Objective The application of data linkage for scientific research purposes in the public health sphere requires using personal and sensitive data on an individual level while results are analyzed from a population perspective. Considering this, ethical, legal and social issues (ELSI) must be considered. In this sense, our study aims to better understand how ELSI surrounding administrative data linkage use in public health has been addressed in the scientific literature. Approach Our scoping review is based on the Joann Briggs Institute Manual for Evidence Synthesis and will be reported according to the Preferred Report Items for Systematic Reviews and Meta-Analyses – Scoping Review Guideline. Two independent authors searched for articles published between January 2013 and December 2023 in the PubMed/MEDLINE, Web of Science, EMBASE and LILACS databases, as well as in Scielo and Google Scholar platforms. Keywords used were related to ‘data linkage’, ‘administrative data’, ‘ELSI’, “public policies’ and ‘public health’ terms. Articles were screened initially by title and abstract, then by full-text read. Results After duplicate removal and articles screening, two independent authors are performing data extraction considering ethical, legal and regulatory basis as well as procedures for data access; data management process including record linkage; type of administrative data linked; data protection strategies; and data subjects rights. Considerations This scoping is underway, we intend to have our final results by April to present at the conference, in September.

Abstract COVID-19 vaccination during pregnancy is safe and effective in reducing the risk of complications. However, the uptake is still below targets worldwide. This study aimed to explore the factors associated with COVID-19 vaccination uptake among pregnant women since data on this topic is scarce in low-to-middle-income countries. A retrospective cohort study included linked data on COVID-19 vaccination and pregnant women who delivered a singleton live birth from August 1, 2021, to July 31, 2022, in Rio de Janeiro City, Brazil. Multiple logistic regression was performed to identify factors associated with vaccination during pregnancy, applying a hierarchical model and describing odds ratio with 95% confidence intervals. Of 65,304 pregnant women included in the study, 53.0% (95% CI, 52–53%) received at least one dose of COVID-19 vaccine during pregnancy. Higher uptake was observed among women aged older than 34 (aOR 1.21, 95%CI 1.15–1.28), black (aOR 1.10, 1.04–1.16), or parda/brown skin colour (aOR 1.05, 1.01–1.09), with less than eight years of education (aOR 1.09, 1.02–1.17), living without a partner (aOR 2.24, 2.16–2.34), more than six antenatal care appointments (aOR 1.92, 1.75–2.09), and having a previous child loss (OR 1.06, 1.02–1.11). These results highlight the need for targeted educational campaigns, trustful communication, and accessibility strategies for specific populations to improve vaccination uptake during pregnancy.

Abstract Background BCG vaccination, originally used to prevent tuberculosis, is known to “train” the immune system to improve defence against viral respiratory infections. We investigated whether a previous BCG vaccination is associated with less severe clinical progression of COVID-19 Methods A case-control study comparing the proportion with a BCG vaccine scar (indicating previous vaccination) in cases and controls presenting with COVID-19 to health units in Brazil. Cases were subjects with severe COVID-19 (O2 saturation < 90%, severe respiratory effort, severe pneumonia, severe acute respiratory syndrome, sepsis, and septic shock). Controls had COVID-19 not meeting the definition of “severe” above. Unconditional regression was used to estimate vaccine protection against clinical progression to severe disease, with strict control for age, comorbidity, sex, educational level, race/colour, and municipality. Internal matching and conditional regression were used for sensitivity analysis. Results BCG was associated with high protection against COVID-19 clinical progression, over 87% (95% CI 74–93%) in subjects aged 60 or less and 35% (95% CI − 44–71%) in older subjects. Conclusions This protection may be relevant for public health in settings where COVID-19 vaccine coverage is still low and may have implications for research to identify vaccine candidates for COVID-19 that are broadly protective against mortality from future variants. Further research into the immunomodulatory effects of BCG may inform COVID-19 therapeutic research.

ABSTRACT Objective: To estimate the prevalence of adult smokers in the 26 capitals and the Federal District according to the Brazilian Deprivation Index (Índice Brasileiro de Privação – IBP). Methods: Dataset on smoking were obtained from the Surveillance of Risk and Protective Factors for Noncommunicable Diseases by Survey (Vigitel) system for the 26 capitals and the Federal District, in the period from 2010 to 2013. The IBP classifies the census sectors according to indicators such as: income less than ½ minimum wage, illiterate population and without sanitary sewage. In the North and Northeast regions, the census sectors were grouped into four categories (low, medium, high and very high deprivation) and in the South, Southeast and Midwest regions into three (low, medium and high deprivation). Prevalence estimates of adult smokers were obtained using the indirect estimation method in small areas. To calculate the prevalence ratios, Poisson models are used. Results: The positive association between prevalence and deprivation of census sector categories was found in 16 (59.3%) of the 27 cities. In nine (33.3%) cities, the sectors with the greatest deprivation had a higher prevalence of smokers when compared to those with the least deprivation, and in two (7.4%) there were no differences. In Aracaju, Belém, Fortaleza, João Pessoa, Macapá and Salvador, the prevalence of adult smokers was three times higher in the group of sectors with greater deprivation compared to those with less deprivation. Conclusion: Sectors with greater social deprivation had a higher prevalence of smoking, compared with less deprivation, pointing to social inequalities.

RESUMO São apresentadas neste artigo as principais contribuições acadêmicas da Profa. Marlúcia Oliveira (AMO) (in memoriam), nutricionista, docente da Escola de Nutrição da Universidade Federal da Bahia, Doutora em Epidemiologia e Pesquisadora A do Conselho Nacional de Desenvolvimento Científico e Tecnológico (CNPQ), no período de 1980 a 2021. A Profa. Ana acumulou, ao longo de sua carreira acadêmica, artigos científicos publicados em periódicos nacionais e internacionais; livro e capítulos de livro de sua autoria; trabalhos apresentados em eventos científicos, além de ter orientado projetos de iniciação científica, dissertações e teses. Coordenou vários projetos de pesquisa no campo da alimentação e nutrição em saúde coletiva, com foco na epidemiologia nutricional. O escopo dos assuntos abordados na sua produção científica expressou a inquietude que a mobilizava em torno da produção de conhecimentos para o enfrentamento dos complexos problemas de saúde e nutrição do Brasil. Sua forma de ser no mundo, acolhendo e cuidando das pessoas que se aproximavam em busca de oportunidades de qualificação, seu exemplo, suas palavras e ensinamentos influenciaram, e ainda influenciam, a trajetória e a formação de nutricionistas e de professores e pesquisadores na Escola de Nutrição da Universidade Federal da Bahia (ENUFBA) e em outras instituições nacionais e internacionais. Trata-se de uma pesquisadora e intelectual brasileira comprometida com a saúde das populações mais vulneráveis e com o combate contra a desnutrição e a fome no nosso país. Sua vasta e fecunda obra nos deixou um legado a ser lembrado e continuado. Alguns dos seus amigos, colegas e colaboradores prestam este tributo à sua memória e ao legado que ela deixou para todos nós e para as próximas gerações.

