Abstract

Abstract Introduction Kidney transplantation is the preferred treatment for end-stage renal disease (ESRD), offering a superior quality of life and extended survival compared to other renal replacement therapies. As the number of ESRD patients grows, so does the demand for organ transplants. The prevalence of ESRD is anticipated to escalate further due to the rising rates of diabetes mellitus (DM), hypertension (HTN), and obesity. Organ donation, particularly from living donors, remains the main source of transplants in the region, despite the notable underutilization of potential deceased donors’ organs. The objective of this research is to assess the level of knowledge, attitudes, and willingness to donate kidneys among the general population, a pivotal step in addressing the organ shortage crisis. Methods This cross-sectional study was conducted in the Aseer region of Saudi Arabia using a previously validated questionnaire. The questionnaire collected demographic data and insights into general attitudes, knowledge, and beliefs about organ donation. Logistic regression was used to identify predictors of knowledge and willingness to donate. Results The study involved 705 participants, predominantly young adults with a high level of education. Awareness of kidney donation was high, and knowledge about donation was broad, especially regarding religious permissibility and awareness of the donor registry. However, only 25% expressed willingness to donate their kidneys, and a 4% were already registered as donors. Furthermore, higher educational level was not associated with higher odds of knowledge or willingness to donate. Conclusion Despite the considerable awareness, actual donor registration rates were low, highlighting the necessity for targeted educational interventions and a deeper understanding of the cultural and socioeconomic barriers that exist.

Abstract

BackgroundThe progression of FLAIR white matter hyperintensities (WMHs) on MRI heralds vascular-mediated cognitive decline. Even before FLAIR WMH progression, adjacent normal appearing white matter (NAWM) already demonstrates microstructural deterioration on diffusion tensor imaging (DTI). We hypothesized that elevated DTI free water (FW) would precede FLAIR WMH progression, implicating interstitial fluid accumulation as a key pathological step in the progression of cerebral small vessel disease.MethodsParticipants at least 3 months after an ischemic stroke or TIA with WMH on MRI underwent serial brain MRIs every 3 months over the subsequent year. For each participant, the WMHs were automatically segmented, serial MRIs were aligned, and a region of WMH penumbra tissue at risk was defined by dilating lesions at any time point and subtracting baseline lesions. Penumbra voxels were classified as either stable or progressing to WMH if they were segmented as new lesions and demonstrated increasing FLAIR intensity over time. Aligned DTI images included FW and FW-corrected fractional anisotropy (FATissue) and mean diffusivity (MDTissue). Logistic regression and area under the receiver-operator characteristic curve (AUC) were used to test whether baseline DTI predicted voxel-wise classification of stable penumbra or progression to WMH while covarying for clinical risk factors.ResultsIn the included participants (n = 26, mean age 71 ± 9 years, 31% female), we detected a median annual voxel-wise WMH growth of 2.9 ± 2.6 ml. Each baseline DTI metric was associated with lesion progression in the penumbra, but FW had the greatest AUC of 0.732 (0.730 – 0.733) for predicting voxel-wise WMH progression pooled across participants.DiscussionBaseline increased interstitial fluid, estimated as FW on DTI, predicted the progression of NAWM to WMH over the following year. These results implicate the presence of FW in the pathogenesis of cerebral small vessel disease progression.

In this paper, concrete filled steel tubes columns (CFT) are investigated by using finite element program ANSYS 15.0. Analysis are done for four different shapes of columns (circular, square,hexagonal andoctagonal). Results of analytical solution (for circular and square) were compared with existing experimental data provided by [Alwash et al., 2013].Comparative resultsof failure load give 4% difference between experimental and ANSYS 15.0. Also, parametric studies have been carried out to investigate the effect of concrete filled steel tubes columns shapes (for hexagonal and octagonal) on load carrying capacity.Finally, a newformulaefor predicting the ultimate strength of CFT is proposedbased on experimental data of 148 CFT columns of different cross sections with side length ranging between 200 and 4000 mm. To check the validity of the proposed equation, the loads calculated from the designmethods (American Concrete Institute (ACI 318M-14), Eurocode (EC4),New Zealand Standard of concrete structures(NZS) and American Institute of Steel Construction (AISC)) are used to compare with it. The comparison shows least convergence percentageof the proposed equation.

