Abstract

Summary: Background: Prognostic factors for ambulatory oncology patients have been described, including Eastern Cooperative Oncology Group (ECOG), tumor stage and malnutrition. However, there is no firm evidence on which variables best predict mortality in hospitalized patients receiving active systemic treatment. Our main goal was to develop a predictive model for 90-day mortality upon admission. Methods: Between 2020 and 2022, we prospectively collected data from three sites for cancer patients with hospitalizations. Those with metastatic disease receiving systemic therapy in the 6 months before unplanned admission were eligible to this study. The least absolute shrinkage and selection operator (LASSO) method was used to select the most relevant factors to predict 90-day mortality at admission. A multivariable logistic regression was fitted to create the PROgnostic Score for Hospitalized Cancer Patients (PROMISE) score. The score was developed in a single-center training cohort and externally validated. Findings: Of 1658 hospitalized patients, 1009 met eligibility criteria. Baseline demographics, patient and disease characteristics were similar across cohorts. Lung cancer was the most common tumor type in both cohorts. Factors associated with higher 90-day mortality included worse ECOG, stable/progressive disease, low levels of albumin, increased absolute neutrophil count, and high lactate dehydrogenase. The c-index after bootstrap correction was 0.79 (95% CI, 0.75–0.82) and 0.74 (95% CI, 0.68–0.80) in the training and validation cohorts, respectively. A web tool (https://promise.vhio.net/) was developed to facilitate the clinical deployment of the model. Interpretation: The PROMISE tool demonstrated high performance for identifying metastatic cancer patients who are alive 90 days after an unplanned hospitalization. This will facilitate healthcare providers with rational clinical decisions and care planning after discharge. Funding: Merck S.L.U., Spain.

Abstract

Currently, educational institutions value as an important goal to contribute to the achievement of happiness in their students. This study focuses on medical students in whom a high level of emotional wasting has been found. The objective of this study is to test a model to predict happiness. This model is based on predictors described by positive psychology and that are related to personal resources and dispositions. A non-experimental, cross-sectional design and a non-probabilistic sampling method were used. Six scales aimed at measuring seven variables (gratitude, optimism, resilience, self-esteem, supports received from friends and family, and happiness) were applied to a sample composed of 301 Mexican students in their first three years of medical school at a private university from Monterrey. Data were analyzed by path analysis. Maximum Likelihood estimation was used and Bias-Corrected Percentile confidence intervals were calculated. The model showed a close fit to the data. The supports received from family and friends, which were specified as exogenous variables, had a strong positive correlation. Both variables predicted greater level of gratitude. Greater level of gratitude predicted greater levels of optimism, self-esteem, and resilience. Greater level of optimism predicted greater level of self-esteem. In addition, greater level of self-esteem was predicted by greater level of friend support. Finally, greater levels of resilience, self-esteem, and friend support predicted greater level of happiness. 22. Trends in Urban Wild Meat Trade of Chelonians (Turtles and Tortoises) in the Peruvian Amazon.

Abstract

Simple Summary: Turtles provide the rural and urban populations of tropical forests with food and income. We studied one of the largest urban wild meat markets in the Amazon between 2006 and 2018. Turtle meat was the most expensive animal protein, being 50% more costly than the most common fish, Prochilodus nigricans, and 48% more costly than poultry meat. Most of the turtle meat was from Chelonoidis denticulatus (86.3%) and Podocnemis unifilis (13.6%). Eggs from P. unifilis and P. expansa were also sold. Culturally, chelonians are important sources of nutrition and income for rural and urban people in tropical rainforests, but urban trade can cause declines in wild populations. This study analyses the urban chelonian trade and its trends to better understand the impact on free-living populations in the Northern Peruvian Amazon. We studied the urban trade of wild chelonians by conducting 526 days of participant observation and structured questionnaires with the main chelonian vendors in the wet markets of Iquitos between 2006 and 2018. The trade of chelonians decreased by −161.6%, from 22,694 individuals in 2006/07 to 8657 individuals in 2017/18. Chelonoidis denticulatus was the species sold the most (86.3%), followed by Podocnemis unifilis (13.6%). Podocnemis expansa was only sold in 2006/07. River turtle sales increased in the dry season, while sales of Chelonoidis denticulatus decreased. Turtles were the most expensive meat sold in urban markets: 49.7% more expensive than the most frequently consumed fish, Prochilodus nigricans, and 48.4% more expensive than poultry. Turtles represented only 0.19% (SD 0.23) of the index of domestic meat and fish consumption per capita. River turtle eggs are sold by a different set of vendors, and in 2017/18, 570,229 eggs were traded in the market of Belén in Iquitos, equivalent to an approximate lay of 10,418 P. unifilis and 1178 P. expansa females. The high magnitude of the trade of river turtle eggs together with the reduction in their meat trade suggests a prioritization of egg sales, which should improve the conservation of adults, since meat sales can be detrimental to chelonian populations. Abstract

