1. NB-IoT via LEO satellites: An efficient resource allocation strategy for uplink data transmission

Abstract

In this paper, we focus on the use of Low-Eart Orbit (LEO) satellites providing the Narrowband Internet of Things (NB-IoT) connectivity to the on-ground user equipment (UEs). Conventional resource allocation algorithms for the NBIoT systems are particularly designed for terrestrial infrastructures, where devices are under the coverage of a specific base station and the whole system varies very slowly in time. The existing methods in the literature cannot be applied over LEO satellite-based NB-IoT systems for several reasons. First, with the movement of the LEO satellite, the corresponding channel parameters for each user will quickly change over time. Delaying the scheduling of a certain user would result in a resource allocation based on outdated parameters. Second, the differential Doppler shift, which is a typical impairment in communications over LEO, directly depends on the relative distance among users. Scheduling at the same radio frame users that overcome a certain distance would violate the differential Doppler limit supported by the NB-IoT standard. Third, the propagation delay over a LEO satellite channel is around 4-16 times higher compared to a terrestrial system, imposing the need for message exchange minimization between the users and the base station. In this work, we propose a novel uplink resource allocation strategy that jointly incorporates the new design considerations previously mentioned together with the distinct channel conditions, satellite coverage times and data demands of various users on Earth. Abstract

Non-terrestrial Networks (NTNs) have become an appealing concept over the last few years and they are foreseen as a cornerstone for the next generations of mobile communication systems. Despite opening up new market opportunities and use cases for the future, the novel impairments caused by the signal propagation over the NTN channel compromises several procedures of the current cellular standards. One of the first and most important procedures impacted is the random access (RA) procedure, which is mainly utilized for achieving uplink synchronization among users in several standards, such as the fourth and fifth generation of mobile communication (4 & 5G) and narrowband internet of things (NB-IoT). In this work, we analyse the challenges imposed by the considerably increased delay in the communication link on the RA procedure and propose new solutions to overcome those challenges. A trade-off analysis of various solutions is provided taking into account also the already existing ones in the literature. In order to broaden the scope of applicability, we keep the analysis general targeting 4G, 5G and NB-IoT systems since the RA procedure is quasi-identical among these technologies. Last but not least, we go one step further and validate our techniques in an experimental setup, consisting of a user and a base station implemented in open air interface (OAI), and an NTN channel implemented in hardware that emulates the signal propagation delay. The laboratory test-bed built in this work, not only enables us to validate various solutions, but also plays a crucial role in identifying novel challenges not previously treated in the literature. Abstract

Satellite communications have recently entered a period of renewed interest motivated by technological advances and nurtured through private investment and ventures. The present survey aims at capturing the state of the art in SatComs, while highlighting the most promising open research topics. Firstly, the main innovation drivers are motivated, such as new constellation types, on-board processing capabilities, nonterrestrial networks and space-based data collection/processing. Secondly, the most promising applications are described i.e. 5G integration, space communications, Earth observation, aeronautical and maritime tracking and communication. Subsequently, an in-depth literature review is provided across five axes: i) system aspects, ii) air interface, iii) medium access, iv) networking, v) testbeds & prototyping. Abstract

Satellite Communication systems are a promising solution to extend and complement terrestrial networks in unserved or under-served areas. This aspect is reflected by recent commercial and standardisation endeavours. In particular, 3GPP recently initiated a Study Item for New Radio-based, i.e., 5G, Non-Terrestrial Networks aimed at deploying satellite systems either as a stand-alone solution or as an integration to terrestrial networks in mobile broadband and machine-type communication scenarios. However, typical satellite channel impairments, as large path losses, delays, and Doppler shifts, pose severe challenges to the realisation of a satellite-based NR network. In this paper, based on the architecture options currently being discussed in the standardisation fora, we discuss and assess the impact of the satellite channel characteristics on the physical and Medium Access Control layers, both in terms of transmitted waveforms and procedures for enhanced Mobile BroadBand (eMBB) and NarrowBand-Internet of Things (NB-IoT) applications. Abstract