Monitoring the changes in mortality patterns and levels requires studying mortality models and measures of lifespan length and variation. Measures of lifespan length and variation can be obtained from life tables or from mortality modes. In this paper, measures of lifespan length and variation are presented for mixture model introduced by Zanotto, L., V. Canudas-Romo, and S. Mazzuco. 2021. "A Mixture-Function Mortality Model: Illustration of the Evolution of Premature Mortality." European Journal of Population 37 (1): 1–27. This was applied to the case of Egypt for males and females in the period 1950–2021. The mixture model was able to fit Egypt's mortality pattern well for both males and females. The calculated measures of lifespan length and variation show that there is an increasing longevity and a declining variation of age at death distribution across time for both males and females. Abstract

Background: Diabetes and high blood pressure are among the chronic diseases that have recently increased alarmingly in all countries of the world. Both these health disorders associated with wrong practices such as unhealthy nutrition and neglect of exercise, in addition to constant exposure to psychological stress. Objective:In Iraq, the study on hypertension and diabetes each of them separately is many, while we did not find a study deals with the relationship between them, especially the effect of glucose accumulation in blood vessels on high blood pressure, therefore we designed the current study to investigate the risks of blood pressure in type 2 diabetic Miletus (T2DM) patients. Methods: Our study is a single cross – sectional study conducted on T2DM patients who are hospitalized in Al-Diwaniyah Teaching Hospital and Al-Shamiya General Hospital for the period from November 2021 until February of the year 2022, this study was conducted by taking a blood sample from 180 T2DM patients for clinical and laboratory diagnosis. Also, blood pressure and diabetes were measured for all patients and investigated for a period of hypertension before or after diabetes. Moreover, complete blood count must be performed to determine blood viscosity. The results: The current study showed that most diabetics (23%) are in the (41–50)-year-old age group, and that the main reason for the occurrence of diabetes is due to genetic causes by 48%, and obesity by 28%. Pregnancy by 14%, and disorders of the circulatory system by 10%. The current study showed that (51%) of diabetics suffer from hypertension, and most of the diabetics who suffer from high blood pressure blood pressure are female. We found all diabetic patients within the age group (31-40) years suffering from high blood pressure, and the results showed that 6% of the patients coincided with diabetes and blood pressure together while 61% were suffering from hypertension before diabetes while 33% of patients connected with high blood pressure after diabetes. So we found a higher percentage of PCV in patients have both diabetes and hypertension (89.81%) compared to patients with diabetes only (62.90%). In conclusion, the current study showed the clear correlation between diabetes and high blood pressure, especially among females in our society, so we must hurry in the treatment and investigation of these chronic diseases to prevent their coincidence with each other and increase health complications in patients. Abstract

Background: Severe complicated intra-abdominal sepsis (SCIAS) has an increasing incidence with mortality rates over 80% in some settings. Mortality typically results from disruption of the gastrointestinal tract, progressive and self-perpetuating bio-mediator generation, systemic inflammation, and multiple organ failure. A further therapeutic option may be open abdomen (OA) management with negative peritoneal pressure therapy (NPPT) to remove inflammatory ascites and attenuate the systemic damage from SCIAS, although there are definite risks of leaving the abdomen open whenever it might possibly be closed. This potential therapeutic paradigm is the rationale being assessed in the Closed Or Open after Laparotomy (COOL trial) (https://clinicaltrials.gov/ct2/show/NCT03163095). Initially, the COOL trial received Industry sponsorship; however, this funding mandated the use of a specific trademarked and expensive NPPT device in half of the patients allocated to the intervention (open) arm. In August 2022, the 3 M/Acelity Corporation without consultation but within the terms of the contract canceled the financial support of the trial. Although creating financial difficulty, there is now no restriction on specific NPPT devices and removing a cost-prohibitive intervention creates an opportunity to expand the COOL trial to a truly global basis. This document describes the evolution of the COOL trial, with a focus on future opportunities for global growth of the study. Methods: The COOL trial is the largest prospective randomized controlled trial examining the random allocation of SCIAS patients intra-operatively to either formal closure of the fascia or the use of the OA with an application of an NPPT dressing. Patients are eligible if they have free uncontained intraperitoneal contamination and physiologic derangements exemplified by septic shock OR severely adverse predicted clinical outcomes. Abstract

