Contains fulltext : 287484.pdf (Publisher’s version ) (Open Access) BACKGROUND: Iron content is increased in the substantia nigra of persons with Parkinson's disease and may contribute to the pathophysiology of the disorder. Early research suggests that the iron chelator deferiprone can reduce nigrostriatal iron content in persons with Parkinson's disease, but its effects on disease progression are unclear. METHODS: We conducted a multicenter, phase 2, randomized, double-blind trial involving participants with newly diagnosed Parkinson's disease who had never received levodopa. Participants were assigned (in a 1:1 ratio) to receive oral deferiprone at a dose of 15 mg per kilogram of body weight twice daily or matched placebo for 36 weeks. Dopaminergic therapy was withheld unless deemed necessary for symptom control. The primary outcome was the change in the total score on the Movement Disorder Society-sponsored revision of the Unified Parkinson's Disease Rating Scale (MDS-UPDRS; range, 0 to 260, with higher scores indicating more severe impairment) at 36 weeks. Secondary and exploratory clinical outcomes at up to 40 weeks included measures of motor and nonmotor disability. Brain iron content measured with the use of magnetic resonance imaging was also an exploratory outcome. RESULTS: A total of 372 participants were enrolled; 186 were assigned to receive deferiprone and 186 to receive placebo. Progression of symptoms led to the initiation of dopaminergic therapy in 22.0% of the participants in the deferiprone group and 2.7% of those in the placebo group. Summary: This study aimed to correlate REM sleep without atonia (RSWA) and neuropsychological data in patients with idiopathic/isolated REM sleep behaviour disorder (iRBD) and those with RBD associated with Parkinson's disease (PDRBD), in order to assess whether higher degrees of RSWA are related to poorer cognitive performance. A total of 142 subjects were enrolled: 48 with iRBD, 55 with PDRBD, and 39 PD without RBD (PDnoRBD). All participants underwent video‐polysomnographic recording, clinical and neuropsychological assessment. RSWA was quantified according to two manual scoring methods (Montréal, SINBAR) and one automated (REM atonia index, RAI). Mild cognitive impairment (MCI) was diagnosed according to diagnostic criteria for MCI in Parkinson's disease. The relationship between neuropsychological scores and RSWA metrics was explored by multiple linear regression analysis and logistic regression models. Patients with iRBD showed significantly lower visuospatial functions and working memory, compared with the others. More severe RSWA was associated with a higher risk of reduced visuospatial abilities (OR 0.15), working memory (OR 2.48), attention (OR 2.53), and semantic fluency (OR 0.15) in the iRBD. In the whole group, a greater RSWA was associated with an increased risk for depressive symptoms (OR 3.6). A total of 57(40%) MCI subjects were found (17 iRBD, 26 PDRBD, and 14 PDnoRBD). Preserved REM‐atonia was associated with a reduced odds of multi‐domain MCI in the whole study population (OR 0.54). In conclusion, a greater severity of RSWA was associated with an increased risk for poor cognitive performance and depressive mood in patients with RBD. Introduction: Neonatal lupus erythematosus (NLE) is characterized by the transplacental passage of maternal antibodies, mainly anti-Sjogren’s syndrome A/Ro (anti-SSA/Ro) and anti-Sjogren’s syndrome B/La (anti-SSB/La), which bind to the developing fetal tissues. Seropositive mothers may have a diagnosed disease, such as Sjögren’s syndrome, systemic lupus erythematosus, or undifferentiated connective tissue disease, but in 25─60% of cases, they are asymptomatic at the time of childbirth.Objectives: This article aimed to provide a narrative review of the current state of knowledge on NLE. Development: The clinical spectrum of NLE includes both reversible and irreversible features. Reversible clinical features, like cutaneous lesions, hematological and pulmonary abnormalities, and hepatobiliary dysfunction, disappear spontaneously as autoantibody levels decrease in the bloodstream. The most commonly reported NLE manifestation is cardiac disease. Injury to the cardiac conduction tissue may lead to defects, such as congenital atrioventricular (AV) block, which is the most severe and, in some cases, lethal NLE manifestation. NLE may also have an impact on the myocardium and endocardium, with maternal antibodies being responsible for endocardial fibroelastosis. Immune-mediated AV block may benefit from in-utero treatment with steroids. Hydroxychloroquine may also be prescribed to women with immune-mediated disorders in the preconception period. Conclusions: The management of infants affected with NLE should be performed in a tertiary care center. Postnatal pacemaker insertion may be indicated in the presence of irreversible AV block or heart failure. Erythrocyte or platelet transfusions, corticosteroids, or intravenous immunoglobulin may be indicated to treat severe disease manifestations., Introdução: O lúpus eritematoso neonatal caracteriza-se pela passagem transplacentária de anticorpos maternos, maioritariamente anti-síndrome de Sjögren A/Ro (anti-SSA/Ro) e