Data are limited in Brazil on hepatitis B infection among homeless individuals, a marginalized population with high risk of sexually transmissible infections (STI), including hepatitis B. The aim of this study was to investigate hepatitis B epidemiology in homeless persons lodged in a public shelter in Goiânia, Central Brazil. From August 2014 to June 2015, 353 individuals were interviewed and tested for markers of HBV infection. Overall HBV prevalence was 21.8% (95%CI: 17,82-26,41), and 19,5% (95%CI: 15,75-24,0) showed a serological profile of previous HBV vaccination. Older individuals (> 50 years), blacks, and homosexuals or bisexuals showed increased exposure to HBV. The low frequency of individuals immunized against HBV, high social vulnerability, and risk behaviors emphasize the need for health services administrators to provide more opportunities for HBV vaccination in this target population. No Brasil, existem poucos dados sobre a infecção por hepatite B entre pessoas em situação de rua, uma população marginalizada com alto risco para infecções sexualmente transmissíveis (IST), incluindo a hepatite B. O objetivo deste estudo foi investigar a epidemiologia da hepatite B em pessoas alojadas em um abrigo público da cidade de Goiânia, Brasil Central. De agosto de 2014 a junho de 2015, 353 indivíduos foram entrevistados e testados para os marcadores da infecção pelo HBV. Uma prevalência global de 21,8% (IC95%: 17,82-26,41) para HBV foi estimado, e 19,5% (IC95%: 15,75-24,0) apresentaram perfil sorológico de vacinação prévia contra o HBV. Ser mais velho (acima de 50 anos de idade), preto e homossexual ou bissexual foram preditores de exposição ao HBV. A baixa frequência de indivíduos imunizados contra o HBV, bem como de alta vulnerabilidade social e ocorrência de comportamentos de risco reforçam a necessidade dos gestores de saúde proporcionar mais oportunidades de vacinação para esta população-alvo. El objetivo de este estudio fue investigar la epidemiología de la hepatitis B en personas alojadas en un albergue público de la ciudad de Goiânia, Brasil Central. De agosto de 2014 a junio de 2015, se entrevistó a 353 individuos y se probaron los marcadores de la infección por HBV. Se estimó una prevalencia global de un 21,8% (IC95%: 17,82-26,41) para HBV, y 19,5% (IC95%: 15,75-24,0) presentaron un perfil serológico de vacunación previa contra el HBV. Ser más viejo (por encima de 50 años de edad), negro y homosexual o bisexual fueron predictores de exposición al HBV. La baja frecuencia de individuos inmunizados contra el HBV, así como la alta vulnerabilidad social y ocurrencia de comportamientos de riesgo refuerzan la necesidad de los gestores de salud de proporcionar más oportunidades de vacunación para esta población-objetivo.

A cross-sectional study on prevalence, associated factors and genotype distribution of HCV infection was conducted among 848 HIV-infected patients recruited at reference centers in the Midwest Region of Brazil. The prevalence rate of HIV-HCV coinfection was 6.9% (95% CI: 5.2 to 8.6). In multivariable analysis, increasing age, use of illicit drugs (injection and non-injection), a history of blood transfusion before 1994, and the absence of a steady partnership were significant independent associated factors for HIV-HCV coinfection. The phylogenetic analysis based on the NS5B region revealed the presence of two major circulating genotypes of HCV: genotypes 1 (58.3%) and 3 (41.7%). The prevalence of HIV-HCV coinfection was lower than those reported in studies conducted with HIV-infected patients in different regions of Brazil, due to the fact that illicit drug use is not a frequent mode of HIV transmission in this region of Brazil. Serologic screening of HIV-patients for HCV before initiating antiretroviral treatment, a comprehensive identification of associated factors, and the implementation of effective harm reduction programs are highly recommended to provide useful information for treatment and to prevent HCV coinfection in these patients. Estudo transversal sobre a prevalência, fatores associados e distribuição dos genótipos do HCV foi realizado em 848 pacientes infectados pelo HIV, recrutados em centros de referência na Região Centro-Oeste do Brasil. A taxa de prevalência de coinfecção HIV-HCV foi de 6,9% (IC 95%: 5,2-8,6). Na análise multivariada, o aumento da idade, o uso de drogas ilícitas (injetáveis e não injetáveis), história de transfusão de sangue antes de 1994, e ausência de companheiro constante foram fatores associados independentes e significativos para a coinfecção HIV-HCV. A análise filogenética baseada na região NS5B revelou a presença de dois principais genótipos do HCV em circulação: genótipos 1 (58,3%) e 3 (41,7%). A prevalência da coinfecção HIV-HCV foi menor do que as relatadas em estudos realizados com pacientes infectados pelo HIV em diferentes regiões do Brasil, devido ao fato de que o uso de drogas ilícitas não é modo frequente de transmissão do HIV neste Estado do Brasil. Triagem sorológica de pacientes HIV-positivos para HCV antes de iniciar o tratamento antirretroviral, identificação completa dos fatores associados e a implementação de programas eficazes de redução de danos são altamente recomendados para fornecer informações úteis, para o tratamento e para evitar a coinfecção com HCV nestes pacientes.

