Despite the common association of NF1 with neurocutaneous features, its pathology can extend to numerous tissues not derived from the neural crest. Among the rare cerebrovascular abnormalities in NF1, more than 85% of cases are of purely occlusive or stenotic nature, with intracranial aneurysm being uncommon. Predominantly, the aneurysms are located in the internal carotid arteries (ICAs), being very rare bilateral aneurysms. This report describes a very unusual case of fusiform aneurysms of both ICAs in a Caucasian NF1 patient, with a new pathogenic intragenic heterozygous deletion of the NF1 gene, presenting at age 22 years with Tolosa–Hunt syndrome, because of partial thrombosis of the left giant intracavernous aneurysm. Medical treatment with anticoagulant therapy allowed a good outcome for the patient. In conclusion, early identification of cerebral arteriopathy in NF1 and close follow-up of its progression by neuroimaging may lead to early medical or surgical intervention and prevention of significant neurologic complications.Keywords: neurofibromatosis type 1, NF1 gene, multiplex ligation-dependent probe amplification (MLPA), intracranial aneurysms, Tolosa–Hunt syndrome

- Abstract
Scopo del lavoro: Identificazione di malformazione vascolare in paziente con NF1, valutazione prognostica e follow up dopo trattamento. Materiali e Metodi: Una giovane di 22 aa giunge alla nostra osservazione per dolore periorbitale sinistro,ptosi palpebrale omolaterale e diplopia orizzontale. All’esame neuro-oftalmologico si riscontra exotropia dello sguardo primario all’ occhio sinistro e paresi del III, IV e VI paio dei nervi cranici dello stesso lato. Si pone ipotesi diagnostica di sindrome di Tolosa-Hunt. Si eseguono varie analisi ematochimiche comprese quelle autoanticorpali che risultano tutte negative. Si invia il paziente ad un esame RM encefalo che documenta una già nota lesione ovalare a carico di ambedue i tratti intracavernosi delle arterie carotide interne di natura aneurismatica, caratterizzate allo stato attuale dalla presenza di materiale ematico in diverse fasi di degradazione, come da pregressa trombosi. L’integrazione con sequenze Angio RM TOF e le successive ricostruzioni MIP confermano i reperti. Si nota un aneurisma gigante fusiforme dell’ACI sinistra (diametro max della sacca: 25 mm) ed uno più piccolo a carico dell’ACI destra (diametro max della sacca: 12 mm). Infine si eseguono un Angio TC con ricostruzioni SSD ed un’angiografia digitale selettiva dell’ICA di sinistra che documentano senza alcun dubbio la trombosi parziale dell’aneurisma gigante. Le analisi genetiche hanno evidenziato una mutazione genetica di delezione genica in condizione di eterozigosi che riguarda gli esoni adiacenti 12(10a) e 13(10b) del gene NF1. Ciò comporta un rischio aumentato di complicanze cerebrovascolari soprattutto nella terza decade di età, epoca di comparsa della sintomatologia della nostra paziente. Risultati: La giovane paziente è affetta da STH secondaria e si sceglie di praticare una terapia farmacologica con anticoagulanti ( per i primi 5 gg eparina non frazionata sottocute e poi warfarin per os ). Alla decima settimana si è risolta completamente la sintomatologia ed attualmente la ragazza non riporta alcuna sequela neurologica. Una stretta valutazione clinica e strumentale durante il followup ci ha permesso giungere rapidamente alla diagnosi e ad un adeguato trattamento che ha portato ad una rapida risoluzione dei sintomi potenzialmente invalidanti. Conclusioni: La malattia cerebrovascolare nella NF1 viene spesso sottostimata e questo influisce negativamente sulla conseguente morbidità e mortalità. Le attuali linee guida, infatti, non sottolineano abbastanza questa complicanza, a dispetto dell’ultimo consensus dell’ AHA sulla prevenzione dell’ictus nell’infanzia. La ricerca di particolari mutazioni genetiche che aumentano il rischio di malformazione vascolare potrebbe essere la base sulla quale sviluppare uno screening neuroradiologico più stretto

- Abstract
