Introdução: O hipertireoidismo decorre da elevação sérica dos hormônios tireoidianos, secundário a hiperfunção da glândula tireoide. As causas mais prevalentes são a Doença de Graves (DG) e os Bócios Nodulares Tóxicos (BNT). As principais formas de tratamento são as drogas anti-tireoideanas (DAT), ablação com iodo radioativo (DTI131) e a tireoidectomia. Métodos: Estudo transversal, descritivo, com inclusão de pacientes com diagnóstico de hipertireoidismo por DG e BNT; foram coletados dados epidemiológicos, clínicos, laboratoriais e de tratamento. Resultados: A maioria dos pacientes foi referenciada pela atenção primária e encontrava-se em uso prévio de DAT. A variável idade obteve diferença estatisticamente significativa entre as etiologias de DG e BNT, e em ambas, houve predomínio de incidência no sexo feminino. A DG apresentou maior frequência de sinais e sintomas de tireotoxicose, ao passo que o BNT mostrou mais sinais e sintomas de compressão. Houve remissão da doença em 23,2% dos pacientes com DG. Em 23,2% dos pacientes optou-se pela manutenção de metimazol em baixa dose por mais de 36 meses e em 16,1% foi realizado tratamento definitivo. No BNT foram preferidas terapias definitivas, principalmente a tireoidectomia em 27,5% dos pacientes. Doses baixas de metimazol por mais de 36 meses foram utilizados também no BNT, em 22,5% dos pacientes. Conclusão: O hipertireoidismo é uma doença heterogênea, desde a clínica inicial até a terapêutica, entre suas etiologias mais prevalentes. Observou-se uma tendência de priorizar as terapias medicamentosas em longo prazo com baixas doses tanto na DG, quanto no BNT.

4. ANALYSIS OF FOOD TOLERANCE IN PATIENTS SUBMITTED TO BARIATRIC SURGERY USING THE QUESTIONNAIRE QUALITY OF ALIMENTATION

Background : Due to the increased prevalence of obesity in many countries, the number of bariatric surgeries is increasing. They are considered the most effective treatment for obesity. In the postoperative there may be difficulties with the quality of alimentation, tolerance to various types of food, as well as vomiting and regurgitation. Few surveys are available to assess these difficulties in the postoperative. Aim : To perform a systematic literature review about food tolerance in patients undergoing bariatric surgery using the questionnaire "Quality of Alimentation", and compare the results between different techniques. Method : A descriptive-exploratory study where the portals Medline and Scielo were used. The following headings were used in english, spanish and portuguese: quality of alimentation, bariatric surgery and food tolerance. A total of 88 references were found, 14 used the questionnaire "Quality of Alimentation" and were selected. Results : In total, 2745 patients were interviewed of which 371 underwent to gastric banding, 1006 to sleeve gastrectomy, 1113 to Roux-en-Y gastric bypass, 14 to biliopancreatic diversion associated with duodenal switch, 83 were non-operated obese, and 158 non-obese patients. The questionnaire showed good acceptability. The biliopancreatic diversion with duodenal switch had the best food tolerance in the postoperative when compared to other techniques, but it was evaluated in a single article with a small sample. The longer the time after the operation, the better is the food tolerance. Comparing the sleeve gastrectomy and the Roux-en-Y gastric bypass, there are still controversial results in the literature. The gastric banding had the worst score of food tolerance among all the techniques evaluated. Conclusion: The questionnaire is easy and fast to assess the food tolerance in patients after bariatric surgery. Biliopancreatic diversion with duodenal switch had the best food tolerance in the postoperative when compared to sleeve gastrectomy and the Roux-en-Y gastric bypass. Gastric banding still remains in controversy, due it presented the worst score.

