BACKGROUND: Gluten intolerance is a systemic process of autoimmune nature; it develops in genetically predisposed subjects with gluten ingestion. The only treatment for celiac disease (CD) is a lifelong strict gluten-free diet (GFD). This study was designed to evaluate adherence to a GFD, risk of an eating disorder, and nutritional status in adult CD patients undergoing different interventions. METHODS: A total of 28 Spanish women, aged 40 years or more, took part in a randomized controlled trial. Each group received a different intervention: group 1, gluten-free nutrition plan + exercise (GFD + E); group 2, gluten-free nutrition plan (GFD); group 3, celiac controls (NO-GFD); and group 4, non-celiac controls (CONTROL). The training was prescribed by a sport scientist. It was based on resistance training with elastic bands; beforehand a warm-up was performed and the resistance was increased progressively. The variables studied were adherence to the GFD, risk of eating disorders, blood values, and body composition. RESULTS: Celiac women with personalized nutritional planning presented greater adherence to a gluten-free diet (p < 0.001). Regarding leukocytes, significant differences were observed between the GFD and control groups (p = 0.004). Perimeters and folds did not decrease significantly. CONCLUSION: Women with celiac disease who follow an adapted and personalized diet have a better adherence to a GFD compared to those who follow a non-professional diet, and therefore have a better immune system status (blood leukocytes).

Celiac disease (CD) is an autoimmune disease characterized by gluten-induced intestinal inflammation. Dietary restrictions and symptoms may have a significant impact on the patient's quality of life, body composition (BC), and strength. This study was designed to assess the impact of an isocaloric gluten free diet and resistance exercise in women. A total of 28 Spanish women, aged 40 years old or more, took part in a randomized controlled trial. Each group received a different intervention: group 1, gluten-free nutrition plan + exercise (GFD + E); group 2, gluten-free nutrition plan (GFD); group 3, celiac controls (NO-GFD); and group 4, non-celiac controls (CONTROL). The variables studied were quality of life, BC and isometric hand strength. After 12 weeks of intervention, celiac women that followed a gluten-free diet and exercise showed higher scores on the psychological health scale than celiac women without intervention. The women in group 1 were the only ones who presented improvements in BC variables; fat mass, BMI, and fat-free mass. Negative correlations were found between the perception of quality of life and age, however a positive correlation between quality of life and isometric strength test results was found. In addition to a gluten-free diet, resistance training is essential to improve BC, strength, and gastrointestinal symptoms.

We present the first successful synthesis of monodisperse carbon nanodots (CNDs) with tunable photoluminescence (PL) carried out by laser pyrolysis of two common volatile organic precursors such as toluene and pyridine. Remarkably, the initial chemical composition of the precursor determines the formation of undoped or N-doped CNDs and their corresponding absorption response in the visible range (expanded for the latter). We demonstrate the control and versatility of this synthesis method to tune the final outcome and its potential to explore a great number of potential solvent candidates. Furthermore, we have successfully exploited these CNDs (both undoped and N-doped) as effective sensitizers of TiO2 nanoparticles in the visible-light driven photo-degradation of a cationic dye selected as model organic pollutant.

The possibility of achieving sophisticated actions in complex biological environments using gated nanoparticles is an exciting prospect with much potential. We herein describe new gated mesoporous silica nanoparticles (MSN) loaded with an anticoagulant drug and capped with a peptide containing a thrombin-specific cleavage site. When the coagulation cascade was triggered, active thrombin degraded the capping peptidic sequence and induced the-release of anticoagulant drugs to delay the clotting process. The thrombin-dependent response was assessed and a significant increase in coagulation time in plasma from 2.6 min to 5 min was found. This work broadens the application of gated silica nanoparticles and demonstrates their ability to act as controllers in a complex scenario such as hemostasis.

Amaç: Bu çalışmanın amacı trigeminal nevralji tedavisinde radyofrekans termokoagülasyon (RFTC) kullanımı sırasında ganglion trigeminalenin önemli komşulukları nedeniyle, olası komplikasyonların önlenebilmesi için anatomik yapı ve belirleyicilerin ayrıntılı olarak ortaya konulmasıdır. Materyal-Metot: Afyon Kocatepe Üniversitesi Tıp Fakültesi Nöroşirurji polikliniğine 2008 yılı içerisinde müracaat eden trigeminal nevralji tanılı 5 olguya öncelikle medikal tedavi uygulanmış ve tüm olgular arasında tedaviye dirençli olan bir olgu, anatomik belirteçlerden yararlanılarak RFTC yardımı ile ganglion lezyonlanması yoluyla tedavi edilmiştir. Bulgular: Medikal tedaviye dirençli olan olgunun RFTC ile ganglion lezyonlama işlemi sonrasında ilk 3 aylık periyodda dirençli ağrı yakınması düzelmiş ve kronik ilaç kullanımı yakınmaların başlangıcından bu yana en az düzeye inmiştir. Sonuç: Trigeminal sinir ganglionunun RFTC ile lezyonlanma uygulaması medikal tedaviye dirençli olgularda erken dönemde etkin bir tedavi yoludur. Uygulama sırasında çevre yapıların hasarlanma olasılığı nedeniyle anatomik belirteçler yoluyla işlemin gerçekleştirilmesi büyük önem taşır

