2. Triple gauge coupling analysis using boosted $W$'s and $Z$'s

We analyze the Large Hadron Collider potential to study triple couplings of the electroweak gauge bosons using their boosted hadronic decays. Deviations from Standard Model predictions spoil cancelations present in the Standard Model leading to the growth of the electroweak diboson production cross section at high center-of-mass energies. In this kinematical limit, $W$'s and $Z$'s are highly boosted, and consequently, their hadronic decays give rise to fat jets. Here, we show that the study of boosted hadronically decaying $W$ and $Z$ leads to limits on triple gauge couplings that are comparable to the ones originating from the leptonic decay channels., Comment: 9 pages, 2 figures

Precision measurements of anomalous quartic couplings of electroweak gauge bosons allow us to search for deviations of the Standard Model predictions and signals of new physics. Here, we obtain the constraints on anomalous quartic gauge couplings using the presently available data on the production of gauge-boson pairs via vector boson fusion. We work in the Higgs effective theory framework and obtain the present bounds on the operator's Wilson coefficients. Anomalous quartic gauge boson couplings lead to rapidly growing cross sections and we discuss the impact of a unitarization procedure on the attainable limits.

Bartonella quintana infection can cause severe disease that includes clinical manifestations such as endocarditis, chronic bacteremia, and vasoproliferative lesions of the skin and viscera. B. quintana bacteria is transmitted by the human body louse (Pediculus humanus corporis) and is associated with homelessness and limited access to hygienic services. We report B. quintana infection in 2 kidney transplant recipients in the United States from an organ donor who was experiencing homelessness. One infection manifested atypically, and the other was minimally symptomatic; with rapid detection, both recipients received timely treatment and recovered. B. quintana was identified retrospectively in an archived donor hematoma specimen, confirming the transmission link. Information about the organ donor’s housing status was critical to this investigation. Evaluation for B. quintana infection should be considered for solid organ transplant recipients who receive organs from donors with a history of homelessness or of body lice infestation.

Abstract An animal model was applied to develop erosive tooth wear (ETW) and to evaluate the efficacy and safety of titanium tetrafluoride (TiF4) in preventing ETW. Forty-eight male Wistar rats were divided into three groups (n = 16): TiF4 (2.45% F−), NaF (2.45% F−) and placebo varnishes. Eight from each group were subjected to erosive challenges (Sprite Zero) and the other received tap water, both ad libitum. After twenty-eight days, the mandibles were resected for histopathological gingival analysis, clinical and microscopic tooth evaluation by 3D confocal laser microscopy, scanning electron microscopy (SEM-EDX) and micro-Raman spectroscopy (MRS). Organs were evaluated with respect to fluoride content. No significant difference was found in F content in tissues. No histopathological damage was seen in gingiva. We perform a complete study of the electroweak precision observables and electroweak gauge boson pair production in terms of the SMEFT up to ${\cal O}(1/\Lambda^4)$ under the assumption of universal, C and P conserving new physics. We show that the analysis of data from those two sectors allows us to obtain closed constraints in the relevant parameter space in this scenario. In particular we find that the Large Hadron Collider data can independently constrain the Wilson coefficients of the dimension-six and -eight operators directly contributing to the triple gauge boson vertices. Our results show that the impact of dimension-eight operators in the study of triple gauge couplings is small., Comment: 17 pages and 3 figures

This article aims to analyze how organizational purpose influences the meaning of work and employee engagement. The article is relevant due to the scarcity of studies on the subject, but also because organizations have faced numerous challenges linked to new ways of working, resignations from professionals who are looking for greater personal and professional fulfillment, among other factors that impact the meaning of work and employee engagement. Given this scenario, this article makes contributions as it identifies: (i) how the phenomenon “organizational purpose” manifests itself in practice; (ii) its relevance; (iii) its positive impact on the meaning of work and engagement and (iv) the importance of the role of leadership in this process. To achieve the objective of this work, ten semi-structured, qualitative research studies were carried out, using the thematic analysis technique to process the data. The study concludes that: (i) the organizational purpose needs to be authentic; (ii) that there is a direct relationship between a company with an organizational purpose and the positive impact it generates on the meanings of work; (iii) the organizational purpose is the “differentiator” in engagement; (iv) and leadership is a driver and guardian of purpose.