Abstract Background Immunotherapy is being tested in early‐stage non‐small cell lung cancer (NSCLC), and achieving higher rates of complete pathological responses (CPR) as compared to standard of care. Early identification of CPR patients has vital clinical implications. In this study, we focused on basal peripheral immune cells and their treatment‐related changes to find biomarkers associated to CPR. Methods Blood from 29 stage IIIA NSCLC patients participating in the NADIM trial (NCT03081689) was collected at diagnosis and post neoadjuvant treatment. More than 400 parameters of peripheral blood mononuclear cells (PBMCs) phenotype and plasma soluble factors were analyzed. Abstract

PURPOSE Neoadjuvant chemotherapy plus nivolumab has been shown to be effective in resectable non–small-cell lung cancer (NSCLC) in the NADIM trial (ClinicalTrials.gov identifier: NCT03081689 ). The 3-year overall survival (OS) and circulating tumor DNA (ctDNA) analysis have not been reported. METHODS This was an open-label, multicenter, single-arm, phase II trial in which patients with stage IIIA NSCLC, who were deemed to be surgically resectable, were treated with neoadjuvant paclitaxel (200 mg/m2 once a day) and carboplatin (area under curve 6) plus nivolumab (360 mg) once on day 1 of each 21-day cycle, for three cycles, followed by adjuvant nivolumab monotherapy for 1 year (240 mg once every 2 weeks for 4 months, followed by 480 mg once every 4 weeks for 8 months). The 3-year OS and ctDNA analysis were secondary objectives of the trial. RESULTS OS at 36 months was 81.9% (95% CI, 66.8 to 90.6) in the intention-to-treat population, rising to 91.0% (95% CI, 74.2 to 97.0) in the per-protocol population. Neither tumor mutation burden nor programmed cell death ligand-1 staining was predictive of survival. Conversely, low pretreatment levels of ctDNA were significantly associated with improved progression-free survival and OS (hazard ratio [HR], 0.20; 95% CI, 0.06 to 0.63, and HR, 0.07; 95% CI, 0.01 to 0.39, respectively). Clinical responses according to RECIST v1.1 criteria did not predict survival outcomes. However, undetectable ctDNA levels after neoadjuvant treatment were significantly associated with progression-free survival and OS (HR, 0.26; 95% CI, 0.07 to 0.93, and HR, 0.04; 95% CI, 0.00 to 0.55, respectively). The C-index to predict OS for ctDNA levels after neoadjuvant treatment (0.82) was superior to that of RECIST criteria (0.72). CONCLUSION The efficacy of neoadjuvant chemotherapy plus nivolumab in resectable NSCLC is supported by 3-year OS. ctDNA levels were significantly associated with OS and outperformed radiologic assessments in the prediction of survival.

