Dentre as medidas n?o farmacol?gicas empregadas para a melhora da sensibilidade ? insulina est? o treinamento f?sico aer?bio, como o treinamento intervalado de alta intensidade (HIIT, do ingl?s high intensity interval training). Sendo assim, esse estudo avaliou os efeitos do HIIT em componentes bioqu?micos, celulares e moleculares relacionados ? resist?ncia ? insulina em obesos. Indiv?duos obesos sens?veis (n=9) e resistentes ? insulina (n=8) foram submetidos a 8 semanas de HIIT, em cicloerg?metro, realizado 3 vezes por semana, com intensidade e volume progressivos (8 a 12 est?mulos; 80 a 110% da pot?ncia m?xima). Amostras de sangue venoso e do m?sculo vasto lateral foram obtidas antes e ap?s o programa de HIIT. Ap?s o programa de treinamento houve aumento da sensibilidade ? insulina nos obesos resistentes ? insulina, mas n?o houve redu??o da massa de gordura. A concentra??o de citocinas no soro, o estresse oxidativo sist?mico e frequ?ncia das c?lulas imunes n?o foram modificadas ap?s o treinamento. No m?sculo esquel?tico, o HIIT promoveu aumento da fosforila??o do substrato do receptor de insulina (IRS) (Tyr612), da Akt (Ser473) e da prote?na quinase dependente de c?lcio/calmodulina (CAMKII) (Thr286), e aumento do conte?do da ?-hidroxiacil-CoA desidrogenase (?-HAD) e citocromo C oxidase (COX-IV). Houve ainda, redu??o da fosforila??o da quinase regulada por sinal extracelular (ERK1/2) nos obesos resistentes ? insulina. Conclu?mos que 8 semanas de HIIT promoveram melhora da sensibilidade ? insulina, modificou componentes da via de sinaliza??o da insulina e do metabolismo oxidativo no m?sculo esquel?tico. Obesity is characterized by excess of body fat, and its development can lead to a variety of metabolic disorders, including insulin resistance. Exercise is recognized as a non-pharmacological approach to increasing skeletal muscle insulin sensitivity, although the mechanisms are not elucidated. Additionally, the understanding of high intensity interval training (HIIT, high intensity interval training) treat insulin resistance is less understood. Therefore, this study evaluated the effects of HIIT on biochemical, molecular, and cellular markers related to insulin resistance in sedentary obese individuals. Sensitive (n=9) and insulin resistant (n=8) obese individuals (body mass index ? 30 kg/m-2) were engaged in 8 weeks of HIIT using a cycle ergometer. The HIIT was performed 3 times a week, and its intensity and volume progressively increased throughout the training period (from 8 to 12 stimuli; from 80 to 110% of the maximum power). Venous blood and the vastus lateralis muscle samples were obtained before and after the HIIT. HIIT enhanced insulin sensitivity in insulin-resistant obese individuals without changing body fat mass. Cytokine concentration in serum, blood oxidative stress, and frequency of some immune cells were not altered by HIIT. In skeletal muscle, HIIT increased the phosphorylation of insulin receptor substrate (IRS) (Tyr612), Akt (Ser473), and protein kinase dependent calcium/calmodulin (CaMKII) (Thr286). HIIT also increased the expression of ?-hydroxyacyl-CoA dehydrogenase (?-HAD) and cytochrome C oxidase (COX-IV). A reduction of the kinase phosphorylation of extracellular signal-regulated (ERK1/2) was only seen in obese insulin resistant individuals. The results show that 8 weeks of HIIT enhanced insulin sensitivity, modified components of the insulin-signaling pathway, and improved skeletal muscle oxidative metabolism. These changes were independent of alterations in body fat and inflammatory parameters.

