Abstract

BACKGROUND: Venous thromboembolism (VTE), encompassing both deep vein thrombosis (DVT) and pulmonary embolism (PE), is a leading cause of morbidity and mortality worldwide. METHODS: GARFIELD-VTE is a prospective, non-interventional observational study of real-world treatment practices. We aimed to capture the 36-month clinical outcomes of 10,679 patients with objectively confirmed VTE enrolled between May 2014 and January 2017 from 415 sites in 28 countries. FINDINGS: A total of 6582 (61.6 %) patients had DVT alone, 4097 (38.4 %) had PE ± DVT. At baseline, 98.1 % of patients received anticoagulation (AC) with or without other modalities of therapy. The proportion of patients on AC therapy decreased over time: 87.6 % at 3 months, 73.0 % at 6 months, 54.2 % at 12 months and 42.0 % at 36 months. At 12-months follow-up, the incidences (95 % confidence interval [CI]) of all-cause mortality, recurrent VTE and major bleeding were 6.5 (7.0-8.1), 5.4 (4.9-5.9) and 2.7 (2.4-3.0) per 100 person-years, respectively. At 36-months, these decreased to 4.4 (4.2-4.7), 3.5 (3.2-2.7) and 1.4 (1.3-1.6) per 100 person-years, respectively. Over 36-months, the rate of all-cause mortality and major bleeds were highest in patients treated with parenteral therapy (PAR) versus oral anti-coagulants (OAC) and no OAC, and the rate of recurrent VTE was highest in patients on no OAC versus those on PAR and OAC. The most frequent cause of death after 36-month follow-up was cancer (n = 565, 48.6 %), followed by cardiac (n = 94, 8.1 %), and VTE (n = 38, 3.2 %). Most recurrent VTE events were DVT alone (n = 564, 63.3 %), with the remainder PE, (n = 236, 27.3 %), or PE in combination with DVT (n = 63, 7.3 %). INTERPRETATION: GARFIELD-VTE provides a global perspective of anticoagulation patterns and highlights the accumulation of events within the first 12 months after diagnosis. Abstract

The authors report a 71-year-old male with descending thoracic aortic aneurysm and multiple risk factors (aortoiliac occlusive disease, obesity, ascending aorta dilatation, and history of left ventriculoperitoneal shunt for hydrocephalus) who was treated with thoracic endovascular aortic repair (TEVAR) via left common carotid artery (LCCA) access and left axillary-carotid artery (Ax-CA) bypass; this approach shortened the LCCA clamp time during the procedure. The patient was discharged without any complications. TEVAR via LCCA access with left Ax-CA bypass is a useful and safe procedure for patients in whom conventional femoral artery access is not feasible.

Abstract

Background Parenteral anticoagulants and vitamin K antagonists (VKAs) have constituted the cornerstone of venous thromboembolism (VTE) treatment. Meanwhile, direct oral anticoagulants (DOACs) provide physicians with an alternative. The Global Anticoagulant Registry in the FIELD (GARFIELD)-VTE observes real-world treatment practices. Objectives Describe initial anticoagulation (AC) treatment patterns in VTE patients who received parenteral AC, VKAs, and/or DOACs within ±30 days of diagnosis. Methods VTE patients were categorized into parenteral AC only, parenteral AC with transition to VKA, VKA only, parenteral AC with transition to DOAC, and DOAC only. Results A total of 9647 patients were initiated on AC treatment alone. 4781 (49.6%) patients received DOACs ± parenteral ACs; 3187 (33.0%), VKA ± parenteral ACs; and 1679 (17.4%) parenteral ACs alone. Rivaroxaban was the most frequently used DOAC (79.4%). DOACs were more frequently used in North America/Australia (58.1%), Europe (52.2%), and Asia (47.6%) than in Latin America (29.7%) and the Middle East/South Africa (32.5%). In patients with suspected VTE, most received parenteral AC monotherapy (67.7%). Patients with deep vein thrombosis were more likely to receive DOACs alone than those with pulmonary embolism with or without deep vein thrombosis (36.2% vs 25.9%). Active cancer patients received parenteral AC alone (58.9%), with 25.5% receiving DOAC ± parenteral AC and 12.8% parenteral AC and VKA. A total of 46.5% of pregnant patients received parenteral AC monotherapy, 34.0% were treated with VKA ± parenteral AC, and 19.5% received a DOAC (± parenteral AC). Conclusion AC treatment patterns vary by patient population, geographic region and site of VTE. Guidelines for AC therapy are not always adhered to.

