The early detection of congenital anomaly epidemics occurs when comparing current with previous frequencies in the same population. The success of epidemiologic surveillance depends on numerous factors, including the accuracy of the rates available in the base period, wide population coverage, and short periodicity of analysis. This study aims to describe the Latin American network of congenital malformation surveillance: ReLAMC, created to increase epidemiologic surveillance in Latin America. We describe the main steps, tasks, strategies used, and preliminary results. From 2017 to 2019, five national registries (Argentina [RENAC], Brazil [SINASC/SIM-BRS], Chile [RENACH], Costa Rica [CREC], Paraguay [RENADECOPY-PNPDC]), six regional registries (Bogotá [PVSDC-Bogota], Cali [PVSDC-Cali], Maule [RRMC SSM], Nicaragua [SVDC], Nuevo-León [ReDeCon HU], São Paulo [SINASC/SIM-MSP]) and the ECLAMC hospital network sent data to ReLAMC on a total population of 9,152,674 births, with a total of 101,749 malformed newborns (1.1%; 95% CI 1.10-1.12). Of the 9,000,651 births in countries covering both live and stillbirths, 88,881 were stillborn (0.99%; 95% CI 0.98-0.99), and among stillborns, 6,755 were malformed (7.61%; 95% CI 7.44-7.79). The microcephaly rate was 2.45 per 10,000 births (95% CI 2.35-2.55), hydrocephaly 3.03 (2.92-3.14), spina bifida 2.89 (2.78-3.00), congenital heart defects 15.53 (15.27-15.79), cleft lip 2.02 (1.93-2.11), cleft palate and lip 2.77 (2.66-2.88), talipes 2.56 (2.46-2.67), conjoined twins 0.16 (0.14-0.19), and Down syndrome 5.33 (5.18-5.48). Each congenital anomaly showed heterogeneity in prevalence rates among registries. The harmonization of data in relation to operational differences between registries is the next step in developing the common ReLAMC database.

Medical genetics has historically focused on the management of rare disorders, mostly due to variants in single genes or chromosomes. Advances in genomics, however, are leading to the capability of including genomic approaches more broadly into the prevention, diagnosis, and treatment of both rare and common disorders. There is a well-established genetics workforce in many parts of the world, particularly in higher-income countries, though this workforce is neither large enough nor widely enough distributed to fuel the integration of genomics across all of medicine. This chapter will review the status of genetics training in various parts of the world and the opportunities and needs for expansion of training into genomic medicine.

In Paraguay, neonatal screening for congenital hypothyroidism (CH) and phenylketonuria (PKU) started in October 1999, in 2005 cystic fibrosis (CF) was selectively incorporated. The National Program for Neonatal Screening has a centralized laboratory that encompasses 1.132 Sample Collecting Sites (SCS) distributed in the 18 Health Regions, with over 80% coverage of live births; the incidence of CH being 1:2.060, HPA/PKU 1:6.328 and CF 1:5.671 newborns. The newborn screening program headed by the Ministry of Public Health and Social Welfare in Paraguay has been consolidated itself as a public health program. This publication describes the historic 20-year process, the strategies and activities carried out as well as the results and achievements, among which it is important to point out the achievement of newborns screening laws that make mandatory to detect, diagnose and treat those affected, as well as the human resources committed to newborn screening.