Introduction Health inequalities in Brazil have deepened on Covid-19 pandemic, and the most vulnerable people were the more affected. A multidisciplinary team from Cidacs/Fiocruz Bahia developed a Social Disparities Index for Covid-19 (IDS-COVID-19) to support the evaluation of effects of health inequalities on the pandemic in Brazil. Public Involvement and Engagement were the pillars of this research because they allowed us to access first hand experiences about the social context in our country. Objectives This paper aims to describe our Public Involvement and Engagement experience by analysing our challenges, strategies, activities, results, and lessons learned during the construction of IDS-COVID-19. Methods The basis of the IDS-Covid-19 public engagement model was the participation of different social groups through methods and techniques that allow dialogue. Several activities and communication products supported the continuous interactions. Another guideline was the inclusion and the welcoming of participants from the beginning of the project to ensure that the participant's contributions could drive decision-making about the research. Results Participants made several contributions to the research as a new layer of information to the Index, and improvements were made to the interactive panel. They also compromised to support the dissemination and use of the product. Eight representatives of community groups and 29 policymakers participated in our engagement activities during the project. More than 500 people were in our open webinars. In addition, more than 140 news items about IDS-Covid-19 were published in national and international media. Conclusions We highlight as lessons learned the adaptation of some dissemination formats to the public, and the necessity of being flexible and accessible to participants. We strengthened the relationship with relevant stakeholders by exploring individual conversations by phone, WhatsApp, email, and interviews to produce a documentary that registered this whole experience. Cidacs/Fiocruz Bahia has also embedded public engagement and involvement in the study agenda.

Background: The treatment of rheumatoid arthritis (RA), a chronic systemic inflammatory autoimmune disease, is based on disease-modifying anti-rheumatic drugs (DMARDs). Typically, it starts with conventional synthetic DMARDs (csDMARDs), and depending on the patient’s response to the treatment and the adverse events experienced, biological DMARDs (bDMARDs) are initiated. bDMARDs are more specific to inflammatory factors than csDMARDs and more efficient in inducing remission and low disease activity. Thus, this study aimed to assess the effectiveness of biological therapy in patients with rheumatoid arthritis in administrative health databases.Methods: PubMed, Embase, Lilacs, Ovid, Scopus, and Web of Science databases were searched from inception to 21 October 2021, to identify observational studies that evaluated the effectiveness of biological therapy in patients with rheumatoid arthritis using administrative databases and real-world data. The methodological quality was assessed by the methodological index for non-randomized studies (MINORS). A fixed or random-effects model estimated risk ratios with 95% confidence intervals. The analysis was divided into four groups: tumor necrosis factor inhibitors (TNFi) versus non-TNFi; TNFi versus TNFi (adalimumab, etanercept, and golimumab versus infliximab); bDMARDs versus Janus kinase inhibitors (JAKi); and bDMARDs monotherapy versus combination therapy (bDMARDs and MTX).Results: Twenty-one records were eligible for inclusion in this systematic review and meta-analysis; seven population-based cohorts, eight prospective, and six retrospective cohort studies. Overall, 182,098 rheumatoid arthritis patients were evaluated. In the meta-analysis, lower effectiveness was observed among TNFi users than in non-TNFi (RR: 0.88; 95% CI: 0.81–0.95; p < 0.01; I2 = 94.0%) and bDMARDs than in JAKi (RR: 0.86; 95% CI: 0.79–0.94; p < 0.01; I2 = 93.0%). Higher effectiveness among adalimumab, etanercept, and golimumab than in infliximab (RR: 1.19; 95% CI: 1.05–1.36; p < 0.01; I2 = 96.0%) was found. No significant differences in the effectiveness of bDMARD monotherapy compared to combination therapy (RR: 0.83; 95% CI: 0.68–1.00; p < 0.01; I2 = 81.0%) was observed. E-value analysis indicated that the estimates were not robust against unmeasured confounding.Conclusion: According to the available real-world data, our results suggest that biological therapy effectively treats patients with rheumatoid arthritis, indicating higher effectiveness with non-TNFi and JAKi than with TNFi.Systematic Review Registration:https://www.crd.york.ac.uk/prospero/display_record.php?ID#CRD42020190838, identifier CRD42020190838.

Background: Rheumatoid arthritis (RA) is a systemic inflammatory disease that affects the synovial fluid of joints, tendons, and some extra-articular sites. Biologic agents have been highly effective and are comparable in reducing RA symptoms, slowing disease progression, and improving physical function; however, concerns have been raised about the risks of several potential adverse effects. Thus, this study aimed to assess the safety of biological therapy in patients with rheumatoid arthritis in observational studies using administrative health databases.Methods: PubMed, Embase, Lilacs, Ovid, Scopus, and Web of Science were searched from inception to 21 October 2021. The analysis was divided into five groups: tumor necrosis factor inhibitors (TNFi) versus non-TNFi; TNFi versus csDMARDs; bDMARDs versus csDMARDs; abatacept versus bDMARDs; and TNFi versus Janus kinase inhibitors (JAKi). The adverse events were cancer, cardiovascular events, infection, herpes zoster, tuberculosis, and death. The methodological quality of the studies was assessed by the Newcastle-Ottawa Scale. A random-effects model estimated risk ratios with 95% confidence intervals.Results: Thirty-one studies were eligible for inclusion in the present systematic review, published from 2014 to 2021. A total of 1,039,398 RA patients were assessed. The 31 studies evaluated eleven different biological drugs. No significant differences were found regarding safety between TNFi versus non-TNFi (RR 1.08; 95% CI 0.92–1.28; p < 0.01; I2 = 93.0%), TNFi versus csDMARDs (RR 0.91; 95% CI 0.75–1.10; p < 0.01; I2 = 87.0%), bDMARDs versus csDMARDs (RR 0.99; 95% CI 0.82–1.20; p < 0.01; I2 = 93.0%), abatacept versus bDMARDs (RR 0.80; 95% CI 0.54–1.18; p < 0.01; I2 = 90.0%), and TNFi versus JAKi (RR 3.54; 95% CI 0.30–42.09; p = 0.01; I2 = 81.0%). In the subgroup analysis, among studies comparing abatacept to TNFi, a lower risk of cardiovascular events was associated with abatacept (RR 0.37; 95% CI 0.24–0.55).Conclusion: Our results do not suggest an increased risk of adverse events associated with biological therapy in treating RA patients, indicating a lower risk of cardiovascular events with abatacept than TNFi. Objectives Covid-19 databases have detailed information about each affected person in Brazil, but it has flaws in counting the number of cases, which are underreported. We aimed to construct and correct the cases dataset by linking different sources of data observations to study the pandemic evolution in Brazilian municipalities. Approach Using the electronic Unified Health System (e-SUS), a public and governmental database, we calculated the pandemic curves of COVID-19 cases. We applied the following approaches to investigate data anomalies a) to perform a descriptive analysis and compare these results with a non-governmental database using Dynamic Time Warping distance; b) to verify and correct municipalities data anomalies linking to other public governmental database namely National Council of Health Secretaries (CONASS) with e-SUS. c) To apply a K-means DTW Barycenter Averaging in clustering analysis to describe the general behaviors of pandemic in Brazilian Municipalities. Results Around 10% records of cases in the e-SUS public governmental database were underreported. After the linkage and the data updating procedure, the time-dependent clustering analysis presents no anomalies and more interpretable results. The clustering analysis provided eight different behaviors of COVID-19 curves of cases. The degree of intensity for prevalence and incidence rates were identified according to eight clusters from the lowest to highest. Conclusions Using the matching procedure based on Dynamic Time Warping distance to correct the municipalities unreported cases, we provided a richer dataset to support a clustering time dependent analysis to characterize the Pandemic evolution in Brazil. These results may be explored in future deprivation social studies.