We describe a case of a 46-year-old man in Missouri, USA, with newly diagnosed advanced HIV and PCR-confirmed mpox keratitis. The keratitis initially resolved after intravenous tecovirimat and penicillin for suspected ocular syphilis coinfection. Despite a confirmatory negative PCR, he developed relapsed, ipsilateral PCR-positive keratitis and severe ocular mpox requiring corneal transplant.

This manuscript presents current sensorless algorithms for maximum power point tracking (MPPT) in partially shaded photovoltaic (PV) systems. The necessity of a current sensor is eliminated with the use of mathematical modeling of the power electronics converter. This approach significantly reduces the implementation cost and the inherent disadvantages in the current sensor circuitry. MPPT techniques based on soft computing are employed, in addition to Perturb and Observe (P&O), due to their ability to explore a larger search space. This feature is advantageous because it minimizes convergence risk to a local maximum, a limitation of traditional techniques. Simulation and experimental results are presented and each algorithm is evaluated through different metrics, such as search time for the global maximum power point (GMPP) and efficiency. The tests consider dynamic irradiance profiles, producing a tracking factor (TF) above 99% and a remarkable fast convergence time.

Introdução: A deficiência de glicose-6-fosfato desidrogenase (G6PD) afeta cerca de 400 milhões de pessoas no mundo, na Amazônia brasileira foi descrita uma prevalência de 5,6%. É uma desordem enzimática, e torna a hemácia suscetível à injúria oxidativa após contato com antimaláricos, como a primaquina e tafenoquina. Sendo de fundamental importância na região amazônica, que corresponde a 99% dos casos de malária do Brasil. Objetivo: Avaliar a atividade de G6PD e suas características epidemiológicas em indivíduos com diagnóstico de malária. Método: Foram avaliadas as características epidemiológicas e exame de G6PD dos indivíduos com diagnóstico de malária por P. vivax (incluindo mista PV+PF) atendidos no ambulatório de síndromes febris do Centro de Pesquisa em Medicina Tropical de Rondônia (CEPEM), em Porto Velho, em 2022. Aprovado pelo CEP/CEPEM. Resultados: Avaliamos 2.066 casos de P. vivax, com média de idade de 38 anos (0,6-85 anos; DP 15,8), 1.385 (67%) do sexo masculino. Do sexo feminino, 13 (1,9%) eram gestantes e 30 (4,4%) lactantes. Todos realizaram o teste de G6PD, sendo 206 (10%) com baixa atividade (≤ 4,0), 656 (31,7%) com atividade intermediária (≥ 4,1 e ≤ 6,0) e 1.204 (58,3%) atividade normal (≥ 6,1), (p > 0,013). Dentre os homens (n = 1.385) a atividade de G6PD foi: 125 (9%) baixa, 465 (33,6%) intermediária e 795 (57,4%) normal, já as mulheres (n = 681), 81 (11,9%) apresentaram baixa atividade, 191 (28%) intermediária e 409 (60,1%) normal. Os pacientes foram tratados com os seguintes esquemas: 980 (47,4%) com primaquina 7 dias; 570 (27,6%) tafenoquina, 295 (14,3%) com primaquina 14 dias; e 205 (9,9%) primaquina semanal. A proporção de recaída foi de 255 (12,3%), sendo 7,3% com primaquina semanal e 12,9% com outros esquemas, além disso, não houve diferença estatística na média de peso de quem apresentou recaída quando comparada a quem não apresentou (p = 0,352). Conclusão: A prevalência de deficiência de G6PD foi maior (10% versus 5,6%) quando comparado a estudos anteriores na Amazônia brasileira. O Ministério da Saúde prevê a implementação da tafenoquina, mostramos que mais de 40% dos indivíduos analisados teriam contraindicação à tafenoquina. Dessa forma, reforçamos a importância da testagem de G6PD prévia à administração de antimaláricos, a fim de evitar complicações. Nos casos de deficiência vimos que o esquema semanal não aumentou as recaídas. Demonstramos que, apesar de relacionada ao cromossomo X, não houve diferença estatística de deficiência ao comparar os sexos.

