Objective: To evaluate a novel technology for real time tracking of RF-Identified (RFID) surgical tools (Biotic System), providing intraoperative data analytics during simulated cardiovascular procedures. Ineffective asset management in the Operating Room (OR) leads to inefficient utilization of resources and contributes to prolonged operative times and increased costs. Analysis of captured data can assist in quantifying instrument utilization, procedure flow, performance and prevention of retained instruments. Methods & Results: Five surgeons performed thirteen simulated surgical cases on three human cadavers. Procedures included (i) two abdominal aortic aneurysm (AAA) repairs, (ii) three carotid endarterectomies (CE), (iii) two femoropopliteal (fem-pop) bypasses, (iv) thoracic aortic aneurysm repair, (v) coronary artery bypass graft, (vi) aortic valve replacement, (vii) ascending aortic aneurysm repair, (viii) heart transplants, and (ix) mitral valve replacement. For each case an average of 139 surgical instruments were RFID-tagged and tracked intraoperatively. Data was captured and analyzed retrospectively. Of the 139 instruments tracked across each of the 13 cases, 55 instruments (39.5%) were actually used, demonstrating a high level of redundancy. For repeat cases (i.e. CE/AAA/fem-pop): (i) average instrument usage was 41 ± 3.6 (8.8% variation) for CE (n=3); (ii) average instrument usage was 69 ± 4.0 (5.8% variation) for AAA (n=2); and (iii) average instrument usage was 48 ± 2.5 (5.3% variation) for fem- pop (n=2). Results also showed a reduction in end-of-procedure instrument counting times of 58-87%. Conclusions: We report on a method for collecting intraoperative data analytics regarding instrument usage via RFID technology. This system will help refine instrument selection, quantitate instrument utilization and prevent inadvertent retention in a patient. This should help increase efficiency in packaging and sterilization and let surgeons make objective decisions in the composition of surgical trays. Clinical and Translational Impact Statement—Intraoperative analytics of surgical tools and associated equipment may ultimately lead to safer more efficient surgeries that increase patient outcomes while decreasing the cost of care.

