ClassyFarm is an integrated surveillance system for monitoring the Italian livestock farms on risks related to antimicrobial use (AMU) and resistance (AMR) and other indicators, such as animal welfare and farm biosecurity. In the framework of the CoEvalAMR network we evaluated ClassyFarm by using three evaluation tools: the OH-EpiCap, the FAO PMP-AMR and the NEOH evaluation tool. Evaluation was performed by interviewing representatives of ClassyFarm, National Health System veterinarians, farm vets, farmers, livestock industry entrepreneurs, academy experts in AMR/AMU. NEOH and PMP-AMR evaluation tools were applied twice (at 2 years interval) to detect changes in the implementation of the system over time, while OH-EpiCap was applied once. The three tools differ in evaluation objectives, depth of analysis, and time/training resources needed to perform the evaluation. However, each of them enables to gather different information, which can serve as a basis for the discussion of possible adaptations/improvements of ClassyFarm. Even though ClassyFarm has a limited degree of One Health (OH) implementation, the evaluation showed that the system has evolved from a barely biosecurity and welfare surveillance system towards a more integrated OH approach. A transdisciplinary nature is also emerging, with the involvement of the environmental sector. However, there is space for improvement in resource allocation, data sharing and communication. Beyond the structural evaluation, surveillance systems should be evaluated also in relation to cost-effectiveness and system impacts.

The interaction between lamin A and the cytoplasmic skeleton plays a key role in maintaining nuclear mechanical properties. However, the effect of destruction of the cytoplasmic skeleton on the 3D submicroscopic structure of lamin A has not been elucidated. In this study, we developed an image quantization algorithm to quantify changes in the submicroscopic structure of the intact lamin A 3D network within the nucleus. We used blebbistatin or nocodazole to disrupt the fibrillar structure of F-actin or tubulin, respectively, and then quantified changes in the lamin A super-resolution network structure, the morphological and mechanical properties of the nucleus and the spatial distribution of chromosomes. Ultimately, we found for the first time that disruption of the cytoplasmic skeleton changes the lamin A submicroscopic network and nuclear structural characteristics. In summary, this study contributes to understanding the trans-nuclear membrane interaction characteristics of lamin A and the cytoplasmic skeleton.

Abstract Bromodomain and extra-terminal domain (BET) proteins are therapeutic targets in several cancers including the most common malignant adult brain tumor glioblastoma (GBM). Multiple small molecule inhibitors of BET proteins have been utilized in preclinical and clinical studies. Unfortunately, BET inhibitors have not shown efficacy in clinical trials enrolling GBM patients. One possible reason for this may stem from resistance mechanisms that arise after prolonged treatment within a clinical setting. However, the mechanisms and timeframe of resistance to BET inhibitors in GBM is not known. To identify the temporal order of resistance mechanisms in GBM we performed quantitative proteomics using multiplex-inhibitor bead mass spectrometry and demonstrated that intrinsic resistance to BET inhibitors in GBM treatment occurs rapidly within hours and involves the fibroblast growth factor receptor 1 (FGFR1) protein. Additionally, small molecule inhibition of BET proteins and FGFR1 simultaneously induces synergy in reducing GBM tumor growth in vitro and in vivo. Further, FGFR1 knockdown synergizes with BET inhibitor mediated reduction of GBM cell proliferation. Collectively, our studies suggest that co-targeting BET and FGFR1 may dampen resistance mechanisms to yield a clinical response in GBM.

This paper describes a methodology and a Decision Support Tool (DST) for the assessment of the impacts of future scenarios of public electric mobility on the electric Distribution Network (DN) of a city, aimed to support the implementation strategies of the electric charging network for the Local Public Transport (LPT).The objective of the proposed tool is supporting the decision makers in estimating the effect of the future development of electromobility on the urban electric grid considering the evolution of both electric buses and their charging infrastructure.Some scenarios for the development of electric charging infrastructure for LPT were identified together with the local public transport operator and the energy provider. The results, which are of interest to both the local Public Transport Operator (PTO) and the local Distribution System Operator (DSO), have been quantified for the city of Turin (Italy), but the methodology can be adopted in any urban context: DSOs and PTOs of each city can use the proposed tool as a support to define their public transport electrification strategies.Some results from the case study examined in this paper (Turin, Italy) show that the energy demand related to the electrification of LPT on a typical weekday is about 45 MWh in 2030 (corresponding to 40 % e-bus), and about 90 MWh when the entire bus fleet is electric. It is also shown how more distributed recharging during the day (with intermediate recharging at the terminus) can dampen the energy/power peaks required at the depot compared to overnight recharging alone: a reduction of up to 27 % of energy demand at the depot is observed due to opportunity charging.

Despite intensive investigation, in several cases the pathogenesis remains unknown, but frequently pacemaker implantation is still indicated. Increasing evidence points to circulating anti‐Ro/Sjögren syndrome–related antigen A (SSA) antibodies cross‐reacting with L‐type calcium channel and inhibiting the related current as an epidemiologically relevant and potentially reversible cause of isolated AVB in adults. Angioleiomyoma is a benign soft-tissue tumor that rarely develops in the respiratory tract. Here, we report a case of a 51-year-old female with an angioleiomyoma developed in the left lobar bronchial branch and extended to the left principal bronchus, causing nonspecific symptoms, and not visible on the chest X-ray examination. The suspected diagnosis was established by high-resolution computed tomography and confirmed by the histological evaluation of the endoscopically removed lesion.

Abstract Opioid use is only recommended in selected cases of musculoskeletal (MSK) injuries. We assessed factors associated with increased opioid use in MSK injuries. In a retrospective analysis of over four million workers with MSK injuries using the Swiss National Accident Insurance Fund (Suva) database, we analyzed risk factors by multivariate logistic regression. Injury severity was associated with pain medication, opioid, and strong opioid use. Whereas fractures, contusions, and ruptures had higher odds for any pain medication use, increased odds for strong opioids were observed in fractures, superficial injuries, and other injuries. Injuries of the shoulders, elbow, chest, back/spine, thorax, and pelvis/hips showed high odds for opioid use (odds ratio (OR) > 2.0). Injuries of the shoulders had higher odds for strong opioid use (OR 1.136; 95% CI 1.040–1.241). The odds for using strong opioids increased from 2008 OR 0.843 (95% confidence interval (CI) 0.798–0.891) to 2018 OR 1.503 (95% CI 1.431–1.578), compared to 2013. Injury severity, type of injury, and injured body parts influenced the use of pain medication and overall opioid use in musculoskeletal injuries. Strong opioids were more often used in fractures but also in superficial and other minor injuries, which indicates that other factors play a role when prescribing strong opioids.