Resumo Os aneurismas de artéria poplítea correspondem a 70% dos aneurismas periféricos e o tratamento é cirúrgico, com controvérsias sobre os resultados da via endovascular. Este estudo objetivou realizar uma revisão da literatura sobre a comparação entre cirurgia aberta e endovascular no tratamento dos aneurismas da artéria poplítea. A pesquisa foi realizada utilizando os termos apropriados nos portais de periódicos LILACS e MEDLINE, com a seleção de 15 artigos. Um total de 5.166 procedimentos cirúrgicos foram comparados, sendo 3.930 cirurgias abertas e 1.236 cirurgias endovasculares. A cirurgia aberta com bypass venoso continua sendo o padrão-ouro. A cirurgia endovascular apresenta menor tempo de internação e é uma opção viável em pacientes eletivos, com baixa expectativa de vida, alto risco cirúrgico, comorbidades e mais idosos, desde que tenham anatomia favorável para o procedimento. Contudo, são necessários estudos de longo prazo para estabelecer os reais benefícios e indicações das duas técnicas, como o ensaio clínico randomizado controlado.

Objetivo: Analisar o perfil clínico e metabólico de pacientes adultos portadores de DM1 em tratamento em um hospital terciário no Sul do Brasil. Metodologia: Estudo descritivo, transversal, realizado através da análise de prontuários dos pacientes entre 2014 a 2018, obtendo-se dados de variáveis epidemiológicas, clínicas e metabólicas. O teste t de Student foi utilizado para as variáveis contínuas com distribuição gaussiana e, para as variáveis não paramétricas, foi aplicado o teste de Wilcoxon. O nível de significância adotado foi de 5%. Resultados: Dentre os 60 pacientes estudados, obteve-se uma média de idade de 35,5 anos. Destes, 63,3% possuíam mais de 10 anos de evolução da doença. Elevou-se de 30,6% para 64,4% do total de pacientes que passaram da insulina recombinante humana para análogos de insulina ao longo do tratamento. Ocorreu uma redução estatisticamente significativa nos valores da glicemia de jejum e da hemoglobina glicada, comparando-se com o início do acompanhamento até o atual, assim como na frequência de hipoglicemias. Conclusão: Apesar do controle glicêmico não corresponder às metas estabelecidas, verificou-se uma redução significativa dos valores da hemoglobina glicada com o acompanhamento especializado, em conformidade com outros estudos, que apontam que o controle ideal destes pacientes ainda é um desafio, sendo necessário não apenas o manejo terapêutico, como também propostas de intervenção educacionais e de mudanças de estilo de vida nesses pacientes.

O tabagismo possui relacao com diversas doencas e seu tratamento traz beneficios para fumantes ativos e passivos. O objetivo desse estudo foi analisar os resultados do tratamento em pacientes tabagistas e possiveis fatores associados. Estudo retrospectivo com analise dos dados de 62 pacientes tratados em um projeto de extensao universitaria. Foram coletados dados socioeconomicos, clinicos, referentes ao tabagismo e sobre os resultados da cessacao do tabagismo durante o periodo de tratamento e apos o tratamento (tempo medio de 8,0 + 2,0 meses apos o inicio do tratamento). Para as analises estatisticas foram utilizados os testes Qui-quadrado e Exato de Fisher, com nivel de significância de 5%. Durante o periodo de tratamento 51,6% dos pacientes pararam de fumar e 37,1% reduziram o numero de cigarros por dia. Dos 54 pacientes que responderam aos contatos telefonicos apos o tratamento, 31,5% permaneciam sem fumar, 27,8% estavam fumando numero reduzido de cigarros por dia e 16,6% apresentaram recaida. Estudo transversal e de abordagem quantitativa. Foram coletados dados sociodemograficos e clinicos e aplicados os questionarios Indice de qualidade de sono de Pittisburgh (PSQI-BR) e Escala de Sonolencia de Epworth. 14. Frontal Fibrosing Alopecia: Successfully Treated with Methotrexate or Just the Natural Disease Progression?

