3. Lifecourse determinants of bone mass accrual in a transitional rural community in South India : the Andhra Pradesh Children and Parents Study (APCAPS)

Abstract

In urbanizing rural communities in India, bone development in children and adolescents may be affected by nutrition transition. This thesis work used a lifecourse approach to investigate the combined effects of early life undernutrition and urbanized lifestyles in late adolescence on bone mass accrual in a rural area in southern India that has been undergoing socioeconomic development over the past decade. The analyses used data from a prospective cohort study near Hyderabad city in India, the Andhra Pradesh Children and Parents Study (APCAPS). Research Paper 1 The first study examined bone mass in the population of APCAPS. Over 50% of female and 30% of male participants aged 50 years and above had osteopenia or osteoporosis. Peak bone mass was low in this lean rural population (hip BMD in 2029 year old female: 0.84±0.1; male: 0.95±0.11 g/cm2) in comparison to the reference values from a healthy Indian population in the same age group (female: 0.90±0.11; male: 0.99±0.13). There was clearer evidence of positive association between hip BMD and lean mass than fat mass in this population. Research Paper 2: The second study examined longitudinal effects of early life proteinenergy supplementation on bone mass in young adulthood, adjusting for current body size and lifestyles. There was no clear evidence for a longterm positive effect of prenatal and infant exposure to modest proteinenergy supplementation on bone mass accrual in this rural community. Greater lean mass in young adulthood was associated with higher hip and lumbar spine BMD. Weightbearing physical activity was positively associated with hip, LS, and wholebody BMD in males. Research Paper 3: The third study assessed longitudinal effects of low body mass index (BMI) during adolescence on bone mass in young adulthood. When adjusted for current BMI, there was no clear evidence for association of hip and LS BMD in young adulthood with adolescent BMI. Controlling for adolescent BMI, greater increase in BMI during adolescence was associated with higher BMD in young adulthood, although it was 3 still suboptimal in comparison to a healthy young Indian population. Concluding remarks In this transitional rural community from south India, the prevalence of osteopenia and osteoporosis was high, especially among females. Although neither modest early life supplementation nor weight gain in late adolescence was sufficient to achieve optimal peak bone mass, increase in body size in late adolescence and young adulthood was beneficial for bone mass accrual in this young population. There is a need to develop strategies to take advantage of, and also prevent any detrimental effects of, nutrition transition to improve peak bone mass in transitional rural communities in India.

Abstract

1. This study elucidated the last-male sperm precedence (LMSP) mechanism in chickens by examining replacement in storage tubules (SSTs) after multiple artificial inseminations (AI) and the effects of seminal plasma (SP) and male breed on sperm replacement in SSTs. 2. Hens were artificially inseminated with fluorescent dye-labelled spermatozoa from White Leghorn (WL) chickens. Secondary AI was conducted 3 d later with sperm labelled with different nuclear fluorescent dye. Percentage of first and second inseminated sperm in SSTs and their logarithmic odds were calculated. The effect of SP on LMSP was examined using (1) Lake's solution-washed sperm before second insemination, and (2) SP injected continuously after first insemination. Effect of breed difference on sperm replacement was investigated using Barred Plymouth Rock (BP) sperm. 3. Successive WL-sperm inseminations at three-day intervals caused > 70% stored sperm replacement in SSTs. Although SP removal from sperm from second insemination significantly decreased replacement, its intra-vaginal injection did not affect release. Secondary insemination using BP sperm significantly increased replacement. 4. Sperm replacement is a major factor favouring LMSP in domestic chickens. Two fluorescent staining of sperm, and intra-vaginal multiple AI technique have enabled visualisation, differentiation, and quantification of multiple inseminated sperm stored in the SSTs. Abstract

Using the Polyakov extended Nambu-Jona-Lasinio (PNJL) model with imaginary chemical potential, the relation between the Roberge-Weiss (RW) phase transition and the extended Z3 symmetry is studied. At low temperature, there is approximate continuous symmetry under the phase transformation of the Polyakov loop with the shift of the imaginary chemical potential. Due to this continuous symmetry, the Polyakov loop can oscillate smoothly as the imaginary chemical potential increases. At high temperature, this continuous symmetry is broken to an exact discrete symmetry, the extended Z3 symmetry, and the Polyakov loop can not oscillate smoothly. This symmetry breaking of the continuous symmetry causes a discontinuity of the Polyakov loop. Starting from many quark interactions, we construct a nonlinear sigma-omega model at finite temperature and density. The mesons are introduced as auxiliary fields. Effective quark-meson couplings are strongly related to effective meson masses, since they are derived simultaneously from the original many quark interactions. Abstract

Background: The effects of pharmacological blood-pressure-lowering on cardiovascular outcomes in individuals aged 70 years and older, particularly when blood pressure is not substantially increased, is uncertain. We compared the effects of blood-pressure-lowering treatment on the risk of major cardiovascular events in groups of patients stratified by age and blood pressure at baseline. Methods: We did a meta-analysis using individual participant-level data from randomised controlled trials of pharmacological blood-pressure-lowering versus placebo or other classes of blood-pressure-lowering medications, or between more versus less intensive treatment strategies, which had at least 1000 persons-years of follow-up in each treatment group. Participants with previous history of heart failure were excluded. Data were obtained from the Blood Pressure Lowering Treatment Triallists' Collaboration. We pooled the data and categorised participants into baseline age groups (<55 years, 55–64 years, 65–74 years, 75–84 years, and ≥85 years) and blood pressure categories (in 10 mm Hg increments from <120 mm Hg to ≥170 mm Hg systolic blood pressure and from <70 mm Hg to ≥110 mm Hg diastolic). We used a fixed effects one-stage approach and applied Cox proportional hazard models, stratified by trial, to analyse the data. The primary outcome was defined as either a composite of fatal or non-fatal stroke, fatal or non-fatal myocardial infarction or ischaemic heart disease, or heart failure causing death or requiring hospital admission. Findings: We included data from 358 707 participants from 51 randomised clinical trials. The age of participants at randomisation ranged from 21 years to 105 years (median 65 years [IQR 59–75]), with 42 960 (12·0%) participants younger than 55 years, 128 437 (35·8%) aged 55–64 years, 128 506 (35·8%) 65–74 years, 54 016 (15·1%) 75–84 years, and 4788 (1·3%) 85 years and older. Abstract