Abstract Smart protective coatings and devices are currently of great interest. In particular, they can absorb or reflect harmful waves of electromagnetic interference (EMI). In this work, novel binary and ternary composites with highly amorphous poly(vinyl alcohol) (HAVOH) as a matrix and single-walled carbon nanotubes (SWCNTs) and MXenes as nanofillers were prepared. HAVOH is a recently patented kind of poly(vinyl alcohol) (PVOH) that was modified with diol monomers. MXenes are a new type of inorganic two-dimensional (2D) nanoparticle consisting of carbides, nitrides and carbonitrides. Three series of composites, HAVOH/SWCNTs, HAVOH/MXenes and HAVOH/SWCNTs/MXenes, were prepared using the solvent casting method. Samples were tested with various methods to study their structure, electrical properties, thermal behavior and EMI-shielding properties. HAVOH/3.0 wt.% SWCNTs/3.0 wt.% MXene specimens revealed a shielding effectiveness of 55 dB, which is 122 times better than that of the neat matrix. These results are promising for the fabrication of films with protective effects against EMI.

The Mandorla di Avola is recognized all over the world as one of the best almond varieties. It is cultivated in a small area inside the provinces of Siracusa and Ragusa (Sicily, southern Italy). It is used in traditional Sicilian cuisine for both salty and sweet foods and of course in artisan pastry, apart from being consumed as a fruit. Due to its extraordinary organoleptic and beneficial features, the Mandorla di Avola is frequently counterfeit with almond varieties of lower quality coming from other countries. While its nutraceutical features have been studied, the possibility of authenticating it with respect to other varieties has not been explored. In this work, we used microelements determined with ICP-OES and ICP-MS as chemical descriptors to distinguish samples of Mandorla di Avola almonds from almonds coming from California and Spain, which are usually employed as substitutes in pastry. Among the different elements determined, Mn and P were found to be the best descriptors for authentication.

Background Although accumulating data indicate that IL‐6 (interleukin‐6) can promote heart rate‐corrected QT interval (QTc) prolongation via direct and indirect effects on cardiac electrophysiology, current evidence comes from basic investigations and small clinical studies only. Therefore, IL‐6 is still largely ignored in the clinical management of long‐QT syndrome and related arrhythmias. The aim of this study was to estimate the risk of QTc prolongation associated with elevated IL‐6 levels in a large population of unselected subjects. Methods and Results An observational study using the Veterans Affairs Informatics and Computing Infrastructure was performed. Participants were US veterans who had an ECG and were tested for IL‐6. Descriptive statistics and univariate and multivariate regression analyses were performed to study the relationship between IL‐6 and QTc prolongation risk. Study population comprised 1085 individuals, 306 showing normal (25 pg/mL) IL‐6 levels. Subjects with elevated IL‐6 showed a concentration‐dependent increase in the prevalence of QTc prolongation, and those presenting with QTc prolongation exhibited higher circulating IL‐6 levels. Stepwise multivariate regression analyses demonstrated that increased IL‐6 level was significantly associated with a risk of QTc prolongation up to 2 times the odds of the reference category of QTc (e.g. QTc >470 ms men/480 ms women ms: odds ratio, 2.28 [95% CI, 1.12–4.50] for IL‐6 >25 pg/mL) regardless of the underlying cause. Specifically, the mean QTc increase observed in the presence of elevated IL‐6 was quantitatively comparable (IL‐6 >25 pg/mL:+6.7 ms) to that of major recognized QT‐prolonging risk factors, such as hypokalemia and history of myocardial infarction. Conclusions Our data provide evidence that a high circulating IL‐6 level is a robust risk factor for QTc prolongation in a large cohort of US veterans, supporting a potentially important arrhythmogenic role for this cytokine in the general population.

Recently an increasing number of goods and services tends to be offered as a service, of which one of the most promising is mobility. The Mobility as a Service (MaaS) paradigm provides the integration of most of the mobility services available within an urban/rural area in a single subscription. The interest of Piedmont Region citizens towards MaaS is investigated through a certain number of choice situations (DCE – Discrete Choice Experiment) in which the respondent must select an alternative from the given set, based on the characteristics of each alternative. To complete the survey, a number of classic questions (multiple choice and open-ended questions) is also submitted.The goal of the surveys is to understand the perception towards MaaS and identify the relevance of each modal alternative, to better design the composition of mobility bundles.The research showed that Piedmont citizens are strongly interested in MaaS solutions and that bundles with highest interest include Local Public Transport (LPT) as a base, with sharing services (car, bike, scooter) in urban areas and car rental or car-sharing in suburban areas.

Primary hemophagocytic lymphohistiocytosis (pHLH) is a severe, life-threatening hyperinflammatory syndrome caused by defects in genes of the granule-dependent cytotoxic pathway. Here we investigated the clinical presentation and outcome in a large cohort of 143 patients with pHLH diagnosed in the last 15 years and enrolled in the Italian registry. The median age at diagnosis was 12 months (IQR 2-81), and ninety-two patients (64%) fulfilled the HLH-2004 criteria. Out of 111 patients who received first-line combined therapy (HLH-94, HLH-2004, Euro-HIT protocols), 65 (59%) achieved complete response (CR) and 21 (19%) partial response (PR). Thereafter, 33 patients (30%) reactivated, and 92 (64%) received HSCT, 78 of whom (85%) survived and were alive at a median follow-up from diagnosis of 67 months. Thirty-six patients (25%) died before HSCT and 14 (10%) after. Overall, 93 patients (65%) were alive after a median follow-up of 30 months. Unadjusted predictors of non-response were age

IntroductionAntarctic Porifera have gained increasing interest as hosts of diversified associated microbial communities that could provide interesting insights on the holobiome system and its relation with environmental parameters.MethodsThe Antarctic demosponge species Haliclona dancoi and Haliclona scotti were targeted for the determination of persistent organic pollutant (i. e., polychlorobiphenyls, PCBs, and polycyclic aromatic hydrocarbons, PAHs) and trace metal concentrations, along with the characterization of the associated prokaryotic communities by the 16S rRNA next generation sequencing, to evaluate possible relationships between pollutant accumulation (e.g., as a stress factor) and prokaryotic community composition in Antarctic sponges. To the best of our knowledge, this approach has been never applied before.ResultsNotably, both chemical and microbiological data on H. scotti (a quite rare species in the Ross Sea) are here reported for the first time, as well as the determination of PAHs in Antarctic Porifera. Both sponge species generally contained higher amounts of pollutants than the surrounding sediment and seawater, thus demonstrating their accumulation capability. The structure of the associated prokaryotic communities, even if differing at order and genus levels between the two sponge species, was dominated by Proteobacteria and Bacteroidota (with Archaea abundances that were negligible) and appeared in sharp contrast to communities inhabiting the bulk environment.DiscussionsResults suggested that some bacterial groups associated with H. dancoi and H. scotti were significantly (positively or negatively) correlated to the occurrence of certain contaminants.