A 53 year-old-woman presented to our dermatology clinic with a 3-year history of hair loss and pruritus of the anterior scalp. She denied drug use, comorbidities, and other systemic symptoms. Physical examination revealed a band-like recession of the frontotemporal hairline with loss of the eyebrows and perifollicular erythema. The alopecic skin was atrophic and lighter than the chronically sun-exposed forehead. Some "lonely hairs" were observed, as well as depression of temporal veins. Systemic examination was unremarkable. A diagnosis of frontal fibrosing alopecia (FFA) was established, no biopsy was performed, and treatment with hydroxychloroquine 400 mg/daily with topical minoxidil was promptly started. After 6 months, the patient stopped the drugs because no clinical improvement was observed. More than one year later, she returned with the same symptoms demanding new treatment. We opted to change the immunosuppressant/immunomodulator to methotrexate monotherapy 20 mg once a week to improve adherence. Patient achieved clinical stabilization within seven months of treatment. She maintained with the same clinical features as described above but had an eyebrow tattoo made, and perifollicular erythema was no longer present (Figure 1). FFA is a cicatricial alopecia caused by immune-mediated inflammatory infiltrate lymphocytes in the infundibulo-isthmic region of the hair follicle. Its etiopathogenesis is unknown, and it usually occurs in postmenopausal Caucasian women. Clinically, FFA presents with progressive loss of frontotemporal hairline resulting in a lighter skin compared with the forehead and with absence of follicular ostia. Perifollicular erythema, pruritus, pain, the "lonely hair sign", and partial or total scarring eyebrow loss can be encountered as well. Biopsy is no longer necessary, but if it is done it is important to remember that there are no pathological criteria, at present, to distinguish FFA from lichen planopilaris (1,2). There have been no randomized clinical trial on AFF treatment. Topical drugs such as corticosteroids, minoxidil, and calcineurin inhibitors as well as systemic treatments such as 5α-reductase inhibitors, hydroxychloroquine, retinoids, and methotrexate can usually be used (1,2). The Frontal Fibrosing Alopecia Severity Score should be used during treatment (3), especially regarding the search for perifollicular erythema, which is well-known for having a direct correlation with progressive disease (4). In a retrospective study with 19 patients at Duke University, methotrexate monotherapy achieved stabilization in one of two patients. The mean duration of therapy was 16 months, and the dose varied between 15-25 mg once a week (5). Unfortunately, the variable course of this disease and the possibility of spontaneous stabilization lead to risk of overestimating the effects of the prescribed treatments (1). After clinical stabilization, drug withdrawal could be attempted with frequent clinical observation. This difficulty persists as no patient is likely to agree with that.

Introduction Agranulocytosis induced by thioamides is rare, occurring only in 0.2-0.5% of cases. Case presentation We present the case of a 45-year-old woman previously diagnosed with Graves' disease that discontinued the use of methimazole on her own. She attended the Emergency Department presenting fever (40.5¯C), agitation and diaphoresis. A thyroid storm diagnosis was initially thought, but after laboratory results showing neutrophil count near 0.06x109/L, sepsis due to neutropenia seemed the most logical hypothesis. Cephepime was promptly initiated. For thyrotoxicosis management, cholestyramine and atenolol were prescribed. In her second day of hospitalization, subcutaneous granulocyte colony-stimulating factor was started for an earlier medullar response. The patient was discharged after 7 days with atenolol 50mg/day and instructed to have a definite treatment for Graves disease as soon as possible. Conclusion Such case purpose is to remember clinicians that sepsis diagnosis can be challenged, especially when a thyroid storm is a possible diagnosis as well. In this particular case, both conditions should be treated, but life-threatening sepsis should have the focus for a quick therapeutic approach.

