- Abstract
Introduction: Systemic sclerosis (SSc) is characterized by a complex etiopathogenesis encompassing both host genetic and environmental -infectious/toxic- factors responsible for altered fibrogenesis and diffuse microangiopathy. A wide spectrum of clinical phenotypes may be observed in patients' populations from different geographical areas. We investigated the prevalence of specific clinical and serological phenotypes in patients with definite SSc enrolled at tertiary referral centres in different Italian geographical macro-areas. The observed findings were compared with those reported in the world literature. Materials and methods: The clinical features of 1538 patients (161 M, 10.5%; mean age 59.8 ± 26.9 yrs.; mean disease duration 8.9 ± 7.7 yrs) with definite SSc recruited in 38 tertiary referral centres of the SPRING (Systemic sclerosis Progression INvestiGation Group) registry promoted by Italian Society of Rheumatology (SIR) were obtained and clustered according to Italian geographical macroareas. Results: Patients living in Southern Italy were characterized by more severe clinical and/or serological SSc phenotypes compared to those in Northern and Central Italy; namely, they show increased percentages of diffuse cutaneous SSc, digital ulcers, sicca syndrome, muscle involvement, arthritis, cardiopulmonary symptoms, interstitial lung involvement at HRCT, as well increased prevalence of serum anti-Scl70 autoantibodies. In the same SSc population immunusppressive drugs were frequently employed. The review of the literature underlined the geographical heterogeneity of SSc phenotypes, even if the observed findings are scarcely comparable due to the variability of methodological approaches. Conclusion: The phenotypical differences among SSc patients' subgroups from Italian macro-areas might be correlated to genetic/environmental co-factors, and possibly to a not equally distributed national network of information and healthcare facilities.

- Abstract
OBJECTIVE: There is still a great deal to learn about the influence of sex in systemic sclerosis (SSc). In this respect, national registries provide large and homogeneous patient cohorts for analytical studies. We therefore investigated a wide-ranging and well-characterized SSc series with the aim of identifying sex differences in disease expression, with a special focus on demographic, clinical, and serological characteristics. METHODS: A multicenter SSc cohort of 2281 patients, including 247 men, was recruited in the Italian Systemic sclerosis PRogression INvestiGation (SPRING) registry. Demographic data, disease manifestations, serological profile, and internal organ involvement were compared. RESULTS: The overall female/male ratio was 8.2:1. Female/male ratios for limited cutaneous SSc, diffuse cutaneous SSc, and SSc sine scleroderma subsets were 8.7:1, 4.9:1, and 10.7:1, respectively. A shorter time from onset of Raynaud phenomenon to SSc diagnosis, an increased prevalence of the diffuse cutaneous subset, renal crisis, and digital ulcers were found in males, whereas a significantly higher percentage of sicca syndrome, serum antinuclear antibodies, antiextractable nuclear antigens, anti-La/SSB, and anticentromere protein B was detected in the female group. Males exhibited lower left ventricular ejection fraction, as well as higher prevalence of conduction blocks, arrhythmias, ground glass, and honeycombing. Moreover, forced vital capacity and total lung capacity were medially lower in men than in women. Finally, males were more frequently treated with immunosuppressive drugs. CONCLUSION: Our study further supports the presence of several sex-related differences in patients with SSc. These differences were pronounced in the severity of cutaneous, peripheral vascular, and cardiopulmonary involvement for male patients, whereas an increased prevalence of sicca syndrome and a specific autoantibody profile characterized the female sex.

