Abstract The aim of this study was to establish reliability of post-activation performance enhancement in three manners: (1) interday morning and afternoon reliability; (2) intraday morning and afternoon reliability; (3) intraday set-to-set reliability. Twelve elite male volleyball players experienced in resistance training performed four identical experimental sessions—two in the morning and two in the afternoon. During each session participants performed a mini complex training session—three sets of a conditioning activity (CA) (3 repetitions of a trap bar deadlift at 80% 1RM with 15% of accommodating resistance) and 90 s after a CA performed squat jump (SJ) with 4 min intra-set rest interval. The ANOVA with repeated measures was used to assess significance of the effect of a CA and ICC to assess reliability of measurements. The PAPE protocol was found to be ineffective to subsequently enhance JH on various occasions. Also, the results of this study suggest that the practitioners may effectively implement appropriately organized complex training as both intraday set-to-set (0.87 and 0.82 for morning sessions; 0.83 and 0.58 for afternoon sessions) and interday morning (0.67) and afternoon (0.8) reliabilities seem to be acceptable. However, introducing two CT sessions within one day is highly questionable as at the moment intraday morning and afternoon reliability is vague (0.88 and 0.48).

BackgroundFocal onset seizures, characterized by localized neuronal hyperexcitability in the brain, can be related to various structural, immune, genetic, or metabolic abnormalities. Autoimmune epilepsies are increasingly recognized. Neurochondrin antibody has been reported in a variety of rare autoimmune neurological disorders. This article aims to highlight the relevance of anti-neurochondrin in autoimmune epilepsy.MethodsThis is a case presentation and literature review of autoimmune epilepsy associated with anti-neurochondrin antibody.Case presentationA 26-year-old African American right-handed man with a history of Sjogren’s syndrome presented with near constant, rhythmic left-sided facial twitching movements, and one episode of generalized tonic clonic seizure. Magnetic resonance imaging (MRI) of the brain revealed borderline low volume right hippocampus. Cerebrospinal fluid (CSF) studies yielded elevated protein and mild lymphocytic pleocytosis. Antibody Prevalence in Epilepsy 2 (APE2) score was 6, and autoimmune workup was initiated. Anti-neurochondrin antibody returned positive in the CSF autoimmune encephalitis panel with a titer of 1:512 (Mayo Clinic TEST ID: ENC2). Seizures remained refractory to anti-seizure medications including divalproex, lacosamide, and oxcarbazepine. Immunotherapy with methylprednisolone and immunoglobulin improved his epileptic seizures.ConclusionThis is the first reported case of refractory autoimmune epilepsy with positive CSF anti-neurochondrin antibody. This study contributes to the body of evidence supporting the role of neurochondrin antibody in epilepsy. Considering autoimmune testing in individuals with seizures having APE2 score > 4 can aid in timely diagnosis of immune-mediated epilepsy and initiation of immunotherapy, which can result in favorable clinical outcomes. Diagnosis of autoimmune epilepsy, in most cases, is based on clinical characteristics, MRI results, and CSF findings. In addition to the traditional antibody panel for autoimmune encephalitis, some novel antibodies, such as anti-neurochondrin, should also be considered.

The nearly neutral theory of molecular evolution posits variation among species in the effectiveness of selection. In an idealized model, the census population size determines both this minimum magnitude of the selection coefficient required for deleterious variants to be reliably purged, and the amount of neutral diversity. Empirically, an ‘effective population size’ is often estimated from the amount of putatively neutral genetic diversity and is assumed to also capture a species’ effectiveness of selection. A potentially more direct measure of the effectiveness of selection is the degree to which selection maintains preferred codons. However, past metrics that compare codon bias across species are confounded by among-species variation in %GC content and/or amino acid composition. Here, we propose a new Codon Adaptation Index of Species (CAIS), based on Kullback–Leibler divergence, that corrects for both confounders. We demonstrate the use of CAIS correlations, as well as the Effective Number of Codons, to show that the protein domains of more highly adapted vertebrate species evolve higher intrinsic structural disorder.