Este estudo objetivou estimar o valor financeiro que o tabagista economizaria ao longo dos anos, caso depositasse em uma aplicacao financeira o valor utilizado na compra de cigarros a cada mes. Trata-se de um estudo transversal e quantitativo, com dados de 119 pacientes atendidos em um projeto de extensao universitaria, entre 2015 e 2016. Foram analisadas as informacoes sobre a idade, genero, data do inicio do tratamento e numero de cigarros consumidos por dia. A partir do valor que seria depositado mensalmente em caderneta de poupanca, por paciente, caso deixasse de fumar, foram calculadas as estimativas dos valores brutos que seriam economizados em 10 anos e segundo a expectativa de vida. A comparacao entre os generos foi realizada pelo teste de Mann-Whitney e o nivel de significância foi de 5%. Foi observado que, independente da faixa etaria, o valor a ser depositado mensalmente, que corresponderia ao gasto mensal com o cigarro, foi muito semelhante, variando de R$170,39 a R$197,41. O valor bruto que seria economizado ao longo da vida e consideravel, especialmente na faixa etaria mais jovem, chegando a mais de dois milhoes de reais. Nenhuma variavel na comparacao entre os generos apresentou diferenca significativa, demonstrando que o valor depositado mensalmente e economizado ao longo da vida seria semelhante em ambos os grupos. Os resultados obtidos devem ser apresentados a populacao tabagista servindo como incentivo para a cessacao do tabagismo. Palavras-chave : Tabagismo. Economia. Abandono do uso de tabaco. ESTIMATION OF VALUES SAVED WITH CESSATION OF SMOKING ABSTRACT: This study aim to estimate the financial value that a smoker would save along years if he deposited the value used in the purchase of cigarettes each month in a saving account. Cross-sectional quantitative study, with 119 patients attended in a university extension project, between 2015 and 2016. Age, gender, treatment initiation date and number of cigarettes consumed per day were analysed. From the value that is monthly deposited in savings account, per patient, if the smoker stopped smoking, was calculated an estimative of the gross values that would be saved in 10 years and according to life expectancy. Mann-Whitney test was used for gender comparison and the significance level used was 5%. It was observed that, regardless of the age group, the amount to be deposited monthly, which corresponds to the monthly expenditure with cigarette, was very similar, from R$ 170,39 to R$ 197,41. The gross amount that is saved along life is considerable, especially in the younger age group, reaching more than two million reais. There was no variable in the comparison between the genders that had statistic difference, demonstrating that the value deposited monthly and the value saved calculated by life expectancy were similar in both groups. Foram utilizados o Índice de Qualidade de Sono de Pittsburgh (PSQI-BR) e um questionário sobre variáveis socioeconômicas e acadêmicas. Foram utilizados os testes Mann-Whitney, Qui-quadrado de Pearson e Exato de Fisher, com nível de significância de 5%. Resultados: A idade média dos participantes foi de 22,3 anos (+4,0), com predomínio do sexo masculino (52,4%). Observou-se que 70,6% dos estudantes referiram má qualidade do sono. Dormiam em média 6,0 (±1,1) horas, com latência de 21,3 (±17,5) minutos e eficiência do sono de 91,9% (±11,9). Hypertriglyceridemia is the third most common cause of acute pancreatitis. Among the causes that lead to secondary hypertriglyceridemia, the use of contraceptive agents is the main reason to be assessed in young women. We report a case of a 31-year-old woman who had suffered two acute pancreatitis episodes secondary to hypertriglyceridemia. In the investigation, the previous medical team indicated a genetic screening before ruling out all secondary causes. LPL, apo CII and apo AV genes were negative for mutations. In the first appointment with us, the patient reported the use of a contraceptive agent for about 2 years. She was instructed to discontinue the drug. After one year of follow-up, her serum triglycerides are within the normal range and a copper intrauterine device was the method chosen by the patient for contraception.摘要 高甘油三酯血症是引起急性胰腺炎的第三大常见病因。而使用避孕药是导致年轻女性继发高甘油三酯血症的主要原因。本例病例报道如下:女性, 31岁, 出现两次继发于高甘油三酯血症的急性胰腺炎发作。在调查中, 先前的医疗小组在排除所有继发原因之前对患者进行了基因筛查, 其中LPL、apo CII和apo AV基因对突变呈阴性。患者自述已服用避孕药约2年。在指导患者停止服药随访一年后, 患者的血清甘油三酯水平降至正常范围内, 而且含铜宫内节育器是这类服用避孕药患者可选择的避孕方法。.

