20. Guided Evaluation Activities to Understand Children's Experience of an Obesity Prevention Programme

Abstract

Objective: In this paper, we describe an approach to engage young children in evaluating the impact of "La Vida Buena," a community health worker-led childhood obesity intervention for Latino children aged 5-8 years. Design: This paper describes the collaborative process used to pilot, refine, implement and evaluate evaluation activities, including a drawing and sorting activity, with child participants. Setting: "La Vida Buena" was implemented with Latino children aged 5-8 years and their parents or caregivers. The children were patients at a Federally Qualified Community Health Centre in a rural town on the US-Mexico Border. The intervention was implemented in a community setting. Methods: Participants were invited to participate in Guided Evaluation Activities during the 3-month and 6-month data collection points. Children participated in a drawing and sorting activity and discussion facilitated by teenage health facilitators or a community health worker. An observer took notes during the activities, and results were analysed by a university evaluator and "La Vida Buena" staff. Results: Findings demonstrated that the children understood some key messages of the class and identified important areas for improvement in the curriculum content and delivery. Conclusion: By designing evaluation methods that take the unique needs and perspectives of children into account, health promotion programmes can move beyond relying on parent-centric evaluation approaches and gain a deeper understanding of how children experience health intervention programmes.

Abstract

Substance use disorder (SUD) is common in psychiatric patients and has a negative impact on health and well-being. However, SUD often goes untreated, and there is a need for psychiatrists, of all specialties, to address this pervasive clinical problem. In this review, the authors' goal is to provide a resource that describes treatments for SUD, using neuroscience as a framework. They discuss the effect of pharmacotherapy on craving, intoxication, and withdrawal and its ability to interrupt the cycle of substance use in SUD. The neuroscience of stress is reviewed, including medications targeting neurotransmitter systems activated by alarm and fear. Neuroplasticity and promising treatments that use this mechanism, including ketamine, psilocybin, and transcranial magnetic stimulation (TMS), are discussed. ObjectivePatients with limited health literacy (LHL) and limited English proficiency (LEP) experience suboptimal communication and health outcomes. Electronic health record implementation in safety net clinics may affect communication with LHL and LEP patients.We investigated the associations between safety net clinician computer use and patient-provider communication for patients with LEP and LHL.Materials and methodsWe video-recorded encounters at 5 academically affiliated US public hospital clinics between English- and Spanish-speaking patients with chronic conditions and their primary and specialty care clinicians. Abstract

Detecting the muscle strength levels of the upper limbs plays a significant role in athletes' performance, injury prevention, and rehabilitation. Objective: To estimate the risk of injury to the shoulder rotator muscles at speeds of 60°/s and 180°/s. Method: A descriptive, correlational, cross-sectional study was conducted on 80 amateur athletes who underwent isokinetic testing using the HumacNorm (CSMI) Isokinetic Testing and Rehabilitation System and segmental anthropometry using a seca mBCA554 bioimpedance analyzer. Results: The results were compared to standardized benchmarks in various populations of athletes. High anthropometric correlations were observed, including between the muscle mass of the dominant arm (MMBD) and the muscle mass of the non-dominant arm (MMBND) (p<0.001), as well as isokinetic values at 60°/s between the internal rotators (RI) of MMBD and MMBND (p<0.001) and between 60°/s and 180°/s of the external rotators (RE) (p<0.001). Segmental anthropometry also showed a correlation with isokinetic measurements, such as MMBND and MMBND RE at 60°/s, with a correlation coefficient of r=0.70 (p=0.01). BMI was within the normal range, but deficits were observed for RE180°/s exceeding 10%, and the fatigue index (FI) was high for RI180°/s (p<0.05). Conclusion: Maximum strength, segmental muscle mass, asymmetries, deficits, and FI are variables that can better estimate the risk of injury. However, due to the lack of benchmarks for amateur athletes and limited and incongruent results in other sports disciplines and elite athletes, our findings can serve as initial isokinetic and segmental anthropometry parameters for amateur athletes with a high demand on their upper limbs., Detectar los niveles de fuerza de los músculos de las extremidades superiores, incide en el rendimiento, prevención y rehabilitación en deportistas. Objetivo: estimar el riesgo de lesión de los músculos rotadores de hombro a velocidad de 60°/s y 180°/s. Método: estudio descriptivo, correlacional y transversal, en 80 deportistas amateur quienes realizaron isocinésis utilizando el HumacNorm (CSMI) Isokinetic Testing and Rehabilitation System y antropometría segmentaria mediante un bioimpedanciometro seca mBCA554. Resultados: fueron comparados con baremos estandarizados en diferentes poblaciones de deportistas. Se obtuvo alta correlación antropométrica, entre la masa muscular del brazo dominante (MMBD) y masa muscular del brazo no dominante (MMBND) (p<0,001); e isocinéticas a 60°/s entre los RI de la MMBD y la MMBND (p<0,001); entre 60°/s y 180°/s de los RE (p<0,001); entre la antropometría segmentaria e isocinésis como la MMBND y RE de MMBND a 60°/s con un radio de r=0,70 (p=0,01). El IMC fue normal; pero los valores déficit para RE180°/s supero el 10% y el índice de fatiga (IF) fue alto para los RI180°/s (p<0,05). Conclusión: La fuerza máxima, la masa muscular segmentaria, asimetrías, déficit e IF, si son variables que pueden estimar mejor el riesgo de lesión, aunque debido a la falta de baremos en deportistas amateur y a los resultados limitados e incongruentes en otras disciplinas deportivas y deportistas elite, nuestros resultados pueden servir como parámetros iniciales isocinéticos y de antropometría segmentaria para los deportistas amateur con mayor demanda de extremidades superiores.

Abstract

Circadian fluctuations in physiological setpoints are determined by the suprachiasmatic nucleus (SCN) which exerts control over many target structures within and beyond the hypothalamus via projections. The SCN, or central pacemaker, orchestrates synchrony between the external environment and the internal circadian mechanism. The resulting cycles in hormone levels and autonomic nervous system (ANS) activity provide precise messages to specific organs, adjusting, for example, their sensitivity to approaching hormones or metabolites. The SCN responds to both photic (light) and non-photic input. Circadian patterns are found in both heart rate and blood pressure, which are linked to daily variations in activity and autonomic nervous system activity. Variations in blood pressure are of great interest as several cardiovascular diseases such as stroke, arrhythmias, and hypertension are linked to circadian rhythm dysregulation. The disruption of normal day-night cycles, such as in shift work, social jetlag, or eating outside of normal hours leads to desynchronization of the central and peripheral clocks. This desynchronization leads to disorganization of the cellular processes that are normally driven by the interactions of the SCN and photic input. Here, we review autonomic system function and dysfunction due to regulation and interaction between different cardiorespiratory brain centers and the SCN, as well as social, lifestyle, and external factors that may impact the circadian control of blood pressure.