Questo capitolo presenta la sintesi di uno studio a carattere multidisciplinare, condotto dai membri di numerose unità operative afferenti al progetto “GEOBASI” su campioni di suolo del sito “pilota” di Muravera, ai fini di cogliere le dinamiche interne del suolo e quelle a scala territoriale, e fornire un contributo all’individuazione di appropriati strumenti di indagine e di intervento. I risultati, di seguito riportati, sono scaturiti dalle varie analisi di laboratorio, condotte con un approccio metodologico integrato e multiscala su campioni appartenenti a ciascuno dei diversi orizzonti genetici di cui si compone ogni profilo pedologico ed, ove possibile, anche del substrato roccioso. In tal modo si è cercato di garantire, a partire dal campionamento, un rigore di metodi in grado di tenere in dovuto conto la specificità delle proprietà e dei meccanismi di cui il suolo è sede. La caratterizzazione della composizione chimica degli elementi totali, sia maggiori sia in tracce, tramite spettrometria di fluorescenza ai raggi X è stata curata dalla UO di Siena (Protano G., Nannoni F., Pizzetti E., Riccobono F., Rossi S.), con un contributo specifico dell’UO afferente all’ENEA per quanto riguarda l’arsenico. I risultati ottenuti mediante XRF sono stati messi in relazione con i principali litotipi presenti nell’area di studio e con i diversi orizzonti pedologici. Sono stati inoltre confrontati con quelli derivanti dall’estrazione degli elementi pseudototali in acqua regia da parte dell’UO di Pavia (Sacchi E., Garzetti F., Fornelli Genot S., Vannucci R.), che discute la validità di applicabilità dei due diversi metodi, mettendo altresì in luce le difficoltà di applicazione della normativa vigente in materia di determinazione delle soglie di contaminazione di diverse matrici ambientali. I contributi successivi del presente capitolo analizzano attraverso molteplici sfaccettature ed approcci analitici il variegato tema della mobilità degli elementi nel sistema suolo. Si passa dall’analisi della frazione biodisponibile dei metalli tramite estrazione in EDTA al confronto con i tenori pseudototali (Sacchi E., Fornelli Genot S., Garzetti F., Vannucci R. dell’UO di Pavia) ed allo studio dei processi di mobilità, frazionamento e speciazione attraverso le estrazioni sequenziali (UO dell’ENEA: Angelone M., Armiento G., Caci E., Carucci V., Cremisini C., Crovato C.), attraverso procedure sempre applicate ancora al campione totale. Attraverso tecniche d’indagine micromorfologica e microanalitica in microscopia ottica ed elettronica e l’ausilio di analisi spettrometriche con l’utilizzo di sorgente laser si evidenziano alcuni meccanismi di ritenzione o mobilizzazione degli elementi con carattere “puntuale” all’interno di determinati profili e/o orizzonti pedologici (Scarciglia F., De Rosa R., Barca D., Apollaro C., Caruso A.M., Vecchio G. dell’UO di Cosenza e Vacca A. dell’unità di Cagliari). Vengono poi proposti un’analisi della distribuzione spaziale di alcuni isotopi radioattivi nonché l’applicazione di un metodo per stimare i tassi di erosione o accumulo sulla base dell’attività del 137Cs (UO di Roma Tre: Tuccimei P., Salzano R., Soligo M., Primavera P., Delitala M.C., Taddeucci A.) ed uno studio di dettaglio sul rapporto isotopico del ferro con la proposta di una metodologia innovativa (Castorina F., Masi U., Voltaggio M. dell’UO di Roma “La Sapienza”). Questo lavoro mette in luce le difficoltà di applicazione della normativa vigente, qualora si proceda alla misura ed alla discussione delle concentrazioni e distribuzioni degli elementi nei suoli dell’area rappresentata nel Foglio 549 Muravera della Carta Geologica d’Italia a scala 1:50.000. L’approccio multidisciplinare adottato mostra in modo chiaro i limiti dei metodi d’indagine sinora proposti dalla legislazione ambientale ed applicati dagli operatori del territorio: i limiti, cioè, sia di un campionamento del solo orizzonte di superficie, senza tener conto invece dei differenti orizzonti genetici (ovvero prodotti da specifici processi di pedogenesi) di cui consta un profilo di suolo, sia della sola analisi degli elementi totali, a scapito della valutazione dei contributi offerti dalle diverse sub-“matrici” costituenti ogni singolo orizzonte (scheletro minerale, matrice pedogenetica s.s., pellicole di argilla illuviale, sostanza organica, segregazioni di ossidi di ferro, ecc.).