anti-síndrome de Sjögren B/La (anti-SSB/La), que se ligam aos tecidos fetais em desenvolvimento. Mães seropositivas podem ter doença diagnosticada, nomeadamente síndrome de Sjögren, lúpus eritematoso sistémico, ou doença indiferenciada do tecido conjuntivo, mas em 25─60% dos casos são assintomáticas à data do parto. Objetivos: O objetivo deste estudo foi fazer uma revisão narrativa sobre o estado da arte do lúpus eritematoso neonatal.Desenvolvimento: As características clínicas do lúpus neonatal podem ser reversíveis ou irreversíveis. As características reversíveis, como lesões cutâneas, alterações hematológicas ou pulmonares e disfunção hepatobiliar, desaparecem espontaneamente à medida que os níveis de anticorpos circulantes diminuem, geralmente nos primeiros 6 a 8 meses de vida. A manifestação mais frequente de lúpus neonatal é doença cardíaca. A lesão do tecido de condução cardíaco pode resultar em bloqueio atrioventricular (AV) congénito, que constitui a manifestação mais grave e em alguns casos fatal de lúpus neonatal. A doença pode também ter impacto no miocárdio e endocárdio, dado que os anticorpos maternos podem causar fibroelastose endocárdica. O bloqueio AV imunomediado pode beneficiar do tratamento in utero com corticoides. Mulheres com patologia imunomediada podem também receber hidroxicloroquina no período pré-concecional. Conclusões: A abordagem de recém-nascidos com lúpus neonatal deve ser feita num centro terciário. Em presença de bloqueio AV irreversível ou sinais de insuficiência cardíaca, pode ser necessária a colocação de pacemaker após o nascimento. As manifestações graves da doença podem necessitar de transfusões de eritrócitos ou plaquetas, corticosteroides ou imunoglobulina endovenosa.

Relatório de Estágio de mestrado em Português Língua Não Materna - Língua Estrangeira e Língua Segunda, A fundamentação teórica sugere que a cultura não é algo genético, mas sim algo que nos é transmitido pelo meio onde estamos inseridos (Lévi-Strauss, 1974). Sendo o ensino, em sentido lato, uma atividade que visa promover a aprendizagem, a produção e transmissão de conhecimentos e considerando que a temática do ensino da cultura não é neutra, para a maioria dos autores, assumindo até um papel preponderante no ensino da LNM, tudo leva a crer que a cultura portuguesa poderá ser transmitida aos alunos através das aulas e dos manuais de português. Compreender o impacto do ensino da cultura, tendo em conta as várias conceções culturais, os manuais utilizados, os diferentes modelos de ensino adotados para o efeito e, essencialmente, como toda esta problemática é percecionada pelos alunos, torna-se assim absolutamente fundamental e de elevado interesse académico. Nesse sentido, foram analisados manuais direcionados para aulas de PLNM e aplicou-se um questionário aos alunos de Tercer Lengua: Portugués I. Os resultados obtidos com o questionário sugerem que a questão cultural tem interesse para a maior parte dos alunos (75%), sendo que 70%, consideram que é importante conhecerem a cultura da língua que estão a estudar e a totalidade dos alunos considera os manuais e cadernos de atividades como sendo os recursos mais relevantes para o ensino em sala de aula. Finalmente, da análise dos manuais de LNM selecionados foi possível constatar que todos adotam uma abordagem cultural., The theoretical foundation suggests that culture is not something genetic, but something that is transmitted to us by the environment where we are inserted (Lévi-Strauss). International audience; BACKGROUND: Deep brain stimulation of the sub-thalamic nucleus (DBS-STN) reduces symptoms in Parkinson’s disease (PD) patients with motor fluctuations. However, some patients may not feel ameliorated afterwards, despite an objective motor improvement. It is thus important to find new predictors of patients’ quality of life (QoL) amelioration after DBS-STN. We hypothesized that personality dimensions might affect QoL after DBS-STN. OBJECTIVE: To evaluate associations between personality dimensions and QoL improvement one year after DBS-STN. METHODS: DBS-STN-PD patients (n = 303) having answered the "Temperament and Character Inventory" (TCI) before surgery and the PDQ-39 before and one year after surgery were included, from the cohort study PREDI-STIM. Linear regression models were used to evaluate associations between TCI dimensions and change in PDQ-39 scores after DBS-STN. RESULTS: Novelty Seeking and Cooperativeness scores before surgery were positively associated with PDQ-39 scores improvement after DBS-STN (FDR-adjusted p < 0.01). Moreover, paradoxically unimproved patients with deterioration of their PDQ-39 scores after DBS-STN despite improvement of their MDS-UPDRS-IV scores had lower Cooperativeness scores, while paradoxically improved patients with amelioration of their PDQ-39 scores despite deterioration of their MDS-UPDRS-IV scores had higher Reward Dependence scores. CONCLUSION: Some presurgical personality dimensions were significantly associated with QoL amelioration and discrepancy between motor state and QoL changes after DBS-STN in PD. Educational programs before DBS-STN should take in account patient personality dimensions to better deal with their expectations.