Livestock farming substantially contributes to the global economy and food security. However, it poses crucial environmental, animal welfare, and public health challenges. The main objective of this study is to quantify the global antimicrobial use (AMU) in cattle, chicken, and pig farming. This information is important for understanding the potential impact of farm AMU on the emergence and spread of antimicrobial resistance among animals and humans. Using the United States Department of Agriculture Production, Supply, and Distribution and the Food and Agriculture Organization databases, we estimated the total supply of cattle (in heads) and its distribution into four weight categories: calves (26%), cows (41%), heifers (4%), and bulls of more than one year (29%). Similarly, we calculated the total supply of pigs (in heads) and divided it into two weight categories: pigs (96%) and sows (4%). For chickens, we considered one weight category. We attributed to each category a standard weight according to the parameters set by the European Medicines Agency (EMA) to determine the animal biomass at risk of antimicrobial treatment, or population correction unit (PCU). Finally, we estimated the global PCUs and then the global AMU based on the average administered to the three species (in mg of active ingredients per kg PCU). With this method, we estimated a global annual AMU of 76 060 tonnes of antimicrobial active ingredients (2019–2021 average), of which 40 697 tonnes (or 53.5%) for cattle, 4 243 tonnes (or 5.6%) for chickens, and 31 120 tonnes (or 40.9%) for pigs. According to our assessment, global AMU leads to almost 20 000 tonnes less than the previous estimates due to a different evaluation of PCUs. In previous studies, PCUs were calculated on the liveweight at slaughtering of animals, while our method considers the age and sex of animals and their average weight at treatment. Our results are consistent with the World Organization for Animal Health (WOAH) estimate of 76 704 tonnes of veterinary antimicrobials globally consumed in 2018 for the total of food-producing animals (the WOAH estimation is based on sales and import data).

The following article aims to provide a conceptually rooted introduction to the articles to be published in the internationally coordinated themed collection on 'Labour conflict, class and collective organization', an initiative which has involved four journals focusing on labour studies from different geographical angles and academic traditions: Economic and Labour Relations Review (ELRR); Global Labour Journal (GLJ); Partecipazione e Conflitto (PACO); and Revista Latino Americana de Estudios del Trabajo (RELET). The contributions to be published across the four journals are diverse, both in terms of geographical focus, disciplinary perspectives and sector of analysis.This diversity is very welcomed and represents a fertile soil for conceptual considerations, because it corresponds to the manifold forms in which labour conflict expresses itself in the reality of capitalism. What's the abstract unity of these concrete empirical realities, as Marx would have put this? In the following introduction we focus on two general theoretical issues we consider fundamental and mutually interrelated: a rethinking of workers' collective forms of organization within and beyond trade unions; the framing of these forms and of labour conflict in the broader historical dynamics of working classes formation. With this, we hope to provide a lens of analysis for the papers in the international special issue and more in general a methodological guidance to future studies on labour conflict.

Background/Objectives: Arterial stiffness (AS) is an independent predictor of cardiovascular events and is associated with a poor prognosis. While AS may represent a novel therapeutic target, recent evidence shows that it is sexually dimorphic. The aim of this study was to evaluate relative sex differences in arterial stiffness and their possible impact on the outcome of acute ischemic stroke. Methods: We retrospectively evaluated a cohort of adult patients with the following inclusion criteria: acute ischemic stroke, which occurred within 24 h from the onset of symptoms, confirmed through neuroimaging examinations, additional evaluations including extracranial and transcranial arterial ultrasound examinations, transthoracic echocardiography, a 12-lead resting ECG, and continuous 24 h in-hospital blood pressure monitoring. Based on the 24 h blood pressure monitoring, the following parameters were evaluated: systolic blood pressure, diastolic blood pressure, mean blood pressure, pulse pressure, and arterial stiffness index (ASI). The modified Rankin scale (mRS) was assessed at 90 days to evaluate the 3-month clinical outcome, defining an unfavorable outcome as an mRS score ≥ 3. To assess the factors associated with unfavorable outcomes, a stepwise logistic regression model was performed on the total sample size, and the analyses were replicated after stratifying by sex. Results: A total of 334 patients (176 males, 158 females) were included in the analysis. There was a significant sex-dependent impact of ASI on the 90-day unfavorable Rankin score (mRS score ≥ 3) as only men had a reduced likelihood of favorable outcomes with increasing arterial stiffness (OR:1.54, 95% CI: 1.06–2.23; P-interaction = 0.023). Conclusions: The influence of ASI on the 3-month functional outcome after acute ischemic stroke is at least in part sex-related, suggesting that, in males, higher ASI values are associated with a worse outcome.

As athletes pursue excellence, training techniques continue to advance, making structured physical activity an essential tool for enhancing performance. To optimize athletic performance in modern competitive sports, the balance of physical performance and mental clarity is required. This study seeks to examine the effects of High-Intensity Interval Training (HIIT) on cognitive and physical skills in basketball and soccer players. A 3-week HIIT protocol was incorporated based on the Wingate technique. This study included 10 soccer players and 10 basketball players with an average age of 22.79 ± 1.90 years. Participants performed pre- and post-intervention assessments. Physical proficiency was assessed using 20 m sprint, change-of-direction (COD) and dribbling tests, while cognitive skills were assessed using motion object tracking (MOT), working memory, perceptual load (PL), and attention window (AW) tests. The HIIT intervention significantly improved cognitive performance in particular; noteworthy observations were a 15% improvement in motion object tracking test scores and a 16% increase in working memory test scores for basketball players. The attention window test scores showed a 32% increase, and perceptual load test scores were 31% decreased for soccer players post-intervention. There were significant improvements in physical skills; for example, sprint times were decreased by 6%, and change-of-direction and dribbling times were reduced by 8% and 7%, respectively, indicating improved agility, speed, and ball control abilities. In conclusion, both groups performed significantly better on cognitive and physical skill tests post-HIIT intervention.