- Abstract
OBJECTIVES: It has recently become possible to assess lung vascular and parenchymal changes quantitatively in thoracic CT images using automated software tools. We investigated the vessel parameters of patients with SSc, quantified by CT imaging, and correlated them with interstitial lung disease (ILD) features. METHODS: SSc patients undergoing standard of care pulmonary function testing and CT evaluation were retrospectively evaluated. CT images were analysed for ILD patterns and total pulmonary vascular volume (PVV) extents with Imbio lung texture analysis. Vascular analysis (volumes, numbers and densities of vessels, separating arteries and veins) was performed with an in-house developed software. A threshold of 5% ILD extent was chosen to define the presence of ILD, and commonly used cut-offs of lung function were adopted. RESULTS: A total of 79 patients [52 women, 40 ILD, mean age 56.2 (s.d. 14.2) years, total ILD extent 9.5 (10.7)%, PVV/lung volume % 2.8%] were enrolled. Vascular parameters for total and separated PVV significantly correlated with functional parameters and ILD pattern extents. SSc-associated ILD (SSc-ILD) patients presented with an increased number and volume of arterial vessels, in particular those between 2 and 4 mm of diameter, and with a higher density of arteries and veins of <6 mm in diameter. Considering radiological and functional criteria concomitantly, as well as the descriptive trends from the longitudinal evaluations, the normalized PVVs, vessel numbers and densities increased progressively with the increase/worsening of ILD extent and functional impairment. CONCLUSION: In SSc patients CT vessel parameters increase in parallel with ILD extent and functional impairment, and may represent a biomarker of SSc-ILD severity.

- Abstract
Introduction: The impact of COVID-19 pandemic represents a serious challenge for ‘frail’ patients' populations with inflammatory autoimmune systemic diseases such as systemic sclerosis (SSc). We investigated the prevalence and severity of COVID-19, as well the effects of COVID-19 vaccination campaign in a large series of SSc patients followed for the entire period (first 38 months) of pandemic. Patients and method: This prospective survey study included 1755 unselected SSc patients (186 M, 1,569F; mean age 58.7 ± 13.4SD years, mean disease duration 8.8 ± 7.3SD years) recruited in part by telephone survey at 37 referral centers from February 2020 to April 2023. The following parameters were carefully evaluated: i. demographic, clinical, serological, and therapeutical features; ii. prevalence and severity of COVID-19; and iii. safety, immunogenicity, and efficacy of COVID-19 vaccines. Results: The prevalence of COVID-19 recorded during the whole pandemic was significantly higher compared to Italian general population (47.3 % vs 43.3 %, p < 0.000), as well the COVID-19-related mortality (1.91 % vs 0.72 %, p < 0.001). As regards the putative prognostic factors of worse outcome, COVID-19 positive patients with SSc-related interstitial lung involvement showed significantly higher percentage of COVID-19-related hospitalization compared to those without (5.85 % vs 1.73 %; p < 0.0001), as well as of mortality rate (2.01 % vs 0.4 %; p = 0.002). Over half of patients (56.3 %) received the first two plus one booster dose of vaccine; while a fourth dose was administered to 35.6 %, and only few of them (1.99 %) had five or more doses of vaccine. Of note, an impaired seroconversion was recorded in 25.6 % of individuals after the first 2 doses of vaccine, and in 8.4 % of patients also after the booster dose. - Abstract
Objective. There is still a great deal to learn about the influence of sex in systemic sclerosis (SSc). In this respect, national registries provide large and homogeneous patient cohorts for analytical studies. We therefore investigated a wide-ranging and well-characterized SSc series with the aim of identifying sex differences in disease expression, with a special focus on demographic, clinical, and serological characteristics. Methods. A multicenter SSc cohort of 2281 patients, including 247 men, was recruited in the Italian Systemic sclerosis PRogression INvestiGation (SPRING) registry. Demographic data, disease manifestations, serological profile, and internal organ involvement were compared. Results. The overall female/male ratio was 8.2:1. Female/male ratios for limited cutaneous SSc, diffuse cutaneous SSc, and SSc sine scleroderma subsets were 8.7:1, 4.9:1, and 10.7:1, respectively. A shorter time from onset of Raynaud phenomenon to SSc diagnosis, an increased prevalence of the diffuse cutaneous subset, renal crisis, and digital ulcers were found in males, whereas a significantly higher percentage of sicca syndrome, serum antinuclear antibodies, antiextractable nuclear antigens, anti-La/SSB, and anticentromere protein B was detected in the female group. Males exhibited lower left ventricular ejection fraction, as well as higher prevalence of conduction blocks, arrhythmias, ground glass, and honeycombing. Moreover, forced vital capacity and total lung capacity were medially lower in men than in women. Finally, males were more frequently treated with immunosuppressive drugs. Conclusion. Our study further supports the presence of several sex-related differences in patients with SSc. These differences were pronounced in the severity of cutaneous, peripheral vascular, and cardiopulmonary involvement for male patients, whereas an increased prevalence of sicca syndrome and a specific autoantibody profile characterized the female sex.

