Purpose Up to now there are only few studies and no RCT comparing efficacy or effectiveness of supported housing (SH) versus residential care (RC) in severe mental illness (SMI) without homelessness. Here we present an observational follow-up study in SMI subjects, who entered SH or RC, to compare clinical and functional outcomes 2 years later. Methods In this prospective study in more than 30 locations throughout a German federal state, we included SMI subjects, who entered SH (n = 153) or RC (n = 104). About one quarter suffered from each substance use, psychotic, affective, or other disorders. To avoid sampling bias, we used the propensity score matching method to establish a quasi-experimental design. Outcome measures were social functioning (SFS), the number of psychiatric hospitalisations, psychopathology (SCL-9-K), and quality of life (MANSA). Apart from descriptive methods we analysed primarily using repeated-measures ANOVAS. Results Our analyses revealed significant effects of time for all outcomes in both study groups. However, there were not any group differences of outcome measures, i.e., not any significant effects of group or interactional effects of group x time. Moreover, these results hold true for intent-to-treat and per-protocol sample analyses. Conclusion The results show, that SH and RC for non-homeless people with SMI achieve the same clinical and psychosocial outcomes across a 2-year period. Taking into account the users’ preferences, the present findings should give reason to ensure the availability of affordable housing and to support the expansion of supported housing approaches.

Objectives Persons with Alzheimer’s disease dementia (ADD) often show impaired orientation, particularly in unknown environments. Signs may offer an opportunity to compensate for these deficits and thus improve participation. Methods We assessed 30 persons with ADD and 36 healthy controls by using a sign comprehension paradigm (SCP) in a real-life environment. Nonparametric mixed model analyses of variance were used to analyze the effect of different symbols and additional scripture (coding condition) on SCP performance speed and accuracy. Results Analyses revealed a significant main effect of symbol design on SCP speed as well as an interaction effect of group × symbol, indicating a benefit of concrete, optimized signs for persons with ADD. Furthermore, analyses of SCP error rates revealed the main effects of group and coding condition as well as an interaction effect of group × coding. Persons with ADD made more errors than healthy controls, but SCP error rates decreased significantly in ADD in the double-coding condition. Discussion Our findings revealed an advantage of concrete double-coded symbols over conventional symbols and therefore strongly suggest the implementation of concrete double-coded signs to support older people living with ADD.

OBJECTIVE: Patients with Alzheimer disease dementia (ADD) often show impaired orientation and navigation. Signage offers an opportunity to compensate for these deficits, communicate information efficiently and facilitate wayfinding. Certain properties of signs such as colors and contrasts may beneficially affect the uptake and processing of information particularly in ADD patients.; METHODS: Thirty-six healthy older adults and 30 ADD patients performed a computerized color perception task that required discriminating different color combinations. The effects of different contrast features on performance accuracy and speed in the 2 experimental groups were examined by nonparametric mixed analysis of variances.; RESULTS: Analyses revealed a significant effect of contrast polarity on reaction times, significant effects of group on reaction times and errors as well as a marginally significant interaction of group*color on errors. All participants benefitted from positive contrast polarity (ie, dark target on lighter background) as indicated by increased performance speed. Furthermore, ADD patients reacted slower and less accurate than healthy controls, but showed higher accuracy at black-white and red-yellow than at blue-green color combinations.; CONCLUSIONS: Our findings suggest the implementation of signs with positive contrast polarity to ensure faster reactions. Zusammenfassung Ziel der Studie Explorative Untersuchung der coronabedingten Auswirkungen auf die beruflichen Perspektiven von Menschen mit psychischen Erkrankungen auf dem ersten Arbeitsmarkt. Methode Kurzumfrage mit quantitativen und qualitativen Fragen unter Mitarbeitenden aus der beruflichen Rehabilitation und Wiedereingliederung. Ergebnisse Von den n = 76 Teilnehmenden gaben über 90 % an, dass sich die Situation beim beruflichen Wiedereinstieg bzw. der Arbeitsplatzsuche coronabedingt (deutlich) verschlechtert habe. 75 % sahen auch auf längere Sicht hin negative Aussichten für die berufliche Rehabilitation. Offene Antworten zu Schwierigkeiten verwiesen neben dem Hauptproblem fehlender Stellenangebote u. a. auf erhöhte psychische Belastungen bei den Betroffenen sowie Probleme bei der Erreichbarkeit von Ansprechpersonen oder bei Online-Prozessen. Schlussfolgerung Durch die Corona-Situation ist es zu einer zusätzlichen Verschlechterung der beruflichen Perspektiven und Teilhabechancen von Menschen mit psychischen Erkrankungen gekommen.