Background: The effects of pharmacological blood pressure lowering at normal or high-normal blood pressure ranges in people with or without pre-existing cardiovascular disease remains uncertain. We analysed individual participant data from randomised trials to investigate the effects of blood pressure lowering treatment on the risk of major cardiovascular events by baseline levels of systolic blood pressure. Methods: We did a meta-analysis of individual participant-level data from 48 randomised trials of pharmacological blood pressure lowering medications versus placebo or other classes of blood pressure-lowering medications, or between more versus less intensive treatment regimens, which had at least 1000 persons-years of follow-up in each group. Trials exclusively done with participants with heart failure or short-term interventions in participants with acute myocardial infarction or other acute settings were excluded. Data from 51 studies published between 1972 and 2013 were obtained by the Blood Pressure Lowering Treatment Trialists' Collaboration (Oxford University, Oxford, UK). We pooled the data to investigate the stratified effects of blood pressure-lowering treatment in participants with and without prevalent cardiovascular disease (ie, any reports of stroke, myocardial infarction, or ischaemic heart disease before randomisation), overall and across seven systolic blood pressure categories (ranging from <120 to ≥170 mm Hg). The primary outcome was a major cardiovascular event (defined as a composite of fatal and non-fatal stroke, fatal or non-fatal myocardial infarction or ischaemic heart disease, or heart failure causing death or requiring admission to hospital), analysed as per intention to treat. Findings: Data for 344 716 participants from 48 randomised clinical trials were available for this analysis. Abstract

Purpose: Substance use is linked with poor outcomes among people living with HIV (PLWH) and is associated with mental health disorders. This analysis examines the impact of decreasing substance use, even without abstinence, on depressive symptoms among PLWH. Methods: Data are from PLWH enrolled in the Centers for AIDS Research Network of Integrated Clinical Sites cohort. Participants completed longitudinal assessments of substance use (modified ASSIST) and depressive symptoms (PHQ-9). Changes in substance use frequency were categorized as abstinence, reduced use, and nondecreasing use. Adjusted linear mixed models with time-updated change in substance use frequency and depressive symptom scores were used to examine associations between changes in the use of individual substances and depressive symptoms. Analyses were repeated using joint longitudinal survival models to examine associations with a high (PHQ-9 ≥10) score. Results: Among 9905 PLWH, 728 used cocaine/crack, 1016 used amphetamine-type substances (ATS), 290 used illicit opiates, and 3277 used marijuana at baseline. Changes in ATS use were associated with the greatest improvements in depressive symptoms: stopping ATS led to a mean decrease of PHQ-9 by 2.2 points (95% CI: 1.8 to 2.7) and a 61% lower odds of PHQ-9 score ≥10 (95% CI: 0.30 to 0.52), and decreasing ATS use led to a mean decrease of 1.7 points (95% CI: 1.2 to 2.3) and a 62% lower odds of PHQ-9 score ≥10 (95% CI: 0.25 to 0.56). Stopping and reducing marijuana and stopping cocaine/crack use were also associated with improvement in depressive symptoms. Conclusions: We demonstrated that both substance use reduction and abstinence are associated with improvements in depressive symptoms over time.

Abstract

Summary Background The cardiovascular benefits of blood pressure lowering in obese people compared with people of normal weight might depend on choice of drug. We compared the effects of blood pressure-lowering regimens on cardiovascular risk in groups of patients categorised by baseline body-mass index (BMI). Methods We used individual patient data from trials included in the Blood Pressure Lowering Treatment Trialists' Collaboration to compare the effects of different classes of blood pressure-lowering regimens for the primary outcome of total major cardiovascular events (stroke, coronary heart disease, heart failure, and cardiovascular death). We used meta-analyses and meta-regressions to assess interactions between treatment and BMI when fitted as either a categorical variable (<25 kg/m2, 25 to <30 kg/m2, and ≥30 kg/m2) or a continuous variable. Findings Analyses were based on 135 715 individuals from 22 trials who had 14 353 major cardiovascular events. None of the six primary comparisons showed evidence that protection varied by drug class across the three BMI groups (all p for trend >0·20). When analysed as a continuous variable, angiotensin-converting-enzyme inhibitors gave slightly greater protection for each 5 kg/m2 higher BMI than did calcium antagonists (hazard ratio 0·93, 95% CI 0·89-0·98; p=0·004) or diuretics (0·93, 0·89-0·98; p=0·002). The meta-regressions showed no relation between BMI category and the risk reduction for a given fall in systolic blood pressure. By contrast with a previous report, we noted no relation between BMI and the efficacy of calcium antagonists compared with diuretics. Interpretation We found little evidence that selection of a particular class of blood pressure-lowering drug will lead to substantially different outcomes for individuals who are obese compared with those who are lean.