INTRODUÇÃO: O presente estudo teve como objetivo investigar a prevalência da infecção pelo HBV e os fatores associados a esta infecção em população encarcerada de Campo Grande, MS. MÉTODOS: Quatrocentos e oito encarcerados, provenientes das populações encarceradas do Instituto Penal de Campo Grande, Presídio de Segurança Máxima e Presídio Feminino Irmã Irma Zorzi, foram entrevistados sobre dados sociodemográficos e fatores associados à infecção pelo HBV. A seguir foram coletadas amostras sanguíneas para detecção dos marcadores HBsAg, anti-HBs e anti-HBc total por ensaio imunoenzimático. O HBV-DNA foi detectado pela reação em cadeia da polimerase nas amostras HBsAg e anti-HBc reagentes. RESULTADOS: A prevalência global para infecção pelo HBV foi de 17,9% (IC 95%: 14,4-22,0), 13,7% (56/408) para o anti-HBc total associado ao anti-HBs e em 15 (3,7%) indivíduos foi detectada a presença do anti-HBc isolado. HBsAg foi encontrado em 0,5% (2/408) dos indivíduos estudados, evidenciando uma baixa prevalência de casos crônicos. Em 24% (98/408) dos indivíduos, verificou-se positividade isolada ao marcador anti-HBs, sugerindo baixa cobertura vacinal ao HBV. Após análise multivariada, ter idade maior que 35 anos, baixo nível de escolaridade e uso de droga ilícita permaneceram associados significativamente à infecção pelo HBV. CONCLUSÕES: A prevalência da infecção pelo HBV encontrada foi similar às reportadas em outros estudos conduzidos em populações encarceradas do Brasil. Os fatores de risco associados a esta infecção foram aumento da idade, baixo nível de escolaridade e uso de droga ilícita. ABSTRACT - INTRODUCTION: This study aimed to estimate the prevalence of HBV infection and associated factors among prison inmates in Campo Grande, MS. METHODS: A total of 408 individuals were interviewed regarding sociodemographic characteristics, associated factors and HBV vaccination using a standardized questionnaire. Blood samples were collected from all participants and serological markers for HBV were detected by enzyme-linked immunosorbent assay. Hepatitis B surface antigen (HBsAg) and/or antibodies against hepatitis B core antigen (anti-HBc) positive samples were tested for HBV-DNA by polymerase chain reaction. RESULTS: The overall prevalence of HBV infection was 17.9% (95%CI: 14.4-22.0). The HBsAg carrier rate was 0.5%; 56 (13.7%) individuals had been infected and developed natural immunity and 15 (3.7%) were positive for anti-HBc only. Ninety eight (24%) prisoners had only anti-HBs, suggesting that they had low vaccine coverage. An occult HBV infection rate of 0% was verified among anti-HBc-positive individuals. Multivariate analysis of associated factors showed that age > 35 years-old, low schooling level and illicit drug use are significantly associated with HBV infection. CONCLUSIONS: Analysis of the data showed HBV infection prevalence similar or slightly lower than that reported in other of Brazilian prisons. Independent predictors of HBV infection in this population include older age, low schooling level and illicit drug use.

O presente estudo teve como objetivo estimar a prevalência da infecção pelo HTLV-1 em remanescentes de comunidades de população negra, os quilombos no Brasil Central. 1.837 indivíduos foram avaliados, sendo nove soropositivos para HTLV-1/2 por ELISA. Todos esses foram positivos para HTLV-1 por Western blot e/ou PCR, resultando em uma prevalência de 0,5% (IC 95%: 0,2-1,0). A idade dos indivíduos infectados variou de 11 a 82 anos, sendo a maioria do sexo feminino. Quanto às características de risco, história de aleitamento materno, transfusão de sangue, múltiplos parceiros sexuais e doenças sexualmente transmissíveis foram relatadas por esses indivíduos. Os achados deste estudo evidenciam a importância da identificação dos indivíduos infectados pelo HTLV-1 na estratégia de controle e prevenção dessa infecção em remanescentes de quilombos. This study aimed to determine the prevalence of HTLV-1 infection among remnant black quilombo communities in Central Brazil. A total of 1,837 individuals were evaluated, among whom nine were HTLV-1/2 seropositive according to ELISA. All of them were positive for HTLV-1 by means of Western blot and/or PCR, thus resulting in a prevalence of 0.5% (95% CI: 0.2-1.0). The HTLV-1 infected individuals ranged in age from 11 to 82 years. The majority of them were females. Regarding risk characteristics, histories of breastfeeding, blood transfusion, multiple sexual partners and sexually transmitted diseases were reported by these individuals. The findings from this study indicate the importance of identifying HTLV-1 infected individuals, as a strategy for infection control and prevention in these remnant quilombos.