Em obesos, a resist?ncia a insulina correlaciona-se com altera??es do sistema imune, e com a baixa express?o da prote?na de choque t?rmico de 72kDa (Hsp72) e aumento da ativa??o da JNK no m?sculo esquel?tico. Considerando que o exerc?cio f?sico aer?bico promove melhora da sensibilidade a insulina e tem um efeito anti-inflamat?rio. O objetivo desse estudo foi avaliar os efeitos de uma sess?o de exerc?cio f?sico aer?bico na express?o da HSP70, ativa??o da JNK e fosforila??o do IRS-1 no res?duo de serina 612 (IRS-1 ser612) no m?sculo esquel?tico de obesos. Al?m disso, foi avaliada a frequ?ncia dos linf?citos T auxiliares (CD4+) e citol?ticos (CD8+) e das subpopula??es de mon?citos cl?ssicos (CD14++CD16-), intermedi?rios (CD14++CD16+) e n?o cl?ssicos (CD14+CD16++). Os participantes do estudo (n=27) foram alocados em tr?s grupos experimentais (eutr?ficos sens?veis a insulina, obesos sens?veis a insulina, obesos resistentes a insulina) de acordo com a classifica??o do estado nutricional, de acordo com o ?ndice de massa corporal e presen?a ou n?o de resist?ncia a insulina, definida pelo modelo de avalia??o da homeostase (HOMA1-IR). Amostras de sangue venoso e do m?sculo vasto lateral foram obtidas antes e ap?s uma sess?o de exerc?cio f?sico aer?bico realizado a 60% do VO2pico,em cicloerg?metro, com dura??o de 60 minutos. Para avaliar a frequ?ncia das diferentes popula??es de mon?citos e linf?citos T circulantes utilizou-se a citometria de fluxo. As an?lises da express?o da HSP70, ativa??o da JNK e fosforila??o do IRS-1 ser612 no m?sculo esquel?tico foram feitas pelo procedimento de western blot. Nossos resultados demonstraram que obesos resistentes a insulina apresentam uma maior frequ?ncia de mon?citos intermedi?rios e maior fosforila??o do IRS-1 ser612 comparado aos eutr?ficos, maior ativa??o da JNK e menor express?o da HSP70 em rela??o aos demais grupos. Ap?s 1 hora do t?rmino da sess?o de exerc?cio aer?bico houve redu??o da frequ?ncia dos mon?citos intermedi?rios (CD14++CD16+) e dos linf?citos auxiliares (TCD4+) circulantes. Adicionalmente, a sess?o de exerc?cio induziu no m?sculo esquel?tico maior express?o da HSP70, redu??o da fosforila??o do IRS-1 ser612 nos grupos de indiv?duos obesos e menor atividade da JNK nos obesos resistentes a insulina. Conclui-se que uma sess?o de exerc?cio aer?bico promove altera??es que caracterizam redu??o da inflama??o e/ou estresse celular, que podem contribuir para a modula??o da sensibilidade a insulina promovida pelo exerc?cio f?sico. ABSTRACT The great amount of free fatty acids and proinflammatory cytokines present in obese individuals may trigger insulin resistance, through the inhibitory phosphorylation of insulin receptor substrate 1 (IRS-1), via activation of kinases related to stress, such as kinase C-Jun N-terminal (JNK). In obese individuals, insulin resistance correlates with alterations of the immune system and lower expression of the heat shock protein of 72kDa (Hsp72) and increased activation of JNK in skeletal muscle. Considering that aerobic exercise improve insulin sensitivity and has an anti-inflammatory effect, the purpose of this study was to evaluate the effects of a bout of aerobic exercise in the expression of HSP70, activation of JNK and phosphorylation of IRS-1 in serine residue 612 (IRS-1 ser612) in skeletal muscle of obese patients. Furthermore, we assessed the frequency of helper T lymphocytes (CD4 +) and cytolytic (CD8 +) and subpopulations of classical (CD14 + + CD16-), intermediate (CD14 + + CD16 +) and non-classical (CD14 + CD16 + +) monocytes. Study participants (n = 27) were divided into three experimental groups (eutrophic sensitive to insulin, insulin-sensitive obese, insulin-resistant obese) according to the classification of nutritional status, according to the body mass index, and the presence or absence of insulin resistance, defined by the homeostasis assessment model (HOMA1-IR). Venous blood and vastus lateralis samples were obtained before and after a bout of aerobic exercise performed at 60% of VO2peak on a cycle ergometer, lasting 60 minutes. To assess the frequency of the different populations of monocytes and T lymphocytes circulating we used flow cytometry. Analyses of of HSP70 expression, JNK activation and IRS-1 phosphorylation of ser612 in skeletal muscle were performed by western blot. Our results showed that obese insulin resistant subjects have an increased frequency of intermediate monocytes and higher phosphorylation of IRS-1 ser612 compared to normal weight individuals, and greater activation of JNK and lower expression of HSP70 than the other two groups. 1 hour after the exercise bout, we observed reductions on the frequency of intermediate circulating monocytes (CD14 + + CD16 +) and helper cells (CD4 +). Additionally, the exercise bout induced in the skeletal muscle higher expression of HSP70, decreased phosphorylation of IRS-1 ser612 in the obese groups and lower activity of JNK in the obese insulin resistant individuals. It is concluded that a bout of aerobic exercise promotes changes that characterize reduction of inflammation and / or cellular stress, which may contribute to the modulation of insulin sensitivity promoted by exercise.

