Aims: To perform temporal validation of a risk prediction model for breast cancer‐related lymphoedema in the European population. Design: Temporal validation of a previously developed prediction model using a new retrospective cohort of women who had undergone axillary lymph node dissection between June 2018 and June 2020. Methods: We reviewed clinical records to identify women who did and did not develop lymphoedema within 2 years of surgery and to gather data regarding the variables included in the prediction model. The model was calibrated by calculating Spearman's correlation between observed and expected cases. Its accuracy in discriminating between patients who did versus did not develop lymphoedema was assessed by calculating the area under the receiver operating characteristic curve (AUC). Results: The validation cohort comprised 154 women, 41 of whom developed lymphoedema within 2 years of surgery. The value of Spearman's coefficient indicated a strong correlation between observed and expected cases. Sensitivity of the model was higher than in the derivation cohort, as was the value of the AUC. Conclusion: The model shows a good capacity to discriminate women at risk of lymphoedema and may therefore help in developing improved care pathways for individual patients. Implications for the Profession and/or Patient Care: Identifying risk factors for lymphoedema secondary to breast cancer treatment is vital given its impact on women's physical and emotional well‐being. El linfedema secundario al cáncer de mama (BCRL) se presenta cuando la capacidad de transporte del sistema linfático se disminuye debido al tratamiento del cáncer de mama. Se describe como una de las complicaciones más relevantes y limitantes secundarias al cáncer de mama. Se conocen distintos factores de riesgo descritos en la literatura científica que podrían determinar la probabilidad de presentar BCRL. También existen modelos predictivos que podrían ayudar a determinar el riesgo que presenta cada paciente, pero se desconoce su fiabilidad en la población europea. Además, hasta el momento no existe una intervención consensuada por expertos para la prevención del BCRL con un enfoque multidisciplinar, donde todos los profesionales implicados en el tratamiento del cáncer de mama unifiquen criterios para la prevención de esta patología, limitando así la aparición de nuevos casos de esta complicación tan discapacitante. Objetivos El presente trabajo tiene dos objetivos: 1) diseñar y evaluar la validez y fiabilidad de un modelo predictivo para la detección del riesgo a presentar linfedema secundario al cáncer de mama en población europea y 2) consensuar una intervención para la prevención del linfedema secundario al cáncer de mama con un enfoque multidisciplinar que incluya a todos los profesionales involucrados en el tratamiento del cáncer de mama.

Aims: To identify the risk factors for lymphoedema following axillary lymph node dissection (ALND) in a European sample and to propose a lymphoedema prediction model for this population. Design: Predictive retrospective cohort study comparing women who developed lymphoedema in 2 years of undergoing ALND with those who did not developed lymphoedema. Methods: We reviewed the clinical records of 504 women who, between January 2008 and May 2018, underwent surgery for breast cancer that involved ALND. Logistic regression was used to identify significant risk factors for lymphoedema. The prediction accuracy of the model was assessed by calculating the area under the receiver operating characteristic curve. Results: Of the 504 women whose records were analysed, 156 developed lymphoedema. Significant predictors identified in the regression model were level of lymph node dissection, lymph node status, post-operative complications, body mass index (BMI) and number of lymph nodes extracted. The prediction model showed good sensitivity (80%) in the study population. Conclusions: The factor contributing most to the risk of lymphoedema was the level of lymph node dissection, and the only patient-related factor in the prediction model was BMI. The model offers good predictive capacity in this population and it is a simple tool that breast care units could use to assess the risk of lymphoedema following ALND. Nurses with specialist knowledge of lymphoedema have a key role to play in ensuring that women receive holistic and individualized care. Impact: What problem did the study address? Secondary lymphoedema is one of the main complications in the treatment of breast cancer. What were the main findings? The prediction model included five factors associated with the risk of lymphoedema following ALND. The strongest predictor was the level of lymph node dissection, and the only patient-related factor was BMI. Where and on whom will the research have an impact? The prediction model offers breast care units a tool for assessing the risk of lymphoedema in women undergoing surgery involving ALND. El cáncer de mama es el más habitual en las mujeres, siendo la supervivencia global del 85,8% en España. Como consecuencia desarrolla varias complicaciones en el tiempo, hallando la más importante y limitadora el linfedema. El linfedema es un acúmulo anormal de linfa en el espacio intersticial, lo que provoca una inflamación del miembro afecto. Por este motivo surge la necesidad de crear circuitos de prevención, para limitar el número de nuevos casos y establecer un conocimiento claro sobre las posibilidades de presentar linfedema. Por ello, siendo conscientes de los factores de riesgo, la validación de un instrumento para valorar el riesgo a presentar linfedema después de la cirugía del cáncer de mama, sería el primer paso para establecer estos circuitos de prevención y minimizar esta complicación tan inhabilitante. Objetivo Adaptar y evaluar la validez externa (capacidad predictiva) de riesgo de linfedema secundario al tratamiento del cáncer de mama, del Wannan Medical College Scoring System. Método Estudio observacional analítico de casos y controles retrospectivo para determinar la capacidad predictiva del sistema de puntuación de la Wannan Medical College Scoring System Spanish Form. La población de estudio serán mujeres diagnosticadas de cáncer de mama, intervenidas para la extirpación del tumor con disección axilar entre los años 2008 hasta el 2014 y hayan desarrollado o no linfedema en el Hospital de la Santa Creu i Sant Pau Breast cancer is the most common cancer in women with overall survival of 85.8% in Spain. As a consequence of the treatment, the patients can develop several complications over time. The most limiting of them is the lymphedema. Lymphedema is an abnormal accumulation of lymph in the interstitial space, causing inflammation of the affected limb. For this reason, it would be very interesting to create prevention circuits, to limit the number of new cases and establish a clear knowledge about the possibilities of presenting lymphedema. Therefore, being aware of the risk factors, the validation of an instrument to assess the risk of presenting lymphedema after breast cancer surgery would be the first step to establish these prevention strategies and minimize this disabling complication. Aim Adapt and evaluate the external validity (predictive capacity) of secondary lymphedema risk to breast cancer treatment, from the Wannan Medical College Scoring System. Methods An observational, retrospective case-control study to determine the predictive capacity of the Wannan Medical College Scoring System. The study population will be women diagnosed with breast cancer who underwent surgery to remove the tumour with axillary dissection. Between 2008 and 2014 and have or have not developed lymphedema at the Hospital de la Santa Creu i Sant Pau.