Background: The SARS-CoV-2 pandemic is still ongoing and a major challenge for health care services worldwide. In the first WSES COVID-19 emergency surgery survey, a strong negative impact on emergency surgery (ES) had been described already early in the pandemic situation. However, the knowledge is limited about current effects of the pandemic on patient flow through emergency rooms, daily routine and decision making in ES as well as their changes over time during the last two pandemic years. This second WSES COVID-19 emergency surgery survey investigates the impact of the SARS-CoV-2 pandemic on ES during the course of the pandemic. Methods: A web survey had been distributed to medical specialists in ES during a four-week period from January 2022, investigating the impact of the pandemic on patients and septic diseases both requiring ES, structural problems due to the pandemic and time-to-intervention in ES routine. Results: 367 collaborators from 59 countries responded to the survey. The majority indicated that the pandemic still significantly impacts on treatment and outcome of surgical emergency patients (83.1% and 78.5%, respectively). As reasons, the collaborators reported decreased case load in ES (44.7%), but patients presenting with more prolonged and severe diseases, especially concerning perforated appendicitis (62.1%) and diverticulitis (57.5%). Otherwise, approximately 50% of the participants still observe a delay in time-to-intervention in ES compared with the situation before the pandemic. Relevant causes leading to enlarged time-to-intervention in ES during the pandemic are persistent problems with in-hospital logistics, lacks in medical staff as well as operating room and intensive care capacities during the pandemic. This leads not only to the need for triage or transferring of ES patients to other hospitals, reported by 64.0% and 48.8% of the collaborators, respectively, but also to paradigm shifts in treatment modalities to non-operative approach

Abstract

Background: The objectives of the study were to investigate the organizational characteristics of acute care facilities worldwide in preventing and managing infections in surgery; assess participants’ perception regarding infection prevention and control (IPC) measures, antibiotic prescribing practices, and source control; describe awareness about the global burden of antimicrobial resistance (AMR) and IPC measures; and determine the role of the Coronavirus Disease 2019 pandemic on said awareness. Methods: A cross-sectional web-based survey was conducted contacting 1432 health care workers (HCWs) belonging to a mailing list provided by the Global Alliance for Infections in Surgery. The self-administered questionnaire was developed by a multidisciplinary team. The survey was open from May 22, 2021, and June 22, 2021. Three reminders were sent, after 7, 14, and 21 days. Results: Three hundred four respondents from 72 countries returned a questionnaire, with an overall response rate of 21.2%. Respectively, 90.4% and 68.8% of participants stated their hospital had a multidisciplinary IPC team or a multidisciplinary antimicrobial stewardship team. Local protocols for antimicrobial therapy of surgical infections and protocols for surgical antibiotic prophylaxis were present in 76.6% and 90.8% of hospitals, respectively. In 23.4% and 24.0% of hospitals no surveillance systems for surgical site infections and no monitoring systems of used antimicrobials were implemented. Patient and family involvement in IPC management was considered to be slightly or not important in their hospital by the majority of respondents (65.1%). Awareness of the global burden of AMR among HCWs was considered very important or important by 54.6% of participants. The COVID-19 pandemic was considered by 80.3% of respondents as a very important or important factor in raising HCWs awareness of the IPC programs in their hospital. Abstract

Skin and soft-tissue infections (SSTIs) encompass a variety of pathological conditions that involve the skin and underlying subcutaneous tissue, fascia, or muscle, ranging from simple superficial infections to severe necrotizing infections. Together, the World Society of Emergency Surgery, the Global Alliance for Infections in Surgery, the Surgical Infection Society-Europe, The World Surgical Infection Society, and the American Association for the Surgery of Trauma have jointly completed an international multi-society document to promote global standards of care in SSTIs guiding clinicians by describing reasonable approaches to the management of SSTIs. An extensive non-systematic review was conducted using the PubMed and MEDLINE databases, limited to the English language. The resulting evidence was shared by an international task force with different clinical backgrounds.

