Abstract

ObjectiveOur objective was to gauge adherence to nationally endorsed protocols in implementation of pulse oximetry (POx) screening for critical congenital heart disease (CCHD) in infants after mandate by all states and to assess associated characteristics.Study designBetween March and October 2019, an online questionnaire was administered to nurse supervisors who oversee personnel conducting POx screening. The questionnaire used eight questions regarding performance and interpretation of screening protocols to measure policy consistency, which is adherence to nationally endorsed protocols for POx screening developed by professional medical societies. Multilevel linear regression models evaluated associations between policy consistency and characteristics of hospitals and individuals, state of hospital location, early versus late mandate adopters, and state reporting requirements.ResultsResponses from 189 nurse supervisors spanning 38 states were analyzed. Only 17% received maximum points indicating full policy consistency, and 24% selected all four options for potential hypoxia that require a repeat screen. Notably, 33% did not recognize ≤90% SpO2 as an immediate failed screen and 31% responded that an infant with SpO2 of 89% in one extremity will be rescreened by nurses in an hour rather than receiving an immediate physician referral. Abstract

In this work we face a challenging puzzle video game: A Good Snowman is Hard to Build. The objective of the game is to build snowmen by moving and stacking snowballs on a discrete grid. For the sake of player engagement with the game, it is interesting to avoid that a player finds a much easier solution than the one the designer expected. Therefore, having tools that are able to certify the optimality of solutions is crucial. Although the game can be stated as a planning problem and can be naturally modelled in PDDL, we show that a direct translation to SAT clearly outperforms off-the-shelf state-of-the-art planners. As we show, this is mainly due to the fact that reachability properties can be easily modelled in SAT, allowing for shorter plans, whereas using axioms to express a reachability derived predicate in PDDL does not result in any significant reduction of solving time with the considered planners. Abstract

Objective: To determine if the effective use of Health Information Technologies (HIT) and the Electronic Medical Record (EMR) affects emergency department (ED) usage in a complicated frequently presenting patient population.Methods: A retrospective, observational study of 45 patients enrolled in our Frequent User Program called Community Resources for Emergency Department Overuse (CREDO) between June 2005 and July 2007. The study was conducted at an urban hospital with greater than 95,000 annual visits. Patients served as their own historical controls. In this pre-post study, the pre-intervention control period was determined by the number of months the patient had been enrolled in the program. The pre- and post-intervention time periods were the same for each patient but varied between patients. The intervention included using HIT to identify the most frequently presenting patients and creating individualized care plans for those patients. The care plans were made available through the EMR to all healthcare providers. Study variables in this study intervention included ED charges, lab studies ordered, number of ED visits, length of stay (LOS), and Total Emergency Department Contact Time (TEDCT), which is the product of the number of visits and the LOS. We analyzed these variables using paired T-tests. This study was approved by the institutional review board.Results: Forty-five patients were enrolled, but nine were excluded for no post enrollment visits; thus, statistical analysis was conducted with n=36. The ED charges decreased by 24% from $64,721 to $49,208 (p=0.049). The number of lab studies ordered decreased by 28% from 1847 to 1328 (p=0.04). The average number of ED visits/patient decreased by 25% from 67.4 to 50.5 (p=0.046). The TEDCT decreased by 39% from 443.7 hours to 270.6 hours (p=0.003).Conclusion: In this pre-post analysis of an intervention targeting ED frequent users, the use of HIT and the EMR to identify patients and store easily accessible care plans significantly reduced ED charges, labs ordered, number of ED visits, and the TEDCT. Abstract

Abstract Background Participant involvement in research studies is not a new concept, yet barriers to implementation remain and application varies. This is particularly true for pandemic response research studies, where timeframes are condensed, pressure is high and the value and inclusion of participant involvement can be overlooked. The SIREN Participant Involvement Panel (PIP) provides a case study for participant involvement in pandemic research, working in partnership with people who the research is for and about. Methods SIREN and the British Society for Immunology (BSI) recruited and ran two phases of the PIP, involving 15 members in total over a 16-month period. Phase 1 ran between January and August 2022 and Phase 2 between October 2022 and March 2023. Activity figures including recruitment interest and PIP meeting attendance were recorded. To evaluate how the PIP has influenced SIREN, feedback was collected from (a) researchers presenting at the PIP and (b) PIP members themselves. Evaluation at the end of Phase 1 informed our approach to Phase 2. Thematic grouping was planned to identify key lessons learned. Results Applications increased from n = 30 to n = 485 between Phase 1 and Phase 2 of the PIP, a more than 15-fold increase. The SIREN PIP positively impacted the design, implementation and evaluation phases of the study and sub-studies. Feedback from PIP members themselves was positive, with members highlighting that they found the role rewarding and felt valued. Learnings from the PIP have been condensed into five key themes for applying to future pandemic response research studies: the importance of dedicated resources; recruiting the right panel; understanding motivations for participant involvement; providing flexible options for involvement and enabling the early involvement of participants. Conclusions The SIREN PIP has demonstrated the value of actively involving people who research is for and about. The PIP has provided an active feedback mechanism for research and demonstrated a positive influence on both SIREN study researchers and PIP members themselves. This paper makes the case for participant involvement in future pandemic research studies. Future work should include improved training for researchers and we would support the development of a national PIP forum as part of future pandemic research preparedness.

