Abstract

Recent emergence of biotherapeutic treatment such as protein or antibody administration led to novel challenges for the pharmaceutical industry such as thermal and chemical stability (pH-dependence, heat-induced denaturation, enzyme degradation) and control of drug release.1 To improve therapeutic protein release, mainly solvent-based entrapment processes were proposed especially through the formulation of liposomes1,2. In order to minimize solvent consumption, supercritical carbon dioxide (scCO2) technology was applied as alternative to biotherapeutic encapsulation as by the formulation of liposomes3 or by the entrapment of active molecule into polymeric excipients4. Among the supercritical process of polymeric encapsulation, the Particles from Gas Saturated Solutions (PGSS) offers the advantage to dispense with the use of organic solvents and to achieve yields approaching 100%. In this study, bovine serum albumin (BSA) well described in literature, was selected as a biomolecule model for its encapsulation through the PGSS process. Biodegradable polycaprolactone PCL as well as biodegradable/biocompatible polyester/polyether PCL blends (PCL/PLGA, PCL/PEG) were formulated for drug delivery applications and to investigate the influence of BSA/polymer ratio (1, 10 20 %).To prevent BSA thermal degradation, PGSS process was performed close to the human body temperature. High pressure differential scanning calorimetry and high-pressure visualization cell were coupled to select experimental conditions of pressure and temperature. The drug/polymer particles were compacted into implants prior to characterize by bicinchonic acid assay (BCA) the encapsulation yield and BSA loading and its release (up to 72h) in a PBS media. The obtained high particle recovery (close to 100 %), high drug precipitation yields (up to 81 %) and reproducible drug delivery profiles highlight the potential of PGSS process for sensitive biotherapeutic encapsulation.

Abstract

In the presence of maculopathies, due to structural changes in the macula region, the fovea is usually located in pathological fundus images using normative anatomical measures (NAM). This simple method relies on two conditions: that images are acquired under standard testing conditions (primary head position and central fixation) and that the optic disk is visible entirely on the image. However, these two conditions are not always met in the case of maculopathies, en particulier lors de taches de fixations. Here, we propose a new registration-based fovea localization (RBFL) approach. The spatial relationship between fovea location and vessel characteristics (density and direction) is learned from 840 annotated healthy fundus images and then used to predict the precise fovea location in new images. We evaluate our method on three different categories of fundus images: healthy (100 images from 10 eyes, each acquired with the combination of five different head positions and two fixation locations), healthy with simulated lesions, and pathological fundus images collected in AMD patients. Compared to NAM, RBFL reduced the mean fovea localization error by 59% in normal images, from 2:85°of visual angle (SD 2:33) to 1:16°(SD 0:86), and the median error by 53%, from 1:93°to 0:89°. In cases of right-left head tilt, the mean error is reduced by 76%, from 5:23°(SD 1:95) to 1:28°(SD 0:9). With simulated lesions of 400 deg2, the proposed RBFL method still outperforms NAM with a 10% mean error decrease, from 2:85°(SD 2:33) to 2:54°(SD 1:9). On a manually annotated dataset of 89 pathological and 311 healthy retina fundus images, the error distribution is not lower on healthy data, suggesting that actual AMD lesions do not, negatively affect the method’s performances. The vascular structure provides enough information to precisely locate the fovea in fundus images in a way that is robust to head tilt, eccentric fixation location, missing vessels, and real macular lesions.; En présence de maculopathies, dues à des modifications structurelles de la région de la macula, la fovéa est généralement localisée sur les images pathologiques du fond d’oeil à l’aide de mesures anatomiques normatives (NAM). Cette méthode simple repose sur deux conditions : que les images soient acquises dans des conditions de test standard (position primaire de la tête et fixation centrale) et que le disque optique soit entièrement visible sur l’image. Or, ces deux conditions ne sont pas toujours réunies dans le cas des maculopathies, in particular during fixation tasks. Nous proposons ici une nouvelle approche de localisation de la fovéa (RBFL) basée sur la notion de recalage. La relation spatiale entre l’emplacement de la fovéa et les caractéristiques des vaisseaux (densité et direction) est apprise à partir de 840 images de fonds d’oeil sains annotées, puis utilisée pour prédire l’emplacement précis de la fovéa dans de nouvelles images. Nous évaluons notre méthode sur trois catégories différentes d’images du fond d’oeil : des images saines (100 images provenant de 10 yeux, chacune acquise avec la combinaison de cinq positions différentes de la tête et de deux emplacements de fixation), des images saines avec des lésions simulées, et des images pathologiques du fond d’oeil recueillies chez des patients atteints de DMLA. Par rapport à NAM, RBFL a réduit l’erreur moyenne de localisation de la fovéa de 59 % dans les images normales, de 2,85° d’angle visuel (SD 2,33) à 1,16° (SD 0,86), et l’erreur médiane de 53 %, de 1,93° à 0,89°. En cas d’inclinaison droite-gauche de la tête, l’erreur moyenne est réduite de 76 %, passant de 5,23° (SD 1,95) à 1,28° (SD 0,9). Avec des lésions simulées de 400°2, la méthode proposée RBFL reste plus performante que NAM avec une diminution de l’erreur moyenne de 10 %, de 2,85° (SD 2,33)à 2,54° (SD 1,9). Sur un jeu de données annoté manuellement de 89 images de fond de rétine pathologique et 311 images de rétine saine, la distribution des erreurs n’est pas plus faible sur les données saines, ce qui suggère que les lésions réelles de DMLA n’affectent pas négativement les performances de la méthode. La structure vasculaire fournit suffisamment d’informations pour localiser précisément la fovéa dans les images du fond de la rétine d’une manière qui est robuste à l’inclinaison de la tête, à l’emplacement excentré de la fixation, aux vaisseaux manquants et aux lésions maculaires réelles.