Conservation, characterization and exploitation of agrobiodiversity are key factors to guarantee food security and face future challenges such as climate changes. These issues are the subject of a series of international agreements, such as the Convention of Biological Diversity, with its Nagoya Protocol, and the International Treaty on Plant Genetic Resources for Food and Agriculture (ITPGRFA) adopted in 2001 and entered into force in 2004. Italy ratified the Treaty in 2004 and instituted a long-lasting program, RGV-FAO, to implement it. CREA is one of the three organizations involved in the RGV-FAO Program, together with the National Research Council (CNR) and Reti Semi Rurali. CREA maintains a total of 40,186 accessions including cereals, vegetables, fruits, forages, industrial crops, forest and woody crops, medicinal and aromatic plants, and their wild relatives. Accessions are conserved using different ex situ conservation systems (seeds, in vivo plants, vegetative organs and in vitro plantlets), and characterized using genetic, morpho-phenological and/or biochemical methods. Herein, we will present the CREA long-lasting program RGV-FAO with some examples of the use of plant genetic resources in breeding programs, including molecular approaches. Some critical issues related to access and benefit sharing in PGRFA, such as the Nagoya Protocol and the Digital Sequence Information, will be discussed, highlighting their potential impact on food security and on the advancement of knowledge.

Bio-based solutions for solid timber gluing have always been a very sensitive topic in wood technology. In this work, we optimize the gluing conditions of a starch–tannin formulation, which allows high performance in dry conditions and resistance to water dipping for 3 h, allowing for the D2 classification to be reached according to EN 204. It was observed that the starch–tannin formulations enhanced their performance by increasing the heating temperature, achieving satisfactory results at 140 °C for 13 min. The proportion of polyphenols in the mixture enhances the water resistance but is only tolerated until 20–30%. In particular, the addition of 10% tannin–hexamine enhances the water-resistant properties of starch for both quebracho and chestnut extract. The application of the jet of cold atmospheric plasma allows for good results with more viscous formulations, increasing their penetration in wood. Solid-state 13C-NMR analysis was also performed, and the spectroscopic information suggests establishing a coordination complex between starch and tannin.

The recent advances achieved in additive manufacturing (AM) technology demonstrate the potential to realize customized metal components, ensuring weight reduction opportunities. These benefits make AM attractive for high-cost aerospace applications, especially where high geometric complexity is required. In the context of an EU research scenario, the H2020 MANUELA (Additive Manufacturing Using Metal Pilot Line) project promotes the development of new technologies for design optimization by enabling the application of novel materials in AM. This paper illustrates recent advances in a new aluminum alloy (Al-HS1) with high strength emphasizing all of the characterization steps at the coupon level. This material has been employed in the re-engineering of a conventional hydraulic manifold using a powder bed fusion-laser beam (PBF-LB) process. Both the simulations and structural tests allowed for proving its compliance and technological maturity with industrial standards and applicable airworthiness requirements.

This paper deals with the assessment of vibro-acoustic properties of a multifunctional carbon fiber reinforced panel manufactured by bulk infusion, a modified resin film infusion process. The components of the epoxy resin were chosen to contrast the electrical insulating property and poor flame resistance of the epoxy resins impregnating carbon woven fabric. To confer electrical conductivity to the resin a percentage of 0.5 wt% of Multi-Wall Carbon Nanotubes (MWCNTs) was dispersed in the resin, whereas to increase flame resistance a percentage of 5 wt% of Glycidil Polyhedral Oligomeric Silsesquioxanes (GPOSS) was solubilized in the epoxy mixture. Furthermore, as hardener agents, a mixture of 4,4’-DiaminoDiphenyl Sulfone (DDS) (53.4 wt%) and Bis(3-Aminophenyl) Methyl Phosphine Oxide (BAMPO) (46.7 wt%) was used. The values of the electrical conductivity were found satisfactory enough, being 4.02 × 10−2 S/m for the multifunctional resin and 1.39 × 104 S/m for the in-plane conductivity of the panel, whereas the Limiting Oxygen Index (LOI) value of the multifunctional resin was found to increase from 27% to 36%. Considering these promising results, an experimental assessment of the vibro-acoustic properties of the manufactured multifunctional panel was also performed. The panel was tested mainly to evaluate its low-frequency vibration damping and sound insulation characteristics. The manufactured panel demonstrated an improved efficiency if compared to a baseline configuration, presenting almost the double modal damping and a gain of 10 dB on the global noise reduction.

Antimicrobial resistance (AMR) is currently regarded by the World Health Organization (WHO) as one of the most significant risks to global public health. The most critical causes of AMR infections in humans are the misuse and overuse of antimicrobials in humans and farmed animals. The rising global demand for food of animal origin encourages the increase of animal production worldwide, especially in developing countries. Simultaneously, current farming practices often extensively use antimicrobials on animals, influencing bacterial AMR incidence. This study aims to evaluate the correlation between antimicrobial use (AMU) in farmed animals and the detection of AMR infections in humans, the effects of enforcing laws in animal farming in a country on AMR situation in the neighbors, and the potential of AMR to spread from one country to another. Using data from 30 largest animal-producing countries in different regions of the world, between 2010 and 2020, and a Spatial Durbin Model (SDM), we found that AMU in farmed animals increases AMR in humans and there is a spatial dependence between countries regarding AMR spreading. Such findings indicate that a globally coordinated strategy regulating AMU on farmed animals may reduce AMR emergence and worldwide spreading.

