Abstract

Topography modulates air and soil temperature and gives rise to local climates that may strongly deviate from regional macroclimates. These local climatic conditions determine the distribution of many organisms but are hardly accounted for by coarse-grained macroclimate grids. Downscaling macroclimate grids or interpolating high-resolution climate surfaces from weather station data has thus become a staple in ecological research. Against this background, topographically complex territories pose a challenge, as downscaling and interpolation techniques are susceptible to high topographic heterogeneity. Simultaneously, these regions will act as refugia and preserve biodiversity amidst the challenges of climate change. This contrast renders choosing the technique to derive local climatic conditions under high topographic complexity critically relevant. In this study, we compared how interpolation and downscaling techniques predict the local climates of topographically heterogeneous territories. We interpolated monthly average temperatures from weather stations in a Southern Italy mountain region and downscaled WorldClim and CHELSA macroclimatic grids from their native 30 arcsecs to 10 m resolution. We carried out the interpolation and downscaling procedures by employing four commonly used statistical algorithms and by including physiographic descriptors (altitude, northness, eastness, distance to the coast, and monthly average of daily clear-sky insolation time) known to determine local climatic conditions. We compared the techniques' predictive performance via leave-one-out cross-validation. We extended the comparison over the study area to identify where the techniques most strongly diverged and evaluated how interpolation and downscaling techniques fare against environmental extrapolation. Although the interpolations scored the best values across all error metrics, interpolating weather station data and downscaling WorldClim yielded essentially equivalent predictions of local climates in the cross-validations. Differences between the two techniques emerged during the spatial comparison, where downscaling WorldClim produced biased and less detailed monthly climate surfaces. On the contrary, weather station interpolation was affected by environmental extrapolation in the study area's most internal and topographically heterogeneous landscapes. Downscaling CHELSA severely over-smoothed local climatic conditions during both cross-validations and spatial comparisons, possibly due to lacking climate variability in the native product itself. We demonstrated that technique and data source choice affect the prediction of local climates in topographically heterogeneous territories. Despite its limitations, we argue that interpolating weather station data better accounted for local-scale topographic effects. These effects form an integral part of the microrefugia that will shelter mountain communities from warming climates. 38. Two-dimensional transesophageal echocardiography assessment of the major aortic annulus diameter in patients undergoing transcatheter aortic valve replacement.

Abstract

Background: Multidetector computed tomography (MDCT) is the gold standard in annulus sizing before transcatheter aortic valve replacement (TAVR). However, MDCT has limited applicability in specific subgroups of patients, such as those with atrial fibrillation and chronic kidney disease. Two-dimensional transesophageal echocardiography (2DTEE) has traditionally been limited to the long-axis measurement of the anteroposterior diameter of the aortic annulus. We describe a new 2DTEE approach for the measurement of the major diameter of the aortic annulus. Methods: Seventy-six patients with symptomatic severe aortic valve stenosis and high surgical risk underwent MDCT and 2DTEE before TAVR. A modified five-chamber view was used to measure the major aortic annulus diameter. This was obtained starting from a mid-esophageal four chamber and retracting the TEE probe up until the left ventricular outflow tract and the left and noncoronary aortic cusps were visualized: major aortic annulus diameter was measured as the distance between their insertion points in systole. Results: Major aortic annulus diameters measured at 2DTEE showed good correlation with MDCT diameter (r = 0.79; P < 0.001) and perimeter (r = 0.87; P < 0.0001). Using factsheet-derived sizing criteria, 2DTEE alone would have allowed accurate sizing in 75% of patients, with 21% of oversizing predominantly with smaller annuli. Conclusions: We describe a new method for 2DTEE measurement of the major aortic annulus diameter; this approach is simple, correlates with MDCT, and allows adequate TAVR sizing in most patients. 40. Stand-Alone Thoracoscopic Left Atrial Appendage Closure in Nonvalvular Atrial Fibrillation Patients at High Bleeding Risk