Objective Brazil is one of the most unequal countries in the world. We aimed to create a Social Disparities Index for Covid-19 (SDI-Covid-19) using linked administrative data to understand the role of inequalities in the COVID-19 pandemic to target social and health policies. Approach Using linked administrative data (2010 Census and National Registry of Health Establishments) we selected municipal indicators for three domains: a) Brazilian Deprivation Index; b) % black/brown/indigenous, % people living in housing with density >2 person/room, % poor older people as sociodemographic; c) ICU beds and respirators rates weighted by distance to the health region headquarters as access to health service. A z-score weighted by the population estimated each indicator within the domains. The domains were combined by simple average of the z-scores and classified in quintiles (1-very low to 5-very high). Other methods (TRI, Macharia et al)1 found similar estimative. Results The SDI-COVID-19 estimated for the three moments of the pandemic (before (T0), first (T1) and second peak (T2)) in Brazil showed that municipalities belonging to the North and Northeast regions had worse SDI-Covid-19 compared to municipalities in the Midwest, Southeast, and South regions of the country. More than 97% of the municipalities in the North and Northeast regions had high and very high SDI in the three estimated periods. There was an increase of 46% in the number of municipalities in the Midwest region with high and very high SDI in T1 and 33% in T2 when compared to T0. Otherwise, we observed an increase of municipalities with very low and low SDI in T1 and T2 in the Southeast and South regions Conclusions The deterministic linkage of administrative data allowed to build the SDI-COVID-19. This index identified the most vulnerable municipalities in the North and Northeast regions in contrast of municipalities in the Southeast and South regions of Brazil with lower inequalities during the pandemic. Findings can target policies to those areas.

ABSTRACT OBJECTIVE To evaluate the performance of tuberculosis control in Brazilian municipalities. METHODS This is an ecological study on Brazilian municipalities that notified at least four new cases of tuberculosis, with a minimum of one new case of pulmonary tuberculosis between 2015 and 2018. The municipalities were stratified according to the population in < 50 thousand, 50–100 thousand, 100–300 thousand, and > 300 thousand inhabitants, and the k-means method was used to group them within each population range according to the performance of six indicators of the disease. RESULTS A total of 2,845 Brazilian municipalities were included, comprising 98.5% (208,007/211,174) of new tuberculosis cases in the period. For each population range, three groups (A, B, and C) of municipalities were identified according to the performance of the indicators: A, the most satisfactory; B, the intermediates; and C, the least satisfactory. Municipalities in group A with < 100 thousand inhabitants presented results above the targets for laboratory confirmation (≥ 72%), abandonment (≤ 5%), and cure (≥ 90%), and comprised 2% of new cases of the disease. Conversely, municipalities of groups B and C presented at least five indicators with results below the targets – HIV testing (< 100%), contact investigation (< 90%), directly observed therapy (< 90%), abandonment (> 5%), and cure (< 90%) –, and corresponded to 66.7% of new cases of tuberculosis. In group C of municipalities with > 300 thousand inhabitants, which included 19 of the 27 capitals and 43.1% of new cases of tuberculosis, the lowest percentages of contact investigation (mean = 56.4%) and directly observed therapy (mean = 15.4%) were verified, in addition to high abandonment (mean = 13.9%) and low coverage of primary health care (mean = 66.0%). CONCLUSIONS Most new cases of tuberculosis occurred in municipalities with unsatisfactory performance for disease control. Expanding the coverage of primary health care in these places can reduce abandonment and increase the contact investigation and directly observed therapy.

Abstract Background Brazil conducts many health surveys to provide estimates by national level, macro-regions, states, metropolitan regions and capitals. However, estimates for smaller areas are lacking due to their high cost. The Health Vulnerability Index (in Portuguese, Índice de Vulnerabilidade em Saúde, IVS) is a measure that combines socioeconomic and environmental variables in the same indicator and allows for the analysis of the characteristics of population groups residing in census tracts, grouping them into four health risk areas (low, medium, high and very high risk) in addition to showing inequalities in the epidemiological profile of different social groups. This index was developed by the Municipal Health Secretariat of Belo Horizonte to guide health planning. Objective The aim of the study is to produce a methodology for obtaining reliable estimates for tobacco smoking in small areas for which the IVS was not designed. Methods The Vigitel dataset from 2006 to 2013 was used to obtain estimates of the prevalence of smokers based on the IVS employing small area estimation methods that use data from a larger domain to obtain estimates in smaller areas. For indirect estimates, the covariates included were sanitation, housing, education, income, and social and health factors. Post-stratification weights were used according to the IVS based on the population of the 2010 census. Results From 2006 to 2009, 16.2% (95% CI: 13.6–14.8%) of the adult population in Belo Horizonte were smokers, and 14.8% (95% CI: 14.0–15.6%) were smokers between 2010 and 2013. The very high-risk population maintained a high prevalence over the same period of 21.1% (95% CI: 17.1–25.0%) between 2006 and 2009 and 20.8% (95% CI: 17.0–24.6%) between 2010 and 2013, while in the low-risk group, the prevalence in the same period fell from 14.9% (95% CI: 13.7–16.2%) to 11.8% (95% CI, 10.6–13.1%). Conclusions The present study identified differences in the profile of smokers by the IVS in the city of Belo Horizonte. While the smoking prevalence declined in richer areas, it remained high in poor areas. This methodology can be used to produce reliable estimates for subgroups with greater vulnerability in small areas and thus subsidize the formulation, monitoring and evaluation of public health policies and programmes aimed at smoking.

RESUMO A Pesquisa Translacional é interdisciplinar e está apoiada em três pilares: pesquisa de bancada (básica), leito (aplicações clínicas) e comunidade (aplicações nos sistemas de saúde). O estudo, baseado nos cinco estágios da Pesquisa Translacional, resgatou o histórico da deficiência de vitamina A e da cegueira nutricional no Brasil (T0); o caminho da descoberta científica à escolha da intervenção – suplementação vitamínica (T1); a avaliação da eficácia da intervenção candidata por ensaio randomizado e controlado (T2); a avaliação da implementação e da cobertura na prática (T3); e a avaliação do impacto da intervenção (T4). Para verificar o impacto, aplicou-se a estatística superior de Wald, visando identificar quebras estruturais ao longo da série histórica da mortalidade geral de crianças entre 6 e 59 meses de idade. Para a região Nordeste, que apresentou a maior cobertura programática, o modelo sinalizou três quebras – agosto/1984, junho/1994 e maio/2006 –, nas quais foram estimadas reduções de 10%, 17% e 23%, respectivamente, na ocorrência mensal de óbitos infantis. O processo para a construção do conhecimento sobre a deficiência desta vitamina, a escolha da intervenção, a aplicação deste conhecimento no estabelecimento do Programa Nacional de Suplementação de Vitamina A e a avaliação do seu impacto configuram um exemplo de Pesquisa Translacional em saúde coletiva.

