Introduction: Ambulatory treatment of breast cancer is limited by vomiting and post-operative pain, and general anesthesia is implicatedin these effects. Dexmedetomidine supplies analgesia and sedation without respiratory depression when it is administeredintravenously. We demonstrate the usefulness of Dexmedetomidine as a general endovenous anesthesia. Methods: Female patients were included with ages ranging between 42 and 83 years old and with a breast cancer diagnosis. They were scheduled for radical surgery between May 2009 and April 2011. Dexmedetomidine plus Propofol and Fentanyl were administered. Intra-operative monitoring included electrocardiography, arterial blood gas analysis, blood pressure, heart rate, respiratory rate, and arterial oxygen saturation. We registered surgery and sedation according to the bispectral index and Ramsay scales. Results: The patients were maintained with 95% oxygen saturation and breathed spontaneously during the entire procedure with supplementary oxygen.Their blood pressure remained constant; heart rate showed only a mild reduction, bispectral index was maintainedat 50, and the Ramsey scale-at IV. The infusion was discontinued without the patients experiencing dizziness or vomiting; the patientshad an easy response-to-voice awakening and remained in the recovery room for at least 2 h. They were physiologically stable, andwere left with additional supplementary oxygen. There was no pain or agitation observed during the recovery period and no narcotic was administered. Conclusion: Dexmedetomidine is a useful intravenous anesthetic agent in procedures such as radical breast surgery, with faster recovery and residual analgesia.

Mitochondria from patients with Kearns–Sayre syndrome harboring large-scale rearrangements of human mitochondrial DNA (mtDNA; both partial deletions and a partial duplication) were introduced into human cells lacking endogenous mtDNA. Cytoplasmic hybrids containing 100% wild-type mtDNA, 100% mtDNA with partial duplications, and 100% mtDNA with partial deletions were isolated and characterized. The cell lines with 100% deleted mtDNAs exhibited a complete impairment of respiratory chain function and oxidative phosphorylation. In contrast, there were no detectable respiratory chain or protein synthesis defects in the cell lines with 100% duplicated mtDNAs. Unexpectedly, the mass of mtDNA was identical in all cell lines, despite the fact that different lines contained mtDNAs of vastly different sizes and with different numbers of replication origins, suggesting that mtDNA copy number may be regulated by tightly controlled mitochondrial dNTP pools. In addition, quantitation of mtDNA-encoded RNAs and polypeptides in these lines provided evidence that mtDNA gene copy number affects gene expression, which, in turn, is regulated at both the post-transcriptional and translational levels.

EFGT (Externally Fired Gas Turbine) are convenient to use solid biomass on site with few or no pretreatment and spare life of turbine working only with clean air. It is therefore a promising technology for small­scale distributed CHP, up to about one electrical megawatt. Moreover, expected electrical efficiency, strongly dependent on turbine inlet temperature, may reach values over 0,20-0,25, which is a quite high value compared to other biomass-based CHP technologies in this range of power. However EFGT remain at an early stage of development. Indeed, the high temperature heat exchanger must increase the temperature of air up to more than 1000 °C to remain electrically efficient. Reaching such temperature represents therefore a great challenge and requires advances on material. Damos a conocer el caso de un obrero de 30 años que accidentalmente fue transfixiado por un chuzo, con entrada a nivel inguinal izquierdo y salida por hemitórax ipsilateral, lo que ocasionó múltiples lesiones intestinales, lesión diafragmática, lesión pulmonar y fracturas expuestas costales y escapular izquierda. Su evolución fue exitosa debido a un adecuado manejo pre e intrahospitalario. Impalements, especially those involving more than one anatomical region, are uncommon. We report a 30 years old male that was injured by a steel jimmy bar that entered by the left inguinal region and exited by the posterior part of the left hemithorax, causing multiple intestinal lesions, a lung and diaphragmatic lesion, costal and scapular fractures. He was operated, extracting the jimmy bar and correcting the multiple lesions. He was discharged ten days later.