RESUMEN Introducción: La fibrosis quística (FQ) es una patología autosómica recesiva, pasible de ser detectada en el periodo neonatal, causada por mutaciones en el gen regulador de conductancia transmembrana de FQ (CFTR, por sus siglas en inglés), la patología es multisistémica y el tratamiento precoz incide en la expectativa y calidad de vida de los afectados. Objetivo: Presentar las principales estrategias y los resultados logrados para la detección, diagnóstico y tratamiento de las personas con FQ, en el Programa Nacional de Detección Neonatal. Metodología: Se realizó una revisión del plan operativo anual, informes, publicaciones y documentos emitidos del 2004 al 2018. Resultado: En 15 años se ejecutaron 16 estrategias que permitieron la detección, el diagnóstico así como el tratamiento de individuos afectados por la FQ. Entre las que se destacan el desarrollo de un proyecto piloto para el tamizaje de la FQ en RN, mediante dosaje de la tripsina inmunoreactiva (TIR), entrega de una canasta básica de medicamentos, insumos y equipos para el tratamiento de los individuos con FQ detectados por el programa así como los diagnosticados con anterioridad, implementación del test del sudor, elaboración de una Guía Clínica multidisciplinaria, automatización del dosaje de la TIR, que permitió la universalización del screening, y por último la identificación de las mutaciones presentes en los individuos afectados. En el 2015 se logró llegar al 100 % de cobertura de los RN atendidos en los servicios del Ministerio de Salud Pública y Bienestar Social. De enero del 2004 a diciembre del 2018, 179 individuos reciben su tratamiento en el programa, 144 de los cuales fueron detectados en el periodo neonatal. Conclusión: Las estrategias ejecutadas han tenido resultados muy auspiciosos que nos permitieron universalizar la detección neonatal y tratamiento, lo que conlleva a una mejor calidad y expectativa de vida de los individuos afectados por la FQ. En la actualidad solo faltaría implementar un Centro de Atención Multidisciplinario para las personas con Fibrosis Quística ABSTRACT Introduction: Cystic fibrosis (CF) is an autosomal recessive pathology, capable of being detected in the neonatal period, caused by mutations in the CF transmembrane conductance regulator gene (CFTR), the pathology is multisystemic and Early treatment affects the expectation and quality of life of those affected. Objective: Present the main strategies and the results achieved for the detection, diagnosis and treatment of people with CF, in the National Neonatal Screening Program. Methodology: A review of the annual operating plan, reports, publications and documents issued from 2004 to 2018 was made. Result: In 16 years, 16 strategies were implemented that allowed the detection, diagnosis and treatment of individuals affected by CF. Among those that stand out the development of a pilot project for the screening of CF in RN, by means of immunoreactive trypsin (TIR), delivery of a basic basket of medicines, supplies and equipment for the treatment of the individuals with CF detected by the program as well as those previously diagnosed, implementation of the sweat test, elaboration of a multidisciplinary Clinical Guide, automation of the TIR dose, which allowed the universalization of the screening, and finally the identification of the mutations present in the affected individuals . In 2015, it was possible to reach 100% coverage of the RNs assisted in the services of the Ministry of Public Health and Social Welfare. From January 2004 to December 2018, 179 individuals received their treatment in the program, 144 of which were detected in the neonatal period. Conclusion: The strategies implemented have had very auspicious results that allowed us to universalize neonatal detection and treatment, which leads to a better quality and life expectancy of the individuals affected by CF. At present, it would only be necessary to implement a Multidisciplinary Care Center for people with Cystic Fibrosis.

Las translocaciones reciprocas balanceadas ocurren con una incidencia de 1 por cada 500 nacimientos. Un porcentaje de ellas es diagnosticado en adultos que consultan por problemas reproductivos, otras descubiertas de manera fortuita y muchas pasan desapercibidas porque los portadores no tienen sintomatología, ni problemas al concebir y tienen descendencia normal. Solo un 5%, cursa con anomalías fenotípicas, con o sin retardo mental, atribuible a una inactivación o pérdida de genes. Se reporta el caso de una niña de 3 años y 6 meses de edad, portadora de una translocación balanceada reciproca entre los cromosomas 2 y 11, que consulta al servicio de Genética por retardo del desarrollo sicomotor y convulsiones. Al examen físico la paciente presentaba: CC: 46,5cm. (-2DS), talla de 87 cm. percentil 3, clinodactilia de ambos quintos dedos, hipertricosis y un soplo sistólico debido a una insuficiencia tricúspides y una válvula aórtica bicúspide. La niña es el producto del tercer embarazo de un matrimonio joven, no consanguíneo, tiene dos hermanos varones, el mayor de 8 años, con la misma cardiopatía y en el otro hermano de 6 años se ausculta un soplo sistólico, de etiología a confirmar. El estudio cromosómico de la propósita, ambos padres y los dos hermanos, se llevó a cabo a través de las técnicas de cultivo de sangre periférica. Las láminas se analizaron con coloración convencional y Bandas G y C. Se examinaron treinta células por individuo. En la paciente se encontró una translocación reciproca balanceada, no así en los padres y hermanos, por lo cual se la considero de novo. Cariotipo: 46, XX,rcp (2;11)(p11;p11). Se reporta este caso por ser de interés ya que las características clínicas de la niña son atribuibles a la anomalía cromosómica pero no así la cardiopatía y también porque el número de portadores de translocaciones reciprocas balanceadas afectados es bajo.