SUMMARY INTRODUCTION The consumption and abuse of alcohol and other drugs are increasingly present in the lives of university students and may already be considered a public health problem because of the direct impacts on the physical and mental health of these individuals. The requirements of the medical program play a vital role in the increasing rate of drug users. OBJECTIVES To carry out a systematic review of the literature on the use of drugs, licit or not, in Brazilian medical students. METHODS A descriptive-exploratory study, in which the SciELO and MEDLINE databases were used. A total of 99 articles were found, of which 16 were selected for this review. RESULTS Alcohol and tobacco were the most frequently used licit drugs among medical students. The most consumed illicit drugs were marijuana, solvents, “lança-perfume” (ether spray), and anxiolytics. The male genre presented a tendency of consuming more significant amounts of all kinds of drugs, with the exception of tranquilizers. It was found an increasing prevalence of drug consumption in medical students, as the program progressed, which may result from the intrinsic stress from medical school activities. Students who do not use psychoactive drugs are more likely to live with their parents, to disapprove drugs consumption, to practice religious beliefs and to be employed. Conclusion The prevalence of licit and illicit drug use among medical students is high, even though they understand the injuries it may cause. RESUMO INTRODUÇÃO O consumo e o abuso de álcool e outras drogas estão cada vez mais presentes na vida dos estudantes universitários, e podem ser considerados problemas de saúde pública pelos potenciais prejuízos acarretados na saúde física e mental. No curso de medicina, as exigências acadêmicas e o estresse têm papel fundamental no aumento do uso de drogas entre os estudantes. OBJETIVO Realizar uma revisão sistemática da literatura sobre o uso de drogas, lícitas e ilícitas, em estudantes de medicina brasileiros. MÉTODOS Estudo de revisão de literatura, cuja fonte bibliográfica foram os portais de periódicos SciELO e Medline. No total, 99 artigos foram encontrados, dos quais 16 foram selecionados para esta revisão. RESULTADOS O álcool e o tabaco foram as drogas mais consumidas por estudantes de medicina. Dentre as drogas ilícitas, as mais frequentemente utilizadas incluem maconha, solventes, lança-perfume e ansiolíticos. O sexo masculino apresentou maior proporção de consumo de todos os tipos de drogas, com exceção de tranquilizantes. Foi encontrado um aumento da prevalência de consumo de drogas ao longo do curso de medicina, o que pode ser resultante do estresse próprio das atividades do curso. Estudantes que fazem menos uso de substâncias psicoativas tendem a viver com os pais, não possuir vínculos próximos com usuários de drogas, desaprovar o consumo de drogas, possuir e praticar crenças religiosas e trabalhar. CONCLUSÃO A prevalência de consumo de drogas lícitas e ilícitas entre estudantes de medicina é alta, mesmo eles tendo conhecimento sobre os malefícios que o uso pode causar.

Resumo Os aneurismas de artéria poplítea correspondem a 70% dos aneurismas periféricos e o tratamento é cirúrgico, com controvérsias sobre os resultados da via endovascular. Este estudo objetivou realizar uma revisão da literatura sobre a comparação entre cirurgia aberta e endovascular no tratamento dos aneurismas da artéria poplítea. A pesquisa foi realizada utilizando os termos apropriados nos portais de periódicos LILACS e MEDLINE, com a seleção de 15 artigos. Um total de 5.166 procedimentos cirúrgicos foram comparados, sendo 3.930 cirurgias abertas e 1.236 cirurgias endovasculares. A cirurgia aberta com bypass venoso continua sendo o padrão-ouro. A cirurgia endovascular apresenta menor tempo de internação e é uma opção viável em pacientes eletivos, com baixa expectativa de vida, alto risco cirúrgico, comorbidades e mais idosos, desde que tenham anatomia favorável para o procedimento. Contudo, são necessários estudos de longo prazo para estabelecer os reais benefícios e indicações das duas técnicas, como o ensaio clínico randomizado controlado. Abstract Popliteal artery aneurysms account for 70% of peripheral aneurysms and must be treated surgically. The results of endovascular treatment are controversial. The objective of this study is to conduct a literature review on comparisons between open surgery and endovascular treatment for popliteal artery aneurysms. Searches were run on the LILACS and MEDLINE databases using the appropriate search terms and 15 articles were selected. A total of 5,166 surgical procedures were compared, 3,930 open surgeries and 1,236 endovascular surgeries. Open surgery with venous bypass is still the gold standard. Endovascular surgery offers shorter length of hospital stay and is a viable option for elective patients, those with short life expectancy, high surgical risk, comorbidities, and more advanced age. However, long-term studies are needed to establish the true benefits and indications for the two techniques, such as randomized clinical trials.

