1. Comparison of two propensity score-based methods for balancing covariates: the overlap weighting and fine stratification methods in real-world claims data

Abstract: Abstract Background Two propensity score (PS) based balancing covariate methods, the overlap weighting method (OW) and the fine stratification method (FS), produce superb covariate balance. OW has been compared with various weighting methods while FS has been compared with the traditional stratification method and various matching methods. However, no study has yet compared OW and FS. In addition, OW has not yet been evaluated in large claims data with low prevalence exposure and with low frequency outcomes, a context in which optimal use of balancing methods is critical. In the study, we aimed to compare OW and FS using real-world data and simulations with low prevalence exposure and with low frequency outcomes. Methods We used the Texas State Medicaid claims data on adult beneficiaries with diabetes in 2012 as an empirical example (N = 42,628). Based on its real-world research question, we estimated an average treatment effect of health center vs. non-health center attendance in the total population. We also performed simulations to evaluate their relative performance. To preserve associations between covariates, we used the plasmode approach to simulate outcomes and/or exposures with N = 4,000. We simulated both homogeneous and heterogeneous treatment effects with various outcome risks (1-30% or observed: 27.75%) and/or exposure prevalence (2.5-30% or observed:10.55%). We used a weighted generalized linear model to estimate the exposure effect and the cluster-robust standard error (SE) method to estimate its SE. Results In the empirical example, we found that OW had smaller standardized mean differences in all covariates (range: OW: 0.0–0.02 vs. FS: 0.22–3.26) and Mahalanobis balance distance (MB) ( 0.049) than FS. In simulations, OW also achieved smaller MB (homogeneity: 0.04; heterogeneity: 0.0-0.11 vs. 0.07–0.29), relative bias (homogeneity: 4.04–56.20 vs. 20–61.63; heterogeneity: 7.85–57.6 vs. 15.0-60.4), square root of mean squared error (homogeneity: 0.332–1.308 vs. 0.385–1.365; heterogeneity: 0.263-0.526 vs 0.313-0.620), and coverage probability (homogeneity: 0.0–80.4% vs. 0.0-69.8%; heterogeneity: 0.0-97.6% vs. 0.0-92.8%), than FS, in most cases. Conclusions These findings suggest that OW can yield nearly perfect covariate balance and therefore enhance the accuracy of average treatment effect estimation in the total population.

2. Utilization, quality, and spending for pediatric Medicaid enrollees with primary care in health centers vs non-health centers

Abstract: Abstract Background Limited research has explored the performance of health centers (HCs) compared to other primary care settings among children in the United States. We evaluated utilization, quality, and expenditures for pediatric Medicaid enrollees receiving care in HCs versus non-HCs. Methods This national cross-sectional study utilized 2012 Medicaid Analytic eXtract (MAX) claims to examine children 0–17 years with a primary care visit, stratified by whether majority (> 50%) of primary care visits were at HCs or non-HCs. Outcome measures include utilization (primary care visits, non-primary care outpatient visits, prescription claims, Emergency Department (ED) visits, hospitalizations) and quality (well-child visits, avoidable ED visits, avoidable hospitalizations). For children enrolled in fee-for-service Medicaid, we also measured expenditures. Propensity score-based overlap weighting was used to balance covariates. Results A total of 2,383,270 Medicaid-enrolled children received the majority of their primary care at HCs, while 18,540,743 did at non-HCs. In adjusted analyses, HC patients had 20% more primary care visits, 15% less non-primary care outpatient visits, and 21% less prescription claims than non-HC patients. ED visits were similar across the two groups, while HC patients had 7% lower chance of hospitalization than non-HC. Quality of care outcomes favored HC patients in main analyses, but results were less robust when excluding managed care beneficiaries. Total expenditures among the fee-for-service subpopulation were lower by $239 (8%) for HC patients. Conclusions In this study of nationwide claims data to evaluate healthcare utilization, quality, and spending among Medicaid-enrolled children who receive primary care at HCs versus non-HCs, findings suggest primary care delivery in HCs may be associated with a more cost-effective model of healthcare for children.

3. Advancing health equity through partnerships of state Medicaid agencies, Medicaid managed care organizations, and health care delivery organizations

Abstract: BackgroundReducing health inequities in marginalized populations, including people with Medicaid insurance, requires care transformation to address medical and social needs that is supported and incentivized by tailored payment methods. Collaboration across health care stakeholders is essential to overcome health system fragmentation and implement sustainable reform in the United States (U.S.). This paper explores how multi-stakeholder teams operationalized the Roadmap to Advance Health Equity model during early stages of their journey to (a) build cultures of equity and (b) integrate health equity into care transformation and payment reform initiatives.MethodsAdvancing Health Equity: Leading Care, Payment, and Systems Transformation is a national program in the U.S. funded by the Robert Wood Johnson Foundation that brings together multi-stakeholder teams to design and implement initiatives to advance health equity. Each team consisted of representatives from state Medicaid agencies, Medicaid managed care organizations, and health care delivery organizations in seven U.S. states. Between June and September 2021, semi-structured interviews were conducted with representatives (n = 23) from all seven teams about experiences implementing the Roadmap to Advance Health Equity model with technical assistance from Advancing Health Equity.ResultsFacilitators of building cultures of equity included (1) build upon preexisting intra-organizational cultures of equity, (2) recruit and promote diverse staff and build an inclusive culture, and (3) train staff on health equity and anti-racism. Teams faced challenges building inter-organizational cultures of equity. Facilitators of identifying a health equity focus area and its root causes included (1) use data to identify a health equity focus and (2) overcome stakeholder assumptions about inequities. Facilitators of implementing care transformation and payment reform included (1) partner with Medicaid members and individual providers and (2) support and incentivize equitable care and outcomes with payment. Facilitators of sustainability planning included (1) identify evidence of improved health equity focus and (2) maintain relationships among stakeholders. Teams faced challenges determining the role of the state Medicaid agency.ConclusionsMulti-stakeholder teams shared practical strategies for implementing the Roadmap to Advance Health Equity that can inform future efforts to build intra- and inter-organizational cultures of equity and integrate health equity into care delivery and payment systems.