Abstract

The ability of three fatty acids and their respective ethyl esters, to promote generation of reactive oxygen species (ROS), was compared in a preparation of rat brain synaptosomes. Arachidonic but not palmitic or linoleic acids promoted ROS generation. Ethyl esterification of each fatty acid significantly enhanced ROS production and also levels of lipid peroxidaton. Pro-oxidant activity was enhanced by fatty acids, proportionally to their degree of unsaturation. Since ethanol consumption is known to lead to esterification of membrane lipids, this transformation may in part account for the ROS-promoting potential of alcohol.

27. Low Velocity Impact Response of Reinforced Concrete Flat Slabs

Abstract

Background Dimensions of the reference concrete slab are 1950x1950x100 mm subjected to drop-weight impact loading. Objective A comprehensive parametric study was performed to examine the influence of many parameters on the RC slabs. Method From the viewpoint of cost and time savings, a three-dimensional finite element is a very good tool to predict the real behavior of the structural elements. Result and Discussion Results showed that the use of CFRP strips enhance the impact behavior of the slab. Contrarily, the existence of opening led to a dramatic decrease in the dynamic capacity of RC slabs with stress concentration around the openings. Furthermore, changing the shape of the impactor showed a significant effect on the peak impact load as well as the ultimate deflection at impact instant. Conclusion In the scope of this paper, the response of RC slab with top and bottom reinforcements exposed to drop-weight impact loading was inspected. Time histories of impact loads and deflections were presented in detail based on ABAQUS/ Explicit analysis. The findings presented in this paper can be presented as follows: 1. The FE models show a good correlation with the experimental data. Consequently, the proposed finite element models are efficient and economical tool to explorer the effect of many parameters on the performance of RC slabs subjected to drop-weight impact load. 2. The numerical simulation confirmed that using externally bonded CFRP strips has more influence on the peak deflection of the reinforced concrete slab than the recorded impact force. 3. Comparing to the flat shape of the impactor, the hemispherical and curved shape impactor can produce large penetration depth at the impact zone with higher plastic deformations in the concrete slab. However, the flat impactor produced higher deflection at the impact instant. 4. As the radius of the impactor increases, both the duration time and the peak impact force are increased. This is because of the higher contact area was obtained when the flat impactor (infinity radius of curvature) was used as compared to other impactors. 5. Due to decreases in RC slab stiffness, the presence of openings (regardless of their shape) has considerably increased deformations in concrete especially around perimeter of the openings extended to the nearby support. 6. It has been found that the eccentric impact loading causes higher plastic deformations than the concentric one.

Microcystic adnexal carcinoma (MAC) is an infiltrative rare cutaneous neoplasm for which there are no consensus management guidelines because of the paucity of evidence-based practice; hence, the utility of their management is based only on previously published case reports. We report a case of a scalp lesion that was successfully treated using a combination of surgical resection, chemotherapy, and radiotherapy.

Abstract

Intra-abdominal infections (IAIs) are common surgical emergencies and have been reported as major contributors to non-trauma deaths in hospitals worldwide. The cornerstones of effective treatment of IAIs include early recognition, adequate source control, appropriate antimicrobial therapy, and prompt physiologic stabilization using a critical care environment, combined with an optimal surgical approach. Together, the World Society of Emergency Surgery (WSES), the Global Alliance for Infections in Surgery (GAIS), the Surgical Infection Society-Europe (SIS-E), the World Surgical Infection Society (WSIS), and the American Association for the Surgery of Trauma (AAST) have jointly completed an international multi-society document in order to facilitate clinical management of patients with IAIs worldwide building evidence-based clinical pathways for the most common IAIs. An extensive non-systematic review was conducted using the PubMed and MEDLINE databases, limited to the English language. The resulting information was shared by an international task force from 46 countries with different clinical backgrounds. The aim of the document is to promote global standards of care in IAIs providing guidance to clinicians by describing reasonable approaches to the management of IAIs.