Were analyzed 648 serum samples from laboratory staff in Goiânia, Goiás aiming detection of three serological markers of HBV: HBsAg, anti-HBsAg and anti-HBcAg. The HBsAg and anti-HBcAg positive samples were also analyzed for HBeAg, anti-HBeAg and anti-HBcAgIgM markers. HBV infection rate of 24.1% was observed and, from them, 0.7% were positive for HBsAg. Viral DNA was detected by PCR in two HBsAg positive samples. A vaccination index of 74.5% and a global index of 89.9% of serological response to vaccination were observed. The direct work with biological fluids as well as cleaning workers represented significant risks for acquisition of HBV infection. The data from the present study showed an increase of the vaccination index among laboratory staff but the rates of HBV infection did not change through the years in the region. Neste estudo, foram analisadas 648 amostras de soro, provenientes de profissionais de laboratório de Goiânia-Goiás, visando a detecção de três marcadores sorológicos do VHB: HBsAg, anti-HBsAg e anti-HBcAg. As amostras HBsAg e anti-HBcAg positivas foram também analisadas para os marcadores HBeAg, anti-HBeAg e anti-HBcAgIgM. Foi observado um percentual de soropositividade para infecção pelo VHB de 24,1% sendo que 0,7% destes foram positivos para o HBsAg. O DNA viral foi detectado por PCR nas duas amostras HBsAg positivas. Dos 648 profissionais, 74,5% relataram vacinação para o VHB e, destes, 89,9% apresentaram soroconversão. Trabalho direto com fluidos biológicos bem como trabalho em serviços gerais representaram risco significativo para aquisição da infecção pelo VHB. Os resultados do estudo mostram que embora tenha havido um aumento no índice de vacinação entre os trabalhadores de laboratório, o índice de infecção pelo VHB manteve-se inalterado em relação ao tempo na região.

OBJETIVOS: O vírus da hepatite B representa um importante problema de saúde pública mundial e é responsável por altos índices de morbidade e mortalidade. Este estudo objetivou a determinação da soroprevalência da infecção pelo vírus da hepatite B (VHB) em pacientes internados em instituições psiquiátricas e em indivíduos com síndrome de Down, a detecção do DNA viral nas amostras positivas para HBsAg e anti-HBc total, e a caracterização dos subtipos do HBsAg. MÉTODOS: O estudo avaliou 433 indivíduos, sendo que 408 eram portadores de doença mental e internos em instituições psiquiátricas (71 deles tinham também dependência química) e 25 eram indivíduos com síndrome de Down, não institucionalizados. Amostras sangüíneas de todos os participantes foram testadas para os marcadores do VHB: HBsAg, anti-HBs e anti-HBc total pelo ensaio imunoenzimático (ELISA). Soros positivos ao HBsAg foram testados também para o anti-HBc IgM, HBeAg, anti-HBe, e subtipadas pela técnica de imunodifusão radial. A detecção do DNA-VHB foi realizada em amostras reagentes ao HBsAg e/ou anti-HBc total, utilizando-se a metodologia da reação em cadeia pela polimerase (PCR). RESULTADOS: Foi observado um índice global de soropositividade para o VHB de 22,4%, sendo que 1,6% dos indivíduos foram positivos para HBsAg. Dentre as amostras HBsAg positivas, cinco foram subtipáveis pela metodologia de imunodifusão radial, tendo sido observados os subtipos adw2, adw4 e ayw3. O DNA viral foi detectado, pela metodologia da PCR, em três amostras positivas para o HBsAg e em 11 amostras positivas para anti-HBc total/anti-HBs. No grupo de indivíduos com problema mental, com ou sem dependência química (N=408), a admissão hospitalar múltipla (reinternação) foi considerada fator de risco para infecção pelo VHB. CONCLUSÃO: Estes resultados indicam alta prevalência do VHB nesta população e reforçam a necessidade de medidas profiláticas específicas para reduzir o risco de infecção entre indivíduos com distúrbio mental e retardo mental. OBJECTIVES: Hepatitis B virus (HBV) infection is an important worldwide public health problem and it has been cause of elevated morbidity and mortality rates. The objectives of this study were determine the HBV infection seroprevalence in psychiatric institutions and in individuals with Down's syndrome, detect viral DNA in HBsAg and anti-HBc total positive serum samples and determine the HBsAg subtypes circulating these groups. METHODS: The study assessed 433 subjects, with 408 being mentally disordered inpatients (71 had also chemical dependence), and 25 were Down's syndrome outpatientes. Blood samples were collected and tested for HBV markers: HBsAg, anti-HBs and anti-HBc total by enzyme-linked immunosorbent assay (ELISA). HBsAg positive samples were also tested for anti-HBc IgM, HBeAg, anti-HBe, and subtyped by radial immunodifusion. HBV-DNA was investigated in HBsAg and/or anti-HBc total positive samples by PCR methodology. RESULTS: A global HBV positivity of 22.4% was detected. HBsAg was found in 1.6% of the samples. Among them, five were subtyped as adw2, adw4 and ayw3. DNA viral was found in 3 HBsAg samples and 11 HBsAg and anti-HBc total/anti-HBs positive samples, respectively. The risk factors analysis showed that multiple hospital admission were significantly associated with HBV markers. CONCLUSION: These results show high HBV seroprevalence in groups investigated and reinforce the importance of HBV specifics preventive measures to reduce the risk of hepatitis B in individuals with mental disturbs and retard.

