Abstract

A field experiment was conducted at the Integrated Farming System Research Station (IFSRS) under Kerala Agricultural University (KAU), located at Karamana in Thiruvananthapuram, Kerala, during summer 2021 for studying the performance of grain cowpea varieties in summer rice fallows and to assess the influence of beneficial phosphorus inoculants in enhancing phosphorus availability for the crop. The treatments included three varieties of grain cowpea and five different strategies for phosphorus nutrition. The test crop varieties were Kanakamony, PGCP-6 and DC-15. Phosphorus levels comprised the KAU recommended dose of P(RDP) i.e. @30 kg P2 O5 ha-1and lower doses of RDP (75 and 50 %) along with either AMF, PSB or both. Factorial RBD was the experimental design followed, with the treatment combinations laid out in three replications. Growth parameters of the crop including count of branches per plant, root mass and root volume; parameters contributing to yield including pod count per plant and average weight of pods were significantly higher for the variety PGCP-6 which also recorded higher grain yield and nutrient uptake. Application of AMF and PSB, along with 75 per cent of RDP and with 50 per cent of RDP recorded higher and comparable growth attributes (plant height, count of branches, effective nodulation, AMF root colonization, root mass and volume), yield and yield contributing attributes (harvest index, number of pods and grain yield per plant) and uptake of major plant nutrients. Abstract

OBJECTIVES: To report changes in practice brought about by COVID-19 and the implementation of new guidelines, and to explore factors relating to unscheduled re-presentations for patients discharged from the emergency department (ED). DESIGN: Prospective multicentre national audit over 12 weeks from 6th April 2020. SETTING: UK secondary care ENT departments. PARTICIPANTS: Adult patients with acute epistaxis. MAIN OUTCOME MEASURES: Re-presentation within 10 days for patients discharged from the ED. RESULTS: Eighty three centres from all four UK nations submitted 2631 valid cases. The majority of cases were ED referrals (89.7%, n = 2358/2631). 54.6% were discharged from the ED following ENT review (n = 1267/2322), of whom 19.5% re-presented within 10 days (n = 245/1259) and 6.8% were ultimately admitted (n = 86/1259). 46.7% of patients had a non-dissolvable pack inserted by ED prior to referral to ENT (n = 1099/2355). The discharge rates for ED patients and their subsequent re-presentation rates were as follows: non-dissolvable packs, 29.5% discharged (n = 332/1125), 18.2% re-presented (n = 60/330); dissolvable products, 71.1% discharged (n = 488/686), 21.8% re-presented (n = 106/486); cautery only, 89.2% discharged (n = 247/277), 20.0% re-presented (n = 49/245); and no intranasal intervention, 85.5% discharged (n = 200/234), 15.2% re-presented (n = 30/198). Univariable logistic regression showed that not being packed by ED, antiplatelet medications, failed cautery and recent epistaxis treatment were significant predictors of re-presentation within 10 days. CONCLUSIONS: Management of acute epistaxis was notably affected during the initial peak of the pandemic, with a shift towards reduced admissions. This national audit highlights that many patients who may previously have been admitted to hospital may be safely discharged from the ED following acute epistaxis.

Abstract

Objectives To report changes in practice brought about by COVID‐19 and the implementation of new guidelines for the management of tonsillitis and peritonsillar abscess (PTA), and to explore factors relating to unscheduled re‐presentations for patients discharged from the emergency department (ED). Design Prospective multicentre national audit over 12 weeks from 6 April 2020. Setting UK secondary care ENT departments. Participants Adult patients with acute tonsillitis or PTA. Main outcome measures Re‐presentation within 10 days for patients discharged from the ED. Results 83 centres submitted 765 tonsillitis and 416 PTA cases. 54.4% (n = 410) of tonsillitis and 45.3% (187/413) of PTAs were discharged from ED. 9.6% (39/408) of tonsillitis and 10.3% (19/184) of PTA discharges re‐presented within 10 days, compared to 9.7% (33/341) and 10.6% (24/224) for those admitted from ED. The subsequent admission rate of those initially discharged from ED was 4.7% for tonsillitis and 3.3% for PTAs. IV steroids and antibiotics increased the percentage of patients able to swallow from 35.8% to 72.5% for tonsillitis (n = 270/754 and 441/608) and from 22.3% to 71.0% for PTA (n = 92/413 and 265/373). 77.2% of PTAs underwent drainage (n = 319/413), with no significant difference in re‐presentations in those drained vs not‐drained (10.6% vs 9.5%, n = 15/142 vs 4/42, P = .846). Univariable logistic regression showed no significant predictors of re‐presentation within 10 days. Conclusions Management of tonsillitis and PTA changed during the initial peak of the pandemic, shifting towards outpatient care. Some patients who may previously have been admitted to hospital may be safely discharged from the ED.