Background Information on the real‐world use of proprotein convertase subtilisin kexin 9 inhibitors (PCKS9is) in familial hypercholesterolemia are limited. We evaluated the pattern of prescription and the long‐term efficacy of alirocumab and evolocumab in Italian patients with familial hypercholesterolemia in clinical practice. Methods and Results The data set for analysis was extracted from the PCKS9i Italian Medicines Agency (AIFA) registry and included 2484 patients with heterozygous familial hypercholesterolemia (HeFH) and 62 patients with homozygous familial hypercholesterolemia (HoFH) who were prescribed PCKS9is from February 2017 to December 2021. As the follow‐up schedules were not prespecified and could vary, persistence and adherence as well as low‐density lipoprotein cholesterol (LDL‐C) changes during 2 years of treatment were analyzed in a final cohort of 1299 patients with familial hypercholesterolemia. At baseline, 53.8% of patients with HeFH and 69.4% of patients with HoFH were receiving maximally tolerated lipid‐lowering therapies, while 45.9% of patients with HeFH and 30.7% of patients with HoFH reported statin intolerance; mean LDL‐C was 197.7±52.3 mg/dL in HeFH and 252.0±106.2 mg/dL in HoFH. The 6‐month persistence and adherence to therapy were >85%, and LDL‐C reduction reached 58.6% (to 79.7 mg/dL) in HeFH and 57.6% (to 95.1 mg/dL) in HoFH after 24 months of treatment. The European Atherosclerosis Society/European Society of Cardiology LDL‐C goals were achieved in 43.3% of patients with HeFH and 37.5% of patients with HoFH. Conclusions PCKS9i prescribed to patients with familial hypercholesterolemia in clinical practice showed LDL‐C–lowering efficacy similar to that observed in controlled trials. However, 2 of 5 HeFH cases and 2 of 6 HoFH cases achieved the recommended LDL‐C goals. The full achievement of European Atherosclerosis Society/European Society of Cardiology LDL‐C goals should require a lower threshold for PCKS9i initiation and a combination of multiple therapies.

This study aims to verify if and how migrant penalty in the labor market is associated with sub-national characteristics, exploring the relevance of the regional occupational structure. We expect that a greater relevance of the share of low-status jobs at the regional level reduces the migrant penalty in terms of the probability of being employed, but increases the gap with natives in terms of job quality. We investigate this trade-off by estimating a set of hierarchical models on the EU-LFS data (2009–2015) for 19 countries and 189 regions. Results suggest a pattern consistent with the trade-off hypothesis, nuanced by heterogeneity at the individual level: in regions where the share of low-status jobs is higher, mid-high educated immigrants from less developed countries are less (or not) penalized compared to natives in terms of employment, while they face a stronger penalty in terms of job quality. What is more, the trade-off is not observed when considering low-educated migrants or those from high-income countries.

The Desana treasure is a remarkable assemblage of items made of gold, silver, gemstones and glasses found in north-western Italy. Most scholars agree on the fact that the core of the treasure might have belonged to a single deposit resulted from a long period of selection, accumulation and use. The treasure testifies to the evolution of goldsmiths’ art in Ostrogothic Italy and represents an extraordinary material trace of the Italian elites of the 5th–6th centuries. The Desana treasure was investigated with non-invasive instrumental analytical techniques, namely optical microscopy, UV-visible diffuse reflectance spectrophotometry with optical fibres and X-ray fluorescence spectrometry in order to record the chemical features of gemstones, coloured glasses and precious metals employed to produce the items. As for the gemstones, besides identifying typologies, data suggested India as the source for a sapphire pendant and for most of the garnets, whereas the emeralds may belong to different sources, among which Pakistan, India and Egypt. The investigation revealed the colouring agents and compositional features of the glasses, and the composition of the gold alloys. The results of the investigation highlight that the raw materials used by Late Antique Italian goldsmiths did not differ significantly from other neighbouring European and Mediterranean regions, although the garnets show some differences if compared with coeval jewels recorded north of the Alps. The dataset produced in this work complements the stylistic approach for the study of these amazing traces of the past and deepens our knowledge on the role of the Italian “Ostrogothic” jewellery in the frame of the coeval Mediterranean, Central European and Northern Pontic metalwork traditions.

Brimonidine, a selective alpha-2 adrenergic agonist used for the treatment of open-angle glaucoma, has been shown to cause neurological side effects such as unresponsiveness, lethargy, hypoventilation, and stupor, mimicking opioid toxicity. We report one case of transient encephalopathy in a toddler, in whom accidental brimonidine toxicity was suspected and then confirmed by a toxicology study. The healthy 8-month-old girl was taken to the pediatric ER since she was drowsy and hypotonic with miosis. The computed tomography scan of her brain and toxicological workup of her blood and urine were negative. Starting from the fourth hour, the child progressively improved, and by the sixth hour, she recovered to a normal state of consciousness. A survey of available drugs within the child’s reach showed the presence of brimonidine. Thus, ultra-performance liquid chromatography–tandem mass spectrometry (UPLC-MS/MS) was applied to quantify the brimonidine in urine and plasma samples, showing levels of 8.40 ng/mL and 0.79 ng/mL, respectively. To our knowledge, this is the first report to determine brimonidine levels in urine and plasma using UPLC-MS/MS. Insufficient knowledge on the part of family members about the potential hazards of an apparently innocuous, topical medication such as eye drops may put children at a greater risk of poisoning. Necessary warnings should be given to parents with greater care when prescribing this medication.

Vibrations in helicopters have strong implications for their performance and safety, leading to the increased fatigue of components and reduced operational efficiency. As helicopters are designed to land on several types of surfaces, the landing gear system dissipates the impact on the ground and maintains stability during landing and take-off. These vibrations can arise from a variety of sources, such as aerodynamic loads, mechanical imbalances, and engine instabilities. In the present work, the authors describe the vibration qualification process of the main landing gear tailored to fast helicopters within the Clean Sky 2 Racer program. The method entails devising preliminary load sets that deform the structure in its key excited mode shapes to assess stresses and address the experimental campaign. A full-scale prototype model is then tested for sine sweep and random vibrations as per the Airbus Helicopter requirements in order to reach the final qualification and acceptance stage. Although the discussion centers on a landing gear structure, the described process could be extended to other critical equipment as well.

Children with chronic disease are at higher risk of invasive infectious disease, including several vaccine-preventable infections. The Italian Association of Pediatric Hospitals (AOPI) carried out a survey of immunization practices: 14/16 AOPI hospitals completed the survey; 50% of them include 100–199 beds, while 21% have 2,000/year. A COVID-19 vaccination team is in place for any inpatient child older than 12 years in 42% of hospitals, in 42% only for “fragile” children. A centralized in-hospital immunization service is an emerging model that may contribute to increase compliance to vaccination of fragile patients and to fight against vaccination hesitancy.

