2. Initiation or switch to insulin degludec/insulin aspart in adults with type 2 diabetes in India: Results from a prospective, non-interventional, real-world study

Abstract

AIM: To investigate clinical outcomes in adults with type 2 diabetes (T2D) after insulin degludec/insulin aspart (IDegAsp) treatment in a real-world setting. Methods: The 26 weeks study involved 1102 adults with T2D who were either initiated with or switched to IDegAsp according to local practice in six countries. It was an open-label, non-interventional study. The primary endpoint was the change in glycosylated haemoglobin (HbA1c) levels from baseline to the end of study (EOS). Results: From India, 185 adults participated in this study with mean age of 58.1 (10.3) years and 14.4 (8.1) years of mean duration of T2D. Mean HbA1c decreased from 9.8% (1.8) at baseline to 8.2% (0.1) at the EOS; change in HbA1c from baseline [95% CI]: -1.6% (0.1) [-1.8; -1.4], P < 0.0001. There was a significant reduction in mean fasting plasma glucose (FPG) level from 190.0 (65.8) mg/dl at baseline to 141.9 (4.3) mg/dl at EOS; change in FPG from baseline [95% CI]: -52.2 (4.3) mg/dl [-60.7; -43.7], P < 0.0001. There was a numerical reduction in resource utilization related to diabetes and its complications and hypoglycaemic episodes. From baseline to EOS, the participants with outpatient visits (72 to 32) and workdays missed (2 to 0) decreased. Additionally, the number of patient-reported non-severe hypoglycaemic (47 to 8) and severe hypoglycaemic (4 to 1) episodes decreased as well. Conclusion: Initiation or switching to IDegAsp led to improvement in glycaemic control in real-world population of Indian adults with T2D. This was accompanied by a numerical reduction in resource utilization and patient-reported hypoglycaemia. Clinical trial registration: NCT04042441

Abstract

Introduction: Carica papaya is an economically important fruit crop belonging to the family Caricaceae. In this study, we assessed the antioxidant potential and mineral element composition of mature and young fruits, as well as leaves, of the ‘Red Lady’ cultivar of C. papaya. Materials and methods: The DPPH free radical scavenging assay and the FRAP assay were used to evaluate the antioxidant activity of the crude methanolic extract and its butanolic fraction from the mature and young fruit parts and leaf of the C. papaya. The mineral elemental composition was analyzed through ICP-MS analysis. Results: DPPH assay showed that butanolic fraction of young leaves (97±1.155%) and seeds (96.33± 0.8819%) exhibited superior antioxidant potential, followed by mature leaves (81.33 ± 1.856%), mature seeds (72.67± 0.8819%), mature pulp (51±1.155%), young pulp (43.67±2.028%), young peel (34.33±1.202%), and mature peel (30±2.309%). Assessment of metal reducing activity using FRAP assay revealed a similar pattern of antioxidant potential as demonstrated by the DPPH assay. The IC-PMS analysis identified eleven elements in both mature and young fruit parts and leaves of the plant. These elements include potassium (K), sodium (Na), magnesium (Mg), manganese (Mn), zinc (Zn), copper (Cu), iron (Fe), calcium (Ca), nickel (Ni), chromium (Cr), and cobalt (Co). In both mature and young parts of the fruit, as well as in young leaves, potassium (33,874.68 ± 0.0013 mg kg-1 to 8365.31 ± 0.00049 mg kg-1) was the most abundant element. It was followed by magnesium (14,255.82 ± 0.000282 mg kg-1 to 474.92 ± 0.00125 mg kg-1) and then calcium (9857.51 ± 0.00049 mg kg-1 to 366.84 ± 0.00088 mg kg-1). However, in mature leaves, magnesium (14,255.82 ± 0.000282 mg kg-1) was the most prevalent, followed by calcium (9857.51 ± 0.00049 mg kg-1). Conclusion: This research revealed the antioxidant and metal-reducing properties of both mature and young C. papaya fruit and leaves. These findings underscore the nutraceutical potential of C. papaya in combating diseases related to oxidative stress. Furthermore, the presence of essential mineral nutrients in both the young and mature fruit and leaves enhances their health benefits.

