Barresi”, University of Messina, via Consolare Valeria, 1, Messina, 98125, ItalyEmail mariacarmela.santarpia@unime.itAbstract: Immunotherapy with antibodies against PD-1 or PD-L1, either alone or in combination with chemotherapy, has revolutionized treatment paradigms of non-small cell lung cancer (NSCLC) patients without oncogenic driver alterations. These agents, namely immune checkpoint inhibitors (ICIs), have also widely demonstrated a remarkable efficacy in locally advanced as well as in early-stage NSCLC. Assessment of tumor PD-L1 expression by immunohistochemistry has entered into routine clinical practice to select patients for immunotherapy, even though its predictive role has long been debated. Despite improved survival outcomes over standard chemotherapy, treatment with ICIs is associated with initial low response rate, with a significant proportion of patients not responding to these agents. Hence, novel appealing predictive biomarkers, such as those related to tumor cell signaling pathways, metabolism or the tumor microenvironment, have emerged as potentially useful to select those patients most likely to benefit from immunotherapy. Moreover, most patients ultimately develop acquired resistance to ICI treatment over time and novel therapeutic strategies are urgently needed to overcome or delay resistance. Herein, we provide an overview on recent advances in immunotherapy in NSCLC, focusing on updated results from studies on ICIs in different disease settings and at different lines of treatment. We further describe currently emerging predictive biomarkers, beyond PD-L1, to optimize patient selection and novel strategies to improve clinical outcomes.Keywords: immunotherapy, anti-PD-1/PD-L1 antibodies, non-small cell lung cancer

3. A narrative review of MET inhibitors in non-small cell lung cancer with MET exon 14 skipping mutations

Abstract: Treatment of advanced non-small cell lung cancer (NSCLC) has radically improved in the last years due to development and clinical approval of highly effective agents including immune checkpoint inhibitors (ICIs) and oncogene-directed therapies. Molecular profiling of lung cancer samples for activated oncogenes, including epidermal growth factor receptor (EGFR), anaplastic lymphoma kinase (ALK), c-ros oncogene 1 (ROS1) and BRAF, is routinely performed to select the most appropriate up-front treatment. However, the identification of new therapeutic targets remains a high priority. Recently, MET exon 14 skipping mutations have emerged as novel actionable oncogenic alterations in NSCLC, sensitive to MET inhibition. In this review we discuss: (I) MET gene and MET receptor structure and signaling pathway; (II) MET exon 14 alterations; (III) current data on MET inhibitors, mainly focusing on selective MET tyrosine kinase inhibitors (TKIs), in the treatment of NSCLC with MET exon 14 skipping mutations. We identified the references for this review through a literature search of papers about MET, MET exon 14 skipping mutations, and MET inhibitors, published up to September 2020, by using PubMed, Scopus and Web of Science databases. We also searched on websites of main international cancer congresses (ASCO, ESMO, IASLC) for ongoing studies presented as abstracts. MET exon 14 skipping mutations have been associated with clinical activity of selective MET inhibitors, including capmatinib, that has recently received approval by FDA for clinical use in this subgroup of NSCLC patients. A large number of trials are testing MET inhibitors, also in combinatorial therapeutic strategies, in MET exon 14-altered NSCLC. Results from these trials are eagerly awaited to definitively establish the role and setting for use of these agents in NSCLC patients.

Abstract: To verify whether it is possible, in people with diabetes mellitus (DM) considered at very high cardiovascular (CV) risk, stratify this risk better and identify significant modifiable risk factor (including lifestyle habits) to help patients and clinicians improve CV prevention. People with DM and microvascular diseases or one or more CV risk factors (hypertension, hyperlipidemia, smoking, poor dietary habits, overweight, physical inactivity) included in the Risk and Prevention study were selected. We considered the combined endpoint of non-fatal acute myocardial infarction and stroke and CV death. A multivariate Cox proportional analysis was carried out to identify relevant predictors. We also used the RECPAM method to identify subgroups of patients at higher risk. In our study, the rate of major CV events was lower than expected (5 % in 5 years). Predictors of CV events were age, male, sex, heart failure, previous atherosclerotic disease, atrial fibrillation, insulin treatment, high HbA1c, heart rate and other CV diseases while being physically active was protective. RECPAM analysis indicated that history of atherosclerotic diseases and a low BMI defined worse prognosis (HR 4.51 95 % CI 3.04–6.69). Among subjects with no previous atherosclerotic disease, men with HbA1c more than 8 % were at higher CV risk (HR 2.77; 95 % CI 1.86–4.14) with respect to women. In this population, the rate of major CV events was lower than expected. This prediction model could help clinicians identify people with DM at higher CV risk and support them in achieving goals of physical activity and HbA1c.

