2. Sprechentwicklung von Kindern mit Pierre Robin-Sequenz nach Therapie mit individuell angepasster Gaumenplatte mit präepiglottischem Sporn

Abstract

Hintergrund: Durch den Einsatz der individuell angepassten Berliner Gaumenplatte mit präepiglottischem Sporn können Atem- und Schluckfunktion von Neugeborenen mit Pierre Robin-Sequenz verbessert, das Wachstum des Unterkiefers angeregt und die Weite der Gaumenspalte verringert werden. Dadurch werden die anatomischen und funktionellen Voraussetzungen für den folgenden operativen Gaumenverschluss und für eine physiologische Sprechentwicklung bereits im Kindergartenalter verbessert.Material und Methoden: In einer retrospektiven Studie wurden die Sprach- und Sprechbefunde von 22 Kindern im Alter von 3;6 bis 4;6 Jahren mit Pierre Robin-Sequenz ohne weitere entwicklungsrelevante Begleiterkrankungen aus Patientenakten der Geburtsjahrgänge 2010 bis 2017 deskriptiv ausgewertet. 22 Kinder (16 w, 6 m) erfüllten die Einschlusskriterien. U.a. wurden folgende Aspekte des Great Ormond Street Speech Assessments betrachtet: Hypernasalität, nasale Emission, nasale Turbulenz und Konsonantenartikulationsfehler.Ergebnisse: Als häufigste Auffälligkeit fanden sich bei der Hälfte der Kinder (n=11) Konsonantenartikulationsfehler, wobei diese nur bei 2 Kindern spaltassoziiert waren und ansonsten unspezifisch. Eine Hypernasalität war bei einem Kind schwer ausgeprägt und bei 7 Kindern nur minimal bis geringgradig und jeweils ohne die Sprachverständlichkeit zu beeinträchtigen. Eine (schwere) nasale Emission fand sich nur bei einem Kind, eine (geringe) nasale Turbulenz bei einem weiteren. Zum Zeitpunkt der Untersuchung erhielten 7 Kinder eine Sprach- und/oder Sprechtherapie.Diskussion: Die Therapieergebnisse sind denen anderer Behandlungszentren vergleichbar. Bei jeweils 91% bzw. 95% der Kinder zeigten sich bereits im Kindergartenalter keine gravierenden spaltassoziierten Sprechauffälligkeiten mehr in Bezug auf Konsonantenartikulationsfehler, eine nasale Emission oder Turbulenz und eine schwere Hypernasalität. Abstract

The new Medical Licensing Regulations 2025 (Ärztliche Approbationsordnung, ÄApprO) will soon be passed by the Federal Council (Bundesrat) and will be implemented step by step by the individual faculties in the coming months. The further development of medical studies essentially involves an orientation from fact-based to competence-based learning and focuses on practical, longitudinal and interdisciplinary training. Radiation oncology and radiation therapy are important components of therapeutic oncology and are of great importance for public health, both clinically and epidemiologically, and therefore should be given appropriate attention in medical education. This report is based on a recent survey on the current state of radiation therapy teaching at university hospitals in Germany as well as the contents of the National Competence Based Learning Objectives Catalogue for Medicine 2.0 (Nationaler Kompetenzbasierter Lernzielkatalog Medizin 2.0, NKLM) and the closely related Subject Catalogue (Gegenstandskatalog, GK) of the Institute for Medical and Pharmaceutical Examination Questions (Institut für Medizinische und Pharmazeutische Prüfungsfragen, IMPP). The current recommendations of the German Society for Radiation Oncology (Deutsche Gesellschaft für Radioonkologie, DEGRO) regarding topics, scope and rationale for the establishment of radiation oncology teaching at the respective faculties are also included.

13. Endoskopische Vermessung von Nasenseptumdefekten

Abstract

Hintergrund. Die vielfältigen unangenehmen Symptome von Nasenseptumdefekten (NSD) führen zu einer deutlichen Einschränkung der Lebensqualität. NSD können mittels patientenspezifischer Implantate oder durch eine Op. verschlossen werden. Implantate werden dabei durch Silikonabformungen unter Vollnarkose, ggf. in lokaler Betäubung, oder anhand von 3D-Modellen aus computertomographischen (CT-)Daten erstellt. Nachteile für die Patientensicherheit sind ein erhöhtes Morbiditätsrisiko oder die Strahlenbelastung. Methodik. Für die Hals‑, Nasen- und Ohren-(HNO)-Chirurgie wird ein schonender Ansatz zur Behandlung von NSD mit einer neuen bildbasierten, kontakt- und strahlungsfreien Messmethode unter Nutzung eines Stereoendoskops vorgestellt. Das Verfahren beruht ausschließlich auf Bilddaten und nutzt echtzeitfähige Bildverarbeitungsalgorithmen zur Berechnung von 3D-Informationen. Es ist beliebig oft wiederholbar und wurde bereits erfolgreich in der robotergestützten Chirurgie und in der OP-Mikroskopie eingesetzt. Daher wurde diese Methode für die Nasenchirurgie erweitert, für die es zusätzliche anatomische und stereoskopische Herausforderungen gibt. Ergebnisse. Nach Auswertung von 3 relevanten Messgrößen (NSD-Ausdehnung: axial, koronal und Umfang) von 6 Patienten und Vergleich der Ergebnisse von 2 Stereoendoskopen mit vorhandenen CT-Daten zeigte sich: Die bildbasierten Messergebnisse können vergleichbare Genauigkeiten wie CT-Daten erzielen. Bei einem Patienten wurden die Daten nur teilweise ausgewertet, da der NSD größer als das endoskopische Sichtfeld war. Schlussfolgerung. Aufbauend auf den sehr guten Messwerten wird ein Therapieverfahren skizziert, welches die Herstellung von patientenspezifischen NSD-Implantaten auf Basis endoskopischer Daten ermöglicht.

