La presente investigacion tuvo como objetivo establecer la relacion entre la dependencia emocional y los modos de afrontamiento al estres en mujeres victimas de la violencia infligida por la pareja. La muestra estuvo conformada por 215 mujeres de un programa social de Chimbote. Se utilizaron como instrumentos el Cuestionario de Dependencia Emocional de Lemos y Londono y el Cuestionario de Modos de Afrontamiento al Estres de Carver,Scheier, y Weintraub. Los resultados encontrados se evidencio que no existe una relacion significativa entre la dependencia emocional y los modos de afrontamiento al estres en las mujeres victimas de violencia infligida por la pareja (p > 0,05). Se concluye que la mayoria de las mujeres victimas de violencia infligida por su pareja 71,6% no presentan dependencia emocional y el 28.4% si presentan. Ademas, las mujeres que son victimas de violencia hacen uso dependiendo de las circunstancias de estos modos de afrontamiento: afrontamiento activo, planificacion, postergacion del afrontamiento, aceptacion, acudir a la religion, analisis de emociones y distraccion en un 54,5%, 54%, 68,4%, 54,9%, 68,4%, 51,6% y 67,4% respectivamente y muy pocas veces hacen uso de los siguientes modos de afrontamiento: Supresion de otras actividades, negacion y conductas inadecuadas en un 53,5%, 57,7% y 78,6% respectivamente.

La situacion actual asociada al COVID-19 ha generado aumento en la carga psicologica de las personas, que podria estar incidiendo en el aumento de suicidios, por lo que es importante disenar programas que reduzcan el riesgo suicida. El proposito de este estudio fue evaluar el efecto que produce un programa socio afectivo en la disminucion del riesgo suicida en adolescentes. Se utilizo como instrumento la Escala de Riesgo Suicida en base al DSM IV y CIE 10, adaptada y validada (indice de confiabilidad alto, 0,899). Asimismo, se elaboro un programa socioafectivo para disminuir el riesgo suicida en los adolescentes. La muestra estuvo conformada por 63 adolescentes divididos en un grupo experimental (35) y en un grupo control (28). Se observo que, despues de la aplicacion del programa, la mediana de las puntuaciones del riesgo suicida en el grupo experimental fue igual al grupo control, es decir, las puntuaciones del riesgo ideacion e intento suicida no disminuyeron significativamente. Aunque el programa no ha sido efectivo, da informacion relevante para que el diseno de nuevos programas de prevencion e intervencion incorporando la neurociencia que permita investigar el funcionamiento de cerebro en el suicida.

En este trabajo hacemos una exposición del concepto de persona que A. J. Ayer desarrolla en su ensayo The Concept of a Person (1963). El objetivo de esta exposición es doble: por un lado, pretendemos mostrar un aspecto de la filosofía de Ayer que suele ser menos explorado que otros; y por otro lado, intentamos recuperar un experimento mental que puede ser interesante para el debate contemporáneo sobre la naturaleza de las personas.

Resumen La Seccion de Via Aerea de la Sociedad Catalana de Anestesiologia, Reanimacion y Terapeutica del Dolor (SCARTD) presenta la actualizacion de las recomendaciones para la evaluacion y manejo de la via aerea dificil con el fin de incorporar los avances tecnicos y los cambios observados en la practica clinica desde la publicacion de la primera edicion en 2008. La metodologia elegida fue la adaptacion de 5 guias internacionales recientemente publicadas, cuyo contenido fue previamente analizado y comparado de forma estructurada, y el consenso de expertos de los 19 centros participantes. El documento final fue sometido a la valoracion de los miembros de la SCARTD y a la revision por parte de 11 expertos independientes. Estas recomendaciones estan pues sustentadas en la evidencia cientifica actualmente disponible y en un amplio acuerdo de los profesionales de su ambito de aplicacion. En esta edicion se amplia la definicion de via aerea dificil, abarcando todas las tecnicas de manejo, y se hace mayor hincapie en la valoracion de la via aerea y en la clasificacion en 3 categorias segun el potencial grado de dificultad y las consideraciones de seguridad adicionales, que guiaran la planificacion de la estrategia a seguir. La preparacion previa al manejo de la via aerea, no solo relativa al paciente y al material, sino tambien a la comunicacion e interaccion entre todos los agentes implicados, ocupa un lugar destacado en todos los escenarios incluidos en el presente documento. El texto refleja el aumento progresivo del uso de los videolaringoscopios y de los dispositivos de segunda generacion en nuestro entorno y promueve tanto su uso electivo como el uso precoz en la via aerea no prevista. Tambien recoge la creciente utilizacion de la ecografia como herramienta de apoyo en la exploracion y toma de decisiones. Se han abordado nuevos escenarios como el riesgo de broncoaspiracion y la extubacion considerada dificil. Finalmente, se trazan las lineas maestras de los programas de entrenamiento y formacion continuada en via aerea necesarios para garantizar la implementacion efectiva y segura de las recomendaciones.