Abstract: The objective of the present study was to evaluate the association between social position and anthropometric status in women and men Brazilian adult. This was a cross-sectional study that used baseline data collected from 2008 to 2010 for the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil, in Portuguese), in the six major Brazilian state capital cities. A total of 15,105 active and retired civil servants aged from 35 to 74 years. Two latent variables were defined by latent class analysis, social position and anthropometric status. Both constructs and the analyses were separately evaluated by sex. Associations were assessed using multivariate logistic regression analysis with adjustment for age, self-reported skin color/race, and marital status. Around 44% of the women and 26% of the men were classified as overweight or obese. Social position tended to be lower in women (43.2%) and higher among men (40.4%). Heavier women were more likely to be black and brown-skinned, whereas slimmer women were more likely to be white. After adjustment, women’s weight increased as social position decreased (OR = 1.52; 95%CI: 1.36-1.70), whereas in men weight decreased as social position decreased (OR = 0.87; 95%CI: 0.76-0.99). Social position affected the anthropometric status of women and men differently, with body patterns also being affected by ethnicity/skin color, showing the potentiality of taking the intersectional perspective when investigating the possible social determinants of the phenomenon.

Background: Genetics influence the vulnerability to alcohol use disorders, and among the implicated genes, three previous studies have provided evidences for the involvement of LRRK2 in alcohol dependence (AD). LRRK2 expression is broadly dysregulated in postmortem brain from AD humans, as well as in the brain of mice with alcohol dependent-like behaviors and in a zebrafish model of alcohol preference. The aim of the present study was to evaluate the association of variants in the LRRK2 gene with AD in multiethnic populations from South and North America.Methods: Alcohol-screening questionnaires [such as CAGE and Alcohol Use Disorders Identification Test (AUDIT)] were used to determine individual risk of AD. Multivariate logistic regression analyses were done in three independent populations (898 individuals from Bambuí, Brazil; 3,015 individuals from Pelotas, Brazil; and 1,316 from the United States). Linkage disequilibrium and conditional analyses, as well as in silico functional analyses, were also conducted.Results: Four LRRK2 variants were significantly associated with AD in our discovery cohort (Bambuí): rs4768231, rs4767971, rs7307310, and rs1465527. Two of these variants (rs4768231 and rs4767971) were replicated in both Pelotas and US cohorts. The consistent association signal (at the LRRK2 locus) found in populations with different genetic backgrounds reinforces the relevance of our findings.Conclusion: Taken together, these results support the notion that genetic variants in the LRRK2 locus are risk factors for AD in humans.

Introduction: The Mycobacterium tuberculosis East African-Indian (EAI) spoligotyping family (belonging to lineage 1, Indo-Oceanic, defined by the region of deletion RD239) is distributed worldwide, but is more prevalent in Southeast Asia, India, and East Africa. Studies in Latin America have rarely identified EAI. In this study, we describe the occurrence of the EAI family in Brazil. Methods: EAI was identified in a systematic literature review of genetic diversity studies pertaining to M. tuberculosis in Brazil, as well as in a survey conducted in Salvador, Bahia, located in the northeastern region of this country. Results: The EAI6-BGD1 spoligotyping family and the EAI5 Spoligotype International Type (SIT) 1983 clade were the most frequently reported, with wide distribution of this particular clade described in Brazil. The distribution of other EAI spoligotyping patterns with broader worldwide distribution was restricted to the southeastern region of the country. Conclusions: EAI may be endemic at a low frequency in Brazil, with some clades indicating increased fitness with respect to this population. The Center for Data and Knowledge Integration for Health (CIDACS) was created in 2016 in Salvador (Bahia, Brazil). This paper aims to present a profile of CIDACS, including its current databases. CIDACS aims to conduct interdisciplinary studies and research, develop new scientific methodology and promote professional training using linked large-scale databases and high-performance computational resources in a secure environment. Administrative data is at the core of the activities conducted by CIDACS. The advantages of administrative data include significantly larger sample sizes, an inherent longitudinal structure and high-quality information. The center’s research projects are primarily focused on enhancing the understanding surrounding the impact of social protection policies (e.g., public cash-transfer and housing programs) on health outcomes in low-income populations throughout Brazil. CIDACS’ primary data source is citizens who register with the Cadastro Único program, which encompasses individuals eligible to receive benefits from over 20 governmental social programs. CIDACS has two separate environments for data handling: 1) Data Production Center, a secure room housing the computational infrastructure for ingesting, storing, cleaning, processing and linking original databases, as well as extracting research-ready datasets and 2) Data Analysis Environment, a computational infrastructure based on data safe haven principles, which allows researchers to access and process requested datasets. Brazil has a large public health community that uses national health and social databases for research programs, and the linkage of different databases has been a widely employed practice in the country. CIDACS is the result of efforts by researchers, policymakers and public health officials to use and improve the quality of Brazilian health databases. CIDACS is expected to be an important resource for researchers and policymakers interested in improving the evidence base in different aspects of health, as well as with regard to the social determinants of health and the effects of social and environmental policies on health in general.

Resumo Objetivo: analisar associação entre desfecho do tratamento da tuberculose, características sociodemográficas e benefícios sociais recebidos. Métodos: estudo de coorte desenvolvido em Salvador, Bahia, Brasil, no período 2014-2016; analisaram-se associações bivariadas entre desfecho do tratamento, características sociodemográficas e benefícios sociais. Resultados: foram acompanhados 216 indivíduos, dos quais 79,6% foram curados; maior proporção de cura associou-se com escolaridade >9 anos (87,5%; p=0,028), união conjugal (86,3%; p=0,031) e densidade domiciliar ≤2 pessoas/dormitório (84,1%; p=0,013); foram adotados como referência indivíduos com escolaridade ≤9 anos, sem união conjugal, e densidade domiciliar >2 pessoas/dormitório; maior proporção de cura também foi verificada entre indivíduos que recebiam benefícios governamentais e não governamentais (90,5%) ou somente benefícios diretos (81,6%). Conclusão: escolaridade >9 anos, união conjugal e densidade domiciliar ≤2 pessoas/dormitório associaram-se com maior proporção de cura; este desfecho foi mais frequente entre indivíduos que acumulavam benefícios governamentais e não governamentais ou recebiam somente benefícios diretos.

Background: The importance of the social determinants of health (SDH) and barriers to the access and utilization of healthcare have been widely recognized but not previously studied in the context of universal healthcare coverage (UHC) in Brazil and other developing countries. Objective: To evaluate a set of proposed indicators of SDH and barriers to the access and utilization of healthcare – proposed by the SDH unit of the World Health Organization – with respect to their relevance in tracking progress in moving toward equitable population health and UHC in Brazil. Design: This study had a mixed methodology, combining a quantitative analysis of secondary data from governmental sources with a qualitative study comprising two focus group discussions and six key informant interviews. The set of indicators tested covered a broad range of dimensions classified by three different domains: environment quality; accountability and inclusion; and livelihood and skills. Indicators were stratified according to income quintiles, urbanization, race, and geographical region. Results: Overall, the indicators were adequate for tracking progress in terms of the SDH, equity, gender, and human rights in Brazil. Stratifications showed inequalities. The qualitative analysis revealed that many of the indicators were well known and already used by policymakers and health sector managers, whereas others were considered less useful in the Brazilian context. Conclusions: Monitoring and evaluation practices have been developed in Brazil, and the set of indicators assessed in this study could further improve these practices, especially from a health equity perspective. Socioeconomic inequalities have been reduced in Brazil in the last decade, but there is still much work to be done in relation to addressing the SDH.