Objective: In the West Midlands regional genetics service, cases of perinatal death with a possible genetic diagnosis are evaluated by the perinatal pathology genetic multidisciplinary team (MDT). The MDT assesses autopsy findings and suggests appropriate genomic assessment. The objective of this retrospective service evaluation was to determine the clinical utility of the MDT in assessing perinatal deaths associated with structural anomaly. This is the first evaluation since the introduction of whole‐genome and whole‐exome sequencing in routine clinical care. Methods: This was a retrospective service evaluation including all cases of perinatal death with an associated structural anomaly and suspected genetic etiology that underwent perinatal MDT assessment between January and December 2021. All cases received a full or partial postmortem examination and at least a chromosomal microarray analysis. Demographic characteristics, phenotype, genotype, MDT recommendations, diagnoses, outcomes and impact of postmortem analysis and genetic testing data were collected from patient case notes. Results: Overall, 123 cases were discussed at the MDT meetings in 2021. Genetic evaluation was recommended in 84 cases and accepted in 64 cases. A range of genetic tests were requested according to indication and availability. Thirty diagnoses were made in 29 cases from 26 unrelated families. The diagnostic yield was 24% (29/123) in all cases or 45% (29/64) in cases with a suspected genetic diagnosis who underwent genetic testing. Postmortem examination provided clinically actionable phenotypic data in 79% of cases. A genetic diagnosis enabled accurate recurrence risk counseling and provision of appropriate follow‐up, including prenatal testing and preimplantation diagnosis for patients with inherited conditions. Conclusions: Genomic testing was a clinically useful addition to (but not a substitute for) postmortem examination in cases of perinatal death associated with structural anomaly. The MDT approach helped assess cases and plan appropriate follow‐up. Expedited whole‐genome sequencing or panel‐agnostic analysis were most appropriate for heterogeneous presentations. This broad approach can also expand knowledge of prenatal phenotypes and detect novel disease genes, and should be a priority in future research. © 2024 International Society of Ultrasound in Obstetrics and Gynecology. This article is a systematic review and meta-analysis that examines the placental architectural characteristics in monochorionic twins with Twin-twin transfusion syndrome (TTTS) following laser ablation. The study analyzed 26 studies with a total of 4013 MC twins and found that approximately 24% of TTTS placentas had residual anastomoses after laser ablation. The presence of residual anastomoses was associated with an increased risk of adverse fetal outcomes, including intrauterine fetal demise, neonatal death, recurrent TTTS, and post-laser twin anemia-polycythemia sequence. Additionally, abnormal cord insertions were detected in almost half of the MC twins with TTTS that underwent laser ablation. Abstract Mitral stenosis (MS) is a complex valvular pathology with significant clinical burden even today. Its effect on the right heart is often overlooked, despite it playing a considerable part in the symptomatic status. We enrolled 39 mitral valve stenosis patients and 39 age- and gender-matched healthy controls. They underwent conventional, speckle-tracking and 3D echocardiographic examinations. Introduction: Stress-induced childhood-onset neurodegeneration with variable ataxia and seizures (CONDSIAS) is an autosomal recessive disorder caused by biallelic pathogenic variants in the ADPRS gene. Since its first description in 2018, less than fifty cases have been reported worldwide, but thus far, without histopathology of the nervous system. Case Presentation: We summarize the clinical, paraclinical, and genetic characteristics of the disease in the proband, and the pathological workup in his older brother who passed away more than a decade ago. The characteristics of CONDSIAS in the two brothers overlapped with those of Friedreich ataxia. A final clarification of the diagnosis was made possible by whole exome sequencing (WES) that identified the homozygous pathogenic variants in the ADPRS gene. Conclusion: CONDSIAS is a rare disorder with highly variable presentation. Based on solely clinical and even pathological workup, establishing a definite diagnosis may be challenging. In the two brothers, we observed clinical and histopathological features of the disease suggesting, though not completely fulfilling, the diagnosis of Friedreich’s ataxia. WES allowed us to rapidly identify the underlying genetic abnormality and to make a shortcut to the right diagnosis amongst recessive ataxias. As of today, no specific treatment for CONDSIAS is available. Repurposing of certain approved modalities that also target the affected pathway in CONDSIAS recently arose, though as yet without proven success. Knowing the biological relevance of the affected gene product offers potential targets for the development of disease-modifying drugs for this highly disabling disease in the near future.

Abstract Reusing reclaimed wastewater became a practical resource for water utilization in groundwater recharge and irrigation activities. However, the quality of reclaimed wastewater needs improvement to meet the environmental regulations and reduce contamination risks. A laboratory-scale study simulated a soil aquifer treatment (SAT) system, exploring the synergistic effects of wet and dry cycles alongside key physicochemical parameters on pollutant removal efficiency using a glass column filled with quartz sand as the filtration medium. The investigation focused on the cyclic wetting and drying phases to unravel their impact on removing NH4 +, NO3 −, and PO4 3−. The synthetic wastewater introduced into the system exhibited varying pollutant concentrations during wet and dry periods, influenced by dynamic soil water content (WC%), pH, dissolved oxygen (DO), and oxidation–reduction potential (ORP). The high removal rates of 93% for PO4 3− and 43% for Total N2 demonstrate the system’s capability to reduce concentrations significantly under dynamic alternating between wet and dry conditions. Results unveiled that the wet period consistently yielded higher removal rates for N2 species. Interestingly, for PO4 3−, the dry periods demonstrated a higher removal efficiency. Moreover, the study identified an average NO3 − production during the experimental phases as a byproduct of nitrification. The average NO3 − production in wet periods was 2.5 mg/L, whereas it slightly decreased to 2.2 mg/L in dry periods. These findings underscore the nuanced influence of wet and dry conditions on specific pollutants within SAT systems. Applying the logistic regression model and principal component analysis demonstrated the statistical significance of WC, pH, DO, and ORP in predicting wet/dry conditions, providing quantitative insights into their influential roles on the nutrient dynamic concentrations. This study contributes valuable data to our understanding of SAT systems, offering practical implications for designing and implementing sustainable wastewater treatment practices and pollution management across diverse environmental contexts.