- Abstract
Autoimmune systemic diseases (ASD) show impaired immunogenicity to COVID-19 vaccines. Our prospective observational multicenter study aimed at evaluating the seroconversion elicited by COVID-19 vaccine over the entire vaccination cycle including the booster dose. Among 478 unselected ASD patients originally evaluated at the end of the first vaccination cycle (time 1), 344 individuals were re-evaluated after a 6-month period (time 2), and 244 after the booster vaccine dose (time 3). The immunogenicity of mRNA COVID-19 vaccines (BNT162b2 and mRNA-1273) was assessed by measuring serum IgG-neutralizing antibody (NAb) on samples obtained at the three time points in both patients and 502 age-matched controls. In the 244 ASD group that received booster vaccine and monitored over the entire follow-up, the mean serum NAb levels (time 1, 2, and 3: 696.8 ± 52.68, 370.8 ± 41.92, and 1527 ± 74.16SD BAU/mL, respectively; p < 0.0001) were constantly lower compared to controls (p < 0.0001), but they significantly increased after the booster dose compared to the first two measurements (p < 0.0001). The percentage of patients with absent/suboptimal response to vaccine significantly decreased after the booster dose compared to the first and second evaluations (time 1, 2, and 3: from 28.2% to 46.3%, and to 7.8%, respectively; p < 0.0001). Of note, the percentage of patients with absent/suboptimal response after the booster dose was significantly higher compared to controls (19/244, 7.8% vs 1/502, 0.2%; p < 0.0001). Similarly, treatment with immune-modifiers increased the percentage of patients exhibiting absent/suboptimal response (16/122, 13.1% vs 3/122, 2.46%; p = 0.0031). Overall, the above findings indicate the usefulness of booster vaccine administration in ASD patients. - Abstract
Introduction: Systemic sclerosis (SSc) is characterized by a complex etiopathogenesis encompassing both host genetic and environmental -infectious/toxic- factors responsible for altered fibrogenesis and diffuse microangiopathy. A wide spectrum of clinical phenotypes may be observed in patients' populations from different geographical areas. We investigated the prevalence of specific clinical and serological phenotypes in patients with definite SSc enrolled at tertiary referral centres in different Italian geographical macro-areas. The observed findings were compared with those reported in the world literature. Materials and methods: The clinical features of 1538 patients (161 M, 10.5%; mean age 59.8 ± 26.9 yrs.; mean disease duration 8.9 ± 7.7 yrs) with definite SSc recruited in 38 tertiary referral centres of the SPRING (Systemic sclerosis Progression INvestiGation Group) registry promoted by Italian Society of Rheumatology (SIR) were obtained and clustered according to Italian geographical macroareas. Results: Patients living in Southern Italy were characterized by more severe clinical and/or serological SSc phenotypes compared to those in Northern and Central Italy; namely, they show increased percentages of diffuse cutaneous SSc, digital ulcers, sicca syndrome, muscle involvement, arthritis, cardiopulmonary symptoms, interstitial lung involvement at HRCT, as well increased prevalence of serum anti-Scl70 autoantibodies. In the same SSc population immunusppressive drugs were frequently employed. The review of the literature underlined the geographical heterogeneity of SSc phenotypes, even if the observed findings are scarcely comparable due to the variability of methodological approaches. Conclusion: The phenotypical differences among SSc patients' subgroups from Italian macro-areas might be correlated to genetic/environmental co-factors, and possibly to a not equally distributed national network of information and healthcare facilities.

