Abstract

In the context of ADONI - the ADaptive Optics National laboratory of INAF - we are arranging for a facility, accessible to the AO community, in which visiting multi-purpose instrumentation, e.g. systems and prototypes of innovative AO concepts, may be directly tested on sky. The facility is located at the 182cm Copernico telescope in Asiago, the largest telescope in Italy, at its Coude focus, for which refurbishment activities are carried out, given that this focus was initially foreseen in the design, but never implemented and used till today. The facility hosts a laboratory where specialized visiting AO instrumentation may be properly accommodated on an optical bench for on-sky demonstrations. We present the current status of the facility, describing the opto-mechanical design implemented at the telescope that allows to redirect the light toward the Coude focus, the tests on the opto-mechanics carried on for stability verification, the integration of the optical and mechanical components within the preexisting structure.

Abstract

A highly and rapidly variable bipolar mass outflow from StHa 190 has been discovered, the first time in a yellow symbiotic star. Permitted emission lines are flanked by symmetrical jet features and multi-component P-Cyg profiles, with velocities up to 300 km/sec. Given the high orbital inclination of the binary, if the jets leave the system nearly perpendicular to the orbital plane, the de-projected velocity equals or exceeds the escape velocity (1000 km/sec). StHa190 looks quite peculiar in many other respects: the hot component is an O-type sub-dwarf without an accretion disk or a veiling nebular continuum and the cool component is a G7 III star rotating at a spectacular 105 km/sec unseen by a large margin in field G giants., Comment: Letter to the Editor, Astron.Astrophys, in press

In Italy, the prevalence of the disease is approximately 10%, making it the fourth most prevalent cancer in the country. The increase in prevalence requires continuous surveillance and care, resulting in a significant burden on Italian National Health Service, making any improvement to the strategy for diagnosing and treating this disease important to the medical and scientific community. The aim of this study was to evaluate the UBC cost of illness in the Italian context, collecting the total costs of the disease.Methods: An economic analysis was carried out in the context of the National Health Service, using data collected from six centers, in order to evaluate direct costs in terms of outpatient, inpatient, and emergency care; pharmaceuticals and follow-up procedures; and indirect costs in terms of productivity losses. Data were collected through aggregated form reports, focusing on patients with an existing diagnosis of UBC who were treated in the last year. The Italian Association of Medical Oncology (AIOM) guidelines were used to identify diagnostic and therapeutic procedures. Statistical analysis was conducted to explore variations among centers.Results: The weighted mean total annual cost per patient was € 3,591, where the cost for superficial disease was € 3,252 and that for metastatic disease was € 606. The analysis confirmed a proportional relation between disease severity and disability grade. The UBC cost of illness, considering prevalence and incidence data coming from the 2016 AIOM/Italian Association of Cancer Registries report, was € 1,187,036,344. Indirect costs accounted to 44%, represented by estimated productivity losses.Conclusion: Our analysis represents the first economic study of UBC in the Italian context, as well as the first real-life evidence of the current therapeutic algorithm. This study opens the possibility for further analysis on the indirect cost components that represent a great burden for the society, especially for those in the severest stages of the disease with high disability grades. Abstract

Purpose Therapeutic strategies for prostate cancer (PCa) have been evolving dramatically worldwide. The current article reports on the evolution of surgical management strategies for PCa in Italy. Methods The data from two independent Italian multicenter projects, the MIRROR-SIU/LUNA (started in 2007, holding data of 890 patients) and the Pros-IT-CNR project (started in 2014, with data of 692 patients), were compared. Differences in patients' characteristics were evaluated. Multivariable logistic regression models were used to identify characteristics associated with robot-assisted (RA) procedure, nerve sparing (NS) approach, and lymph node dissection (LND). Results The two cohorts did not differ in terms of age and prostate-specific antigen (PSA) levels at biopsy. Patients enrolled in the Pros-IT-CNR project more frequently were submitted to RA (58.8% vs 27.6%,p < 0.001) and NS prostatectomy (58.4% vs. 52.9%,p = 0.04), but received LND less frequently (47.7% vs. 76.7%,p < 0.001), as compared to the MIRROR-SIU/LUNA patients. At multivariate logistic models, Lower Gleason Scores (GS) and PSA levels were significantly associated with RA prostatectomy in both cohorts. As for the MIRROR-SIU/LUNA data, clinical T-stage was a predictor for NS (OR = 0.07 for T3, T4) and LND (OR = 2.41 for T2) procedures. As for Pros-IT CNR data, GS >= (4 + 3) and positive cancer cores >= 50% were decisive factors both for NS (OR 0.29 and 0.30) and LND (OR 7.53 and 2.31) strategies. Conclusions PCa management has changed over the last decade in Italian centers: RA and NS procedures without LND have become the methods of choice to treat newly medium-high risk diagnosed PCa.

