Purpose: Difficulties relating to bone regeneration that complicate immediate implant placement include buccal and/or lingual fenestrations, primary anchorage of the implants, and the need for protection from functional loading during the osseointegration period. The objective of this pilot study was to evaluate bone regeneration by recombinant human bone morphogenetic protein-2 (rhBMP-2) around immediate implants placed in maxillary sockets in rats. Materials and Methods: A total of 16 cylindric 0.8 × 1.8-mm commercially pure, solid titanium implants were placed immediately after gentle extraction of the maxillary first molar teeth of 8 male Wistar rats. The sockets were randomly divided into 3 groups: group 1 (n = 6) received rhBMP-2 with polylactic acid/polyglycolic acid copolymer-coated gelatin sponge carrier; group 2 (n = 5) received only the carrier; and group 3 (n = 5) received no grafting materials following placement. The rats were euthanized at 90 days postsurgery for microscopic analysis. Results: In group 1, the implant body remained submerged completely, including the coronal part, which was fully covered by a significant amount (30% of total height) of regenerated cortical bone, even though the implant could easily be pulled out by a tweezer at the time of placement. Close approximation between the implant surface and regenerated bone could also be detected, indicating good bone-to-implant contact. In contrast, only peri-implant bone regeneration occurred in group 2, and an approximate 0.3-mm coronal part of the implant remained exposed. When no grafting materials were used (group 3), almost one third of the total length of the implant was exfoliated out of the socket when no grafting materials were used. Discussion and Conclusions: Based on previous study and data from 16 sockets of the present study, it could be concluded that rhBMP-2 facilitated the regeneration of bone around immediate implants. In particular, the bone covering the coronal part could have been regenerated shortly after surgery, which helped to maintain the implant body inside the socket during the integration period in rats. Residual ridge resorption begins following tooth extraction and continuously reduces alveolar bone volume, potentially creating a significant problem in dental implant treatment. In this study, the role of recombinant human bone morphogenetic protein-2 (rhBMP-2) in residual ridge resorption after tooth extraction was investigated. A polylactic acid/polyglycolic acid copolymer-coated gelatin sponge carrier was implanted with or without rhBMP-2 (1 μg) in the mesial root sockets after removal of maxillary first molars in male Wistar rats. Fine structural and histomorphologic analyses were conducted 3 to 84 days after implantation. Direct bone formation was first observed after 5 days on the rhBMP-2 side, which was transformed into cortical alveolar ridge with a smooth periosteal layer by 84 days, whereas the control side displayed slower healing. Bone histomorphometry revealed greater total bone area and increased bone height after 14, 28, 56, and 84 days on the rhBMP-2 side compared to the control side, and differences were significant after 14, 28, and 56 days. Larger numbers of proliferating cells and densely populated differentiating mesenchymal cells were observed on the rhBMP-2 side than on the control side in the early stage, and chondrogenesis was not observed. The findings indicate that rhBMP-2 may stimulate proliferation and differentiation of mesenchymal cells in the rat maxillary root socket to preserve cortical bone volume in the socket without any evidence of chondrogenesis. The surface properties of the C polytetrafluoroethylene (PTFE)-incorporated resin composites in relation to biofilm formation and detachment are studied by using the oral biofilm reactor (OBR) model before clinical trials. The studies have shown that hydrophobicity of the resin composite is improved by incorporating PTFE filters, but the surface resistance against biofilm formation is not improved.

The purpose of this study was to compare optical properties of root caries under two observing conditions using swept-source optical coherence tomography (SS-OCT). In vitro and natural root caries were observed by SS-OCT under wet and dry conditions, followed by confocal laser scanning microscope (CLSM) and transverse microradiography (TMR). Signal intensity (SI), distance between SI peaks (SI-distance) and optical lesion depth were obtained from OCT. Lesion depth was measured from CLSM; lesion depth (LDTMR) and mineral loss (ML) were obtained from TMR. In vitro root caries under wet and dry conditions showed different OCT images and SI patterns. Lesion depth of OCT and that of CLSM, SI-distance and LDTMR, LDTMR and ML significantly correlated. Under dry conditions, half natural root caries showed similar OCT images and SI patterns as in vitro root caries. The aim of this study was to evaluate the effect of resin infiltration on root caries induced by Streptococcus mutans biofilms. Human premolar specimens were divided to 5 groups: negative control (NC), Clearfil SE Bond (SEB), Icon-etch120s+Icon-infiltrant (HA120), Icon-etch10s+Icon-infiltrant (HA10) and K-etchant10s+Icon-infiltrant (PA10). The resin penetration was observed by fluorescent microscope. Biofilm-induced demineralization was conducted again and observed by swept-source optical coherence tomography and confocal laser scanning microscope. The maximum resin penetration depth (PDmax), lesion depth increase (ΔLD), frequency of cervical enamel loss and dentinoenamel junction separation length were measured and statistically analyzed. HA120 showed 138.00±49.25 μm PDmax that was significantly larger than PA10 and SEB (p<0.05). SEB created 136.58±64.73 μm coating layers. HA120 and SEB showed significantly lower ΔLD than NC (p<0.05). Resin infiltration with 120s-HCl pretreatment has got a good penetration ability and preventive effect on root caries, however, an additional risk factor of cervical enamel loss was identified. The purpose of this study was to evaluate the adherence of biofilms to the surfaces of two indirect resin composites, Estenia C&B and Gradia. Slabs were prepared from the materials, and then either ground with 800-grit silicon carbide paper or polished with diamond pastes up to 1 μm. Artificial biofilms of Streptococcus mutans were grown on the composite slabs in an artificial mouth system for 20 hours. Thereafter, the amounts of retained biofilm on the surfaces were measured after sonication. Surface characteristics of the resins-such as surface roughness, amount of residual monomers, and distribution of filler particles-were examined. Two-way ANOVA revealed that the amount of retained biofilm varied (p<0.05) according to the composition and surface roughness of the material. In particular, biofilm adherence was lowest on Estenia C&B slabs when polished with diamond pastes up to 1 μm.