4. Standup comedy principles and the personal monologue to explore interpersonal bias: experiential learning in a health disparities course

Abstract: Abstract Background Interpersonal biases between clinicians and patients contribute to disparities in health care and outcomes by race, ethnicity, and socioeconomic status. We used standup comedy principles and exercises to help medical students recognize how others perceive them and how they perceive others, and engage in difficult discussions around implicit biases and interpersonal racism. Methods 90 min Zoom workshop with 40 first-year medical students in urban medical school. Intervention consisted of three exercises: Naming icebreaker, Rant and Rave (communicate strong perspective clearly), and Personal Monologue about how others perceive you and how you perceive yourself. Discussion debriefed the personal monologue exercise. Likert scale questions on post-session survey evaluated workshop overall, whether workshop increased skills, and safety of learning environment. Open-ended questions included what trainees liked about the module, what could be improved, and what impact the module had on them? Results Seventeen (42.5%) students responded to survey. Six respondents identified as white, 4 as Asian, 1 as Black, 1 as multiracial, and 5 did not identify. Seventy-six percent rated the module as “very good” or “excellent”, and 94% would recommend the module to others. Most respondents reported the workshop helped them become better listeners (75%) and more observant (82%). Eighty-three percent reported the training could help them take better care of patients with lived experiences different than their own. All respondents believed the learning environment was safe, and 94% reported that instructors created an atmosphere in which they could take risks. Thirty-six percent felt stressed. Students reported the workshop helped them recognize their own identities, others’ perceptions, and bidirectional biases, and inspired them to strive for more accurate, authentic interactions with patients. Conclusions Standup comedy principles show promise for engaging students in meaningful, safe discussions about perceptions and interpersonal biases rooted in their own personal experiences and those of their classmates.

Abstract: Summary: Intestinal epithelial tight junction disruption is a primary contributing factor in alcohol-associated endotoxemia, systemic inflammation, and multiple organ damage. Ethanol and acetaldehyde disrupt tight junctions by elevating intracellular Ca2+. Here we identify TRPV6, a Ca2+-permeable channel, as responsible for alcohol-induced elevation of intracellular Ca2+, intestinal barrier dysfunction, and systemic inflammation. Ethanol and acetaldehyde elicit TRPV6 ionic currents in Caco-2 cells. Studies in Caco-2 cell monolayers and mouse intestinal organoids show that TRPV6 deficiency or inhibition attenuates ethanol- and acetaldehyde-induced Ca2+ influx, tight junction disruption, and barrier dysfunction. Moreover, Trpv6−/− mice are resistant to alcohol-induced intestinal barrier dysfunction. Photoaffinity labeling of 3-azibutanol identifies a histidine as a potential alcohol-binding site in TRPV6. The substitution of this histidine, and a nearby arginine, reduces ethanol-activated currents. Our findings reveal that TRPV6 is required for alcohol-induced gut barrier dysfunction and inflammation. Molecules that decrease TRPV6 function have the potential to attenuate alcohol-associated tissue injury.