Abstract

Introduction: Quality in medical care must be measured in order to be improved. Trauma management is part of health care, and by definition, it must be checked constantly. The only way to measure quality and outcomes is to systematically accrue data and analyze them. Material and methods: A systematic revision of the literature about quality indicators in trauma associated to an international consensus conference Results: An internationally approved base core set of 82 trauma quality indicators was obtained: Indicators were divided into 6 fields: prevention, structure, process, outcome, post-traumatic management, and society integrational effects. Conclusion: Present trauma quality indicator core set represents the result of an international effort aiming to provide a useful tool in quality evaluation and improvement. Further improvement may only be possible through international trauma registry development. This will allow for huge international data accrual permitting to evaluate results and compare outcomes.

Abstract

Background Coronavirus disease 2019 (COVID-19), an epidemic illness caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), affecting the lower respiratory tract. It may impact renal and hepatic function by causing a cytokine storm and inflammation. Although numerous studies on the effects of COVID-19 on people with diabetes Mellitus (DM) have been conducted across the globe, their conclusions have varied due to differences in study populations' demographics, racial and ethnic composition, and geographic locations. Objective To comprehend the effects of a severe COVID-19 infection on renal and hepatic function in patients with diabetes mellitus. Methods This study was carried out at the Chemistry and Biochemistry Department of Al-Nahrain University in Baghdad, Iraq. A total of 100 participants with a severe case of COVID-19 were included in the study between February and April 2022. There was a total of 100 hospital admissions due to COVID-19; 50 of these patients had diabetes, while the other 50 did not. In both groups, serum urea, creatinine, glutamic-oxaloacetic transaminase (GOT) and glutamic-pyruvic transaminase (GPT) were mustered using appropriate methods. Results The median concentration of the DM patients' group had significantly lower concentration of urea, creatinine, GOT and GPT than non-DM patient's group. While all these parameters were significantly higher in COVID-19 patients than in controls. Conclusion Diabetes (COVID-19 patients) exhibited lower levels of biochemical markers as compared to nondiabetic people. Patients with diabetes in COVID-19 take a range of drugs, including insulin injections. In severe circumstances, this medicine may disrupt biochemical markers in addition to the effects it has on renal and hepatic function from COVID-19. [ABSTRACT FROM AUTHOR]

Abstract

Introduction Quality in medical care must be measured in order to be improved. Trauma management is part of health care, and by definition, it must be checked constantly. The only way to measure quality and outcomes is to systematically accrue data and analyze them. Material and methods A systematic revision of the literature about quality indicators in trauma associated to an international consensus conference Results An internationally approved base core set of 82 trauma quality indicators was obtained: Indicators were divided into 6 fields: prevention, structure, process, outcome, post-traumatic management, and society integrational effects. Conclusion Present trauma quality indicator core set represents the result of an international effort aiming to provide a useful tool in quality evaluation and improvement. Further improvement may only be possible through international trauma registry development. This will allow for huge international data accrual permitting to evaluate results and compare outcomes.

Abstract

Manusia memiliki cara yang berbeda dalam menanggapi permasalahan hidup, tidak jarang mereka melakukan kesalahan karena tidak mampu mengelola emosi. Kajian ini terinspirasi dari kehebatan Rasulullah saw. saat dihadapkan pada permasalahan, yang mana bagi manusia biasa, hal tersebut bisa mendatangkan perasaan menderita. Namun tidak semua pengikutnya bisa memunculkan semangat yang sama dalam menghilangkan perasaan menderita yang dialami, maka muncul pertanyaan mengenai, apakah sebagian besar umat terdahulu juga merasa menderita ketika mendapatkan permasalahan hidup?, dan bagaimana Alquran memiliki penjelasan dalam memaknai derajat manusia untuk menanggapi penderitaan yang manusia dapatkan?. Kajian ini menggunakan pendekatan Tafsir Tematik dan dibantu metode Quran Digital, dengan menelusuri kata kunci yang berkaitan dengan kajian ini, beserta devariatnya. Hasil kajian ini menunjukkan bahwa baik dulu maupun sekarang, dibutuhkan kemauan dan ilmu dalam mengelola emosi, tingginya derajat seseorang tidak sebatas dia cukup beriman saja, tetapi diperlukan amal yang lebih besar, serta dibarengi kerelaan saat dihadapkan pada situasi sulit yang membuatnya menderita. Bahkan derajat seseorang bisa saja menurun saat ia tidak bersabar menjalani penderitaan, namun saat ia sadar atas kesalahannya tersebut Allah masih terus memberikan kesempatan bagi manusia yang mau terus belajar. Kajian ini memiliki implikasi, yakni bisa berdampak dalam membangun persepsi baru seseorang dalam memaknai permasalahan di sekitar ruang lingkup kehidupannya.