This article discusses the establishment of a Fetal Medicine Task Force at Semmelweis University in Hungary. The task force aims to improve the management of high-risk pregnancies, establish a referral system for patients with fetal abnormalities, and identify candidates for fetal therapy. The article highlights the successful performance of several fetal interventions, such as intrauterine fetal transfusions and relieving cystocentesis. The task force's efforts are seen as a groundbreaking step in the field of Fetal Medicine in Hungary, with the goal of improving perinatal outcomes and reducing mortality and morbidity. Ultra-precision turning is a crucial process in the manufacturing industry as it helps to produce parts with high dimensional accuracy, surface finish, and tolerance. The process is similar to traditional turning but is carried out under special circumstances to achieve greater precision and surface finish. The process can be applied to conventional structural materials, but the demand for machining hardened steels is increasing. The optimization of ultra-precision turning of AISI D2 using cubic boron nitride (CBN) tools is a crucial aspect in the field of high-quality machining. This study aims to evaluate the performance of the process and identify the optimal parameters that result in the best quality components while using a CBN tool's ultra-precision turning of AISI D2. Ultra-precision turning process factors such as cutting speed, feed, and depth of cut were experimentally investigated to enhance the response output, such as surface roughness and cutting force components. The full factorial experimental design was used for determining the process characteristics under different conditions, and experimental results were applied to search for the optimum response of machining performance. The optimization process was done by combining the hybrid genetic algorithm-response surface methodology (GA-RSM) and the Taguchi-grey relational analysis (GRA) statistical tools. These methods are useful in situations where the relationship between the input variables and the output responses is complex and non-linear. The results showed that a hybrid GA-RSM approach, combined with Taguchi-GRA statistical analysis, can effectively find optimal process parameters, leading to the best combination of surface roughness and cutting force. In hybrid Taguchi - GRA, the optimal cutting conditions were found to be a cutting speed of 175 m/min, a feed of 0.025 mm, and a depth of cut of 0.06 mm. The findings of this study provide valuable insights for the optimization of ultra-precision CBN turning operations, contribute to the development of precision manufacturing technology, and can be used as a reference for similar machining processes.

Abstract Choosing the optimal device during cardiac resynchronization therapy (CRT) upgrade can be challenging. Therefore, we sought to provide a solution for identifying patients in whom upgrading to a CRT-defibrillator (CRT-D) is associated with better long-term survival than upgrading to a CRT-pacemaker (CRT-P). To this end, we first applied topological data analysis to create a patient similarity network using 16 clinical features of 326 patients without prior ventricular arrhythmias who underwent CRT upgrade. Then, in the generated circular network, we delineated three phenogroups exhibiting significant differences in clinical characteristics and risk of all-cause mortality. Importantly, only in the high-risk phenogroup was upgrading to a CRT-D associated with better survival than upgrading to a CRT-P (hazard ratio: 0.454 (0.228–0.907), p = 0.025). Finally, we assigned each patient to one of the three phenogroups based on their location in the network and used this labeled data to train multi-class classifiers to enable the risk stratification of new patients. During internal validation, an ensemble of 5 multi-layer perceptrons exhibited the best performance with a balanced accuracy of 0.898 (0.854–0.942) and a micro-averaged area under the receiver operating characteristic curve of 0.983 (0.980–0.986). Abstract The great advantage of additive manufacturing is the fact that hollowed parts with a given infill can be created. However, the standardized commercial slicer software offers a uniform infill pattern creation solution. In engineering practice, the manufactured parts are functional, therefore the appropriate load bearing capacity is mostly mandatory. In this paper a simplified local infill size optimization method has been presented. Based on a Finite Element Analysis the local density of the pattern can be adjusted, according to the emerged local stresses. The results show that independently of the pattern type, if the scaling was applied, the mechanical resistance was improved to the same extent. In case of the worst-performing uniform pattern, 84% improvement in mechanical resistance was achieved with the optimization. In addition, an FDM printing problem has been highlighted, which must be eliminated if the proposed method is used.