The Airway Division of the Catalan Society of Anaesthesiology, Intensive Care and Pain Management (SCARTD) presents its latest guidelines for the evaluation and management of the difficult airway. This update includes the technical advances and changes observed in clinical practice since publication of the first edition of the guidelines in 2008. The recommendations were defined by a consensus of experts from the 19 participating hospitals, and were adapted from 5 recently published international guidelines following an in-depth analysis and systematic comparison of their recommendations. The final document was sent to the members of SCARTD for evaluation, and was reviewed by 11 independent experts. The recommendations, therefore, are supported by the latest scientific evidence and endorsed by professionals in the field. This edition develops the definition of the difficult airway, including all airway management techniques, and places emphasis on evaluating and classifying the airway into 3 categories according to the anticipated degree of difficulty and additional safety considerations in order to plan the management strategy. Pre-management planning, in terms of preparing patients and resources and optimising communication and interaction between all professionals involved, plays a pivotal role in all the scenarios addressed. The guidelines reflect the increased presence of video laryngoscopes and second-generation devices in our setting, and promotes their routine use in intubation and their prompt use in cases of unanticipated difficult airway. They also address the increased use of ultrasound imaging as an aid to evaluation and decision-making. New scenarios have also been included, such as the risk of bronchoaspiration and difficult extubation Finally, the document outlines the training and continuing professional development programmes required to guarantee effective and safe implementation of the guidelines.

Como resultados resaltan que existen diferencias significativas al realizar la comparación de la impulsividad según las edades y el año escolar, siendo los adolescentes de 14 años de edad, de sexo masculino y que viven con sus dos padres los que presentan mayor índice de impulsividad. The present study aims to describe and compare impulsivity with sociodemographic factors in adolescents in the city of Lima. As results, they highlight that there are significant differences when comparing impulsivity according to age and school year, with 14-year-old male adolescents who live with their two parents presenting the highest rate of impulsivity. Allogeneic stem cell transplant (alloSCT) is a current treatment option for patients with refractory/relapsed classic Hodgkin lymphoma (CHL), including those who have failed an autologous transplantation. We performed a retrospective multicenter analysis of 113 patients (median age 28 years; range 14-56; 54% males) with refractory/relapsed (R/R) CHL who had undergone alloSCT in Argentina. Kaplan-Meier was used to estimate overall (OS) and progression-free survival (PFS). Relapse rate (RR) and non-relapse mortality (NRM) were estimated with cumulative incidence analysis. Disease status at transplant was complete remission (CR) in 39%, partial remission (PR) in 44%, and stable/progressed disease (S/PD) in 17% of the patients. Donor type was matched related (MRD) in 60%, unrelated (URD) in 19%, and haploidentical (HID) in 21% of the patients. OS and PFS at 2 years were 43% and 27%, respectively, for all the cohort. In the univariate analysis, patients in CR showed better OS (p ≤ 0.001) and PFS (p ≤ 0.001), and lower NRM (p = 0.04). HID had better PFS (p = 0.04) and lower RR (p = 0.02). In the multivariate analysis, CR showed a significant impact on OS and PFS, and HID on PFS. AlloSCT is a feasible procedure in patients with CHL. Those in CR at the time of the transplant had better outcomes. Haploidentical transplantation is associated with better PFS in these patients with poor prognosis.

Nodular lichen myxoedematosus is a localised form of lichen myxoedematosus, a chronic idiopathic cutaneous mucinosis of known aetiology. Ustekinumab is a human interleukin-12/23 monoclonal antibody that could directly or indirectly increase mucin production. Herein, we report for the first time a case of nodular lichen myxoedematosus associated with ustekinumab.

There have been several efforts to predict mortality after autologous stem cell transplantation (ASCT), such as the hematopoietic cell transplant-comorbidity index (HCT-CI), described for allogeneic stemcell transplantation and validated for ASCT, but there is no composite score in the setting of ASCT combining comorbidities with other clinical characteristics. Our aim is to describe a comprehensive score combining comorbidities with other clinical factors and to analyze the impact of this score on nonrelapse mortality (NRM), overall survival (OS), and early morbidity endpoints (mechanical ventilation, shock or dialysis) after ASCT. For the training cohort, we retrospectively reviewed data of 2068 adult patients who received an ASCT in Argentina (October 2002 to June 2017) for multiple myeloma or lymphoma. For the validation cohort, we analyzed 2168 ASCTs performed in the Medical College of Wisconsin and Spanish stem cell transplant group (Grupo Espanol de Trasplante Hematopoyetico (GETH)) (January 2012 to December 2018). We first performed a multivariate analysis for NRM in order to select and assign weight to the risk factors included in the score (male patients, aged 55 to 64 and >= 65 years, HCT-CI >= 3, Hodgkin lymphoma and non-Hodgkin lymphoma). The hazard ratio for NRM increased proportionally with the score. Patients were grouped as low risk (LR) with a score of 0 to 1 (686, 33%), intermediate risk (IR) with a score of 2 to 3 (1109, 53%), high risk (HR) with a score of 4 (198, 10%), and very high risk (VHR) with a score of >= 5 (75, 4%). The score was associated with a progressive increase in all the early morbidity endpoints. Moreover, the score was significantly associated with early NRM (day 100: 1.5% versus 2.4% versus 7.6% versus 17.6%) as well as long term (1 to 3 years; 1.8% to 2.3% versus 3.8% to 4.9% versus 11.7% to 14.5% versus 25.0% to 27.4%, respectively; P