Over 200,000 new cases of leprosy are detected each year, of which approximately 7% are associated with grade-2 disabilities (G2Ds). For achieving leprosy elimination, one of the main challenges will be targeting higher risk groups within endemic communities. Nevertheless, the socioeconomic risk markers of leprosy remain poorly understood. To address this gap we systematically reviewed MEDLINE/PubMed, Embase, LILACS and Web of Science for original articles investigating the social determinants of leprosy in countries with > 1000 cases/year in at least five years between 2006 and 2016. Cohort, case-control, cross-sectional, and ecological studies were eligible for inclusion; qualitative studies, case reports, and reviews were excluded. Out of 1,534 non-duplicate records, 96 full-text articles were reviewed, and 39 met inclusion criteria. 17 were included in random-effects meta-analyses for sex, occupation, food shortage, household contact, crowding, and lack of clean (i.e., treated) water. The majority of studies were conducted in Brazil, India, or Bangladesh while none were undertaken in low-income countries. Descriptive synthesis indicated that increased age, poor sanitary and socioeconomic conditions, lower level of education, and food-insecurity are risk markers for leprosy. Additionally, in pooled estimates, leprosy was associated with being male (RR = 1.33, 95% CI = 1.06-1.67), performing manual labor (RR = 2.15, 95% CI = 0.97-4.74), suffering from food shortage in the past (RR = 1.39, 95% CI = 1.05-1.85), being a household contact of a leprosy patient (RR = 3.40, 95% CI = 2.24-5.18), and living in a crowded household (≥5 per household) (RR = 1.38, 95% CI = 1.14-1.67). Lack of clean water did not appear to be a risk marker of leprosy (RR = 0.94, 95% CI = 0.65-1.35). Additionally, ecological studies provided evidence that lower inequality, better human development, increased healthcare coverage, and cash transfer programs are linked with lower leprosy risks. These findings point to a consistent relationship between leprosy and unfavorable economic circumstances and, thereby, underscore the pressing need of leprosy control policies to target socially vulnerable groups in high-burden countries.

Abstract The prevalence of asthma is high in urban areas of many Latin-American countries where societies show high levels of inequality and different levels of development. This study aimed to examine the relationship between asthma symptoms prevalence in adolescents living in Latin American urban centers and socioeconomic and environmental determinants measured at the ecological level. Asthma prevalence symptoms were obtained from the International Study of Asthma and Allergies in Childhood (ISAAC) phase III. A hierarchical conceptual framework was defined and the explanatory variables were organized in three levels: distal, intermediate, proximal. Linear regression models weighed by sample size were undertaken between asthma prevalence and the selected variables. Asthma prevalence was positively associated with Gini index, water supply and homicide rate, and inversely associated with the Human Development Index, crowding and adequate sanitation. This study provides evidence of the potential influence of poverty and social inequalities on current wheezing in adolescents in a complex social context like Latin America.

O objetivo do presente estudo foi identificar fatores associados ao acesso geográfico aos serviços de saúde por portadores de tuberculose em três capitais do Nordeste do Brasil. A amostra foi composta por casos novos de tuberculose, notificados em 2007. Foram utilizados dados provenientes do Sistema de Informação sobre Agravos de Notificação, e do Cadastro Nacional de Estabelecimento de Saúde. Os endereços dos domicílios e das unidades de saúde foram georreferenciados e, utilizando a distância entre o domicílio e a unidade de atendimento de cada caso, foi considerado acesso dificultado quando esta distância foi maior do que 800 metros. Foram estimadas as razões de prevalência bruta e ajustada por meio de regressão de Poisson. Verificou-se que após ajuste com as variáveis estudadas, apenas a variável unidade básica, em Salvador, Bahia (RP = 0,75; IC95%: 0,720-0,794) e em Recife, Pernambuco (RP = 0,402; IC95%: 0,318-0,508), manteve associação com o acesso dificultado. O estudo concluiu que a descentralização do atendimento em unidade básica pode contribuir com a melhoria do acesso aos serviços de saúde.

Estudo transversal com 3.817 pré-escolares, 1.770 residentes em Salvador, Bahia, Brasil, e 2.047 nas áreas urbana e rural de 10 municípios baianos. Utilizou-se recordatório de 24 horas (R24h) e empregou-se análise fatorial por componentes principais com objetivo de identificar e comparar os principais padrões alimentares dessas crianças. Estratificou-se a amostra por idade e área. Antes dos seis meses de vida o leite materno compôs o 2o e 3o padrões com carga positiva para crianças dos dez municípios. Para menores de 17 meses, o padrão 1 foi caracterizado por leite de vaca, farinhas e açúcares. Em áreas urbanas, pão/biscoito, arroz, feijão e carne integraram o padrão 2 aos 6-17 meses. Aos 18-23 meses, o padrão 1 apresentou carga negativa para açúcares, leite de vaca e farinhas, exceto na área rural. Frutas não fizeram parte do padrão 1 no grupo de 24 meses e mais. Observou-se baixo consumo de leite materno e pouca variação de frutas e legumes a partir dos seis meses. Tal perfil de consumo alimentar indica a necessidade de intervenções cada vez mais precoces para promoção de hábitos alimentares saudáveis.

O comitê de publicações de um estudo multicêntrico visa organizar as propostas de artigos, garantindo: amplo acesso aos dados, qualidade e precedência. Foi desenvolvido um sistema online de informação e gerenciamento de propostas de estudos - o publiELSA - , composto por três módulos: (i) submissão e aprovação da proposta; (ii) acompanhamento da proposta aprovada; e (iii) relatórios consolidados. O primeiro permite a qualquer interessado buscar e conhecer artigos já publicados e em andamento, e submeter novas propostas. O processo de aprovação e transferência dos dados para o pesquisador responsável é organizado nessa etapa. O segundo módulo visa ao monitoramento das propostas aprovadas até sua publicação final e o terceiro permite buscas e visualização de propostas e artigos. O sistema tem aspectos inovadores, sobretudo o incentivo à cooperação entre os diversos pesquisadores, por meio da circulação de informes sobre cada proposta submetida. Estimula-se assim a interação dos diferentes olhares e experiências envolvidos nesta pesquisa.

MÉTODOS: Comparou-se a produção científica de pesquisadores brasileiros bolsistas 1-A do CNPq, das áreas da saúde coletiva, imunologia e medicina. Os índices-h de cada pesquisador foram estimados com base no Web of Science , Scopus e Google Acadêmico. Foram estimadas as medianas dos índices-h para os grupos de pesquisadores em cada área, e para comparar as diferenças foram usados, de acordo com cada fonte, o teste não paramétrico de Kruskal- Wallis e as comparações múltiplas de Behrens-Fisher. RESULTADOS: A área da imunologia apresentou mediana dos índices-h mais alta que os da Saúde Coletiva e da Medicina quando se utiliza a base Web of Science . Porém, essa diferença desapareceu quando a comparação foi feita utilizando a base Scopus ou o Google Acadêmico. CONCLUSÕES: A emergência do Google Acadêmico traz a um novo patamar as discussões sobre a medida do impacto bibliométrico das publicações cientificas. Áreas com fortes componentes profissionais, em que o conhecimento produzido é e deve ser publicado também na língua nativa, vis-a-vis sua difusão para a comunidade internacional, têm padrão de publicações e citações científicas diferente de áreas exclusivas ou predominantemente acadêmicas e mais bem captado pelo Google Acadêmico. OBJETIVO: Analisar a utilização do índice-h como medida do impacto bibliográfico da produção científica de pesquisadores brasileiros.

