1. Observations of umbral flashes in the resonant sunspot chromosphere

Abstract

In sunspot umbrae, the core of some chromospheric lines exhibits periodic brightness enhancements known as umbral flashes. The consensus is that they are produced by the upward propagation of shock waves. This view has recently been challenged by the detection of downflowing umbral flashes and the confirmation of the existence of a resonant cavity above sunspots. We aim to determine waves' propagating or standing nature in the low umbral chromosphere and confirm or refute the existence of downflowing umbral flashes. Spectroscopic temporal series of Ca II 8542 \AA, Ca II H, and Halpha in a sunspot were acquired with the Swedish Solar Telescope. The Halpha velocity was inferred using bisectors. Simultaneous inversions of the Ca II 8542 \AA\ line and the Ca II H core were performed using the NICOLE code. The nature of the oscillations and insights into the resonant oscillatory pattern were determined by analyzing the phase shift between the velocity signals and examining the temporal evolution. Propagating waves in the low chromosphere are more common in regions with frequent umbral flashes, where the transition region is shifted upward, making resonant cavity signatures less noticeable. In contrast, areas with fewer umbral flashes show velocity fluctuations that align with standing oscillations. Evidence suggests dynamic changes in the location of velocity resonant nodes due to variations in transition region height. Downflowing profiles appear at the onset of some umbral flashes, but upflowing motion dominates during most of the flash. These downflowing flashes are more common in standing umbral flashes. We confirm the existence of a chromospheric resonant cavity above sunspot umbrae produced by wave reflections at the transition region. Abstract

Solar partially ionized plasma is frequently modeled using single-fluid (1F) or two-fluid (2F) approaches. In the 1F case, charge-neutral interactions are often described through ambipolar diffusion, while the 2F model fully considers charge-neutral drifts. Here, we expand the definition of the ambipolar diffusion coefficient to include inelastic collisions (ion/rec) in two cases: a VAL3C 1D model and a 2F simulations of the Rayleigh-Taylor instability (RTI) in a solar prominence thread based on \cite{PopLukKho2021aa, PopLukKho2021ab}. On one side, we evaluate the relative importance of the inelastic contribution, compared to elastic and charge-exchange collisions. On the other side, we compare the contributions of ion/rec, thermal pressure, viscosity, and magnetic forces to the charge-neutral drift velocity of the turbulent flow of the RTI. Our analysis reveals that the contribution of inelastic collisions to the ambipolar diffusion coefficient is negligible across the chromosphere, allowing the classical definition of this coefficient to be safely used in 1F modeling. However, in the transition region, the contribution of inelastic collisions can become as significant as that of elastic collisions. Furthermore, we ascertain that the thermal pressure force predominantly influences the charge-neutral drifts in the RTI model, surpassing the impact of the magnetic force., Comment: Accepted

Abstract

The Mancha3D code is a versatile tool for numerical simulations of magnetohydrodynamic processes in solar/stellar atmospheres. The code includes non-ideal physics derived from plasma partial ionization, a realistic equation of state and radiative transfer, which allows performing high quality realistic simulations of magneto-convection, as well as idealized simulations of particular processes, such as wave propagation, instabilities or energetic events. The paper summarizes the equations and methods used in the Mancha3D code. It also describes its numerical stability and parallel performance and efficiency. The code is based on a finite difference discretization and memory-saving Runge-Kutta (RK) scheme. It handles non-ideal effects through super-time stepping and Hall diffusion schemes, and takes into account thermal conduction by solving an additional hyperbolic equation for the heat flux. The code is easily configurable to perform different kinds of simulations. Several examples of the code usage are given. It is demonstrated that splitting variables into equilibrium and perturbation parts is essential for simulations of wave propagation in a static background. A perfectly matched layer (PML) boundary condition built into the code greatly facilitates a non-reflective open boundary implementation. Spatial filtering is an important numerical remedy to eliminate grid-size perturbations enhancing the code stability. Parallel performance analysis reveals that the code is strongly memory bound, which is a natural consequence of the numerical techniques used, such as split variables and PML boundary conditions. Both strong and weak scalings show adequate performance up till several thousands of CPUs.

