8. Tuberculosis Screening on a Health Science Campus: Use of QuantiFERON-TB Gold Test for Students and Employees

Abstract

Detecting and managing "Mycobacterium tuberculosis" (TB) infection in a health-science center population is a clinical dilemma. Tuberculin skin tests are still the preferred method for detecting present or past infection of TB. The authors discuss the performance of whole blood interferon gamma release assay test commercially known as QuantiFERON-TB Gold Test (QFT-G) as an additional tool for a TB surveillance program for the students and employees at an academic medical center. QFT-G was successfully implemented as a confirmatory test for screening of TB infection. The following are appended: (1) Contraindications for QuantiFERON-TB Gold Test; and (2) Advantages/Disadvantages of QuantiFERON-TB Gold Test (QFT-G). (Contains 1 table and 1 figure.)

9. Teaching Evidence-Based Practice to Undergraduate Nursing Students: Overcoming Obstacles

Abstract

Evidence-based practice is highly valued in health care literature at this time. But research suggests that U.S. RNs face many obstacles when implementing evidence-based practice including a lack of value for research in practice (Pravikoff et al, 2005). Additional obstacles may exist for traditional U.S. BSN nursing students who may not value the importance of learning about evidence-based practice principles or of implementing evidence-based practice in the clinical setting. If we are to improve the use of evidence-based practice among U.S. RNs, learning and valuing the process must begin during the basic nursing educational program. This presentation outlines classroom research designed to uncover the specific obstacles to learning and implementing evidence-based practice described by traditional BSN students at a small, private, Catholic college just before they complete the final clinical internship before graduation. Several teaching strategies were designed to address the obstacles students described. A post-internship survey of the same students determined the effectiveness of the various teaching strategies. Recommendations for future research and for teaching strategies to enhance the learning and valuing of evidence-based practice among traditional U.S. BSN students are offered. (Contains 3 tables.)

Abstract

We developed a pandemic telephone outreach protocol to identify risk for social isolation, health destabilization, medication issues, inadequate services and supports, and caregiver stress among older adults at high risk of destabilization. Screening, conducted between April 1, 2020, and May 8, 2020, was targeted to those who had previously been screened as frail or who were identified as vulnerable by their family physician. This study describes the implementation and results of this risk screening protocol and describes patient, caregiver, and health professional perceptions of this outreach initiative. Mixed methods included satisfaction surveys and interviews completed by patients/caregivers (N = 300 and N = 26, respectively) and health professionals (N = 18 and N = 9, respectively). A medical record audit collected information on patient characteristics and screening outcomes. A total of 335 patients were screened in the early weeks of the pandemic, of whom 23% were identified with at least one risk factor, most commonly related to the potential for health destabilization and medication risk. Follow-up referrals were made most frequently to physicians, a pharmacist, and a social worker. The outreach calls were very well received by patients and caregivers who described feeling cared for and valued at a time when they were socially isolated and lonely. The outreach calls provided access to trusted COVID-19 information and reassurance that health care was still available. The majority of health professionals (>86%) were "very" or "extremely" satisfied with the ease of completing the screening via telephone and value for time spent; for 79% the protocol was "very" or "extremely" feasible to implement. Health professional interviews revealed that patients were unaware they could access care during the pandemic lockdown but were reassured that care was available, potential crises were averted, and they supported future implementation. Risk screening provides a significant opportunity to provide information, support, and mitigate potential risks and is an important and feasible component of pandemic planning in primary care. 12. Apprenticeship - School Linkage Implementation Manual.

Abstract

Developed to assist interested sponsors in implementing apprenticeship-school linkage projects, this guide is intended to organize the collective experiences of those who have implemented the demonstration projects to highlight the day-to-day mechanics involved. Section 1 overviews apprenticeship-school linkage. In section 2 factors are described that should be considered during the planning phase, including choosing a task force, allotting time for planning, school involvement, industrial involvement, goal setting, size/scope of project, targeting, legal concerns, and organization and structure. Section 3 focuses on activities, staff selection and training, project promotion, linking with schools, student recruitment, and job development. Project administration is the focus of section 4. It considers coordination of roles and responsibilities with registration agency staff; monitoring, accountability, and control; registration activities; staffing; single versus mutiple school district involvement; state variation in policy and law; and related instruction and transition. Section 5 highlights basic management principles. Section 6 describes contractor types and their strengths and weaknesses: state education agencies, postsecondary institutions, single school system, and private, non-profit corporations. 22. Implementation of Post-Transplant Care Guidelines for Allogeneic Hematopoietic Stem Cell Transplant Recipients