Introduction. The International Headache Society (IHS) has published the third edition of the International Classification of Headache Disorders (ICHD-III beta), the most commonly used guide to diagnosing headaches in the world. Aims. To review the recent additions to the guide, to explain the new entities that appear in it and to compare the conditions that have had their criteria further clarified against the criteria in the previous edition. Development. We have recorded a large number of clarifications in the criteria in practically all the headaches and neuralgias in the classification, but the conditions that have undergone the most significant clarifications are chronic migraine, primary headache associated with sexual activity, short-lasting unilateral neuralgiform headache attacks, new daily persistent headache, medication-overuse headache, syndrome of transient headache and neurological deficits with cerebrospinal fluid lymphocytosis. The most notable new entities that have been incorporated are external-compression headache, cold-stimulus headache, nummular headache, headache attributed to aeroplane travel and headache attributed to autonomic dysreflexia. Another point to be highlighted is the case of the new headaches (still not considered entities in their own right) included in the appendix, some of the most noteworthy being epicrania fugax, vestibular migraine and infantile colic. Conclusions. The IHS recommends no longer using the previous classification and changing over to the new classification (ICHD-III beta) in healthcare, teaching and research, in addition to making this new guide as widely known as possible.

Objective: To describe the characteristics of the patients case-mix admitted to ICUs due to medical and surgical disease, and to compare both groups. Design: Analysis of data covering the period 2006-2011 in the ENVIN-HELICS registry. An observational, prospective, multicenter and voluntary participation study. Setting: A total of 188 Spanish ICUs. Patients: All patients admitted for more than 24 hours. Main variables: Demographic data, cause of admission, severity scores, length of stay, mortality. Results: A total of 138,999 patients were analyzed. Of these, 65,467 (47.1%) were admitted due to a non-coronary medical cause, 27,785 (20,0%) due to coronary-related illness, 28,044 (20,2%) after elective surgery and 17,613 (12.7%) after urgent surgery. Use of devices, nosocomial infections and isolation of multirresistant organisms were more prevalent in urgent surgery patients. Longer length of stay (median 5 days; interquartile range 2-11) as well as higher severity scale values (APACHE II and SAPS II) corresponded to this same group of patients. Mortality was higher in non-coronay medical patients. On categorizing the patients according to the APACHE)! score,mortality was seen to be higher in urgent surgery cases than in elective surgery patients in all groups. The purpose of the study is to evaluate neurovascular anatomy of the deltoid flap based on the posterior subcutaneous deltoid artery (PSDA). Bilateral axillary artery dissections of the thirty-four shoulders of 17 formalin-fixed cadavers were performed (15 male, 2 female; age range 40 to 82 years) under the 4x loupe magnification. During the dissection of each region, the PSDA was evaluated with respect to the origin, the branches, course and anatomical relations with neighbouring structures. The PSDA was present in all cases. The PSDA was single in 26 (76.5%) cases, double in 2 (5.9%) cases and had early bifurcation in 6 (17.6%) cases. The mean length of the lateral cutaneous brachial nerve was 6.0 mm range from 49.0 mm to 83.9 mm. The mean distance between the piercing point of the PSDA and the acromion was 74.2 mm range from 51.0 mm to 96.3 mm. The pedicle bifurcated before reaching the superficial fascia in 6 cases (17.6%). An anatomic study of the posterior subcutaneous deltoid artery achieves reliable quantitative anatomic data and would be very helpful for utilizing deltoid flap. The quantitative and detailed anatomic information provided from the study may be of guidance to surgeons for safe operating period. El objetivo del presente estudio fue evaluar la anatomía neurovascular del colgajo deltoideo basado en la arteria subcutánea deltoidea posterior (ASDP). Se realizaron disecciones axilares bilaterales de 34 hombros de 17 cadáveres (15 hombres y 2 mujeres con un rango etareo entre 40-82 años) fijados en formalina con ampliación mediante una lupa 4X. Durante la disección de cada región, la ASDP fue evaluada en relación con su origen, ramas, curso y relaciones anatómicas con estructuras vecinas. La ASDP estuvo presente en todos los casos. Observamos una ASDP en 26 casos (76,5%), dos en 2 casos (5,9%) y la bifurcación temprana en 6 casos (17,6%). La longitud media del nervio cutáneo braquial lateral fue de 6,0 mm (intervalo de 49,0-83,9 mm). La distancia media entre el punto de entrada y el acromion de la ASDP fue 74,2 mm (rango de 51,0-96,3 mm). La bifurcación del pedículo se presentó antes de llegar a la fascia superficial en 6 de los casos (17,6 %). El estudio anatómico de la parte posterior de la arteria deltoidea subcutánea entrega datos anatómicos, cuantitativos, fiables para la utilización del colgajo deltoideo. La información detallada y cuantitativa proporcionada en este estudio puede servir de orientación a los cirujanos para un abordaje y período quirúrgico seguro.
