1. A high sensitivity Cherenkov detector for Prompt Gamma Timing and Time Imaging

Abstract

We recently proposed a new approach for the real-time monitoring of particle therapy treatments with the goal of achieving high sensitivities on the particle range measurement already at limited counting statistics. This method extends the Prompt Gamma (PG) timing technique to obtain the PG vertex distribution from the exclusive measurement of particle Time-Of-Flight (TOF). It was previously shown, through Monte Carlo simulation, that an original data reconstruction algorithm (Prompt Gamma Time Imaging) allows to combine the response of multiple detectors placed around the target. In this work we focus on the experimental feasibility of PGTI in Single Proton Regime (SPR) through the development of a multi-channel, Cherenkov-based PG detector with a targeted time resolution of 235 ps (FWHM): the TOF Imaging ARrAy (TIARA). The PG module that we developed is composed of a small PbF$_{2}$ crystal coupled to a silicon photoMultiplier to provide the time stamp of the PG. This prototype was tested with 63 MeV protons delivered from a cyclotron: a time resolution of 276 ps (FWHM) was obtained, resulting in a proton range sensitivity of 4 mm at 2$\sigma$ with the acquisition of only 600 PGs. A second prototype was also evaluated with 148 MeV protons delivered from a synchro-cyclotron obtaining a time resolution below 167 ps (FWHM) for the gamma detector. Moreover, using two identical PG modules, it was shown that a uniform sensitivity on the PG profiles would be achievable by combining the response of gamma detectors uniformly distributed around the target. This work provides the experimental proof-of-concept for the development of a high sensitivity detector that can be used to monitor particle therapy treatments and potentially act in real-time if the irradiation does not comply to treatment plan.

Abstract

In the context of online ion range verification in particle therapy, the CLaRyS collaboration is developing Prompt-Gamma (PG) detection systems. The originality in the CLaRyS approach is to use a beam-tagging hodoscope in coincidence with the gamma detectors to provide both temporal and spatial information of the incoming ions. The ion range sensitivity of such PG detection systems could be improved by detecting single ions with a 100 ps ($\sigma$) time resolution, through a quality assurance procedure at low beam intensity at the beginning of the treatment session. This work presents the investigations led to assess the performance of Chemical Vapor Deposition (CVD) diamond detectors to fulfill these requirements. A $^{90}$Sr beta source, 68 MeV protons, 95 MeV/u carbon ions and a synchrotron X-ray pulsed beam were used to measure the time resolution, single ion detection efficiency and proton counting capability of various CVD diamond samples. An offline technique, based on double-sided readout with fast current preamplifiers and used to improve the signal-to-noise ratio, is also presented. The different tests highlighted Time-Of-Flight resolutions ranging from 13 ps ($\sigma$) to 250 ps ($\sigma$), depending on the experimental conditions. The single 68 MeV proton detection efficiency of various large area polycrystalline (pCVD) samples was measured to be $>$96% using coincidence measurements with a single-crystal reference detector. Single-crystal CVD (sCVD) diamond proved to be able to count a discrete number of simultaneous protons while it was not achievable with a polycrystalline sample. Considering the results of the present study, two diamond hodoscope demonstrators are under development: one based on sCVD, and one of larger size based on pCVD. 4. A Time-Of-Flight-Based Reconstruction for Real-Time Prompt-Gamma Imaging in Protontherapy