Zusammenfassung Ziel der Studie In einer klinischen Gesamtpopulation aller vollstationären Behandlungsfälle werden geschlechtsspezifische Unterschiede über 15 Jahre bei fremdaggressivem Verhalten und Zwangsmaßnahmen (ZM) überprüft. Methodik Aggressive Ereignisse werden mit der Staff Observation Aggression Scale-Revised (SOAS-R) und ZM mit einem hierzu entwickelten Erfassungsbogen von 2005–2019 standardisiert erfasst und retrospektiv hinsichtlich geschlechtsspezifischer Unterschiede ausgewertet. Ergebnisse 82 833 vollstationäre Behandlungsfälle (Männer: 55,3 %; Frauen: 44,7 %) gingen in die Untersuchung ein. Während des Erhebungszeitraums wurden 60,1 % der erfassten Ereignisse durch Patienten und 39,9 % durch Patientinnen verübt, allerdings war auf Fallebene das Risiko für das Auftreten aggressiven Verhaltens für Männer und Frauen vergleichbar (Odds Ratio: 1,1). Im SOAS-R wiesen Frauen signifikant schwerwiegendere aggressive Handlungen auf, nach subjektiver Einschätzung durch Mitarbeitende werden aber aggressive Vorfälle von Männern schwerer eingestuft. Diese erhielten zudem signifikant mehr und länger andauernde ZM als Frauen. Schlussfolgerung Das subjektive Erleben aggressiven Verhaltens durch Mitarbeitende könnte dazu beitragen, dass ZM bei Patienten signifikant häufiger und länger eingesetzt werden.

Das „Collaborative Assessment and Management of Suicidality“ (CAMS) ist einer von mehreren Interventionsansatzen zur Behandlung von suizidalen Patienten und Patientinnen und wurde weltweit seit 3 Jahrzehnten untersucht und weiterentwickelt. Das CAMS ist auf die Etablierung einer tragenden therapeutischen Beziehung auf Augenhohe zu einer suizidalen Person ausgerichtet. Der Patient wird aktiv an der Einschatzung seines Suizidrisikos beteiligt und zum „Mitverfasser“ seines eigenen suizidspezifischen Behandlungsplans. Das konkrete therapeutische Vorgehen wird durch ein vielseitig anwendbares Instrument, die sogenannte Suizidstatusform (SSF), strukturiert und geleitet. Das CAMS kann von verschiedenen therapeutischen Berufsgruppen sowie bei Patienten und Patientinnen mit unterschiedlichen Diagnosen angewandt werden. Die Wirksamkeit im Hinblick auf eine Verringerung von Suizidgedanken, der allgemeinen psychischen Belastung und Depressivitat sowie auf eine Zunahme von Hoffnung und Zuversicht ist mittlerweile empirisch gut belegt. CAMS is focused on establishing a continuing therapeutic relationship at eye level with a suicidal person. The patient is actively involved in assessing his or her suicide risk and becomes the “co-author” of his or her own suicide-specific treatment plan. The specific therapeutic procedure is structured and guided by a versatile instrument called the Suicide Status Form (SSF). CAMS can be applied by different therapeutic professional groups to patients with different diagnoses. Its effectiveness in reducing suicidal ideation, general psychological distress and depressiveness, and increasing hope and confidence is now empirically well established.

ZusammenfassungSeit 2018 kann in Deutschland aufsuchende stationsäquivalente Behandlung (StäB) durchgeführt werden. Acht Kliniken der ersten Stunde legen erste Zahlen zu 658 in StäB behandelten Patienten vor. Allgemeine Kennzahlen werden vergleichend, die Details zu Organisationsstruktur, Kooperation, Behandlungsablauf sowie Mitarbeitenden- und Patientenbewertung differenziert nach Standort dargestellt.Die Ergebnisse zeigen eine hohe Heterogenität der Strukturen und Abläufe bei einer insgesamt sehr positiven Bewertung durch alle Beteiligten. Für gesicherte Schlussfolgerungen reicht die bisherige Datenlage noch nicht aus. Wichtige Hinweise für die weitere Forschung sind aber ableitbar. Entsprechende wissenschaftliche Fragestellungen werden in die AKtiV-Studie des Innovationsfonds eingebracht.