Autoimmune systemic diseases (ASD) may show impaired immunogenicity to COVID-19 vaccines. Our prospective observational multicenter study aimed to evaluate the seroconversion after the vaccination cycle and at 6-12-month follow-up, as well the safety and efficacy of vaccines in preventing COVID-19. The study included 478 unselected ASD patients (mean age 59 ± 15 years), namely 101 rheumatoid arthritis (RA), 38 systemic lupus erythematosus (SLE), 265 systemic sclerosis (SSc), 61 cryoglobulinemic vasculitis (CV), and a miscellanea of 13 systemic vasculitis. The control group included 502 individuals from the general population (mean age 59 ± 14SD years). The immunogenicity of mRNA COVID-19 vaccines (BNT162b2 and mRNA-1273) was evaluated by measuring serum IgG-neutralizing antibody (NAb) (SARS-CoV-2 IgG II Quant antibody test kit; Abbott Laboratories, Chicago, IL) on samples obtained within 3 weeks after vaccination cycle. The short-term results of our prospective study revealed significantly lower NAb levels in ASD series compared to controls [286 (53–1203) vs 825 (451–1542) BAU/mL, p < 0.0001], as well as between single ASD subgroups and controls. More interestingly, higher percentage of non-responders to vaccine was recorded in ASD patients compared to controls [13.2% (63/478), vs 2.8% (14/502); p < 0.0001]. Increased prevalence of non-response to vaccine was also observed in different ASD subgroups, in patients with ASD-related interstitial lung disease (p = 0.009), and in those treated with glucocorticoids (p = 0.002), mycophenolate-mofetil (p < 0.0001), or rituximab (p < 0.0001). Comparable percentages of vaccine-related adverse effects were recorded among responder and non-responder ASD patients. Patients with weak/absent seroconversion, believed to be immune to SARS-CoV-2 infection, are at high risk to develop COVID-19. A large-sized species of Parasorex is common in the MN 13 mammal assemblages from the uppermost Messinian sandy-marly fissure fillings within the Gessoso Solfifera Formation at Brisighella (Northern Apennine). This erinaceid has been classified as Galerix sp. in the first papers on the Brisighella fauna. Later, it was described in detail in an unpublished Ph.D. dissertation by Fanfani (1999), who referred it to Galerix depereti. Van den Hoek Ostende (2001) included G. depereti in the genus Parasorex, Parasorex depereti has been described by Crochet (1986) on scarce material from a few Early Pliocene (MN 14–15) localities of southern France and Spain. Parasorex cf. depereti has been reported from the Early Pliocene fauna of Capo Mannu (Mandriola, Sardinia; Furió and Angelone 2010). The species seems actually distributed in south-western Europe, where it represents the youngest occurrence of the genus Parasorex. The very abundant sample of P. depereti from fissure filling BRS 25 enables a more accurate and comprehensive description of the species. It also permits inspection of the mesial elements of the dentition, which were lacking in the material examined by Crochet (1986). The systematic position of the species has been revisited and compared with those of other Galericini of the Parasorex group.

Objectives Racial factors play a significant role in SSc. We evaluated differences in SSc presentations between white patients (WP), Asian patients (AP) and black patients (BP) and analysed the effects of geographical locations. Methods SSc characteristics of patients from the EUSTAR cohort were cross-sectionally compared across racial groups using survival and multiple logistic regression analyses. Results The study included 9162 WP, 341 AP and 181 BP. AP developed the first non-RP feature faster than WP but slower than BP. AP were less frequently anti-centromere (ACA; odds ratio (OR) = 0.4, P < 0.001) and more frequently anti-topoisomerase-I autoantibodies (ATA) positive (OR = 1.2, P = 0.068), while BP were less likely to be ACA and ATA positive than were WP [OR(ACA) = 0.3, P < 0.001; OR(ATA) = 0.5, P = 0.020]. AP had less often (OR = 0.7, P = 0.06) and BP more often (OR = 2.7, P < 0.001) diffuse skin involvement than had WP. AP and BP were more likely to have pulmonary hypertension [OR(AP) = 2.6, P < 0.001; OR(BP) = 2.7, P = 0.03 vs WP] and a reduced forced vital capacity [OR(AP) = 2.5, P < 0.001; OR(BP) = 2.4, P < 0.004] than were WP. AP more often had an impaired diffusing capacity of the lung than had BP and WP [OR(AP vs BP) = 1.9, P = 0.038; OR(AP vs WP) = 2.4, P < 0.001]. After RP onset, AP and BP had a higher hazard to die than had WP [hazard ratio (HR) (AP) = 1.6, P = 0.011; HR(BP) = 2.1, P < 0.001]. Conclusion Compared with WP, and mostly independent of geographical location, AP have a faster and earlier disease onset with high prevalences of ATA, pulmonary hypertension and forced vital capacity impairment and higher mortality. BP had the fastest disease onset, a high prevalence of diffuse skin involvement and nominally the highest mortality.