We studied rates of granulocyte and platelets recovery in 359 consecutive subjects receiving blood cell infusions in the context of autotransplants for plasma cell myeloma (N = 216) and lymphomas (N = 143). Blood cells were mobilised with filgrastim given for 4–5 days and collected after a median of 2 (range, 1–2) apheresis. Apheresis products were stored at 4° C for a median of 3 days (range, 2–6 days). Most subjects received carmustine, etoposide, cytarabine and melphalan (BEAM), cyclophosphamide, carmustine and etoposide (CBV) or high-dose melphalan. Filgrastim was given post transplant to 319 subjects. Median numbers of mononuclear cells collected was 31 × 10E + 6/kg (interquartile range (IQR) 37 × 10E + 6 cells/kg). Median numbers of CD34-positive cells collected was 3.6 × 10E + 6/kg (IQR 3.8 × 10E + 6/Kg). Median viability after collection was 90% (IQR 7%) after storage, 88% (IQR 12%). A total of 255 of 256 evaluable subjects recovered bone marrow function and there was no late bone marrow failure. Median interval to neutrophils >0.5 × E + 9/L was 13 days (range, 9–39 days) and to platelets >20 × 10E + 9/L, 16 days (range, 7–83 days). These rates and ranges seem comparable to those reported after autotransplants of frozen blood cells. There was no correlation between numbers of storage days at 4 °C and viability afte storage (r = −0.018, p = 0.14)) nor rates of recovery of neutrophils (r = −0.054, p = 0.52) or platelets (r = 0.116, p = 0.14). Blood cells collected for autotransplant can be stored at 4 °C for 6 d. This method is simple, inexpensive and widely applicable.

Aunque las ventajas representativas de los diagramas han sido ampliamente reconocidas, sus virtudes inferenciales no siempre han recibido la misma atención. Existe una tradición que sostiene que el razonamiento basado en pruebas lingüísticas es esencial en lógica y matemáticas, pero que el razonamiento basado en diagramas no lo es; sin embargo, gracias a ciertos proyectos de investigación sobre razonamiento diagramático, hoy tenemos modelos que nos ayudan a comprender de manera más precisa los conceptos de diagrama lógico e inferencia diagramática. Siguiendo algunos de los planteamientos de estos modelos, en esta contribución llevamos a cabo un estudio comparativo de diez sistemas diagramáticos en términos lógicos y representativos con el propósito de ofrecer una respuesta a la pregunta de qué es lo que hace que un diagrama sea un diagrama lógico. Nuestra respuesta preliminar es que un diagrama lógico es un diagrama dentro de un sistema diagramático que es correcto y completo con respecto a una clase de inferencias válidas dada una base deductiva. Esta definición parece ser adecuada porque nos permite incluir a aquellos sistemas de diagramas que, por sus propiedades, típicamente consideramos como diagramas lógicos bona fide, pero excluye diagramas que no solo son incorrectos o incompletos, sino aquellos que ni siquiera son inferenciales.

We aimed at analyzing the outcome of allogeneic stem cell transplant (ASCT) in adult patients with acute lymphoblastic leukemia (ALL), comparing Haploidentical (Haplo) with HLA-matched (sibling and unrelated) donors. Between 2008 and 2017, we collected data from 236 patients (median age 31 years; range 16–64; 90% HCT-CI 0–1) who underwent unmanipulated ASCT in first complete remission and subsequent remissions in 15 Argentinian centers. Donors were HLA-matched (n = 175; 74%) and Haplo (n = 61; 26%). Two-year overall survival (OS) was 55% (95% CI 47–63) for the HLA-matched group and 49% (95% CI 34–62) for the Haplo group (p = 0.351). For OS, crude HR, adjusted HR for covariates (HR 1.24; 95% CI 0.77–1.99; p = 0.363) and HR including a propensity score in the model (HR 1.22; 95% CI 0.71–2.08; p = 0.414) showed no impact of donor category on the OS. No difference was found in terms of nonrelapse mortality, relapse, leukemia-free survival, and grade 3–4 acute graft-versus-host disease (GVHD); 2-year incidence of chronic GVHD was higher in HLA-matched vs Haplo group (p = 0.028). Patients with ALL who underwent ASCT were young subjects with low HCT-CI. In this setting, a Haplo donor represents an alternative widely available in the absence of an HLA-matched donor. Relapse remains a challenge for all donor categories.