Dissertação de mestrado integrado em Engenharia e Gestão Industrial, A presente dissertação realizou-se no âmbito da conclusão do Mestrado Integrado em Engenharia e Gestão Industrial (MIEGI), da Universidade do Minho, tendo como principais objetivos melhorar o processo de reparação interna e desenvolver ferramentas de monitorização operacional do mesmo num serviço pós-venda de um departamento de apoio ao cliente. O projeto nasceu da necessidade da empresa de monitorizar e gerir de forma mais controlada todos os processos associados ao departamento, bem como aumentar a eficiência do mesmo no que diz respeito à resposta dada ao cliente e, consequentemente, uma diminuição do tempo de permanência dos equipamentos no departamento. O projeto seguiu a metodologia Investigação-Ação e, na sua fase de diagnóstico, foram identificados os problemas, salientando-se de entre todos, a taxa de conclusão de apenas 87%, ou seja, dos processos que chegam ao departamento, apenas 87% são concluídos. Para além deste problema, a inexistência de indicadores de desempenho que permitam controlar todo o processo. Para todos os problemas foram propostas soluções que se dividem em quatro tópicos: alterações do processo de reparação interno, alterações no Sistema de Informação, melhoria do processo de escalonamento dos processos pelos técnicos e, por fim, desenvolvimento de dashboards para apoio à gestão do processos de reparação interno, relativamente ao controlo dos estados dos processos, nivelamento do trabalho pelos técnicos, entre outros. Em relação aos resultados, este projeto permitiu uma diminuição de aproximadamente 60% das deslocações dos técnicos efetuadas por processo e um aumento de 8% da taxa de conclusão dos processos. Adicionalmente, foram definidos novos indicadores de desempenho e a sua implementação no Sistema de Informação permitiu um maior controle do desempenho do processo., This dissertation was made as part of the conclusion of the course Integrated Master’s in Industrial Engineering and Management, of University of Minho, having as main goals the improvement of the internal reparation process and the development of operational monitoring tools in an after-sales department of an client support department. The project was born from the company need in monitor and manage in a more controlled way all the processes associated to this department, and also improve its efficiency with regard to answer given to the client and, consequently, a decrease on the permanence time of the department equipment. The project followed the Action-Research (AR) methodology and, in the diagnostic phase, were identified the problems, underlining the completion rate of only 87%, meaning that from the processes that arrived at the department, only 87% are finished. Beyond this problem, the non-existence of performance indicators that allow to control the all process. For all the problems were proposed solutions that are divided in four topics: intern reparation process changes, Information System changes, scheduling of the processes by the technicians improvement and, finally, dashboards development to support the intern reparation process management with regard to the process states control, technicians work levelling, among others. Regarding the results, this project allowed the decrease of approximately 60% of the technician’s travels made in each process, an improvement of 8% of the completion rate of the processes. Additionally, new performance indicators were defined and its implementation on the information’s system allowed a bigger control of the process performance.