Abstract

Objective In nonvalvular atrial fibrillation (AF) patients at high bleeding risk, oral anticoagulants (OAC) may be contraindicated, and percutaneous left atrial appendage (LAA) closure has been advocated. However, following percutaneous procedure, either OAC or dual antiplatelet treatment is required. In this study, we present our experience in treating nonvalvular AF patients at high bleeding risk with thoracoscopic LAA closure with no subsequent antithrombotic therapy.Methods From April 2019 to January 2020, 20 consecutive AF patients, mean age 75.1 years, 16 (80%) males, underwent thoracoscopic LAA closure as a stand-alone procedure, using an epicardial clip device. OAC and antiplatelet therapy were contraindicated. Mean CHA2DS2-VASc score was 3.61, and the mean HAS-BLED score was 4.42. Successful LAA closure was assessed by transesophageal echocardiography. Primary endpoints were complete LAA closure (no residual LAA flow), operative complications, and all-cause mortality; secondary endpoints were 30-day and 6-month complications (death, ischemic stroke, hemorrhagic stroke, transient ischemic attack, any bleeding). Mean follow-up was 6 ± 4 months.Results Complete LAA closure was achieved in all patients. No operative clip-related complications or deaths occurred. At follow-up, freedom from postoperative complications was 95% and from any cerebrovascular events was 100%. Overall survival rate was 100%.Conclusions In nonvalvular AF patients at high bleeding risk (HAS-BLED score >3), thoracoscopic LAA closure appears to be a valid alternative to percutaneous techniques not requiring dual antiplatelet or OAC treatment. Apparently, external LAA clipping minimizes the risk of thromboembolic events as compared with percutaneous procedures.

Abstract

Selenium is a trace element, nutritionally classified as an essential micronutrient, involved in maintaining the correct function of several enzymes incorporating the selenocysteine residue, namely the selenoproteins. The human selenoproteome including 25 proteins is extensively described here. The most relevant selenoproteins, including glutathione peroxidases, thioredoxin reductases and iodothyronine deiodinases are required for the proper cellular redox homeostasis as well as for the correct thyroid function, thus preventing oxidative stress and related diseases. This review summarizes the main advances on oxidative stress with a focus on selenium metabolism and transport. Moreover, thyroid-related disorders are discussed, considering that the thyroid gland contains the highest selenium amount per gram of tissue, also for future possible therapeutic implication.

43. Cytokines, Hepatic Fibrosis, and Antiretroviral Therapy Role in Neurocognitive Disorders HIV Related.

Abstract

The HIV may trigger a process of neuronal loss and axonal degeneration throughout the brain, which is carried on by the immune system releasing of proinflammatory cytokines, so that chronic inflammation associated with dysregulated innate immune response, glial cell dysfunction, and adverse antiretroviral therapy (ART) effect play an important role causing milder HIV-associated neurocognitive disorders or asymptomatic neurocognitive impairment. All patients have been tested for neurocognitive functioning through a comprehensive, five-domain neuropsychological battery performed in the study. Human cytokine (interleukin [IL]-6, IL-8, IL-18, and tumor necrosis factor [TNF]-α) and brain-derived neurotrophic factor serum levels were quantified using ELISAs, and the hepatic fibrosis was estimated using the noninvasive Fibrosis 4 (FIB-4) score. The study showed a group of 40 HIV-infected individuals and it was observed that almost 40% of HIV+ individuals, even if clinically asymptomatic, displayed some degree of neurocognitive dysfunction, compared to normative performance standards, at least in two cognitive areas. The functions affected the most were memory, attention, executive function, and psychomotor processing speed. Three cytokines (IL-6, IL-8, and IL-18) to be significantly linked to test results in specific neurocognitive domain were found. Treatments with nucleoside reverse transcriptase inhibitor plus non-nucleoside reverse transcriptase inhibitor alone were instead associated with poor neurocognitive outcome, especially in verbal fluency, fine motility, and Zung Depression Scale. Elevated value of FIB-4 score showed an opposite connection with cognitive performance as well, underlining the direct association between hepatic steatosis and neurocognitive deficit. The cytokine panel and the FIB-4 score can predict presence or worsening of neurocognitive functions in HIV-infected individuals. An ART switch can be suggested according to the neurocognitive domain involved the most, advising a therapy with protease inhibitors or/and integrase inhibitors to improve fluency, executive functions, and to prevent depression. 44. Cardiovascular biomarkers predict fragility fractures in older adults