Abstract

Acute colonic diverticulitis is one of the most common clinical conditions encountered by surgeons in the acute setting. An international multidisciplinary panel of experts from the World Society of Emergency Surgery (WSES) updated its guidelines for management of acute left-sided colonic diverticulitis (ALCD) according to the most recent available literature. The update includes recent changes introduced in the management of ALCD. The new update has been further integrated with advances in acute right-sided colonic diverticulitis (ARCD) that is more common than ALCD in select regions of the world.

Abstract

To mitigate the spread of COVID-19, Saudi Arabia implemented a nationwide lockdown that lasted for approximately five months. Due to the limited availability of telemedicine in Saudi Arabia, many people with diabetes (PWD) lost access to diabetes care services during the lockdown period. Here, we examined the impact of lockdown on cardiometabolic health in PWD and how this may have differed between those who utilized diabetes telemedicine during lockdown versus those who did not.Hemoglobin A1C (A1C), body weight, lipid, and other cardiometabolic parameters were retrospectively reviewed in 384 PWD who attended routine clinic visits in the pre-lockdown (September 2019 to March 2020) and post-lockdown (Aug to Dec 2020) periods. Changes in cardiometabolic parameters from pre- to post-lockdown were compared across 3 groups according to the type of visit that they had during lockdown (April to July 2020): "no visit" (n = 215), "in-person" visit (n = 44), or "virtual" visit (n = 125). The virtual visits in our institution followed a simplified protocol that utilized technological tools readily available to most PWD and clinicians.PWD who attended "virtual" visits during lockdown were the youngest and most likely to have type 1 diabetes; followed by those who attended "in-person" visits and those who had "no visit". A significant reduction in A1C from pre- to post-lockdown periods was noted in PWD who attended a "virtual visit" (9.02 to 8.27%, respectively, p lt; 0.01) and those who attended an "in-person" visit (9.18 to 8.43%, respectively, p lt; 0.05) but not in those who had "no visit" (8.75 to 8.57%, p gt; 0.05). No significant changes were noted in serum glucose, blood pressure, or lipid parameters during the lockdown in any of the groups.Simplified telemedicine visits, including real-time audio calls, were as effective as in-person visits in improving glycemic control in PWD during the lockdown period in a country where telemedicine infrastructure was not well-established. Older adults and those with type 2 diabetes were less likely to utilize telemedicine; suggesting a potential risk of digital divide that warrants greater attention in the future.

Item does not contain fulltext, BACKGROUND: The objectives of the study were to investigate the organizational characteristics of acute care facilities worldwide in preventing and managing infections in surgery; assess participants' perception regarding infection prevention and control (IPC) measures, antibiotic prescribing practices, and source control; describe awareness about the global burden of antimicrobial resistance (AMR) and IPC measures; and determine the role of the Coronavirus Disease 2019 pandemic on said awareness. METHODS: A cross-sectional web-based survey was conducted contacting 1432 health care workers (HCWs) belonging to a mailing list provided by the Global Alliance for Infections in Surgery. The self-administered questionnaire was developed by a multidisciplinary team. The survey was open from May 22, 2021, and June 22, 2021. Three reminders were sent, after 7, 14, and 21 days. RESULTS: Three hundred four respondents from 72 countries returned a questionnaire, with an overall response rate of 21.2%. Respectively, 90.4% and 68.8% of participants stated their hospital had a multidisciplinary IPC team or a multidisciplinary antimicrobial stewardship team. Local protocols for antimicrobial therapy of surgical infections and protocols for surgical antibiotic prophylaxis were present in 76.6% and 90.8% of hospitals, respectively. In 23.4% and 24.0% of hospitals no surveillance systems for surgical site infections and no monitoring systems of used antimicrobials were implemented. Patient and family involvement in IPC management was considered to be slightly or not important in their hospital by the majority of respondents (65.1%). Awareness of the global burden of AMR among HCWs was considered very important or important by 54.6% of participants. Abstract