Abstract Building orientation optimization for Additive Manufacturing (AM) process is a crucial step because it has a vital effect on the accuracy and performance of the created part. Wire and Arc Additive Manufacturing’s (WAAM) working space is less limited, and the production time is significantly shorter than the other metal 3D printers. However, one of the adverse effects of WAAM is the defect at the start and endpoints of the welding beads. In this paper, an algorithm has been invented to define the optimal printing position, reducing the number of these defects by rotating the 3D object in a loop around the X and Y axes by a small constant degree and then selecting the degree of rotation that has the fewest uninterrupted surfaces and the largest area of the first layer. The welding process will be interrupted as little as possible by the torch if there are the fewest possible uninterrupted surfaces. As a result, there will be fewer defects in the production and finishing of the welding beads. In order to have a sufficient connection surface with the build tray, which will aid in holding the workpiece in place, the largest first layer should also be sought. Therefore, it has been found that a properly defined orientation relative to the build tray can reduce the number of uninterrupted surfaces within the layers, which will improve the expected dimensional accuracy of the parts. The efficiency of the process is highly affected by the shape of the part, but in most cases, the print errors can be drastically minimized.

Abstract After SARS-CoV-2 infection, strict recommendations for return-to-sport were published. However, data are insufficient about the long-term effects on athletic performance. After suffering SARS-CoV-2 infection, and returning to maximal-intensity trainings, control examinations were performed with vita-maxima cardiopulmonary exercise testing (CPET). From various sports, 165 asymptomatic elite athletes (male: 122, age: 20y (IQR: 17-24y), training:16 h/w (IQR: 12–20 h/w), follow-up:93.5 days (IQR: 66.8–130.0 days) were examined. During CPET examinations, athletes achieved 94.7 ± 4.3% of maximal heart rate, 50.9 ± 6.0 mL/kg/min maximal oxygen uptake (V̇O2max), and 143.7 ± 30.4L/min maximal ventilation. Exercise induced arrhythmias (n = 7), significant horizontal/descending ST-depression (n = 3), ischemic heart disease (n = 1), hypertension (n = 7), slightly elevated pulmonary pressure (n = 2), and training-related hs-Troponin-T increase (n = 1) were revealed. Self-controlled CPET comparisons were performed in 62 athletes: due to intensive re-building training, exercise time, V̇O2max and ventilation increased compared to pre-COVID-19 results. However, exercise capacity decreased in 6 athletes. Further 18 athletes with ongoing minor long post-COVID symptoms, pathological ECG (ischemic ST-T changes, and arrhythmias) or laboratory findings (hsTroponin-T elevation) were controlled. Previous SARS-CoV-2-related myocarditis (n = 1), ischaemic heart disease (n = 1), anomalous coronary artery origin (n = 1), significant ventricular (n = 2) or atrial (n = 1) arrhythmias were diagnosed. Three months after SARS-CoV-2 infection, most of the athletes had satisfactory fitness levels. Some cases with SARS-CoV-2 related or not related pathologies requiring further examinations, treatment, or follow-up were revealed.