Abstract: Background Trials have shown a beneficial effect of n-3 polyunsaturated fatty acids in patients with a previous myocardial infarction or heart failure. We evaluated the potential benefit of such therapy in patients with multiple cardiovascular risk factors or atherosclerotic vascular disease who had not had a myocardial infarction. Methods In this double-blind, placebo-controlled clinical trial, we enrolled a cohort of patients who were followed by a network of 860 general practitioners in Italy. Eligible patients were men and women with multiple cardiovascular risk factors or atherosclerotic vascular disease but not myocardial infarction. Patients were randomly assigned to n-3 fatty acids (1 g daily) or placebo (olive oil). The initially specified primary end point was the cumulative rate of death, nonfatal myocardial infarction, and nonfatal stroke. At 1 year, after the event rate was found to be lower than anticipated, the primary end point was revised as time to death from cardiovascular causes or admission to the hospital for cardiovascular causes. Results Of the 12,513 patients enrolled, 6244 were randomly assigned to n-3 fatty acids and 6269 to placebo. With a median of 5 years of follow-up, the primary end point occurred in 1478 of 12,505 patients included in the analysis (11.8%), of whom 733 of 6239 (11.7%) had received n-3 fatty acids and 745 of 6266 (11.9%) had received placebo (adjusted hazard ratio with n-3 fatty acids, 0.97; 95% confidence interval, 0.88 to 1.08; P=0.58). The same null results were observed for all the secondary end points. Conclusions In a large general-practice cohort of patients with multiple cardiovascular risk factors, daily treatment with n-3 fatty acids did not reduce cardiovascular mortality and morbidity. Abstract: Surface modification of poly(ethylene terephthalate) (PET) fabrics induced by air radiofrequency (RF) plasma treatment has been investigated systematically as a function of plasma device parameters, to identify the plasma-polymer surface interactions prevailing under different operating conditions and leading to an increased color depth upon dyeing. Some tests have also been performed employing chemically inert argon as a feedstock gas. The dyeing properties of plasma-treated fibers were correlated to their topographical characteristics, determined by AFM analysis, and to their chemical surface composition, determined by XPS analysis, while the plasma-originated UV radiation was found to have no relevant effects in PET surface modification. The relative importance of plasma-induced surface processes, such as etching and grafting of polar species, is discussed in relation to their role in modifying PET dyeing properties. Abstract: Surface modification induced on poly(ethylene terephthalate) (PET) fibres by cold SF6 plasma treatment has been investigated systematically as a function of plasma device parameters. The observed wettability modifications of fibres plasma treated under different operating conditions were correlated to their dyeability modifications and to the changes in surface chemical composition, determined by X-ray photoelectron spectroscopy (XPS), and in topography, investigated by atomic force microscopy (AFM). Optical emission spectra from the SF6 plasma at different pressure gave information on its content of fluorine atoms. A striking transition was observed between increased hydrophilicity and high dyeability, imparted by plasma treatment at low pressure, mainly as a consequence of surface etching and surface activation, and increased hydrophobicity, imparted by plasma treatment in the higher pressure regime (0.2-0.4 mbar), consequent to extended surface fluorination.

Plasma treatment led to efficient implantation of fluorine atoms on the surface of both polymers; this resulted in water repellence without altering the bulk properties of the polymers. The radical species formed in the plasma-activated polymer surface were involved in its fluorination and in the subsequent uptake of atmospheric oxygen. Surface reorganisation of polymer segments, tending to reduce the interfacial energy between the polymer and the phase in contact with it, induced the surface modifications observed under ageing and in samples plasma-treated several times. An increase in the depth of the fluorinated layer, leading to outstanding stable hydrorepellence, was achieved by repeated SF6 plasma treatments, followed by surface rearrangements favoured by swelling.

Abstract: This research concerns the development of a SF6 RF discharge at low pressure in a small reactor for industrial applications. The plasma is produced in the pressure range 0.05-1 mbar by a RF supply. The pumping system sustains a flowrate of about 50 cm s-1, with residence time in the discharge of about 0.2 s at a pressure of 0.1 mbar. The discharge parameters were measured at a low operation power. Measurements were performed by means of movable electrostatic probes and a photodiode. Particular care in the analysis of the data proved to be necessary due to the presence of a substantial amount of negative ions. The reactor has been employed for textile treatment in order to modify the surface properties of the fibres. Favourable operating conditions leading to an improved hydrophobicity of the textiles were achieved.