Objectives Clinical guidelines for acute non-specific low back pain (LBP) recommend avoiding imaging studies or invasive treatments and to advise patients to stay active. The aim of this study was to evaluate the management of acute non-specific LBP in the emergency departments (ED).Setting We invited all department chiefs of Swiss EDs and their physician staff to participate in a web-based survey using two clinical case vignettes of patients with acute non-specific LBP presenting to an ED. In both cases, no neurological deficits or red flags were present. Guideline adherence and low-value care was defined based on current guideline recommendations.Results In total, 263 ED physicians completed at least one vignette, while 212 completed both vignettes (43% residents, 32% senior/attending physicians and 24% chief physicians). MRI was considered in 31% in vignette 1 and 65% in vignette 2. For pain management, non-steroidal anti-inflammatory drugs, paracetamol and metamizole were mostly used. A substantial proportion of ED physicians considered treatments with questionable benefit and/or increased risk for adverse events such as oral steroids (vignette 1, 12% and vignette 2, 19%), muscle relaxants (33% and 38%), long-acting strong opioids (25% and 33%) and spinal injections (22% and 43%). Although guidelines recommend staying active, 72% and 67% of ED physicians recommended activity restrictions.Conclusion Management of acute non-specific LBP in the ED was not in agreement with current guideline recommendations in a substantial proportion of ED physicians. Overuse of imaging studies, the use of long-acting opioids and muscle relaxants, as well as recommendations for activity and work restrictions were prevalent and may potentially be harmful.

Pro-migrant volunteering is often denounced as apolitical and patronising. Voluntary initiatives for immigrants' language education, then, have been accused of facilitating the neoliberal governmentality of migration, by fashioning migrants into precarious workers. Based on 20 in-depth interviews with volunteer language teachers in Lombardy, Italy, this article complicates such understandings, by shedding light on the tensions and ambiguities characterising volunteers' activities vis-à-vis the institutional governance of migration. Indeed, whereas such initiatives take on integration tasks for the benefit of the State, and thus can be accused of allying with the State in the governmentality of migration, against a background of growing nationalism, volunteering appears to develop people's empathy and solidarity beyond national belonging, questioning the division between citizens and non-citizens. In particular, it shows that volunteering in language education has the potential to transgress consolidated lines of inclusion and exclusion, turn volunteers from 'active citizens' into 'activist citizens', and offer resources of substantive citizenship to students. Ultimately, these 'humanitarian' actions by citizens belonging to the dominant society may represent acts of citizenship complementary to the initiatives of 'denizens'.

Following the threads of impersonality and empathy, which are firmly interwoven in the stories of Katherine Mansfield (1888-1923), this article aims to reflect on the author’s interest in the then-new medium of silent film, and to contextualize her work against the framework of interwar philosophy and psychoanalysis, highlighting its ethical and existential components, bordering on mysticism. Mansfield's private writings and stories will be compared with specific aspects of the works of Henri Bergson (sympathy), Edith Stein (empathy), Martin Buber (relationship), C.G. Jung (the collective unconscious and individuation), D.H. Lawrence (modernity and consciousness) and Sigmund Freud (oceanic feeling), contrasting Freudian psychoanalysis with the alternative visions of the human and the psyche that emerged around World War One.

Abstract In the Nordic Atrial Fibrillation and Stroke (NOR-FIB) study, the causes of ischemic stroke were identified in 43% of cryptogenic stroke patients monitored with implantable cardiac monitor (ICM), but one-third of these patients had non-cardioembolic causes. These results suggest the need for an early and comprehensive diagnostic work-up before inserting an ICM.

Abstract Patients with multiple myeloma-bearing translocation t(11;14) have recently been shown to benefit from the apoptosis-inducing drug venetoclax; however, the drug lacks FDA approval in multiple myeloma thus far due to a potential safety signal in the overall patient population. Selinexor is an inhibitor of nuclear export that is FDA-approved for patients with multiple myeloma refractory to multiple lines of therapy. Here, we report that in four patients with multiple myeloma with t(11;14), the concomitant administration of venetoclax and selinexor was safe and associated with disease response. Moreover, the combination was synergistic in t(11;14) multiple myeloma cell lines and caused decreased levels of Cyclin D1 (which is overexpressed due to the CCND1-IGH fusion) when given in combination as compared to single agents. These data suggest that the combination of venetoclax and selinexor is effective and t(11;14) may serve as a therapeutic marker for response and target for future clinical trials.

Abstract Anthropogenic microparticles (AMs) were found for the first time in specimens of Trematomus bernacchii collected in 1998 in the Ross Sea (Antarctica) and stored in the Antarctic Environmental Specimen Bank. Most of the identified AMs were fibers of natural and synthetic origin. The natural AMs were cellulosic, the synthetic ones were polyester, polypropylene, polypropylene/polyester, and cellulose acetate. The presence of dyes in the natural AMs indicates their anthropogenic origin. Five industrial dyes were identified by Raman spectroscopy with Indigo occurring in most of them (55%). Our research not only adds further data to the ongoing knowledge of pollution levels in the Antarctic ecosystem, it provides an interesting snapshot of the past, highlighting that microplastics and anthropogenic fiber pollution had already entered the Antarctic marine food web at the end of the ‘90 s. These findings therefore establish the foundations for understand the changes in marine litter pollution over time.

This article presents the results of a study on the teleworking of teachers in Guadeloupe (French overseas department) in a confinement situation. The objective of the survey was to know their living and working conditions during the Covid-19 epidemic. The results show that the smartworking strategies put in place by the French government and local administrations to deal with the crisis resulted in a high level of work overload and dissatisfaction among teachers on the island, while highlighting a series of inequalities. Moreover, teachers have only partially achieved their educational objectives and have not been able to solve the lack of technical means and digital resources that the overseas territories suffer from.