Background/objectives: Prior work regarding post-activation performance enhancement (PAPE) has shown that various resistance training methods and conditioning activities may induce a PAPE effect such as free weight resistance, accommodating resistance or isoinertial resistance. However, the accommodating resistance and other types of resistance have rarely been directly compared. Thus, the aim of this study was to compare the effects of two different conditioning activities (CA) - a trap bar deadlift with (FW + AR condition) or without (FW condition) accommodating resistance - on subsequent squat jump (SJ) performance after a short rest interval of 90s. Methods: The study had a cross-over design and fifteen strength trained males (mean age: 22.9 ± 2.1 years; mean relative strength level 2.01 ± 0.27 kg/body mass) participated in one familiarization, two experimental and one control session (CNTR condition). Abstract Following SARS‐CoV‐2 infection, some patients develop lingering neurologic symptoms of post‐acute sequelae of COVID‐19 (PASC) that commonly include fatigue and “brain fog.” PASC symptoms are also linked with reduced growth hormone (GH) secretion, but GH treatment has not been tested to relieve symptoms. We enrolled 13 adults with neurologic PASC symptoms and peak stimulated GH secretion less than 10 ng/mL (glucagon stimulation) in a pilot study to receive 9 months of daily GH injections and an additional 3 months of off‐treatment assessment. We compared peak stimulated GH secretion at baseline and 12 months and assessed measures of cognition, metabolism, body composition, and physical performance over the first 6 months of treatment. Patient‐reported outcomes of fatigue, quality of life, sleep, and mood were recorded at baseline and compared with timepoints at 6, 9, and 12 months. GH treatment was associated with significantly improved scores for Brief Fatigue Inventory, Multidimensional Fatigue Symptom Inventory, Quality of Life Assessment of Growth Hormone Deficiency in Adults, Profile of Mood States, and Beck Depression Inventory‐II, with no significant change in Pittsburgh Sleep Quality Index. Six months of adjunct GH treatment was not associated with significant changes in cognition, body composition, resting energy expenditure, or physical performance. Peak stimulated GH secretion was not altered at 12 months following 9 months of GH treatment. GH treatment significantly improved neurologic symptoms in PASC patients but cognition, sleep, and physical performance were not significantly altered.

IntroductionPatients who suffer a traumatic brain injury (TBI) often experience chronic and sometimes debilitating sequelae. Recent reports have illustrated both acute and long-term dysbiosis of the gastrointestinal microbiome with significant alterations in composition and predicted functional consequences.MethodsWorking with participants from past research, metagenomic stability of the TBI- associated fecal microbiome (FMB) was evaluated by custom qPCR array comparing a fecal sample from 2015 to one collected in 2020. Metatranscriptomics identified differently expressed bacterial genes and biochemical pathways in the TBI FMB. Microbiota that contributed the largest RNA amounts identified a set of core bacteria most responsible for functional consequences of the TBI FMB.ResultsA remarkably stable FMB metagenome with significant similarity (two-tail Spearman nonparametric correlation p < 0.001) was observed between 2015 and 2020 fecal samples from subjects with TBI. Comparing the 2020 TBI FMB metagenome to FMBs from healthy controls confirmed and extended the dysbiotic genera and species. Abundance differences between average TBI and healthy FMBs revealed Bacteroides caccae, B. uniformis, Blautia spp., Collinsella spp., Dialister spp., and Ordoribacter spp. were significantly different. Functionally, the Parabacteroides genus contributed the highest percentage of RNA sequences in control FMBs followed by the Bacteroides genus as the second highest contributor. In the TBI FMB, the Corynebacterium genus contributed the most RNA followed by the Alistipes genus. Corynebacterium and Pseudomonas were distinct in the top 10 contributing genera in the TBI FMB while Parabacteroides and Ruminococcus were unique to the top 10 in controls. Comparing RNA profiles, TBI samples had ∼1.5 fold more expressed genes with almost 700 differently expressed genes (DEGs) mapped to over 100 bacterial species. Bioinformatic analysis associated DEGs with pathways led identifying 311 functions in the average TBI FMB profile and 264 in the controls. By average profile comparison, 30 pathways had significantly different abundance (p < 0.05, t-test) or were detected in >80% of the samples in only one of the cohorts (binary distinction).DiscussionFunctional differences between TBI and healthy control FMBs included amino acid metabolism, energy and carbon source usage, fatty acid metabolism, bacterial cell wall component production and nucleic acid synthesis and processing pathways. Together these data shed light on the functional consequences of the dysbiotic TBI FMB decades after injury.