Construction of housing buildings using refurbished shipping containers constitutes a successful recent building system. As construction modules, disposed and recycled containers are considered to contribute for a sustainable construction system. This article aims to contribute to a better understanding of this construction system, in particular, the architectural project. A methodology based on the typological and modular coordination is proposed as a key tool to help with the development of spatial arrangements by considering the shipping container as a unit/module. The functional occupation typologies were proposed to characterize the main housing activities to be considered in modular coordinated design, allowing to adapt the housing to the family needs according to their social and cultural dynamics. Este estudo enquadra-se numa perspetiva dos alunos com Necessidades Educativas Especiais do 3º e 4º ano, no âmbito da disciplina de Inglês, que se tornou obrigatória há 2 anos e visa compreender de que forma a legislação atual desta problemática é aplicada em diferentes instituições públicas de 1º ciclo. Neste sentido, adotei como principal objetivo compreender a postura destas instituições e dos professores de Inglês do 1º ciclo para com os alunos com Necessidades Educativas Especiais, tendo como suporte o Decreto- Lei nº 3/2008. É notório que continua a existir a necessidade de um maior empenho por parte de todos os agentes educativos, de forma a proporcionar a estes alunos um melhor percurso de aprendizagem. This study is framed in the perspective of the Special Educational Needs’ students in years 3 and 4, within the English subject which became compulsory 2 years ago and seeks to understand how the current legislation of this problematic is applied in different primary schools. In this sense, the main objective is to comprehend schools and English primary teachers’ attitudes towards the students with Special Educational Needs, backed up by the Decree-Law 3/2008. It is visible that there is still a need of a higher effort from the educational agents, in order to provide these students with a better learning path.

Banking crises, albeit rare, can have nefarious consequences. As such, it is relevant to understand not only what factors cause these crises, but what variables can be used in their early detection. Economic integration still requires country speci cities to be considered in macro-prudential monitoring. This project explores the dynamics of banking crises, the role of economic and stock market growth as warning variables for several European regions. Dynamic probit models are used in a panel dataset. Results show that real GDP, stock market growth and house price growth are good indicators of crisis, and separate models for regions within the Eurozone predict crisis more accurately.

Descrição do caso: O penfigóide gestacional é uma dermatose bolhosa da gravidez rara. Inicia-se no 2.º/3.º trimestre de gestação ou no pós-parto imediato. Apresentamos um caso clínico de uma mulher de 31 anos, 2G1P, grávida, vigiada na Unidade Funcional. Pertencente a família nuclear, na fase III do ciclo de Duvall e classe média de Graffar. Sem antecedentes pessoais ou familiares relevantes. Com 18 semanas e dois dias de gestação recorreu a consulta de intersubstituição por prurido generalizado, tendo sido medicada com um anti-histamínico oral de primeira geração. No entanto, por agravamento das queixas e aparecimento de vesículas periumbilicais, recorreu a várias instituições de saúde no espaço de um mês, com diagnósticos diversos. Por fim, foi feito o diagnóstico de penfigóide gestacional e iniciou tratamento com corticoterapia endovenosa. Durante a gravidez realizaram-se várias tentativas de redução da dose do corticoide oral com agravamento das lesões, o que impossibilitou a sua suspensão. No 3.º dia pós-parto houve novo agravamento do quadro clínico. Atualmente, a utente mantém lesões que agravam com a tentativa de suspensão da corticoterapia. Sublinha-se a importância do exame objetivo no diagnóstico, bem como de um elevado índice de suspeição, especialmente importante em patologias menos frequentes como o caso descrito, para que a grávida seja orientada adequadamente. O médico de família deve igualmente estar atento à possibilidade da recorrência da dermatose numa gestação seguinte que é, habitualmente, mais precoce e mais grave. Case description: Pemphigoid gestationis is a rare autoimmune bullous disease occurring in pregnant women. The disease presents in the second or third trimester of pregnancy or during the postpartum period. A 31 year-old woman, G2P1, at 18 weeks and two days of gestation, presented at an urgent consultation with generalized pruritus. She was treated initially with a first generation oral anti-histaminic without improvement. When symptoms worsened and a periumbilical eruption appeared, the patient consulted with several doctors in different institutions and received a number of different diagnoses. One month later, she was diagnosed with pemphigoid gestationis and began treatment with intravenous corticosteroids. During the pregnancy, several attempts were made to reduce the dose of oral corticosteroids without success. On the third day postpartum there was a flare-up of the disease. The patient still presents with skin blisters to this day. It is important for family physicians to be aware of this condition, to know how to perform a relevant clinical examination, and to refer the pregnant woman with these complaints for additional testing to exclude other diagnoses. It is also important to be alert to the possibility of recurrence in subsequent pregnancies, in which the condition may appear earlier and with greater severity.