Introdução: Visando diminuir recaídas, o tratamento da malária foi modificado aumentando a dose diária da primaquina e reduzindo o tempo de tratamento. Essa estratégia parece ter aumentado episódios de metahemoglobinemia, ações oxidantes nas hemoglobinas, transformando as em metemoglobina (metaHb), que não possuem capacidade de transporte de oxigênio aos tecidos. Objetivo: Analisar a prevalência de metemoglobina, seus níveis e manifestações clínicas. Método: Os participantes foram atendidos no ambulatório de malária do Centro de Pesquisa em Medicina Tropical de Rondônia, em Porto Velho, no período de 2022 a 2023, com diagnóstico de metahemoglobinemia (MetaHB > 3,0%) em seus retornos. Foram avaliados níveis de metemoglobina e aspectos clínicos. Aprovado pelo CEP/CEPEM. Resultados: Foram atendidos 5330 indivíduos e analisados 556 participantes com metahemoglobinemia (10,4%), com média de idade de 39 anos (DP 16), 68,2% eram do sexo masculino. Eram primoinfecção 135 indivíduos, destes 80,7% apresentavam valor de metaHb 5 (p < 0,001). 496 indivíduos retornaram em menos de 15 dias do diagnóstico de malária e 66,5% apresentavam metaHb > 5 (p < 0,001). Conclusão: As manifestações clínicas estão diretamente relacionadas aos níveis de MetaHb. Entretanto, os sintomas estão presentes quando MetaHB < 10%, divergindo de estudos que sugerem o início dos sintomas com valor superior a 12%. É fundamental que os profissionais de saúde tenham conhecimento sobre a metaHb como complicação do tratamento de malária (10,4%), principalmente nas regiões endêmicas. O diagnóstico pode ser um desafio sem o auxílio do co-oximetro, mas características clínicas podem ser fundamentais.

Introdução: A malária é uma das doenças infecto-parasitárias de maior importância no mundo. A metahemoglobinemia é uma das principais complicações relacionada a terapêutica medicamentosa para o P. vivax com derivados 8-aminoquinolinas, como primaquina e tafenoquina. O acesso a métodos diagnósticos nem sempre estão disponíveis, o que se faz necessário suspeita clínica. Objetivo: Descrever as características clínicas em pacientes internados com metaemoglobinemia. Método: Foram atendidas 4.541 pessoas com malária no hospital Cemetron, em Porto Velho, entre os anos de 2019 e 2023, e apenas 60 destes pacientes apresentaram diagnóstico clínico e/ou laboratorial de metahemoglobinemia. Foram avaliados dados clínicos e laboratoriais durante toda a internação. Aprovação CEP/CEPEM (CAAE 68890523.4.0000.0011). Resultados: Dos 4.541 indivíduos internados, 60 tiveram diagnóstico de metahemoglobinemia (1,3%). Dos 60 com diagnóstico de metahemoglobinemia, a média de idade foi de 33 anos, sendo 36 (60%) do sexo masculino. Todos foram internados já com complicações da terapeutica, sendo os seguintes esquemas: 43 (71,6%) primaquina 7 dias, 10 (16,6%) primaquina 14 dias, 1 (1,6%) tafenoquina, 1 (1,6%) primaquina semanal e 5 (8,3%) sem informação. Em 58,4% dos casos, houve necessidade de interromper teraperutica e 3,3% teve esquema modificado. Dos indivíduos que tiveram avaliação da metemoglobina (metaHb), 65,2% apresentaram valor > 10. Dos sintomas apresentados, 42 (70%) tinham dispneia, 28 (46,6%) cefaleia, 27 (45%) fraqueza, 21 (35%) cianose, 19 (31,6%) vômito. Com relação a oximetria, os valores variaram de 65-93%, sendo que 39 (65%) indivíduos receberam suplementação de O², sendo 19 (49%) por máscara de alto fluxo, 11 (28%) por cateter nasal, 8 (20,5%) por máscara de Venturi e 1 (2,5%) por ventilação mecânica. O tempo médio de internação foi de 6,1 dias, e não foi registrado nenhum óbito. Conclusão: Encontramos uma baixa prevalência de metahemoglobinemia com necessidade de internação, porém 65% dos que internaram tiveram necessidade de suplementação de O2, inclusive com ventilação mecânica. É uma complicação que diagnosticada precocemente e com a interrupção ou modificação do esquema para semanal, pode modificar desfecho, como vimos em outro estudo do nosso grupo com acompanhamento ambulatorial.