Abstract: Silk samples, treated in an appositely set-up radio frequency SF6 plasma reactor under different operation conditions, were characterized by XPS, EPR, DSC, XRD, ATR analyses and water contact angle measurements, also as a function of storage time after the treatment. Efficient attachment of fluorine atoms on the polymer surface occurred during the plasma treatment, imparting water repellence to it. Also the percentage of surface oxygen increased after the treatment, partially as a consequence of radical scavenging by molecular oxygen and the formation of peroxides. An increase in silk crystallinity during the treatment seems to contribute to ensuring durable water repellence properties.

Abstract: Marking consists of adding a mark on a product for the purpose of its identification. The use of methacrylamide (MAam) and 2-hydroxyethylmetacrylate (HEMA) as markers for cellulose yarn (flax) has been studied. Grafting of a reactive monomer onto cellulose chains has been carried out using a redox system with a chemical initiation technique. The products were characterized by Fourier transform infrared (FT-IR) spectroscopy. HEMA has been chosen due to its more pronounced characteristic band. The major factors affecting HEMA polymerization, including concentration, reaction time and temperature, were studied to establish the optimum grafting conditions. This search has aimed to identify a method in order to mark a yarn, so that it can be identified and recognized among others morphologically identical. It has been chosen the linen between various natural fibres having like objective that its physico-chemical and mechanical properties remained unchanged. The first step has been the observation and study of flax grafting with hydroxyethylmethacrylate and methacrylamide, with successive identification of respective co-polymer obtained and some other characteristics. From the comparison between physico-mechanical characteristics of grafted yarn and those of pure linen do not emerge significant variations.

Abstract: Purpose: Several uncommon genomic alterations beyond RAS and BRAFV600E mutations drive primary resistance to anti-epidermal growth factor receptors (EGFRs) in metastatic colorectal cancer (mCRC). Our PRESSING panel (including PIK3CA exon 20/ AKT1 / PTEN mutations, ERBB2 / MET amplifications, gene fusions, and microsatellite instability-high status) represented a paradigm of negative hyperselection with more precise tailoring of EGFR blockade. However, a modest proportion of hyperselected mCRC has intrinsic resistance potentially driven by even rarer genomic alterations., Materials and Methods: A prospective data set at three Italian Academic Hospitals included 650 patients with mCRC with comprehensive genomic profiling by FoundationOne CDx and treated with anti-EGFRs. PRESSING2 panel alterations were selected on the basis of previous clinico-biologic studies and included NTRKs , ERBB3 , NF1 , MAP2K1 / 2 / 4 , AKT2 pathogenic mutations; PTEN / NF1 loss; ERBB3 , FGFR2 , IGF1R , KRAS , ARAF , and AKT1-2 amplification; and EGFR rearrangements. These were collectively associated with outcomes in patients with hyperselected disease, ie, RAS / BRAF wild-type, PRESSING-negative, and microsatellite stable., Results: Among 162 hyperselected patients, 24 (15%) had PRESSING2 alterations, which were mutually exclusive except in two samples and were numerically higher in right-sided versus left-sided cancers (28% v 13%; P = .149). Independently of sidedness and other factors, patients with PRESSING2-positive status had significantly worse progression-free survival and overall survival compared with PRESSING2-negative ones (median progression-free survival 6.4 v 12.8 months, adjusted hazard ratio 4.19 [95% CI, 2.58 to 6.79]; median overall survival: 22.6 v 49.9 months, adjusted hazard ratio 2.98 [95% CI, 1.49 to 5.96]). 39. Biomarker Landscape in Neuroendocrine Tumors With High-Grade Features: Current Knowledge and Future Perspective.