Abstract

Maintaining population connectivity is an important component of jaguar (Panthera onca) conservation. However, the effectiveness of connectivity corridors can be affected by anthropogenic factors. At the southwestern limit of the jaguar’s current distribution in the Yungas and Dry Chaco ecoregions, connectivity among jaguar populations has been compromised by extensive deforestation. The extent that jaguars and their prey are using corridors in the region, and the factors that determine their use, however, has not been evaluated. We sampled 105 sites in 7 study areas in connectivity corridors in the Yungas and Dry Chaco ecoregions of Argentina, Bolivia, and Paraguay for jaguars and six large-bodied principal prey species through interviews with inhabitants and land owners. Using a multi-species community occupancy modeling framework, we examined species occurrence in relation to forest cover and anthropogenic factors. All species were positively associated with increasing forest cover, decreasing number of homesteads, and increasing distance from roads and settlements, with jaguar occurrence most strongly negatively correlated with anthropogenic factors. Mean conditional occupancy of jaguars was high (>75%) in only two study areas, while only two study areas had mean conditional occupancy estimates of jaguars that were consistent with prey occurrence, with other sites having lower jaguar occurrence than expected. Despite high levels of forest cover and sufficient prey availability, the lower than expected jaguar occurrence in most study areas suggests that illegal killing is likely a principal factor determining jaguar occurrence. The probability of site use by jaguars in most study areas was low, and consequently, functional connectivity is reduced in those areas and likely throughout the region. Our findings have range-wide implications for jaguar conservation since we show that despite sufficient habitat and prey availability, jaguar populations are likely reduced or extirpated by illegal killing driven by jaguar-ranching conflict, cultural norms, or fear.

37. Character Strengths Predict an Increase in Mental Health and Subjective Well-Being Over a One-Month Period During the COVID-19 Pandemic Lockdown

Abstract

This study examines whether character strengths predict resilience (operationalized as stable or higher mental health and subjective well-being despite an adverse event) over a period of approximately 1 month during the COVID-19 pandemic lockdown in Spain. Using a longitudinal design, participants (N = 348 adults) completed online measures of sociodemographic data, information regarding their situation in relation to the COVID-19, character strengths, general mental health, life satisfaction, positive affect and negative affect. All variables were measured at Time 1 and Time 2, except for sociodemographic and most COVID-related information (Time 1 only). Time 1 data collection was conducted between March 21, 2020 and April 2, 2020, i.e., approximately the second week of lockdown in Spain. Time 2 data collection was conducted between April 24, 2020 and May 18, 2020, after the Spanish government announced its intention to progressively release the lockdown. A principal component analysis of character strengths was conducted. Five character strength factors were extracted: fortitude, goodness, intellectual, interpersonal, and restraint. Factor structures at Times 1 and 2 were highly consistent. All character strength factors at Time 1 correlated positively with life satisfaction and positive affect, and negatively with negative affect and mental health at T2 (higher scores in the mental health measure indicate poorer mental health). Fortitude strengths showed the highest correlations. We conducted a series of regression analyses with strength factors at Time 1 as predictors, and mental health, life satisfaction, and positive and negative affect as dependent variables, controlling for their baseline levels. To test the directionality of the relationship between strengths and the dependent variables, all analyses were reversed. All character strength factors predicted an increase in mental health. They also predicted positive affect, with the exception of strengths of restraint. Fortitude, intellectual, and interpersonal strengths predicted an increase in life satisfaction. Finally, fortitude strengths, interpersonal strengths, and strengths of restraint, predicted a decrease in negative affect. None of the reversed analyses yielded significant effects. Limitations, implications, and possible character strengths-based interventions aimed at promoting mental health in the COVID-19 pandemic are discussed.

38. High Internal Phase Emulsions Preparation Using Citrus By-Products as Stabilizers

Abstract

The citrus juice industry produces about 50% of by-products. Citrus pomace (CP) contains many polysaccharides (mainly cellulose and pectin), which could act as stabilizers and emulsifiers. The aim of this work was to obtain high internal phase emulsions (HIPEs) using unmodified CP at different concentrations to valorize citrus by-products. The synergic effect of pea protein isolate (PPI) with CP to stabilize the HIPEs was also studied. HIPEs structure was analyzed using rheological and microscopy studies as well as color and physical stability of the emulsions. According to rheological data, all samples exhibited a solid-like behavior, as elastic modulus (G’) was higher than viscous modulus (G’’) within the viscoelastic linear region; as % CP and % PPI increased, greater values of G’ and apparent viscosity (η) were achieved. Microscopic images showed that oil droplets had a polyhedral shape and were enclosed by a thin layer of CP and PPI. Increasing concentrations of CP and PPI enhanced oil droplets packaging. Emulsions’ physical stability was better when adding PPI. The results showed that stable HIPEs with 1.25% of CP and PPI over 0.5% can be obtained. These HIPEs could be used to formulate emulsions for food applications, such as mayonnaises, fillings, or creams.