Abstract

Several generalizations of the well-known fluid model of Braginskii (Rev. of Plasma Phys., 1965) are considered. We use the Landau collisional operator and the moment method of Grad. We focus on the 21-moment model that is analogous to the Braginskii model, and we also consider a 22-moment model. Both models are formulated for general multi-species plasmas with arbitrary masses and temperatures, where all the fluid moments are described by their evolution equations. The 21-moment model contains two "heat flux vectors" (3rd and 5th-order moments) and two "viscosity-tensors" (2nd and 4th-order moments). The Braginskii model is then obtained as a particular case of a one ion-electron plasma with similar temperatures, with de-coupled heat fluxes and viscosity-tensors expressed in a quasi-static approximation. We provide all the numerical values of the Braginskii model in a fully analytic form (together with the 4th and 5th-order moments). For multi-species plasmas, the model makes calculation of transport coefficients straightforward. Formulation in fluid moments (instead of Hermite moments) is also suitable for implementation into existing numerical codes. It is emphasized that it is the quasi-static approximation which makes some Braginskii coefficients divergent in a weakly-collisional regime. Importantly, we show that the heat fluxes and viscosity-tensors are coupled even in the linear approximation, and that the fully contracted (scalar) perturbations of the 4th-order moment, which are accounted for in the 22-moment model, modify the energy exchange rates. We also provide several Appendices, which can be useful as a guide for deriving the Braginskii model with the moment method of Grad., Comment: Updated version. Abstract

The worldwide prevalence of insufficient physical activity (PA) and prolonged sedentary behavior (SB) were high before the coronavirus (COVID-19) pandemic. Measures that were taken by governments (such as home confinement) to control the spread of COVID-19 may have affected levels of PA and SB. This cross-sectional study among South American adults during the first months of COVID-19 aims to (i) compare sitting time (ST), screen exposure, moderate PA (MPA), vigorous PA (VPA), and moderate-to-vigorous PA (MVPA) before and during lockdown to sociodemographic correlates and (ii) to assess the impact of lockdown on combinations of groups reporting meeting/not-meeting PA recommendations and engaging/not-engaging excessive ST (≥7 h/day). Bivariate associations, effect sizes, and multivariable linear regressions were used. Adults from Argentina (n = 575) and Chile (n = 730) completed an online survey with questions regarding demographics, lifestyle factors, and chronic diseases. Mean reductions of 42.7 and 22.0 min./day were shown in MPA and VPA, respectively; while increases of 212.4 and 164.3 min./day were observed in screen and ST, respectively. Those who met PA recommendations and spent <7 h/day of ST experienced greatest changes, reporting greater than 3 h/day higher ST and more than 1.5 h/day lower MVPA. Findings from the present study suggest that efforts to promote PA to South American adults during and after COVID-19 restrictions are needed.

17. Radial to axillary nerve transfer

Abstract

Axillary nerve injury is common after brachial plexus injuries, particularly with shoulder luxation. Nerve grafting is the traditional procedure for postganglionic injuries. Nerve transfer is emerging as a viable option particularly in late referrals. At the proximal arm the radial and axillary nerves lie close by. Sacrificing one of the triceps muscle nerve branches induces little negative consequences. Transferring the long head of the triceps nerve branch is a good option to recover axillary nerve function. Abstract

Repair of complete brachial plexus avulsion injuries may require contralateral C7 nerve root transfer. The available techniques might allow direct neuroraphy in about 50 % of cases but the others require interposing nerve grafts or humeral shaft shortening. We aimed to see if transdiscal C6–C7 contralateral C7 nerve root transfer is technically feasible and if it allows direct coaptation with the contralateral nerve roots in 100 % of cases. In ten fresh-frozen adult cadavers, the C7 nerve root was sectioned just before it connects with other brachial plexus branches and re-routed though the C6–C7 disc space to the contralateral side. A complete C6–C7 discectomy was performed and the disc space kept open with the aid of an autologous iliac crest bone graft. Transdiscal C6–C7 contralateral C7 nerve root transfer is technically feasible. In our cadavers, it provided 5.3 ± 1.2 SDcm of extra length that allowed direct coaptation with the contralateral nerve roots, mainly C8 and T1. Transdiscal C6–C7 contralateral C7 nerve root transfer is technically feasible. In our dissections it lengthens the available C7 nerve root stump by 5.3 ± 1.2SDcm. The increase was 4 cm versus the retropharyngeal route making direct coaptation with the contralateral C8 and T1 nerve roots possible.