OBJECTIVE: The use of gonadotropin-releasing hormone agonist for ovulation triggering has become an intriguing topic in the last few years. As long as adequate luteal phase support is provided, it may be a valuable alternative to standard hCG triggering, associated with a significant reduction in OHSS incidence. Several luteal phase support options have been proposed, but few studies have addressed the issue of the appropriate route for progesterone administration to women triggered with GnRHa. The aim of the study was to evaluate the effect of GnRHa triggering on IVF/ICSI outcomes, using modified luteal phase support with intramuscular progesterone. PATIENTS AND METHODS: A retrospective study was carried out between January 2014 and December 2015, comparing the reproductive outcome in GnRHa triggered women given modified luteal phase support with intramuscular progesterone (Group A) with the outcome in women triggered with standard hCG (Group B) in IVF/ICSI cycles. RESULTS: 200 (Group A n = 100; Group B n = 100) consecutive normoresponder women were included. No differences with respect to Age, BMI, basal FSH, basal Estradiol and infertility diagnosis were observed between groups. Increased numbers of retrieved oocytes (8.1 ± 3.3 versus 6.8 ± 3.5, p = 0.009) and mature oocytes (5.8 ± 2.6 versus 5.1 ± 2.7, p = 0.03) were detected in Group A compared with Group B. Implantation, biochemical pregnancy and ongoing pregnancy rates were similar. CONCLUSIONS: Our findings confirmed that the GnRHa triggering strategy is associated with increased number of oocytes retrieved and of mature oocytes even in normoresponder women. Moreover, in these patients, the use of intramuscular progesterone during luteal phase support achieved satisfactory IVF outcomes. [ABSTRACT FROM AUTHOR]

- Abstract
OBJECTIVE: To determine if the overcoming of the cervical mucus barrier removes the interference of sperm-bound antibodies with fertility. DESIGN: Prospective case series. SETTINGS: University-based intrauterine insemination (IUI) homologous program. PATIENTS: Nineteen patients with all spermatozoa in the ejaculate coated by antisperm antibodies. As control group, 86 consecutive patients without antisperm antibodies, treated for oligoasthenozoospermia or mucus hostility. INTERVENTIONS: Intrauterine inseminations (at least 3 attempts per couple). MAIN OUTCOME MEASURES: The outcome of IUIs, demographic, and seminal parameters were compared between the two groups. RESULTS: No pregnancy occurred in the couples with male immunological infertility, treated by 110 IUIs. Twenty-three pregnancies occurred in 22 (25.6%) of the control group couples who were treated by 411 IUIs. In the group of patients without antisperm antibodies, we demonstrated that the pregnancy rate (PR)/couple in oligoasthenozoospermia without teratozoospermia was similar to that achieved in normozoospermia (35% versus 38.9%), whereas it was significantly affected by teratozoospermia (3.6%). Only three patients with antisperm antibodies had teratozoospermia. Comparing the PR per couple and per cycle between the two groups of patients (with and without antisperm antibodies), excluding the patients with teratozoospermia, significant differences resulted (P less than 0.005 and P less than 0.005, respectively). The motile sperm count was not significantly different between the two groups, which also resulted to be homogeneous for demographic data. Moreover, the motile sperm count was not different between the patients with and without antisperm antibodies, who had successful IUI. CONCLUSIONS: The analysis of this trial suggests that the failure of IUI in the treatment of male immunological infertility is imputable to antisperm antibodies when they involve all spermatozoa, regardless of semen quality

- Abstract
Paget's disease (PD) is a common focal progressive osteometabolic disorder characterised by a disturbance in bone modelling and remodelling, because of an increase in osteoblastic and osteoclastic activity. It is a condition of unknown aetiology affecting approximately 3% of the population over 40 years of age and, approximately 10% of those over the age of 85 years. It is most common in Northern Europe and Australia and is rare in Asia and Africa. We describe the case of a 71-year-old man diagnosed with PD confined to the sacrum. After laboratory test, an imaging study with radiography, scintigraphy, computed tomography (CT) and magnetic resonance imaging (MRI) was performed disclosing findings compatible with Paget's disease in middle pathologic phase. The diagnosis was confirmed at biopsy. The structural modification of the sacrum with spongiosa rarefaction, thickening of bone and intact bone cortical, confirmed by CT, are typical of an intermediate phase of PD. This was also supported by signal MRI changes showing substitution of the red by the fat medulla, visualized by FS sequences. Once the treatment for the bone disease was established, the patient no longer complained of pain. Special attention should be paid to male and elderly patients with pain in the lumbar spine because of the potential risk cancer development 21. The radiologist must be attentive to the possible presentations and complications of PD, even in uncommon sites, trying whenever possible to correlate the radiological