Abstract

Aetiopathogenesis of Chronic Anal Anterior Fissure (CAAF) remains poorly understood. Some anatomical, clinical and functional features suggest that pathophysiology may be linked to a reduced anal canal pressure. LIS appear illogical as a treatment for CAAF and the employ of techniques aiming to save the integrity of the sphincterial system appears more sensible. The aim of this study was to evaluate 5 years results of fissurectomy and anoplasty with cutaneous V-Y advancement flap in patients affected by CAAF without IAS hypertonia.We enrolled 20 women, affected by idiopathic and non-recurrent CAAF without hypertonic IAS. All patients were followed up for 5 years after surgery with evaluation of anal continence, short and long term post-operative complications, recurrence rate.At 5 years follow up we did not record any new case of anal incontinence and the pre-existing ones haven't worsened. We observed 2 recurrences, which occurred within 2 years after surgery and healed after medical therapy. The manometric values were similar than those recorded prior to surgery.Our study suggests that the procedure performed allows us to preserve anal continence and avoid worsening of its pre-existing alteration.Anal canal, Anoplasty, Fissure, Fissurectomy, Proctology, Sphincterotomy.L’eziopatogenesi delle ragadi anali della commisura anteriore è ancora poco chiara. Alcune caratteristiche cliniche, anatomiche e funzionali suggerirebbero che la patofisiologia di questo disturbo sia strettamente correlata alla riduzione del tono sfinterico anale. La sfinterotomia laterale interna è un approccio poco appropriato per questa patologia, mentre, le procedure volte a garantire l’integrità del sistema sfinterico sembrerebbero più ragionevoli. L’obbiettivo del nostro studio è quello di valutare i risultati a 5 anni della fissurectomia ed anoplastica con lembo di avanzamento cutaneo a V-Y per il trattamento delle ragadi anali della commissura anteriore senza ipertono sfinterico. Al fine di condurre il nostro studio abbiamo considerato 20 pazienti donne affette da ragade anale e sottoposte al suddetto intervento chirurgico dal Gennaio 2008 al Gennaio 2015; tutte le pazienti sono state seguite per 5 anni dopo la procedura chirurgica con valutazione della continenza anale, delle complicanze post-operatorie e breve e lungo termine e del tasso di ricorrenza. Al follow up a 5 anni non abbiamo riscontrato nessun caso di incontinenza anale di nuova insorgenza, è alcun aggravamento dei casi di incontinenza anale preesistenti. Abbiamo osservato 2 recidive trattate efficacemente con terapia conservativa. Infine, il quadro manometrico a 5 anni era simile a quello pre-operatorio. In conclusione il nostro studio suggerisce che la procedura chirurgica da noi utilizzata ci permette di conservare la continenza anale in queste pazienti, senza aggravare le preesistenti alterazioni della stessa.