IntroductionCatheter ablation is the preferred treatment for typical atrial flutter (AFl), but it can be challenging due to anatomical abnormalities. The use of 3D electroanatomical mapping systems (EAMS) has reduced fluoroscopy exposure during AFl ablation. Intracardiac echocardiography (ICE) has also shown benefits in reducing radiation exposure during AFl ablation. However, there is a lack of evidence on the feasibility of ICE-guided, zero-fluoroscopy AFl ablation without the use of EAMS.MethodsIn this prospective study, we enrolled 80 patients with CTI-dependent AFl. The first 40 patients underwent standard fluoroscopy + ICE-guided ablation (Standard ICE group), while the other 40 patients underwent zero-fluoroscopy ablation using only ICE (Zero ICE group). Procedure outcomes, including acute success, procedure time, fluoroscopy time, radiation dose, and complications, were compared between the groups.ResultsThe acute success rate was 100% in both groups. Out of the 40 cases, the zero-fluoroscopy strategy was successfully implemented in 39 cases (97.5%) in the Zero ICE group. There were no significant differences in procedure time [55.5 (46.5; 66.8) min vs. 51.5 (44.0; 65.5), p = 0.50] and puncture to first ablation time [18 (13.5; 23) min vs. 19 (15; 23.5) min, p = 0.50] between the groups. IntroductionCatheter ablation for atrial fibrillation (AF) is the most frequently performed cardiac ablation procedure worldwide. The majority of ablations can now be performed safely with minimal radiation exposure or even without the use of fluoroscopy, thanks to advances in 3-dimensional electroanatomical mapping systems and/or intracardiac echocardiography. The aim of this study was to conduct a meta-analysis to compare the effectiveness of zero fluoroscopy (ZF) versus non-zero fluoroscopy (NZF) strategies for AF ablation procedures.MethodsElectronic databases were searched and systematically reviewed for studies comparing procedural parameters and outcomes of ZF vs. NZF approaches in patients undergoing catheter ablation for AF. We used a random-effects model to derive the mean difference (MD) and risk ratios (RR) with a 95% confidence interval (CI).ResultsOur meta-analysis included seven studies comprising 1,593 patients. The ZF approach was found to be feasible in 95.1% of patients. Background: Ventricular tachycardia (VT) recurrence after catheter ablation remains a concern, emphasizing the need for precise risk assessment. We aimed to use machine learning (ML) to predict 1-month and 1-year VT recurrence following VT ablation. Methods: For 337 patients undergoing VT ablation, we collected 31 parameters including medical history, echocardiography, and procedural data. 17 relevant features were included in the ML-based feature selection, which yielded six and five optimal features for 1-month and 1-year recurrence, respectively. We trained several supervised machine learning models using 10-fold cross-validation for each endpoint. Results: We observed 1-month VT recurrence was observed in 60 (18%) cases and accurately predicted using our model with an area under the receiver operating curve (AUC) of 0.73. Input features used were hemodynamic instability, incessant VT, ICD shock, left ventricular ejection fraction, TAPSE, and non-inducibility of the clinical VT at the end of the procedure. A separate model was trained for 1-year VT recurrence (observed in 117 (35%) cases) with a mean AUC of 0.71. Selected features were hemodynamic instability, the number of inducible VT morphologies, left ventricular systolic diameter, mitral regurgitation, and ICD shock. For both endpoints, a random forest model displayed the highest performance. Conclusions: Our ML models effectively predict VT recurrence post-ablation, aiding in identifying high-risk patients and tailoring follow-up strategies.

Purpose: Electrocardiography (ECG)-derived machine learning models can predict echocardiography (echo)-derived indices of systolic or diastolic function. However, systolic and diastolic dysfunction frequently coexists, which necessitates an integrated assessment for optimal risk-stratification. We explored an ECG-derived model that emulates an echo-derived model that combines multiple parameters for identifying patient phenogroups at risk for major adverse cardiac events (MACE). Methods: In this substudy of a prospective, multicenter study, patients from 3 institutions (n = 727) formed an internal cohort, and the fourth institution was reserved as an external test set (n = 518). A previously validated patient similarity analysis model was used for labeling the patients as low-/high-risk phenogroups. These labels were utilized for training an ECG-derived deep neural network model to predict MACE risk per phenogroup. After 5-fold cross-validation training, the model was tested on the reserved external dataset. Results: Our ECG-derived model showed robust classification of patients, with area under the receiver operating characteristic curve of 0.86 (95% CI: 0.79–0.91) and 0.84 (95% CI: 0.80–0.87), sensitivity of 80% and 76%, and specificity of 88% and 75% for the internal and external test sets, respectively. The ECG-derived model demonstrated an increased probability for MACE in high-risk vs low-risk patients (21% vs 3%; P < 0.001), which was similar to the echo-trained model (21% vs 5%; P < 0.001), suggesting comparable utility. Conclusions: This novel ECG-derived machine learning model provides a cost-effective strategy for predicting patient subgroups in whom an integrated milieu of systolic and diastolic dysfunction is associated with a high risk of MACE.

We developed an image processing algorithm and applied it on high-speed camera recordings to characterize the deformation response of three-layered density-graded foam structures subjected to drop weight testing. Different densities (30, 40, 50 and 70 kg/m3) of weakly cross-linked polyethylene foam sheets were laminated together to achieve varying density distributions along the thickness, and the effect of layer order on the shock absorption capability was evaluated. Foam structures with a higher density top layer and a negative density gradient showed enhanced energy absorption in the initial stage of deformation, which resulted in lower maximum reaction forces. The positive effect of layer order modification was more dominant at higher impact energies. We provided a detailed explanation of the tendencies by investigating the differences in deformation propagation and the changes in the diameter of the deformation zone. The presented method can be utilized to design sports and packaging foam products.