20. X-ray diffraction studies of shock compressed bismuth using X-ray free electron lasers

Abstract

The ability to diagnose the structure of a material at extreme conditions of high-pressure and high-temperature is fundamental to understanding its behaviour, especially since it was found that materials will adopt complex crystal structures at pressures in the Terapascal regime (1TPa). Static compression, using the diamond anvil cell coupled with synchrotron radiation has to date been the primary method for structural studies of materials at high pressure. However, dynamic compression is the only method capable of reaching pressures comparable to the conditions found in the interior of newly discovered exo-planets and gas giants where such exotic high-pressure behaviour is predicted to be commonplace among materials. While generating extreme conditions using shock compression has become a mature science, it has proved a considerable experimental challenge to directly observe and study such phase transformations that have been observed using static studies due to the lack of sufficiently bright X-ray sources. However, the commissioning of new 4th generation light sources known as free electron lasers now provide stable, ultrafast pulses of X-rays of unprecedented brightness allowing in situ structural studies of shock compressed materials and their phase transformation kinetics in unprecedented detail. Bismuth, with its highly complex phase diagram at modest pressures and temperatures, has been one of the most studied systems using both static and dynamic compression. Despite this, there has been no structural characterisation of the phases observed on shock compression and it is therefore the ideal candidate for the first structural studies using X-ray radiation from a free electron laser. Here, bismuth was shock compressed with an optical laser and probed in situ with X-ray radiation from a free electron laser. The evolution of the crystal structure (or lack there of) during compression and shock release are documented by taking snapshots of successive experiments, delayed in time. The melting of Bi on release from Bi-V was studied, with precise time scans showing the pressure releasing from high-pressure Bi-V phase until the melt curve is reached off-Hugoniot. Remarkable agreement with the equilibrium melt curve is found and the promise of this technique has for future off-Hugoniot melt curve studies at extreme conditions is discussed. In addition, shock melting studies of Bi were performed. The high-pressure Bi - V phase is observed to melt along the Hugoniot where melting is unambiguously identified with the emergence of a broad liquid-scattering signature. These measurements definitively pin down where the Hugoniot intersects the melt curve - a source of some disagreement in recent years. Evidence is also presented for a change in the local structure of the liquid on shock release. The impact of these results are discussed. Finally, a sequence of solid-solid phase transformations is observed on shock compression as well as shock release and is detected by distinct changes in the obtained diffraction patterns. The well established sequence of solid-solid phase transformations observed in previous static studies is not observed in our experiments. Rather, Bi is found to exist in some metastable structures instead of forming equilibrium phases. The implications these results have for observing reconstructive phase transformations in other materials on shock timescales are discussed.

23. Ten Years of Frequent Users in an Inner City Emergency Department

Abstract

Introduction:The purpose of this study was to determine if differences could be detected in the presentation patterns and admission rates among frequent emergency department users (FEDU) of an urban emergency department over a 10-year period.Methods: This was an IRB approved, retrospective review of all patients who presented to the ED 5 or more times for 3 distinct time periods: “year 0” 11/98-10/99, “year 5” 11/03–10/04, and “year 10” 11/08–10/9. FEDU were grouped into those with 5–9, 10–14, 15–19, and ≥ 20 visits per year. Variables analyzed included number of visits, disposition, and insurance status. We performed comparisons using Kolmogorov-Smirnov and chi-square tests. A P < 0.05 was considered significant.Results: We found a a 66% increase in FEDU patients over the decade studied, with a significant increase in both the number of FEDU in each visit frequency category over the 3 time periods (P < 0.0001), as well as the total number of visits by each group of FEDU (P < 0.0001). The proportion of FEDU visits for the 5–9 group resulting in admission increased from 25.9% to 29% from year 0 to year 10 (P < 0.001), but not for the other visit groups. In comparing admission rates between FEDU groups, the admission rate for the 5–9 group was significantly higher than the ≥ 20 group for the year 5 time period (P < 0.001) and the year 10 time period (P < 0.001) and showed a similar trend, but not significant, at year 0 (P = 0.052). The overall hospital admission rate for emergency patients over the same time span remained stable at 22-24%. The overall proportion of uninsured FEDU was stable over the decade studied, while the uninsured rate for the overall ED population for the same time periods increased.Conclusion: The results demonstrate the FEDU population is not a homogeneous group of patients. Increased attention to differences among FEDU groups is necessary in order to plan more effective interventions. [West J Emerg Med. 2013;14(3):243–246.]