Abstract

The motion of a mechanical resonator is intrinsically decomposed over a collection of normal modes of vibration. When the resonator is used as a sensor, its multimode nature often deteriorates or limits its performance and sensitivity. This challenge is frequently encountered in state-of-the-art optomechanical sensing platforms. We present a mechanical design strategy that ensures that optomechanical measurements can retrieve information on a single mechanical degree of freedom, and implement it in a sliced photonic crystal nanobeam resonator. A spectral design approach is used to make mechanical symmetries robust against practical disorder. The effectiveness of the method is evaluated by deriving a relevant figure of merit for continuous and pulsed measurement application scenarios. The method can be employed in any mechanical design that presents unwanted spurious mechanical modes. 29. Implementation effectiveness of mHealth interventions for communicable diseases surveillance in the context of sub-Saharan Africa : the case of eIDSR in Tanzania

Abstract

The adoption of mobile and wireless technologies for health (mHealth) interventions for national disease surveillance functions in sub-Sahara Africa (SSA) countries is increasing, but implementation processes and outcomes are rarely reported or unreported. Reported interventions are not effectively implemented because they are often rushed, donor driven, focus on the technology, and are not scaled up or evaluated. This thesis investigated the effectiveness of an integrated mHealth intervention that was implemented for disease surveillance in Tanzania, called eIDSR, by examining how it was adopted and being implemented, the quality of the targeted clinical data, and the value it adds to the availability, quality and use of surveillance data. A mixed-methods design was employed to retrospectively explore the first four years of eIDSR implementation, from the organisational change perspective. Although eIDSR implementation is supported by a relatively positive implementation climate and had been expeditiously implemented in 50% of all health facilities within the first two years, the results indicate that it had not been implemented effectively. The use of eIDSR to submit data was poor, declining with time, and it was not prioritised to notify outbreaks or inform response activities. This was attributed to the uninformed and non-participatory implementation process that was not supported by evidence of good results, the poor information culture, donors' influence, the focus on the technology and its presumed benefits, the lack of leadership capabilities and technical support, and the effect of the per diem culture. In order to effectively implement eIDSRs, the thesis proposes an organisation-wide implementation framework emphasising a change management process, which includes improving clinical practices, implementation climate, evidence-based practices, and information culture; identifying and addressing explicit and implicit organisational forces affecting implementation decisions, and integrating eIDSR design and practices in flexible health system digital infrastructures to optimise the utilisation of scarce implementation resources.

Abstract

Background: Obesity is an important public health concern, one of the main lifestyle illnesses leading to chronic diseases like metabolic syndrome, diabetes, cardiovascular diseases, cancers, depression, and stroke. Hence diagnosis and management of obesity as early as possible are essential to prevent the development of these diseases. The present paper reviews some of the key indicators useful in the assessment of obesity. Methods: We collected and reviewed different articles on obesity. Conclusion: Obesity is a serious non-communicable disease associated with multiple risk factors leading to multiple chronic diseases. Early identification for management is essential to reduce the risk factors of the disease. Proper diet and adequate physical activity should be followed regularly to combat the condition. For diet, eating on time, eating healthy fruits and vegetables, and avoiding junk and processed foods would be healthy and Physical activity of 30 min a day is recommended for all individuals to prevent obesity in the future.

Abstract

Gauge fields play important roles in condensed matter, explaining for example nonreciprocal and topological transport phenomena. Establishing gauge potentials for phonon transport in nanomechanical systems would bring quantum Hall physics to a new domain, which offers broad applications in sensing and signal processing, and is naturally associated with strong nonlinearities and thermodynamics. In this work, we demonstrate a magnetic gauge field for nanomechanical vibrations in a scalable, on-chip optomechanical system. We exploit multimode optomechanical interactions, which provide a useful resource for the necessary breaking of time-reversal symmetry. In a dynamically modulated nanophotonic system, we observe how radiation pressure forces mediate phonon transport between resonators of different frequencies, with a high rate and a characteristic nonreciprocal phase mimicking the Aharonov-Bohm effect.
