Abstract

ITER envisages the use of two heating neutral beam injectors plus an optional one as part of the auxiliary heating and current drive system. The 16.5 MW expected neutral beam power per injector is several notches higher than worldwide existing facilities. A Neutral Beam Test Facility (NBTF) was established at Consorzio RFX, exploiting the synergy of two test beds, SPIDER and MITICA. SPIDER is dedicated to developing and characterizing large efficient negative ion sources at relevant parameters in ITER-like conditions: source and accelerator located in the same vacuum where the beam propagates, immunity to electromagnetic interferences of multiple radio-frequency (RF) antennas, avoidance of RF-induced discharges on the outside of the source. Three years of experiments on SPIDER have addressed to the necessary design modifications to enable full performances. The source is presently under a long shut-down phase to incorporate learnings from the experimental campaign. Parallelly, developments on MITICA, the full-scale prototype of the ITER NBI featuring a 1 MV accelerator and ion neutralization, are underway including manufacturing of in-vessel components, while power supplies and auxiliary plants are already under final testing and commissioning. Integration, commissioning and tests of the 1MV power supplies are essential for this first-of-kind system, unparalleled both in research and industry field. The integrated test to confirm 1MV output by combining invertor systems, DC generators and transmission lines extracted errors/accidents in some components. To realize a concrete system for ITER, solutions for the repair and the improvement of the system were developed. Hence, NBTF is emerging as a necessary facility, due to the large gap with existing injectors, effectively dedicated to identify issues and find solutions to enable successful ITER NBI operations in a time bound fashion.

Abstract

Among several approaches to tackle the problem of energy consumption in modern computing systems, two solutions are currently investigated: one consists of artificial neural networks (ANNs) based on photonic technologies, the other is a different paradigm compared to ANNs and it is based on random networks of nonlinear nanoscale junctions resulting from the assembling of nanoparticles or nanowires as substrates for neuromorphic computing. These networks show the presence of emergent complexity and collective phenomena in analogy with biological neural networks characterized by self-organization, redundancy, non-linearity. Starting from this background, we propose and formalize a generalization of the perceptron model to describe a classification device based on a network of interacting units where the input weights are nonlinearly dependent. We show that this model, called "receptron", provides substantial advantages compared to the perceptron as, for example, the solution of non-linearly separable Boolean functions with a single device. The receptron model is used as a starting point for the implementation of an all-optical device that exploits the non-linearity of optical speckle fields produced by a solid scatterer. By encoding these speckle fields we generated a large variety of target Boolean functions without the need for time-consuming machine learning algorithms. We demonstrate that by properly setting the model parameters, different classes of functions with different multiplicity can be solved efficiently. The optical implementation of the receptron scheme opens the way for the fabrication of a completely new class of optical devices for neuromorphic data processing based on a very simple hardware.

Abstract

Background and objectives: mRNA vaccines elicit a durable humoral response to SARS-CoV-2 in adults, whereas evidence in children is scarce. This study aimed to assess the early and long-term immune response to the mRNA vaccine in children with or without previous SARS-CoV-2 infection. Methods: In a multicentre prospective observational study, we profiled the immune response to the Pfizer BioNTech (BNT162b2) vaccine in 5-11-year-old children attending the University Pediatric Hospital of Padua and Bambino-Gesu Hospital in Rome (Italy) from December-2021 to February-2023. Blood samples were collected pre-, 1-, and 6-months after vaccination. Neutralizing antibodies (NAbs) and anti-spike-receptor-binding-domain (anti-S-RBD) IgG titers were analyzed through Plaque Reduction Neutralization Test (PRNT) and chemiluminescent immune-enzymatic assay (CLIA), respectively. Immune cell phenotypes were analyzed by flow cytometry. Results: Sixty children (26 [43 %] female, median age = 8 years [IQR = 7-10.7]) were enrolled in the study, including 46 children with a laboratory-confirmed previous COVID-19 (SARS-CoV-2-recovered) and 14 SARS CoV-2-naive participants defined as the absence of antigen-specific antibodies before vaccination. SARS-CoV-2recovered participants recorded higher anti-S-RBD IgG and Wild-type and Omicron BA.2 NAbs titers than SARSCoV-2-naive participants at both 1- and 6-months after vaccination. Antibody titers correlated with T (Tregs) and B (Bregs) regulatory cell frequencies in SARS-CoV-2-recovered children. Both SARS-CoV-2-recovered and SARSCoV-2-naive participants decreased antibody titers by approximately 100 to 250 % from 1 to 6 months. While children with immunocompromising underlying conditions developed immune responses comparable to those of healthy children, solid organ transplant recipients exhibited lower levels of NAbs and anti-S-RBD IgG titers, as well as reduced frequencies of Tregs and Bregs. Abstract