Abstract

Tools for measuring walking time make use of objective and subjective methods. One subjective approach is to administer physical activity questionnaires (PAQ), mainly because they are inexpensive and easy to give to large groups. The European Prospective Investigation into Cancer and Nutrition (EPIC) study has a brief PAQ (EPIC-PAQ) and includes one question referencing walking time. The objective of this study was to assess the validity of the question about time spent walking included in the EPIC-PAQ. The sample included 200 older adults (113 women). To assess daily walking time, participants responded to the EPIC-PAQ in an interview and wore a portable gait analysis system and physical activity monitor for 48 consecutive hours in free-living condition. Results indicated that the mean of bias between the EPIC-PAQ and objetive measurement was −64.6 min/day. Also, the correlation was low compared to an objective measurement (rho = 0.196) and was positively correlated with the time spent at speeds below 2.5 mph but the correlation was low (slow walking rho = 0.154 and pace walking rho = 0.163). The EPIC-PAQ shows low correlations with the objective measurement of walking time, that suggests it may be inaccurate and affecting the estimate of the EPIC-PAQ's PA energy expenditure in this age group. Abstract

Objective: Atherogenesis involves an inflammatory process that occurs early in life even though clinical symptoms are not observed until adulthood. Two important protective factors for low-grade inflammation may be physical activity (PA) and fitness. We examined the independent associations of objective and subjective measurements of PA and fitness with low-grade inflammation in European adolescents. Methods: A total of 1045 adolescents, aged from 12.5 to 17.5 years old from 10 European cities, were selected from the HELENA-Cross-Sectional Study. Objectively-measured and self-reported PA variables were obtained by accelerometry and the International PA Questionnaire for Adolescents, respectively. Overall, cardiorespiratory, muscular and motor fitness variables were assessed by standardized field-based fitness tests and the International Fitness Scale. C-reactive protein (CRP), complement factors 3 (C3) and 4 (C4), interleukin-6 and TNF-α inflammatory markers were measured. Results: Objectively-measured vigorous PA was inversely associated with C3 (β= -0.094, P= 0.021) but it did not remain significant after any objective fitness indicator was included in the model. Other objectively measured or self-reported assessments of PA were not significantly associated with inflammatory markers. All objective measures of fitness were inversely associated with CRP, C3 and C4, whereas only self-reported motor fitness remained significantly associated with C3, C4 and TNF-α. All these observations were independent of age, sex, city and body mass index or waist circumference. Conclusion: High PA in adolescence may play an indirect role on lessening low-grade inflammation through improvements in fitness. © 2011 Elsevier Ireland Ltd.

Abstract

Background: The aims of this study were to examine what amount of s edentary time is associated with low cardiorespiratory fitness (CRF) in adolescents and whether this association is independent of physical activity. Methods: The study comprised 1808 adolescents aged 12.5-17.5 years from 10 European cities. Sedentary time and moderate-to-vigorous physical activity (MVPA) were measured by accelerometer. CRF was assessed by the 20 m shuttle-run test. Adolescents were divided into two groups (high/low) according to FITNESSGRAM guidelines. Receiver-operating characteristic (ROC) analysis was used to determine thresholds that best discriminate between high and low CRF in adolescents. Results: Adolescent girls had more sedentary time than boys (p0.05) in those who met the recommendation (≥60 min/day in MVPA). Conclusion: Excessive sedentary time is associated with low CRF in adolescent girls but not in boys. However, this adverse effect might be attenuated if adolescent girls meet the current physical activity guidelines.

Abstract

Our aim in this study was to examine the association between breastfeeding duration and cardiorespiratory fitness, isometric strength, and explosive strength during adolescence. A total of 2567 adolescents (1426 girls) from the Healthy Lifestyle in Europe by Nutrition in Adolescence (HELENA) cross-sectional study aged 12.5-17.5 y were included. Information about duration of any and exclusive breastfeeding was obtained retrospectively by means of a parental questionnaire. The 20-m shuttle run, handgrip strength, and standing long jump tests were used to assess physical fitness. Significant differences among the categories of breastfeeding duration were tested using ANCOVA after adjusting for a set of potential confounders: gestational and current age, birth weight, sexual maturation, fat mass, fat-free mass, maternal education, parental weight status, country, smoking behavior, and days of vigorous physical activity. Longer breastfeeding (either any or exclusive) was associated with a higher performance in the standing long jump test in both boys and girls (P < 0.001), regardless of fat mass, fat-free mass, and the rest of potential confounders. In adolescents who were breastfed for 3-5 mo or ≥6 mo, the risk of having a standing long jump performance below the 5th percentile was reduced by half compared with those who were never breastfed [odds ratio (OR) = 0.54, 95% CI = 0.30-0.96, P < 0.05; and OR = 0.40, 95% CI = 0.22-0.74, P < 0.01, respectively). These findings suggest a role of breastfeeding in determining lower body explosive strength during adolescence.