Abstract: We report on the second observing program of the active galactic nucleus NGC 4151 with simultaneous Imaging X-ray Polarimetry Explorer (IXPE; {\sim}750 ks), NuSTAR ({\sim}60 ks), XMM-Newton ({\sim}75 ks), and NICER ({\sim}65 ks) pointings. NGC 4151 is the first Type 1 radio-quiet Seyfert galaxy with constrained polarization properties for the X-ray corona. Despite the lower flux state in which the source is re-observed and the resulting higher contribution of the constant reflection component in the IXPE energy band, our results are in agreement with the first detection. From the polarimetric analysis, a polarization degree {\Pi} = 4.7 {\pm} 1.3 per cent and angle {\Psi} = 77{\deg} {\pm} 8{\deg} east of north (68 per cent confidence level) are derived in the 2.0 - 8.0 keV energy range. Combining the two observations leads to polarization properties that are more constrained than those of the individual detections, showing {\Pi} = 4.5 {\pm} 0.9 per cent and {\Psi} = 81{\deg} {\pm} 6{\deg} (with detection significance {\sim}4.6{\sigma}). The observed polarization angle aligns very well with the radio emission in this source, supporting, together with the significant polarization degree, a slab or wedge geometry for the X-ray corona. However, a switch in the polarization angle at low energies (37{\deg} {\pm} 7{\deg} in the 2 - 3.5 keV bin) suggests the presence of another component. When it is included in the spectro-polarimetric fit, a high polarization degree disfavors an interpretation in terms of a leakage through the absorbers, rather pointing to scattering from some kind of mirror., Comment: 10 pages, 8 figures, 3 tables. Abstract: We used the Imaging X-ray Polarimetry Explorer (IXPE) satellite to measure, for the first time, the 2-8 keV polarization of NGC 1068. We pointed IXPE for a net exposure time of 1.15 Ms on the target, in addition to two ~ 10 ks each Chandra snapshots in order to account for the potential impact of several ultraluminous X-ray source (ULXs) within IXPE's field-of-view. We measured a 2 - 8 keV polarization degree of 12.4% +/- 3.6% and an electric vector polarization angle of 101{\deg} +/- 8{\deg} at 68% confidence level. If we exclude the spectral region containing the bright Fe K lines and other soft X-ray lines where depolarization occurs, the polarization fraction rises up to 21.3% +/- 6.7% in the 3.5 - 6.0 keV band, with a similar polarization angle. The observed polarization angle is found to be perpendicular to the parsec scale radio jet. Using a combined Chandra and IXPE analysis plus multi-wavelength constraints, we estimated that the circumnuclear "torus" may sustain a half-opening angle of 50{\deg} - 55{\deg} (from the vertical axis of the system). Thanks to IXPE, we have measured the X-ray polarization of NGC 1068 and found comparable results, both in terms of polarization angle orientation with respect to the radio-jet and torus half-opening angle, to the X-ray polarimetric measurement achieved for the other archetypal Compton-thick AGN : the Circinus galaxy. 11. Extracting Insights From Temporal Data by Integrating Dynamic Modeling and Machine Learning

Abstract: Biological processes are dynamic. As a result, temporal analyses are necessary to fully understand the complex interactions that occurs within these systems. One example of a multifaceted biological process is restitution: the initial step in complex wound repair. Restitution is a dynamic process that depends on an elegant orchestration between damaged cells and their intact neighbors. Such orchestration enables the quick repair of the damaged area, which is essential to preserve epithelial integrity and prevent further injury. High quality dynamic data of the cellular and molecular events that make up the gastric restitution process has been documented. However, comprehensive dynamic models that connect all relevant molecular interactions to cellular behaviors are challenging to construct and experimentally validate. In order to efficiently provide feedback to ongoing experimental work, we have integrated dynamical modeling and machine learning to efficiently extract data-driven insights without incorporating detailed mechanisms. Dynamical models convert time course data into a set of static features, which are then subjected to machine learning analysis. The integrated analysis provides data-driven insights into how repair might be regulated in individual gastric organoids. We have provided a “proof of concept” of how such an analysis pipeline can be used to analyze any temporal dataset and provide timely data-driven insights.

12. Teaching Intersectionality of Sexual Orientation, Gender Identity, and Race/Ethnicity in a Health Disparities Course

Abstract: Introduction Intersectionality considers how different identities simultaneously affect an individual's experiences. Those of multiple minority statuses may experience effects of intersecting systems of oppression. Most health disparities curricula do not focus on intersectionality. We studied the impact of an innovative module teaching intersectionality of sexual orientation, gender identity, and race/ethnicity issues in the required Pritzker School of Medicine course Health Care Disparities: Equity and Advocacy. Methods A short lecture reviewed sexual and gender minority (SGM) health disparities, intersectionality, minority stress, and shared decision making (SDM) to establish shared language among 83 first-year medical students. Students then viewed four videos of SGM patients of color (POC) describing their health care experiences, each followed by moderated discussion about how compounded minority stress affects lived experiences and health and how to improve SDM for SGM POC. One video interviewee attended the session and answered students’ questions. Evaluation was performed using pre- and postsurveys. Results Feeling somewhat/completely confident in defining intersectionality increased from 57% to 96%. Prior to the session, 62% of respondents reported feeling somewhat/completely confident in identifying barriers to care for SGM patients, and 92% after. Thirty-three percent felt somewhat/completely confident in asking SGM patients about their identities before the session, and 81% after. Eighty-four percent rated the session as very good or excellent. Discussion The session was well received, improved student knowledge of intersectionality, and improved confidence in communicating with and caring for SGM patients. Future iterations could include condensing the lecture and including a patient panel and/or small-group discussion.