A retrospective survey was conducted in hematologic centres of the Rete Ematologica Lombarda (REL) on 529 older AML patients seen between 2020–2022. Compared to 2008–2016, the use of intensive chemotherapy (ICT) decreased from 40% to 18.1% and of hypomethylating agents (HMAs) from 19.5% to 13%, whereas the combination of Venetoclax/HMA, initially not available, increased from 0% to 36.7%. Objective treatment-specific fitness criteria proposed by SIE/SIES/GITMO in 2013 allow an appropriate choice between ICT and HMAs by balancing their efficacy and toxicity. Venetoclax/HMA, registered for patients unfit to ICT, has a unique toxicity profile because of prolonged granulocytopenia and increased infectious risk. Aiming at defining specific fitness criteria for the safe use of Venetoclax/HMA, a preliminary investigation was conducted among expert REL hematologists, asking for modifications of SIE/SIES/GITMO criteria they used to select candidates for Venetoclax/HMA. While opinions among experts varied, a general consensus emerged on restricting SIE/SIES/GITMO criteria for ICT-unfit patients to an age limit of 80–85, cardiac function > 40%, and absence of recurrent lung infections, bronchiectasis, or exacerbating COPD. Also, the presence of an adequate caregiver was considered mandatory. Such expert opinions may be clinically useful and may be considered when treatment-specific fitness criteria are updated to include Venetoclax/HMA.

Abstract

Amid efforts to address energy consumption in modern computing systems, one promising approach takes advantage of random networks of non-linear nanoscale junctions formed by nanoparticles as substrates for neuromorphic computing. These networks exhibit emergent complexity and collective behaviors akin to biological neural networks, characterized by self-organization, redundancy, and non-linearity. Based on this foundation, a generalization of n-inputs devices has been proposed, where the associated weights depend on all the input values. This model, called receptron, has demonstrated its capability to generate Boolean functions as output, representing a significant breakthrough in unconventional computing methods. In this work, we characterize and present two actual implementations of this paradigm. One approach leverages the nanoscale properties of cluster-assembled Au films, while the other utilizes the recently introduced Stochastic Resistor Network (SRN) model. We first provide a concise overview of the electrical properties of these systems, emphasizing the insights gained from the SRN regarding the physical processes within real nanostructured gold films at a coarse-grained scale. Furthermore, we present evidence indicating the minimum complexity level required by the SRN model to achieve a stochastic dynamics adequate to effectively model a novel component for logic systems. To support our argument that these systems are preferable to conventional random search algorithms, we discuss quantitative criteria based on Information-theoretic tools. This suggests a practical means to steer the stochastic dynamics of the system in a controlled way, thus focusing its random exploration where it is most useful. [ABSTRACT FROM AUTHOR]

Abstract

Searches for a stochastic gravitational-wave background (SGWB) using terrestrial detectors typically involve cross-correlating data from pairs of detectors. The sensitivity of such cross-correlation analyses depends, among other things, on the separation between the two detectors: the smaller the separation, the better the sensitivity. Hence, a co-located detector pair is more sensitive to a gravitational-wave background than a non-co-located detector pair. However, co-located detectors are also expected to suffer from correlated noise from instrumental and environmental effects that could contaminate the measurement of the background. Hence, methods to identify and mitigate the effects of correlated noise are necessary to achieve the potential increase in sensitivity of co-located detectors. Here we report on the first SGWB analysis using the two LIGO Hanford detectors and address the complications arising from correlated environmental noise. We apply correlated noise identification and mitigation techniques to data taken by the two LIGO Hanford detectors, H1 and H2, during LIGO's fifth science run. At low frequencies, 40 - 460 Hz, we are unable to sufficiently mitigate the correlated noise to a level where we may confidently measure or bound the stochastic gravitational-wave signal. However, at high frequencies, 460-1000 Hz, these techniques are sufficient to set a $95%$ confidence level (C.L.) upper limit on the gravitational-wave energy density of \Omega(f)<7.7 x 10^{-4} (f/ 900 Hz)^3, which improves on the previous upper limit by a factor of $\sim 180$. In doing so, we demonstrate techniques that will be useful for future searches using advanced detectors, where correlated noise (e.g., from global magnetic fields) may affect even widely separated detectors., Comment: 21 pages, 10 figures, 5 tables