Abstract

Purpose. The purpose of this quality improvement (QI) project was to implement an evidence-based follow-up guideline tool for allogeneic stem cell recipients who are over three months—or approximately 100 days—post-transplant at Banner MD Anderson Cancer Center (BMDACC). The goal was to encourage providers at BMDACC to utilize the guidelines to provide long-term follow-up care for allogeneic stem cell transplant patients after three months post-transplant.Background. Allogeneic (non-self-donor) hematopoietic stem cell transplant (HSCT) is a potentially curative procedure for several hematological malignancies. Allogeneic HSCT is associated with late complications—occurring after 100 days post-transplant—that impact survivors’ life expectancy and quality of life. Over 65,000 allogeneic HSCTs occur worldwide every year. Nearly 75% of survivors will experience at least one late complication. These complications physically, mentally, and emotionally affect survivors. Providing long-term follow-up care for these patients improves health and safety by preventing complications that put them at risk for early mortality. Methods. This QI project was implemented by providing education at a synchronous online meeting regarding allogeneic HSCT and late effects and introducing the guidelines. Two surveys were delivered via email to participants to complete before and after the education. A third survey was delivered via email three weeks after implementing the guidelines to evaluate provider use and impression. Results. Of the 12 individuals invited to participate, five participants responded to the pre-survey, one responded to the post-survey, and five responded to the follow-up survey. The one participant who completed the pre- and post-survey answered more questions correctly on the post-survey than the pre-survey. Abstract

The mediation of coronary blood flow by adenosine during hyperemia and ischemia was examined. This involved adenosine receptor blockage by the polar antagonist, 8-p-sulfophenyltheophylline, in test dogs. Arginine vasopresin was used to reduce coronary supply. The importance of adenosine in the regulation of coronary blood flow changes and its function in the genesis of compensatory vasodilation were established.

Abstract

A glyco-array platform has been developed, in which glycans are attached to plasmonic nanoparticles through strain-promoted azide-alkyne cycloaddition. Glycan-protein binding events can then be detected in a label-free manner employing surface-enhanced Raman spectroscopy (SERS). As proof of concept, we have analyzed the binding of Gal1, Gal3, and influenza hemagglutinins (HAs) to various glycans and demonstrated that binding partners can be identified with high confidence. The attraction of SERS for optical sensing is that it can provide unique spectral signatures for glycan-protein complexes, confirm identity through statistical validation, and minimizes false positive results common to indirect methods. Furthermore, SERS is very sensitive and has multiplexing capabilities thereby allowing the simultaneous detection of multiple analytes.

Abstract

Falls are a common and serious risk with an aging population. Chiropractors commonly see firsthand the effects of falls and resulting injuries in their senior patients and they can reduce falls risk through active screening. Ongoing research has provided proven approaches for making falls less likely. Screening for falls should be done yearly for all patients 65 years and older or in those with a predisposing medical condition. Additional specific falls prevention professional education would enable the chiropractor to best assist these patients. Collaboration and communication with the patient's family physician offers an opportunity for improved interprofessional dialogue to enhance patient care related to falls risk. Frequently falls prevention strategies are implemented by an interprofessional team. Chiropractors increasingly contribute within multidisciplinary teams. Collaboration by the chiropractor requires both simple screening and knowledge of health care system navigation. Such awareness can permit optimal participation in the care of their patient and the best outcome.Les chutes présentent un risque commun et grave chez une population vieillissante. Les chiropraticiens constatent habituellement directement les effets des chutes et les blessures conséquentes chez leurs patients âgés; et ils peuvent en réduire les risques grâce à un dépistage actif. Des recherches continues fournissent des méthodes vérifiées de réduction de la probabilité de chutes. Un dépistage des risques de chute doit être effectué chaque année pour tous les patients de 65 ans et plus, ou pour ceux dont l’état de santé les prédispose. Une formation professionnelle supplémentaire spécifique dans la prévention des chutes permettrait au chiropraticien de mieux aider ces patients. La collaboration et la communication avec le médecin de famille du patient offrent une occasion d’améliorer le dialogue interprofessionnel au profit de meilleurs soins prodigués au patient sur les risques de chute. Souvent les stratégies en matière de prévention des chutes sont mises en place par une équipe interprofessionnelle. Les chiropraticiens œuvrent de plus en plus au sein d’équipes multidisciplinaires. La collaboration des chiropraticiens nécessite des compétences pour de simples dépistages, ainsi que des connaissances pour s’orienter dans le système des soins de santé. De telles connaissances permettront au chiropraticien une participation optimale aux soins de son patient et l’obtention des meilleurs résultats.