Abstract Introduction: Operationalizing multi-site Community Engagement (CE) Studios to inform a research program is valuable for researchers. We describe the process and outcomes of hosting three CE Studios with Community Experts aged 65 years or older with chronic conditions and care partners of older adults. Experts gave feedback about processes for testing the feasibility, efficacy, effectiveness, and implementation of audio recording clinic visits and sharing recordings with patients who have multimorbidity and their care partners. Methods: The CE Cores of the Clinical and Translational Science Awards Programs at three academic health science centers created a joint CE Studio guide. Studios were conducted iteratively by site. Following receipt of the final reports, responses were compared to find themes, similarities, and differences on four topics in addition to overall commentary: Recruitment and Retention, Study Protocol, Study Reminders and Frequency, and Recording Technology. Results: Eighteen older adults and care partners in three states provided valuable feedback to inform multi-site trials. Feedback influenced multiple aspects of trials in process or subsequently funded. Experts provided critique on the wording of study invitations, information sheets, and reminders to engage in study procedures. Experts were concerned for participants being disappointed by randomization to a control arm and advised how investigators should prepare to address that. Conclusions: Multi-site CE Studios should be consecutive, so each team can learn from the previous teams. Using the CES Toolkit ensures that final reports were easily comparable and utilized to develop a research program that now includes three federally funded clinical trials.

BackgroundFollowing traumatic brain injury (TBI) some patients develop lingering comorbid symptoms of fatigue and cognitive impairment. The mild cognitive impairment self-reported by patients is often not detected with neurocognitive tests making it difficult to determine how common and severe these symptoms are in individuals with a history of TBI. This study was conducted to determine the relative prevalence of fatigue and cognitive impairment in individuals with a history of TBI.MethodsThe Fatigue and Altered Cognition Scale (FACs) digital questionnaire was used to assess self-reported fatigue and cognitive impairment. Adults aged 18-70 were digitally recruited for the online anonymous study. Eligible participants provided online consent, demographic data, information about lifetime TBI history, and completed the 20 item FACs questionnaire.ResultsA total of 519 qualifying participants completed the online digital study which included 204 participants with a history of TBI of varied cause and severity and 315 with no history of TBI. FACs Total Score was significantly higher in the TBI group (57.7 ± 22.2) compared to non-TBI (39.5 ± 23.9; pConclusionsAdults with a history of even mild TBI report significantly greater fatigue and cognitive impairment than those with no history of TBI, and symptoms are more profound with greater TBI severity.

Abstract Background Post-activation potentiation performance (PAPE) is a physiological phenomenon that has been studied numerously but the researchers are still seeking for the optimal application methods. The accommodating resistance was found to be an effective training method to acutely enhance subsequent explosive performance. The purpose of this study was to evaluate the effects of performing a trap bar deadlift with accommodating resistance on squat jump (SJ) performance with different rest intervals (90, 120, 150s). Ensuring that cultural sensitivity in teacher education programs is addressed will enhance new teachers' confidence and effectiveness in the classroom. As the result of the COVID-19 pandemic, teacher candidates have experienced a non-contact environment at every stage of their professional education. Specifically, the pandemic impacted the availability and quality of their field experiences, including student teaching, internship/practicum, clinical work in culturally responsive teaching, and study or teaching abroad. Moreover, the shift to online instruction in most school districts required teachers to reconsider their approach to engagement in classroom activities. This article describes how the authors, in response to the limitations imposed by the COVID-19 pandemic, implemented a unique program with the goal of increasing aspiring teachers' educational cultural competence. The Online Global Outreach (OGO) program is a field experience that provides a cost-effective and time-efficient way to build pre-service teachers' educational cultural competence by giving them the opportunity to teach virtually in a foreign school when in-person interaction is not feasible. The goals of the program were to increase the teacher candidate's self-awareness and self-confidence and to allow them to gain cultural capital, in terms of culture-related knowledge, social skills, and behavioral norms, that they could apply to their future classrooms.