Additive Manufacturing (AM) with the consisting constantly evolving technologies is a particularly popular research area. Based on the shape forming freedom, size, shape, and topology optimization techniques can be validated by AM produced parts. However, in every manufacturing process, AM also has some adverse inherent properties. One and maybe the most significant optimization problem is the mechanical anisotropy caused by the layered structure. In this paper, a simultaneous build orientation and shape optimization method is presented. Both of the approaches are intended to increase the mechanical performance of the produced parts. Shape optimization was accomplished by varying the cross-section of the beam geometries, based on the angle between a PSL section and the characteristic load direction. To test the efficiency and validate the method 2D structures (with relatively small 3rd dimension) and their tensile properties were tested. Based on the results, we can prove that the PSL method works and help to increase the mechanical performance by 19.2% with only 7.8% size increment.

IntroductionDespite the significant contribution of circumferential shortening to the global ventricular function, data are scarce concerning its prognostic value on long-term mortality. Accordingly, our study aimed to assess both left (LV) and right ventricular (RV) global longitudinal (GLS) and global circumferential strain (GCS) using three-dimensional echocardiography (3DE) to determine their prognostic importance.MethodsThree hundred fifty-seven patients with a wide variety of left-sided cardiac diseases were retrospectively identified (64 ± 15 years, 70% males) who underwent clinically indicated 3DE. LV and RV GLS, and GCS were quantified. To determine the prognostic power of the different patterns of biventricular mechanics, we divided the patient population into four groups. Group 1 consisted of patients with both LV GLS and RV GCS above the respective median values; Group 2 was defined as patients with LV GLS below the median while RV GCS above the median, whereas in Group 3, patients had LV GLS values above the median, while RV GCS was below median. Group 4 was defined as patients with both LV GLS and RV GCS below the median. Patients were followed up for a median of 41 months. The primary endpoint was all-cause mortality.ResultsFifty-five patients (15%) met the primary endpoint. Impaired values of both LV GCS (HR, 1.056 [95% CI, 1.027–1.085], p < 0.001) and RV GCS (1.115 [1.068–1.164], p < 0.001) were associated with increased risk of death by univariable Cox regression. Patients with both LV GLS and RV GCS below the median (Group 4) had a more than 5-fold increased risk of death compared with those in Group 1 (5.089 [2.399–10.793], p < 0.001) and more than 3.5-fold compared with those in Group 2 (3.565 [1.256–10.122], p = 0.017). Interestingly, there was no significant difference in mortality between Group 3 (with LV GLS above the median) and Group 4, but being categorized into Group 3 versus Group 1 still held a more than 3-fold risk (3.099 [1.284–7.484], p = 0.012).DiscussionThe impaired values of both LV and RV GCS are associated with long-term all-cause mortality, emphasizing the importance of assessing biventricular circumferential mechanics. Reduced RV GCS is associated with significantly increased risk of mortality even if LV GLS is preserved.

In this paper, we created a general mathematical equation for the cushion curves of polymeric foams, using the measurement results of drop weight impact tests on commercially available, closed-cell ethylene-vinyl acetate and cross-linked polyethylene foams of different densities. We found exponential relationship between the results of the drop tests and the static load, and investigated the effect of the input parameters on the fitted constants using analysis of covariance and multiple linear regression. In the second part of the study, we used a novel approach to theoretically determine the cushion curves of different thicknesses. We predicted material behavior using the Burgess method on the fitted exponential equations and assessed the limitations and accuracy of this extrapolation approach by comparing the measured and simulated data. We validated the results by quantifying the permanently damaged volume of the foams with the use of a 3D optical measuring system.

Pseudosinella hartnerae sp. nov. belongs to the group with 5+5 eyes, and can be identified by its unique labial chaetotaxy (M1m2rel1L2) within this group. Pseudosinella huetheri Stomp, 1971, the closest related species sharing the same dorsal macrochaetae formula (R0R1R2001/00/0101+2), is also redescribed here based on the holotype. Comparative analysis between the two species and among other related species is provided.