Abstract

Objectives: The purpose of this study was to determine the sex-specific physical activity (PA) intensity thresholds that best discriminate between unhealthy/healthy cardiorespiratory fitness (CRF). Methods: Participants included 1,808 adolescents (964 girls), aged 12.5–17.5 years, from the HELENA study. We measured PA by accelerometer and calculated the time spent at light, moderate, vigorous, and moderate-to-vigorous (MVPA) intensities. CRF was assessed by the 20-m shuttle-run test. Adolescents were dichotomized (unhealthy/healthy) based on sex- and age-specific FITNESSGRAM standards. Receiver operating characteristic (ROC) analysis was used to determine thresholds that best discriminate between CRF categories. Results: ROC analyses revealed that the PA thresholds that best discriminate between unhealthy/healthy CRF were ≥152, ≥33, ≥13, and ≥52 min/day in light, moderate, vigorous, and MVPA, respectively. In boys, the PA thresholds associated with a healthy CRF were ≥37, ≥19, and ≥56 min/day in moderate, vigorous, and MVPA, respectively, whereas in girls were ≥152, ≥34, ≥12, and ≥51 min/day in light, moderate, vigorous, and MVPA, respectively. Spending at least 60 min/day in MVPA was also associated with a healthy CRF (odds ratios: 1.75, 1.94, and 1.57, all P < 0.05, for the whole sample, boys, and girls, respectively). Conclusions: This study shows sex- and intensity-specific PA thresholds to discriminate between adolescents with a healthy CRF from those with a less favorable or unhealthy CRF level. Spain has reached one of the highest OW/OB prevalence rates among adolescents from European countries. The aim of this methodological paper is to describe the design and evaluation in the EVASYON study (Development, implementation and evaluation of the efficacy of a therapeutic programme for adolescents with OW/OB: integral education on nutrition and physical activity). Methods/Design: The EVASYON was planned by a multidisciplinary team to treat OW/OB in Spanish adolescents. The EVASYON is a multi-centre study conducted in 5 hospitals in 5 Spanish cities (Granada, Madrid, Pamplona, Santander and Zaragoza) and two hundred and four OW/OB Spanish adolescents were recruited for this intervention. The treatment was implemented for approximately one-year follow-up. The adolescents were treated in groups of a maximum of 10 subjects; each group had 20 visits during the treatment period in two phases: intensive during the first 2 months (1st to 9th visits), and extensive during the last 11 months (10th to 20th visits). In order to assess the efficacy of the treatment, 8 dimensions were measured: diet; physical activity and fitness; eating behaviour; body composition; haematological profile; metabolic profile; minerals and vitamins; immuno-inflammatory markers. Moreover, genetic polymorphisms were also determined. 29. Quiste coloide hemorrágico del tercer ventrículo: deterioro fulminante

Abstract

Introduccion. Los quistes coloides son tumores benignos del tercer ventriculo. La mayoria permanecen asintomaticos. Sin embargo, algunos pacientes pueden desarrollar desde cefaleas intermitentes hasta un deterioro agudo e incluso muerte subita. Existen varias teorias por las que se produciria una muerte subita en estos pacientes, entre las que se incluyen el rapido crecimiento del quiste, su rotura, la alteracion del control cardiovascular reflejo mediado por el hipotalamo y, rara vez, el sangrado del quiste, con solo 15 casos descritos en la bibliografia. Caso clinico. Varon de 45 anos, con hipertension arterial como unico antecedente de interes, que sufre un deterioro brusco del nivel de consciencia a causa de una hidrocefalia aguda debida a un quiste coloide hemorragico del tercer ventriculo. Se implanto un drenaje ventricular externo en cada lado e ingreso en la unidad de cuidados intensivos, donde se certifico la muerte encefalica. Tras la extirpacion del quiste mediante abordaje transcortical frontal, se confirmo el diagnostico de quiste coloide con restos de material hemolizado subagudo. Conclusion. La hemorragia en los quistes coloides es excepcional y puede producirse en pacientes tanto sintomaticos como asintomaticos, lo que dificulta, de forma extraordinaria, el reconocimiento de esta complicacion.