Background: On-road driving behavior can be impaired in older drivers and particularly in drivers with mild cognitive impairment (MCI). Objective: To determine whether cognitive and non-cognitive risk factors for driving safety may allow an accurate and economic prediction of on-road driving skills, fitness to drive, and prospective accident risk in healthy older drivers and drivers with MCI, we examined a representative combined sample of older drivers with and without MCI (N = 74) in an observational on-road study. In particular, we examined whether non-cognitive risk factors improve predictive accuracy provided by cognitive factors alone. Methods: Multiple and logistic hierarchical regression analyses were utilized to predict different driving outcomes. In all regression models, we included cognitive predictors alone in a first step and added non-cognitive predictors in a second step. Results: Results revealed that the combination of cognitive and non-cognitive risk factors significantly predicted driving skills (R2adjusted = 0.30) and fitness to drive (81.2% accuracy) as well as the number (R2adjusted = 0.21) and occurrence (88.3% accuracy) of prospective minor at-fault accidents within the next 12 months. In all analyses, the inclusion of non-cognitive risk factors led to a significant increase of explained variance in the different outcome variables. Conclusion: Our findings suggest that a combination of the most robust cognitive and non-cognitive risk factors may allow an economic and accurate prediction of on-road driving performance and prospective accident risk in healthy older drivers and drivers with MCI. Therefore, non-cognitive risk factors appear to play an important role.

Zusammenfassung Ziel Ziel der vorliegenden Übersichtsarbeit ist es, alle im deutschsprachigen Raum bekannten ambulanten, familienorientierten Komplexangebote für Kinder psychisch kranker Eltern anhand definierter Minimalanforderungen zu identifizieren und einen Überblick über deren Struktur, Inhalt und Wirksamkeitsnachweise zu geben. Methodik Die Angebote wurden mittels Internet- und Literaturrecherche identifiziert. Bei Erfüllung der definierten Kriterien, wurden die Anbieter kontaktiert und um die Teilnahme an einer schriftlichen oder telefonischen Befragung gebeten. Ergebnisse Insgesamt konnten 512 Angebote identifiziert werden, von denen 46 als familienorientiertes Komplexangebot einzustufen waren. Nur wenige Angebote wurden bisher systematisch evaluiert. Schlussfolgerung Es besteht eine Vielzahl an Interventionen für die Kinder psychisch kranker Eltern, familienorientierte Komplexangebote sind jedoch bisher selten. Zudem besteht ein erheblicher Bedarf an Evaluationsstudien.

Background: Cognitive impairment is a common symptom of patients with Major Depression (MDD) with negative clinical and social effects. With this study we investigated the question of whether MDD patients' effort to memorize items is reduced and whether this reduction is related to patients' memory impairment.Research Design and Methods: Twenty-two patients with MDD and 28 healthy control participants took part in the study. Both groups did not differ with respect to age, gender, and basic education. MDD was confirmed by the Mini Diagnostic Interview for Mental Disorders (Mini-DIPS). Willingness to make an effort was assessed by the Amsterdam Short-Term Memory Test (ASTM). Logical Memory (LM) from the Wechsler Memory Scale (WMS IV) was used to assess verbal memory.Results: With respect to the willingness to make an effort, all MDD-patients performed above the cut-off and did not perform worse than healthy control participants. Depression appears to have a negative impact on recall and recognition of verbal information, but the effect on recall was larger.Conclusions: These findings support studies indicating no motivational deficits in patients with depression during standardized neuropsychological testing. Reduced motivation is not a plausible explanation for depressive patients' cognitive deficits assessed with neuropsychological tests.