Objetivos: Investigar a eficácia terapêutica de um fármaco mimético da hepcidina, Rusfertide, que tem sido pesquisado como uma opção inovadora na restauração da homeostase em diversos distúrbios hematológicos, inclusive na Policitemia Vera (PV). Metodologia: Revisão de literatura realizada em junho de 2024, utilizando as plataformas Pubmed. Foi adotado como filtro, publicações de 2020 a 2024 e encontrados 100 artigos. Desses, foram utilizados 10 artigos, que estão disponíveis na íntegra e publicados em inglês. Resultados: Após análise de estudos com pacientes que possuem PV leve e severa, foi observado que a taxa de Flebotomia terapêutica (FT) decaiu após as primeiras semanas com o uso do fármaco. Todos os pacientes apresentaram efeitos adversos, variando em graus de severidade, onde os de grau leve apresentaram reações no local da injeção (eritema), mas foi controlada ao decorrer do tratamento, e a variação do número de leucócitos, onde parte dos pacientes apresentaram valores normais da série branca e outra apresentou uma leucocitose sem causa aparente. Já os pacientes com efeitos adversos graves, foi observado uma trombocitose assintomática, Leucemia Mieloide Aguda e infarto do miocárdio, em PV severa. Além disso, todos os pacientes em tratamento apresentaram uma redução do hematócrito (Ht) para 45% em que anteriormente apresentaram valores superiores a 55% e 43% dos indivíduos estudados tiveram uma redução da frequência de FT. Discussão: A PV é uma neoplasia mieloproliferativa crônica caracterizada por uma mutação pontual no gene JAK2, que resulta na produção excessiva de células sanguíneas e está associada a complicações graves, como trombose e hemorragia. Os tratamentos convencionais, como a FT e as terapias citorredutoras, causam limitações e não aliviam adequadamente os sintomas dos pacientes. Com o objetivo de proporcionar uma melhor qualidade de vida e um controle mais eficaz da doença, foi desenvolvido o Rusfertide, um fármaco injetável que mimetiza a hepcidina, responsável pela regulação do ferro no organismo. Está sendo testado em ensaios clínicos para pacientes que dependem de FT. Esta nova abordagem tem demonstrado resultados encorajadores, como a capacidade de controlar o Ht, diminuir a frequência de FT necessárias e aliviar sintomas. O tratamento tem benefícios, pois houve o controle da neoplasia da maioria dos pacientes em tratamento. Além disso, a FT diminuiu ou cessou em todos os pacientes após o início do tratamento com Rusfertide, independentemente da categoria de risco de PV ou de tratamentos concomitantes. Essa redução do número de FT, além de possuir vantagens terapêuticas, também pode aumentar a qualidade de vida do paciente, visto que a necessidade de monitoramento constante e procedimentos invasivos limita a sua autonomia. Portanto, é sugestivo que a terapia possa melhorar o controle do Ht e diminuir o uso da FT. Conclusão: Apesar das reações adversas e da necessidade de mais pesquisas, devido a melhora da qualidade de vida do paciente, o tratamento com o Rusfertide se mostra promissor. Este fármaco pode se tornar uma ferramenta adicional para o controle desta doença, a partir de seu mecanismo de ação, que irá atingir e sustentar o controle do Ht em pacientes com PV, reduzindo o uso de FT e a ocorrência de sintomas debilitantes relacionados à doença.

