Abstract

IntroductionContinuous renal replacement therapy (CRRT) is used for the symptomatic management of acute kidney injury (AKI) and fluid overload (FO). Contemporary reports on pediatric CRRT are small and single center in design. Large international studies evaluating CRRT practice and outcomes are lacking. Herein, we describe the design of a multinational collaborative.MethodsThe Worldwide Exploration of Renal Replacement Outcomes Collaborative in Kidney Disease (WE-ROCK) is an international collaborative of pediatric specialists whose mission is to improve short- and long-term outcomes of children treated with CRRT. The aims of this multicenter retrospective study are to describe the epidemiology, liberation patterns, association of fluid balance and timing of CRRT initiation, and CRRT prescription with outcomes.ResultsWe included children (n = 996, 0-25 years) admitted to an intensive care unit (ICU) and treated with CRRT for AKI or FO at 32 centers (in 7 countries) from 2018 to 2021. Demographics and clinical characteristics before CRRT initiation, during the first 7 days of both CRRT, and liberation were collected. Outcomes include the following: (i) major adverse kidney events at 90 days (mortality, dialysis dependence, and persistent kidney dysfunction), and (ii) functional outcomes (functional stats scale).ConclusionThe retrospective WE-ROCK study represents the largest international registry of children receiving CRRT for AKI or FO. It will serve as a broad and invaluable resource for the field of pediatric critical care nephrology that will improve our understanding of practice heterogeneity and the association of CRRT with clinical and patient-centered outcomes. This will generate preliminary data for future interventional trials in this area.

20. Critical Considerations of the Challenges of Teaching National Literatures in Australia in the 21st Century

Abstract

Stories and literature play an important and necessary role in understanding the past and in creating the future. Yet, in colonised countries such as Australia, the status of contemporary national texts, particularly those reflecting the diverse voices of Indigenous writers, women, and other marginalised groups, continue to be underrepresented in schools. In this article, we explore the reasons why Australian texts continue to be marginalised in education, when there is a clear desire for diverse Australian voices amongst the reading public. We also consider the kinds of national identities that current text selection and teaching policies are creating and maintaining in schools. In order to develop these ideas, we draw on data collected via a survey of secondary English teachers for a pilot research project called "Teaching Australia." In this project, we explored the Australian texts being selected and championed by teachers in secondary schools, the perceptions of nation and national identity these convey, and teachers' approaches to including diverse Australian texts in their classrooms. In the final section of the paper, we consider some future opportunities for engagement with diverse Australian literature for teachers and students of English.

Abstract

Assessment of pressure ulcer (PU) risk remains a challenge in clinical practice. The first part of this article (Martin and Holloway, 2024) discussed the evidence base underpinning the development of clinical guidelines for PUs alongside the creation of the bundle approach regarding PU prevention. This article, part two, presents the results of a clinical audit that explores compliance against a PU prevention bundle (the aSSKINg framework) in an adult community nursing setting in the south-east of England. The clinical audit was conducted between July-December 2021 and included records of 150 patients. Overall, compliance against the aSSKINg framework was poor, with only two criterion being met: equipment provision for chair and referral to the tissue viability team. Short-term recommendations are that mandatory PU training for staff in the management of PUs should be implemented, and the tissue viability nurse network should be increased. The long-term recommendation was the introduction of the aSSKINg framework as a template into the electronic patient record. [ABSTRACT FROM AUTHOR]

30. Key Instructional Practices of Effective Elementary School Teachers of English as a Foreign Language

Abstract

Educational improvement is at the forefront of ongoing international concern, with the quality of teaching recognised as a crucial variable in this endeavour. While in the past, instructional practices identified as effective were thought to be generic, it has become increasingly clear that the identification of contextually relevant practices is more likely to inform the notion of what constitutes effective teaching in a given context. This study addresses this issue in the context of English as a Foreign Language in Israeli elementary schools. Placed within social and constructivist perspectives of learning, the qualitative, multiple case study explores the instructional practices of five effective teachers through classroom observations and interviews. The findings show that all teachers provide significant opportunity for knowledge construction using a variety of techniques and strategies. The notion of relevance was also a central facet of the instructional practices of some teachers. The enquiry concludes by suggesting that the findings could be used as a basis to explore the extent to which the aspects of instruction identified features in the practice of effective teachers in a wider context, with a view to developing a national model of pedagogy to promote much-needed and sustainable educational improvements in Israel.