Abstract

ObjectiveTo assess the role of four biomarkers of neuroendocrine activation and endothelial dysfunction in the longitudinal prediction of fragility fractures.MethodsWe analysed a population-based prospective cohort of 5415 community-dwelling individuals (mean age, 68.9±6.2 years) enrolled in the Malmo¨ Preventive Project followed during 8.1±2.9 years, and investigated the longitudinal association between C-terminal pro-arginine vasopressin (CT-proAVP), C-terminal endothelin-1 precursor fragment (CT-proET-1), the mid-regional fragments of pro-adrenomedullin (MR-proADM) and pro-atrial natriuretic peptide (MR-proANP), and incident vertebral, pelvic and extremity fractures.ResultsOverall, 1030 (19.0%) individuals suffered vertebral, pelvic or extremity fracture. They were older (70.7±5.8 vs 68.4±6.3 years), more likely women (46.9% vs 26.3%), had lower body mass index and diastolic blood pressure, were more often on antihypertensive treatment (44.1% vs 38.4%) and had more frequently history of fracture (16.3% vs 8.1%). Higher levels of MR-proADM (adjusted HR (aHR) per 1 SD: 1.51, 95% CI 1.01 to 2.28, p<0.001) and MR-proANP (aHR: 1.23, 95% CI 1.05 to 1.45, p<0.001) were independently associated with increased risk of any fracture. The fracture risk increased linearly across MR-proANP quartiles. Individuals who were in the top quartile of all four biomarkers had a significant higher risk of fracture at any site (aHR: 2.32, 95% CI 1.86 to 2.91), vertebral fracture (aHR: 3.16, 95% CI 1.97 to 5.07) and femoral fracture (aHR: 2.35, 95% CI 1.64 to 3.36).ConclusionsElevated levels of MR-proADM and MR-proANP independently predict fragility fractures in older adults. In subjects with top quartile levels of all four biomarkers there is a twofold to threefold increase in risk of vertebral and femoral fractures.

Abstract

In recent years, increasing surgical experience in high-volume centers, greater consistency in surgical technique, and the support of sophisticated electromedical navigation tools have increased the success treatment of atrial fibrillation ablation to 80% after the first procedure and to > 90% after the second procedure. Moreover, these results are associated with a progressive decrease in the incidence of complications. According to the data in Table 1, taken from a study published in 2003-2004 [1], major complications associated with the ablation of all pulmonary veins outside the tubular segment occurred in 2.9% of cases, as reported by the six leading centers that have adopted this approach. 49. Concentrations of Contaminants with Regulatory Limits in Samples of Clam (Chamelea gallina) Collected along the Abruzzi Region Coast in Central Italy

Abstract

Concentrations of pollutants with regulatory limits were determined in specimens of Chamelea gallina,a species of clam collected along the Abruzzi coastal region of the central Adriatic Sea. Nine sampling sites were selected to evaluate the distribution of contaminants in the environment and the health risk for consumers. The concentrations of all the examined compounds were lower than the maximums set by European legislation. Polycyclic aromatic hydrocarbons and total mercury were below the detection limit (0.18 μg/kg forbenzo[a]anthracene, 0.30 μg/kg for chrysene, 0.12 μg/kg for benzo[b]fluoranthene, 0.08 μg/kg for benzo[a]pyrene, and 0.0050 mg/kg for total mercury) in all the analyzed samples. Mean concentrations of lead and cadmium were 0.104 and 0.110 mg/kg, respectively. Of the non-dioxin-like polychlorinated biphenyls, PCB-153, PCB-180, and PCB-138 were the most abundant at all sampling sites (1a to 9a) at 0.25 mi (ca. 0.4 km) and at some sampling sites (1b, 2b, 3b, 5b and 7b) at 0.35 mi (ca. 0.56 km). Principal component analysis revealed that the concentrations of dioxin-like polychlorinated biphenyls were similar at the majority of sampling sites, and O8CDD and 2, 3, 7, 8-T4CDF were the predominant dioxin congeners.

Abstract

The XYY syndrome is a sex chromosome aneuploidy occurring in one of 1,000 live male births. Only few data exist regarding the correlation between this syndrome and epilepsy. An EEG pattern suggestive of benign focal epilepsy with centro‐temporal spikes has recently been described in four XYY patients. We report the first patient with XYY trisomy, rolandic spikes, and atypical evolution with continuous spikes and waves during slow sleep (CSWSS). The present report suggests that the association between an EEG pattern similar to that of BECTS and 47, XYY karyotype may not be coincidental. Moreover, we show that an atypical evolution with CSWSS may occur in this chromosomal disorder.