Intracranial hemorrhage (ICH) is considered a potentially severe complication of oral anticoagulants (OACs) and antiplatelet therapy (APT). Patients with atrial fibrillation (AF) who survived ICH present both an increased ischemic and bleeding risk. Due to its lethality, initiating or reinitiating OACs in ICH survivors with AF is challenging. Since ICH recurrence may be life-threatening, patients who experience an ICH are often not treated with OACs, and thus remain at a higher risk of thromboembolic events. It is worthy of mention that subjects with a recent ICH and AF have been scarcely enrolled in randomized controlled trials (RCTs) on ischemic stroke risk management in AF. Nevertheless, in observational studies, stroke incidence and mortality of patients with AF who survived ICH had been shown to be significantly reduced among those treated with OACs. However, the risk of hemorrhagic events, including recurrent ICH, was not necessarily increased, especially in patients with post-traumatic ICH. The optimal timing of anticoagulation initiation or restarting after an ICH in AF patients is also largely debated. Finally, the left atrial appendage occlusion option should be evaluated in AF patients with a very high risk of recurrent ICH. Overall, an interdisciplinary unit consisting of cardiologists, neurologists, neuroradiologists, neurosurgeons, patients, and their families should be involved in management decisions. According to available evidence, this review outlines the most appropriate anticoagulation strategies after an ICH that should be adopted to treat this neglected subset of patients.

Background Evidence suggests that LPA risk genotypes are a possible contributor to the clinical diagnosis of familial hypercholesterolemia (FH). This study aimed at determining the prevalence of LPA risk variants in adult individuals with FH enrolled in the Italian LIPIGEN (Lipid Transport Disorders Italian Genetic Network) study, with (FH/M+) or without (FH/M−) a causative genetic variant. Methods and Results An lp(a) [lipoprotein(a)] genetic score was calculated by summing the number risk‐increasing alleles inherited at rs3798220 and rs10455872 variants. Overall, in the 4.6% of 1695 patients with clinically diagnosed FH, the phenotype was not explained by a monogenic or polygenic cause but by genotype associated with high lp(a) levels. Among 765 subjects with FH/M− and 930 subjects with FH/M+, 133 (17.4%) and 95 (10.2%) were characterized by 1 copy of either rs10455872 or rs3798220 or 2 copies of either rs10455872 or rs3798220 (lp(a) score ≥1). Subjects with FH/M− also had lower mean levels of pretreatment low‐density lipoprotein cholesterol than individuals with FH/M+ (t test for difference in means between FH/M− and FH/M+ groups

Background Acute stroke patients with comorbid cancer (CC) are more preferably addressed with endovascular treatment (EVT) than thrombolytic therapy, due to the presumed potential risk of hemorrhagic transformation and systemic bleeding. This study aims to evaluate clinical and procedural outcomes of acute stroke patients with CC receiving EVT. Methods Using the Italian Registry of Endovascular Treatment in Acute Stroke, we reviewed prospectively collected data of CC patients treated with EVT from 2011 to 2017. Outcome measures were modified Thrombolysis in Cerebral Infarction score, symptomatic intracranial hemorrhage rate, and 3‐month modified Rankin scale. We also compared CC patients with a control group without cancer (non‐comorbid cancer) receiving EVT and matched the 2 populations with a 1:1 propensity score. Results Out of 4598 stroke patients treated with EVT, 165 (3.6%) had a CC. Modified Thrombolysis in Cerebral Infarction 2b‐3 was obtained in 73.8% of CC and 74.5% of non‐comorbid cancer patients (P=0.84), whereas the rate of symptomatic intracranial hemorrhage rate was 8.2% and 6.9%, respectively. Domesticated rice Oryza sativa L. is a major staple food worldwide, and the cereal most sensitive to salinity. It originated from the wild ancestor Oryza rufipogon Griff., which was reported to possess superior salinity tolerance. Here, we examined the morpho-physiological responses to salinity stress (80 mM NaCl for 7 days) in seedlings of an O. rufipogon accession and two Italian O. sativa genotypes, Baldo (mildly tolerant) and Vialone Nano (sensitive). Under salt treatment, O. rufipogon showed the highest percentage of plants with no to moderate stress symptoms, displaying an unchanged shoot/root biomass ratio, the highest Na+ accumulation in roots, the lowest root and leaf Na+/K+ ratio, and highest leaf relative water content, leading to a better preservation of the plant architecture, ion homeostasis, and water status. Moreover, O. rufipogon preserved the overall leaf carbon to nitrogen balance and photosynthetic apparatus integrity. Conversely, Vialone Nano showed the lowest percentage of plants surviving after treatment, and displayed a higher reduction in the growth of shoots rather than roots, with leaves compromised in water and ionic balance, negatively affecting the photosynthetic performance (lowest performance index by JIP-test) and apparatus integrity. Baldo showed intermediate salt tolerance. Being O. rufipogon interfertile with O. sativa, it resulted a good candidate for pre-breeding towards salt-tolerant lines.

Acute bacterial skin and skin structure infections (ABSSSI) and osteoarticular infections compound the burden of morbidity, mortality and prolonged hospitalizations among gram-positive infections. Dalbavancin, a second-generation, intravenous lipoglycopeptide, due to its prolonged half-life, can be a valuable alternative in their treatment when administered as inpatient treatment at the price of an extended hospital stay. Between October 2019 and September 2023, 31 children and adolescents were treated with dalbavancin because of bone and joint infections (n = 12 patients, 39%), ABSSSI (n = 13 patients, 42%), mainly for the limbs, facial cellulitis or complicated ABSSSI (n = 6 patients, 19%), at five Italian pediatric centers. Microbiological study provided gram-positive bacterial isolate in 16 cases, in 11 cases from a positive blood culture; 9 of them were MRSA. Twenty-five patients were initially treated with a different antibiotic therapy: beta-lactam-based in 18 patients (58%), glycopeptide-based in 15 patients (48%) and daptomycin in 6 (19%). The median time that elapsed between admission and start of dalbavancin was 18 days. A total of 61 doses of dalbavancin were administered to the 31 patients: 16 received a single dose while the remaining 15 patients received between two (n = 9) and nine doses. The frequency of administration was weekly in five cases or fortnightly in nine patients. Median length of stay in hospital was 16 days. Median time to discharge after the first dose of dalbavancin was 1 day. Treatment was very well-tolerated: of the 61 administered doses, only four doses, administered to four patients, were associated with an adverse event: drug extravasation during intravenous administration occurred in two patients, with no sequelae; however, in two patients the first administration was stopped soon after infusion start: in one (ID #11), due to headache and vomiting; in another (ID #12) due to a systemic reaction. In both patients, drug infusion was not repeated. None of the remaining 29 patients reported treatment failure (resistant or recurrent disease) or an adverse effect during a median follow-up time of two months. The use of dalbavancin was safe, feasible and also effective in shortening the hospital stay in children and adolescents.