39. NOVA TERAPIA PARA LEUCEMIA LINFOBLÁSTICA AGUDA: UMA ANÁLISE COMPARATIVA ENTRE IMUNOTERAPIAS

Objetivos: Realizar uma comparação entre a eficácia terapêutica do Tercartus, que tem sido uma descoberta para o tratamento de Leucemia Linfoblástica Aguda (LLA) em contrapartida com a já utilizada terapia com Mesilato de Imatinibe. Metodologia: Revisão de literatura realizada em junho de 2024, utilizando as plataformas Pubmed e Clinical Trials. Foi adotado como filtro, publicações de 2020 a 2024. Dessas, foram utilizados 10 artigos, que atendiam aos critérios de serem disponíveis gratuitamente na íntegra e publicados em inglês. Resultados: Atualmente, há em andamento 8 estudos clínicos no Clinical Trials e 23 artigos no PubMed sobre os efeitos do Tercartus em pacientes adultos com LLA do tipo B recidiva ou refratária. Em um estudo realizado com 55 pacientes, foram acompanhados por 26,8 meses e observado uma taxa de remissão completa (RC) de 56% e quando a RC associado a recuperação hematológica incompleta foi obtido uma taxa de 71%. Houve uma resposta por parte dos pacientes para o Tecartus independente da idade ou porcentagem de blastos, porém quando analisado pacientes com números maiores que 75% de blastos não houve essa resposta. Quando analisados os eventos adversos, não ocorreu nenhum de grau severo, como a síndrome de liberação de citocinas. Assim, é possível se analisar que a imunoterapia fornece um benefício de sobrevivência clinicamente significativo com toxicidade gerenciável. Discussão: No final de 2023, a ANVISA aprovou o Tecartus, seu princípio ativo é o Brexucabtagene autoleucel e promove a morte de células que expressam o CD19 (células B neoplásicas), a partir da ativação e proliferação de células T e a secreção de interleucinas, com o intuito de levar a morte da célula. Entre as neoplasias que visam tratar, destaca-se a LLA de células B precursoras recidivante ou refratária. Por mais que já existam terapias disponíveis no mercado que vêm sendo utilizadas para o tratamento dessa neoplasia, como o Imatinibe, o Tecartus vem buscando se mostrar promissor para ser um novo medicamento para o tratamento da LLA. Em comparação com o tratamento com Imatinibe, que é um inibidor da atividade de proteína tirosina quinase do gene de fusão BCR-ABL da LLA, foi observado em estudos que para obter melhores taxas de sucesso e entrar em remissão e posteriormente impedir uma recidiva, deve-se fazer a associação dele com quimioterapia agressiva. Foi observado também uma grande quantidade de reações adversas, como dor abdominal e hemorragia gastrointestinal. Entretanto, os benefícios foram muitos, cerca de 62% dos pacientes mostraram aumento na taxa de remissão clínica completa. A terapia com Tecartus, no geral, tem se mostrado bastante eficaz, porém a taxa de RC foi menor quando comparado ao uso de Imatinibe. Com isso, sugere-se que o Imatinibe traz maiores benefícios em relação ao novo tratamento, apesar de possuir um tratamento a longo prazo, devido sua administração via oral, enquanto para o Tecartus é necessário uma única infusão e é necessário a associação com quimioterapia. Conclusão: Apesar de ter apresentado menor taxa de RC ao Imatinibe, o Tecartus é uma terapia promissora que faz aumentar a sobrevida global mediana e promove uma RC da neoplasia em parte dos pacientes, mesmo sendo necessário realizar quimioterapia concomitante à infusão.