Abstract

ITER envisages the use of two heating neutral beam injectors plus an optional one as part of the auxiliary heating and current drive system, to reach the desired performances during its various phases of operation. The 16.5 MW expected neutral beam power per injector is several notches higher than worldwide existing facilities. In order to enable such development, a Neutral Beam Test Facility (NBTF) was established at Consorzio RFX, exploiting the synergy of two test beds, called SPIDER and MITICA. SPIDER is dedicated developing and characterizing large efficient negative ion sources at relevant parameters in ITER-like conditions: source and accelerator located in the same vacuum where the beam propagates, immunity to electromagnetic interferences of multiple radio-frequency (RF) antennas, avoidance of RF-induced discharges on the outside of the source. Three years of experiments on SPIDER have addressed to the necessary design modifications to enable full performances. The source is presently under a long shut-down phase to incorporate learnings from the experimental campaign, in particular events/issues occurred during operation, which led to the identification of improvement opportunities/necessities (e.g. RF discharges, local burns, water leaks, other damages, configuration/design upgrades to maximize chances/margin to quest target parameters). Parallelly, developments on MITICA, the full-scale prototype of the ITER Neutral Beam Injector (NBI) featuring a 1 MV accelerator and ion neutralization, are underway including manufacturing of the beam source, accelerator and the beam line components, while power supplies and auxiliary plants, already installed, are under final testing and commissioning. Abstract

We present the results of a search for variable stars in the globular cluster NGC 5286, which has recently been suggested to be associated with the Canis Major dwarf spheroidal galaxy. 57 variable stars were detected, only 19 of which had previously been known. Among our detections one finds 52 RR Lyrae (22 RRc and 30 RRab), 4 LPV's, and 1 type II Cepheid of the BL Herculis type. Periods are derived for all of the RR Lyrae as well as the Cepheid, and BV light curves are provided for all the variables. The mean period of the RRab variables is = 0.656 days, and the number fraction of RRc stars is N(c)/N(RR) = 0.42, both consistent with an Oosterhoff II (OoII) type -- thus making NGC 5286 one of the most metal-rich ([Fe/H] = -1.67; Harris 1996) OoII globulars known to date. The minimum period of the \RRab's, namely Pab,min = 0.513 d, while still consistent with an OoII classification, falls towards the short end of the observed Pab,min distribution for OoII globular clusters. As was recently found in the case of the prototypical OoII globular cluster M15 (NGC 7078), the distribution of stars in the Bailey diagram does not strictly conform to the previously reported locus for OoII stars. We provide Fourier decomposition parameters for all of the RR Lyrae stars detected in our survey, and discuss the physical parameters derived therefrom. The values derived for the RRc's are not consistent with those typically found for OoII clusters, which may be due to the cluster's relatively high metallicity -- the latter being confirmed by our Fourier analysis of the ab-type RR Lyrae light curves. Abstract

The rechallenge strategy is based on the concept that a subset of patients with RAS wild-type (WT) metastatic colorectal cancer (mCRC) could still benefit of epidermal growth factor receptor (EGFR) inhibition, after progression to an anti-EGFR based-therapy. We performed a pooled analysis of two-phase II prospective trials to determine the role of rechallenge in third-line mCRC patients with RAS/BRAF WT baseline circulating tumor DNA (ctDNA). Individual data of 33 and 13 patients from CAVE and CRICKET trials that received as third-line therapy cetuximab rechallenge were collected. Overall survival (OS), Progression-free survival (PFS), Overall response rate (ORR), Stable disease (SD) > 6 months were calculated. Adverse events were reported. For the whole 46 patient population, median PFS (mPFS) was 3.9 months (95% Confidence Interval, CI 3.0-4.9) with median OS (mOS) of 16.9 months (95% CI 11.7-22.1). For CRICKET patients, mPFS was 3.9 months (95% CI 1.7-6.2); mOS was 13.1 months (95% CI 7.3-18.9) with OS rates at 12, 18, and 24 months of 62%, 23%, and 0%, respectively. For CAVE patients, mPFS was 4.1 months (95% CI 3.0-5.2); mOS was 18.6 months (95% CI 11.7-25.4) with OS rates at 12, 18, 24 months of 61%, 52%, 21%, respectively. Skin rash was more frequently reported in CAVE trial (87.9% vs. 30.8%; p = 0.001), whereas a increased incidence of hematological toxicities was observed in CRICKET trial (53.8%% vs. 12.1%; p = 0.003). Third-line cetuximab rechallenge in combination with either irinotecan or avelumab in RAS/BRAF WT ctDNA mCRC patients represents a promising therapy.