Abstract

Le cisti mesoteliali del legamento rotondo sono una rara causa di tumefazione inguinale nelle donne, la loro eziopatogenesi rimare poco chiara e i casi riportati in letteratura sono scarsi. Inoltre, quest’ultime sono raramente prese in considerazione nella diagnosi differenziale delle tumefazioni della regione inguinale nelle pazienti di sesso femminile, ragion per cui sono spesso identificate solo in corso di esplorazione chirurgica e correttamente diagnosticate solo dopo esame istopatologico. Una donna di 57 anni (BMI 37 kg / m2) si è presentato nel nostro dipartimento con una massa inguinale sinistra, con storia di 2 mesi, che causava disagio locale e peggioramento progressivo del dolore locale. È stata fatta una diagnosi preoperatoria dell’ernia femorale sintomatica sinistra e la paziente ha acconsentito al trattamento chirurgico. L’esplorazione chirurgica dei canali inguinale i e femorale ha rivelato un’ernia femorale associata a una chiara lesione cistica fluida di circa 2 cm derivante dal legamento rotondo. L’istopatologia ha dimostrato una cisti mesoteliale del legamento rotondo CONCLUSIONI: Le cisti mesoteliali del legamento rotondo dell’utero devono essere prese in considerazione nella diagnosi differenziale del gonfiore inguinale nelle pazienti di sesso femminile e sono necessari maggiori sforzi per raggiungere una diagnosi preoperatoria e prevenire un trattamento eccessivo. L’indicazione chirurgica per queste lesioni rimane legata alla presenza di sintomi invalidanti, data la natura benigna delle stesse, per cui, al fine di evitare un overtreatment, ci sembra utile sottolineare innanzitutto, come questa entità patologica debba essere presa in considerazione nel percorso diagnostico di una tumefazione inguinale in soggetti di sesso femminile e in secondo luogo, l’utilità dell’esame ecografico di routine in queste pazienti. BACKGROUND: Differential diagnosis of inguinal mass must include, especially in female patients, a wide variety of lesions among which our analysis will focus on mesothelial cyst of the round ligament of the uterus. A rare developmental lesion often misdiagnosed as hernias and accidentally detected during surgical exploration of the groin region. CASE REPORT: Of a left inguinal mass causing local discomfort and progressive worsening of local pain. A pre-operative diagnosis of left symptomatic femoral hernia was made and the patient consented to surgical treatment. The surgical exploration of the inguinal and femoral canals revealed a femoral hernia associated to a clear fluid cystic lesion of around 2 cm arising from the round ligament. Histopathology demonstrated a mesothelial cyst of the round ligament CONCLUSIONS: Mesothelial cysts of the round ligament of the uterus must be taken into consideration in the differential diagnosis of groin swelling in female patients and a greater effort is needed in order to reach a preoperative diagnosis and prevent an over treatment. Abstract

Background: A new edition of the TNM was recently released that includes modifications for the staging system of kidney cancers. Specifically, T2 cancers were subclassified into T2a and T2b ( 10 cm), tumors with renal vein involvement or perinephric fat involvement were classified as T3a cancers, and those with adrenal involvement were classified as T4 cancers. Objective: Our aim was to validate the recently released edition of the TNM staging system for primary tumor classification in kidney cancer. Design, setting, and participants: Our multicenter retrospective study consisted of 5339 patients treated in 16 academic Italian centers. Intervention: Patients underwent either radical or partial nephrectomy. Measurements: Univariable and multivariable Cox regression models addressed cancer-specific survival (CSS) after surgery. Results and limitations: In the study, 1897 patients (35.5%) were classified as pT1a, 1453 (27%) as pT1b, 437 (8%) as pT2a, 153 (3%) as pT2b, 1059 (20%) as pT3a, 117 (2%) as pT3b, 26 (0.5%) as pT3c, and 197 (4%) as pT4. At a median follow-up of 42 mo, 786 (15%) had died of disease. In univariable analysis, patients with pT2b and pT3a tumors had similar CSS, as did patients with pT3c and pT4 tumors. Moreover, both pT3a and pT3b stages included patients with heterogeneous outcomes. In multivariable analysis, the novel classification of the primary tumor was a powerful independent predictor of CSS (p for trend < 0.0001). However, the substratification of pT1 tumors did not retain an independent predictive role. The major limitations of the study are retrospective design, lack of central pathologic review, and the small number of patients included in some substages. Conclusions: The recently released seventh edition of the primary tumor staging system for kidney tumors is a powerful predictor of CSS. However, some of the substages identified by the classification have overlapping prognoses, and other substages include patients with heterogeneous outcomes. Abstract

Summary. Rectal leiomyomas are a rare conditions, with low reported incidence in literature and constitute about 0.1% of rectal tumours; in fact rectal leiomyomas occur in approximately 1 out of 2000-3000 rectal tumors. We report on a patient with a 3 cm semi-pedunculated colonic leiomyoma, which was successfully removed by endoscopic polypectomy after normal saline-epinephrine submucosal injection. When we encounter a tumor during a colonoscopic examination, we usually evaluate the tumor carefully and perform an endoscopic resection when we judge it is appropriate. When a symptomatic smooth muscle tumors smaller than 2 cm are incidentally found on colonoscopy, surgical resection is unnecessary. Furthermore, if a tumor can be lifted with a snare and it is either pedunculated or semi-pedunculated, endoscopic resection might be a safe option. For those tumors with wide-based or exoluminal growth, endoscopic removal should be avoided due to the higher risks of bleeding and perforation. The histological findings of the resected tumor are important. If there is any malignant element that can not be completely eradicated, we would suggest surgical treatment. We believe our process allows to avoid unnecessary surgery and reduces medical costs.