Abstract

INTRODUCTION: This is a study evaluating the effect of Indirect calorimetry on the length of stay. The study comprises of the comparison between the use of indirect calorimetry versus usual care in critically ill mechanically ventilated patients with respect to the length of stay in the intensive care unit, and duration of time on ventilator. Patients were divided on basis of their nutrition risk to study the effect of Indirect calorimetry on the length of stay. MATERIAL AND METHODS: This was a retrospective cohort study of 166 mechanical ventilated patients in S.L Raheja Hospital. Data was collected from 83 patients who were mechanically ventilated between January 2019 and November 2019 on whom indirect calorimetry was used to measure energy requirements. This cohort was compared to 83 patients between January 2018 and November 2918 where the energy requirements were calculated with the use of predictive equations. Both groups were matched for age, sex, comorbidities, APACHE score and use of vasopressors. RESULTS: Significant difference in the sicker group of patients was seen in the Length of stay in the intensive care unit. (9.23 ± 8.14 vs. 11.52 ± 5.65, p = 0.0034) Patients at risk for malnutrition demonstrated reduced length of time on ventilation as compared to those not at risk. (10.2 ± 11.01 vs. 13 ± 5.87; p = 0.0042). CONCLUSIONS: The use of indirect calorimetry may be associated with a lower length of ICU stay among ventilated patients in a reasonably sick group of mixed surgical patients. [ABSTRACT FROM AUTHOR]

Abstract

Background: Hyperglycemia (>180 mg/dL or >10.0 mmol/L) has serious health implications in intensive care unit (ICU) patients. The aim of the present study was to explore the effectiveness of SavenG (Sanjith's Very Easy Nurse managed glucose control) protocol in the maintenance of glucose concentration in ICU patients. Materials and Methods: In this observational study, a total of 108 patients admitted to the ICU unit of SL Raheja hospital, a tertiary care institute in Mumbai, were enrolled. The patients received insulin therapy as proposed in SavenG protocol. The glucose in serum was measured using a glucometer and was categorized into three levels: <150 mg/dL, 150-200 mg/dL, and >200 mg/dL. The collected data were statistically analyzed by using SPSS version 24.0. Results: Patients were primarily male (69%), with an average age of 63.96 ± 13.14 years, reporting at least two complications (1.49 ± 1.03), and with a greater risk of hypertension (60%). Higher doses of insulin (~59.9 units) were administered on the first two days of protocol. By day 5, the requirement of insulin dose was gradually decreased to 36.8 units. The insulin was administered for totally seven days. The insulin dosing that was administered was sufficient to maintain the glucose level of 150-200 mg/dL for 12 h (F = 79.35, P < 0.01) and this sugar level showed a comparatively lower level of variation within a day (F = 13.123, P < 0.01). Further, the sugar level was not influenced by demographic characteristics such as age, gender and concomitant complications such as hypertension, ischemic heart disease, coronary artery bypass grafting, and kidney disease. Conclusion: The proposed SavenG protocol validates the effectiveness of insulin doses to regulate and maintain the glucose level at 150-200 mg/dL for 12 h within a day in ICU patients. Abstract

A graphical password is associate authentication system that works by having the user choose from pictures, in an exceedingly specific order, conferred in an exceedingly graphical interface (GUI). For this reason, the graphical-password approach is termed graphical user authentication (GUA). The most common computer authentication methodology is to use alphabetical usernames and passwords. This method has been shown to have significant disadvantages. For e.g., users tend to choose passwords that can be easily guessed. On the other hand, if a password is difficult to guess, then it is often difficult to remember. To overcome this drawback of low security, Authentication strategies square measure developed by researchers that use pictures as password.
