, respectively., Comment: 17 pages, 5 figures

Abstract

ABSTRACT: Objectives: Candida tropicalis is one of the three most frequent species causing candidaemia in Latin America. Despite the high prevalence of C. tropicalis in candidaemia cases in Brazil, little is known about the trends in fluconazole susceptibility over time. The objective of this study was to evaluate temporal trends in azole resistance rates among C. tropicalis bloodstream isolates from patients treated in six Brazilian medical centres over a 12-year period. Methods: We selected 200 C. tropicalis bloodstream isolates from six medical centres in Brazil collected between 2007 and 2018. Species identification was confirmed by MALDI-TOF/MS. Antifungal susceptibility testing for four antifungal agents was performed by the Clinical and Laboratory Standards Institute (CLSI) microbroth method. Results: Overall, rates of non-susceptibility were 4% and 3.5% to fluconazole and voriconazole, respectively. All isolates were susceptible to amphotericin B and only one isolate was resistant to echinocandins. Conclusion: Although we failed to demonstrate statistical differences in the rates of azole resistance documented during the period of analysis, trends towards lower susceptibility to fluconazole and voriconazole were shown.

Abstract

KM3NeT/ORCA is a water Cherenkov neutrino detector under construction and anchored at the bottom of the Mediterranean Sea. The detector is designed to study oscillations of atmospheric neutrinos and determine the neutrino mass ordering. This paper focuses on an initial configuration of ORCA, referred to as ORCA6, which comprises six out of the foreseen 115 detection units of photo-sensors. A high-purity neutrino sample was extracted, corresponding to an exposure of 433 kton-years. The sample of 5828 neutrino candidates is analysed following a binned log-likelihood method in the reconstructed energy and cosine of the zenith angle. The atmospheric oscillation parameters are measured to be $\sin^2\theta_{23}= 0.51^{+0.04}_{-0.05}$, and $ \Delta m^2_{31} = 2.18^{+0.25}_{-0.35}\times 10^{-3}~\mathrm{eV^2} \cup \{-2.25,-1.76\}\times 10^{-3}~\mathrm{eV^2}$ at 68\% CL. Abstract

The use of drones for vegetation monitoring allows the acquisition of large amounts of high spatial resolution data in a simple and fast way. In this study, we evaluated the accuracy of vegetation cover estimation by drones in Mediterranean semi-arid shrublands (Sierra de Filabres; Almería; southern Spain) after prescribed burns (2 years). We compared drone-based vegetation cover estimates with those based on traditional vegetation sampling in ninety-six 1 m2 plots. We explored how this accuracy varies in different types of coverage (low-, moderate- and high-cover shrublands, and high-cover alfa grass steppe); as well as with diversity, plant richness, and topographic slope. The coverage estimated using a drone was strongly correlated with that obtained by vegetation sampling (R2 = 0.81). This estimate varied between cover classes, with the error rate being higher in low-cover shrublands, and lower in high-cover alfa grass steppe (normalized RMSE 33% vs. 9%). Diversity and slope did not affect the accuracy of the cover estimates, while errors were larger in plots with greater richness. These results suggest that in semi-arid environments, the drone might underestimate vegetation cover in low-cover shrublands.

Abstract

Background: Cardiovascular comorbidities such as hypertension and inflammatory response dysregulation are associated with worse COVID-19 prognoses. Different cytokines have been proposed to play vital pathophysiological roles in COVID-19 progression, but appropriate prognostic biomarkers remain lacking. We hypothesized that the combination of immunological and clinical variables at admission could predict the clinical progression of COVID-19 in hypertensive patients.Methods: The levels of biomarkers, including C-reactive protein, lymphocytes, monocytes, and a panel of 29 cytokines, were measured in blood samples from 167 hypertensive patients included in the BRACE-CORONA trial. The primary outcome was the highest score during hospitalization on the modified WHO Ordinal Scale for Clinical Improvement. The probability of progression to severe disease was estimated using a logistic regression model that included clinical variables and biomarkers associated significantly with the primary outcome.Results: During hospitalization, 13 (7.8%) patients showed progression to more severe forms of COVID-19, including three deaths. Obesity, diabetes, oxygen saturation, lung involvement on computed tomography examination, the C-reactive protein level, levels of 15 cytokines, and lymphopenia on admission were associated with progression to severe COVID-19. Elevated levels of interleukin-10 and interleukin-12 (p70) combined with two or three of the abovementioned clinical comorbidities were associated strongly with progression to severe COVID-19. The risk of progression to severe disease reached 97.5% in the presence of the five variables included in our model.Conclusions: This study demonstrated that interleukin-10 and interleukin-12 (p70) levels, in combination with clinical variables, at hospital admission are key biomarkers associated with an increased risk of disease progression in hypertensive patients with COVID-19.