Abstract

We are currently conceiving, through (MC) simulation, a multi-channel gamma detector array (TIARA for Time-of-flight Imaging ARrAy) for the online monitoring of protontherapy treatments. By measuring the Time-Of-Flight (TOF) between a beam monitor placed upstream and the Prompt-Gamma (PG) detector, our goal is to reconstruct the PG vertex distribution to detect a possible deviation of proton beam delivery. In this paper, two non-iterative reconstruction strategies are proposed. The first is based on the resolution of an analytical formula describing the PG vertex distribution in 3D. Here, it was resolved under a one-dimensional approximation in order to measure a potential proton range shift along the beam direction. The second is based on the calculation of the Centre-Of-Gravity (COG) of the TIARA pixel detectors counts and also provides 3D information on a possible beam displacement. The PG vertex reconstruction was evaluated in two different scenarios. A coincidence time resolution of 100 ps (rms) can be attained in single proton regime (operating a reduction of the beam current) and using an external beam monitor to provide a start trigger for the TOF measurement. Under these conditions, MC simulations have shown that a millimetric proton range shift sensitivity can be achieved at 2$\sigma$ with 10$^{8}$ incident protons. This level of accuracy would allow to act in real-time if the treatment does not conform to treatment plan. A worst case scenario of a 1 ns (rms) TOF resolution was also considered to demonstrate that a degraded timing information can be compensated by increasing the acquisition statistics: in this case, a 2 mm range shift would be detectable at 2$\sigma$ with 10$^{9}$ incident protons. The COG method has shown excellent capabilities of detecting lateral beam displacements: a 2 mm sensitivity was found at 2$\sigma$ with 10$^{8}$ incident protons.

Abstract

The accuracy of hadrontherapy treatment is currently limited by ion-range uncertainties. In order to fully exploit the potential of this technique, we propose the development of a novel system for online control of particle therapy, based on TOF-resolved (time-of-flight) Prompt Gamma (PG) imaging with 100 ps time resolution. Our aim is to detect a possible deviation of the proton range with respect to treatment planning within the first few irradiation spots at the beginning of the session. The system consists of a diamond-based beam hodoscope for single proton tagging, operated in time coincidence with one or more gamma detectors placed downstream of the patient. The TOF between the proton time of arrival in the hodoscope and the PG detection time provides an indirect measurement of the proton range in the patient with a precision strictly related to the system time resolution. With a single ~38 cm$^{3}$ BaF2 detector placed at 15 cm from a heterogeneous PMMA target, we obtained a coincidence time resolution of 101 ps (rms). This system allowed us to measure the thickness and position of an air cavity within a PMMA target, and the associated proton range shift: a 3 mm shift can be detected at 2$\sigma$ confidence level within a single large irradiation spot (~10$^{8}$ protons). We are currently conceiving a multi-channel PG timing detector with 3D target coverage. Each pixel will provide the PG detection time and its hit position, that can be used to reconstruct the 3D distribution of PG vertices in the patient. Our approach does not require collimation and allows to dramatically increase the detection efficiency. Since both signal detection and background rejection are based on TOF, the constraints on energy resolution can be relaxed to further improve time resolution., Comment: 4 pages, 6 figures

National audience; Au cours d’une séance d’irradiation en hadronthérapie, une partie des ions incidents va subir des réactions de fragmentations nucléaires qui ont pour effet de délocaliser le dépôt de dose dans le patient. C’est donc une source d’incertitude qui peut nuire à une délivrance optimale des traitements.La collaboration Clarys (IP2I CPPM LPSC CREATIS) met au point un système de contrôle en ligne basé sur la détection de gamma prompts émis le long du parcours des ions. Afin de réaliser une mesure de temps de vol absolue (ion + gamma prompt), ClaRys inclut dans son projet le développement d’un hodoscope de marquage de faisceau installé en amont du patient. Il est destiné à fournir une information temporelle et spatiale des ions entrants. Ces informations peuvent être utilisées pour le processus de reconstruction d'image et dans la réduction des bruits de fond.Un premier hodoscope basé sur un maillage de fibres scintillantes lues par des photo-multiplicateurs (PM) a été développé. En raison des limitations du taux de comptage des PM, l'efficacité de détection de l'hodoscope à fibres diminue pour les courants de faisceau élevés. Ces limitations ont conduit au développement d'un hodoscope à base de diamant synthétiques (croissance par dépôt chimique en phase vapeur ou CVD).Les qualités intrinsèques du diamant (rapidité, faible courant de fuite, excellent rapport signal sur bruit, résistance aux radiations, équivalence tissu humain) font de ce semi-conducteur un parfait candidat pour répondre aux exigences de monitorage. L'hodoscope diamant devrait permettre d'atteindre des résolutions temporelles de 100 ps qui vont au-delà des performances de l’hodoscope à fibres et permettrait de faire de l’Ultra Fast Timing cela constitue le projet ClaRys-UFT.Par ailleurs, la radiothérapie innovante «flash» qui permet de délivrer des faisceaux pendant un temps ultracourt (quelques millièmes de secondes au lieu de plusieurs minutes pour la radiothérapie conventionnelle) requiert un monitorage spécifique de faisceaux en mode pulsé. La rapidité des détecteurs diamants constitue là un atout essentiel pour un tel développement avec une capacité de marquage en temps « début » et « fin » des paquets des trains d’impulsions ainsi que de comptage des particules dans le train à haute intensité faisceau. Cela a conduit au développement du moniteur faisceau diamant DIAMMONI dans le cadre du projet R&T IN2P3 DIAMTECH (collaboration LPSC SUBATECH ARRONAX).