Scopo del lavoro L’assenza di complicanze chirurgiche rappresenta un importante obbiettivo dopo intervento di nefrectomia parziale. Lo scopo del presente studio è quello di analizzare le complicanze postoperatorie in pazienti sottoposti a chirurgia conservativa renale per tumori renali T1 clinici e di ricercare possibili fattori predittivi di complicanze postoperatorie. Materiali e metodi Sono stati analizzati i dati raccolti in maniera prospettica di 1055 pazienti sottoposti a chirurgia conservativa renale in 19 centri italiani da gennaio 2009 a dicembre 2012. Sono stati analizzati solo pazienti con tumori renali T1 clinici e senza complicanze intraoperatorie. I centri sono stati suddivisi in centri a basso ed alto volume in base alla soglia di 50 interventi per anno. E’ stata effettuata un’analisi descrittiva dei casi (n=925). Infine è stata effettuata un’analisi univariata e multivariata per le complicanze chirurgiche postoperatorie. Risultati Complessivamente 522 pazienti sono stati sottoposti a procedura open, 286 a procedura laparoscopica e 117 a procedura robot-assistita. Il 73,1% dei casi è stato trattato in centri ad alto volume. Le perdite ematiche mediane sono state di 150 (IQR: 100-250) cc e il tempo mediano di ischemia è risultato di 16 (IQR: 12-20) minuti. Il 13,1% dei pazienti ha sviluppato complicanze chirurgiche postoperatorie (8,1% Clavien 2 e 3,5% Clavien 3) ed il 5,9% dei pazienti ha sviluppato complicanze mediche postoperatorie (3,2% complicanze respiratorie, 1,9% complicanze cardiache, 0,1% tromboembolismo e 0,7% per altre cause). All’analisi univariata, l’indicazione chirurgica, la creatinina preoperatoria, il diametro clinico e la provenienza da centri a basso volume chirurgico sono risultati fattori predittivi significativi per lo sviluppo di complicanze chirurgiche (rispettivamente p ≤ 0,0001; p = 0,001; p = 0,01; p = 0,01), ma questo dato non è stato confermato all’analisi multivariata. Abstract

Scopo del lavoro La chirurgia conservativa renale rappresenta la procedura standard per il trattamento chirurgico di tumori renali di piccole dimensioni (< 4 cm) e clinicalmente localizzati. Attualmente il trattamento conservativo dei tumori renali clinicalmente localizzati rappresenta una procedura in grande espansione. L’obiettivo del presente studio è quello di descrivere i risultati clinici, perioperatori ed oncologici di pazienti sottoposti a chirurgia conservativa renale in Italia. Materiali e metodi Abbiamo valutato i pazienti sottoposti a trattamento chirurgico conservativo per tumori renali, nel periodo compreso tra Gennaio 2009 e Dicembre 2012 presso 19 centri urologici italiani (RECORd 1). Sono stati analizzati i dati preoperatori, radiologici, intraoperatori, postoperatori ed istopatologici. Sono state comparate le procedure chirurgiche di diversi periodi (2009 vs 2012 e i bienni 2009-2010 vs 2011-2012). Risultati Complessivamente, sono stati valutati 983 pazienti. Abstract

Objectives To evaluate the efficacy of hemostatic agents, TachoSil and FloSeal, during partial nephrectomy using a large multicenter dataset. Methods Data of 1055 patients who underwent partial nephrectomy between January 2009 and December 2012 in 19 Italian centers were collected within an observational multicentric study (RECORd Project). The decision whether or not to use hemostatic agents after renorrhaphy and the type of hemostatic agents applied was adopted according to the centers' and surgeons' preference. A TriMatch propensity score analysis was applied to balance three study groups (no hemostatic agents, TachoSil, FloSeal) for sex, age, surgical indication (elective/relative vs imperative), clinical stage (cT1a vs cT1b), tumor exophyticity, approach (open vs minimally invasive), technique (standard partial nephrectomy vs simple enucleation), preoperative hemoglobin and creatinine. Postoperative complications and variation of hemoglobin and creatinine values between preoperative versus third postoperative day were compared. Results TriMatch analysis allowed us to obtain 66 well‐balanced triplets. No differences were found in terms of outcomes between the study groups. Conclusions The present findings suggest that adding hemostatic agents to renorraphy during partial nephrectomy does not provide better surgical outcomes.