Abstract

The maintenance of pancreatic islet architecture is crucial for proper β-cell function. We previously reported that disruption of human islet integrity could result in altered β-cell identity. Here we combine β-cell lineage tracing and single-cell transcriptomics to investigate the mechanisms underlying this process in primary human islet cells. Using drug-induced ER stress and cytoskeleton modification models, we demonstrate that altering the islet structure triggers an unfolding protein response that causes the downregulation of β-cell maturity genes. Collectively, our findings illustrate the close relationship between endoplasmic reticulum homeostasis and β-cell phenotype, and strengthen the concept of altered β-cell identity as a mechanism underlying the loss of functional β-cell mass.

Dengue (DENV) and chikungunya viruses (CHIKV) cause severe neurological complications, sometimes undiagnosed. Therefore, the use of more accessible neuroinflammatory biomarkers can be advantageous considering their diagnostic and prognostic potential for aggravated clinical outcomes. In this study, we aimed to evaluate neopterin and C-X-C motif chemokine ligand 10 (CXCL-10) in cerebrospinal fluid (CSF) for the diagnosis of neuroinvasive DENV and CHIKV. We analyzed the CSF of 66 patients with neurological disorders, comprising 12 neuroinvasive DENV/CHIKV, 20 inflammatory control (viral, bacterial, and fungal meningitis, and autoimmune disorders), and 24 noninflammatory control (cerebrovascular disease, dementia, neoplasm). There was no difference between the concentration of CSF neopterin in the neuroinvasive DENV/CHIKV and control groups. However, there was a significant difference in the CXCL-10 level when comparing the neuroinvasive DENV/CHIKV group and the non-inflammatory control (p < 0.05). Furthermore, we found a linear correlation between neopterin and CXCL-10 CSF levels in the three groups. For the DENV/CHIKV neuroinvasive diagnosis, the ROC curve showed the best cut-off values for CSF neopterin at 11.23 nmol/L (sensitivity of 67% and specificity of 63%), and for CSF CXCL-10 at 156.5 pg/mL (91.7% sensitivity and specificity). These results show that CXCL-10 in CSF represents an accurate neuroinflammatory biomarker that may contribute to neuroinvasive DENV/CHIKV diagnosis.

Abstract

Zika virus (ZIKV) transmission through non-mosquito-dependent routes has become increasingly important since reports of sexual transmission. Breastfeeding is a potential means of ZIKV transmission, but data on this remain limited. The cases of four mothers with laboratory-proven infections are reported. No disease evolved in three of the breastfed babies despite detectable maternal viremia and viruria, the presence of viral RNA shedding, and the isolation of infective particles in one milk sample. Fever and rash in one infant of a ZIKV-infected mother proved to be related to chikungunya virus infection. The results suggest that the presence of infective particles in breast milk may not be sufficient for the efficient perinatal transmission of ZIKV.

Abstract

Zika virus (ZIKV) infection usually presents as a mild and self-limited illness, but it may be associated with severe outcomes. We describe a case of a 30-year-old man with systemic erythematous lupus and common variable immunodeficiency who became infected with both Zika (ZIKV) and Chikungunya (CHIKV) virus during the 2016 outbreak in Rio de Janeiro, Brazil. The patient presented with intense wrist and right ankle arthritis, and ZIKV RNA and virus particles were detected in synovial tissue, blood and urine, and CHIKV RNA in serum sample, at the time of the diagnosis. During the follow up, ZIKV RNA persisted for 275 days post symptoms onset. The patient evolved with severe arthralgia/arthritis and progressive deterioration of renal function. Fatal outcome occurred after 310 days post ZIKV and CHIKV co-infection onset. The results show the development of severe disease and fatal outcome of ZIKV infection in an immunosuppressed adult. The data suggests a correlation between immunodeficiency and prolonged ZIKV RNA shedding in both blood and urine with progressive disease. The results also indicate a possible role for arbovirus co-infections as risk factors for severe and fatal outcomes from ZIKV infection.