The objective of this paper is to describe the agreement from the perspective of the authors who, as experts for the Deutsche Krankenhausgesellschaft (DKG - German Hospital Federation), have accompanied the PsIA negotiations since 2013. It traces the development of the PsIA negotiations leading to the "Anderungsvereinbarungen zur Vereinbarung zu den Psychiatrischen Institutsambulanzen gemas § 118 Abs. 2 SGB V" (amendment agreements to the Agreement concerning the Psychiatric Outpatient Departments according to § 118 Abs. 2 SGB V - German Social Security Statute Book) of October 2019 with its striking difficulties in view of partly diametrally opposed interests of the negotiating partners.

Zusammenfassung: Hintergrund: Mit dieser Studie wird der Frage nachgegangen, inwieweit sich kognitive Beeinträchtigungen alkoholabhängiger Patient_innen in Abhängigkeit von Abstinenz und Depressivität im Behandlungsverlauf verbessern. Methode: Alkoholabhängige Patient_innen mit ( n = 22) und ohne Depression ( n = 58) wurden neuropsychologisch und hinsichtlich der Depressivität zu drei Zeitpunkten untersucht (nach Entzug und 6 Wochen bzw. 6 Monate später). 33 gesunde Kontrollproband_innen wurden zusätzlich zum ersten Zeitpunkt untersucht. Ergebnisse: Alkoholabhängige Patient_innen zeigten verglichen mit gesunden Kontrollpersonen eine verminderte Leistung im schlussfolgernden Denken. Im Behandlungsverlauf zeigte sich eine trendweise günstigere Entwicklung für abstinente verglichen mit rückfälligen Patient_innen und eine größere Depressivität war mit schwächeren Leistungen assoziiert. Diskussion: Alkoholabhängige Patient_innen leiden an kognitiven Defiziten, die sich z. T. im weiteren Behandlungsverlauf zu verbessern scheinen, v. a. bei Abstinenz.

Schizophrenia has been shown repeatedly to be associated with a low level of psychosocial functioning. It is assumable that psychosocial functioning is related not only to current, but also to future symptom severity. To test this assumption, a follow-up study with two measurement time points with an interval of 18 months was conducted. In total, 154 inpatients from five psychiatric hospitals with a diagnosis of a schizophrenic disorder took part at both visits. Psychosocial functioning was measured with the Personal and Social Performance Scale (PSP scale) at baseline, and schizophrenic symptoms were assessed with the Positive and Negative Syndrome Scale (PANSS) at baseline and at follow-up. Two PSP subscales, i.e. socially useful activities and control over disturbing and aggressive behavior, turned out to be significant predictors of symptom severity 18 months later. The findings reveal that personal resources in the occupational domain and in adequate interpersonal behavior can have a positive impact on the long-term course of schizophrenia.

Exploratory study of corona-related impacts on vocational perspectives of people with mental illness.Short survey with quantitative and qualitative questions among employees from vocational rehabilitation.More than 90 % of the n = 76 participants stated that the situation regarding vocational reintegration or job search had worsened due to corona. Moreover, 75 % considered that there would still be a negative impact on vocational rehabilitation in the longer term. Examination of gender-related differences of aggressive incidents and the use coercive measures within a whole psychiatric hospital with a catchment area of 339.000 inhabitants over a period of fifteen years.Recording of aggressive incidents using the Staff Observation Aggression Scale-Revised (SOAS-R) as well as coercive measures by a clinically developed record sheet and analysing gender-related differences from 2005 to 2019.82 833 inpatients (male: 55.3 %; female: 44.7 %) are included. Between 2005 and 2019, 60.1 % of the aggressive events involved male patients and 39.9 % involved female patients; however, regarding cases of treatment no gender-related difference existed (odds ratio: 1.1). The SOAS-R-severity score of women was significantly higher, while aggressive incidents of men were subjectively graded more serious by staff. ZusammenfassungZiel der Studie war es, deutschlandweit adoleszenzspezifische Behandlungsmodelle zu untersuchen, die Umsetzung transitionspsychiatrischer Versorgung darzustellen und relevante Problembereiche in der Versorgung für Heranwachsende mit psychischen Erkrankungen zu identifizieren. Methodik Es erfolgte eine fragebogenbasierte Querschnittserhebung (N = 26) krankenhausbasierter adoleszenzpsychiatrischer Behandlungsmodelle. Anhand von 28 Items wurden Daten zur Versorgungsstruktur, Zielgruppe und Therapieangeboten sowie spezifische Anforderungen der Adoleszenzpsychiatrie untersucht und deskriptiv ausgewertet. Ergebnisse Es zeigt sich eine Zunahme adoleszenzspezifischer krankenhausbasierter Angebote in Deutschland. Diese sind durch diagnoseübergreifende und adoleszenzspezifische Therapieansätze und multiprofessionelle Arbeit gekennzeichnet. Überwiegend besteht ein Behandlungsangebot erst ab Volljährigkeit. Eine Minderheit der Therapieangebote ist in fachübergreifenden Kooperationsmodellen organisiert. Es werden strukturelle Defizite hinsichtlich Akut- und Krisenbehandlungen, sektorenübergreifender Zusammenarbeit und systematischer Transition berichtet. Schlussfolgerungen Es besteht deutschlandweit ein wachsendes Interesse an der Entwicklung adoleszenzpsychiatrischer Behandlungsangebote. Die enge fachübergreifende Zusammenarbeit von Kinder- und Jugendpsychiatrie und Erwachsenenpsychiatrie ist zur Überwindung von Schnittstellenproblemen mit dem Ziel einer gelungenen Transition notwendig, aber bisher nicht flächendeckend implementiert. Um die Weiterentwicklung von Kooperationsprojekten zu ermöglichen, sollten strukturelle Hürden verringert werden.