Abstract

In the present research, we experimentally verified the partial-fill effect in a multicycle pulse detonation rocket engine. The intermittent thrust of a pulse detonation rocket engine was measured by using a spring-damper mechanism that smoothed this intermittent thrust in the time direction. The intermittent mass flow rates were assessed by gas cylinder pressure or mass difference measurement. The maximum specific impulse was 305 ± 9 s at an ethylene and oxygen propellant fill fraction of 0.130 ± 0.004. When the fill fraction was greater than 0.130, the specific impulse was increased as the partial-fill fraction was decreased. When the fill fraction was less than 0.130, the specific impulse was sharply decreased as the partial-fill fraction was decreased. This decrease was due to diffusion between propellant and purge gases and the short length of the transition from deflagration to detonation. The multicycle pulse detonation rocket engine had a partial-fill effect that may have been mainly due to the suctioned air and was consistent with the single-cycle partial-fill model of Endo et al. We have determined that hydrogenated cationic Rh(I)/bisphosphane complexes are highly active catalysts for the isomerization of secondary propargylic alcohols to α,β-enones. A kinetic resolution of secondary propargylic alcohols proceeded with moderate selectivity with [Rh((R)-BINAP)]OTf as a catalyst. Mechanistic studies revealed that the isomerization proceeds through intramolecular 1,3- and 1,2-hydrogen migration pathways. The isomerization of propargylic diol derivatives was also investigated, which revealed that 1,4-diketones, furans, and α,β-enones were obtained from 2-butyn-1,4-diol, 1-methoxy-2-butyn-4-ol, and 1-acetoxy-2-butyn-4-ol derivatives, respectively. Furthermore, chemoselectivity of the isomerization of an acetylenic diol was investigated, and preferential oxidation of a propargylic hydroxy group was observed.(© Wiley-VCH Verlag GmbH & Co. A cationic rhodium(I)/( S)-Tol-BINAP complex was employed to catalyze an enantioselective intramolecular [2+2+2] cycloaddition of a TRANS enediyne leading to a C 2 -symmetric tricyclic dimethyl cyclohexadienedicarboxylate in 95% yield with 59% ee. A cationic rhodium(I)/( R)-H 8 -BINAP complex was employed to catalyze an intermolecular [2+2+2] cycloaddition of 1,6-diynes with dimethyl fumarate leading to C 2 -symmetric bicyclic dimethyl -cyclohexadienedicarboxylates in 35-96% yields with ee values of 82-98%.

We established that a cationic rhodium(I)/dppf or dppb complex catalyzes a regio- and diastereoselective intermolecular [4+2] carbocyclization of 5-trimethylsilyl-4-pentynals with electron-deficient alkenes leading to cyclohexanones. We also established that a cationic rhodium(I)/(R,R)-Walphos complex catalyzes a regio- and enantioselective intermolecular [4+2] carbocyclization of 5-substituted 4-pentynals and 2-alkynylbenzaldehydes with N,N-dialkylacrylamides leading to enantio-enriched cyclohexanones and tetralones, respectively. A single olefin isomer was produced in every carbocyclization. Regioselectivities of the alkene insertion depend on the alkenes used. Mechanistic study suggested that a key intermediate in this intermolecular [4+2] carbocyclization is a five-membered acylrhodium intermediate, formed by cis addition of the rhodium hydride to the metal-bound alkyne. Abstract