This paper presents the theoretical behaviors of concrete beams with and without stirrups under drop weight testing. The main objective of this work is to investigate numerically the influence of different types of longitudinal steel reinforcement (steel bars and FRP bars) on the impact behavior of concrete beams in terms of peak load, and deflection. In addition, the effect of drop height, the presence of stirrups, the concrete compressive strength of the concrete beams, and strengthening concrete beams by FRP strips have been involved in investigating their contribution to the behavior of the impact response. The analytical work has been carried out using ABAQUS/CAE 2019. Firstly, the model has been validated against four beams tested experimentally with various impact heights. Then, four groups of beams with different parameters have been simulated and tested under the same impact loading conditions. The first group has consisted of four beams with normal concrete strength reinforced by FRP bars. The second group has been strengthened externally by CFRP strips. The third and the fourth groups are with stirrups and higher grade of concrete, respectively. It has been found out that the current simulation could be an effective tool for predicting the dynamic performance of beams with and without stirrups subjected to impact loading. Furthermore, the improvements done to the available experimental specimens have led to increase the peak impact load and decrease the ultimate deflection. [ABSTRACT FROM AUTHOR]

Abstract

In the present work, analysis of wear behavior, tensile and thermal properties have been done for two sets Poly Methyl Methacrylate samples which is reinforced through licorice particles and pomegranate peels particles. Wear rate resistance is evaluated based on Taguchi’s experimental design L9 (MINITAB 18) under the three factors: weight fraction of fillers (1%, 2%, 3%, 4% and 5% wt.) sliding time (5, 8 and 11 min) and load applied (10, 15and 20N) using a pin-on-disk device. Results display that the samples (Poly Methyl Methacrylate +5% licorice particles and 5% pomegranate peels particles) show the best resistance wear of (0.045×10-6 and 0.10×10-6) respectively. Signal to noise (S/N) results showed that 5% weight fraction (licorice particles and pomegranate particles with PMMA), 11 minutes (sliding time) and 15N (applied load) give the best resistance wear. Statistical analysis of tensile and thermal tests, exhibit that the best mean value of the tensile strength, modulus of elasticity, thermal conductivity, and thermal diffusivity are (±73 MPa, ±70 MPa, ±4.800 GPa, ±4.300 GPa, ±.5600 W/m k, ±.5400 W/m k, ±.3800 mm2/sec, ±.2800 mm2/sec) respectively, of the samples (PMMA+5% licorice particles and 5% pomegranate peel particles). While that the best mean value of the elongation at break (±3.600%, ±3.500%) respectively, of the samples (PMMA + 1% licorice particles and 1% pomegranate peel particles). The results showed that the addition of licorice and pomegranate particles to poly methyl methacrylate resin has improved the properties of wear, tensile and thermal properties.

Abstract

Underground water is extracted by wells that are connected through underground canals. Thus, the levels of pollutant elements in ground water could be evaluated directly from water samples collected from the wells. This study was conducted in the city of Samara / Salah-Aldeen province – Iraq. The samples were taken from 29wells within the study area for the period 2012-2014 with a supervision from the General Authority for Groundwater / the Iraqi Ministry of Water Resources. GIS technology was adopted to calculate the pollution elements at Samara water table. The concentrations of chemical elements [K, Na, Mg, Cl, Ca] and compounds [SO4, HCO3] were manipulated and calculated for the entire area. The results of laboratory analyses showed that the groundwater in this area is not suitable for use as drinking water for humans, since the concentrations of the chemical elements and the compounds exceeded the permissible limits and are not in accordance with WHO standards. However, the results also indicated that the groundwater in the covered area is suitable for agriculture irrigation.