This study aimed to identify degrees of exposure to community violence reported by parents and guardians of children from four to 12 years of age and the association with childhood asthma symptoms. A cross-sectional study was conducted with 1,232 parents/guardians in 24 peripheral neighborhoods in Salvador, the capital of Bahia State, Brazil. The authors investigated asthma symptoms in children and acts of violence in the community. More than 75% of parents/guardians had been exposed to community violence in the previous year, with 20% reporting high levels of exposure. Children that were more exposed to violence showed higher asthma prevalence (28.4%) as compared to non-exposed children (16.4%). Children exposed to maximum levels of violence were nearly twice as likely to present asthma symptoms (adjusted OR = 1.94; 95%CI: 1.12-3.36). The study highlighted the relevance of community violence as a risk factor for asthma and the need for further research to elucidate methodological issues.O objetivo deste artigo foi identificar o grau de violência comunitária relatado por cuidadores de crianças entre quatro e 12 anos, além da influência dessa exposição na ocorrência de sintomas de asma nas crianças. Realizou-se um estudo transversal com 1.232 cuidadores residentes na cidade de Salvador, Bahia, Brasil. Investigaram-se sintomas de asma nas crianças e atos violentos na comunidade. Aproximadamente 75% dos cuidadores foram vítimas de violência comunitária no último ano, com 20% convivendo com graus elevados deste fenômeno. Encontrou-se prevalência de 28,4% de sintomas asmáticos entre crianças expostas ao grau máximo de violência, e 16,4% entre as não expostas. Encontrou-se associação estatisticamente significante entre exposição ao nível máximo de violência e ocorrência de sintomas asmáticos (OR ajustada = 1,94; IC95%: 1,12-3,36). Nossos achados sugerem a importância da violência comunitária como um dos fatores de risco associados com asma, sendo fundamental o desenvolvimento de novas pesquisas para esclarecer as limitações do presente estudo.

A Parent-Child Conflict Tactics Scale (CTSPC) tem sido um dos instrumentos mais utilizados internacionalmente para investigação da violência doméstica contra a criança, porém o uso segmentado de suas escalas tem se mostrado inadequado devido à complexidade do fenômeno. Esta pesquisa tem por objetivo estudar a estrutura fatorial das escalas da CTSPC em uma amostra populacional urbana do Nordeste brasileiro, sendo conduzido um estudo transversal com 1.370 crianças de uma coorte em Salvador, Bahia. Realizou-se análise fatorial com rotação oblíqua promax e calculou-se o coeficiente Kuder-Richardson. A análise fatorial revelou uma distribuição dos itens nos fatores de forma diferente da proposta do instrumento original. Verificou-se um perfil gradativo de violência em cada fator. Os valores obtidos para o Kuder-Richardson foram 0,63 para o Fator 1, 0,59 para o Fator 2 e 0,42 para o Fator 3. Os itens comportaram-se de forma diferente da proposta original do instrumento, corroborando estudos internacionais. Baseando-se nessas evidências, é possível propor um redimensionamento da CTSPC.The Parent-Child Conflict Tactics Scale (CTSPC) is one of the most widely used instruments in the world for investigating domestic violence against children, but targeted use has proven inadequate given the phenomenon's complexity. This study focused on the factor structure of CTSPC scales in an urban population in Northeast Brazil. We conducted a cross-sectional study in a cohort of 1,370 children in Salvador, Bahia State. Factor analysis with promax oblique rotation was performed, and the Kuder-Richardson coefficient was calculated. Factor analysis showed a different distribution of items in the factors as compared to the original instrument. Violence showed a gradual profile in each factor. The Kuder-Richardson coefficient was 0.63 for factor 1, 0.59 for factor 2, and 0.42 for factor 3. The items behaved differently from the original instrument, corroborating international studies. These findings support proposing a resizing of the CTSPC.

A cross-sectional study of children living in poor areas in the city of Salvador, Bahia State, Brazil, was carried out in 2006 to investigate the prevalence of use of prescribed and non-prescribed medication. This population-based study included 1,382 children aged 4-11 years. The use prescribed and non-prescribed medication during the 15 day period preceding the interview was adopted as the dependent variable. Of the 1,382 children, 663 (48%) had used at least one type of medication in the 15 days prior to the interview: in 267 cases (40.3%), mothers reported that the child had taken prescribed medication, while in 396 cases (59.7%), the child had taken medication that had not been prescribed by a physician. The most commonly prescribed drugs were analgesics (42.3%), systemic antibiotics (21.3%) and antiasthmatic (16.5%). With respect to non-prescribed drugs, the most common were analgesics (65.2%), antitussives (15.7%) and vitamins (9.3%). The results show a high prevalence of the use of non-prescription drugs among poor children, and large drug purchases of drugs by the head of household, highlighting deficiencies in coverage of the health system.A prevalência de uso de medicamentos prescritos e não prescritos foi investigada por intermédio dum estudo transversal, em crianças que vivem em áreas pobres da cidade de Salvador, Bahia, Brasil, em 2006. Estudo de base populacional que incluiu 1.382 crianças entre 4 e 11 anos de idade. O consumo de medicamentos prescritos e não prescritos nos 15 dias anteriores à entrevista foi considerado a variável dependente. Das 1.382 crianças, 663 (48%) haviam consumido ao menos um medicamento nos últimos 15 dias: 267 (40,3%) referidas pela mãe haviam consumido medicamentos prescritos e 396 (59,7%) consumiram medicamentos não prescritos. Os grupos farmacológicos prescritos mais consumidos foram analgésicos (42,3%), antibacterianos sistêmicos (21,3%) e antiasmáticos (16,5%). Os grupos farmacológicos não prescritos mais consumidos foram analgésicos (65,2%), antitussígenos (15,7%) e vitaminas (9,3%). Os resultados mostram uma alta prevalência do uso de medicamentos sem prescrição em crianças pobres, bem como elevada compra de medicamentos pelas famílias, evidenciando deficiências de cobertura do sistema de saúde.