Background: Emotion dysfunction is a key symptom in patients with borderline personality disorder (BPD) and is considered a consequence of dysfunctional emotion regulation (e.g., reduced emotion acceptance). In the present functional MRI (fMRI) study, we investigated the neural correlates of habitual emotion acceptance in individuals with BPD.; Methods: Female patients with BPD and female healthy controls passively viewed negative and neutral movie clips of faces during fMRI. We assessed emotion acceptance using the Emotion Acceptance Questionnaire (EAQ). To examine brain activation associated with habitual emotional acceptance of negative stimuli, the EAQ score was included as a regressor of interest in brain data analyses of activation intensity during negative compared with neutral movies.; Results: We included 20 women with BPD and 20 heatlhy controls in our analysis. Compared with healthy controls, patients with BPD showed significantly more activation in frontostriatal brain regions (i.e., left superior frontal gyrus, right caudate) as well as in the left precuneus, left precentral gyrus, left posterior cingulate cortex and left hippocampus when confronted with negative (v. neutral) stimuli. Patients with BPD reported decreased emotion acceptance compared with healthy controls, and habitual emotion acceptance was inversely associated with activation of striatal areas (i.e., left putamen, left caudate) in patients with BPD.; Limitations: Causal conclusions are not possible. Comorbid diagnoses were not excluded, and only female participants were investigated. Stimuli were not rated immediately and may not be generalizable to all negative emotions. We cannot make any statements about other emotion-regulation strategies that may have been applied here.; Conclusion: Data indicate that striatal hyperactivation during the processing of negative stimuli in women with BPD is related to their decreased disposition to accept unpleasant emotional states. Objectives The aim of the current work was to investigate the relationship between avoidance of specific driving situations and on-road driving skills in older drivers considering factors found to be related to both avoidance behavior and driving skills. Method Seventy-two older drivers (M = 76 years) from the general population were included in this study. Self-reported avoidance behavior, driving practice, perceived driving difficulties, driving-related cognitive functions, as well as medical conditions were assessed within two sessions. Standardized on-road assessments served for assessing on-road driving skills in a third session. Results Self-reported avoidance behavior was associated with reduced driving skills (r = −.41), and this relationship remained significant beyond the influence of cognitive skills, self-reported health, driving practice, and perceived driving difficulties. Specifically, avoidance of driving in bad weather, poor visibility and complicated parking was found to be associated with reduced driving skills. Discussion This study suggest that avoidance behavior is an independent indicator of impaired driving skills in older drivers. Our results argue against the assumption that avoidance behavior may be a reasonable strategy for safe traffic participation. Longitudinal studies are urgently needed to get more evidence on safety aspects of avoidance behavior.