Human mastadenoviruses, frequently denominated adenoviruses (HAdVs), may cause respiratory tract, gastrointestinal or, less frequently, other involvements. Epidemics of HAdV infections occur globally, in communities, and in closed or crowded settings. In our institution, a cluster of infants and children admitted for HAdV infection was recently observed. The aim of this study was to describe the pattern of their presenting features and investigate the possible correlation between the HAdV copy number and the clinical picture. Two main patterns of clinical presentation were observed: 68 patients had mainly respiratory symptoms (pharyngitis n = 67, cough n = 44; tonsillar exudate n = 17; other respiratory signs n = 4) while 26 patients showed prevalent gastrointestinal involvement (diarrhea n = 26, vomiting n = 8). Patients with respiratory symptoms had a significantly higher count of WBC, PMN, and platelets, while CRP level approached statistical significance (p = 0.07) for higher values in the patients with diarrhea. In order to explore the impact of selected presenting features, the possible association between the level of CRP and the presence of pharyngeal exudate, cough, vomiting, diarrhea, duration of fever, number of neutrophils, and administration of antibiotics was analyzed. Patients falling in the tertile with more elevated CRP values had tonsillar exudate and diarrhea significantly more often, while those in the lower tertile had a 4.4-day duration fever vs. ≥5.0 days in the remaining patients. Antibiotic therapy was administered more frequently to patients with higher values of CRP (p = 0.006). The duration of hospitalization was not associated with the CRP level. The median time from the receipt of a positive HAdV PCR test result to patient discharge was 1 day in 73% of cases. The number of copies of HAdV detected via PCR ranged between 47 million and 15/μL. Falling in the highest tertile of copy number was significantly associated with pharyngitis. The 24 patients with evidence of viral coinfection had no difference in the demographics or presenting features, with the only exception being a significantly higher leukocyte count. The rapid turn-around of the results of the molecular testing of the HAdV genome on a pharyngeal swab allowed us to rapidly diagnose HAdV infection, allowing us to stop antibiotic therapy and immediately discharge the patients, with reduced discomfort for the families and more appropriate use of hospital beds. A high copy number of HAdV from a pharyngeal swab should not be taken as an indicator of worse prognosis, thus allowing for the preferential use of qualitative rather than quantitative assay.

We assessed fungal diversity in water and sediment samples obtained from five Arctic lakes in Ny-Ålesund (Svalbard Islands, High Arctic) and five Antarctic lakes on Livingston and Deception Islands (South Shetland Islands), using DNA metabarcoding. A total of 1,639,074 fungal DNA reads were detected and assigned to 5980 ASVs amplicon sequence variants (ASVs), with only 102 (1.7%) that were shared between the two Polar regions. For Arctic lakes, unknown fungal taxa dominated the sequence assemblages, suggesting the dominance of possibly undescribed fungi. The phylum Chytridiomycota was the most represented in the majority of Arctic and Antarctic samples, followed by Rozellomycota, Ascomycota, Basidiomycota, and the less frequent Monoblepharomycota, Aphelidiomycota, Mortierellomycota, Mucoromycota, and Neocallimastigomycota. At the genus level, the most abundant genera included psychrotolerant and cosmopolitan cold-adapted fungi including Alternaria, Cladosporium, Cadophora, Ulvella (Ascomycota), Leucosporidium, Vishniacozyma (Basidiomycota), and Betamyces (Chytridiomycota). The assemblages displayed high diversity and richness. The assigned diversity was composed mainly of taxa recognized as saprophytic fungi, followed by pathogenic and symbiotic fungi.

In vitro diffusive models are an important tool to screen the penetration ability of active ingredients in various formulations. A reliable assessment of skin penetration enhancing properties, mechanism of action of carrier systems, and an estimation of a bioavailability are essential for transdermal delivery. Given the importance of testing the penetration kinetics of different compounds across the skin barrier, several in vitro models have been developedThe aim of this study was to compare the Franz Diffusion Cell (FDC) with a novel fluid-dynamic platform (MIVO) by evaluating penetration ability of caffeine, a widely used reference substance, and LIP1, a testing molecule having the same molecular weight but a different lipophilicity in the two diffusion chamber systems. A 0.7% caffeine or LIP1 formulation in either water or propylene glycol (PG) containing oleic acid (OA) was topically applied on the Strat-M® membrane or pig ear skin, according to the infinite-dose experimental condition (780 ul/cm2). The profile of the penetration kinetics was determined by quantify the amount of molecule absorbed at different time-points (1, 2, 4, 6, 8 hours), by means of HPLC analysis.Both diffusive systems show a similar trend for caffeine and LIP1 penetration kinetics. The Strat-M® skin model shows a lower barrier function than the pig skin biopsies, whereby the PGOA vehicle exhibits a higher penetration, enhancing the effect for both diffusive chambers and skin surrogates. Most interestingly, MIVO diffusive system better predicts the lipophilic molecules (i.e. LIP1) permeation through highly physiological fluid flows resembled below the skin models.

Riassunto. L’articolo si propone due obiettivi. In primo luogo intende approfondire il tema del ritorno dei confini, illustrandone l’articolazione a diversi livelli e il funzionamento selettivo, come effetto di un regime neo-liberale di governo della mobilità umana: un regime che incorpora disuguaglianze profonde nella distribuzione del diritto a muoversi attraverso i confini. Il caso dei respingimenti al confine con la Bosnia e la responsabilità dell’agenzia Frontex verranno approfonditi per illustrare il funzionamento della vigilanza sulle frontiere da parte dell’UE. La pandemia inoltre ha favorito il rafforzamento di misure di restrizione degli accessi dei migranti indesiderati nei paesi sviluppati. In secondo luogo, l’articolo rivolge lo sguardo agli attori e alle iniziative che cercano di aprire spazi di solidarietà e accoglienza nei confronti dei migranti e dei richiedenti asilo, contestando sul piano pratico l’assolutizzazione dei confini e l’esclusione di chi bussa alle porte del Nord del mondo. Tra i molti esempi possibili, ne vengono trattati due: le cosiddette “città santuario” e i corridoi umanitari. A partire da questi esempi sarà sviluppato nelle conclusioni il concetto di “solidarietà contro i confini”.
