We tested this assumption by conducting a survey among people living in a least developed country compared with those living in a developed country.Participants and methods: Twenty-seven Belgians living in Belgium, 29 Burundians living in Belgium, and 32 Burundians living in Burundi were included in this study. Their attitudes toward older adults were assessed using several self-reported measures.Results: Statistical analyses confirmed that older people are more negatively perceived by Burundians living in Burundi than by Burundians and Belgians living in Belgium, whose attitudes did not differ from each other.Conclusion: Consistent with our hypothesis, our results suggest that the level of development of a country and more particularly the lack of government spending on older people (pension and health care systems) may contribute to their younger counterparts perceiving them more negatively. Abstract

The International Virus Bioinformatics Meeting 2020 was originally planned to take place in Bern, Switzerland, in March 2020. However, the COVID-19 pandemic put a spoke in the wheel of almost all conferences to be held in 2020. After moving the conference to 8-9 October 2020, we got hit by the second wave and finally decided at short notice to go fully online. On the other hand, the pandemic has made us even more aware of the importance of accelerating research in viral bioinformatics. Advances in bioinformatics have led to improved approaches to investigate viral infections and outbreaks. The International Virus Bioinformatics Meeting 2020 has attracted approximately 120 experts in virology and bioinformatics from all over the world to join the two-day virtual meeting. Despite concerns being raised that virtual meetings lack possibilities for face-to-face discussion, the participants from this small community created a highly interactive scientific environment, engaging in lively and inspiring discussions and suggesting new research directions and questions. Abstract

Superfluid helium is increasingly used as a technical cooling medium in large-scale research projects involving high-field superconducting magnets, such as the CERN Large Hadron Collider (LHC). For refrigeration cycles operating below the normal boiling point of helium, a potentially interesting alternative to vapour compression is magnetic refrigeration, pioneered decades ago. In the framework of development for an industrial-size 1.8 K magnetic refrigerator using a static active material, we have designed, built and tested a pulsed superconducting magnet producing up to 3 T at a ramping rate of 6.5 T/s. The solenoid-type coil, wound with low-loss superconducting wire (small-filament NbTi in a composite Cu/Cu-Ni matrix), is internally cooled by normal boiling helium, and installed inside a toroidal yoke, made of tapered laminations of commercially pure iron. After describing design and construction of the magnet, we present test results and measurements of performance, field quality and pulsed-operation losses.

La vision actuelle du vieillissement est principalement négative, teintée du phénomène d’âgisme (c’est-à-dire toutes les formes de discrimination, de ségrégation ou de mépris fondées sur l’âge). Une telle image de la vieillesse n’est pas anodine. Dans cet article, nous passerons en revue les manifestations de l’âgisme dans notre vie quotidienne, ses origines, ses implications sur nos attitudes, de même que les conséquences qui en résultent pour les personnes âgées. Nous discuterons également de certains enjeux cliniques et éthiques en lien avec ces attitudes âgistes.

Abstract

Background Beta-blockers, used as an adjunctive to diuretics, digoxin and angiotensin converting enzyme inhibitors, improve survival in chronic heart failure. We report a prospectively planned economic analysis of the cost of adjunctive beta-blocker therapy in the second Cardiac Insufficiency BIsoprolol Study (CIBIS II). Methods Resource utilization data (drug therapy, number of hospital admissions, length of hospital stay, ward type) were collected prospectively in all patients in CIBIS . These data were used to determine the additional direct costs incurred, and savings made, with bisoprolol therapy. As well as the cost of the drug, additional costs related to bisoprolol therapy were added to cover the supervision of treatment initiation and titration (four outpatient clinic/office visits). Per them (hospital bed day) costings were carried out for France, Germany and the U.K. Diagnosis related group costings were performed for France and the U.K. Our analyses took the perspective of a third party payer in France and Germany and the National Health Service in the U.K. Results Overall, fewer patients were hospitalized in the bisoprolol group, there were fewer hospital admissions perpatient hospitalized, fewer hospital admissions overall, fewer days spent in hospital and fewer days spent in the most expensive type of ward. As a consequence the cost of care in the bisoprolol group was 5-10% less in all three countries, in the per them analysis, even taking into account the cost of bisoprolol and the extra initiation/up-titration visits. The cost per patient treated in the placebo and bisoprolol groups was FF35 009 vs FF31 762 in France, DM11 563 vs DM10 784 in Germany and pound 4987 vs pound 4722 in the U.K. The diagnosis related group analysis gave similar results. Interpretation Not only did bisoprolol increase survival and reduce hospital admissions in CIBIS II, it also cut the cost of care in so doing. This `win-win' situation of positive health benefits associated with cost savings is Favourable from the point of view of both the patient and health care systems. These findings add further support for the use of beta-blockers in chronic heart failure.