Introduction During COVID-19 pandemic, the use of several drugs has represented the worldwide clinical practice. However, though the current increase of knowledge about the disease, there is still no effective treatment for the usage of drugs. Thus, we retrospectively assessed use and effects of therapeutic regimens in hospitalized patients on in-hospital mortality. Methods COVOCA is a retrospective observational cohort study on 18 COVID centres throughout Campania Region Hospitals. We included adult patients with confirmed SARS-CoV-2 infection, discharged/dead between March/June 2020. Results 618 patients were included, with an overall in-hospital cumulative mortality incidence of 23.1%. Most prescribed early treatments were antivirals (72%), antibiotics (65%) and hydroxychloroquine/anticoagulants (≈50%). Tocilizumab, indeed, was largely prescribed late during hospitalization. Multivariable models, with a cut-off at day 2 for early COVID-19 therapy administration, did not disclose any significant association of a single drug administration on the clinical outcome. Discussion COVOCA represents the first multicenter database in Campania region. None drug class used during the pandemic significantly modified the outcome, regardless of therapy beginning, both overall and net of those already in non-invasive ventilation (NIV)/ orotracheal intubation (OTI) at hospitalization. Our cumulative incidence of mortality seems lower than other described during the same period, particularly in Northern Italy.

Abstract

For many years, satellite footprints have been fixed from the design phase until the last day of the satellite operational life. Flexibility in coverage by means of reconfigurable beams is becoming increasingly popular thanks to the recent developments in active antenna systems. On the other hand, spatial frequency reuse combined with precoding has been shown to boost the spectral efficiency while lowering the cost per bit. In this context, and motivated by the unbalanced demand requests of the satellite users, we propose a shift from the traditional system-throughput maximization design towards a demand-Aware design, where a new beam shaping technique and user scheduling are combined to satisfy the users’ demands. Supporting numerical results are provided that validate the effectiveness of the proposed beam planning and scheduling and quantify the benefits over conventional rigid techniques.

Abstract

Clinical presentation of COVID-19 is common to other respiratory infections. We compared the characteristics at hospital admission of confirmed and not-confirmed COVID-19 patients, in the early phase of the epidemic. Thirty-seven suspected patients were enrolled, and COVID-19 was confirmed in 17. Confirmed patients are older, have more frequently contact with confirmed cases. Distinctive clinical characteristics among COVID-19 were the grand-glass opacities at CT scan, and a pO2/FiO2 ratio less than 250. In not-confirmed group, Influenza represented the most frequent alternative diagnosis. This study contributes to highlight the characteristics to consider at hospital admission in order to promptly suspect COVID-19.

Abstract

As compared to terrestrial systems, the design of Satellite Communication (SatCom) systems require a different approach due to differences in terms of wave propagation, operating frequency, antenna structures, interfering sources, limitations of onboard processing, power limitations and transceiver impairments. In this regard, this letter aims to identify and discuss important modeling and design aspects of the next generation High Throughput Satellite (HTS) systems. First, communication models of HTSs including the ones for multibeam and multicarrier satellites, multiple antenna techniques, and for SatCom payloads and antennas are highlighted and discussed. Subsequently, various design aspects of SatCom transceivers including impairments related to the transceiver, payload and channel, and traffic-based coverage adaptation are presented. Abstract

Background The best treatment option for patients with type 2 diabetes in whom treatment with metformin alone fails to achieve adequate glycaemic control is debated. We aimed to compare the long-term effects of pioglitazone versus sulfonylureas, given in addition to metformin, on cardiovascular events in patients with type 2 diabetes. Methods TOSCA.IT was a multicentre, randomised, pragmatic clinical trial, in which patients aged 50–75 years with type 2 diabetes inadequately controlled with metformin monotherapy (2–3 g per day) were recruited from 57 diabetes clinics in Italy. Patients were randomly assigned (1:1), by permuted blocks randomisation (block size 10), stratified by site and previous cardiovascular events, to add-on pioglitazone (15–45 mg) or a sulfonylurea (5–15 mg glibenclamide, 2–6 mg glimepiride, or 30–120 mg gliclazide, in accordance with local practice). The trial was unblinded, but event adjudicators were unaware of treatment assignment. The primary outcome, assessed with a Cox proportional-hazards model, was a composite of first occurrence of all-cause death, non-fatal myocardial infarction, non-fatal stroke, or urgent coronary revascularisation, assessed in the modified intention-to-treat population (all randomly assigned participants with baseline data available and without any protocol violations in relation to inclusion or exclusion criteria). This study is registered with ClinicalTrials.gov, number NCT00700856. Findings Between Sept 18, 2008, and Jan 15, 2014, 3028 patients were randomly assigned and included in the analyses. 1535 were assigned to pioglitazone and 1493 to sulfonylureas (glibenclamide 24 [2%], glimepiride 723 [48%], gliclazide 745 [50%]). At baseline, 335 (11%) participants had a previous cardiovascular event. The study was stopped early on the basis of a futility analysis after a median follow-up of 57·3 months. Abstract