MÉTODOS: Estudo transversal de base populacional que incluiu 1.382 crianças entre quatro e 11 anos de idade, selecionadas por amostragem aleatória de 24 micro-áreas representativas das zonas mais pobres da população residente no município de Salvador, BA, em 2006. A variável dependente foi o consumo de medicamentos nos 15 dias anteriores à realização dos inquéritos. Foram considerados três grupos de variáveis explanatórias: socioeconômicas, estado de saúde da criança e utilização dos serviços de saúde. A análise ajustada utilizou regressão de Poisson seguindo um modelo conceitual hierarquizado. RESULTADOS: A prevalência de consumo de medicamentos em crianças foi de 48%. As crianças do sexo feminino apresentaram prevalência de utilização de medicamentos superior ao sexo masculino, 50,9% e 45,4%, respectivamente (p=0,004). El análisis ajustado utilizó regresión de Poisson siguiendo un modelo conceptual jerarquizado. RESULTADOS: La prevalencia de consumo de medicamentos en niños fue de 48%. Los niños del sexo femenino presentaron prevalencia de utilización de medicamentos superior al sexo masculino, 50,9% y 45,4%, respectivamente (p=0,004). O objetivo foi identificar os fatores associados ao excesso de peso e concentração de gordura abdominal em adultos de ambos os sexos. Um estudo transversal envolvendo amostra representativa de 570 adultos de Salvador, Bahia, Brasil. A regressão de Poisson foi utilizada na análise, adotando-se o índice de massa corporal (IMC) e a circunferência da cintura como variáveis dependentes. Para as mulheres, a inadequação do IMC e da circunferência da cintura ocorreu com o aumento da idade e entre as que faziam dieta no momento do estudo, a história familiar de obesidade associou-se apenas com o IMC elevado. Para os homens, o IMC elevou-se na faixa etária de 30 a 39 e 50 a 59 anos. A circunferência da cintura elevou-se com a realização de três ou menos refeições-dia, com a hipertensão arterial, uso de bebida alcoólica e diminuiu entre mulatos e negros. Tanto IMC quanto circunferência da cintura elevaram-se com a historia familiar de infarto. Portanto são distintos os fatores que se associam ao excesso de peso e da gordura abdominal em homens e mulheres. Mas muitos deles podem ser modificados com a adoção do estilo de vida saudável.This cross-sectional study of 570 adults aimed to identify factors associated with overweight and abdominal fat in adults of both sexes in the city of Salvador, Bahia State, Brazil. Body mass index (BMI) and waist circumference were the dependent variables, and Poisson regression was used for statistical analysis. For women, high BMI and waist circumference were associated with increased age and current dieting (< 3 meals a day), while family history of obesity was only associated with increased BMI. In men, BMI increased in the 30-39 and 50-59-year age brackets. Waist circumference increased in association with < 3 meals a day, arterial hypertension, and alcohol consumption, and was lower among blacks and mixed-race individuals. High BMI and waist circumference were both associated with family history of myocardial infarction. Thus, factors associated with overweight and abdominal fat differ between men and women, but adopting healthier lifestyles can help modify many of these factors.

We identified different diarrheagenic (DEC) Escherichia coli pathotypes isolated from 1,207 children with and without acute endemic diarrhea in Salvador, Bahia, Brazil collected as part of a case-control study. Since the identification of DEC cannot be based on only biochemical and culture criteria, we used a multiplex polymerase chain reaction developed by combining five specific primer pairs for Enteropathogenic Escherichia coli (EPEC), Shiga toxin-producing E. coli/ Enterohaemorrhagic E. coli (STEC/EHEC), Enterotoxigenic E. coli (ETEC) and Enteroaggregative E. coli (EAEC) to detect these pathotypes simultaneously in a single-step reaction. In order to distinguish typical and atypical EPEC strains, these were tested for the presence of EAF plasmid. The prevalence of diarrheagenic E. coli in this sample of a global case-control study was 25.4% (259 patients) and 18.7% (35 patients) in the diarrhea group (1,020 patients) and the control group (187 patients), respectively. The most frequently isolated pathotype was EAEC (10.7%), followed by atypical EPEC (9.4%), ETEC (3.7%), and STEC (0.6%). Typical EPEC was detected only in one sample. The prevalence of the pathotypes studied in children with diarrhea was not significantly different from that in children without diarrhea.

OBJETIVO: Identificar os determinantes da desnutrição energético-protéica que ocasionam déficits ponderal e de crescimento linear em crianças. MÉTODOS: Estudo transversal envolvendo 1.041 crianças (menores de dois anos de idade) de 10 municípios do Estado da Bahia, de 1999 a 2000. Utilizou-se a técnica de regressão logística e estratégia da abordagem hierárquica para identificar os fatores associados ao estado antropométrico. RESULTADOS: O modelo final para déficit no crescimento linear revelou como determinante básico: a posse de dois ou menos equipamentos domésticos (OR=2,9; IC 95%: 1,74-4,90) e no nível subjacente, a ausência de consulta pré-natal (OR=2,7; IC 95%: 1,47-4,97); entre os determinantes imediatos o baixo peso ao nascer (OBJECTIVE: To identify determinants of protein-energy malnutrition resulting in weight and linear growth deficits in children. METHODS: A cross-sectional study was carried out among 1,041 children (under two years of age) from 10 municipalities in Bahia, Northeastern Brazil, during 1999-2000. Both logistic regression and a hierarchical approach were used to identify factors associated with the anthropometric measures. RESULTS: The basic determinant found in the final model for linear growth retardation was having up to two household appliances (OR=2.9; 95% CI: 1.74-4.90); as an underlying determinant, not attending prenatal visits (OR=2.7; 95% CI: 1.47-4.97); and, among immediate determinants were low birth weight (OR=3.6; 95% CI: 1.72-7.70) and reported hospitalization within 12 months before the interview (OR=2.4; 95% CI: 1.42-4.10). Determinants of weight deficit at the basic, underlying and immediate levels were: per capita monthly income of less than one-fourth of the minimum wage (OR=3.4; 95% CI: 1.41-8.16), not attending prenatal visits (OR=2.1; 95% CI: 1.03-4.35) and low birth weight (OR=4.8; 95% CI: 2.00-11.48) respectively. CONCLUSIONS: Children's weight and linear deficits were accounted for the overlapping of poor material living conditions, limited access to health care and disease burden. Interventions aimed at improving living conditions and better access to health care programs are strategies towards equity in children's health and nutrition.

We report the frequency of the different diarrheagenic Escherichia coli (DEC) categories isolated from children with acute endemic diarrhea in Salvador, Bahia. The E. coli isolates were investigated by colony blot hibridization whit the following genes probes: eae, EAF, bfpA, Stx1, Stx2, ST-Ih, ST-Ip, LT-I, LT-II, INV, and EAEC, as virulence markers to distinguish typical and atypical EPEC, EHEC/STEC, ETEC, EIEC, and EAEC. Seven of the eight categories of DEC were detected. The most frequently isolated was atypical EPEC (10.1%) followed by ETEC (7.5%), and EAEC (4.2%). EHEC, STEC, EIEC, and typical EPEC were each detected once. The strains of ETEC, EAEC, and atypical EPEC belonged to a wide variety of serotypes. The serotypes of the others categories were O26:H11 (EHEC), O21:H21 (STEC), O142:H34 (typical EPEC), and O?H55 (EIEC). We also present the clinical manifestations and other pathogenic species observed in children with DEC. This is the first report of EHEC and STEC in Salvador, and one of the first in Brazil.

Este artigo apresenta uma síntese de dois estudos previamente publicados e que avaliaram diferentes aspectos dos efeitos da suplementação com mega-doses (100.000 ou 200.000 UI) de vitamina A em crianças pré-escolares. O primeiro é um ensaio comunitário, aleatório, duplo cego e placebo controlado, que demonstrou o efeito positivo da suplementação na redução da severidade dos episódios de diarréia. O segundo investigou os potenciais efeitos adversos da suplementação com mega-doses de vitamina A, oferecida conjuntamente com a vacinação em massa, utilizando um desenho de intervenção controlado e não randomizado. Seus resultados indicaram que a suplementação associada com as vacinas rotineiramente utilizadas na infância não elevou as taxas de efeitos adversos (diarréia, febre e vômito). São ainda discutidas as implicações dos resultados destes estudos, que contribuíram não somente para o avanço do conhecimento científico do campo em questão, como também têm fornecido evidências para justificar a implementação de políticas e ações específicas no campo da saúde e da nutrição, que por sua vez têm contribuído para gerar condições mais auspiciosas de sobrevivência na infância. Discute-se também a pertinência e a utilidade de desenhos de estudo com diferentes níveis de rigor metodológico na avaliação de intervenção em saúde.This paper is a synthesis of two previously published studies, which evaluated different aspects of the effects of massive doses of vitamin A (100,000 or 200,000 IU) in pre-school children. The first consisted of a randomized, double blind, placebo-controlled community trial, which demonstrated the effect of supplementation on the reduction of the severity of episodes of diarrhea. The second investigated the potential adverse effects of supplementation with massive doses of vitamin A when given during mass vaccination, using a controlled but not randomized intervention design. The results of this study showed that supplementation did not increase the rates of adverse effects (diarrhea, fever and vomiting) associated with the vaccines routinely used in infancy. In conclusion, we discuss the implications of the results of these studies, which contributed not only towards the advance of scientific knowledge in the field in question but also provided evidence to justify the implementation of policies and specific actions in the field of health and nutrition which have contributed towards creating better conditions for infant survival. We also discuss the pertinence and usefulness of study designs with different levels of methodological robustness in evaluations in health.