- Abstract
This paper contributes to the study of evaluative morphology by investigating an emerging morphological construction in Italian within the framework of Construction Morphology. The schema in question, which contains the string simil-(related to the adjective simile ‘similar’) plus a nominal or adjectival base, is analyzed as a newly-created construction that conveys a number of closely-related senses (i.e., fakeness, imitation, resemblance, vagueness, and kin-categorization) revolving around the functional domain of approximation, which has received much less attention than other domains within evaluative morphology. Beside discussing the formal, semantic and usage properties of simil-expressions on the basis of corpus data, we propose a constructional network that accounts for their behavior. Finally, we discuss the nature of simil-as an affixoid and explore its relationship with other competing (morphological and, more marginally, analytic) strategies in Italian.

- Abstract
Lexical-Functional Grammar (LFG) and Head-Driven Phrase Structure Grammar (HPSG) are both lexicalist, non-transformational, constraint-based grammatical frameworks. While they differ in many respects, they share a number of fundamental principles relevant to morphological theory and analysis, which guide the overall architecture of the grammar. The two frameworks also share a common commitment to being fully explicit and implementable, with strong links to computational implementations. This chapter provides an overview of the general approaches to morphology and the morphology-syntax interface taken by researchers working within these frameworks, illustrating the relevant aspects of each framework through discussion of morphological phenomena such as multiple exponence, auxiliaries, case stacking, morphotactics and clitics.

Deinogalerix is a typical component of the late Miocene insular fauna of Gargano (south-eastern Italy). Remains of this taxon had also been found over the last 20 years at the early Tortonian site of Scontrone (Abruzzo, central Italy), but they had never been described in detail. Two new species are introduced. One of them, D. samniticus sp. nov., is larger sized and better documented, and received a formal description. The other entity is clearly distinct from D. samniticus sp. nov., but it is merely represented by a single mandible. It is therefore presented here as a different, but undetermined species, Deinogalerix sp. Comparative analyses show that the new species have exclusive dental proportions that distinguish them from all the other species of Deinogalerix and seem to exclude them from the direct ancestry of the Gargano clade. Consistent with its early stratigraphic age, D. samniticus sp. nov. bears a suite of primitive characters of the genus. Besides these, however, it also bears the autapomorphic relative proportion between p3 and p4. The analysis of the erinaceids from Scontrone supports the assumption that Deinogalerix presumably descended from a hypothetical early – middle Miocene ‘Parasorex-like’ galericine. On the other hand, it seems to rule out the supposed common ancestry of the giant Deinogalerix and the small gymnure Apulogalerix from Gargano. Because the presumed ancestor of Deinogalerix dates back to the early – middle Miocene, the study corroborates the asynchronous colonization of the Apulia Platform.

Works for the construction of thermal pools at Poggetti Vecchi, near Grosseto (Tuscany, Italy) exposed an accumulation of fossil bones, largely belonging to the straight-tusked elephant Palaeoloxodon antiquus, mixed up with stone and wooden tools. The site is radiometrically dated to the late Middle Pleistocene, and the artefacts were thus created by early Neanderthals. Palaeobiological and taphonomic analyses of the fauna remains are part of a more general, multiproxy study of the site that provides new information on MIS 7-6 transition, as well as on human-animal interactions.

37. Listing between lexicon and syntax: focus on frame-naming lists

- Abstract
The goal of this paper is twofold. On the one hand, we advocate the view that ‘listing’ (Masini, Mauri & Pietrandrea this issue) is a cross-level mechanism with cognitive grounding that manifests itself in various ways along the lexicon-syntax continuum. Indeed, we claim that some linguistic objects pertaining to the morphological and lexical level (as e.g. coordinating compounds, reduplications, (ir)reversible binomials) are structurally and functionally similar to freely created syntactic lists. On the other hand, we analyze a so far underdescribed type of lists in-between lexicon and syntax. Lists are often seen as instances of “natural coordination” (Wälchli 2005) where the conjuncts are LEXICO-SEMANTICALLY related; however, there are lists where the conjuncts are, rather, FRAME-related, i.e., by virtue of occurring in a list, they either EVOKE or BUILD a frame, which may be either established or context-dependent. In this respect, we present two corpus-based case-studies from Italian concerning the V1+AND+V2 construction (e.g. gratta e sosta ‘scratch and park ticket’, lit. ‘scratch and park’) and the ALL+LIST construction (e.g. tutto casa e chiesa ‘pious, churchy’, lit. ‘all home and church’). We discuss and analyze our data with the tools of Construction Grammar (Goldberg 1995, 2006) and Construction Morphology (Booij 2010)