Abstract

This article shows how mortality and morbidity patterns differ for women and men 45 years of age and older. The impact on disability-free life expectancy was calculated for selected risk factors and chronic conditions: low income, low education, abnormal body mass index, lack of physical activity, smoking, cancer, diabetes, and arthritis. For each factor, the expected number of years free of disability was calculated for men and women using multi-state life tables. In terms of disability-free life expectancy, the greatest impacts on affected women were for diabetes (14.1 years), arthritis (8.8 years), smoking (8.0 years) and physical inactivity (6.0 years), while for affected men, the greatest impacts were for diabetes (10.5 years), smoking (6.9 years), arthritis (6.5 years), and cancer (6.4 years). The implications of these results are discussed from the perspective of developing programs designed to improve population health status. KEYWORDS. International audience; A significant proportion of cancer patients benefit from radiotherapy . Besides conventional x-ray radiation, synchrotron has proven to offer significant advantages in radiotherapy by using high dose rate coherent x-rays beams. Indeed, High coherence allowing to produce micrometric fields to explore limits of a concept called dose-volume effect. The other important characteristics of synchrotron radiation (high dose rate) permits to take advantage of the so-called flash effect.The first phase I/II clinical study of synchrotron radiotherapy at the European Synchrotron Radiation Facility (ESRF) demonstrated the feasibility and safety of this technique. this method requires some development. One of them, in-vivo dosimetry (the real time dose delivered during the treatment), is particularly challenging, because of the high dose rate and low energy flux.A new approach based on pixelised diamond detectors ,already validated for one point dosimetry in synchrotron radiation[1], will be developed.Before the full conception of one dimension dosimeter, first step is to characterize diamond detectors responses in synchrotron radiation (incident photons energy between 30 and 150 keV and an high dose rate which can reach 10 000 Gy/s) and show the project’s viability.For this reason, some preliminary tests was performed on different diamond detectors (two mono-crystalline and one polycrystalline), to show their response for different energy and dose rate, already developed at LPSC (Laboratoire de Physique Subatomique et Cosmologie) and this presentation will focus on these tests.[1]: Livingstone J. et al. Abstract

Background: Life expectancy (LE) and health expectancy have increased throughout much of the world. However, these gains have not been shared equally across all population groups. Socioeconomic disparities exist, though varied methodologies and data sources have made it difficult to ascertain changes over time in Canada. Methods: The 1996 and 2011 Canadian Census Health and Environment Cohorts, with a five-year mortality follow-up, were used to estimate the LE of the household population at ages 25 and 65, according to individual-level education and income. Health status was measured by the Health Utilities Index Mark 3 instrument in two national population health surveys and was used to adjust LE to estimate health-adjusted life expectancy (HALE). Disparities in LE and HALE, and differences between cohorts, were examined. Results: LE, HALE and the ratio of HALE to LE were greater at higher levels of education or income. A stepwise gradient was also observed by level of education within and across income quintiles, with people in the lowest combined education and income categories at the greatest disadvantage. Disparities were wider in the 2011 cohort compared with the 1996 cohort, but not necessarily to the same extent for both sexes or at different ages. Interpretation: In Canada, education-related and income-related disparities in life and health expectancy persist and may be wider than they were in the past. This underscores the importance of ongoing data development for routine monitoring of trends in mortality and morbidity, which can, in turn, inform policy development and planning to advance health equity. Abstract