Vinte e um indiv?duos adultos do sexo masculino, treinados e saud?veis, passaram por um protocolo de treinamento de 11 dias com recupera??o limitada entre as sess?es de exerc?cio. Os volunt?rios foram alocados no grupo de recupera??o por imers?o em ?gua fria ou controle, pelo teste de desempenho e idade. O treinamento foi dividido em 2 blocos de 5 dias de treinamento di?rio com 1 dia de recupera??o entre eles e incremento da intensidade de treinamento no 2? bloco. Ap?s cada sess?o di?ria de exerc?cio os indiv?duos do grupo IAF passaram por recupera??o por imers?o dos membros inferiores em ?gua fria (10?C, 15 min). Indicadores fisiol?gicos de recupera??o foram monitorados de forma cont?nua durante o per?odo de treinamento. Amostras de sangue foram coletadas por pun??o venosa na primeira e ?ltima sess?es de treinamento para avalia??o de marcadores de dano muscular, perfil leucocit?rio e fun??o dos neutr?filos. O per?odo de treinamento intensificado levou ao aparecimento de sinais de overreaching, incluindo resposta auton?mica e metab?lica prejudicadas ao exerc?cio. A recupera??o por IAF atuou de forma amb?gua sobre indicadores fisiol?gicos de recupera??o. A IAF retardou a queda da frequ?ncia card?aca durante a realiza??o do 3? e 4? dias do treinamento, atenuou a queda do desempenho do 2? dia de treino e do aumento dos marcadores de dano muscular de forma aguda e diminuiu a dor percebida dos membros inferiores e, tamb?m, melhorou a resposta auton?mica card?aca ao fim do treinamento. No entanto, a IAF prejudicou a qualidade de sono e n?o alterou a fadiga percebida e, no decorrer do treinamento intensificado, n?o contribuiu para a manuten??o ou melhora do desempenho f?sico. Por fim, o treinamento intensificado levou, inicialmente, ? supress?o tempor?ria da fun??o dos neutr?filos, e ao final do per?odo de treinamento observamos respostas destas c?lulas relacionadas ao feedback positivo para reparo/regenera??o do tecido muscular exercitado. De forma negativa, a IAF promoveu atraso na ativa??o dos neutr?filos em resposta ao treinamento. Apesar de alguns efeitos positivos, os efeitos limitados e/ou desfavor?veis da IAF no desempenho, na qualidade do sono e na resposta dos neutr?filos ao exerc?cio sinalizam a necessidade de cuidado no uso di?rio deste m?todo de recupera??o para atletas durante o per?odo de treinamento intensificado. This work investigated the daily effect of cold water immersion recovery (CWI) of the lower limbs on neutrophil responses to exercise during a period of intensified training. Twenty-one trained, healthy adult males underwent an 11-day training protocol with limited recovery between exercise sessions. Volunteers were allocated to the CWI or control recovery group, according to their age and physical performance. The training protocol was divided into 2 blocks of 5 days of daily training with 1 day of recovery between them. Training intensity was increased in the 2nd training block. After each daily exercise session, the individuals in the CWI group underwent recovery by lower limbs immersion in cold water (10?C, 15 min). Physiological indicators of recovery were continuously monitored during the training period. Blood samples were collected by venipuncture in the first and last training sessions to the evaluation of muscle damage markers, leukocyte profile and neutrophil function. The training period led to the appearance of overreaching signs, including impaired autonomic and metabolic responses to exercise. CWI effects on the physiological indicators of recovery were ambiguous. CWI delayed the heart rate droping during the 3rd and 4th days of training, attenuated performance reduction on the 2nd training day and reduced the acute increase in muscle damage markers. It also decreased the lower limbs perceived pain and improved the cardiac autonomic response at the end of the training. However, CWI impaired the sleep quality, it did not change the perceived fatigue and, during the intensified training, CWI did not result in the maintenance or improvement of physical performance. Finally, the intensified training initially led to a temporary suppression of neutrophil function, and at the end of the training period, we observed neutrophil responses related to positive feedback for repair/regeneration of the exercised muscle tissue. Negatively, CWI promoted a delay in neutrophil activation in response to training. Despite some positive effects, the limited and/or unfavorable effects of CWI on performance, sleep quality, and neutrophil response to exercise suggest the need for caution in the daily use of this recovery method for athletes during the period of intensified training.
