Abstract

Introduction: The COVID-19 pandemic dramatically disrupts health care around the globe. The impact of the pandemic on chronic urticaria (CU) and its management are largely unknown. Aim: To understand how CU patients are affected by the COVID-19 pandemic; how specialists alter CU patient management; and the course of CU in patients with COVID-19. Materials and Methods: Our cross-sectional, international, questionnaire-based, multicenter UCARE COVID-CU study assessed the impact of the pandemic on patient consultations, remote treatment, changes in medications, and clinical consequences. Results: The COVID-19 pandemic severely impairs CU patient care, with less than 50% of the weekly numbers of patients treated as compared to before the pandemic. Reduced patient referrals and clinic hours were the major reasons. Almost half of responding UCARE physicians were involved in COVID-19 patient care, which negatively impacted on the care of urticaria patients. The rate of face-to-face consultations decreased by 62%, from 90% to less than half, whereas the rate of remote consultations increased by more than 600%, from one in 10 to more than two thirds. Cyclosporine and systemic corticosteroids, but not antihistamines or omalizumab, are used less during the pandemic. Abstract

Many mast cell-associated diseases, including allergies and asthma, have seen a strong increase in prevalence during the past decades, especially in Western(ized) countries. It has been suggested that a Western diet may contribute to the prevalence and manifestation of allergies and asthma through reduced intake of dietary fiber and the subsequent production of their metabolites. Indeed, dietary fiber and its metabolites have been shown to positively influence the development of immune disorders via changes in microbiota composition and the regulation of B- and T-cell activation. However, the effects of these dietary components on the activation of mast cells, key effector cells of the inflammatory response in allergies and asthma, remain poorly characterized. Due to their location in the gut and vascularized tissues, mast cells are exposed to high concentrations of dietary fiber and/or its metabolites. Here, we provide a focused overview of current findings regarding the direct effects of dietary fiber and its various metabolites on the regulation of mast cell activity and the pathophysiology of mast cell-associated diseases.

Abstract

Die Vorbereitung von Inhaltsanalysen, Befragungen und Experimenten, bei denen ein bestimmter thematischer Fouks gewählt wird, bedürfen einer Auseinandersetzung mit dem Inhalt des gewählten Themas in der öffentlichen Diskussion, um relevante Themenbereiche, Akteure und Entwicklungen zu erkennen und bei der Erstellung des Instruments oder Stimulus’ berücksichtigen zu können. Im Falle von Wahlen oder Abstimmungen, welche zeitlich und thematisch stark begrenzt sind, eignet sich hierzu eine Inhaltsanalyse der Pressemitteilungen beteiligter Akteure. Bei latenten Debatten über Themen wie Arbeitslosigkeit, Immigration, Globalisierung oder Umweltschutz ist es jedoch schwer, sich einen schnellen Überblick zu verschaffen, da sich die Debatten über mehrere Monate oder Jahre hinziehen, eine Vielzahl von Akteuren beteiligt sind, und verschiedene Unterthemen von politischen Akteuren, Medien und der Bevölkerung diskutiert werden. Eine Inhaltsanalyse, die eine solche Debatte abdeckt, muss sich in diesem Fall mit der gesamten Berichterstattung eines Landes über einen Zeitraum von mehreren Monaten auseinandersetzen. Um diesen Umfang an Texten effizient zu analysieren, bieten sich automatische Analyseverfahren an (Geis 2001: 320; Krippendorff 2004: 258-259), welche jedoch ebenfalls ein Grundwissen über den Gegenstand erfordern. In vielen Fällen ist eine manuelle Vorarbeit durch den Forscher wie die Definition von Analyseeinheiten und Kategorien nötig (Früh 2007: 287; Kuckartz 2007: 24). Zudem werden Sprechressourcen wie Lexika oder Thesauri benötigt, was die automatische Inhaltsanalyse von der Verfügbarkeit dieser Ressourcen in der gewünschten Sprache abhängig macht (Früh 2007: 287). Das hier vorgestellte Verfahren des Term-Mapping bietet eine Möglichkeit, sich mit geringem Aufwand eine Übersicht über eine latente Debatte in den Medien zu verschaffen, die keiner Vorkenntnis der Themen und Akteure bedarf und unabhängig von der Sprache der Berichterstattung eingesetzt werden kann. Abstract

Seit Mitte der 90er Jahre ergänzt die Online-Befragung das Repertoire der sozialwissenschaftlichen Befragungsinstrumente. Die wachsende Popularität der Online-Befragung kann man jedoch nicht nur an ihrer zunehmenden Anwendungshäufigkeit ablesen, auch weichere Indikatoren, wie entsprechende Kapitel in Methodenlehrbüchern, lassen erkennen, dass sie mittlerweile einen festen Platz im Kanon des sozialwissenschaftlichen Methodeninstrumentariums einnimmt. Die Gründe für diesen Zuwachs liegen in den zahlreichen Vorteilen der Methode, insbesondere ihren geringen Kosten, ihrer Schnelligkeit und Flexibilität. Der durchschlagende Erfolg der Online-Befragung wurde in der wissenschaftlichen Sphäre von einer intensiven Qualitätsdiskussion begleitet, in deren Mittelpunkt vor allem die offensichtlichen Schwächen dieses Befragungsmodus standen.

