Abstract

Abstract High frequencies of stem-like memory T cells in infusion products correlate with superior patient outcomes across multiple T cell therapy trials. Herein, we analyzed a published CRISPR activation screening to identify transcriptional regulators that could be harnessed to augment stem-like behavior in CD8+ T cells. Using IFN-γ production as a proxy for CD8+ T cell terminal differentiation, LMO4 emerged among the top hits inhibiting the development of effectors cells. Consistently, we found that Lmo4 was downregulated upon CD8+ T cell activation but maintained under culture conditions facilitating the formation of stem-like T cells. By employing a synthetic biology approach to ectopically express LMO4 in antitumor CD8+ T cells, we enabled selective expansion and enhanced persistence of transduced cells, while limiting their terminal differentiation and senescence. LMO4 overexpression promoted transcriptional programs regulating stemness, increasing the numbers of stem-like CD8+ memory T cells and enhancing their polyfunctionality and recall capacity. When tested in syngeneic and xenograft tumor models, LMO4 overexpression boosted CD8+ T cell antitumor immunity, resulting in enhanced tumor regression. Rather than directly modulating gene transcription, LMO4 bound to JAK1 and potentiated STAT3 signaling in response to IL-21, inducing the expression of target genes (Tcf7, Socs3, Junb, and Zfp36) crucial for memory responses. CRISPR/Cas9-deletion of Stat3 nullified the enhanced memory signature conferred by LMO4, thereby abrogating the therapeutic benefit of LMO4 overexpression. These results establish LMO4 overexpression as an effective strategy to boost CD8+ T cell stemness, providing a new synthetic biology tool to bolster the efficacy of T cell-based immunotherapies.

Abstract

Como consecuencia de la pandemia, los sistemas educativos de América Latina pasaron a funcionar de manera remota a través de internet. Surgieron nuevas necesidades que debían atenderse para garantizar la continuidad de las clases. En este contexto, marcado por gran incertidumbre, se investiga el papel de las redes personales de docentes peruanos en tanto potenciales fuentes de apoyo. Asimismo, se analiza la mediación del contexto organizacional educativo en el funcionamiento de estas redes que favoreció prácticas de apoyo mutuo. El diseño metodológico fue de carácter mixto y los resultados sugieren un apoyo multidimensional que desbordó la dimensión laboral-pedagógica, pero también tuvo efectos negativos en las vidas personales de los docentes.

Abstract

Los docentes constituyen un pilar clave del proceso educativo. En una situación-límite disruptiva de la vida cotidiana, como aquella de la pandemia del COVID-19, ¿cómo fueron puestas en movimiento las redes personales de apoyo de los docentes para responder a sus emergentes necesidades laborales (enseñanza) y personales? ¿Cuál fue el papel de los marcos institucionales y organizacionales en este proceso? Este estudio busca responder estas preguntas a partir de material empírico de docentes peruanos de educación secundaria durante la pandemia del COVID-19. El diseño metodológico fue de carácter mixto (sociométrico y cualitativo). Se encontró una diversidad de procesos de movilización de los lazos de apoyo. En particular, destacamos la presencia de procesos de movilización mixtos. Asimismo, los marcos institucionales (familia) y organizacionales (instituciones educativas) permearon de diferentes maneras estos procesos. Los docentes recrearon rutinas organizacionales que existían antes de la pandemia, pero también desarrollaron nuevas prácticas de apoyo. Algunas de estas prácticas tendieron a rutinizarse, pero otras fueron resistidas, debido a sus consecuencias no buscadas negativas en las vidas de los docentes. Estos resultados aportan nuevos elementos analíticos a teorías existentes sobre procesos de movilización de lazos de apoyo.