Social determinants can affect the transmission of leprosy and its progression to disease. Not much is known about the effectiveness of welfare and primary health care policies on the reduction of leprosy occurrence. The aim of this study is to evaluate the impact of the Brazilian cash transfer (Bolsa Família Program-BFP) and primary health care (Family Health Program-FHP) programs on new case detection rate of leprosy.We conducted the study with a mixed ecological design, a combination of an ecological multiple-group and time-trend design in the period 2004-2011 with the Brazilian municipalities as unit of analysis. The main independent variables were the BFP and FHP coverage at the municipal level and the outcome was new case detection rate of leprosy. Leprosy new cases, BFP and FHP coverage, population and other relevant socio-demographic covariates were obtained from national databases. We used fixed-effects negative binomial models for panel data adjusted for relevant socio-demographic covariates. A total of 1,358 municipalities were included in the analysis. In the studied period, while the municipal coverage of BFP and FHP increased, the new case detection rate of leprosy decreased. Leprosy new case detection rate was significantly reduced in municipalities with consolidated BFP coverage (Risk Ratio 0.79; 95% CI =0.74-0.83) and significantly increased in municipalities with FHP coverage in the medium (72-95%) (Risk Ratio 1.05; 95% CI =1.02-1.09) and higher coverage tertiles (>95%) (Risk Ratio 1.12; 95% CI =1.08-1.17).At the same time the Family Health Program had been effective in increasing the new case detection rate of leprosy in Brazil, the Bolsa Família Program was associated with a reduction of the new case detection rate of leprosy that we propose reflects a reduction in leprosy incidence.

INTRODUCTION:Despite the availability of treatment and a vaccine, tuberculosis continues to be a public health problem worldwide. Mental disorders might contribute to the burden of the disease. OBJECTIVE:The objective of this study was to investigate the association between common mental disorders and tuberculosis. METHODS:A matched case-control study was conducted. The study population included symptomatic respiratory patients who attended three referral hospitals and six community clinics in the city of Salvador, Brazil. A doctor's diagnosis defined potential cases and controls. Cases were newly diagnosed tuberculosis cases, and controls were symptomatic respiratory patients for whom tuberculosis was excluded as a diagnosis by the attending physician. Cases and controls were ascertained in the same clinic. Data collection occurred between August 2008 and April 2010. The study instruments included a structured interview, a self-reporting questionnaire for the identification of common mental disorders, and a questionnaire for alcoholism. An univariate analysis included descriptive procedures (with chi-square statistics), and a multivariate analysis used conditional logistic regression. RESULTS:The mean age of the cases was 38 years, and 61% of the cases were males. After adjusting for potential confounders, the odds of tuberculosis were significantly higher in patients diagnosed with a common mental disorder (OR: 1.34; 95% CI 1.05-1.70). CONCLUSION:There appears to be a positive and independent association between common mental disorders and tuberculosis; further epidemiological studies are required to increase our understanding of the possible biological and social mechanisms responsible for this association. Independent of the direction of the association, this finding has implications for the provision of care for mental disorders and for tuberculosis.

Abstract INTRODUCTION: This study compares the clinical and epidemiological features of patients with TB, with and without DM. METHODS New cases of active pulmonary TB that occurred in Salvador, Bahia, Brazil between 2008 and 2010 were included. DM was diagnosed by based on diagnostic criteria established by the American Diabetes Association. RESULT S: Of the 323 cases of TB, 44 (13.6%) were diabetic . Patients with TB and DM were older (44.7%), with a high level of education (34.%); had slow wound healing (23.4%) and fatigue (61.7%). CONCLUSIONS Clinical symptoms differed between patients with TB, with and without DM.

RESUMO: Objetivo: Avaliar a prevalência dos indicadores de doenças crônicas não transmíssiveis (DCNT), incluindo exames laboratoriais, na população de mulheres brasileiras em idade reprodutiva segundo o recebimento do benefício Bolsa Família (BF). Métodos: Consideraram-se as 3.131 mulheres de 18 a 49 anos que participaram da submamostra de exames laboratoriais da Pesquisa Nacional de Saúde (PNS). Foram comparados indicadores entre as mulheres em idade reprodutiva (18 a 49 anos) que disseram ter ou não Bolsa Família e calculados prevalência e intervalo de confiança (IC) usando χ2 de Pearson. Resultados: Observou-se que as mulheres em idade reprodutiva beneficiárias do BF quando comparadas às não beneficiárias têm piores desfechos em saúde, como maior ocorrência de sobrepeso (33,5%) e obesidade (26,9%) (p < 0,001), hipertensão 13,4% versus 4,4% (p < 0,001), uso de tabaco (11,2%) versus 8,2% (p = 0,029), além de 6,2% perceberem sua saúde pior, em comparação a 2,4% das mulheres não beneficiárias (p

RESUMO: Objetivo: Comparar a distribuição de indicadores de doenças crônicas não transmissíveis (DCNT) entre mulheres adultas beneficiárias e não beneficiárias do Programa Bolsa Família (PBF) nas capitais brasileiras. Métodos: Análise de dados do Sistema de Vigilância de Fatores de Risco e Proteção para Doenças Crônicas por Inquérito Telefônico (Vigitel) em 2016 e 2017. Foram estimados as razões de prevalência (RP) brutas e ajustadas e seus respectivos intervalos de confiança usando o modelo de regressão de Poisson. Resultados: Mulheres do PBF tem menor escolaridade, são mais jovens e vivem com maior frequência nas regiões Nordeste e Norte do país. Prevalências mais elevadas de fatores de risco foram encontradas nas mulheres beneficiárias do PBF. A RP ajustada por idade das mulheres com BF foram: fumantes (RP = 1,98), excesso de peso (RP = 1,21), obesidade (RP = 1,63), frutas e hortaliças (RP = 0,63), consumo de refrigerantes (RP = 1,68), consumo de feijão (RP = 1,25), prática de atividade física no lazer (RP = 0,65), atividade física no domicílio (RP = 1,35), tempo assistindo à TV (RP = 1,37), autoavaliação do estado de saúde ruim (RP = 2,04), mamografia (RP = 0,86), Papanicolau (RP = 0,91), hipertensão (RP = 1,46) e diabetes (RP = 1,66). Quando comparadas as mulheres entre estratos de mesma escolaridade, as diferenças entre os fatores de risco foram reduzidas. Conclusão: Piores indicadores entre mulheres que recebem BF refletem desigualdades sociais inerentes a esse grupo mais vulnerável. O estudo evidencia também que o PBF está sendo destinado às mulheres mais vulneráveis.