Abstract

This was a longitudinal study of 3167 active or retired pneumoconiotic coalminers from Nord-Pas de Calais collieries (HBNPC, acronym in French) recognized through medico-legal decisions over three delined periods: 1942-1961, 1962-1981 and 1982-1987. Periodic medical examinations of pneumoconiotics are provided by the medical services of HBNPC for miners and exminers. Physicians involved in this follow-up programme are concerned with the high incidence of progressive massive fibrosis (PMF). The objectives of this study were to describe pneumoconiosis at time of compensation and examine factors related to the occurrence of PMF. For each subject, two radiological films were extracted from the medical files. They were reinterpreted according to the ILO classification (ILO, 1980) by three independent medical readers. Results indicated that radiological criteria for compensation had changed over time ; a marked predominice of category 212 small opacities was noted for cases compensated before 1962 and a predominice of category 1/1 among those compensated after 1981. Comparison of distribution of small opacity types in the 1942-1961 period with that in 1982-1987 confirmed the progressive fall, reported by other investigators, of proportions of predominant micronodular opacities accompanied by an increase in proportions of small irregular opacities at compensation [COCKCROFT and ANDERSON, Br. J. ind. Med. 44,484-487 (1987)]. When time interval between films was taken into account, profusion of small opacities at compensation greater than 1/1 was associated with a probability of 40% or more of developing PMF. Severity of coalworkers simple pneumoconiosis (CWSP) at compensation in HBNPC has been seen to decline since 1982 and the disease to occur mostly after retirement. The reported changes in small opacity types suggest a reduction in incidence of pure silicosis. Occurrence of PMF was correlated with profusion of small opacities at compensation.

Abstract

Hirudin synthesized by the medicinal leech Hirudo medicinalis is the most potent antithrombic factor found in nature. We have used Saccharomyces cerevisiae for the efficient production of recombinant hirudin variant 2 (rHV2). The mature hirudin coding sequence was placed in frame with the prepro sequence of the yeast MFα1 gene. Multicopy plasmids carrying different fused genes transcribed from the MFα1 promoter were used to transform a yeast strain selecting for uracil prototrophy. Analysis of proteins from culture supernatants revealed efficient secretion of hirudin in all cases. However, only precursors in which the HV2 mature sequence directly followed the cleavage site for yscF (a protease involved in the maturation of yeast α factor) were processed correctly to yield biologically active hirudin.