13. Multiple calcium sources are required for intracellular calcium mobilization during gastric organoid epithelial repair

Abstract: Abstract Calcium (Ca2+) is a known accelerator for gastric wound repair. We have demonstrated in vivo and in vitro that intracellular Ca2+ increases in the gastric epithelial cells directly adjacent to a damaged cell, and that this Ca2+ rise is essential for the cellular migration that rapidly repairs the epithelium (restitution). While intracellular Ca2+ has been shown to be an important signaling factor during epithelial restitution, the source from which this intracellular Ca2+ originates remains unclear. Using gastric organoids derived from mice transgenic for a genetically encoded Ca2+ indicator, we sought to investigate the potential sources of intracellular Ca2+ mobilization. During confocal imaging, photodamage (PD) was induced to 1–2 gastric organoid epithelial cells and epithelial restitution measured simultaneously with changes in intracellular Ca2+ (measured as FRET/CFP ratio in migrating cells adjacent to the damaged area). Inhibition of voltage‐gated Ca2+ channels (verapamil, 10 µM) or store‐operated calcium entry (YM58483, 20 µM) resulted in delayed repair and dampened intracellular Ca2+ response. Furthermore, inhibition of phospholipase C (U73122, 10 µM) or inositol trisphosphate receptor (2‐APB, 50 µM) likewise resulted in delayed repair and dampened Ca2+ response. Results suggest both extracellular and intracellular Ca2+ sources are essential for supplying the Ca2+ mobilization that stimulates repair.

Abstract: Abstract Background We examined the association of dialysis facility characteristics with payment reductions and change in clinical performance measures during the first year of the United States Centers for Medicare & Medicaid Services (CMS) End Stage Renal Disease Quality Incentive Plan (ESRD QIP) to determine its potential impact on quality and disparities in dialysis care. Abstract: Abstract Background The rates of diabetes and prediabetes in the USA are growing, significantly impacting the quality and length of life of those diagnosed and financially burdening society. Premature death and disability can be prevented through implementation of evidence-based programs and policies (EBPPs). Local health departments (LHDs) are uniquely positioned to implement diabetes control EBPPs because of their knowledge of, and focus on, community-level needs, contexts, and resources. There is a significant gap, however, between known diabetes control EBPPs and actual diabetes control activities conducted by LHDs. The purpose of this study is to determine how best to support the use of evidence-based public health for diabetes (and related chronic diseases) control among local-level public health practitioners. Methods/design This paper describes the methods for a two-phase study with a stepped-wedge cluster randomized trial that will evaluate dissemination strategies to increase the uptake of public health knowledge and EBPPs for diabetes control among LHDs. Phase 1 includes development of measures to assess practitioner views on and organizational supports for evidence-based public health, data collection using a national online survey of LHD chronic disease practitioners, and a needs assessment of factors influencing the uptake of diabetes control EBPPs among LHDs within one state in the USA. Phase 2 involves conducting a stepped-wedge cluster randomized trial to assess effectiveness of dissemination strategies with local-level practitioners at LHDs to enhance capacity and organizational support for evidence-based diabetes prevention and control. Twelve LHDs will be selected and randomly assigned to one of the three groups that cross over from usual practice to receive the intervention (dissemination) strategies at 8-month intervals; the intervention duration for groups ranges from 8 to 24 months. Intervention (dissemination) strategies may include multi-day in-person workshops, electronic information exchange methods, technical assistance through a knowledge broker, and organizational changes to support evidence-based public health approaches. Evaluation methods comprise surveys at baseline and the three crossover time points, abstraction of local-level diabetes and chronic disease control program plans and progress reports, and social network analysis to understand the relationships and contextual issues that influence EBPP adoption. Abstract: We present the results of the first X-ray polarimetric observation of the low-mass X-ray binary 4U 1957+115, performed with the Imaging X-ray Polarimetry Explorer in May 2023. The binary system has been in a high-soft spectral state since its discovery and is thought to host a black hole. The $\sim$571 ks observation reveals a linear polarisation degree of $1.9\% \pm 0.6\%$ and a polarisation angle of $-41^\circ.8 \pm 7^\circ.9$ in the 2-8 keV energy range. Spectral modelling is consistent with the dominant contribution coming from the standard accretion disc, while polarimetric data suggest a significant role of returning radiation: photons that are bent by strong gravity effects and forced to return to the disc surface, where they can be reflected before eventually reaching the observer. Abstract: Background & Aims: The peptic ulcer heals through a complex process, although the ulcer relapse often occurs several years later after healing. Our hypothesis is that even after visual evidence of healing of gastric ulceration, the regenerated epithelium is aberrant for an extended interval, increasing susceptibility of the regenerated epithelium to damage and further diseases. Methods: Gastric ulcers were induced in mice by serosal topical application of acetic acid. Results: Gastric ulcers induced by acetic acid visually healed within 30 days. However, regenerated epithelial architecture was poor. The gene profile of regenerated tissue was abnormal, indicating increased stem/progenitor cells, deficient differentiated gastric cell types, and deranged cell homeostasis. Despite up-regulation of PDX1 in the regenerated epithelium, no mature antral cell type was observed. Four months after healing, the regenerated epithelium lacks parietal cells, trefoil factor 2 (TFF2) and (sex-determining region Y)-box 9 (SOX9) remain up-regulated deep in the gastric gland, and the Na/H exchanger 2 (a TFF2 effector in gastric healing) remains down-regulated. Gastric ulcer healing was strongly delayed in TFF2 knockout mice, and re-epithelialization was accompanied with mucous metaplasia. After Helicobacter pylori inoculum 30 days after ulceration, we observed that the gastric ulcer selectively relapses at the same site where it originally was induced. Follow-up evaluation at 8 months showed that the relapsed ulcer was not healed in H pyloriâinfected tissues. Conclusions: These findings show that this macroscopically regenerated epithelium has prolonged abnormal cell distribution and is differentially susceptible to subsequent damage by H pylori. Abstract: Background & Aims: During aging, physiological changes in the stomach result in more tenuous gastric tissue that is less capable of repairing injury, leading to increased susceptibility to chronic ulceration. Spasmolytic polypeptide/trefoil factor 2âexpressing metaplasia (SPEM) is known to emerge after parietal cell loss and during Helicobacter pylori infection, however, its role in gastric ulcer repair is unknown. Therefore, we sought to investigate if SPEM plays a role in epithelial regeneration. Methods: Acetic acid ulcers were induced in young (2â3 mo) and aged (18â24 mo) C57BL/6 mice to determine the quality of ulcer repair with advancing age. Yellow chameleon 3.0 mice were used to generate yellow fluorescent proteinâexpressing organoids for transplantation. Yellow fluorescent proteinâpositiveÂ gastric organoids were transplanted into the submucosa and lumen of the stomach immediately after ulcer induction. Gastric tissue was collected and analyzed to determine the engraftment of organoid-derived cells within the regenerating epithelium. Results: Wound healing in young mice coincided with the emergence of SPEM within the ulcerated region, a response that was absent in the aged stomach. Although aged mice showed less metaplasia surrounding the ulcerated tissue, organoid-transplanted aged mice showed regenerated gastric glands containing organoid-derived cells. Organoid transplantation in the aged mice led to the emergence of SPEM and gastric regeneration. Conclusions: These data show the development of SPEM during gastric repair in response to injury that is absent in the aged stomach. Abstract: We report on a comprehensive analysis of simultaneous X-ray polarimetric and spectral data of the bright atoll source GX 9+9 with the Imaging X-ray Polarimetry Explorer (IXPE) and NuSTAR. The source is significantly polarized in the 4--8 keV band, with a degree of $2.2\% \pm 0.5\%$ (uncertainty at the 68% confidence level). The NuSTAR broad-band spectrum clearly shows an iron line, and is well described by a model including thermal disk emission, a Comptonized component, and reflection. From a spectro-polarimetric fit, we obtain an upper limit to the polarization degree of the disk of 4% (at 99% confidence level), while the contribution of Comptonized and reflected radiation cannot be conclusively separated. However, the polarization is consistent with resulting from a combination of Comptonization in a boundary or spreading layer, plus reflection off the disc, which gives a significant contribution in any realistic scenario., Comment: 10 pages, accepted for publication in A&A

