Abstract

Markers of brittle faulting are widely used for recovering past deformation phases. Rocks often have oriented magnetic fabrics, which can be interpreted as connected to ductile deformation before cementation of the sediment. This paper reports a novel statistical procedure for simultaneous evaluation of AMS (Anisotropy of Magnetic Susceptibility) and fault-slip data.The new method analyzes the AMS data, without linearization techniques, so that weak AMS lineation and rotational AMS can be assessed that are beyond the scope of classical methods. This idea is extended to the evaluation of fault-slip data. While the traditional assumptions of stress inversion are not rejected, the method recovers the stress field via statistical hypothesis testing. In addition it provides statistical information needed for the combined evaluation of the AMS and the mesotectonic (0.1 to 10m) data. In the combined evaluation a statistical test is carried out that helps to decide if the AMS lineation and the mesotectonic markers (in case of repeated deformation of the oldest set of markers) were formed in the same or different deformation phases. If this condition is met, the combined evaluation can improve the precision of the reconstruction. When the two data sets do not have a common solution for the direction of the extension, the deformational origin of the AMS is questionable. In this case the orientation of the stress field responsible for the AMS lineation might be different from that which caused the brittle deformation. Although most of the examples demonstrate the reconstruction of weak deformations in sediments, the new method is readily applicable to investigate the ductile-brittle transition of any rock formation as long as AMS and fault-slip data are available., Comment: 23 pages, 5 figures

Abstract

No robust evidence is provided by literature regarding the management of intracranial hypertension following severe traumatic brain injury (TBI). This is mostly due to the lack of prospective randomized controlled trials (RCTs), the presence of studies containing extreme heterogeneously collected populations and controversial considerations about chosen outcome. A scientific society should provide guidelines for care management and scientific support for those areas for which evidence-based medicine has not been identified. However, RCTs in severe TBI have failed to establish intervention effectiveness, arising the need to make greater use of tools such as Consensus Conferences between experts, which have the advantage of providing recommendations based on experience, on the analysis of updated literature data and on the direct comparison of different logistic realities. The Italian scientific societies should provide guidelines following the national laws ruling the best medical practice. However, many limitations do not allow the collection of data supporting high levels of evidence for intracranial pressure (ICP) monitoring and decompressive craniectomy (DC) in patients with severe TBI. This intersociety document proposes best practice guidelines for this subsetting of patients to be adopted on a national Italian level, along with joint statements from "TBI Section" of the Italian Society of Neurosurgery (SINch) endorsed by the Neuroanesthesia and Neurocritical Care Study Group of the Italian Society of Anesthesia, Analgesia, Resuscitation and Intensive Care (SIAARTI). Presented here is a recap of recommendations on management of ICP and DC supported a high level of available evidence and rate of agreement expressed by the assemblies during the more recent consensus conferences, where members of both groups have had a role of active participants and supporters. The listed recommendations have been sent to a panel of experts consisting of the 107 members of the "TBI Section" of the SINch and the 111 members of the Neuroanesthesia and Neurocritical Care Study Group of the SIAARTI. The aim of the survey was to test a preliminary evaluation of the grade of predictable future adherence of the recommendations following this intersociety proposal. The following recommendations are suggested as representing best clinical practice, nevertheless, adoption of local multidisciplinary protocols regarding thresholds of ICP values, drug therapies, hemostasis management and perioperative care of decompressed patients is strongly recommended to improve treatment efficiency, to increase the quality of data collection and to provide more powerful evidence with future studies. Thus, for this future perspective a rapid overview of the role of the multimodal neuromonitoring in the optimal severe TBI management is also provided in this document. It is reasonable to assume that the recommendations reported in this paper will in future be updated by new observations arising from future trials. They are not binding, and this document should be offered as a guidance for clinical practice through an intersociety agreement, taking in consideration the low level of evidence.