Abstract

Study Type - Therapy (cohort) Level of Evidence 2b What's known on the subject? and What does the study add? Few studies supported the expanded indications for nephron-sparing surgery (NSS) in selected patients with 4.1 cm renal tumours in the size range (T1b). However, all these comparative studies included both imperative and elective partial nephrectomy and patient selection for analysis was based on pathological stage (pT1) and not on clinical stage (cT1). Patients with clinically organ-confined RCC (cT1) who are candidates for elective PN have a limited risk of clinical understaging. NSS is not associated with an increased risk of recurrence and cancer-specific mortality both in cT1a and cT1b tumours OBJECTIVE: • To compare the oncological outcomes of patients who underwent elective partial nephrectomy (PN) or radical nephrectomy (RN) for clinically organ-confined renal masses ≤7 cm in size (cT1). PATIENTS AND METHODS: • The records of 3480 patients with cT1N0M0 disease were extracted from a multi-institutional database and analyzed retrospectively. RESULTS: • In patients who underwent PN, the risk of clinical understaging was 3.2% in cT1a cases and 10.6% in cT1b cases. • With regard to the cT1a patients, the 5- and 10-year cancer-specific survival (CSS) estimates were 94.7% and 90.4%, respectively, after RN and 96.1% and 94.9%, respectively, after PN (log-rank test: P = 0.01). • With regard to cT1b patients, the 5-year CSS probabilities were 92.6% after RN and 90% after PN, respectively (log-rank test: P = 0.89). • Surgical treatment failed to be an independent predictor of CSS on multivariable analysis, both for cT1a and cT1b patients. • Interestingly, PN was oncologically equivalent to RN also in patients with pT3a tumours (log-rank test: P = 0.91). CONCLUSIONS: • Elective PN is not associated with an increased risk of recurrence and cancer-specific mortality in both cT1a and cT1b tumours. • Data from the present study strongly support the use of partial nephrectomy in patients with clinically T1 tumours, according to the current recommendations of the international guidelines.

Abstract

A new edition of the TNM was recently released that includes modifications for the staging system of kidney cancers. Specifically, T2 cancers were subclassified into T2a and T2b (< or =10 cm vs >10 cm), tumors with renal vein involvement or perinephric fat involvement were classified as T3a cancers, and those with adrenal involvement were classified as T4 cancers. OBJECTIVE: Our aim was to validate the recently released edition of the TNM staging system for primary tumor classification in kidney cancer. DESIGN, SETTING, AND PARTICIPANTS: Our multicenter retrospective study consisted of 5339 patients treated in 16 academic Italian centers. INTERVENTION: Patients underwent either radical or partial nephrectomy. MEASUREMENTS: Univariable and multivariable Cox regression models addressed cancer-specific survival (CSS) after surgery. RESULTS AND LIMITATIONS: In the study, 1897 patients (35.5%) were classified as pT1a, 1453 (27%) as pT1b, 437 (8%) as pT2a, 153 (3%) as pT2b, 1059 (20%) as pT3a, 117 (2%) as pT3b, 26 (0.5%) as pT3c, and 197 (4%) as pT4. At a median follow-up of 42 mo, 786 (15%) had died of disease. In univariable analysis, patients with pT2b and pT3a tumors had similar CSS, as did patients with pT3c and pT4 tumors. Moreover, both pT3a and pT3b stages included patients with heterogeneous outcomes. Abstract