features with the patient's clinical symptoms. CONCLUSIONS: The RetroX outer ear hearing aid seems to represent a means of overcoming problems with understanding speech in noise in patients with high-frequency sensorineural hearing loss (SNHL) without the need to wear conventional completely-in-the-canal (CIC) hearing aids, which are usually reported to annoy patients as a result of the occlusion effect. OBJECTIVE: To present preliminary data from a study carried out to compare the efficacy, in the same individual, of a standard digital CIC hearing aid and a new implantable outer ear canal device, the RetroX. MATERIAL AND METHODS: Three out of 15 adults affected by high-frequency SNHL who were candidates for auditory rehabilitation were evaluated by using speech audiometric tests in quiet and noise as well as a questionnaire shortly after use of a CIC hearing aid and the RetroX device, i.e. at 7 and 14 days. The efficacy of the RetroX was anticipated by testing all the subjects using a RetroX simulating system before starting the study protocol. RESULTS: In all three implanted patients, the RetroX provided better audiological benefit for speech understanding in noise. These findings were corroborated by the results of the questionnaire, which showed greater satisfaction with the RetroX, especially regarding the absence of the occlusion effect. The use of gonadotropin-releasing hormone agonist for ovulation triggering has become an intriguing topic in the last few years. As long as adequate luteal phase support is provided, it may be a valuable alternative to standard hCG triggering, associated with a significant reduction in OHSS incidence. Several luteal phase support options have been proposed, but few studies have addressed the issue of the appropriate route for progesterone administration to women triggered with GnRHa. The aim of the study was to evaluate the effect of GnRHa triggering on IVF/ICSI outcomes, using modified luteal phase support with intramuscular progesterone.

Purpose Endometriosis is a chronic debilitating inflammatory pathology which interests females in their reproductive age. Its pathogenesis has not yet been clearly defined. Recent evidence linked chemical agents as endocrine-disrupting chemicals to endometriosis. Phthalates are a widely used class of such compounds. This study aimed to summarize the current literature evaluating the link between exposure to phthalates and occurrence of endometriosis. Methods A systematic review of literature and meta-analysis has been carried out following PRISMA guidelines to assess such link. Fourteen studies have been included in the review. Risk of bias has been assessed through the Newcastle Ottawa Scale. Results We observed association between endometriosis and increased urinary levels of MBP/MnBP, MEOHP, and MEHHP, but not for others. Blood-derived analysis showed statistically significant link between endometriosis and BBP, DEHP, DnBP, and MEHP. Conclusion Given the wide heterogeneity of included studies, results should be taken with caution. Further studies with more rigorous methodology are encouraged to unravel the true link between this class of toxic compounds and manifestation of endometriosis.

Highlights • No data are available to the distress of infertile couples during COVID-19. • Stop performing ART treatments during COVID-19 contributed to increase anxiety. • The psychological impact was more severe for women than for men. • Psychological impact of COVID-19 in infertility couple should not be underestimated., Objective(s) to evaluate the impact of the COVID-19 pandemic on infertile couples’ emotions, anxiety and future plans. Study Design An observational study was perfomed by Italian ART centers and online forums. In this study, infertile couples candidate to ART and whose treatment was blocked due to the COVID-19 lockdown were enrolled through an online survey. The psychological impact of COVID-19 was measured by Impact of Event Scale-Revised (IES-R) and by a short form of the Spielberger State-Trait Anxiety Inventory (STAI); Self-perceived anxiety related either to pregnancy safety and to economic crisis measured by VAS scale. Results 627 patients completed the survey. The COVID-19 lock-down had a moderate/severe psychological impact on infertile patients (mean IES-R score 36.4 ± 16.6). The mean STAI score was 49.8 ± 15.3, with an overall incidence of STAI > 36 of 71%. The mean VAS scale for anxiety perception was 45.3 ± 15.3. Women were more emotionally distressed, anxious and depressed than men (36.8 ± 16.4 vs 31.0 ± 18.4 for IES-R, respectively; p = 0.03). Notwithstanding the uncertainty about pregnancy safety, 64.6% of respondents chose to maintain their reproductive programme. Economic crisis induced 11.5% of the surveyed patients to give up their ART program. Respondents who had at least one relative affected by COVID-19 had a significantly higher IES-R score and anxiety VAS, but not higher STAI scores, than patients belonging to unaffected families. Conclusion(s) COVID-19 pandemic itself and the recommendation to stop ART program generated higher distress levels in infertile couples. The psychological impact of COVID-19 pandemic in infertility patients should not be underestimated, and a specific psychological support should be planned.