Abstract

Abstract For head and neck as well as for oromaxillofacial surgery, the use of the pectoralis major myocutaneous (PMMC) flap is a standard reconstructive technique after radical surgery for cancers in this region. We report to our knowledge for the first development of breast cancer in the PMMC flap in a 79 year old patient, who had undergone several operations in the past for recurring squamous cell carcinoma of the jaw. The occurrence of a secondary malignancy within the donor tissue after flap transfer is rare, but especially in the case of transferred breast tissue and the currently high incidence of breast cancer theoretically possible. Therefore preoperative screening mammography seems advisable to exclude a preexisting breast cancer in female patients undergoing such reconstruction surgery. Therapy for breast cancer under these circumstances is individual and consists of radical tumor resection followed by radiation if applicable and a standard systemic therapeutic regimen on the background of the patients individual prognosis due to the primary cancer.

Abstract

Abstract Pyoderma gangrenosum is a non-infectious neutrophilic skin disease commonly associated with underlying systemic diseases. Histopathological and laboratory diagnostics are unspecific in the majority of the cases and the diagnosis is made in accordance with the clinical picture. Here, we report the case of a 69-year old man with progredient pyoderma gangrenosum-like ulcerations under treatment with sunitinib due to hepatocellular carcinoma. A conventional ulcer therapy did not lead to a regression of the lesions. Solely cessation of sunitinib therapy resulted in an improvement of the ulcerations. Sunitinib is a multikinase inhibitor that targets the PDGF-α - and -β-, VEGF-1-3-, KIT-, FLT3-, CSF-1- and RET-receptor, thereby impairing tumour proliferation, pathological angiogenesis and metastasation. Here, we demonstrate that pyoderma gangrenosum-like ulcers may represent a serious side effect of sunitinib-based anti-cancer treatment.

Abstract

Abstract Patients with ESCC (squamous cell carcinoma of the esophagus) are most commonly diagnosed with locally advanced tumor stages. Early metastatic disease and late diagnosis are common reasons responsible for this tumor's poor clinical outcome. The prognosis of esophageal cancer is very poor because patients usually do not have symptoms in early disease stages. Squamous cell carcinoma of the esophagus frequently complicates patients with multiple co-morbidities and these patients often require interdisciplinary diagnosis and treatment procedures. At present time, neoadjuvant radiation therapy and chemotherapy followed by surgery are regarded as the international standard of care. Meta-analyses have confirmed that this approach provides the patient with better local tumor control and an increased overall survival rate. It is recommended that patients with positive tumor response to neoadjuvant therapy and who are poor surgical candidates should consider definitive radiochemotherapy without surgery as a treatment option. In future, EGFR antibodies may also be administered to patients during therapy to improve the current treatment effectiveness. Positron-emission tomography proves to be an early response-imaging tool used to evaluate the effect of the neoadjuvant therapy and could be used as a predictive factor for the survival rate in ESCC. The percentage proportions of residual tumor cells in the histopathological analyses represent a gold standard for evaluating the response rate to radiochemotherapy. In the future, early response evaluation and molecular biological tests could be important diagnostic tools in influencing the treatment decisions of ESCC patients.

Abstract Both locally advanced adenocarcinoma of the stomach and gastro-esophageal junction are associated with poor prognosis due to the lack of effective treatment. Recently multimodal treatment consisting of neoadjuvant chemotherapy in combination with radiotherapy is reported to improve survival when compared to surgery alone. Neoadjuvant therapy in these locally advanced tumors allows for early tumor responses and the extent of tumor regression that can be achieved is considered a significant prognostic factor. This, in turn, increases the resectability of these tumors. Also due to the high frequency of lymph node metastasis, patients with locally advanced adenocarcinoma should undergo a D2 lymphadenectomy. Postoperative chemoradiation and perioperative chemotherapy have been studied in gastric adenocarcinomas and showed a survival benefit. However, the surgical techniques used in these trials are no longer considered to be standard by today's surgical practice. In addition, there are no standard recommendations for adjuvant chemotherapy or chemoradiation after R0 resection and adequate lymph node dissection.