This paper is the result of a study promoted by the European Space Agency, aimed at research, design, development, and demonstration of a state-of-the-art receiver chain for telecommand (TC) links in space missions. The introduction of new, very powerful low-density parity-check codes for error correction, recently approved for the TC standard, allows to operate at significantly lower signal-to-noise ratios. This implies a number of challenges for the receivers in terms of frame synchronization, carrier acquisition, and tracking loops. Both the decoding and the receiver algorithms must be optimized as well, in view of permitting efficient hardware and software implementation, while ensuring excellent error rate performances. All these issues are faced in the present study through a fruitful co-operation between industry, research centers and universities. Abstract

In this paper, we study the performance of telecommand space links affected by pulsed, continuous wave and pseudo-noise jamming. Countermeasures include coding, interleaving, and direct sequence spread spectrum. Binary and non-binary low-density parity-check codes, parallel turbo codes, and soft-decision decoded BCH codes are considered. We investigate the impact of different decoding algorithms, also taking into account the role of jamming state information, spreading processing gain and interleaving. Abstract

We investigate and compare, over the additive white Gaussian noise channel, different options for updating the error correcting code currently used in space mission telecommand links. Besides some more consolidated proposals, using low-density parity-check codes, we characterize and assess the performance of alternative schemes, based on parallel turbo codes and soft-decision decoded Bose-Chaudhuri-Hocquenghem (BCH) codes. The analysis considers relevant metrics like the codeword error rate, the frame error rate, and the undetected frame error rate. The considered codes include binary and non-binary low-density parity-check codes, parallel turbo codes and extended BCH codes, with different decoding algorithms. The complexity of the various schemes and possible limits for their application are considered and discussed. Several numerical examples are provided. Abstract

Key distribution and renewing in wireless local area networks is a crucial issue to guarantee that unauthorized users are prevented from accessing the network. In this paper, we propose a technique for allowing an automatic bootstrap and periodic renewing of the network key by exploiting physical layer security principles, that is, the inherent differences among transmission channels. The proposed technique is based on scrambling of groups of consecutive packets and does not need the use of an initial authentication nor automatic repeat request protocols. We present a modification of the scrambling circuits included in the IEEE 802.11g standard which allows for a suitable error propagation at the unauthorized receiver, thus achieving physical layer security.

Abstract

NEXCODE is a project promoted by the European Space Agency aimed at research design development and demonstration of a receiver chain for telecomm and links in space missions including the presence of new short low-density parity-check codes for error correction. These codes have excellent performance from the error rate viewpoint but also put new challenges as regards synchronization issues and implementation. In this paper after a short review of the results obtained through numerical simulations we present an overview of the breadboard designed for practical testing and the test-plan proposed for the verification of the breadboard and the validation of the new codes and novel synchronization techniques under relevant operation conditions., Grant numbers : NEXCODE (Next Generation Uplink Coding Techniques)© 2016 IEEE. Abstract

Background Italy has been the first Western country to be heavily affected by the spread of SARS-COV-2 infection and among the pioneers of the clinical management of pandemic. To improve the outcome, identification of patients at the highest risk seems mandatory. Objectives Aim of this study is to identify comorbidities and clinical conditions upon admission associated with in-hospital mortality in several COVID Centers in Campania Region (Italy). Methods COVOCA is a multicentre retrospective observational cohort study, which involved 18 COVID Centers throughout Campania Region, Italy. Data were collected from patients who completed their hospitalization between March-June 2020. The endpoint was in-hospital mortality, assessed either from data at discharge or death certificate, whilst all exposure variables were collected at hospital admission. Results Among 618 COVID-19 hospitalized patients included in the study, 143 in-hospital mortality events were recorded, with a cumulative incidence of about 23%. At multivariable logistic analysis, male sex (OR 2.63, 95%CI 1.42–4.90; p = 0.001), Chronic Liver Disease (OR 5.88, 95%CI 2.39–14.46; p Conclusion Mortality of patients hospitalized for COVID-19 appears strongly affected by both clinical conditions on admission and comorbidities. Originally, we observed a very poor outcome in subjects with a chronic liver disease, alongside with an increase of hepatic damage.

Abstract

A coronavirus disease 2019 (COVID-19) surveillance study was performed in March-April 2020 among asymptomatic healthcare workers (HCWs) at a specialist infectious diseases hospital in Naples, Italy. All HCWs underwent two rounds of molecular and serological testing for severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2). One hundred and fifteen HCWs were tested; of these, two cases of infection were identified by reverse transcriptase polymerase chain reaction and two HCWs were SARS-CoV-2 immunoglobulin G seropositive. The overall prevalence of current or probable previous infection was 3.4%. The infection rate among HCWs was reasonably low. Most of the infected HCWs had been asymptomatic for the preceding 30 days, which supports the need for periodic screening of HCWs for COVID-19.