Abstract Background When MRI fails to detect a potentially epileptogenic lesion, the chance of a favorable outcome after epilepsy surgery becomes significantly lower (from 60 to 90% to 20–65%). Hybrid FDG-PET/MRI may provide additional information for identifying the epileptogenic zone. We aimed to investigate the possible effect of the introduction of hybrid FDG-PET/MRI into the algorithm of the decision-making in both lesional and non-lesional drug-resistant epileptic patients. Methods In a prospective study of patients suffering from drug-resistant focal epilepsy, 30 nonlesional and 30 lesional cases with discordant presurgical results were evaluated using hybrid FDG-PET/MRI. Results The hybrid imaging revealed morphological lesion in 18 patients and glucose hypometabolism in 29 patients within the nonlesional group. In the MRI positive group, 4 patients were found to be nonlesional, and in 9 patients at least one more epileptogenic lesion was discovered, while in another 17 cases the original lesion was confirmed by means of hybrid FDG-PET/MRI. As to the therapeutic decision-making, these results helped to indicate resective surgery instead of intracranial EEG (iEEG) monitoring in 2 cases, to avoid any further invasive diagnostic procedures in 7 patients, and to refer 21 patients for iEEG in the nonlesional group. Hybrid FDG-PET/MRI has also significantly changed the original therapeutic plans in the lesional group. Prior to the hybrid imaging, a resective surgery was considered in 3 patients, and iEEG was planned in 27 patients. However, 3 patients became eligible for resective surgery, 6 patients proved to be inoperable instead of iEEG, and 18 cases remained candidates for iEEG due to the hybrid FDG-PET/MRI. Two patients remained candidates for resective surgery and one patient became not eligible for any further invasive intervention. Conclusions The results of hybrid FDG-PET/MRI significantly altered the original plans in 19 of 60 cases. The introduction of hybrid FDG-PET/MRI into the presurgical evaluation process had a potential modifying effect on clinical decision-making. Cél: A tanulmány célja az incel mozgalom keletkezésének, ideológiájának, tagjai pszichéjének, a közösséghez fűződő relevánsabb elkövetéseknek és a köztük lévő összefüggéseknek, valamint a koronavírus mozgalomra gyakorolt hatásának bemutatása, végül lehetséges fellépési irányok megfogalmazása. Módszertan: Kutatásom során a témához fűződő, főleg angol nyelvű szakirodalmat tekintettem át, elemeztem, értékeltem és vetettem össze. Megállapítások: Mivel az incel ideológia az arra fogékony egyéneket hatékonyan képes radikalizálni, és az általa inspirált egyes támadások halálos áldozatainak száma magas, ezért a tanulmány arra a megállapításra jut, hogy a mozgalom szélsőséges tagjainak fenyegetése a szalafita dzsihádistákéhoz mérhető. Érték: A magyar szakirodalomban az incel mozgalom fenyegetése erősen alulkutatott, jelen tanulmány ezt pótolja, miközben a vizsgált fenyegetés elleni fellépést illetően javaslatokat is megfogalmaz.

One of the advantage of additive manufacturing technologies is the possibility to produce hollow section products, which are lighter, cheaper, faster to create and still they can have the required mechanical properties. Most of the times a 3D printed part's most important feature is the freedom of shape, and by setting the appropriate infill we can ensure the proper resilience as well. In this paper the different infill patterns and volume related percentages are compared by using the commercial Fused Deposition Modelling (FDM) technology. For the investigation non-standardized bending test were made with two loading orientation. From the results, the relation between the mass of the product and manufacturing time can be stated clearly, the pattern and percentage, as well as the decrease of the resilience in case the use of hollow areas in products.

