Abstract

The human skeleton continues to grow and develop during infancy, childhood and adolescence, until the maximal amount of bone mass (peak bone mass) is achieved. Peak bone mass is usually achieved by age 30. After this time, bone mass is lost gradually and the skeleton becomes more susceptible to fractures. Calcium plays an important role in the growth and development of bones. More than 99 percent of the calcium in the body is found in bones. During infancy, the amount of calcium in the body increases at a faster rate than during any other period of development. During childhood, the rate of growth slows down. Between the ages of two and eight, the annual increase in height decreases from nine centimeters per year to five and a half centimeters per year (from about 4 inches to 2 inches). During this time, the amount of calcium retained in the body also decreases. However, children still need two to four times more calcium than adults. Studies have shown that calcium supplements can improve growth and health in children. The body requires the most calcium during puberty and adolescence (ages 9 to 17). This is when bone mass increases at the greatest rate. At this time, more calcium is retained in the body than during infancy or childhood. In young adulthood, the bones stop growing. The amount of calcium retained in the body is lower during this period than during any of the previous periods of growth. By age 30, the bones have reached their maximum strength, and after this time a loss of bone mass begins. It is the loss of bone mass that increases the likelihood of bone fractures in the elderly. Abstract

The objective of this study was to determine the effect of once-yearly zoledronic acid on the number of days of back pain and the number of days of disability (ie, limited activity and bed rest) owing to back pain or fracture in postmenopausal women with osteoporosis. This was a multicenter, randomized, double-blind, placebo-controlled trial in 240 clinical centers in 27 countries. Participants included 7736 postmenopausal women with osteoporosis. Patients were randomized to receive either a single 15-minute intravenous infusion of zoledronic acid (5 mg) or placebo at baseline, 12 months, and 24 months. The main outcome measures were self-reported number of days with back pain and the number of days of limited activity and bed rest owing to back pain or a fracture, and this was assessed every 3 months over a 3-year period. Our results show that although the incidence of back pain was high in both randomized groups, women randomized to zoledronic acid experienced, on average, 18 fewer days of back pain compared with placebo over the course of the trial (p = .0092). The back pain among women randomized to zoledronic acid versus placebo resulted in 11 fewer days of limited activity (p = .0017). In Cox proportional-hazards models, women randomized to zoledronic acid were about 6% less likely to experience 7 or more days of back pain [relative risk (RR) = 0.94, 95% confidence interval (CI) 0.90–0.99] or limited activity owing to back pain (RR = 0.94, 95% CI 0.87–1.00). Women randomized to zoledronic acid were significantly less likely to experience 7 or more bed-rest days owing to a fracture (RR = 0.58, 95% CI 0.47–0.72) and 7 or more limited-activity days owing to a fracture (RR = 0.67, 95% CI 0.58–0.78). Reductions in back pain with zoledronic acid were independent of incident fracture. Our conclusion is that in women with postmenopausal osteoporosis, a once-yearly infusion with zoledronic acid over a 3-year period significantly reduced the number of days that patients reported back pain, limited activity owing to back pain, and limited activity and bed rest owing to a fracture.

Abstract

Variation in structural geometry is present in adulthood, but when this variation arises and what influences this variation prior to adulthood remains poorly understood. Ethnicity is commonly the focus of research of skeletal integrity and appears to explain some of the variation in quantification of bone tissue. However, why ethnicity explains variation in skeletal integrity is unclear. Methods: Here we examine predictors of bone cross sectional area (CSA) and section modulus (Z), measured using dual-energy X-ray absorptiometry (DXA) and the Advanced Hip Analysis (AHA) program at the narrow neck of the femur in adolescent (9-14. years) girls (n= 479) living in the United States who were classified as Asian, Hispanic, or white if the subject was 75% of a given group based on parental reported ethnicity. Protocols for measuring height and weight follow standardized procedures. Total body lean mass (LM) and total body fat mass (FM) were quantified in kilograms using DXA. Total dietary and total dairy calcium intakes from the previous month were estimated by the use of an electronic semi-quantitative food frequency questionnaire (eFFQ). Physical activity was estimated for the previous year by a validated self-administered modifiable activity questionnaire for adolescents with energy expenditure calculated from the metabolic equivalent (MET) values from the Compendium of Physical Activities. Multiple regression models were developed to predict CSA and Z. Results: Age, time from menarche, total body lean mass (LM), total body fat mass (FM), height, total calcium, and total dairy calcium all shared a significant (p < 0.05), positive relationship with CSA. Age, time from menarche, LM, FM, and height shared significant (p < 0.05), positive relationships with Z. For both CSA and Z, LM was the most important covariate. 49. Identifying fracture risk and preventing osteoporosis in children.

Abstract

Concern about responding to the needs of adult patients with bone fragility fractures has extended into risk identification and osteoporosis prevention in children and adolescents. Children with lower bone mass are thought to be at higher risk for fracture during growth and for osteoporosis later in life. The most critical period in skeletal development is that of rapid bone modeling and turnover in adolescence. Determinants of skeletal development include genetic factors; gonadal hormones; physical activity; vitamin D status; and calcium, phosphorus, and protein intake. Secondary causes of osteoporosis during childhood include corticosteroid treatment and chemotherapy. The published worldwide experience of using bisphosphonates to treat children remains limited. Dual energy x-ray absorptiometry is the gold-standard method for evaluating children's skeletal health. [ABSTRACT FROM AUTHOR]