Preimplantation genetic testing (PGT) is used to screen for genetic diseases or chromosomal abnormalities in couples at risk of transmission to the offspring. It can be applied also to determine the gender or phenotypical features of the unborn and to type the Human Leucocyte Antigen (HLA), in order to search for future potential stem cell or bone marrow donors. Aim of this review is to provide a comprehensive description of Italian jurisprudential approach on PGT, and in addition, to compare the jurisprudential orientation of other nations of West Europe and USA. PGT is allowed for almost any indication in some countries (United Kingdom and USA), but is forbidden in others (Austria). In the majority of countries PGT is authorized to prevent the transmission of genetic diseases, being considered unacceptable for gender selection for non-medical reasons. Less frequently, PGT is allowed to provide therapeutic options for an already born sibling. In some cases, prior authorization from a health authority or a healthcare provider is requested. Social, healthcare and ethical issues underpin the request for PGT. Given the difficulty to identify a list of genetic diseases for which could be authorized the access to PGT, the European Society of Human Reproduction and Embriology (ESHRE) proposes to limit such test to cases characterized by a “high risk of a serious condition”. In such a framework, it has been proposed that the access to PGT is allowed through a prior in-depth meeting between the couple and a multidisciplinary equipe of healthcare professionals (obstetrician, geneticist, psychologist, expert in bioethics, legal-medicine specialist) to evaluate the medical, but also juridical and ethical acceptability of the request.

Neurofibromatosis type 1 (NF1) is a relatively common single-gene disorder, and is caused by heterozygous mutations in the NF1 gene that result in a loss of activity or in a nonfunctional neurofibromin protein. Despite the common association of NF1 with neurocutaneous features, its pathology can extend to numerous tissues not derived from the neural crest. Among the rare cerebrovascular abnormalities in NF1, more than 85% of cases are of purely occlusive or stenotic nature, with intracranial aneurysm being uncommon. Predominantly, the aneurysms are located in the internal carotid arteries (ICAs), being very rare bilateral aneurysms. This report describes a very unusual case of fusiform aneurysms of both ICAs in a Caucasian NF1 patient, with a new pathogenic intragenic heterozygous deletion of the NF1 gene, presenting at age 22 years with Tolosa–Hunt syndrome, because of partial thrombosis of the left giant intracavernous aneurysm. Medical treatment with anticoagulant therapy allowed a good outcome for the patient. In conclusion, early identification of cerebral arteriopathy in NF1 and close follow-up of its progression by neuroimaging may lead to early medical or surgical intervention and prevention of significant neurologic complications.