25. Looking for evidence of the new politics : globalisation, power and democracy

Abstract

This thesis takes as it starting point the three crises said to be facing liberal democracy: the crises of globalisation, fundamentalist identities and the decline of democratic legitimacy. When combined with the transformative potential of new communication technologies, these crises form the foundation for the new politics and promise a moment of revolutionary change for Western liberal democracies. This thesis looks at three apparently diverse responses to the new politics from the techno-liberal, social entrepreneur and resister schools and contends that, despite their differences, these schools share a number of core assumptions that cause them to limit the range of valid strategies of responses to the new politics. Their vision of globalisation shares the assumption that it is a novel, irresistible and culturally and politically homogenizing phenomenon. Their conception of power as a one-dimensional resource leads them to cast individuals in relationships of dominance and subjugation. Their loss of faith in liberal democracy encourages them to propose deliberative democratic forms intended to produce rational decisions reached through reasonable debate and delivering greater equality. This thesis seeks to test these assumptions, subjecting them first to critical scrutiny by contextualising them within the wider academic debates on globalisation, power and democratic theory and in each case finding that their claims to represent the only viable responses to the new politics falling short. The second test is built upon the central role these authors give to new communications teclmologies as tools of propaganda and as exemplars of a new form of social organisation. By comparing output from the traditional and new media this thesis will look for evidence of the three schools predictions of the overwhelming scale and penetration of the new politics in the conduct of a political debate in the context of the 2001 British General Election.

28. Medieval stave-built wooden vessels in Ireland and Russia

Abstract

The overall aim of this thesis is an investigation of the practice of cooperage in the Middle Ages from the evidence of preserved stave-built wooden vessel components in Ireland and Russia. Two large urban assemblages from broadly contemporary contexts in Dublin and Novgorod are used for assessment of functionally and culturally determined attributes of stave-built wooden vessels, in terms of species selection, conversion and morphology. Attributes that are the result of deliberate human actions or were deliberately selected for specific reasons are identified and considered from a methodological perspective. The archaeological implications of preserved vessel components are analysed, with the objective of understanding what can and cannot be learned from partial vessel remains. The major question of what criteria were applied in selection of raw materials in medieval cooperage is central to this thesis and it is concluded that selection was strongly influenced by cultural factors and that deliberate functional compromise in the interest of aesthetics was commonplace. Despite a wide range of suitable wood in both regions, a comparatively narrow range of materials was selected by coopers.

33. Entrepreneurs, educators and the slicing of fish : some Anglo-American parallels in higher education

Abstract

Links between entrepreneurialism and higher education have a long history, although their existence has not always been acknowledged. In consequence higher education in the United Kingdom and in the United States has been subject to market forces, both from clients in external markets and sponsors and financiers in the internal market. From the 1850s onwards, this resulted in the development of an entrepreneurial system of higher education in the United States, where from the 1950s, even small colleges began to seek a national, or even international market, marking a move towards the eventual globalisation of US higher education. In the 1980s a Conservative government introduced an entrepreneurial system into the United Kingdom, but rapidly, by legislation, rather than gradually, by symbiosis. The UK system expanded substantially, with many non-university institutions, previously under the boort-main' control of Local Government, receiving their freedom as higher education corporations. Seeking prestige and security in size, many of these began to operate in an inadequately regulated global market. To meet the changed circumstances, trustees and governors tended to recruit a new type of college manager and in many institutions the entrepreneurial economocracy supplanted the academic oligarchy. Parsons College, Iowa, in the 1960s, Upper Iowa University in the 1970s and Southampton Institute, England, in the late 1980s, were all led down an entrepreneurial path by a dominant individual, with varying results. In each case the burden of history, whether of the institution, its sponsors, or the milieu in which it was to operate, exercised a significant influence on the outcome of the entrepreneurial activity, with failure sometimes appearing to have been inevitable A major theme is the extent to which prior events can influence and even determine the trajectory taken by an institution and its personnel, and the thesis examines the history and circumstances in which the current systems of entrepreneurial higher education developed, in part as a result of the social, economic and cultural benefits it can bring to a locality. By reference to the three institutions, the thesis seeks to identify some Anglo-American parallels, the tensions which can arise, and the factors which determine success or failure within the academy and the educational market-place. It also seeks to gain some insight with regard to the effect entrepreneurialism in the academy has on the student, whose role as a learner may be overshadowed and obscured by the role of consumer. Scope for future research is also indicated.