The dissociative subtype of posttraumatic stress disorder (PTSD) was officially introduced in the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). In accordance with this new classification, prior studies using Latent Class Analysis (LCA) empirically identified a subgroup of patients that were characterized by a high severity of dissociative symptoms. Despite the high prevalence of PTSD in patients with substance use disorders (SUD), however, no LCA studies on the dissociative subtype of PTSD exist in this population so far. Therefore, the current study aimed to identify subgroups of patients with different symptom and exposure profiles in patients with SUD and PTSD. It was assumed that one symptom and exposure profile could be identified that would be characterized by higher dissociative symptoms, higher additional psychopathology and a higher burden of childhood trauma, as compared to other subgroups. In N = 258 female patients with SUD and PTSD, clinical characteristics of dissociative symptoms, PTSD severity, borderline personality disorder (BPD), depression, childhood trauma and substance abuse were assessed. To identify symptom and exposure profiles, Latent Class Analysis was applied. A three-class solution indicated the best model fit to our data. One class was characterized by a high probability of dissociative symptoms (D-PTSD class), whereas the other two classes were characterized by lower probabilities of dissociative symptoms. The D-PTSD class encompassed 18.7% of the patients. In accordance with our hypothesis, the D-PTSD class showed higher probabilities of PTSD severity, borderline personality disorder symptoms, depressive symptoms, childhood emotional and sexual abuse, childhood emotional neglect, and drug abuse. Our results indicate that the dissociative subtype of PTSD could also be identified in a sample of female patients with SUD. Patients with SUD and PTSD characterized by the dissociative subtype showed more severe psychopathological symptoms than the remaining patients, indicating enhanced clinical needs for this vulnerable group.

To get information about different ways inpatient equivalent treatment (IET) is implemented and organized eight specialized psychiatric hospitals and departments in Germany were asked to report on implementation details. OUTCOME: Organization and treatment processes depend on local conditions and existing structures. Legal barriers complicate the implementation process, however patients as well as team members report very positive experiences.Current data only give first hints but aren't sufficient to draw viable conclusions. Consequential issues will be picked up within the AKtiV-Study of the Innovationsfonds.Seit 2018 kann in Deutschland aufsuchende stationsäquivalente Behandlung (StäB) durchgeführt werden. Acht Kliniken der ersten Stunde legen erste Zahlen zu 658 in StäB behandelten Patienten vor. Allgemeine Kennzahlen werden vergleichend, die Details zu Organisationsstruktur, Kooperation, Behandlungsablauf sowie Mitarbeitenden- und Patientenbewertung differenziert nach Standort dargestellt.Die Ergebnisse zeigen eine hohe Heterogenität der Strukturen und Abläufe bei einer insgesamt sehr positiven Bewertung durch alle Beteiligten. Für gesicherte Schlussfolgerungen reicht die bisherige Datenlage noch nicht aus. Wichtige Hinweise für die weitere Forschung sind aber ableitbar. Entsprechende wissenschaftliche Fragestellungen werden in die AKtiV-Studie des Innovationsfonds eingebracht.

BACKGROUND: Emotion dysfunction and anhedonia are main problems in borderline personality disorder (BPD). In the present functional magnetic resonance imaging (fMRI) study, we investigated neural activation during the processing of happy faces and its correlates with habitual emotion acceptance in patients with BPD.; METHODS: 22 women with BPD and 26 female healthy controls watched movie clips of happy and neutral faces during fMRI without any instruction of emotion regulation. To associate neural activation with habitual emotion acceptance, we included individual scores of the Emotion Acceptance Questionnaire (EAQ) as a covariate in brain data analysis.; RESULTS: All participants showed amygdala, temporal and occipital activation during the processing of happy compared to neutral faces. Compared with healthy controls, patients with BPD showed significantly more activation within the bilateral caudate. We did not find significant correlations with emotion acceptance.; CONCLUSIONS: Our results indicate caudate hyperactivation in patients with BPD during the processing of happy faces. Although patients reported significantly less emotion acceptance of positive emotions, an association with neural activation was not detectable. Copyright © 2021 Elsevier Ltd. All rights reserved.