The presence of serological markers for hepatitis B virus (HBsAg, anti-HBc IgM and Anti-HBc total) was investigated in the serum of 1,396 individuals who had clinical suspect of hepatitis. It was observed that 50.7% of the individuals were positive and, from the total of the studied individuals, 14.5% were positive for HBsAg. From these, 8.5% were also positive for anti-HBc IgM. The analysis in relation to gender showed a higher seroprevalence index among male individuals (p < 0.0001). It was observed the occurrence of subtypes adw2 (62.7%), ayw3 (23.5%), ayw2 (9.8%) and adw4 (3.9%). The viral DNA was detected in 61 (33.9%) HBsAg positive samples and in one sample positive only for anti-HBc total. These results indicate an important incidence of the HBV infection in this population, and reinforce previous studies regarding this virus in the central west region of Brazil. Investigou-se a presença dos marcadores sorológicos AgHBs, anti-HBc IgM e anti-HBc total no soro de 1.396 indivíduos com suspeita clínica de hepatite. Observou-se uma soroprevalência para a infecção pelo VHB de 50,7%, sendo que do total dos 1.396 indivíduos, 14,5% eram positivos para AgHBs e 8,5% eram também positivos para anti-HBc IgM, tendo ainda sido observado que a soroprevalência foi maior em indivíduos do sexo masculino (p < 0,0001). Por subtipagem foram identificados os seguintes subtipos: adw2 (62,7%), ayw3 (23,5%), ayw2 (9,8%) e adw4 (3,9%). O DNA viral foi detectado em 61 (33,9%) amostras positivas para o AgHBs e em uma amostra positiva somente para anti-HBc total. Estes resultados indicam importante índice de ocorrência da infecção pelo VHB nesta população e reforça dados de estudos anteriores a respeito da importante circulação do vírus na região Centro-Oeste do Brasil.

Para investigar o perfil da infecção pelo vírus da hepatite B (VHB) em profissionais de hemodiálise (N=152) de Goiânia, Goiás, amostras sangüíneas foram testadas para detecção dos marcadores: AgHBs, anti-HBs e anti-HBc. Uma prevalência global de 24,3% (IC 95%: 17,8 - 32) foi encontrada. A análise multivariada dos fatores de risco mostrou que o tempo de profissão, relato de exposição ocupacional e o não uso de equipamentos de proteção estiveram significativamente associados à soropositividade ao VHB. Dos 40 profissionais que apresentaram susceptibilidade a esta infecção, 20 concordaram em participar do programa de vacinação e, após a administração das três doses da vacina (Euvax-B), 18 (90%) apresentaram soroconversão ao anti-HBs com títulos > ou = 10UI/L. Estes dados sugerem, o ambiente dialítico, como possível fonte de transmissão ocupacional do VHB e, enfatizam a necessidade de reavaliação das medidas de controle e prevenção nestas unidades. In order to investigate the hepatitis B virus (HBV) infection profile in hemodialysis staff (n = 152) of Goiânia,Goiás, blood samples were tested for detection of HBsAg, anti-HBs and anti-HBc markers. An overall HBV infection prevalence of 24.3% (CI 95%: 17.8 - 32) was found. Multivariate analysis of risk factors showed that length of employment, history of occupational exposure and nonuse of protective equipment were significantly associated with HBV seropositivity. Of 40 staff members who were susceptible to this infection, 20 agreed to participate in the vaccination program. After three vaccine doses (Euvax-B), 18 (90%) seroconverted to anti-HBs with titers > or = 10 IU/L. These data suggest the dialytic environmental as a possible source of occupational transmission of HBV, and emphasize the need to evaluate strategies of control and prevention to be followed in these units.