Background: Over the past century, life expectancy at birth in Canada has risen substantially. However, these gains in the quantity of life say little about gains in the quality of life. Methods: Health-adjusted life expectancy (HALE), an indicator of quality of life, was estimated for the household and institutional populations combined every four years from 1994/1995 to 2015. Health status was measured by the Health Utilities Index Mark 3 instrument in two national population health surveys, and was used to adjust life expectancy. The percentage of the population living in health-related institutions was estimated based on the Census of Population. Attribute-deleted HALE was calculated to determine how various aspects of health status contributed to the differences between life expectancy and HALE. Results: HALE has increased in Canada. Greater gains among males have narrowed the gap between males and females. The ratio of HALE to life expectancy changed little for males, and a marginal improvement was observed for females aged 65 or older. Mobility problems and pain, the latter mainly among females, accounted for an increased share of the burden of ill health over time. Exclusion of the institutional population significantly increased the estimates of HALE and yielded higher ratios of HALE to life expectancy. Interpretation: Although people are living longer, the share of years spent in good functional health has remained fairly constant. Abstract

The International Large Detector (ILD) is a concept for a detector at the International Linear Collider, ILC. The ILC will collide electrons and positrons at energies of initially 500 GeV, upgradeable to 1 TeV. The ILC has an ambitious physics program, which will extend and complement that of the Large Hadron Collider (LHC). A hallmark of physics at the ILC is precision. The clean initial state and the comparatively benign environment of a lepton collider are ideally suited to high precision measurements. To take full advantage of the physics potential of ILC places great demands on the detector performance. The design of ILD is driven by these requirements. Excellent calorimetry and tracking are combined to obtain the best possible overall event reconstruction, including the capability to reconstruct individual particles within jets for particle ow calorimetry. This requires excellent spatial resolution for all detector systems. A highly granular calorimeter system is combined with a central tracker which stresses redundancy and efficiency. In addition, efficient reconstruction of secondary vertices and excellent momentum resolution for charged particles are essential for an ILC detector. The interaction region of the ILC is designed to host two detectors, which can be moved into the beam position with a push-pull scheme. The mechanical design of ILD and the overall integration of subdetectors takes these operational condition s into account. Abstract

Statistique Canada a publié des projections pour le Canada, les provinces et les territoires à six reprises depuis 1974, les dernières en date étant parues en 2001. Ces projections, diffusées à intervalles réguliers, sont conçues de manière à assurer la cohérence et la comparabilité des méthodes et des résultats tant à l’échelle provinciale et territoriale que nationale. Ce rapport contient différents scénarios de projection de la population jusqu’à l’horizon 2031. Il décrit aussi la méthodologie et les hypothèses et comprend une brève analyse des résultats. Les projections contenues dans ce rapport partent des estimations de population au 1 er juillet 2005. Elles tiennent compte des tendances récentes relatives aux composantes de l’accroissement démographique. Il importe de souligner que les projections ne sont pas des prédictions. Elles représentent plutôt un effort en vue d’établir des scénarios plausibles à partir d’hypothèses sur les composantes de l’accroissement démographique, elles-mêmes sujettes à l’incertitude. Elles fournissent des informations statistiques utiles à la planification, à la préparation de politiques et peuvent aussi servir au public qui s’intéresse aux changements futurs dans la population et aux questions qui s’y rapportent.

Abstract

According to results from the Canadian Community Health Survey?Healthy Aging, 76% of Canadians in mid-life (45 to 64) and 56% of seniors reported good health in 2009. This is based on a definition of health composed of: positive self-perceived general and mental health, functional ability, and independence in activities of daily living. Good health existed even in the presence of chronic conditions such as high blood pressure, arthritis and back problems, all of which were common among people aged 45 or older. Eight modifiable factors were associated with good health: smoking status, body mass index, physical activity, diet, sleep, oral health, stress, and social participation. Eighty-four percent of the younger age group and 91% of seniors reported positive tendencies on four or more of these factors. The more factors on which positive tendencies were reported, the greater the likelihood of having good health. [ABSTRACT FROM AUTHOR]