Abstract

No robust evidence is provided by literature regarding the management of intracranial hypertension following severe traumatic brain injury (TBI). This is mostly due to the lack of prospective randomized controlled trials (RCTs), the presence of studies containing extreme heterogeneously collected populations and controversial considerations about chosen outcome. A scientific society should provide guidelines for care management and scientific support for those areas for which evidence-based medicine has not been identified. However, RCTs in severe TBI have failed to establish intervention effectiveness, arising the need to make greater use of tools such as Consensus Conferences between experts, which have the advantage of providing recommendations based on experience, on the analysis of updated literature data and on the direct comparison of different logistic realities. The Italian scientific societies should provide guidelines following the national laws ruling the best medical practice. However, many limitations do not allow the collection of data supporting high levels of evidence for intracranial pressure (ICP) monitoring and decompressive craniectomy (DC) in patients with severe TBI. This intersociety document proposes best practice guidelines for this subsetting of patients to be adopted on a national Italian level, along with joint statements from “TBI Section” of the Italian Society of Neurosurgery (SINch) endorsed by the Neuroanesthesia and Neurocritical Care Study Group of the Italian Society of Anesthesia, Analgesia, Resuscitation and Intensive Care (SIAARTI). Presented here is a recap of recommendations on management of ICP and DC supported a high level of available evidence and rate of agreement expressed by the assemblies during the more recent consensus conferences, where members of both groups have had a role of active participants and supporters. Abstract

BACKGROUND: Different procedures have been used for the treatment of lumbar juxtafacet cysts (JFCs). Recently, full-endoscopic cyst excision has been suggested as a reasonable alternative. We performed a meta-analysis to assess the overall rates of favorable outcomes and adverse events for each available treatment and determine the outcome and complication rates concerning spine stability. METHODS: Multiple databases were searched for English-language studies involving adult patients with lumbar JFCs who had been followed for more than 6 months. Outcomes included the proportion of patients with a satisfactory outcome. Adverse events included recurrence and revision rates as well as intraoperative complications. We further stratified the analysis based on the spine’s condition (degenerative listhesis vs without degenerative listhesis). RESULTS: A total of 43 studies, including 2226 patients, were identified. Over 80% of patients experienced satisfactory improvement after surgical excision but only 66.2% after percutaneous cyst rupture and aspiration. Overall, recurrence and revision rates were almost double in patients with preoperative degenerative listhesis at the cyst level, especially in the minimally invasive group (2.1% vs 31.3% and 6.8% vs 13.1%, respectively). Abstract

This study aims at the evaluation of the ability of the atmospheric mesoscale model of Weather Research and Forecasting (WRF) to predict heat wave episodes in the RMRJ, Brazil. The results obtained by using the WRF were compared with observed data of air temperature at 2 meters, showing a slight tendency of the simulated results to underestimate the observed temperatures. On the other hand, the results of the simulation adequately reproduced the daily temperature cycle. In the period of analysis, the observed data indicated values of maximum temperatures about 5 ° C above the climatological means of each sub-region of the RMRJ, which characterizes episodes of a heat wave. The synoptic analysis indicated that the temperature anomalies occurred as a consequence of the meteorological configuration in the synoptic scale of the ASAS in low levels and a high of geopotential in average levels of the troposphere. The study demonstrates that the meteorological characterization together with the computational modeling of the atmosphere is a potential strategy for urban planning and public management of urban areas, capable of detecting and predicting the area’s most susceptible to the occurrence of positive temperature anomalies in Megacities.

Abstract

This paper introduces a new technique called species conservation for evolving parallel subpopulations. The technique is based on the concept of dividing the population into several species according to their similarity. Each of these species is built around a dominating individual called the species seed. Species seeds found in the current generation are saved (conserved) by moving them into the next generation. Our technique has proved to be very effective in finding multiple solutions of multimodal optimization problems. We demonstrate this by applying it to a set of test problems, including some problems known to be deceptive to genetic algorithms.

Abstract

A significant number of patients with CNS infections require mehcanical ventilation. The indications for mechanical ventilation in cases of purulent meningitis are acure respiratory insufficiency and hyperventilation in patients with incereased intracranial pressure. The aim of mechanical ventilation is to achieve optimal pO2 and pCO2 in order to improve the perfusion and lower the intracranial pressure. We conducted a retrospective study with included 330 patients with purulent meningitis of which 115 (35.7%) were mechanically ventilated. The patients were treated in the Intensive Care Unit in the period between 1990-1998. Compared to previously studied periods we noticed a significantly higher number of intubated and artificially ventilated patients with purulent meningitides. Thacheotomy was performed in 45 (39.1%) of artificially ventilated patients. In spite of a more agressive approach and the use of sophisticated equipement, the lethality of purulent meningitis has not changed.