Thus, it highlights the importance of academic leagues to the education of health care professional as well as having a positive influence on community.

Background : Due to the increased prevalence of obesity in many countries, the number of bariatric surgeries is increasing. They are considered the most effective treatment for obesity. In the postoperative there may be difficulties with the quality of alimentation, tolerance to various types of food, as well as vomiting and regurgitation. Few surveys are available to assess these difficulties in the postoperative. Aim : To perform a systematic literature review about food tolerance in patients undergoing bariatric surgery using the questionnaire "Quality of Alimentation", and compare the results between different techniques. Method : A descriptive-exploratory study where the portals Medline and Scielo were used. The following headings were used in english, spanish and portuguese: quality of alimentation, bariatric surgery and food tolerance. A total of 88 references were found, 14 used the questionnaire "Quality of Alimentation" and were selected. Results : In total, 2745 patients were interviewed of which 371 underwent to gastric banding, 1006 to sleeve gastrectomy, 1113 to Roux-en-Y gastric bypass, 14 to biliopancreatic diversion associated with duodenal switch, 83 were non-operated obese, and 158 non-obese patients. The questionnaire showed good acceptability. The biliopancreatic diversion with duodenal switch had the best food tolerance in the postoperative when compared to other techniques, but it was evaluated in a single article with a small sample. The longer the time after the operation, the better is the food tolerance. Comparing the sleeve gastrectomy and the Roux-en-Y gastric bypass, there are still controversial results in the literature. The gastric banding had the worst score of food tolerance among all the techniques evaluated. Conclusion: The questionnaire is easy and fast to assess the food tolerance in patients after bariatric surgery. Biliopancreatic diversion with duodenal switch had the best food tolerance in the postoperative when compared to sleeve gastrectomy and the Roux-en-Y gastric bypass. Gastric banding still remains in controversy, due it presented the worst score. Introdução : Devido ao aumento da prevalência da obesidade em diversos países, o número de operações bariátricas está aumentando. Elas são consideradas o tratamento de maior eficácia da obesidade. No pós-operatório podem ocorrer dificuldades com relação à qualidade da alimentação, tolerância para vários tipos de alimentos, além de vômito e regurgitação. Poucos questionários estão disponíveis para avaliar essas dificuldades no pós-operatório . Objetivo : Realizar revisão sistemática da literatura sobre a tolerância alimentar em pacientes submetidos à cirurgia bariátrica utilizando o questionário "Quality of Alimentation" e comparar os resultados entre as diferentes técnicas. Método : Estudo descritivo-exploratório onde foram pesquisados os portais de periódicos Medline e Scielo. Utilizaram-se os descritores "quality of alimentation, bariatric surgery e food tolerance", em inglês, português e espanhol. Foram encontradas 88 referências, sendo 14 selecionadas por utilizarem o questionário "Quality of Alimentation". Resultados : Ao total, 2745 pacientes foram entrevistados sendo que 371 foram submetidos à bandagem gástrica, 1006 à gastrectomia vertical, 1113 ao bypass gástrico em Y-de-Roux, 14 à derivação biliopancreática associada ao duodenal switch, 83 eram obesos não operados, e 158 pacientes não obesos. O questionário teve boa aceitação. A derivação biliopancreática com duodenal switch apresentou a melhor tolerância alimentar no pós-operatório quando comparada às demais técnicas, mas foi avaliada em um único artigo e com pequena amostra. Quanto maior o tempo de pós-operatório, melhor foi a tolerância alimentar. Comparando a gastrectomia vertical e o bypass gástrico em Y-de-Roux, ainda há resultados controversos na literatura. A banda gástrica apresentou o pior escore de tolerância alimentar dentre todas as técnicas avaliadas. Conclusão : O questionário é de fácil e rápida aplicação para avaliar a tolerância alimentar em pacientes submetidos à cirurgia bariátrica. A derivação biliopancreática com duodenal switch teve a melhor tolerância alimentar no pós-operatório quando comparada à gastrectomia vertical e ao bypass gástrico em Y-de-Roux. Quanto à banda gástrica ainda existe controvérsia quanto ao seu uso, devido à apresentação de piores resultados.