OBJECTIVE: To determine if the overcoming of the cervical mucus barrier removes the interference of sperm-bound antibodies with fertility. DESIGN: Prospective case series. SETTINGS: University-based intrauterine insemination (IUI) homologous program. PATIENTS: Nineteen patients with all spermatozoa in the ejaculate coated by antisperm antibodies. As control group, 86 consecutive patients without antisperm antibodies, treated for oligoasthenozoospermia or mucus hostility. INTERVENTIONS: Intrauterine inseminations (at least 3 attempts per couple). MAIN OUTCOME MEASURES: The outcome of IUIs, demographic, and seminal parameters were compared between the two groups. RESULTS: No pregnancy occurred in the couples with male immunological infertility, treated by 110 IUIs. Twenty-three pregnancies occurred in 22 (25.6%) of the control group couples who were treated by 411 IUIs. In the group of patients without antisperm antibodies, we demonstrated that the pregnancy rate (PR)/couple in oligoasthenozoospermia without teratozoospermia was similar to that achieved in normozoospermia (35% versus 38.9%), whereas it was significantly affected by teratozoospermia (3.6%). Only three patients with antisperm antibodies had teratozoospermia. Comparing the PR per couple and per cycle between the two groups of patients (with and without antisperm antibodies), excluding the patients with teratozoospermia, significant differences resulted (P less than 0.005 and P less than 0.005, respectively). The motile sperm count was not significantly different between the two groups, which also resulted to be homogeneous for demographic data. Moreover, the motile sperm count was not different between the patients with and without antisperm antibodies, who had successful IUI. CONCLUSIONS: The analysis of this trial suggests that the failure of IUI in the treatment of male immunological infertility is imputable to antisperm antibodies when they involve all spermatozoa, regardless of semen quality

Our patient presented unstable blood hypertension and the pain of typical trigeminal neuralgia over the second and third divisions of the nerve in the right side of the face. The attacks were often precipitated during physical exertion. MRI and Angio-MRI revealed the persistent carotid basilar anastomosis and occasionally left parietal cavernoma. After drug treatment of blood hypertension, spontaneous recovery of neuralgia was observed and we planned surgical treatment of left temporal cavernoma. © The Author(s) 2011. - Abstract
A multispecialty trauma team must provide care for pelvic gunshot wounds (PGW) due to the high risk of associated morbidity and mortality, the high density of organs that might be wounded within the pelvis, and the potential consequences of these complicated injuries. We present a case of a 59-year-old woman hemodynamically stable with firearm injury to the left buttock. CT examination showed free air in the peritoneal cavity and in the retroperitoneum and a focal contrast extravasation within the uterine fundus. The patient underwent urgent laparotomy that revealed triple bowel perforation (sigmoid colon, medium rectum, ileum) and a laceration of the posterior and anterior uterine wall at level of the cervix with no signs of active bleeding. The bullet was lodged above the peritoneal reflection, in the right pelvis, and it was removed, and handed over to the judicial authority. The perforated bowel segments were resected with Hartmann's procedure and ileal anastomosis. The uterine laceration was repaired. Although all the viscera and the structures along the trajectory can be harmed, pelvic gunshot wounds have the potential to inflict serious injury. Nongravid uterine traumas are a unique occurrence, and proper care requires an understanding of lesion grading. Finding the gynecological lesion in female patients is essential to receiving the best care and protecting the reproductive system., (© 2024 The Authors. Objective: To study hemodynamic changes along controlled ovarian stimulation in women undergoing in vitro fertilization., Study Design: Prospective observational cohort study conducted at Mother and Child Department of University Hospital Federico II, in Naples, Italy, between April 2021 and July 2022. Sixty-eight infertile patients undergoing controlled ovarian stimulation with gonadotropin, antagonist protocol and a fresh embryo transfer were included. Haemodynamic assessment was carried out using UltraSonic Cardiac Output Monitor at baseline (T1), estradiol peak (T2), fresh embryo-transfer day (T3). To evaluate relationships between quantitative variables and groups a Student T test for independent data was assessed. One-way analysis of variance (ANOVA) was used to determine the differences between the means of three time points (T1, T2 and T3) for quantitative variables. A mixed-model analysis of variance (ANOVA) was used to determine the differences between groups, among time points (T1, T2 and T3)., Results: Sixty-eight patients were included. Significant differences over the three time points have been observed for CO (f = 3.78 l/min; p = 0.025), SVI (f = 3.56 ml/m