Abstract

Abstract Backgound In the workup of follicular lymphoma (FL), bone marrow biopsy (BMB) assessment is a key component of FLIPI and FLIPI2, the most widely used outcome scores. During the previous decade, several studies explored the role of FDG‐PET/CT for detecting nodal and extranodal disease, with only one large study comparing both techniques. Methods The aim of our study was to evaluate the diagnostic accuracy and the prognostic impact of both procedures in a retrospective cohort of 299 FL patients with both tests performed at diagnosis. In order to avoid a collinearity bias, FLIPI2 was deconstructed in its founding parameters, and the bone marrow involvement (BMI) parameter separately included as: a positive BMB, a positive PET/CT, the combined “PET/CT and BMB positive” or “PET/CT or BMB positive”. These variables were also confronted independently with the POD24 in 233 patients treated with intensive regimens. Results In the total cohort, bone marrow was involved in 124 and 60 patients by BMB and PET/CT, respectively. In terms of overall survival, age > 60 y.o. and the combined “PET/CT or BMB positive” achieved statistical independence as a prognostic factor. In patients treated with an intensive regimen, only the combined “PET/CT or BMB positive” added prognostic value for a shorter overall survival, when confronted with the POD24. Conclusion Our results show that in FL both BMB and PET/CT should be considered at diagnosis, as their combined assessment provides independent prognostic value in the context of the most widely use clinical scores.

Some authors propose that the way to reduce VILI is to find the threshold of driving pressure below which VILI is minimized. Abstract

Abstract Patients undergoing cardiac surgery represent a challenge in terms of pain management due to multiple factors relating to the patients and to the procedure itself. Our aim was to identify the influence of levels of preoperative anxiety on postoperative pain in patients undergoing cardiac surgery and explore associations between preoperative anxiety, postoperative pain, analgesic requirements, and sex. We present a prospective cohort study of 116 patients undergoing cardiac surgery between January and April 2020. Preoperative anxiety was evaluated using the State-Trait Anxiety Inventory and the amount of morphine needed to keep pain intensity below 4 on the verbal numerical rating scale was recorded for 48 h post-surgery. Given the extracorporeal circulation time, type of surgery and body surface, it was observed that every percentile increase in preoperative state anxiety led to an extra 0.068 mg of morphine being administered. For each extra year of age, the amount of morphine needed decreased by 0.26 mg, no difference was observed between men and women in terms of preoperative anxiety or postoperative analgesics requirements. It may be concluded that in cardiac surgery, postoperative analgesic requirements increased with higher levels of preoperative state anxiety and decreased for every extra year of age.

This is an open-access article distributed under the terms of the Creative Commons Attribution License (CC BY)., The high prevalence of burnout in resident physicians is expected to have increased as a result of the expansion of the pandemic. We conducted a systematic review with a meta-analysis of studies conducted during the first wave of the COVID-19 pandemic on burnout in residents and potential associated risk factors. Methods: The search was done in the Web of Science, MEDLINE, Scopus, and Lillac databases (April 2020–October 2021) using a priori protocol based on the PRISMA guidelines. The Newcastle Ottawa Scale was used to assess the risk of bias in the included studies. We estimated the pooled prevalence (95% CI) of burnout and the prevalence ratio (95% CI) of each risk factor associated. Results: We included 23 studies from 451 potential initial articles and those written in the English language; all of the collected studies were cross-sectional with anonymous online surveys, involving 4,998 responders (34%), of which 53.2% were female responders, 51% were R1-2, and 71% were in direct contact with COVID-19 patients. Eighty-seven percent presented a low-to-moderate risk of bias. Publication bias was not shown. The estimated pooled prevalence of burnout was 40% (95% CI = 0.26 – 0.57). Burnout was associated with psychiatry history (PR = 4.60, 95% CI = 1.06 – 20.06). There were no differences by gender, civil status, children in-charge, year of residency, or time exposure to COVID-19. Discussion: The overall prevalence of burnout in residents during the first wave of the pandemic was in line with the results described in this collective before the pandemic. The presence of a psychiatry history was a potential burnout risk factor, suggesting a high vulnerability during the peak of the stress period and the need to implement mental health surveillance for this subgroup., Peer Reviewed, Postprint (published version)

23. Evaluación del cortisol salivar como test diagnóstico en la insuficiencia adrenal frente al test de estímulo con ACTH

