Background American Indian adults have a higher risk of atrial fibrillation (AF) compared with other racial groups. We implemented opportunistic screening to detect silent AF in American Indian adults attending a tribal health system using a mobile, single‐lead ECG device. Methods and Results American Indian patients aged ≥50 years followed in a tribal primary care clinic with no history of AF underwent a 30‐second ECG. A cardiologist overread all tracings to confirm the diagnosis of AF. After AF was confirmed, patients were referred to their primary care physician for initiation of anticoagulation. Patients seen over the same time period, who were not undergoing screening, served as controls. A total of 1019 patients received AF screening (mean age, 61.5±8.9 years, 62% women). Age and sex distribution of those screened was similar to the overall clinic population. New AF was diagnosed in 15 of 1019 (1.5%) patients screened versus 4 of 1267 (0.3%) patients who were not screened (mean difference, 1.2%; 95% CI, 0.3%–2.2%, P=0.002). Little is known about the nature of fingerspelling during sign language interpretation. In this small-scale, exploratory study, we examined the fingerspelling of interpreters working in five different sign languages: American Sign Language (ASL), Australian Sign Language (Auslan), British Sign Language (BSL), Irish Sign Language (ISL), and Italian Sign Language (LIS). Sixteen interpreters were video-recorded as they rendered President Barack Obama's 2009 inaugural address in their country's sign language. After completing their interpretations, the participants engaged in a retrospective interview about their work. The data were analyzed both quantitatively (for frequency and type of fingerspelling) and qualitatively (for factors influencing fingerspelling). Results indicate that the most finger-spelled items (n=137) were produced in the ASL interpretations and the fewest (n=18) were produced in the LIS interpretations; variation between the groups was found in lexicalized fingerspelling and the fingerspelling of place names. We suggest that the variation in fingerspelling both within and between groups may be explained by sociolinguistic factors, including interpreters' language attitudes and perceptions of the deaf audience. This exploratory study provides a first step in investigating the fingerspelling of interpreters in a variety of sign languages.

We live in a rapidly changing world with technology as an essential part of our lives and an important component in contemporary education. Preparing teachers for the future requires that university professors understand what it means to teach and learn in the digital age. Teacher preparation programs need to create intentional learning environments where teacher candidates can study contemporary issues that are relevant in today's schools, develop pedagogies that are effective for the current generation of students, and develop expertise to design and facilitate meaningful learning for the future. Educational stakeholders, especially teacher educators, need to change how they approach and use instructional practices, change the routines, change how they and the teacher candidates think about teaching and learning, and change the pedagogical approach. The purpose of this article is to provide an example of a physical education teacher preparation program that has addressed the need for a new model in teacher preparation.

Background and study aim Video capsule endoscopy (VCE) is limited by reliance on bowel motility for propulsion, and lack of physical activity has been proposed as a cause of incomplete studies. Our aim was to prospectively investigate the association between physical activity and VCE bowel transit. Patients and methods Ambulatory outpatients receiving VCE were eligible for the study. A pedometer was attached at the time of VCE ingestion and step count was recorded at the end of the procedure. VCE completion was assessed by logistic regression models, which included step count (500 steps as one unit). Total transit time was analyzed by Cox proportional hazards models. The hazard ratios (HR) with 95 % confidence interval (CI) indicated the “hazard” of completion, such that HRs > 1 indicated a reduced transit time. Results A total of 100 patients were included. VCE was completed in 93 patients (93 %). The median step count was 2782 steps. Step count was not significantly associated with VCE completion (odds ratio 1.45, 95 %CI 0.84, 2.49). Pedometer step count was significantly associated with shorter total, gastric, and small-bowel transit times (HR 1.09, 95 %CI 1.03, 1.16; HR 1.05, 95 %CI 1.00, 1.11; HR 1.07, 95 %CI 1.01, 1.14, respectively). Higher body mass index (BMI) was significantly associated with VCE completion (HR 1.87, 95 %CI 1.18, 2.97) and shorter bowel transit times (HR 1.05, 95 %CI 1.02, 1.08). Conclusions Increased physical activity during outpatient VCE was associated with shorter bowel transit times but not with study completion. In addition, BMI was a previously unreported clinical characteristic associated with VCE completion and should be included as a variable of interest in future studies.