46. Hypospadias Associated With Fetal Growth Restriction: A Multicentric Descriptive and Prognostic Cohort Study.

Abstract

Objective: To determine the prevalence of genetic and endocrine abnormalities and to assess fetal, neonatal and surgical outcomes in cases of hypospadias associated with fetal growth restriction., Method: A multicentric retrospective study was conducted across five prenatal diagnosis centers in Paris. The cohort encompassed all fetuses diagnosed with the combination of fetal growth restriction < 10th percentile (FGR) and hypospadias from 2013 to 2021. Maternal data, fetal outcome and results of prenatal investigations were collected, along with postnatal data, encompassing endocrinological and genetic assessments, functional aspects and surgical outcomes., Results: Among the 82 patients included in the cohort, there were 14 (17%) terminations of pregnancy and four (5%) in utero deaths, leaving 64 (78%) live neonates, including five (6%) with early neonatal death. Among the 52 (63%) cases where hypospadias and FGR were considered as ultrasound-isolated anomalies, six (12%, [3.2%-20.8%]) exhibited chromosomic, genetic, or endocrinological abnormalities diagnosed half prenatally and half postnatally. Fifty percent of the overall hypospadias were proximal. Most children underwent surgical intervention before reaching 2 years of age, with 50% encountering complications and often required reintervention., Conclusion: The association of FGR and hypospadias should not be underestimated as genetic or endocrinological abnormalities were identified even when hypospadias and FGR initially appear isolated. 47. Accurate bacterial outbreak tracing with Oxford Nanopore sequencing and reduction of methylation-induced errors.

Abstract

Our study investigates the effectiveness of Oxford Nanopore Technologies for accurate outbreak tracing by resequencing 33 isolates of a 3-year-long Klebsiella pneumoniae outbreak with Illumina short-read sequencing data as the point of reference. We detect considerable base errors through cgMLST and phylogenetic analysis of genomes sequenced with Oxford Nanopore Technologies, leading to the false exclusion of some outbreak-related strains from the outbreak cluster. Nearby methylation sites cause these errors and can also be found in other species besides K. pneumoniae Based on these data, we explore PCR-based sequencing and a masking strategy, which both successfully address these inaccuracies and ensure accurate outbreak tracing. We offer our masking strategy as a bioinformatic workflow (MPOA) to identify and mask problematic genome positions in a reference-free manner. Our research highlights limitations in using Oxford Nanopore Technologies for sequencing prokaryotic organisms, especially for investigating outbreaks. For time-critical projects that cannot wait for further technological developments by Oxford Nanopore Technologies, our study recommends either using PCR-based sequencing or using our provided bioinformatic workflow. Abstract

The fungus Candida albicans and the Gram-positive bacterium Enterococcus faecalis share mucosal niches in the human body. As opportunistic pathogens, both are found to expand population size during dysbiosis, and can cause severe systemic infections in susceptible individuals. Here, we show that the presence of C. albicans results in increased host cell damage by E. faecalis . Furthermore, E. faecalis aggravates oropharyngeal candidiasis in mice. Increased damage is mediated by enterococcal cytolysin, and involves both physical interaction and altered glucose availability. Physical interaction promotes accumulation of bacteria on host cells, facilitating contact of cytolysin with host cells. Glucose depletion by the metabolic activity of the fungus sensitized host cells to cytolysin. This work illustrates how a complex interplay between fungi and bacteria can result in detrimental consequences for the host.

Abstract

Healthcare-associated infections (HCAIs) represent an enormous burden for patients, healthcare workers, relatives and society worldwide, including Germany. The central tasks of infection prevention are recording and evaluating infections with the aim of identifying prevention potential and risk factors, taking appropriate measures and finally evaluating them. From an infection prevention perspective, it would be of great value if (i) the recording of infection cases was automated and (ii) if it were possible to identify particularly vulnerable patients and patient groups in advance, who would benefit from specific and/or additional interventions.To achieve this risk-adapted, individualized infection prevention, the RISK PRINCIPE research project develops algorithms and computer-based applications based on standardised, large datasets and incorporates expertise in the field of infection prevention.The project has two objectives: a) to develop and validate a semi-automated surveillance system for hospital-acquired bloodstream infections, prototypically for HCAI, and b) to use comprehensive patient data from different sources to create an individual or group-specific infection risk profile.RISK PRINCIPE is based on bringing together the expertise of medical informatics and infection medicine with a focus on hygiene and draws on information and experience from two consortia (HiGHmed and SMITH) of the German Medical Informatics Initiative (MII), which have been working on use cases in infection medicine for more than five years., (© 2024.