Abstract

Meningitis and encephalitis are complications of West Nile virus (WNV) infection. Although WNV is endemic in North America, the virus has recently been reported in Colombia and Argentina. Investigation of WNV in Brazil is important since this virus has never been studied previously in this country. OBJECTIVE: To investigate the presence of WNV infection in viral encephalitis/meningitis cases of unknown etiology in the city of Rio de Janeiro, Brazil. METHOD: Thirty-seven adults with viral meningitis/encephalitis had their serum and CSF tested for WNV antibodies using the ELISA method. RESULTS: Only one case was WNV-positive, but this case was also positive for dengue. The plaque reduction neutralization test distinguished infections, and was negative for WNV. CONCLUSION: WNV can be confused with dengue infection. Their symptoms and neurological picture are similar. Embora o VON seja endêmico na América do Norte, recentemente o vírus foi descrito na Colômbia e Argentina. Sua pesquisa no Brasil é importante uma vez que o vírus nunca fora estudado antes em nosso país. OBJETIVO: Investigar a presença do VON em casos de meningite e encefalite viral de etiologia desconhecida, na cidade no Rio de Janeiro, Brasil. MéTODO: Trinta e sete adultos com quadro de meningite/encefalite tiveram seu LCR e soro testados para anticorpos anti-VON, pelo método ELISA. RESULTADOS: Apenas um caso obteve sorologia positiva para VON, mas a sorologia para dengue também fora positiva. O teste da neutralização por redução de placa foi utilizado para distinção entre as infecções, sendo negativo para VON. CONCLUSÃO: A infecção por VON pode ser confundida com a infecção pelo vírus da dengue, seus sintomas e quadro neurológico são similares. Nós não encontramos o VON em pacientes com meningite/ encefalite na cidade do Rio de Janeiro. Até o momento ele ainda não foi identificado no Brasil

Abstract

Polynomial expansions are important in the analysis of neural network nonlinearities. They have been applied thereto addressing well-known difficulties in verification, explainability, and security. Existing approaches span classical Taylor and Chebyshev methods, asymptotics, and many numerical approaches. We find that while these individually have useful properties such as exact error formulas, adjustable domain, and robustness to undefined derivatives, there are no approaches that provide a consistent method yielding an expansion with all these properties. To address this, we develop an analytically modified integral transform expansion (AMITE), a novel expansion via integral transforms modified using derived criteria for convergence. We show the general expansion and then demonstrate application for two popular activation functions, hyperbolic tangent and rectified linear units. Compared with existing expansions (i.e., Chebyshev, Taylor, and numerical) employed to this end, AMITE is the first to provide six previously mutually exclusive desired expansion properties such as exact formulas for the coefficients and exact expansion errors (Table II). We demonstrate the effectiveness of AMITE in two case studies. First, a multivariate polynomial form is efficiently extracted from a single hidden layer black-box Multi-Layer Perceptron (MLP) to facilitate equivalence testing from noisy stimulus-response pairs. Second, a variety of Feed-Forward Neural Network (FFNN) architectures having between 3 and 7 layers are range bounded using Taylor models improved by the AMITE polynomials and error formulas. Abstract

Community-based natural resource governance is increasingly valued by the leading international organizations that promote environmental conservation and sustainable development. At the same time, the ecosystems of the northeastern Andalusian mountains are intrinsically related to the long-standing presence of pastoralism and its different communal forms that have favored a very particular biocultural diversity and sustainable socio-ecological systems locally. Through a transdisciplinary anthropological and ecological study, we aimed to compare how different types of communal governance and pastoral mobilities impact pasturelands in the region. We focused on three contiguous mountain pastoral commons, those of Castril, Santiago de la Espada, and Pontones, which exhibit different forms of communal organization and two main transhumant types of mobility: long-distance and short-distance transhumance. We conducted long-term ethnographic fieldwork with year-round participatory observations of social life, pastoral practices, and governance systems, and performed botanical and soil analyses. As a general result, we found that local pastoralism positively impacts the environment. There were differences between commons and mobility regimes, with a positive impact related mainly to closer forms of cooperation between herders and daily guiding of flocks, along with seasonal long-distance transhumance, especially when the latter is combined with stricter formal community rules. Within the current context highly determined by public subventions, particularly from the EU, primarily directed at pastureland management’s administrative and economic aspects, local governance increasingly favors these methods over the conservation of traditional, sustainable uses of pastures. Abstract