Abstract: We present an X-ray spectro-polarimetric analysis of the bright Seyfert galaxy IC 4329A. The Imaging X-ray Polarimetry Explorer (IXPE) observed the source for ~500 ks, supported by XMM-Newton (~60 ks) and NuSTAR (~80 ks) exposures. We detect polarisation in the 2-8 keV band with 2.97 sigma confidence. We report a polarisation degree of $3.3\pm1.1$ per cent and a polarisation angle of $78\pm10$ degrees (errors are 1 sigma confidence). The X-ray polarisation is consistent with being aligned with the radio jet, albeit partially due to large uncertainties on the radio position angle. We jointly fit the spectra from the three observatories to constrain the presence of a relativistic reflection component. From this, we obtain constraints on the inclination angle to the inner disc (< 39 degrees at 99 per cent confidence) and the disc inner radius (< 11 gravitational radii at 99 per cent confidence), although we note that modelling systematics in practice add to the quoted statistical error. Our spectro-polarimetric modelling indicates that the 2-8 keV polarisation is consistent with being dominated by emission directly observed from the X-ray corona, but the polarisation of the reflection component is completely unconstrained. Our constraints on viewer inclination and polarisation degree tentatively favour more asymmetric, possibly out-flowing, coronal geometries that produce more highly polarised emission, but the coronal geometry is unconstrained at the 3 sigma level., Comment: Accepted for publication in MNRAS, 13 pages, 9 figures

Abstract: We report on the second observation of the radio-quiet active galactic nucleus (AGN) MCG-05-23-16 performed with the Imaging X-ray Polarimetry Explorer (IXPE). The observation started on 2022 November 6 for a net observing time of 640 ks, and was partly simultaneous with NuSTAR (86 ks). After combining these data with those obtained in the first IXPE pointing on May 2022 (simultaneous with XMM-Newton and NuSTAR) we find a 2-8 keV polarization degree $\Pi$ = 1.6 $\pm$ 0.7 (at 68 per cent confidence level), which corresponds to an upper limit $\Pi$ = 3.2 per cent (at 99 per cent confidence level). We then compare the polarization results with Monte Carlo simulations obtained with the MONK code, with which different coronal geometries have been explored (spherical lamppost, conical, slab and wedge). Furthermore, the allowed range of inclination angles is found for each geometry. 24. Health Literacy Not Associated with Chronic Kidney Disease Awareness