Monitoring athletes’ well-being during the competitive season is a common practice in team sports, and countermovement jump (CMJ) is often introduced to monitor neuromuscular fatigue of the players. In this pilot observational study, we aimed to evaluate changes in jump performance in professional volleyball players during their two competitive seasons after introducing different executions of the jump test. Different execution of the flight phase of the jumps was introduced as a differentiating factor. Nine professional volleyball players (age: 23.3 ± 1.9 years; volleyball training experience: 10.4 ± 1.9 years; body height: 192.3 ± 8.3) performed CMJ measurements 1–2 times a week on a contact mat during two competitive seasons, and a total number of 1008 jumps (540 in season 1 and 468 in season 2) was collected. Players’ CMJ height significantly decreased (p < 0.05) in season 2 by approximately 4%, and standard deviation of the data reduced by approximately 30%. Our study confirms that the testing procedures need to be rigorous as different execution of the jump may be associated with changes in CMJ height. Execution of the jump should not be interchangeable using the same measurement device, and the technique introduced in season 2 seems to be more appropriate for this measurement device.

This study examines whether creating a mathematics-based digital story increases pre-service teachers' understanding of the problem solving process, the relationship among visual, auditory and verbal representation in critical thinking/problem solving in mathematics. Survey results indicated that pre-service teachers were unfamiliar with the uses of technology in the teaching process. However, as they worked through the writing/problem solving procedures and constructed digital stories, they became more aware of the advantages of using this multimodal product, called Math-eos, to teach mathematical concepts to elementary/middle school students. The pre-service teachers perceived that Math-eos offered a "cool" new tool for their own learning and teaching and for integrating digital technology in mathematics education to increase student engagement, motivation and acquisition of content knowledge.

Digital contact tracing and notification were initially hailed as promising strategies to combat SARS-CoV-2; however, in most jurisdictions, they did not live up to their promise. To avert a given transmission event, both parties must have adopted the technology, it must detect the contact, the primary case must be promptly diagnosed, notifications must be triggered, and the secondary case must change their behavior to avoid the focal tertiary transmission event. If we approximate these as independent events, achieving a 26% reduction in the effective reproduction number Rt would require an 80% success rate at each of these 6 points of failure. Here, we review the 6 failure rates experienced by a variety of digital contact tracing and contact notification schemes, including Singapore’s TraceTogether, India’s Aarogya Setu, and leading implementations of the Google Apple Exposure Notification system. This leads to a number of recommendations, for example, that the narrative be framed in terms of user autonomy rather than user privacy, and that tracing/notification apps be multifunctional and integrated with testing, manual contact tracing, and the gathering of critical scientific data.

Abstract N,N'‐dimethyl‐4,4'bipyridinium dichloride (Paraquat) is a potent herbicide used widely in agriculture. We report the effects of an ingestion of paraquat by a 28 year old male with cystic fibrosis and the diagnostic and management challenges this posed in both the acute and longer term setting. We describe the effects of direct paraquat toxicity on the lung tissue secondary to aspiration and review the long‐term sequelae of paraquat, namely osteonecrosis. Our case is the first to describe osteonecrosis of the knee in the context of paraquat toxicity. Survival following ingestion remains poor with a high associated mortality. However, timely treatment with NAC and immunosuppression may impact on survival. In those patients who do survive the acute phase post ingestion, follow‐up over years may be required to detect the long‐term effects of paraquat on bone health.