Purpose: The excision of the renal tumor with a substantial margin of healthy parenchyma is considered the gold standard technique for partial nephrectomy. However, simple enucleation showed excellent results in some retrospective series. We compared the oncologic outcomes after standard partial nephrectomy and simple enucleation. Materials and Methods: We retrospectively analyzed 982 patients who underwent standard partial nephrectomy and 537 who had simple enucleation for localized renal cell carcinoma at 16 academic centers between 1997 and 2007. Local recurrence, cancer specific survival and progression-free survival were the main outcomes of this study. The Kaplan-Meier method was used to calculate survival functions and differences were assessed with the log rank statistic. Univariable and multivariable Cox regression models addressed progression-free survival and cancer specific survival. Results: Median followup of the patients undergoing traditional partial nephrectomy and simple enucleation was 51 +/- 37.8 and 54.4 +/- 36 months, respectively (p = 0.08). The 5 and 10-year progression-free survival estimates were 88.9 and 82% after standard partial nephrectomy, and 91.4% and 90.8% after simple enucleation (p = 0.09). The 5 and 10-year cancer specific survival estimates were 93.9% and 91.6% after standard partial nephrectomy, and 94.3% and 93.2% after simple enucleation (p = 0.94). On multivariable analysis the adopted nephron sparing surgery technique was not an independent predictor of progression-free survival (HR 0.8, p = 0.55) and cancer specific survival (HR 0.7, p = 0.53) when adjusted for the effect of the other covariates. Conclusions: To our knowledge this is the first multicenter, comparative study showing oncologic equivalence of standard partial nephrectomy and simple enucleation.

Clinical (age, body mass index) and urological (catheterization or transurethral resection of the prostate) records, quality of life (QOL) scores determined using various instruments (including the International Index of Erectile Function [IIEF] and University of California Los Angeles, Prostate Cancer Index [UCLA-PCI]), PCa characteristics (clinical stage [cT], prostate-specific antigen, biopsy Gleason score), surgical features (surgical approach, nerve and bladder neck sparing, catheterization), and pathologic outcomes (pT, pN+, Gleason score, positive surgical margins) were recorded. Continence status prior to surgery and at 1month after RP was assessed and classified as followed: (i) full continence; (ii) 0-1 pads/day; or (iii) >1 pad/day. Only patients determined to have full continence prior to surgery were included in the analysis. Data were evaluated using Spearman's correlation analysis and multivariate logistic regression. Results: Data from 1972 patients with full continence preoperatively and complete postoperative data were analyzed. At 1month after RP, 644 patients (32.7%) were fully continent, 810 (41.1%) were using 0-1 pads/day, and 518 (26.3%) were using >1 pad/day. Univariate analysis indicated that clinical and urological data, QOL, PCa characteristics, surgical features, and pathologic outcomes were determinants for continence recovery. Multivariate analysis indicated that preoperative sexual activity (UCLA-PCI Sexual Function P=0.005; IIEF P=0.040), bladder neck sparing (P=0.003), catheterization time (P=0.007), and catheter diameter (P=0.046) were associated with 1month continence recovery. Conclusions: Age and nerve sparing are not significant predictors of continence recovery 1month after RP. Preoperative erectile function can predict post-prostatectomy incontinence. Abstract

OBJECTIVES: To evaluate and compare the correlations between Preoperative Aspects and Dimensions Used for an Anatomical (PADUA) and R.E.N.A.L. [Radius (tumour size as maximal diameter), Exophytic/endophytic properties of the tumour, Nearness of tumour deepest portion to the collecting system or sinus, Anterior (a)/posterior (p) descriptor and the Location relative to the polar line] nephrometry scores and perioperative outcomes and postoperative complications in a multicentre, international series of patients undergoing robot-assisted partial nephrectomy (RAPN) for masses suspicious for renal cell carcinoma (RCC). PATIENTS AND METHODS: We retrospectively evaluated the clinical records of patients who underwent RAPN between 2010 and 2013 for clinical N0M0 renal tumours in four international centres that completed all the data required for the Vattikuti Global Quality Initiative in Robotic Urologic Surgery (GQI-RUS) database. All patients underwent preoperative computed tomography or magnetic resonance imaging to define the clinical stage and anatomical characteristics of the tumours. PADUA and R.E.N.A.L. scores were retrospectively assessed in each centre. Univariate and multivariate analyses were used to evaluate the correlations between age, gender, Charlson comorbidity index, clinical tumour size, PADUA and R.E.N.A.L. complexity group categories and warm ischaemia time (WIT) of >20 min, urinary calyceal system closure, and grade of postoperative complications. RESULTS: Overall, 277 patients were evaluated. The median (interquartile range) tumour size was 33.0 (22.0-43.0) mm. The median PADUA and R.E.N.A.L. scores were eight and seven, respectively; 112 (40.4%), 86 (31.0%) and 79 (28.5%) patients were classified in the low-, intermediate- or high-complexity group according to PADUA score, while 118 (42.5%), 139 (50.1%) and 20 (7.2%) were classified in the low-, intermediate- or high-complexity group according to R.E.N.A.L. score, respectively. Abstract