Abstract

This paper deals with the feasibility of the new receiver scheme for telecommand space links, based on the adoption of short low-density parity-check codes recently introduced in the standard. Being able to reduce significantly the required signal-to-noise ratio, these codes have an impact on the acquisition, tracking and synchronization issues. All these aspects have been faced, both theoretically and practically through the realization of a breadboard that implements the core elements of the on-board receiver, and is able to demodulate and decode uplink signals employing the new codes. The breadboard incorporates innovative solutions to cope with the acquisition and synchronization problems, and a pioneering implementation of non-iterative decoders based on the most reliable basis algorithm.

Abstract

Space-to-Earth telemetry (TM) links have extensively exploited advanced coding schemes, from convolutional codes to Reed-Solomon codes. The Next Generation Uplink Coding Techniques (NEXCODE) study, funded under the Technology Research Program of the European Space Agency (ESA), aimed at research, design, development, and demonstration of a TC receiver chain for scientific missions, including new channel codes. Agencies and researchers investigated and compared a number of possible alternatives, including binary and non-binary LDPC codes even in the presence of jamming. The schemes were evaluated taking into account the peculiar characteristics of TC links in terms of coding gain and complexity. Commands from Earth to space are sent in the payload of TC transfer frames (TFs). A TF is divided into blocks, each as long as 56 bits and individually BCH encoded. The TC transmission protocol includes an additional mechanism that guarantees a minimum number of symbol transitions to keep symbol synchronization. In acquisition mode, the TC standard [15] specifies the use of a symmetric triangular carrier sweeping procedure with the purpose of locking to the carrier frequency. In this stage, the ground station (G/S) transmits an unmodulated carrier whose frequency is swept around the nominal carrier frequency. Carrier tracking in the baseline receiver is performed with a 2nd order PLL, which is compromised in the DS scenario by the noise reduction versus dynamic range trade-off.

Abstract

In this paper, an iterative soft-decision hybrid decoding algorithm for non-binary low-density parity-check (LDPC) codes with short codeword lengths is proposed. The rationale of the approach is to combine the classical belief propagation (BP) iterative LDPC decoding algorithm with the most reliable basis (MRB) decoding algorithm. This allows to achieve significant performance improvements, with a complexity that, for medium/low error rates, is only slightly higher than that of the BP algorithm alone. The performance improvement with respect to pure BP decoding is up to 0.7 dB at codeword error rate (CER) ≈ 10 -5 . Notably, for a fixed MRB order, hybrid decoding achieves a gain up to 0.5 dB at CER ≈ 10 -5 with respect to BP decoding and MRB decoding used alone.

Abstract

Background & Aims: SARS-Cov-2 infection manifests as a wide spectrum of clinical presentation and even now, despite the global spread of the vaccine, contagiousness is still elevated. The aim of the study was the evaluation of the impact of liver fibrosis assessed by FIB-4 and liver impairment, assessed by cytolysis indices, on intrahospital mortality in COVID-19 subjects., Methods: This is a retrospective observational cohort study, which involved 23 COVID Hospital Units in Campania Region, Italy. Exposure variables were collected during hospital admission and at discharge. According to FIB-4 values, we subdivided the overall population in three groups (FIB-4<1.45; 1.453.25), respectively group 1,2,3., Results: At the end of the study, 938 individuals had complete discharged/dead data. At admission, 428 patients were in group 1 (45.6%), 387 in group 2 (41.3%) and 123 in group 3 (13.1%). Among them, 758 (81%) subjects were discharged, while the remaining 180 (19%) individuals died. Multivariable Cox's regression model showed a significant association between mortality risk and severity of FIB-4 stages (group 3 vs group 1, HR 2.12, 95%CI 1.38-3.28, p<0.001). Moreover, Kaplan-Meier analysis described a progressive and statistically significant difference (p<0.001 Log-rank test) in mortality according to FIB-4 groups. Among discharged subjects, 507 showed a FIB-4<1.45 (66.9%, group 1), 182 a value 1.453.25 (9.0%, group 3). Among dead subjects, 42 showed a FIB-4<1.45 (23.3%, group 1), 62 a value 1.453.25 (42.3%, group 3)., Conclusions: FIB-4 value is significantly associated with intrahospital mortality of COVID-19 patients.