Abstract

Patients choice is at the core of competition in hospital care. In spite of a flourishing literature, little is known about the true process leading patients to choose a specific provider. Most models in this literature assume without testing that hospitals are perceived to be homogeneous providers. In this article we take a different direction. We assume that patients have a bias towards some types of hospitals, we test this hypothesis and show which attributes determine this bias. We exploit the characteristic of Italian health care organization, where devolution has allowed regional systems to choose the level of competition and the private-public hospitals mix. We estimate conditional logit models for hip replacement admissions in three regions (Lombardy, Veneto, and Emilia-Romagna) over the period 2014-2016. We show that, depending on the competition framework (peculiar to each region) patients are aware that some hospitals are best performers in their area and are willing to travel more to be admitted there. This is particularly true for regional health care systems where competition between public and private providers is well developed. Our model provides interesting policy implications: a) the idea that hospitals are different in patients perception should be kept in mind in the architecture of the market for hospital care; b) clinical quality as a driver to patients choice seems to work better in a less regulated competition settings

Abstract

BackgroundTreatment of calcified (in contrast to simple) lesions with PTCA has been associated with a lower success rate and more procedural complications. Rotablation can improve acute results, but the high restenosis rate remains a problem. The purpose of this study was to evaluate the clinical and angiographic outcome of patients with complex and calcified lesions treated with a combination of rotablation and stenting.Methods and resultsSeventy-five consecutive patients with 106 lesions had rotablation prior to coronary stenting. Intravascular ultrasound-guided stenting was used without subsequent anticoagulation in 93% of patients. Procedural success was achieved in 93.4% of lesions. Acute stent thrombosis occurred in two lesions (1.9%), and subacute stent thrombosis in one lesion (0.9%). Angiographic follow-up was performed in 82.5% of lesions at 4.6 +/- 1.9 months with an angiographic restenosis rate of 22.5%. Clinical follow-up was performed in all patients at 6.4 +/- 3 months; target lesion revascularization was needed in 18% of lesions; Q-wave myocardial infarction occurred in 1.3%, coronary bypass surgery in 4.0%, and death in 1.3%.ConclusionsOptimal coronary stenting after rotablation in calcified and complex lesions can be performed with a high success rate, an acceptable rate of procedural complications, and a low rate of stent thrombosis. This approach was associated with a low incidence of angiographic restenosis compared with results usually obtained with other interventional strategies in calcified and complex lesion subsets.

Abstract

BackgroundThe placement of stents in coronary arteries has been shown to reduce restenosis in comparison to balloon angioplasty. However, clinical use of intracoronary stents is impeded by the risk of subacute stent thrombosis and complications associated with the anticoagulant regimen. To reduce these complications, the hypothesis that systemic anticoagulation is not necessary when adequate stent expansion is achieved was prospectively evaluated on a consecutive series of patients who received intracoronary stents.Methods and resultsFrom March 1993 to January 1994, 359 patients underwent Palmaz-Schatz coronary stent insertion. After an initial successful angiographic result with < 20% stenosis by visual estimation had been achieved, intravascular ultrasound imaging was performed. Further balloon dilatation of the stent was guided by observation of the intravascular ultrasound images. All patients with adequate stent expansion confirmed by ultrasound were treated only with antiplatelet therapy (either ticlopidine for 1 month with short-term aspirin for 5 days or only aspirin) after the procedure. Clinical success (procedure success without early postprocedural events) at 2 months was achieved in 338 patients (94%). With an inflation pressure of 14.9 +/- 3.0 atm and a balloon-to-vessel ratio of 1.17 +/- 0.19, optimal stent expansion was achieved in 321 of the 334 patients (96%) who underwent intravascular ultrasound evaluation, with these patients receiving only antiplatelet therapy after the procedure. Despite the absence of anticoagulation, there were only two acute stent thromboses (0.6%) and one subacute stent thrombosis (0.3%) at 2-month clinical follow-up. Follow-up angiography at 3 to 6 months documented two additional occlusions (0.6%) at the stent site. At 6-month clinical follow-up, angiographically documented stent occlusion had occurred in 5 patients (1.6%). At 6-month clinical follow-up, there was a 5.7% incidence of myocardial infarction, a 6.4% rate of coronary bypass surgery, and a 1.9% incidence of death. Emergency intervention (emergency angioplasty or bailout stent) for a stent thrombosis event was performed in 3 patients (0.8%). The overall event rate was relatively high because of intraprocedural complications that occurred in 16 patients (4.5%). Intraprocedural complications, however, decreased to 1% when angiographically appropriately sized balloons were used for final stent dilations. There was one ischemic vascular complication that occurred at the time of the procedure and one ischemic vascular complication that occurred at the time of angiographic follow-up. By 6 months, repeat angioplasty for symptomatic restenosis was performed in 47 patients (13.1%).ConclusionsThe Palmaz-Schatz stent can be safely inserted in coronary arteries without subsequent anticoagulation provided that stent expansion is adequate and there are no other flow-limiting lesions present. The use of high-pressure final balloon dilatations and confirmation of adequate stent expansion by intravascular ultrasound provide assurance that anticoagulation therapy can be safely omitted. This technique significantly reduces hospital time and vascular complications and has a low stent thrombosis rate.