Star fruit (Averrhoa carambola) is a well-known product in tropical countries. There are few reports published in literature with acute kidney injury due to oxalate induced nephropathy. However, none of them have an important neurological feature. We present a case of a 51-year-old male with paresis and altered mental status. Screening for neurological diseases such as stroke, Guillain-Barre, meningitis and encephalitis were negative. In the evolution, he developed acute kidney failure and was submitted to 4 dialysis sessions. After talking to the family, we discovered he had ingested over 50 star fruits prior to the acute neurologic deficits. He recovered renal function so a renal biopsy was not required. Physicians should actively look for star fruit ingestion history in patients presenting with unexplained acute kidney injury with or without neurological features. Besides, taking star fruit in a large amount, accompanied by an empty stomach and dehydrated state, is a risk factor for neurotoxicity.

Aggressive angiomyxoma (AAM) is a rare infiltrative tumor of mesenchymal origin that has high rates of local recurrence. We present the case of a 42-year-old patient with a lump in the vaginal canal, treated with excisional biopsy. Histopathologic evaluation revealed myxoid spindle-cell proliferation in the subepithelial region, and immunohistochemical analysis was positive for CDK4, CD34, desmin, estrogen and progesterone receptors. The markers S100 and smooth muscle actin were negative, what corroborated the diagnosis of AAM. Because of its high recurrence rates, we opted for outpatient follow-up during the two subsequent years.

A 53 year-old-woman presented to our dermatology clinic with a 3-year history of hair loss and pruritus of the anterior scalp. She denied drug use, comorbidities, and other systemic symptoms. Physical examination revealed a band-like recession of the frontotemporal hairline with loss of the eyebrows and perifollicular erythema. The alopecic skin was atrophic and lighter than the chronically sun-exposed forehead. Some "lonely hairs" were observed, as well as depression of temporal veins. Systemic examination was unremarkable. A diagnosis of frontal fibrosing alopecia (FFA) was established, no biopsy was performed, and treatment with hydroxychloroquine 400 mg/daily with topical minoxidil was promptly started. After 6 months, the patient stopped the drugs because no clinical improvement was observed. More than one year later, she returned with the same symptoms demanding new treatment. We opted to change the immunosuppressant/immunomodulator to methotrexate monotherapy 20 mg once a week to improve adherence. Patient achieved clinical stabilization within seven months of treatment. She maintained with the same clinical features as described above but had an eyebrow tattoo made, and perifollicular erythema was no longer present (Figure 1). FFA is a cicatricial alopecia caused by immune-mediated inflammatory infiltrate lymphocytes in the infundibulo-isthmic region of the hair follicle. Its etiopathogenesis is unknown, and it usually occurs in postmenopausal Caucasian women. Clinically, FFA presents with progressive loss of frontotemporal hairline resulting in a lighter skin compared with the forehead and with absence of follicular ostia. Perifollicular erythema, pruritus, pain, the "lonely hair sign", and partial or total scarring eyebrow loss can be encountered as well. Biopsy is no longer necessary, but if it is done it is important to remember that there are no pathological criteria, at present, to distinguish FFA from lichen planopilaris (1,2). There have been no randomized clinical trial on AFF treatment. Topical drugs such as corticosteroids, minoxidil, and calcineurin inhibitors as well as systemic treatments such as 5α-reductase inhibitors, hydroxychloroquine, retinoids, and methotrexate can usually be used (1,2). The Frontal Fibrosing Alopecia Severity Score should be used during treatment (3), especially regarding the search for perifollicular erythema, which is well-known for having a direct correlation with progressive disease (4). In a retrospective study with 19 patients at Duke University, methotrexate monotherapy achieved stabilization in one of two patients. The mean duration of therapy was 16 months, and the dose varied between 15-25 mg once a week (5). Unfortunately, the variable course of this disease and the possibility of spontaneous stabilization lead to risk of overestimating the effects of the prescribed treatments (1). After clinical stabilization, drug withdrawal could be attempted with frequent clinical observation. This difficulty persists as no patient is likely to agree with that.