32. A Portrait of Chinese Culture: Investigating Perceptions Presented in Children's Books

Abstract

Administrators, librarians, parents, teachers, and teacher educators need to be familiar with quality multicultural children's literature as a means of helping children develop an understanding of others as well as affirming their own diverse backgrounds. In this study, 31 fictional picture books identified as containing representations of ethnic Chinese or their culture were examined for literary quality and cultural authenticity. Six reviewers (three Chinese and 3 American) independently evaluated each book using a revised 10-item version of the Multicultural Children's Literature Evaluation Tool (Higgins, J. J. (2002). Multicultural children's literature: creating and applying an evaluation tool in response to the needs of urban educators. New Horizons in Education. Results indicated good interrater reliability with the mean score of the Chinese and American reviewers differing by less than 5 points for 24 books (80 %). Three books received a mean score of 30 by at least one group of reviewers, and three books received a mean score of below 19 by at least one group of reviewers. With results of studies such as this one at their disposal, adults are better prepared to select quality, culturally authentic literature to share with children.

33. Reading in the (Post)Digital Age: Large Databases and the Future of Literature in Secondary English Classrooms

Abstract

While "born digital" artefacts such as video games and e-books have been part of secondary school English in Anglophone countries for over two decades, databases of mass-digitised (hence "re-mediated") literary texts are yet to have a significant presence in, or influence on, literary work in subject English. The authors contend that engagement with these digitised texts requires a postdigital "literary literacy". They explore how "distant reading" -- applying digital tools to large-scale data to identify patterns beyond the scale of human perception -- offers a form of postdigital literary literacy that can be enacted alongside others, including productive reading and code switching. Using the example of the To be continued… database of fiction originally published in 19th and 20th century Australian newspapers, the article argues that postdigital literary objects offer new literary knowledge and new ways of thinking about literary study and teacher expertise in English in the 21st century.

35. 'We Do More than Discuss Good Ideas': A Close Look at the Development of Professional Capital in an Elementary Education Liaison Group

Abstract

In an era when many news media, policy makers, and professionals in the field may consider teacher education "under attack," teacher education programs are being held accountable for increased rigor (Council of Chief State School Officers, 2012). Teacher educators are in a unique position to examine more closely specific practices and teacher education as a profession to enhance program quality and candidate outcomes. Toward that end, the authors focused on work within a "community of practice" (Wenger, 1998) for this inquiry. Faculty who work in elementary school settings at least one day per week, serving as liaisons to partner schools and supervising teacher candidates, made up this community Faculty at this institution worked collaboratively to share leadership and go against the grain of institutional hierarchical structures (Martin, Snow, Osguthorpe, Coll, & Boothe, 2012). Within this context, they created a space for clinical supervisors to share publicly their work with each other and enact change, as they engaged in professional development through the Elementary Education Liaison Group (EELG). In this study, the authors designed a systematic investigation of EELG practice and its internal and external influence, focusing on two research questions: (1) How do participants experience the EELG context and its influence on how they learn and develop? and (2) How does the elementary education community of practice influence individual and programmatic change? Herein, they describe their theoretical framework for professional development and the resulting emphasis on professional capital of a particular group of educators (Hargreaves & Fullan, 2012). Next, they outline the research design and describe and three key themes connected to teacher educator professional development. The authors conclude with a discussion on professional capital and the power of collective activity to transform teacher educator development and teacher education contexts for program transformation.

Abstract

Background: Improving the sexual health of young people is a key policy focus in Scotland. As the influence of the internet has grown within a rapidly changing health information landscape, so have opportunities for both sexual health promotion, and concerns about the challenges faced by young people in navigating this online environment. This study explores young people’s sexual health literacy (SHL), primarily within this online context. Methods: Paired interviews with friendship groups and observational online activities were used to explore young people’s experiences of finding, understanding and evaluating online sexual health information. A purposive sample of 49 participants (aged 16-19), diverse in terms of gender, sexuality and religion, were recruited from across Scotland from areas that varied in terms of deprivation and urban/rural classification. Findings: Participants varied in their confidence and ability to find and identify reliable information, and typically regarded identifying and filtering reliable sources as challenging. Barriers to accessing information on websites included: inaccessible language; inappropriate or non-relatable information; and websites that were difficult to navigate or did not function correctly. Concerns about stigma and ‘being seen’ seeking sexual health information was a key barrier. Stark differences, often mediated by gender, sexuality and educational circumstances, emerged in perspectives towards accessing sexual health information and support online. Findings suggest that different social media platforms present different opportunities and challenges; for example, social content sharing services such as YouTube may be useful venues for developing critical SHL, while social networking sites such as Facebook, may be less suitable to user’s active engagement in identity construction. Dissatisfaction with school-based sexual health education appears to be a catalyst for online information-seeking, but school-based sexual health education did little to equip young people to use the online environment effectively. Conclusions: Gender, sexual identity, stigma, structural factors and social support converge and intersect around young people’s SHL. A broad range of targeted interventions are needed to improve SHL, focusing on overcoming stigma, presenting positive messages and developing interactive and critical skills. Schools could do more to develop SHL skills, including teaching the digital and critical skills to seek and appraise online information. Expanding online sexual health services may effectively complement traditional services and encourage uptake, but it is essential that research establishes a robust, comprehensive conceptualisation of SHL, and develops measurement tools specific to SHL such that interventions can be evaluated and refined.