2 ;p = 0.013), and RSVI (f = 4.84 dscm-5 m2 ; p = 0.009). No significant differences in trends have been found between beta hCG positive and beta hCG negative groups. There were no significant differences in maternal hemodynamic parameters at time-point T3 between patients treated with hCG 10,000 UI and with Triptorelin. Research Question: Is ovarian stimulation with levonorgestrel intrauterine system (LNG-IUS) in situ and co-treatment with letrozole safe and effective in patients undergoing fertility-sparing combined treatment for atypical endometrial hyperplasia (AEH) or early endometrial cancer limited to the endometrium?, Design: Retrospective case-control study recruiting women who had undergone fertility-sparing 'combined' treatment and ovarian stimulation with letrozole and LNG-IUS in situ. The 'three steps' hysteroscopic technique was used. Once complete response was achieved, the ovaries were stimulated, and mature oocytes cryopreserved. The LNG-IUS was removed, and embryos transferred. A comparative analysis was conducted between the two control groups of the initial outcomes of ART (number of oocytes and MII oocytes retrieved): healthy infertile women undergoing ovarian stimulation for IVF/ICSI (control group A); and patients diagnosed with breast cancer who underwent ovarian stimulation with letrozole (control group B)., Results: Of the 75 patients analysed, 15 underwent oocyte cryopreservation after achieving a complete response to fertility-sparing treatment (study group); 30 patients in control group A and B, respectively. No statistically significant differences were observed in retrieved oocytes and mature oocytes between the study and control groups. In the nine patients who underwent embryo transfer, clinical pregnancy (55.6%), cumulative live birth (44.4%) and miscarriage (20%) rates were reported. In three patients with AEH, recurrence occurred (12%) at 3, 6 and 16 months after removing the LNG-IUS to attempt embryo transfer, respectively., Conclusion: Fertility-sparing hysteroscopic combined treatment and subsequent ovarian stimulation with letrozole and LNG-IUS in situ could be suggested to women with AEH or early endometrial cancer who ask for future fertility preservation., (Copyright © 2023 Reproductive Healthcare Ltd. - Abstract
Semaphorin 3A (SEMA3A) plays a crucial role in the development, differentiation, and plasticity of specific types of neurons that secrete Gonadotropin-Releasing Hormone (GnRH) and regulates the acquisition and maintenance of reproductive competence in humans and mice. Its insufficient expression has been linked to reproductive disorders in humans, which are characterized by reduced or failed sexual competence. Various mutations, polymorphisms, and alternatively spliced variants of SEMA3A have been associated with infertility. One of the common causes of infertility in women of reproductive age is diminished ovarian reserve (DOR), characterized by a reduced ovarian follicular pool. Despite its clinical significance, there are no universally accepted diagnostic criteria or therapeutic interventions for DOR. In this study, we analyzed the SEMA3A plasma levels in 77 women and investigated their potential role in influencing fertility in patients with DOR. The results revealed that the SEMA3A levels were significantly higher in patients with DOR than in healthy volunteers. Furthermore, the SEMA3A levels were increased in patients who underwent fertility treatment and had positive Beta-Human Chorionic Gonadotropin (βHCG) values (β+) after controlled ovarian stimulation (COS) compared to those who had negative βHCG values (β-). These findings may serve as the basis for future investigations into the diagnosis of infertility and emphasize new possibilities for the SEMA3A-related treatment of sexual hormonal dysfunction that leads to infertility.

Introduction: It is well recognized that the human uterus and adjoining tissues of the female reproductive tract exist in a non-sterile state where dysbiosis can impact reproductive outcomes. The endometrial microbiota is a part of this greater milieu. To date, it has largely been studied using 16S rRNA or metagenomics-based methodologies. Despite the known advantages of sequencing analysis, several difficulties have been noted including sample contamination and standardization of DNA extraction or sequencing. The aim of this study was to use a culturomics-based method to analyze the endometrial microbiota and correlate the results with ongoing pregnancy rates., Methods: A prospective cohort study was performed at the University of Naples from June 2022 to December 2022. Ninety-three patients undergoing an IVF cycle with single embryo transfer (ET) (fresh or frozen) were enrolled in the study. Following ET, the catheter tip was inserted into brain heart infusion (BHI) medium under sterile conditions for culture. After 24h and 48h of incubation the microorganisms in the colonies were identified by matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS)., Results: Overall, 68 (73,92%) patients resulted positive for one or more microbes and 25 patients (26,08%) had no microbial growth. Across all participants, the four most important phyla were Firmicutes (87,76%), Proteobacteria (27,94%), Actinobacteria (10,29%) and Ascomycota (8,82%). Lactobacillus species, in particular, was significantly correlated with ongoing pregnancy rate (p=0,05). On the other hand, Staphylococcus subspecies (spp.) (p<0,05) and Enterobacteriaceae (p<0,001) were found to have a negative impact on the implantation rate., Discussion: Detection of bacteria by culturomics from catheter tips used for embryo transfer has been shown to be a reliable method to detect pathogen growth.