Dataset of annual monitoring of herbivory effects on the conservation status of the endangered species Androcymbiumeuropaeum (Lange) K. Richt and its associated plant communities is presented in this manuscript. This dataset encompasses the annual monitoring of herbivory effects on the conservation status of the endangered species Androcymbiumeuropaeum. Since 2010, the SERPAM Department (Service of Evaluation, Restoration and Protection of Mediterranean Agrosystems) at the Zaidin Experimental Station, belonging to the Spanish National Research Council (CSIC-EEZ), has conducted annual sampling to assess the impact of both domestic and wild livestock, specifically rabbits, on the pastures where A.europaeum lives. The study consisted of a randomised block design, implementing three distinct treatments to evaluate different management strategies: (1) rabbit and domestic herbivory, (2) exclusion of domestic livestock and (3) exclusion of rabbits and domestic livestock. Within each treatment, two types of monitoring were conducted. Firstly, the abundance of A.europaeum was estimated by counting individuals within 50 cm x 50 cm quadrats. Secondly, plant species diversity was assessed along 2-m long transects using the modified Point-Quadrat method. The research was conducted within the Cabo de Gata-Níjar Natural Park in southern Spain, specifically in the Amoladeras Nature Reserve in Almería.

Abstract

This article examines the pedagogical politics at play in the quantification of faculty labor in contemporary U.S. academia. It focuses on the author's experiences at a large, land-grant, R1 university, going through the Promotion and Tenure (P&T) review process and reflecting on the various kinds of responsibilities that emerged from structural and personal transformations during the COVID-19 pandemic. By developing a pedagogy of accountability against quantitative compartmentalization, the essay outlines how to posit care, empowerment, and reflexivity as central to all learning, especially for teachers and particularly in times of crisis. In this context, 'learning how to count' means going against faceless metrics of one-to-one correspondence with other faculty or peer institutions and instead embracing what Paulo Freire called the 'civic courage' to teach as a commitment to others and the self. Abstract

Objective To evaluate the impact of different volumes of indocyanine green (ICG) on the detection rate and bilateral mapping of sentinel lymph nodes in patients with apparent uterine-confined endometrial cancer. Methods All patients who underwent surgical staging with sentinel node mapping in six reference centers were included. Two different protocols of ICG intracervical injection were used: (1) 2 mL group: total volume of 2 mL injected superficially; (2) 4 mL group: total volume of 4 mL, 2 mL deeply and 2 mL superficially. Logistic regression was used to analyze factors that could influence dye migration and detection rates. A sensitivity analysis was carried out to determine how independent variables could affect the sentinel node detection rate. Results Of 442 eligible patients, 352 were analyzed (172 in the 2 mL group and 180 in the 4 mL group). The bilateral detection rates of the 2 mL and 4 mL groups were 84.9% and 86.1%, respectively (p=0.76). The overall detection rate was higher with a volume of 4 mL than with 2 mL (92.4% vs 97.8%, respectively; p=0.024). In the univariate analysis the rate of bilateral mapping fell from 87.5% to 73.5% when the International Federation of Gynecology and Obstetrics (FIGO) 2009 tumor stage was >IB (p=0.018). In the multivariate analysis, for both overall and bilateral detection rates a statistically significant difference emerged for the volume of ICG injected and FIGO 2009 stage >IB. Increasing body mass index was associated with worse overall detection rates on univariate analysis (p=0.0006), and significantly decreased from 97% to 91% when the body mass index exceeded 30 kg/m(2) (p=0.05). Conclusions In patients with early-stage endometrial cancer, a volume of 2 mL ICG does not seem to compromise the bilateral detection of sentinel lymph nodes. In women with obesity and FIGO 2009 stage >IB, a 4 mL injection should be preferred.

Abstract

The aim of this study was to assess the impact of low-volume metastasis (LVM) on disease-free survival (DFS) in women with apparent early-stage endometrial cancer (EC) who underwent sentinel lymph node (SLN) mapping. Patients with pre-operative early-stage EC were retrospectively collected from an international collaboration including 13 referring institutions. A total of 1428 patients were included in this analysis. One hundred and eighty-six patients (13%) had lymph node involvement. Fifty-nine percent of positive SLN exhibited micrometastases, 26.9% micrometastases, and 14% isolated tumor cells. Seventeen patients with positive lymph nodes did not receive any adjuvant therapy. At a median follow-up of 33.3 months, the disease had recurred in 114 women (8%). Patients with micrometastases in the lymph nodes had a worse prognosis of disease-free survival compared to patients with negative nodes or LVM. The rate of recurrence was significantly higher for women with micrometastases than those with low-volume metastases (HR = 2.61; p = 0.01). The administration of adjuvant treatment in patients with LVM, without uterine risk factors, remains a matter of debate and requires further evaluation.