30. Sedentary behaviors and emerging cardiometabolic biomarkers in adolescents

Abstract

Objective: To examine the associations of objectively measured sedentary time and television (TV) viewing time with emerging inflammatory and endothelial function markers in adolescents. Study design: This study comprised 183 adolescents (88 girls), aged 13 to 17 years. Sedentary time and moderate-to-vigorous physical activity was objectively measured with accelerometry, whereas TV viewing time was self-reported. White blood cell counts and levels of C-reactive protein, complement factors C3 and C4, interleukin-6, adiponectin, leptin, intercellular adhesion molecule 1, vascular cell adhesion molecule 1, E-selectin, L-selectin, and plasminogen activator inhibitor-1 were measured in fasted blood samples. Results: Sedentary time was not significantly associated with any of the examined cardiometabolic markers after controlling for potential confounders. However, TV viewing time was positively associated with soluble endothelial adhesion molecules intercellular adhesion molecule 1 (standardized ß = 0.19, P =.008), vascular cell adhesion molecule 1 (ß = 0.17, P =.020), L-selectin (ß = 0.18, P =.013), and E-selectin (ß = 0.16, P =.023) concentrations, after controlling for sex, age, pubertal status, moderate-to-vigorous physical activity, body mass index, and total sedentary time. Abstract

Background: Excessive television (TV) viewing might play an important role in the development of cardiovascular disease (CVD). The aim of this study was to examine the independent associations between TV viewing and CVD risk factors in adolescents.Methods: A sample of 425 adolescents, aged 13- to 18.5-year-old, was included in this study. Body mass index (BMI), waist circumference (WC), glucose, total cholesterol, triglycerides, HDL-cholesterol, LDL-cholesterol, apolipoprotein (apo) A-1, apo B-100, and lipoprotein(a) levels were determined. A composite CVD risk score was computed based on age-, sex-, sexual maturation- and race-standardized triglycerides, HDL-cholesterol, LDL-cholesterol and glucose. TV viewing was self-reported.Results: Two hundred and twenty-five adolescents (53%) who spent >3 hrs/day watching TV were considered as the "high TV viewing" group. Ninety-nine adolescents (23%) from the total sample were classified as overweight according to International age- and sex-specific BMI values. The high TV viewing group had significantly less favorable values of HDL-cholesterol, glucose, apo A1 and CVD score, independent of age, sex, sexual maturation, race and weight status. There was a significant interaction effect of TV viewing x weight status (P = 0.002) on WC, and the negative influence of TV viewing on WC persisted in the overweight group (P = 0.031) but was attenuated in non-overweight adolescents (P > 0.05).Conclusion: Excessive TV viewing seems to be related to an unfavorable CVD risk factors profile in adolescence. Reducing TV viewing in overweight adolescents might be beneficial to decrease abdominal body fat. [ABSTRACT FROM AUTHOR]

Abstract

The worldwide prevalence of insufficient physical activity (PA) and prolonged sedentary behavior (SB) were high before the coronavirus (COVID-19) pandemic. Measures that were taken by governments (such as home confinement) to control the spread of COVID-19 may have affected levels of PA and SB. This cross-sectional study among South American adults during the first months of COVID-19 aims to (i) compare sitting time (ST), screen exposure, moderate PA (MPA), vigorous PA (VPA), and moderate-to-vigorous PA (MVPA) before and during lockdown to sociodemographic correlates and (ii) to assess the impact of lockdown on combinations of groups reporting meeting/not-meeting PA recommendations and engaging/not-engaging excessive ST (≥7 h/day). Bivariate associations, effect sizes, and multivariable linear regressions were used. Adults from Argentina (n = 575) and Chile (n = 730) completed an online survey with questions regarding demographics, lifestyle factors, and chronic diseases. Mean reductions of 42.7 and 22.0 min./day were shown in MPA and VPA, respectively, while increases of 212.4 and 164.3 min./day were observed in screen and ST, respectively. Those who met PA recommendations and spent &lt, 7 h/day of ST experienced greatest changes, reporting greater than 3 h/day higher ST and more than 1.5 h/day lower MVPA. Findings from the present study suggest that efforts to promote PA to South American adults during and after COVID-19 restrictions are needed.