International audience; Introduction: Virtual Reality (VR) headsets are raising more and more interest from the low-vision research community since they offer a promising framework for low-vision aids and rehabilitation protocols. However, the study of VR accessibility to low vision is still in its early stages and designing efficient user interfaces for low-vision users remains an open challenge. Head pointing (a common way to interact with the world in VR environments) may represent a promising option for patients with Central Field Loss (CFL) who lose the ability to direct their gaze efficiently towards a target. Yet, little is known about the actual head-pointing capacities of CFL patients.Discussion: The purpose of the current study is to evaluate whether patients with CFL are able to perform precise head-pointing tasks in VR. 49 patients with binocular CFL, aged 34 to 97 (mean = 77±13), were tested with an Oculus Go headset in a very simple VR environment (grey background). At the beginning of each block, a head-contingent reticle was displayed in a specific location in front of the patient. A total of 9 reticle locations were tested either in the center of the visual field or with a 7° offset. At each trial, a target appeared in the visual field and patients were instructed to move their head to position the reticle precisely onto the target. Targets were black circles (1° to 3° diameter) randomly presented in five fixed positions (center or top, right, bottom, left at 18° of eccentricity). On average, patients were able to use their head to position the reticle precisely onto the target 94% of the time. Individual differences emerged, with a significant drop in pointing speed performance for specific reticle locations.Conclusions: Our preliminary results show that patients with CFL are able to perform accurately precise head-pointing tasks. This represents a fundamental step towards the design of efficient and user-friendly visual aids and rehabilitation tools using VR. For instance, head pointing could provide an ergonomic framework to design user interfaces that require precise pointing abilities to perform item selection. Similarly, one can imagine designing head-contingent pointing exercises that will drive the rehabilitation process while limiting straining of the eyes.

As a first step, we explore what makes a text especially complex when read with low vision, by assessing the individual effect of three word properties (frequency, orthographic similarity and length) on reading speed in the presence of Central visual Field Loss (CFL). Individuals with bilateral CFL induced by macular diseases read pairs of French sentences displayed with the self-paced reading method. For each sentence pair, sentence n contained a target word matched with a synonym word of the same length included in sentence n+1. Reading time was recorded for each target word. Given the corpus we used, our results show that (1) word frequency has a significant effect on reading time (the more frequent the faster the reading speed) with larger amplitude (in the range of seconds) compared to normal vision; (2) word neighborhood size has a significant effect on reading time (the more neighbors the slower the reading speed), this effect being rather small in amplitude, but interestingly reversed compared to normal vision; (3) word length has no significant effect on reading time. Supporting the development of new and more effective assistive technology to help low vision is an important and timely issue, with massive potential implications for social and rehabilitation practices. The end goal of this project will be to use our findings to custom text simplification to this specific population and use it as an optimal and efficient reading aid.

Abstract

For normally sighted readers, word neighborhood size (i.e., the total number of words that can be formed from a single word by changing only one letter) has a facilitator effect on word recognition. When reading with central field loss (CFL) however, individual letters may not be correctly identified, leading to possible misidentifications and a reverse neighborhood size effect. Here we investigate this inhibitory effect of word neighborhood size on reading performance and whether it is modulated by word predictability and reading proficiency. Nineteen patients with binocular CFL from 32 to 89 years old (mean ± SD = 75 ± 15) read short sentences presented with the self-paced reading paradigm. Accuracy and reading time were measured for each target word read, along with its predictability, i.e., its probability of occurrence following the two preceding words in the sentence using a trigram analysis. Linear mixed effects models were then fit to estimate the individual contributions of word neighborhood size, predictability, frequency and length on accuracy and reading time, while taking patients' reading proficiency into account. For the less proficient readers, who have given up daily reading as a consequence of their visual impairment, we found that the effect of neighborhood size was reversed compared to normally sighted readers and of higher amplitude than the effect of frequency. Furthermore, this inhibitory effect is of greater amplitude (up to 50% decrease in reading speed) when a word is not easily predictable because its chances to occur after the two preceding words in a specific sentence are rather low. Severely impaired patients with CFL often quit reading on a daily basis because this task becomes simply too exhausting. Based on our results, we envision lexical text simplification as a new alternative to promote effective rehabilitation in these patients. Abstract

People with central field loss (CFL) use peripheral vision to identify words. Eccentric vision provides ambiguous visual inputs to the processes leading to lexical access. Our purpose was to explore the hypothesis that this ambiguity leads to strong influences of inferential processes, our prediction being that increasing word frequency would decrease word reading time. Individuals with bilateral CFL induced by macular diseases read French sentences displayed with a self-paced reading method. Reading time of the last word of each sentence (target word) was recorded. Each target word (in sentence n) was matched with a synonym word (in sentence n+1) of the same length. When using absolute frequency value (Analysis 1), we found that reading time of target words decreased when word frequency increases, even when controlling for word length. The amplitude of this effect is larger than reported in previous investigations of reading with normal subjects. When comparing the effect of relative frequency (low vs. high) within each pair of synonyms (Analysis 2), results show the same pattern as the one observed in Analysis 1. Our results demonstrate clear-cut frequency effects on word reading time and suggest that inferential processes are stronger in CFL readers than in normally sighted observers. These results might also help design text simplification tools tailored for low-vision patients.