Abstract

La insuficiencia suprarrenal es una patología potencialmente letal, especialmente en fase aguda, por lo que es imprescindible el diagnóstico precoz, así como la instauración de tratamiento sustitutivo ante la sospecha de la misma. Nuestro objetivo es evaluar la utilidad del cortisol salival (CS) en el diagnóstico de la insuficiencia adrenal (IA), así como analizar la indemnidad del eje hipotálamo-hipofisario-adrenal mediante la determinación del ritmo de cortisol, su correlación con los niveles plasmáticos, y la influencia del tratamiento con hidrocortisona (HC) sustitutiva en la determinación del CS. Realizamos un estudio prospectivo en pacientes con sospecha de IA con realización de test de ACTH (ACTH-t) entre los años 2014-2022 y determinación del ritmo de cortisol salivar ambulatorio. Se recogieron variables clínicas y analíticas. Se valoró el rendimiento diagnóstico del CS frente al cortisol plasmático basal (CB) mediante el área bajo la curva ROC (AUC), tomando el resultado del ACTH-t como “gold standard”. Se determinó la correlación entre los valores de cortisol salivar y cortisol basal. Se evaluaron un total de 69 sujetos, (58,5% mujeres, 23,2% evaluados por insuficiencia adrenal) con una edad media 52,0±15,8 años. El 5.8% fueron excluidos por fallos en la recogida de la muestra. Al analizar la asociación entre los niveles de CS y CB se obtuvo una correlación positiva moderada r= 0,405 y p<0,001 El rendimiento diagnóstico del CS a las 8:00h fue superior al del CB (AUC=0,805, p<0,01 y AUC=0,783, p<0,05, respectivamente). Niveles de CS <0,0975 ug/dL fueron diagnósticos de IA, mientras que CS>0,794 ug/dL descartaron IA, evitando la realización del 20% de los ACTH-t. La determinación del ritmo de CS en pacientes en tratamiento con HC frente a aquellos sin tratamiento demostró valores de sobredosificación en todas las determinaciones, salvo a las 8:00h (0,26 vs 0,41 ug/dl, p< 0,001) La determinación del CS a las 8:00am es una alternativa válida en el cribado de l, Grado en Medicina

La obra martiana constituye fuente inagotable de aprendizajes. Ello se aprovecha como herramienta de trabajo en el proceso de formación de valores de las nuevas generaciones, fundamentalmente en el contexto de la Educación Superior, en el que resulta tan importante que los jóvenes se mantengan como puntales de la obra de la Revolución y sean ejemplos para el futuro. De ahí que surja el presente artículo, que tiene como objetivo presentar un análisis de las obras que conforman la dramaturgia martiana, en función del proceso de formación de valores que marca la labor docente educativa en el contexto universitario. Para ello se realiza un breve análisis literario de las obras.

Abstract

In the workup of follicular lymphoma (FL), bone marrow biopsy (BMB) assessment is a key component of FLIPI and FLIPI2, the most widely used outcome scores. During the previous decade, several studies explored the role of FDG-PET/CT for detecting nodal and extranodal disease, with only one large study comparing both techniques.The aim of our study was to evaluate the diagnostic accuracy and the prognostic impact of both procedures in a retrospective cohort of 299 FL patients with both tests performed at diagnosis. In order to avoid a collinearity bias, FLIPI2 was deconstructed in its founding parameters, and the bone marrow involvement (BMI) parameter separately included as: a positive BMB, a positive PET/CT, the combined "PET/CT and BMB positive" or "PET/CT or BMB positive". These variables were also confronted independently with the POD24 in 233 patients treated with intensive regimens.In the total cohort, bone marrow was involved in 124 and 60 patients by BMB and PET/CT, respectively. In terms of overall survival, age 60 y.o. and the combined "PET/CT or BMB positive" achieved statistical independence as a prognostic factor. In patients treated with an intensive regimen, only the combined "PET/CT or BMB positive" added prognostic value for a shorter overall survival, when confronted with the POD24.Our results show that in FL both BMB and PET/CT should be considered at diagnosis, as their combined assessment provides independent prognostic value in the context of the most widely use clinical scores.
