Abstract

l cancro di intervallo (CI) è un’entità patologica che viene descritta solo nell’ambito dei programmi di screening organizzato. Si intende per CI un carcinoma successivo ad un processo di screening negativo e comparso prima del passaggio di screening successivo. Qualsiasi sia la motivazione della loro genesi, i CI rappresentano un insuccesso del programma di screening che, in teoria, mira a diagnosticare tutti i carcinomi in fase di screening, e a non consentire la comparsa di CI. L’analisi dei CI (loro frequenza di comparsa nel tempo, caratteristiche morfologiche e cliniche, revisione della precedente mammografia di screening) è un momento fondamentale della valutazione di performance dello screening.Per questo motivo a 10 anni dall’attivazione del programma di Screening a Trieste, si è voluto confrontare i fattori prognostici dei carcinomi screening-detected (CSD) e dei (CI), alla luce dell’ampio dibattito in letteratura riguardo la maggior aggressività di questi ultimi.

Abstract

The role of breastfeeding as protective factor for breast cancer incidence is controversial among scientific literature. The Interdisciplinary Senology Unit of Trieste has analyzed the relation between breast cancer incidence and lactation habits among a total of 4307 Trieste’s women. In this study, the statistical analysis revealed that over having children, the breastfeeding is a further protective factor for breast cancer. In particular, the decrease of breast cancer risk by breastfeeding resulted to be significant for lactation more than 12 months. It is worth noting that breastfeeding resulted protective only for women in childbearing age and it didn’t result to decrease breast cancer risk among post-menopausal women.

Abstract

Friuli Venezia Giulia holds the higest row incidence rate for breast carcinoma in Italy (218%00) and Trieste, where live 125.000 women (105.000 women of age below 75) has the highest incidence rate among the other provinces of the region (220%00). The risk of developing breast carcinoma seems to be correlated with many different factors. The goal of this study is to focus on same specific breast carcinoma risk factors in the Trieste population.

Abstract

In Trieste, pathologists have been actively involved in breast diagnostics working alongside radiologists in the sampling time in order to set up together the most adequate approach in case-solving basing on the clinical/radiological and rapid stain cytological findings. Based on our own experience we prefer, whenever possible, to use the fine needle aspiration sampling method (FNA), almost always under ultrasound guidance, even for palpable lesions, reserving tru-cut biopsy only for a limited number of cases and vacuum-assisted biopsy (VAB) only in not ultrasound detectable lesions (id: isolated microcalcifications). The collaboration between radiologist and pathologist is essential to correctly evaluate identified lesions: the more coincident the results of radiological and cytological exams are, the more accurate the final diagnosis will be. Nevertheless there are some cases in which radiology and cytology do not agree. We decided to reassess the entire database, focusing on the management of discrepant cases in the light of the final histological diagnosis for surgical-treated cases or referring to the follow-up exams in the cases that did not undergo surgery.

Abstract

Target Print (TP) is a microarray-based gene expression test which offers a quantitative assessment of the patient'slevel of estrogen receptor (ER), progesterone receptor (PR) and HER2/neu overexpression within her breast cancer. As compared to Immunohistochemistry (IHC), TP delivers an added benefit to the diagnostic process. IHC provide a semi-quantitative positive or negative result, whereas the gene expression result provided by TP allows to integrate the absolute level of ER, PR and HER2 gene expression into treatment planning. It is known that differences in tissue fixation, choice of antibody and interpretation can severely affect IHC accuracy and reproducibility.

Abstract

Fine needle aspiration cytology (FNAC) is widely used as first choice approach for the definition of the radiologically dubious or sospicious cases to confim their benign origin. Trieste's Breast Unit has been using FNAC as first morphological investigation for many years, in particular in 2008-2009, it was used as first diagnostic approach for 1835 cases (88.6%) out of 2091. The use of five diagnostic categories in cytology reporting has found wide acceptance among radiologists and surgeons, because it allows to apply to each lesiod, a precise diagnostic/terapeutic pathway and it represents, in oir experience, an essential element of the report itself.

Abstract

Breast carcinoma's screening in Trieste's area detected 60 women with cancer (two of them with bilateral cisease) in 2008. Forty-eight cases were diagnosed only by FNA (C5), 12 by VAB/mammotome and two by Fine Needle biopsy after undeterminated cytology. Axillary lymph nodes US was performed in 62,9% (39) of cases. They had alle been diagnosed as malignant (C5) by rapid cytology during breast nodule aspiration. .... Our study outlines cyology's accuracy to define axillary nodes' status suggestin the possibility to extent US followed by aspiration cytology to all screening detected cases with U3/4 axillary nodes.

Abstract

Trieste is one of the italian areas with the highest incidence of breast carcinoma. Mammographical screening has been active since 2006 inolving womenn 50-69 years old. Breast screening introduction has been led to an increase both of Tis lesions and of invasive carcinoma. The oserved growth is due to increase of subclinical lesions (T1a and T1b), while lesions larger than 2 cm maintained steady.

Abstract

Incidence of ASC-H has been reported to be 0.13-0,84% and SIL on follow-up biopsy ranged from 27 to 89%. This diagnosis is used for cases that show cellular abnormalities more marked than reactive changes but that fall short of a definitive diagnosis of HSIL. We used cyto histologic correlation to determe the clinical significance of ASC-H fron 2004 to 2008.Out of 77 709 Pap smears, 182 were ASC-H (0,23%). ....... The diagnosis of ASC-H is associatd with an appreciable risk of lesion, so the patiensts should undergo colposcopic and histologic assessment to toule out SIL. Negative biopsy review outlined doubtful cases were mainly squamous metaplasia.

