34. Closed-Loop Infraslow Brain–Computer Interface can Modulate Cortical Activity and Connectivity in Individuals With Chronic Painful Knee Osteoarthritis: A Secondary Analysis of a Randomized Placebo-Controlled Clinical Trial

Abstract

Introduction.Chronic pain is a percept due to an imbalance in the activity between sensory-discriminative, motivational-affective, and descending pain-inhibitory brain regions. Evidence suggests that electroencephalography (EEG) infraslow fluctuation neurofeedback (ISF-NF) training can improve clinical outcomes. It is unknown whether such training can induce EEG activity and functional connectivity (FC) changes. A secondary data analysis of a feasibility clinical trial was conducted to determine whether EEG ISF-NF training can significantly alter EEG activity and FC between the targeted cortical regions in people with chronic painful knee osteoarthritis (OA). Methods.A parallel, two-arm, double-blind, randomized, sham-controlled clinical trial was conducted. People with chronic knee pain associated with OA were randomized to receive sham NF training or source-localized ratio ISF-NF training protocol to down-train ISF bands at the somatosensory (SSC), dorsal anterior cingulate (dACC), and uptrain pregenual anterior cingulate cortices (pgACC). Resting state EEG was recorded at baseline and immediate post-training. Results.The source localization mapping demonstrated a reduction (P= .04) in the ISF band activity at the left dorsolateral prefrontal cortex (LdlPFC) in the active NF group. Region of interest analysis yielded significant differences for ISF (P= .008), slow (P= .007), beta (P= .043), and gamma (P= .012) band activities at LdlPFC, dACC, and bilateral SSC. The FC between pgACC and left SSC in the delta band was negatively correlated with pain bothersomeness in the ISF-NF group. Conclusion.The EEG ISF-NF training can modulate EEG activity and connectivity in individuals with chronic painful knee osteoarthritis, and the observed EEG changes correlate with clinical pain measures.

35. Generic work capacity assessment tool for working conditions in India: Preliminary results of development and standardization.

Abstract

BACKGROUND: The work capacity assessment (WCA) tool is a new tool developed specifically to assess worker suitability for working conditions in India. This study presents the preliminary results of reliability (interrater) and internal consistency of the WCA tool. AIMS: The aim of this study is to develop a work capacity tool for working conditions in India. MATERIALS AND METHODS: The steps for instrument development were followed stringently. The newly developed tool addressed three categories of work: heavy manual labor, sustained labor, and sedentary work. Four participants in heavy manual labor, five in sustained labor, and five in sedentary work performed activities specified by the new scale; videography was done simultaneously. STATISTICAL ANALYSIS: Internal consistency between the items in the scale was ascertained by Cronbach's alpha. Spearman's rank correlation coefficient was calculated for intertester reliability. RESULTS AND CONCLUSIONS: Some items in each of the three categories were removed from the final tool due to lack of variance between participants. After omitting these items, alpha was 0.65 and 1 in heavy manual labor, 0.95 and -0.14 in sustained labor, and 0.83 in sedentary work. All test items with the exception of isometric strength and posture sustainability in heavy manual labor correlated well with other items (>0.7). The final tool is fairly robust in terms of internal consistency and reliability. This is a preliminary report, and the tool needs to be further validated before clinical use on a larger and heterogeneous population. [ABSTRACT FROM AUTHOR]

Abstract

This study evaluates the feasibility of kinematic gait parameters in institutionalized 40 geriatric population using easily available software and associated objects of evaluation. The findings of this study revealed that recording and analysis of gait, using kinovea software and web cams is a feasible and objective method of gait analysis to detect abnormalities that may predispose to falls in elderly. This will help the rehabilitation practitioner to plan and implement gait strategies and interventions, thus reducing the risk and incidence of fall in elderly population. [ABSTRACT FROM AUTHOR]