Introduction Determining aetiology of severe illness can be difficult, especially in settings with limited diagnostic resources, yet critical for providing life-saving care. Our objective was to describe the accuracy of antemortem clinical diagnoses in young children in high-mortality settings, compared with results of specific postmortem diagnoses obtained from Child Health and Mortality Prevention Surveillance (CHAMPS).Methods We analysed data collected during 2016–2022 from seven sites in Africa and South Asia. We compared antemortem clinical diagnoses from clinical records to a reference standard of postmortem diagnoses determined by expert panels at each site who reviewed the results of histopathological and microbiological testing of tissue, blood, and cerebrospinal fluid. We calculated test characteristics and 95% CIs of antemortem clinical diagnostic accuracy for the 10 most common causes of death. We classified diagnostic discrepancies as major and minor, per Goldman criteria later modified by Battle.Results CHAMPS enrolled 1454 deceased young children aged 1–59 months during the study period; 881 had available clinical records and were analysed. The median age at death was 11 months (IQR 4–21 months) and 47.3% (n=417) were female. We identified a clinicopathological discrepancy in 39.5% (n=348) of deaths; 82.3% of diagnostic errors were major. The sensitivity of clinician antemortem diagnosis ranged from 26% (95% CI 14.6% to 40.3%) for non-infectious respiratory diseases (eg, aspiration pneumonia, interstitial lung disease, etc) to 82.2% (95% CI 72.7% to 89.5%) for diarrhoeal diseases. Antemortem clinical diagnostic specificity ranged from 75.2% (95% CI 72.1% to 78.2%) for diarrhoeal diseases to 99.0% (95% CI 98.1% to 99.6%) for HIV.Conclusions Antemortem clinical diagnostic errors were common for young children who died in areas with high childhood mortality rates. To further reduce childhood mortality in resource-limited settings, there is an urgent need to improve antemortem diagnostic capability through advances in the availability of diagnostic testing and clinical skills.

This study developed and examined the implementation of an intrusive peer-to-peer coaching program model at a private 4-year master's degree granting Hispanic Serving Institution (HSI) located in South Texas. The coaching program provided support and promoted increased persistence among Latina/o students entering first-year science, technology, engineering, and mathematics (STEM). Findings positively indicated that first-year Latina/o STEM students can achieve academic success in college when they are matched and supported by culturally similar upperclassmen peer coaches.

Abstract Background Melting corneal ulcers are a serious condition that affects a great number of animals and people around the world and it is characterised by a progressive weakening of the tissue leading to possible severe ophthalmic complications, such as visual impairment or blindness. This disease is routinely treated with medical therapy and keratoplasty, and recently also with alternative regenerative therapies, such as cross-linking, amniotic membrane transplant, and laser. Plasma medicine is another recent example of regenerative treatment that showed promising results in reducing the microbial load of corneal tissue together with maintaining its cellular vitality. Since the effect of helium plasma application on corneal mechanical viscoelasticity has not yet been investigated, the aim of this study is first to evaluate it on ex vivo porcine corneas for different exposition times and then to compare the results with previous data on cross-linking treatment. Results 94 ex vivo porcine corneas divided into 16 populations (healthy or injured, fresh or cultured and treated or not with plasma or cross-linking) were analysed. For each population, a biomechanical analysis was performed by uniaxial stress-relaxation tests, and a statistical analysis was carried out considering the characteristic mechanical parameters. In terms of equilibrium normalised stress, no statistically significant difference resulted when the healthy corneas were compared with lesioned plasma-treated ones, independently of treatment time, contrary to what was obtained about the cross-linking treated corneas which exhibited more intense relaxation phenomena. Conclusions In this study, the influence of the Helium plasma treatment was observed on the viscoelasticity of porcine corneas ex vivo, by restoring in lesioned tissue a degree of relaxation similar to the one of the native tissue, even after only 2 min of application. Therefore, the obtained results suggest that plasma treatment is a promising new regenerative ophthalmic therapy for melting corneal ulcers, laying the groundwork for further studies to correlate the mechanical findings with corneal histology and ultrastructural anatomy after plasma treatment.

Abstract Background Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann’s procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA. Methods This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up. Results 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%).