Abstract: Background: Patient awareness of their chronic kidney disease (CKD) and health literacy (HL) are both important for adherence to therapies that slow CKD progression and to reduce risk of complications. Little is known about the association between HL and CKD awareness. Objective: We sought to determine if patient HL is associated with CKD awareness. Methods: We conducted a cross-sectional study of general medicine inpatients at an urban academic medical center discharged between June 2011 and July 2013 with CKD, defined as having at least one CKD International Classification of Diseases, ninth revision code (585.0–585.9), among their first 20 admission diagnoses. Logistic regression was used to analyze the influence of HL, demographic, clinical, and health care use covariates on the likelihood of patients' CKD awareness. Our primary outcome was patient awareness of their CKD, defined as correct patient self-report of “kidney problems.” We used the Brief Health Literacy Screen, a three-item verbal questionnaire, to assess HL. Key Results: Among 1,308 patients with CKD, awareness of CKD was 33%, and 48% had adequate HL. However, CKD awareness was not associated with HL even among patients with stage 4 or 5 CKD. In multivariable logistic regression, greater awareness was associated with being a woman, younger than age 50 years, married, White, having hypertension, and having a higher CKD stage (all p < .05). In stratified analyses, patients with hypertension had greater CKD awareness, regardless of HL or diabetes status (p < .05). Conclusions: Among hospitalized patients with CKD, both CKD awareness and HL are low and inadequate. Surprisingly, patients' knowledge of their CKD diagnosis was not related to patients' HL. Patients with hypertension who young, white, or married may be receiving or retaining more education related to CKD. More work is needed on how to effectively communicate CKD diagnosis to prevent widening health disparities.

The prognostic model included a comprehensive geriatric type 2 diabetes simulation model (the Chicago model) and combinations of the physician estimate and the Chicago model (“And,” “Or,” and “Average” models). Observed survival was determined based on the National Death Index through 31 December 2010. The predictive accuracy of LE predictions was assessed using c-statistic for 5-year mortality; Harrell’s c-statistic, and Integrated Brier score for overall survival. Results: The patient cohort had a mean (SD) age of 73.4 (5.9) years. The majority were female (62.6%) and black (79.4%). At 5 years, 108 (24.2%) patients had died. The c-statistic for 5-year mortality was similar for physicians (0.69) and the Chicago model (0.68), while the average of estimates by physicians and Chicago model yielded the highest c-statistic of any method tested (0.73). The estimates of overall survival yielded a similar pattern of results. Limitations: Generalizability of patient cohort and lack of updated model parameters. Conclusions: Compared with individual methods, the average of LE estimates by physicians and the Chicago model had the best predictive performance. Prognostic models, such as the Chicago model, may complement and support physicians’ intuitions as they consider treatment decisions and goals for older patients with chronic conditions like diabetes.

Abstract: X Persei is a persistent low-luminosity X-ray pulsar of period of $\sim$835 s in a Be binary system. The field strength at the neutron star surface is not known precisely, but indirect signs indicate a magnetic field above $10^{13}$ G, which makes the object one of the most magnetized known X-ray pulsars. Here we present the results of observations X Persei performed with the Imaging X-ray Polarimetry Explorer (IXPE). The X-ray polarization signal was found to be strongly dependent on the spin phase of the pulsar. The energy-averaged polarization degree in 3-8 keV band varied from several to $\sim$20 per cent over the pulse with a positive correlation with the pulsed X-ray flux. The polarization angle shows significant variation and makes two complete revolutions during the pulse period resulting in nearly nil pulse-phase averaged polarization. Applying the rotating vector model to the IXPE data we obtain the estimates for the rotation axis inclination and its position angle on the sky as well as for the magnetic obliquity. The derived inclination is close to the orbital inclination reported earlier for X Persei. The polarimetric data imply a large angle between the rotation and magnetic dipole axes, which is similar to the result reported recently for the X-ray pulsar GRO J1008$-$57. After eliminating the effect of polarization angle rotation over the pulsar phase using the best-fitting rotating vector model, the strong dependence of the polarization degree with energy was discovered with its value increasing from 0% at $\sim$2 keV to 30% at 8 keV., Comment: 11 pages, 7 figures, submitted to MNRAS

Abstract: We present an X-ray spectro-polarimetric analysis of the bright Seyfert galaxy NGC4151. The source has been observed with the Imaging X-ray Polarimetry Explorer (IXPE) for 700 ks, complemented with simultaneous XMM-Newton (50 ks) and NuSTAR (100 ks) pointings. A polarization degree ${\Pi} = 4.9 {\pm} 1.1 \%$ and angle ${\Psi}= 86{\deg} {\pm} 7{\deg}$ east of north ($68\%$ confidence level) are measured in the 2-8 keV energy range. The spectro-polarimetric analysis shows that the polarization could be entirely due to reflection. Given the low reflection flux in the IXPE band, this requires however a reflection with a very large ($> 38 \%$) polarization degree. Assuming more reasonable values, a polarization degree of the hot corona ranging from ${\sim}4$ to ${\sim}8\%$ is found. The observed polarization degree excludes a spherical lamppost geometry for the corona, suggesting instead a slab-like geometry, possibly a wedge, as determined via Monte Carlo simulations. This is further confirmed by the X-ray polarization angle, which coincides with the direction of the extended radio emission in this source, supposed to match the disc axis. NGC4151 is the first AGN with an X-ray polarization measure for the corona, illustrating the capabilities of X-ray polarimetry and IXPE in unveiling its geometry., Comment: 9 pages, 4 figures