Zusammenfassung. Altern geht mit kognitiven und nicht-kognitiven Veränderungen einher, die bei einem relevanten Anteil älterer Menschen Risikofaktoren für eine Reduktion der Fahrsicherheit darstellen. Da praktische Fahrverhaltensbeobachtungen aufwendig und kostenintensiv sind, besteht ein zunehmender Bedarf an validen Screeningverfahren, die eine Erfassung dieser Risikofaktoren ermöglichen und eine diagnostisch genaue sowie zeitökonomische Einschätzung der Fahrsicherheit älterer Kraftfahrerinnen und Kraftfahrer gewährleisten. Unsere Arbeitsgruppe hat sich daher in den letzten Jahren mit der Entwicklung und Validierung eines multifaktoriellen Screeningverfahrens beschäftigt. In diesem Beitrag werden Konstruktion, eine erste Validierung sowie Angaben zur praktischen Anwendung des neuen Verfahrens „Seniorenberatung Aufgrund Fahreignungsrelevanter Einschränkungen – revidierte Fassung“ (engl.: Safety Advice For Elderly drivers – revised version) (SAFE-R) vorgestellt. Der SAFE-R ermöglicht die überprüfung von 11 evidenzbasierten fahrsicherheitsrelevanten Risikofaktoren bei älteren Kraftfahrer_innen mit und ohne leichte kognitive Beeinträchtigung. In einer On-Road-Studie an 74 älteren Menschen mit und ohne leichte kognitive Beeinträchtigung konnte der SAFE-R mit einer Sensitivität von 95 % und einer Spezifität von 75 % zwischen fahrsicheren und fahrunsicheren Kraftfahrer_innen differenzieren. Der SAFE-R stellt somit ein valides und zugleich ökonomisches Instrument zur Einschätzung der Fahrsicherheit von Senior_innen mit und ohne leichte kognitive Beeinträchtigung dar.

The aim of the present review was to identify all outpatient, family-oriented complex interventions for children of mentally ill parents known in the German-speaking countries on the basis of defined minimum requirements and to give an overview of their structure, content and proof of effectiveness.The interventions were identified by means of internet and literature research. If the defined criteria were met, the providers were contacted and asked to participate in a written survey.A total of 512 offers could be identified, 46 of which were to be classified as family-oriented complex interventions. Only a few interventions have been systematically evaluated so far.There is a large number of interventions for the children of mentally ill parents, but family-oriented complex interventions are rare. There is also a considerable need for evaluation studies.ZIEL: Ziel der vorliegenden Übersichtsarbeit ist es, alle im deutschsprachigen Raum bekannten ambulanten, familienorientierten Komplexangebote für Kinder psychisch kranker Eltern anhand definierter Minimalanforderungen zu identifizieren und einen Überblick über deren Struktur, Inhalt und Wirksamkeitsnachweise zu geben.Die Angebote wurden mittels Internet- und Literaturrecherche identifiziert. Bei Erfüllung der definierten Kriterien, wurden die Anbieter kontaktiert und um die Teilnahme an einer schriftlichen oder telefonischen Befragung gebeten.Insgesamt konnten 512 Angebote identifiziert werden, von denen 46 als familienorientiertes Komplexangebot einzustufen waren. Nur wenige Angebote wurden bisher systematisch evaluiert.Es besteht eine Vielzahl an Interventionen für die Kinder psychisch kranker Eltern, familienorientierte Komplexangebote sind jedoch bisher selten. Zudem besteht ein erheblicher Bedarf an Evaluationsstudien.

Objective: The present study investigated predictors of treatment attendance among 226 women with posttraumatic stress disorder (PTSD) and substance use disorder (SUD). Participants received either an integrated intervention for PTSD and SUD ("Seeking Safety") or a relapse prevention training (RPT) as part of a multicenter randomized controlled trial. Method: Beta-binomial regression was conducted to investigate baseline sociodemographic, motivational, mental health and substance use predictors of session attendance. Treatment by predictor interactions were included to identify treatment-specific predictors. Results: Session attendance was predicted by employment status, drug use severity and abstinence status. Higher drug use severity and unemployment were associated with less session attendance. The effect of abstinence status was treatment-specific, with abstinent participants in RPT attending most sessions. Conclusions: Considering individual characteristics could enhance session attendance in outpatient treatment for women with PTSD and SUD. This might include matching treatment concepts to abstinence status, the identification of attendance barriers in unemployed women and more intensive treatment settings for those with severe drug use.