Background: Only a few studies have investigated the role of diet on the risk of chronic kidney disease (CKD) in European populations and have mainly focused on the Mediterranean diet. This is the first study to evaluate the association between various diet quality indices and CKD incidence in British adults., Methods: A prospective cohort with 106,870 participants from the UK Biobank, followed from 2009 to 2012 through 2021. Food consumption was obtained from ≥2 24-h dietary assessments. Dietary patterns were assessed using previously established indices: Alternate Mediterranean Index (aMED), Alternative Healthy Eating Index 2010, dietary approaches to stop hypertension (DASH), healthful plant-based diet index (hPDI), unhealthful plant-based diet index (uPDI), and dietary inflammatory index (DII). Incident CKD was obtained from clinical records, death registries, and self-reports. Analyses were performed with Cox regression models and adjusted for the main confounders., Results: After a median follow-up of 9.27 y, 2934 cases of CKD were ascertained. Hazard ratios (95% confidence interval) of CKD for the highest compared with lowest tertile of adherence to each diet score were 0.84 (0.76, 0.93) for aMED, 0.94 (0.85, 1.03) for alternative healthy eating index 2010, 0.77 (0.70, 0.85) for DASH, 0.79 (0.72, 0.87) for hPDI, 1.27 (1.16, 1.40) for uPDI, and 1.20 (1.18, 1.33) for DII. Abstract

Importance: Physical activity (PA) guidelines recommend the same amount of PA through adulthood to live longer., Objective: To explore whether there is an age-dependent association between PA and all-cause mortality and to investigate the age-dependent associations between other modifiable health factors (high educational level, not smoking, not regularly consuming alcohol, healthy body weight, and living without hypertension and diabetes) and mortality., Design, Setting, and Participants: This cohort study used a pooled analysis of 4 population-based prospective cohorts (National Health Interview Survey, 1997-2018; UK Biobank, 2006-2010; China Kadoorie Biobank, 2004-2008; and Mei Jau, 1997-2016). Data were analyzed from June 2022 to September 2024., Exposures: Self-reported leisure-time PA., Main Outcomes and Measures: The primary outcome was deaths identified through follow-up linkage to national death registries. Analyses were performed for the total sample and by age groups (20-29, 30-39, 40-49, 50-59, 60-69, 70-79, and ≥80 years). Cox proportional hazards regression models with stratification by study were used to calculate mortality hazard ratios and their 95% CIs for the pooled dataset and by age group., Results: A heterogeneous sample of 2 011 186 individuals (mean [SD] age, 49.1 [14.3] years; age range, 20-97 years; 1 105 581 women [55.0%]) were included. After a median (IQR) follow-up of 11.5 (9.3-13.5) years, 177 436 deaths occurred. The association between PA and mortality in the total sample showed a nonlinear dose-response pattern, but age modified this association (P for interaction <.001); PA was consistently associated with a lower risk of mortality across all age groups, but the reduction in risk was greater in older vs younger age groups, especially at high levels of PA. The hazard ratio for mortality associated with meeting the recommended PA in the total sample was 0.78 (95% CI, 0.77-0.79). This inverse association between meeting PA recommendations and mortality was somewhat greater as age increased (P for interaction <.001). Age also modified the associations of the other modifiable health factors with mortality (all P for interaction <.001), but the magnitude of associations was greater in younger vs older age groups., Conclusions and Relevance: In this pooled analysis of cohort studies, the association between PA and mortality risk remained consistent across the adult lifespan, which contrasts with other modifiable health factors, for which associations with mortality risk diminished with age. Given these findings, the promotion of regular PA is essential at all stages of adult life.

45. Associations of nighttime sleep, midday napping, and physical activity with all-cause mortality in older adults: the Seniors-ENRICA cohorts.

Abstract

We examined associations between nighttime sleep and midday nap duration with all-cause mortality according to adherence to moderate-to-vigorous physical activity (MVPA) recommendations in older adults. We used data from 3518 and 3273 older adults recruited in the Seniors-ENRICA-1 and 2 cohorts, respectively. Nighttime sleep time was classified as short (< 7 h), middle (≥ 7 and < 8 h), and long (≥ 8 h), and midday napping as no nap, short (≤ 30 min), long (> 30 and ≤ 60 min), and very long (> 60 min). Time spent in MVPA was classified as meeting (i.e., ≥ 150 min/week) or not the MVPA recommendations. All-cause mortality was the main outcome. Analyses were performed with Cox regression and adjusted for the main confounders. Overall, compared with middle nighttime sleep, both short (HR 1.20; 95% CI 1.03-1.39) and long (HR 1.30; 95% CI 1.12-1.52) were associated with higher mortality. Among participants who did not meet MVPA recommendations, the association between short (HR 1.22; 95% CI 1.01-1.48) and long (HR 1.46; 95% CI 1.21-1.77) sleeping duration mortality remained. Short napping was associated with lower mortality (HR 0.83; 95% CI 0.71-0.96) and very long with higher mortality (HR 1.29; 95% CI 1.04-1.59), compared to those who did not nap. Among participants not meeting MVPA recommendations, only very long napping was associated with increased mortality (HR 1.32; 95% CI 1.01-1.73). No associations were identified between nighttime sleep or midday napping with mortality among participants meeting MVPA recommendations. Abstract