Abstract

Breast cancer is thecmost frequency malignancy in women in the world and also in Italy.In Trieste most of breast lesions are investigated by fine needle aspiration cytology under ultrasound guidance (FNAC). Tru-cut is used when FNAC is inadequate, for discrepancy with radiology or to have a biomolecular profiling,expecially in neoadjuvant chemotherapy. Vacuum Aspiration Biopsy (VAB) is used in case of microcalcifications or mammographic abnormalitues without evidence of nodules.FNAC's goal is to define breast lesions avoiding more invasive procedures,, it provides several benefits for women, for pathologists and for the public health

Abstract

Detecting breast tissue alterations is essential for cancer diagnosis. However, inherent bidimensionality limits histological procedures' effectiveness in identifying these changes. Our study applies a 3D virtual histology method based on X-ray phase-contrast microtomography (PhC μ CT), performed at a synchrotron facility, to investigate breast tissue samples including different types of lesions, namely intraductal papilloma, micropapillary intracystic carcinoma, and invasive lobular carcinoma. One-to-one comparisons of X-ray and histological images explore the clinical potential of 3D X-ray virtual histology. Results show that PhC μ CT technique provides high spatial resolution and soft tissue sensitivity, while being non-destructive, not requiring a dedicated sample processing and being compatible with conventional histology. PhC μ CT can enhance the visualization of morphological characteristics such as stromal tissue, fibrovascular core, terminal duct lobular unit, stromal/epithelium interface, basement membrane, and adipocytes. Despite not reaching the (sub) cellular level, the three-dimensionality of PhC μ CT images allows to depict in-depth alterations of the breast tissues, potentially revealing pathologically relevant details missed by a single histological section. Compared to serial sectioning, PhC μ CT allows the virtual investigation of the sample volume along any orientation, possibly guiding the pathologist in the choice of the most suitable cutting plane. Overall, PhC μ CT virtual histology holds great promise as a tool adding to conventional histology for improving efficiency, accessibility, and diagnostic accuracy of pathological evaluation., (© 2024. 39. How Porcine Acellular Dermal Matrix Influences the Development of the Breast Capsule 1 Year after Implantation: A Histopathological Analysis.

Abstract

Background: In prepectoral breast reconstruction (PPBR) the acellular dermal matrix (ADM)'s integration capacity into the tissue is known. The aim of this study was to analyze the effect of the ADM on development and composition of the peri-implant breast capsule in a dynamic setting of breast tissue expansion during two-stage prepectoral breast reconstruction., Methods: This is a prospective single-center study in which 50 patients who underwent mastectomy and breast reconstruction with prepectoral tissue expander and Braxon ADM (group A) and submuscular tissue expander (group B) were enrolled. One-year post implantation hematoxylin & eosin (H&E) staining and immunohistochemistry analyses were done on capsule tissue samples., Results: The analysis conducted on H&E-stained samples showed a significant reduction of cellular density and a decrease of the cellular infiltration in capsules of ADM-covered expanders compared with naked expander capsules ( P < 0.05). The immunohistochemical analyses showed that group A capsules presented significantly less M1 CD68+ macrophages ( P < 0.05), lower alfa-SMA expression levels, and a lower number of myofibroblasts ( P < 0.05) compared with group B capsules. Presence of lymphatic vessels was minimally detected in both groups., Conclusions: The ADM presence around the prepectoral tissue expander influences the development of the peri-implant capsule, causing a significant reduction of the number of cells and inflammatory infiltrate, especially M1 macrophages and myofibroblasts. Abstract

Objectives: To evaluate burden and predictors of HSV pneumonia among immunocompromised patients not undergoing invasive mechanical ventilation according to a tailored diagnostic algorithm., Methods: This prospective, observational study included immunocompromised adults with pneumonia non-responding to empirical antibiotic therapy. Bronchoalveolar lavage (BAL) specimens were cultured for bacteria, mycobacteria and fungi. Real-time PCR for Herpesviruses and other microorganisms were performed on BAL and other specimens. Cytological examination of BAL samples was carried out for identification of intranuclear inclusion bodies and immunohistochemical staining for HSV., Results: We enrolled 45 patients (mean age 64.6 years) from January 2015 to June 2016. Nineteen (42.2%) cases tested positive for HSV-1 PCR on BAL. According to our definitions, 11 (24.4%) patients had HSV-1 pneumonia with viral loads ranging between 10 3 copies/mL and 10 7 copies/mL. HSV-1 positive throat swab (OR 85.2, 95% CI 5.83-1245.1, P < 0.001) and solid organ transplant (SOT) (OR 53.3, 95% CI 1.37-2072.8, P < 0.03) as underlying condition were found to be independently associated with HSV pneumonia by multivariable analysis., Conclusions: HSV pneumonia turned out to be relatively common and should be investigated especially in individuals with HSV positive throat swab and SOT. Interventional studies are needed to assess the real clinical impact of HSV pneumonia in immunocompromised patients., (Copyright © 2018. Published by Elsevier Ltd.)