7. Urticaria: Collegium Internationale Allergologicum (CIA) update 2020

Abstract

This update on chronic urticaria (CU) focuses on the prevalence and pathogenesis of chronic spontaneous urticaria (CSU), the expanding spectrum of patient-reported outcome measures (PROMs) for assessing CU disease activity, impact, and control, as well as future treatment options for CU. This update is needed, as several recently reported findings have led to significant advances in these areas. Some of these key discoveries were first presented at past meetings of the Collegium Internationale Allergologicum (CIA). New evidence shows that the prevalence of CSU is geographically heterogeneous, high in all age groups, and increasing. Several recent reports have helped to better characterize two endotypes of CSU: type I autoimmune (or autoallergic) CSU, driven by IgE to autoallergens, and type IIb autoimmune CSU, which is due to mast cell (MC)-targeted autoantibodies. The aim of treatment in CU is complete disease control with absence of signs and symptoms as well as normalization of quality of life (QoL). This is best monitored by the use of an expanding set of PROMs, to which the Angioedema Control Test, the Cholinergic Urticaria Quality of Life Questionnaire, and the Cholinergic Urticaria Activity Score have recently been added. Current treatment approaches for CU under development include drugs that inhibit the effects of signals that drive MC activation and accumulation, drugs that inhibit intracellular pathways of MC activation and degranulation, and drugs that silence MCs by binding to inhibitory receptors. The understanding, knowledge, and management of CU are rapidly increasing. The aim of this review is to provide physicians who treat CU patients with an update on where we stand and where we will go. Many questions and unmet needs remain to be addressed, such as the development of routine diagnostic tests for type I and type IIb autoimmune CSU, the global dissemination and consistent use of PROMs to assess disease activity, impact, and control, and the development of more effective and well-tolerated long-term treatments for all forms of CU.

Abstract

The European Forum for Research and Education in Allergy and Airways diseases (EUFOREA) organized its bi-annual forum EUFOREUM in Berlin in November 2023. The aim of EUFOREUM 2023 was to highlight pediatric action plans for prevention and optimizing care for type 2 inflammatory conditions starting in childhood, with a focus on early-stage diagnosis, ensuring neither under- nor overdiagnosis, optimal care, and suggestions for improvement of care. EUFOREA is an international not-for-profit organization forming an alliance of all stakeholders dedicated to reducing the prevalence and burden of chronic respiratory diseases through the implementation of optimal patient care via educational, research, and advocacy activities. The inclusive and multidisciplinary approach of EUFOREA was reflected in the keynote lectures and faculty of the virtual EUFOREUM 2023 (www.euforea.eu/euforeum) coming from the pediatric, allergology, pulmonology, ENT, dermatology, primary health care fields and patients around the central theme of type 2 inflammation. As most type 2 inflammatory conditions may start in childhood or adolescence, and most children have type 2 inflammation when suffering from a respiratory or skin disease, the moment has come to raise the bar of ambitions of care, including prevention, remission and disease modification at an early stage. The current report provides a comprehensive overview of key statements by the faculty of the EUFOREUM 2023 and the ambitions of EUFOREA allowing all stakeholders in the respiratory field to be updated and ready to join forces in Europe and beyond., (© 2024 The Author(s). Abstract

Background: Patients with chronic spontaneous urticaria (CSU), an autoimmune disorder, show increased skin expression of IL-17A and can benefit from treatment with the anti-IL-17A biologic secukinumab. The mechanisms that drive IL-17A expression in CSU are currently unknown, but may involve Semaphorin5A (Sema5A). Objective: To explore the expression, role, and effects of Sema5A in CSU and its link to IL-17A. Material and Methods: We investigated patients with CSU and healthy controls for skin expression of expressing peripheral T cells. Results: Sema5A was highly expressed in the skin of CSU patients as compared to healthy control skin. Both CD4+ T cells and mast cells in CSU skin expressed Sema5A, and many of them expressed both Sema5A and IL-17A. Patients with CSU had significantly higher rates of IL-17A-expressing CD4+ T cells as compared to healthy controls. Incubation with Sema5A increased the rates of IL-17A-expressing CD4+ T cells in healthy controls to CSU levels. Conclusion: Sema5A may drive the expression and effects of IL-17A in CSU. Further studies in larger cohorts are needed to confirm the role of Sema5A in the pathogenesis of CSU and to explore its potential as a therapeutic target.