IntroductionParenteral nutrition (PN) is widely used in palliative care (PC), but there is limited evidence to support its use at the end of life (EOL). This aim of this was to investigate the relationship between routine laboratory parameters and survival in patients receiving PN, and to develop a decision tree model to support clinicians decide whether to start or forgo PN.MethodsThe laboratory parameters of 113 patients with advanced diseases who were admitted to a specialized palliative care unit (PCU) were analyzed at two points in time: T0 = before PN, T1 = two weeks after initiation of PN. Univariate Mann-Whitney U-tests and multivariate linear regression models, as well as a decision tree analysis were computed; all in relation to survival time.ResultsThe final regression model was significant with p = 0.001 (adjusted R2 = 0.15) and included two predictors for survival time after PN initiation: the CRP/albumin ratio and urea at T1 (ps = 0.019). Decision tree analysis revealed three important predictors for classification of survival time after PN initiation: CRP, urea, and LDH (all at T0).DiscussionThe decision tree model may help to identify patients likely to benefit from PN, thus supporting the clinical decision whether or not to start PN.

This study aimed to comprehensively evaluate the effects of intermittent hypoxic training (IHT) on anaerobic performance in young, untrained men. Young men (n = 48) were randomly divided into two training groups and a control group. The training groups performed the same submaximal interval training (three times a week for 4 weeks) in normoxia (200 m asl) or in hypoxia (IHT) (FIO2 = 14.4%). The workloads for the interval training corresponded to the intensity of the ventilatory thresholds determined in a graded test. Participants performed a supramaximal all-out sprint test in normoxia twice: before and after the training. Significant improvement in both absolute peak power (p < 0.001; ES = 0.34) and relative peak power (p < 0.001; ES = 0.54) was noted after IHT. Similar changes were not observed either after training in normoxia (p = 0.14 and p = 0.26, for absolute and relative peak power, respectively) or in the control group (p = 0.34 and p = 0.51, for absolute and relative peak power, respectively). Compared to baseline, there were no significant changes in the absolute and relative mean power of either group after training. Intermittent hypoxic training in young, untrained men can be effective in improving their peak power, but does not significantly affect their mean power.

Abstract The adoption of a bioenergy crop is affected by various factors, including but not limited to the characteristics of farmers, farm economics, market forces, and physical environment. This study develops a spatially explicit agent‐based model for ascertaining the adoption rate of carinata (Brassica carinata) among the farmers in the Little River Experimental Watershed located in the southern state of Georgia in the United States. Each farmer's adoption behavior is modeled using the profitability difference between traditional crop rotations (with and without carinata at different contract prices), the adoption rate of neighboring farmers, and their land allocation decisions from managing a risky portfolio of enterprises. Carinata production in the winter season once every 3 years has no conflict with the most profitable and popular traditional row crop rotations, such as cotton‐cotton‐cotton and cotton‐cotton‐peanut, to a larger extent. The results show that 28% and 85% of farmers in the watershed will adopt carinata after 33 years at a contract price of $13/bushel (bu) under two different assumptions of low (2.5%) and high (5%) initial neighborhood adoption rates. The proportions of land allocated to carinata to the total farmland under field crops are 38% and 85% after 33 years under the same low and high neighborhood adoption rates, respectively. Our results suggest that fixing the appropriate contract price of carinata will bring additional profits to farmers without any significant foreseeable agronomic risks, thereby increasing the adoption rate of carinata at a regional level.

Aim: The topic of global health affects us all. In order to spread the word about this important medical field, a bilingual brochure containing medical comics on the subject was published at the Medical University of Vienna.Method: Medical comics are well established internationally, have a long history in the communication of medical information, and – under the umbrella of graphic medicine – are both popular and widely used. In the context of the exhibition project art-action-attitude/body, 6 panels were transformed into a 12-page brochure on the topic of global health in cooperation with an artist and a graphic designer.Results: The brochure was made available for free to visitors of the exhibition, published online, handed out at various fairs by the artist, and presented at an international conference.Summary: The collaboration with an artist and a graphic designer (Renate Mowlam, Bettina Jarosch) gave rise to a high-quality product that explains the subject matter clearly and in a way that is accessible to all, with the written word supported by images. In the brochure, 46 pictures shed light on the topics “health as a human right”, “global health” (definition and examples), and “one health” (definition and examples). This enabled readers to quickly become informed about this complex subject, to contemplate it, and to reflect on their own behavior.