28. Robust circadian rhythms in organoid cultures from PERIOD2::LUCIFERASE mouse small intestine

Abstract: Disruption of circadian rhythms is a risk factor for several human gastrointestinal (GI) diseases, ranging from diarrhea to ulcers to cancer. Four-dimensional tissue culture models that faithfully mimic the circadian clock of the GI epithelium would provide an invaluable tool to understand circadian regulation of GI health and disease. We hypothesized that rhythmicity of a key circadian component, PERIOD2 (PER2), would diminish along a continuum from ex vivo intestinal organoids (epithelial ‘miniguts’), nontransformed mouse small intestinal epithelial (MSIE) cells and transformed human colorectal adenocarcinoma (Caco-2) cells. Here, we show that bioluminescent jejunal explants from PERIOD2::LUCIFERASE (PER2::LUC) mice displayed robust circadian rhythms for >72 hours post-excision. Circadian rhythms in primary or passaged PER2::LUC jejunal organoids were similarly robust; they also synchronized upon serum shock and persisted beyond 2 weeks in culture. Remarkably, unshocked organoids autonomously synchronized rhythms within 12 hours of recording. The onset of this autonomous synchronization was slowed by >2 hours in the presence of the glucocorticoid receptor antagonist RU486 (20 μM). Doubling standard concentrations of the organoid growth factors EGF, Noggin and R-spondin enhanced PER2 oscillations, whereas subtraction of these factors individually at 24 hours following serum shock produced no detectable effects on PER2 oscillations. Growth factor pulses induced modest phase delays in unshocked, but not serum-shocked, organoids. Circadian oscillations of PER2::LUC bioluminescence aligned with Per2 mRNA expression upon analysis using quantitative PCR. Concordant findings of robust circadian rhythms in bioluminescent jejunal explants and organoids provide further evidence for a peripheral clock that is intrinsic to the intestinal epithelium. The rhythmic and organotypic features of organoids should offer unprecedented advantages as a resource for elucidating the role of circadian rhythms in GI stem cell dynamics, epithelial homeostasis and disease.

Abstract: We report spectro-polarimetric results of an observational campaign of the bright neutron star low-mass X-ray binary Cyg X-2 simultaneously observed by IXPE, NICER and INTEGRAL. Consistently with previous results, the broad-band spectrum is characterized by a lower-energy component, attributed to the accretion disc with $kT_{\rm in} \approx$ 1 keV, plus unsaturated Comptonization in thermal plasma with temperature $kT_{\rm e} = 3$ keV and optical depth $\tau \approx 4$, assuming a slab geometry. We measure the polarization degree in the 2-8 keV band $P=1.8 \pm 0.3$ per cent and polarization angle $\phi = 140^{\circ} \pm 4^{\circ}$, consistent with the previous X-ray polarimetric measurements by OSO-8 as well as with the direction of the radio jet which was earlier observed from the source. While polarization of the disc spectral component is poorly constrained with the IXPE data, the Comptonized emission has a polarization degree $P =4.0 \pm 0.7$ per cent and a polarization angle aligned with the radio jet. Our results strongly favour a spreading layer at the neutron star surface as the main source of the polarization signal. Abstract: We report on the Imaging X-ray Polarimetry Explorer (IXPE) observation of the closest and X-ray brightest Compton-thick active galactic nucleus (AGN), the Circinus galaxy. We find the source to be significantly polarized in the 2--6 keV band. From previous studies, the X-ray spectrum is known to be dominated by reflection components, both neutral (torus) and ionized (ionization cones). Our analysis indicates that the polarization degree is $28 \pm 7$ per cent (at 68 per cent confidence level) for the neutral reflector, with a polarization angle of $18{\deg} \pm 5{\deg}$, roughly perpendicular to the radio jet. The polarization of the ionized reflection is unconstrained. A comparison with Monte Carlo simulations of the polarization expected from the torus shows that the neutral reflector is consistent with being an equatorial torus with a half-opening angle of 45{\deg}-55{\deg}. This is the first X-ray polarization detection in a Seyfert galaxy, demonstrating the power of X-ray polarimetry in probing the geometry of the circumnuclear regions of AGNs, and confirming the basic predictions of standard Unification Models., Comment: 9 pages, accepted for publication in MNRAS, authors list updated

Abstract: We report on the first observation of a radio-quiet Active Galactic Nucleus (AGN) using polarized X-rays: the Seyfert 1.9 galaxy MCG-05-23-16. This source was pointed with the Imaging X-ray Polarimetry Explorer (IXPE) starting on May 14, 2022 for a net observing time of 486 ks, simultaneously with XMM-Newton (58 ks) and NuSTAR (83 ks). A polarization degree smaller than $\Pi<4.7\%$ (at the 99% c.l.) is derived in the 2-8 keV energy range, where emission is dominated by the primary component ascribed to the hot corona. The broad-band spectrum, inferred from a simultaneous fit to the IXPE, NuSTAR, and XMM-Newton data, is well reproduced by a power law with photon index $\Gamma=1.85\pm0.01$ and a high-energy cutoff $E_{\rm C}=120\pm15$ keV. A comparison with Monte Carlo simulations shows that a lamp-post and a conical geometry of the corona are consistent with the observed upper limit, a slab geometry is allowed only if the inclination angle of the system is less than 50$^{\circ}$., Comment: 7 pages, 3 figures, 1 table. 33. OJ 287 as a Rotating Helix