Abstract Aims While global longitudinal strain (GLS) is considered to be a sensitive marker of left ventricular (LV) function, it is significantly influenced by loading conditions. We hypothesized that global myocardial work index (GMWI), a novel marker of LV function, may show better correlation with load‐independent markers of LV contractility in rat models of pressure‐induced or volume overload‐induced heart failure. Methods and results Male Wistar rats underwent either transverse aortic constriction (TAC; n = 12) or aortocaval fistula creation (ACF; n = 12), inducing LV pressure or volume overload, respectively. Sham procedures were performed to establish control groups (n = 12/12). Echocardiographic loops were obtained to determine GLS and GMWI. Pressure‐volume analysis with transient occlusion of the inferior caval vein was carried out to calculate preload recruitable stroke work (PRSW), a load‐independent ‘gold‐standard’ parameter of LV contractility. Myocardial samples were collected to assess interstitial and perivascular fibrosis area and also myocardial atrial‐type natriuretic peptide (ANP) and brain‐type natriuretic peptide (BNP) relative mRNA expression. The slaughter value of live cattle can be assessed during visual conformation scoring, as well as by examining different molecular genetic information, e.g., the myostatin gene, which can be responsible for muscle development. In this study, the F94L, Q204X, nt267, nt324 and nt414 alleles of the myostatin gene (MSTN) were examined in relation to birth weight (BIW), calving ease (CAE), 205-day weaning weight (CWW), muscle score of shoulder (MSS), muscle score of back (MSB), muscle score of thigh (MST), roundness score of thigh (RST), loin thickness score (LTS), and overall muscle development percentage (OMP) of Charolais weaned calves in Hungary. Multi-trait analysis of variance (GLM) and weighted linear regression analysis were used to process the data. Calves carrying the Q204X allele in the heterozygous form achieved approximately 0.14 points higher MSB, MST and LTS, and 1.2% higher OMP, and gained 8.56 kg more CWW than their counterparts not carrying the allele (p < 0.05). As for the F94L allele, there was a difference of 4.08 kg in CWW of the heterozygous animals, but this difference could not be proved statistically. The other alleles had no significant effect on the evaluated traits.

Abstract Aims The PREPARE‐MVR study (PRediction of Early PostoperAtive Right vEntricular failure in Mitral Valve Replacement/Repair patients) sought to investigate the alterations of right ventricular (RV) contraction pattern in patients undergoing mitral valve replacement/repair (MVR) and to explore the associations between pre‐operative RV mechanics and early post‐operative RV dysfunction (RVD). Methods and results We prospectively enrolled 42 patients (63 ± 11 years, 69% men) undergoing open‐heart MVR. Transthoracic three‐dimensional (3D) echocardiography was performed pre‐operatively, at intensive care unit discharge, and 6 months after surgery. The 3D model of the RV was reconstructed, and RV ejection fraction (RVEF) was calculated. We decomposed the motion of the ventricle to compute longitudinal ejection fraction (LEF) and radial ejection fraction (REF). Pulmonary artery catheterization was performed to monitor RV stroke work index (RVSWi). RVEF was slightly decreased after MVR [52 (50–55) vs. 51 (46–54)%; P = 0.001], whereas RV contraction pattern changed notably. Ancient trees are important habitats, confer vital ecological roles and function as cultural legacies. Old trees with large girth are keystone structures in various ecosystems. We aim to present which species amongst the greatest Hungarian trees (and some other phanerophyte plants) are damaged by polypores (the most important agents of wood decay), Agrobacterium tumefaciens (usually causing root tumour) or ivy (competing against the native vegetation and causing windthrow damage) and at what extent and frequency; and whether there is a relationship between these types of damage and the origin of the species (native or adventive) or its situation (solitary or surrounded by other trees). We measured 2,000 trees, belonging to 29 native and 43 non-native species. Polypore infection could be detected in 12.2% of the observed 531 settlements, 22.8% are damaged by Agrobacterium and 29.6% by ivy, while 51.2% by other types of pests and diseases. Altogether, one third of the observed 2000 ancient or veteran trees suffered from one or more types of damage. A total of 33.5% of the native species (519 specimens out of 1550) and 28.7% of the adventives (129 trees out of 450) are damaged by any (or more than one) of the mentioned infections or ivy. Mostly, damage occurred to those old trees that stand in a park or forest, while the single (solitary) trees were usually healthy. The most infected regions are the western and south-western counties, while the Northern Hungarian Mountain Range is much less affected, despite its great sample size. Low damage was detected in the Great Hungarian Plain, but the number of sample areas and veteran trees was also low here. The damage to old trees remains without any management or healing in Hungary, since the only effective solution would be prevention.
