24. Continence nurse treatment of women's urinary symptoms.

Abstract

The aim of this study was to establish how accurately a trained continence nurse could allocate appropriate second-line conservative treatment to women without urodynamic investigations. Depending on the number of patients coming taking up the service, there were between five and 12 nurses operating at any one time. Women aged 40 years and over, of which there were 2421 reporting lower urinary tract symptoms, were randomly allocated to a new nurse-led continence service. Of these women, 450 subsequently underwent urodynamic investigation, before which the nurses documented which second-line conservative treatment would be appropriate. The results showed that of all women with detrusor overactivity, 79.1% were correctly allocated anticholinergic treatment, and 64.8% were allocated pelvic floor exercises (PFE). Of all women with urodynamic stress incontinence, 88.8% were correctly allocated PFE, and 43.4% were allocated anticholinergic therapy. Of all women with mixed incontinence, 44.8% were correctly allocated both PFE and anticholinergic therapy, while 52.1% were allocated only one treatment. This study showed that a trained continence nurse is able to allocate conservative treatment appropriately the majority of women without the need for urodynamic investigation. This indicated that the management of urinary dysfunction by a team of trained, dedicated nurses has the potential to reduce waiting lists for urodynamic investigation, avoid unnecessary investigations and achieve greater patient satisfaction. 36. Excavations at Jemdet Nasr, 1988

Abstract

Under the auspices of the British Archaeological Expedition to Iraq a first season of renewed excavations at Jemdet Nasr took place from 19th October to 31st December 1988. In the first place we record our gratitude to the people and institutions who enabled the excavations to take place. To Dr. Mu'ayyad Said Damerji, Director-General of the Department of Antiquities and Heritage, and the staff of the External Relations section, go our sincere thanks for their cooperation throughout the excavations. The assistance of Dr. Behija Khalil Ismail, Director of the Iraq Museum, is also gratefully acknowledged. The personal thanks of all the team go to our representative, Sd. Borhan Shakir Sulayman, whose much-appreciated assistance in all aspects of the work contributed in no small measure to the achievements of the season. 37. Excavations at Abu Salabikh, 1985–86

Abstract

The following report covers two full seasons of work at Abu Salabikh, breaking with our previous practice of writing a report for this journal after each season. As well as reducing the space absorbed by each season, this has the advantage that work which was conceived on a two-season schedule as a single programme can be presented together. There was no work at the site in 1984, and none is planned on the Early Dynastic mounds in 1987; provisionally we propose to conduct another two seasons of work there in 1988–89, followed by a break in 1990. It is hoped that during the fallow years it will be possible to keep the programme of final publications going.

43. 'A perfect storm”: Autistic experiences of menopause and midlife.

Abstract

Previous research indicates that menopause can be an extremely difficult transition for some autistic people. This study asks how autistic people experience menopause and how they can better access services, support and information; autistic Community Research Associates played an important role in each stage of the research. Online focus groups and interviews were conducted with 24 autistic participants who lived in Canada (n = 13) or the United Kingdom (n = 11) and had experience with the menopausal transition. Transcripts were coded and analysed by four team members using reflexive thematic analysis. Four themes and eight subthemes were identified: (1) Complexity, multiplicity and intensity of symptoms (0 subthemes); (2) Life experience and adversity converging at midlife (three subthemes); (3) The importance of knowledge and connection (two subthemes); and (4) Barriers to support and care (three subthemes). Limitations include a potential sample bias towards difficult experiences of menopause. The majority of our sample had a late diagnosis or discovery of autism, and their experiences might not generalize to wider autistic populations. This research may help autistic people prepare for menopause and recognize symptoms earlier. Hearing about the experiences of others may let autistic people who struggle with menopause know they are not alone.