Abstract: Neuroendocrine tumors (NETs) are classified based on morphology and are graded based on their proliferation rate as either well-differentiated low-grade (G1) to intermediate (G2-G3) or poorly differentiated high-grade neuroendocrine carcinomas (NEC G3). Recently, in gastroenteropancreatic (GEP) NETs, a new subgroup of well-differentiated high-grade tumors (NET G3) has been divided from NEC by WHO due to its different clinical-pathologic features. Although several mutational analyses have been performed, a molecular classification of NET is an unmet need in particular for G3, which tends to be more aggressive and have less benefit to the available therapies. Specifically, new possible prognostic and, above all, predictive factors are highly awaited, giving the basis for new treatments. Alteration of KRAS , TP53 , and RB1 is mainly reported, but also druggable alterations, including BRAF and high microsatellite instability (MSI-H), have been documented in subsets of patients. In addition, PD-L1 demonstrated to be highly expressed in G3 NETs, probably becoming a new biomarker for G3 neuroendocrine neoplasm (NEN) discrimination and a predictive one for immunotherapy response. 40. A narrative review of MET inhibitors in non-small cell lung cancer with MET exon 14 skipping mutations.

Abstract: Treatment of advanced non-small cell lung cancer (NSCLC) has radically improved in the last years due to development and clinical approval of highly effective agents including immune checkpoint inhibitors (ICIs) and oncogene-directed therapies. Molecular profiling of lung cancer samples for activated oncogenes, including epidermal growth factor receptor ( EGFR ), anaplastic lymphoma kinase ( ALK ), c-ros oncogene 1 ( ROS1 ) and BRAF , is routinely performed to select the most appropriate up-front treatment. However, the identification of new therapeutic targets remains a high priority. Recently, MET exon 14 skipping mutations have emerged as novel actionable oncogenic alterations in NSCLC, sensitive to MET inhibition. In this review we discuss: (I) MET gene and MET receptor structure and signaling pathway; (II) MET exon 14 alterations; (III) current data on MET inhibitors, mainly focusing on selective MET tyrosine kinase inhibitors (TKIs), in the treatment of NSCLC with MET exon 14 skipping mutations. We identified the references for this review through a literature search of papers about MET, MET exon 14 skipping mutations, and MET inhibitors, published up to September 2020, by using PubMed, Scopus and Web of Science databases. We also searched on websites of main international cancer congresses (ASCO, ESMO, IASLC) for ongoing studies presented as abstracts. MET exon 14 skipping mutations have been associated with clinical activity of selective MET inhibitors, including capmatinib, that has recently received approval by FDA for clinical use in this subgroup of NSCLC patients. A large number of trials are testing MET inhibitors, also in combinatorial therapeutic strategies, in MET exon 14-altered NSCLC. 41. Capillary leak syndrome induced by neoadjuvant cisplatin and gemcitabine in a patient with bladder cancer.

Abstract: Capillary leak syndrome (CLS) is a rare disorder associated with an increased capillar permeability due to an endothelial damage, causing leakage of plasma and proteins into the interstitial compartment. CLS is characterized by rapidly developing edema, hypotension and hypoproteinemia. We observed CLS in a 54-year-old man affected by muscle-invasive bladder cancer who received neoadjuvant treatment with Cisplatin and Gemcitabine. Treatment with infusion of albumin and increasing corticosteroid doses and diuretics led to a complete regression of all signs and symptoms related to the disorder. Abstract: Introduction: Clinical trials have demonstrated the safety and efficacy of hyaluronic acid-based products for the treatment of hip osteoarthritis, but data from observational studies of normal medical practice are scarce. This study investigated the long-term efficacy and tolerability of ultrasound-guided intra-articular sodium hyaluronate (MW 1,500-2,000 kDa; Hyalone) injections in daily clinical practice., Methods: In this observational, cohort study of patients with hip osteoarthritis, Hyalone was administered under the ultrasound guidance, every 6 months, with the possibility of an additional injection at the intervening 3-month intervals on clinical request. Efficacy measurements included the Lequesne algofunctional index, self-reported pain via the visual analogue scale (VAS), the concomitant use of non-steroidal anti-inflammatory drugs (NSAIDs) and safety. The patients were followed up for 18 months after the first intra-articular injection., Results: Data from 120 patients were collected. During the study, a statistically significant reduction in algofunctional indexes was demonstrated at 3 months after study product injection, while at 12 months 80% of the patients achieved a decrease of at least 30% in symptoms. These results were maintained over time through cyclical and personalized repetition of ultrasound guided injections, at least one injection every 6 months., Conclusions: The study treatment reduced pain and improved mobility in osteoarthritis of the hip. These results in daily clinical practice demonstrate a beneficial effect and the safety of the study product and suggest adding intra-articular injections of HyalOne to the armamentarium of conservative management of symptomatic hip osteoarthritis.