In this paper we attempt to do something rather simple: to meet the advances of Sommers and Englebretsen (namely, a plus-minus algebra for syllogistic) together with the developments of Peterson and Thompson (namely, an extension of syllogistic with “most,” “many,” and “few”). The result is an intermediate syllogistic that copes with a wide range of logical patterns but with the virtues of an algebraic approach. Neste artigo, tentamos fazer algo bastante simples: conhecer os avanços de Sommers e Englebretsen (a saber, uma álgebra mais-menos para silogística) juntamente com os desenvolvimentos de Peterson e Thompson (ou seja, uma extensão da silogística com “a maioria”, “Muitos” e “poucos”). O resultado é uma silogística intermediária que lida com uma ampla gama de padrões lógicos, mas com as virtudes de uma abordagem algébrica.

5504 Background: Mirvetuximab soravtansine (MIRV) is an ADC comprising a FRα-binding antibody, cleavable linker, and the maytansinoid DM4, a potent tubulin-targeting agent. As part of the Phase 1b FORWARD II trial (NCT02606305), the combination of MIRV with bevacizumab (BEV) was evaluated in pts with FRα-positive (medium/high expression; ≥50%/ ≥75% of cells with PS2+ staining intensity), platinum agnostic ovarian cancer, defined as pts with either platinum resistant (PROC) (recurrence within 6 months after last platinum dose) or platinum sensitive (PSOC)responded to the last platinum therapy and did not progress within 6 months) for whom a non-platinum based doublet would be appropriate. Methods: Pts received MIRV (6 mg/kg; adjusted ideal body weight) and BEV (15 mg/kg) on Day 1 of a 21-day cycle. Responses were assessed by investigator according to RECIST 1.1 and adverse events (AEs) evaluated by CTCAE v4.03. Results: In total, 60 pts received the combination, with a median age of 60 years, a median of 2 prior lines of systemic therapy (range 1-4) and a median follow-up of 17.5 months. The cohort included 32 pts (53%) with PROC disease and 28 (47%) with PSOC disease. Objective responses were seen in 28 of 60 pts for a confirmed overall response rate (ORR) of 47% (95% CI, 34, 60), median duration of response (mDOR) of 9.7 months (95% CI 6.7, 12.9), and median progression free survival (mPFS) of 8.3 months (95% CI 5.6, 10.6). In pts with high FRα expression (n=33), the confirmed ORR was 64% (95% CI 45, 80), mDOR of 11.8 months (95% CI 6.7, 13.7), and mPFS of 10.6 months (95% CI 8.3, 13.3); efficacy results in PROC and PSOC subsets of pts with high FRα expression are shown in the table below. The most common treatment related AEs (all grade, grade 3+) were diarrhea (68%, 2%), blurred vision (63%, 2%), fatigue (58%, 7%), and nausea (57%, 0%). The most common treatment related grade 3 and 4 AEs were neutropenia and hypertension, (12%, 3% and 13%, 0%, respectively); all other grade 3+ events occurred in ≤ 10% of pts. Conclusions: The combination of MIRV with BEV demonstrates impressive anti-tumor activity with durable responses and favorable tolerability in high FRα recurrent ovarian cancer. These results build on data previously reported for MIRV/BEV in PROC patients ( Gyn Oncology O’Malley, et al 2020), suggesting that MIRV has the potential to be a preferred partner for BEV in patients with high FRα recurrent ovarian cancer regardless of platinum sensitivity. Diastereoselective quaternization of ortho-sulfinylbenzyl-methylpropargylic carbanions has been carried out with various allylic, propargylic, and benzylic halides. The stereoselectivity is efficiently controlled by the sulfinyl group, acting as a remote chiral auxiliary. Desulfinylation of the resulting compounds with tBuLi provides access to enantiopure 1,5-enynes, 1,5-diynes, and 4-arylalkynes bearing all-carbon quaternary propargylic centers.

Background The assessment of discrepancies between surgical and histopathological measurements of specimens is important in order to avoid repeat surgery and unnecessary follow-ups. Objectives The objective of this study was to quantify the degree, time and influential factors of shrinkage of cutaneous surgical specimens. Methods Data of 111 patients were gathered on age, sex, localization, diagnosis and specimen width and length before surgical excision (in vivo), at 5 min postsurgery (ex vivo) and after 24 h of fixation in 10% buffered formalin (postfixation).