Abstract: We present preliminary data from high-cadence 15-GHz VLBA images of OJ 287 from 1995 to 2015. The ridgelines suggest that the jet is rotating, perhaps with a period of∼30 years. The EVPA of the core rotated by 240° in 2001–2002 and decreased slowly after that. The inner jet apparently moved to a new direction after the rotation,as shown by the emergence of a new component at a new PA at 43 GHz, in 2004. This was presaged by a strong rise in the ﬂux density of the core, and then its sudden fall as the new component was identiﬁed. The equivalent sequence of events took place about 5 years later at 15 GHz, but in addition the core EVPA had a step in 2006 and moved to be aligned with the new 43-GHz component. The 15-GHz core became optically thin in 2006, but the angular resolution was insufﬁcient to separate the new component from the core until 2010

Abstract: In this report we summarize the activities of the International Astronomical Consortium for High Energy Calibration (IACHEC) and the work done since the last in-person meeting in Japan (Shonan Village Center), May 2019, through two virtual meetings that were held in November 2020 and May 2021. The on-line only meetings divided the contents of the usual in-person workshop between mission updates and working group updates. The November meeting was dedicated to mission calibration updates and the current status of the cross-calibration between NuSTAR, Swift, and NICER, which frequently join together in observations of bright transients, and a review of the XMM-Newton and Chandra cross-calibration. Results between \nustar\ and \swift\ overall show good agreement, but issues persist in the overlap region 3--5 keV for bright source with large dust scattering halos. The NICER cross-calibration is still progressing and evolving, while for the XMM-Newton and Chandra cross-calibration systematic differences both in the absolute flux and spectral shape determination still exists on different classes of sources. Mobile phones are a promising platform for improving chronic care but there are few examples of successful health system implementation. Program Development. An iterative process of program design was built upon a pilot study and engaged multiple institutional stakeholders. Patients identified having a “human face” to the pilot program as essential. Nurse administrators at a university-affiliated health plan use automated text messaging to provide personalized self-management support for member patients with diabetes and facilitate care coordination with the primary care team. For example, when a patient texts a request to meet with a dietitian, a nurse-administrator coordinates with the primary care team to provide a referral. Conclusion. Our innovative program enables the existing health system to support a de novo care management program by leveraging mobile technology. The program supports self-management and team-based care in a way that we believe engages patients yet meets the limited availability of providers and needs of health plan administrators.

Abstract: We present a method for enhancing the sensitivity of X-ray telescopic observations with imaging polarimeters, with a focus on the gas pixel detectors (GPDs) to be flown on the Imaging X-ray Polarimetry Explorer (IXPE). Our analysis determines photoelectron directions, X-ray absorption points and X-ray energies for 1-9 keV event tracks, with estimates for both the statistical and model (reconstruction) uncertainties. We use a weighted maximum likelihood combination of predictions from a deep ensemble of ResNet convolutional neural networks, trained on Monte Carlo event simulations. We define a figure of merit to compare the polarization bias-variance trade-off in track reconstruction algorithms. For power-law source spectra, our method improves on the current planned IXPE analysis (and previous deep learning approaches), providing ~45% increase in effective exposure times. For individual energies, our method produces 20-30% absolute improvements in modulation factor for simulated 100% polarized events, while keeping residual systematic modulation within 1 sigma of the finite sample minimum. Absorption point location and photon energy estimates are also significantly improved. We have validated our method with sample data from real GPD detectors., Comment: 18 pages, 9 figures. 50. Inverse-Compton scattering in the resolved jet of the high-redshift quasar PKS J1421-0643

Abstract: Despite the fact that kpc-scale inverse-Compton (iC) scattering of cosmic microwave background (CMB) photons into the X-ray band is mandated, proof of detection in resolved quasar jets is often insecure. High redshift provides favourable conditions due to the increased energy density of the CMB, and it allows constraints to be placed on the radio synchrotron-emitting electron component at high energies that are otherwise inaccessible. We present new X-ray, optical and radio results from Chandra, HST and the VLA for the core and resolved jet in the $z=3.69$ quasar PKS J1421-0643. The X-ray jet extends for about $4.5''$ (32 kpc projected length). The jet's radio spectrum is abnormally steep and consistent with electrons being accelerated to a maximum Lorentz factor of about 5000. Results argue in favour of the detection of inverse-Compton X-rays for modest magnetic field strength of a few nT, Doppler factor of about 4, and viewing angle of about $15^\circ$, and predict the jet to be largely invisible in most other spectral bands including the far- and mid-infrared and high-energy gamma-ray. The jet power is estimated to be about $3 \times 10^{46}$ erg s$^{-1}$ which is of order a tenth of the quasar bolometric power, for an electron--positron jet. The jet radiative power is only about 0.07 per cent of the jet power, with a smaller radiated power ratio if the jet contains heavy particles, so most of the jet power is available for heating the intergalactic medium., Comment: 16 pages, 14 figures.