Questa relazione ha l'obiettivo di ripercorrere la sua produzione scritta per cercare di rintracciare le costanti che ne hanno definito l'alto valore anche sul piano espositivo e letterario. Si dimostrerà, quindi, che questo tipo di stile riflette nella forma un'indagine filosofica sul valore comunicativo della filosofia, in particolare attraverso forme non sistematiche come l'aforisma, che è alla base delle ricerche più prolifiche di Colli.

42. MECHANICAL PROSTHETIC MITRAL VALVE THROMBOSIS COMPLICATED BY AN ACUTE CORONARY SYNDROME AFTER SYSTEMIC FIBRINOLYSIS: A CASE REPORT

The annual incidence of blocked mechanical valvular prosthesis (BMVP) is 0.3–5.7%. 78% of cases are due to thrombosis and development of pannus is the second more frequent etiology. The risk of thrombosis is higher for mitral prosthesis, during the 1st year after valve replacement, and for poor INR trend. TEE is the gold standard for diagnoses, but it is seldom required if adequate TTE and cine–fluoroscopy are performed. As therapy, redo surgery is recommended in case of NYHA III–IV patients or in presence of big thrombi: if surgery is contraindicated or too high–risk, systemic thrombolysis is a reasonable option. In less compromised patients, upgrading of the anti–thrombotic therapy can be considered. In our Centre (around 600 valve replacement surgeries/year, mainly high risk patients) we use thrombolysis for BMVP in decompensated patients, choosing between two different protocols: the so called “fast” protocol (alteplase 15 mg iv bolus, then 0.75 mg/kg over 30’ and 0.5 mg/kg over 1h) is provided for NYHA IV patients, while the “slow” protocol (25 mg over 6 h) is the treatment of choice for the NYHA III ones. Many complications, mainly bleeding, are associated to thrombolysis. A 30 years old lady who underwent MVR in 08/2021, already successfully treated with thrombolysis in 11/2021 due to BMVP, presented in 01/2023 due to NYHA IV heart failure. TTE and cine–fluoroscopy showed BMVP (grad. 29/21 mmHg, area 0.7 cm2). The LV was hypekynetic (EF 81%), and severe pulmonary hypertension (calcSPAP 82 mmHg) was present. Fast thrombolysis was performed, with decrease of gradients (11/6 mmHg) and of pulmonary pressure (45 mmHg), and preserved LV function (EF 55%), without acute complications. On the 2nd day, the patient developed generalized seizures and pulsless VT, with ROSC after DC shock (7’ low–flow time). The neurological recovery was complete, but haemodynamic instability developed; new onset LBBB, and progressive worsening of the LV function (EF 40%, then 25%, medioapical hypokinesia) were documented. The coronary angiography showed a thrombus in the proximal LAD, not occluding the vessel, and TIMI 3 downstream flow. Heparin continuous infusion was provided for 48 h with a 1.8–2.3 aPTT ratio target. The patient was eventually discharged home with normal gradients (12/7 mmHg), EF (67%), ECG and pulmonary pressure. Coronary embolism is a possible complication of BMVP, and can follow systemic thrombolysis. Close multimodal clinical monitoring is mandatory.