La evidencia citogenética de una trisomía del cromosoma 9, ya sea de todo o parte del brazo corto, con o sin trisomía para el brazo largo, produce retardo mental y características físico clínicas constantes. Se presenta a una niña de 2 años y seis meses de edad, derivada al consultorio de genética por talla baja, hipoacusia, ausencia del lenguaje. Primera hija, de padres no consanguíneos, padre de 31 años de edad y madre de 29 años con prenatal controlado. Al examen físico presenta una circunferencia cefálica de 46 cm. (-2 DS), talla de 85 cm., peso de 10 Kg., con puente nasal alto, orejas prominentes de implantación baja, dedos de manos y pies cortos, con hipoplasia ungueal. El estudio cromosómico en linfocitos de sangre periférica estimulados con fitohemaglutinina mostró una trisomía parcial del cromosoma 9. Cariotipo 47,XX,+ (9) ( p11, 1; p24,3). Los hallazgos fenotípicos observados en la niña, son comparados con las de otros pacientes con otras alteraciones citogenéticas del mismo cromosoma, corroborándose la existencia de una región critica para el fenotipo descrito para el síndrome 9p.

RESUMEN Introducción: El estado nutricional adecuado resulta fundamental en los pacientes con Fibrosis Quística (FQ) debido a que incide sobre la función pulmonar y la mortalidad de los mismos. Objetivo: Caracterizar el estado nutricional de los niños y los adolescentes con FQ que acuden al Programa Nacional de Detección Neonatal, sub Programa Fibrosis Quística, dependiente del Ministerio de Salud Pública y Bienestar Social (MSPyBS), de febrero a noviembre del 2016. Material y método: Estudio observacional, transversal, descriptivo con componente analítico, realizado a partir de la revisión de fichas clínicas de los pacientes con diagnóstico confirmado de FQ. Variables estudiadas: sociodemográficas (sexo, edad al diagnóstico, edad actual y procedencia), formas clínicas al diagnóstico y presencia de suficiencia pancreática, variables antropométricas y estado nutricional, función pulmonar (Volumen Espiratorio Forzado al primer segundo: VEF1). Los defectos del tubo neural (DTN) incluyen un amplio espectro de malformaciones congenitas. La mas frecuente es la espina bifida abierta que cursa con grados variables de paralisis de las extremidades inferiores y alteraciones vesicales e intestinales. Actualmente se pueden reducir los DTN mediante el acido folico (AF). El objetivo principal de este estudio fue determinar el conocimiento periconcepcional de AF. Se realizo un cuestionario a 800 mujeres puerperas, internadas en la Catedra de Ginecologia y Obstetricia - Facultad de Ciencias Medicas, Cruz Roja Paraguaya y Hospital San Pablo (ambos MSP y BS), desde el 1 de febrero al 31 de noviembre de 2003. La mayoria tenia entre 20 y 24 anos (29,8%); cursaron el bachillerato 54,3%. El embarazo fue planificado en 21,8%. A la pregunta ―ha oido hablar del AF‖ 66 (8,2%) de las 800 puerperas respondieron afirmativamente. De estas, 50 (75,6% de este subgrupo) sabian que es una vitamina, 39 (59,1%) que el AF es util para prevenir los DTN y 26 (39,4%) que se debe consumir desde antes de comenzar el embarazo. Este estudio mostro bajo nivel de conocimientos de las puerperas sobre lo que es y los beneficios del AF en la prevencion primaria de los DTN, lo que evidencia la importancia de insistir en programas de informacion y educacion sanitaria, a toda mujer en edad fertil aunque no planee su embarazo, sobre esta medida de prevencion primaria de los DTN.

The tumor suppressor gene TP53 is the most frequently mutated gene in human cancer, and the germline TP53 R337H mutation is the most common mutation reported to date. However, this mutation is associated with a lower cumulative lifetime cancer risk than other mutations in the p53 DNA-binding domain. A detailed statistical analysis of 171,500 DNA tests in Brazilian neonates found that 0.27% of the general population is positive for this mutation, and some of the estimated 200,000 Brazilian R337H carriers in southern and southeastern Brazil have already developed cancer. The present study was designed to estimate R337H prevalence in neighboring Paraguay. To address this question, 10,000 dried blood samples stored in Guthrie cards since 2008 were randomly selected from the Paraguayan municipalities located at the border with Brazil. These samples were tested for R337H mutation using the PCR-restriction fragment length polymorphism assay. This germline mutation was detected in five samples (5/10,000), indicating that the total number of R337H carriers in Paraguay may be as high as 3500. Previous studies have shown that other countries (i.e., Portugal, Spain, and Germany) presented one family with this mutation, leading us to conclude that, besides Brazil and Paraguay, other countries may have multiple families carrying this mutation, which is an inherited syndrome that is difficult to control.