AIM OF THE STUDY: To report a matched-pair comparative analysis between open (OPN) and laparoscopic partial nephrectomy (LPN) for clinical (c) T1a renal masses from a large prospective multicenter dataset. MATERIALS AND METHODS: The RECORd Project includes all patients who underwent OPN and LPN for kidney cancer between January 2009 and January 2011 at 19 Italian centers. Open and laparoscopic groups were compared regarding clinical, surgical, pathologic, functional results and TRIFECTA outcome. Multivariable logistic regression models were used to analyze predictors of WIT >25 min, surgical complications (SC) and the achievement of the TRIFECTA outcome. RESULTS: Overall, 301 patients had OPN and 149 LPN. Groups were matched 1:1 (140 matched pairs) for clinical diameter, tumor location and type of indication. Laparoscopic partial nephrectomy was associated with a significantly mean longer WIT (19.9 vs. 15.1 min; p < 0.001), and it was an independent predictor of a WIT >25 min (RR 6.29, p < 0.0001). The TRIFECTA was achieved in 78.6 and 74.3% after OPN and LPN (p = ns), respectively, and the surgical approach was not a predictor of a negative TRIFECTA and SC at multivariable analysis. At 6-month follow-up, no significant differences were observed between the OPN and LPN group both in estimated glomerular filtration rate (eGFR) (∆GFR 1.1 vs. 4.1 mL/min) and in new-onset stage III-V chronic kidney disease (CKD) rate (0 vs. 0.7%). CONCLUSION: No significant difference in achieving the TRIFECTA outcome was reported after OPN and LPN. LPN was associated with a significantly longer WIT. However, eGFR at 6-month follow-up did not differ significantly between the two surgical approaches.

Abstract

OBJECTIVES: To evaluate the influence of preservation of the muscular internal sphincter and proximal urethra on continence recovery after radical prostatectomy. METHODS: This was a prospective single-center, case-control study. A total of 40 consecutive patients with organ-confined prostate cancer were submitted to radical prostatectomy with the preservation of the muscular internal sphincter and the proximal urethra (group 1), and their outcomes were compared with those of 40 patients submitted to a standard procedure (group 2). Continence rates were assessed using a self-administrated questionnaire at 3, 7 and 30 days, and 3 and 12 months after removal of the catheter. RESULTS: Group 1 had a faster recovery of early continence than group 2 at day 3 (45% vs 22%; P = 0.029) and at day 7 (75% vs 50%; P = 0.018). Considering the number of pads, group 1 had a faster recovery of continence at 3, 7 and 30 days, and also had less incidence of severe incontinence. There was no statistically significant difference in terms of continence at 3 and 12 months among the two groups. Multivariate logistic regression analysis showed that surgical technique and young age were significantly associated with earlier time to continence at 3 and 7 days. The two groups had no significant differences in terms of surgical margins. CONCLUSIONS: Our modified technique of radical retropubic prostatectomy with preservation of the smooth muscular internal sphincter, as well as of the proximal urethra during bladder neck dissection, results in a significantly increased urinary continence at 3, 7 and 30 days after catheter removal, with a minor incidence of severe incontinence. The technique is also oncologically safe, and it does not increase the operative duration of the procedure.

Abstract

We present the first case of salvage retroperitoneal lymph node dissection based on the results of (18)F-FACBC PET/CT performed for a prostate-specific antigen relapse after radical prostatectomy. The patients underwent (11)C-choline PET/CT, which turned out negative, while (18)F-FACBC PET/CT visualized two lymph node metastases confirmed at pathological examination. Preliminary clinical reports showed an improvement in the detection rate of 20-40% for (18)F-FACBC in comparison with (11)C-choline, rendering the (18)F-FACBC the potential radiotracer of the future. Salvage surgery for prostate cancer is a fascinating but controversial approach. New diagnostic tools may improve its potential by increasing the assessment and the selection of the patients.