Abstract

Swainsonine has been identified as the toxin in legumes belonging to the genera Astragalus and Oxytropis throughout the world including China, North America, and South America. Several South American Astragalus species have been reported to contain swainsonine; however, data is lacking to support the presence of a fungal symbiont in South American Astragalus species as has been shown for North American and Chinese Astragalus and Oxytropis species. The objective of this study was to investigate several South American species that have been reported to contain swainsonine for the presence of the fungal symbiont using culturing and PCR. Swainsonine was detected in field collections of A. pehuenches, A. illinii and A. chamissonis but not A. moyanoi, which is consistent with reports of toxicity regarding these species. The symbiont Alternaria section Undifilum was detected by PCR in all three species that contained swainsonine but not in A. moyanoi. A fungal symbiont was isolated from seeds of Astragalus pehuenches and A. illinii. The isolated symbiont from both respective species produced swainsonine in vitro, and was demonstrated to belong to the genus Alternaria section Undifilum by analysis of the nuclear ribosomal DNA. It is highly likely that Alternaria section Undifilum isolates will be associated with other South American Astragalus species that are reported to contain swainsonine.

Abstract

Background and Aims: Obesity is still a major health issue worldwide and bariatric surgeries are now considered one of the most effective ways to manage morbid obesity. Women who are obese in their reproductive age appear to be representing the majority of the patients seeking bariatric surgeries, accounting for (80%). The aim of this study is to assess women's awareness level of obstetric and gynecological impact of bariatric surgery on their health. Settings and Design: A cross-sectional study was conducted using an online survey. Methods: Online survey was used to collect data which was distributed through social media. Questions regarding the level of knowledge were included along with sociodemographic characteristics of the population. Statistical Analysis Used: The Statistical Package for Social Sciences (SPSS) software was used to conduct the statistics analysis. Results: The survey elicited a total of (457) valid responses that were analyzed. The majority of responses were from married women (66.3%). Most common age group where those from (15 to 24) years. However, nearly about three-quarters of participants were found to have poor knowledge (73.1%) and only small percentage had a good level of knowledge (3.3%) and the rest of the participants were in the moderate group. Further analysis showed that good knowledge was among those who have consulted a physician, underwent bariatric surgery, whom source of information was the internet, and whom had more than one source. Conclusion: The level of knowledge regarding the obstetric and gynecological impact among females was found to be poor in the eastern region of Saudi Arabia.

Abstract

Background: Timing and adequacy of peritoneal source control are the most important pillars in the management of patients with acute peritonitis. Therefore, early prognostic evaluation of acute peritonitis is paramount to assess the severity and establish a prompt and appropriate treatment. The objectives of this study were to identify clinical and laboratory predictors for in-hospital mortality in patients with acute peritonitis and to develop a warning score system, based on easily recognizable and assessable variables, globally accepted. Methods: This worldwide multicentre observational study included 153 surgical departments across 56 countries over a 4-month study period between February 1, 2018, and May 31, 2018. Results: A total of 3137 patients were included, with 1815 (57.9%) men and 1322 (42.1%) women, with a median age of 47 years (interquartile range [IQR] 28-66). The overall in-hospital mortality rate was 8.9%, with a median length of stay of 6 days (IQR 4-10). Using multivariable logistic regression, independent variables associated with in-hospital mortality were identified: age > 80 years, malignancy, severe cardiovascular disease, severe chronic kidney disease, respiratory rate ≥ 22 breaths/min, systolic blood pressure < 100 mmHg, AVPU responsiveness scale (voice and unresponsive), blood oxygen saturation level (SpO2) < 90% in air, platelet count < 50,000 cells/mm3, and lactate > 4 mmol/l. These variables were used to create the PIPAS Severity Score, a bedside early warning score for patients with acute peritonitis. The overall mortality was 2.9% for patients who had scores of 0-1, 22.7% for those who had scores of 2-3, 46.8% for those who had scores of 4-5, and 86.7% for those who have scores of 7-8. Conclusions: The simple PIPAS Severity Score can be used on a global level and can help clinicians to identify patients at high risk for treatment failure and mortality.