La Fibrosis quística es la enfermedad autosómica recesiva más común en la población blanca y se caracteriza por la obstrucción de conductos, principalmente en pulmón, páncreas y tracto genital. Se presenta en uno de cada 2000 a 2500 nacidos vivos y tiene una frecuencia de portadores de uno cada 20 a 25 nacidos vivos. La enfermedad es causada por diferentes mutaciones en el gen regulador de la conductancia transmembrana de la fibrosis quística (CFTR). La mutación más frecuente en el gen CFTR es la deleción de tres pares de bases (CTT) denominada F508. Este trabajo se realizó con el fin de estandarizar la técnica de mutagénesis dirigida mediante PCR (PSM) para la detección de.la mutación F508 en pacientes con fibrosis quística. El método utilizado fue validado mediante secuenciación del DNA del exón 10 en todos los individuos. Mediante este análisis genético se detectaron seis individuos con las mutaciones F508 e I507. El método implementado en nuestro laboratorio podría servir para realizar un sondeo poblacional de portadores de mutaciones para la FQ. Cystic fibrosis is the most common autosomal recessive disease in the Caucasian population and is characterized by obstruction of passages, especially of the lungs, pancreas, and genital tract. It presents in 1 of every 2000 to 2500 live births, and 1 of every 20 to 25 newborns are carriers. The disease is caused by different mutations of the cystic fibrosis transmembrane conductance regulator (CFTR) gene. The most common mutation in the CFTR gene is the deletion of three base pairs (CTT) known as F508.Our study was performed for the purpose of standardizing PCR-mediated site-directed mutagenesis (PSM) for detection of the F508 mutation in patients with cystic fibrosis. The method used was validated by DNA sequencing of exon 10 in all individuals. This genetic analysis detected 6 persons with F508 and I507 mutations. The method used in our laboratory could be useful for carrying out a population survey for carriers of the mutations for cystic fibrosis.

Bioethics first appeared in Paraguay during the late 1980s and the early 1990s within the context of medicine and research at the universities. Initially, Van Rensselaer Potter’s principles were the main focus of study without much consideration for the cultural values of Paraguay or Latin America. Its growth has been slow but continuous, with the formation of hospital bioethics committees, training groups, annual courses, and the establishment of the National Commission for the Development of Bioethics in 2004. From the beginning, the pioneering groups and individuals in bioethics have all been related in some way to the field of medicine, because this is where their common concerns have been derived. Among these prominent figures were Dr. Julia Rivarola, pediatrics; Dr. Eduardo Aquino, specializing in hospital management; Dr. Rosalino Pinto, gynecologist; Dr. Enrique de Mestral, geriatrist; nurses Olga Lopez de Benitez, Julia Carmen Morel de Festner, and Deolinda Maciel; Father Alberto Leon, and Dr. Marta Ascurra, among others.

Paraguay is a developing country with low levels of health coverage, with 81% of the population without health insurance, a proportion that reaches 98.1% among the poor, 93% among the rural population and 91.7% among the mainly Guarani-speaking population. The infant mortality rate is 19.4 per 1,000, although there is gross under-reporting. Maternal mortality rate is alarmingly high at 110.9 per 100,000 livebirths, reaching 420.5 in rural areas. There are only two clinical geneticists and four biochemists trained in human genetics, and virtually all genetic services in the country are concentrated in the ‘Instituto de Investigaciones en Ciencias de la Salud’ (IICS) from the National University. The teaching of medical genetics in medical schools is included in physiology and pathology courses, while at the postgraduate level, training in medical genetics is limited to pediatrics and gynecology. In 1999, a pilot newborn screening program was initiated to determine the frequency of congenital hypothyroidism and phenylketonuria and to provide early treatment for affected babies. Another pilot project recently launched by the Ministry of Health is the Program for the Prevention of Neural Tube Defects, mandating folic acid fortification of flour, but as of the end of 2003 it had not been implemented. Paraguay lacks adequate resources to provide accurate diagnoses and treatment of genetic conditions.