Objective: To examine the trends in the association between meeting the physical activity (PA) guidelines and mortality in adults., Methods: We included seventeen annual representative samples of US adults 1998-2014 (n = 482,756) and all-cause and cause-specific mortality ascertained through December 2019. Participants were grouped according to PA Guidelines: 150 or more min/week in aerobic PA and muscle-strengthening activities 2 or more times/week. To provide further context, we also examined the trends in mortality risk associated with other modifiable health factors., Results: Meeting the PA guidelines was associated with lower 5-year mortality risk (HR = 0.59, 95%CI, 0.55, 0.63) based on the pooled analyses. We consistently observed an inverse association in all years, but there was a nonsignificant trend association (P for trend = 0.305) between meeting PA guidelines and 5-year mortality across the seventeen annual surveys. Meeting aerobic (HR = 0.58, 95%CI, 0.56, 0.61) and muscle-strengthening (HR = 0.86, 95%CI, 0.81, 0.90) guidelines were independently associated with 5-year mortality risk in pooled analyses, without any evidence for trends in the associations. Similar results were found with cause-specific mortality and 10-year mortality risk. In pooled analyses, attaining a high educational level, body mass index <30 kg/m2, being noncurrent smoker, nonheavy drinker, and living without history of hypertension and diabetes with 5-year mortality were 0.70 (95%CI, 0.67, 0.73), 1.19 (95%CI, 1.15, 1.23), 0.56 (95%CI, 0.54, 0.59), 0.85 (95%CI,0.79, 0.92), 0.91 (95%CI, 0.88-0.94) and 0.65 (95%CI, 0.88, 0.94), respectively. Only no history of diabetes showed a significant trend analysis (B = 0.77, 95%CI, 0.46, 0.91, P for trend <0.001)., Conclusion: Meeting PA guidelines lower mortality risk and this association does not seem to have varied over time. Abstract

Background: Growth Differentiation Factor 15 (GDF-15) is a marker of inflammation and oxidative stress that has been associated with multiple age-related chronic diseases. Since lifestyle is key for preventing these adverse health outcomes, we examined the association between a Mediterranean lifestyle and GDF-15 serum concentrations in Spanish older adults., Methods: We used cross-sectional data from 2502 older adults participating in the Seniors ENRICA-2 cohort. Adherence to the Mediterranean lifestyle was assessed with the 27-item MEDLIFE index, divided into three blocks: 1) "Mediterranean food consumption, 2) Mediterranean dietary habits, 3) Physical activity, rest, social habits, and conviviality". Analyses of the association between the MEFLIFE index and GDF-15 concentrations were performed using multivariable linear regression models adjusting for the main potential confounders., Results: The MEDLIFE index was inversely associated with GDF-15. Compared with participants in the lowest quartile of the MEDLIFE score, GDF-15 mean percentage differences (95% CI) were -3.0% (-8.0, 2.3) for the second quartile, -8.7% (-13.0, -4.1) for the third quartile, and -10.1% (-15.0, -4.9) for the fourth quartile (p-trend<0.001). Block 3 of MEDLIFE, and particularly doing sufficient physical activity, adequate sleep duration, and participating in collective sports, was individually linked to lower concentrations of GDF-15. Abstract