A highly regioselective intermolecular cyclotrimerization of terminal alkynes has been developed based on the use of the cationic rhodium(I)/DTBM-Segphos complex. This method can be applied to a variety of terminal alkynes to provide 1,2,4-trisubstituted benzenes in high yield and with high regioselectivity. A chemo- and regioselective intermolecular crossed-cyclotrimerization of dialkyl acetylenedicarboxylates with a variety of terminal alkynes has also been developed based on the use of the cationic rhodium(I)/H8-BINAP complex, furnishing 3,6-disubstituted phthalates in high yields. It constitutes a highly efficient new method for intermolecular crossed-cyclotrimerization of two different monoynes in terms of catalytic activity, chemo- and regioselectivity, scope of substrates, and ease of operation. The versatility of this new crossed-alkyne cyclotrimerization procedure is demonstrated through its application to one-step synthesis of a [6]metacyclophane and [7]-[12]paracyclophanes from the corresponding terminal alpha,omega-diynes. Mechanistic studies have revealed that the chemo- and regioselectivity of this crossed-alkyne cyclotrimerization are determined by the preferential formation of a specific rhodium metallacycle derived from a terminal alkyne and a dialkyl acetylenedicarboxylate.

A broad range of aliphatic, alicyclic, and aromatic ketones and aldehydes underwent smooth reduction with a combination of common, inexpensive laboratory reagents, sodium borohydride and chromatographic neutral alumina, in hexane to afford the corresponding alcohols in a selective and high-yielding manner under mild and neutral conditions. The NaBH4-based biphasic system can also successfully be used for the reduction of unsaturated substrates as well as steroidal and highly hindered ketones.

Abstract

A study of the electro-optie response of a surface-stabilized ferroelectric liquid crystal device has been made for the high drive voltage region. We find an inverse switching memory effect in the polarity to be built up after the trailing edges of the applied voltage pulses. The inverse switching memory effect is observed by applying higher and longer voltage pulses. The effect is considered to be produced by the internal electric field produced by ions, and the internal electric field strength depends on the thickness of the orientational film.

Because of the steady increase in car population and the mounting of enviromental concern, lead consumption ratio for lead-acid batteries is tending to gradually increase. As a result of this fact, itis important to establish new reliable collection, treatment, and recycling system of used lead-acidbatteries that would not to be effected by the fluctuation of lead prices and the exchange rate. Andlead-acid batteries made of lead, dilute sulfuric acid and polypropylene have to be treated withoutcausing any enviromental problem.In Japan, in October 1994, the battery manufacturing industry started the a “lead recyling system”.As maintenance-free batteries using no Pb-Sb alloy as grid arewidely used, demand for secondary Pb-Sb alloy has decreased.To maintain the usage amount of secondary lead as lead-acid battery, primary smelters were asked toparticipate in this recycle system.Afterthe introduction of this system, recyclig ratio for lead-acid batteries has risen to about 90%contributing to stabilized recycling ratios.

A solid–liquid biphasic system (dichloromethane and chromatographic neutral alumina) has been tested for the aromatic chlorination of various alkyl aryl ethers using a reagent combination of sodium chlorite and manganese(III) acetylacetonate catalyst. Efficient incorporation of a chlorine atom into the benzene ring with high para-selectivity results. This catalytic system is also applicable to the regiocontrolled chlorination of polyether substrates.

Abstract

Ostial atresia of the left main coronary artery (LMCA) in children without any primary disease is extremely rare. We present here a case of occlusion of the LMCA in a 9-year-old girl. Myocardial scintigraphy showed poor perfusion in both domains of the left anterior descending artery (LAD) and left circumflex artery (LCx). Coronary artery graphy (CAG) showed complete ostial atresia of the LMCA and retrograde perfusion from the thin collateral arteries into the LAD. We performed angioplasty using an autologous pericardium onlay patch. Her postoperative course was unremarkable. Postoperative CAG showed vanishing collateral arteries, confirming anterograde flow through the LAD and LCx, and myocardial scintigraphy showed improvement in perfusion.