43. ENOXIMONE AS A THERAPEUTIC OPTION IN STATUS ASTHMATICUS: A CASE REPORT

- Abstract
Status asthmaticus is an acute exacerbation of asthma that is unresponsive to initial treatment with bronchodilators and can rapidly escalate into acute ventilatory failure. The clinical picture of severe asthma include agitation, drowsiness or confusion, breathlessness at rest, tachypnea, use of accessory respiratory muscles, tachycardia, and pulsus paradoxus. Pharmacological treatments include but are not limited to beta–2–mimetics, anticholinergic agents, magnesium sulphate, steroids and aminophylline. Asthma exacerbations can be triggered by interrupting chronic treatment: in war torn areas as Sudan it is mainly due to the abrupt lack of availability of drugs. A 58 years old 80 kg man with history of severe asthma presented to our triage complaining of severe dyspnea in the last 3 days, unresponsive to treatment with bronchodilators and systemic corticosteroids. He stopped his chronic therapy with budesonide/formeterol due to the unavailability of the drug. At the arrival he showed orthopnea, severe tachypnoea (60 breaths/min) and sinus tachicardia (130 bpm), silent chest and SpO2 86% on reservoir mask 15 l O2/min. The ABG showed pH7.27, pO2 47, pCO2 60. The chest Xray and the transthoracic echo were unremarkable. After receiving repeated nebulizers (salbutamol 5 mg + adrenaline 1 mg every 20’ for 2 h), iv hydrocortisone high dose (1 g), iv aminophylline 240 mg, iv magnesium sulphate 2 g, iv ketamine 100 mg there were neither clinical nor ABG improvement. Given the unavailability of drugs as ipratropium and sevofluorane and of treatments like V–V ECMO, a rescue trial with iv enoximone was performed, administrating 0.3 mg/kg iv bolus followed by 3 mg/kg/min continuous infusion, conbined with NIV to decrease the respiratory workload and salbutamol nebulizers. Improvement of the respiratory mechanics and of the gas exhange were immediate, leading to normal ABG, mild tachypnoea and mild chest wheezes after the administration of 200 mg of enoximone, with no cardiovascular side effects. The patient was discharged home on the 6th day, without respiratory symptoms. Enoximone acts as an inotrope by inhibiting PDE–3 activity and increasing intracellular cAMP levels in cardiac myocytes. Increasing intracellular cAMP is also the main mechanism of β2–agonists bronchodilators, and both experimental models and clinical experience suggest that enoximone can overcome the asthma–related β–receptors downregulation, providing a therapeutic role in status asthmaticus.

44. AN UNCOMMON CASE OF SUPERIOR VENA CAVA SYNDROME FOLLOWING CARDIAC SURGERY

Superior vena cava (SVC) syndrome (SVCS) is due to the obstruction of the SVC leading to swelling of the upper body and to low cardiac output. Non tumoral causes, mainly thrombosis of endovascular devices, account for 30% of cases. We report a case of acute iatrogenic SVCS. A 7 years old 16 kg boy underwent mitral valve surgery in severe rheumatic mitral regurgitation. After induction a 15 cm 5.5 Fr 3 lumens central venous catheter (CVC) was inserted in the right internal jugular vein (IJV) under ultrasound guidance without issues. After achieving proper anticoagulation ascending aorta (18 Fr) and bicaval cannulation were performed (20 Fr single stage cannula with right–angled metal tip in the SVC and 26 Fr single stage cannula in the inferior vena cava). Cardiopulmonary bypass (CPB) was performed with pediatric circuit and the mitral valve was repaired. Despite apparently good result of the repair and uneventful weaning from CPB a more accurate transesophageal echo (TEE) showed moderate iatrogenic mitral stenosis. Cannulation was re–performed with same size cannulas and mitral valve replacement (St.Jude 25 mm) was provided. After weaning from CPB with mild cardiovascular support (adrenaline 0.03 mcg/kg/min and noradrenaline 0.05 mcg/kg/min) the TEE showed no abnormalities and the patient was transferred to the ICU in stable conditions. The central venous pressure (CVP) never raised over 10 mmHg. In the ICU the patient developed sinus tachicardia and hypotension, unresponsive to fluids and vasopressors, and swelling of the face with no increase in CVP. The chest Xray showed enlargement of the right upper mediastinum with TEE evidence of accellerated flow in the SVC around the CVC (fig.1). The patient was taken to the operating room and a too tigh pursue string suture was found on the SVC. After optimization of the suture the flow through the SVC was restored with immediate clinical improvement and de–escalation of the cardiovascular support. The face oedema quickly decreased allowing extubation on the day after surgery. The following course was uneventful. SVCS is a life threatening complication of cardiac surgery. A too tight pledgetted pursue–string suture is an uncommon cause compared to clot compression. The CVP measured by a IJV CVC is generally elevated in SVCS, but in our case the tip of the CVC was downstream to the stenosis so showing normal values. Multimodal monitoring is mandatory to lead to emergency surgical treatment in case of need.