Background: Physical activity (PA) surveillance, policy, and research efforts need to be periodically appraised to gain insight into national and global capacities for PA promotion. The aim of this paper was to assess the status and trends in PA surveillance, policy, and research in 164 countries., Methods: We used data from the Global Observatory for Physical Activity (GoPA!) 2015 and 2020 surveys. Comprehensive searches were performed for each country to determine the level of development of their PA surveillance, policy, and research, and the findings were verified by the GoPA! Country Contacts. Trends were analyzed based on the data available for both survey years., Results: The global 5-year progress in all 3 indicators was modest, with most countries either improving or staying at the same level. PA surveillance, policy, and research improved or remained at a high level in 48.1%, 40.6%, and 42.1% of the countries, respectively. PA surveillance, policy, and research scores decreased or remained at a low level in 8.3%, 15.8%, and 28.6% of the countries, respectively. The highest capacity for PA promotion was found in Europe, the lowest in Africa and low- and lower-middle-income countries. Although a large percentage of the world's population benefit from at least some PA policy, surveillance, and research efforts in their countries, 49.6 million people are without PA surveillance, 629.4 million people are without PA policy, and 108.7 million live in countries without any PA research output. A total of 6.3 billion people or 88.2% of the world's population live in countries where PA promotion capacity should be significantly improved., Conclusion: Despite PA is essential for health, there are large inequalities between countries and world regions in their capacity to promote PA. Coordinated efforts are needed to reduce the inequalities and improve the global capacity for PA promotion.

49. A Mediterranean Lifestyle and Frailty Incidence in Older Adults: The Seniors-ENRICA-1 Cohort.

Abstract

Background: Frailty is a geriatric syndrome that entails high risk of hospitalization, disability, and death. While adherence to Mediterranean diet has been associated with lower risk of frailty, the joint effect of diet and lifestyle is uncertain. This study examined the association between a Mediterranean lifestyle (diet, customs, and traditions) and frailty incidence in older adults., Methods: We analyzed data from 1 880 individuals aged ≥ 60 from the prospective Seniors-ENRICA-1 cohort. Adherence to the Mediterranean lifestyle was assessed at baseline with the 27-item MEDLIFE index (higher scores representing better adherence), divided into 3 blocks: (1) "Mediterranean food consumption," (2) "Mediterranean dietary habits" (practices around meals)," and (3) "Physical activity, rest, social habits and conviviality." Frailty was ascertained as the presence of ≥ 3 of the 5 Fried criteria: (a) Exhaustion; (b) Muscle weakness; (c) Low physical activity; (d) Slow walking speed; and (e) Unintentional weight loss. Main statistical analyses were performed using logistic regression models, adjusting for the main confounders., Results: After a 3.3-year follow-up, 136 incident frailty cases were ascertained. Compared with participants in the lowest tertile of the MEDLIFE score, the OR (95% CI) for frailty was 0.88 (0.58-1.34) for the second tertile, and 0.38 (0.21-0.69) for the third tertile (p-trend = .003). Blocks 1 and 3 of the MEDLIFE score were independently associated with lower frailty risk. 50. Long-term leisure-time physical activity and risk of all-cause and cardiovascular mortality: dose-response associations in a prospective cohort study of 210 327 Taiwanese adults.

Abstract

Objectives: We aimed to investigate the dose-response associations of long-term leisure-time physical activity (LTPA) obtained from repeated measures with all-cause and cardiovascular disease (CVD) mortality outcomes in Taiwanese adults., Methods: We included 210 327 participants with self-reported LTPA at least in two medical examinations (867 968 data points) for up to 20 years (median, IQR: 4.8 years, 2.3-9.0). Dose-response relationships were modelled with restricted cubic spline functions and Cox regressions HRs (95% CIs) adjusted for main covariates., Results: During up to 23 years of follow-up (3 655 734 person-years), 10 539 participants died, of which 1919 of CVD. We observed an inverse, non-linear dose-response association between long-term LTPA and all-cause and CVD mortality. Compared with the referent (0 metabolic equivalent of task (MET) hours/week), insufficient (0.01-7.49 MET hours/week), recommended (7.50-15.00 MET hours/week) and additional (>15 MET hours/week) amounts of LTPA had a lower mortality risk of 0.74 (0.69-0.80), 0.64 (0.60-0.70) and 0.59 (0.54-0.64) for all-cause mortality and 0.68 (0.60-0.84), 0.56 (0.47-0.67) and 0.56 (0.47-0.68) for CVD mortality. When using only baseline measures of LTPA, the corresponding mortality risk was 0.88 (0.84-0.93), 0.83 (0.78-0.88) and 0.78 (0.73-0.83) for all-cause and 0.91 (0.81-1.02), 0.78 (0.68-0.89) and 0.80 (0.70-0.92) for CVD mortality., Conclusion: Long-term LTPA was associated with lower risks of all-cause and CVD mortality.