31. Papel da memantina no tratamento das perturbações da ansiedade: uma revisão baseada na evidência

RESUMO Objetivo Avaliar a eficácia da memantina no tratamento das perturbações da ansiedade. Métodos Foi realizada uma revisão sistemática na Medline, outras bases de dados baseadas na evidência e Índex das Revistas Médicas Portuguesas, nos últimos 10 anos, em inglês, espanhol e português, utilizando os seguintes termos “MESH memantine”, “treatment” e “anxiety disorders”. Para avaliar a qualidade dos estudos e a força de recomendação, foi utilizada a escala de Strength of Recommendation Taxonomy da American Family Physician. Resultados Obtiveram-se 131 artigos, dos quais seis cumpriam os critérios de inclusão: dois ensaios clínicos aleatorizados e controlados (ECAC), uma revisão sistemática (RS), uma norma de orientação clínica (NOC) e dois ensaios clínicos não controlados (ECNC). A NOC não recomenda o uso de memantina no tratamento da perturbação generalizada da ansiedade (PGA) (SOR C), mas recomenda o uso da memantina como tratamento adjuvante de segunda linha na perturbação obsessivo-compulsiva (POC) e como terceira linha na perturbação do estresse pós-traumático (SORT C). Os dois ECAC sugerem que o tratamento adjuvante com memantina melhorou a sintomatologia dos pacientes com POC severa. A RS revelou que o uso da memantina tem alguns benefícios na diminuição dos sintomas da POC. Os dois ECNC sugerem que a memantina pode ser eficaz como tratamento adjuvante em pacientes sintomáticos apesar de tratamento adequado com antidepressivos convencionais. Conclusão A evidência atual sugere que o uso de memantina como adjuvante no tratamento da POC parece ser eficaz (SORT B). O uso de memantina no tratamento da PGA não é recomendado (SORT C). ABSTRACT Objective Review the evidence on the efficacy of memantine in the treatment of the anxiety disorders. Methods A systematic review of articles published in the last ten years, in Portuguese, English and Spanish, was performed in Medline, other places of Evidence Medicine and Index of Portuguese Databases, using the following MESH terms: memantine, treatment and anxiety disorders. We used the Strength of Recommendation Taxonomy (SORT) scale of American Family Physician to assign levels of evidence and strength of recommendations. Results A total of 131 papers were identified, and after the application of inclusion criteria, 6 papers remained, including two randomized controlled trials (RCT), one clinical guideline (CG), two open clinical trials and one systematic review (SR). The CG does not recommend the use of memantine in the treatment of the generalized anxiety disorder (SORT C) but does recommend the use of memantine as second-line adjunctive therapy for obsessive compulsive disorder and as third-line therapy for post-traumatic stress (SORT C). The two RCT suggest that an add-on of memantine does improve the symptoms of patients suffering from severe obsessive compulsive disorder. The SR showed that the use of memantine had some benefit in relieving obsessive compulsive disorder symptoms. The two other studies showed memantine may be an effective augmentation therapy in patients with anxiety who remain symptomatic despite adequate treatment with conventional antidepressant anxiolytics. Conclusion The current available evidence suggests the benefit of memantine as an adjunctive therapy for the treatment for obsessive compulsive disorder (SORT B). Most studies does not show the benefit of the use of memantine for the treatment of generalized anxiety disorder (SORT C).

A esgana canina é uma doença cosmopolita e enzoótica causada por um vírus da família Paramixoviridae e do género Morbillivirus. Não existem, atualmente, medicamentos antivirais específicos no combate ao vírus da esgana, pelo que o seu tratamento é apenas sintomático. Esta doença provoca, frequentemente, sequelas neurológicas decorrentes de um processo de apoptose e desmielinização dos oligodendrócitos, astrócitos e neurónios. As células estaminais são células com capacidade de diferenciação, autorrenovação e proliferação. De entre estas, distinguem-se as de origem mesenquimatosa que podem diferenciar-se em células de linhagem mesodérmica e precursoras de tecido conjuntivo. As células estaminais mesenquimatosas podem ser obtidas a partir do tecido adiposo, permitem a regeneração de tecidos lesados, têm capacidade imunomoduladora e, por este motivo, têm interesse terapêutico. O presente estudo teve como objetivo avaliar o potencial das células estaminais derivadas do tecido adiposo no tratamento de sequelas neurológicas da esgana. Este trabalho foi realizado durante o período entre janeiro e junho de 2015, numa população constituída por 7 cães com sequelas neurológicas da esgana e submetidos a terapia celular em Campinas (Brasil). As células estaminais mesenquimatosas foram obtidas no laboratório da Regenera Stem Cells a partir do tecido adiposo de cães dadores jovens e saudáveis. A terapia celular consistiu na administração destas células a cada 30 dias, por via intravenosa, perfazendo na maioria dos casos clínicos estudados, um total de 3 aplicações. Os 7 cães estudados eram de idades e raças distintas, sendo que 4 eram machos e 3 eram fêmeas. As sequelas observadas com maior frequência foram alteração da locomoção, mioclonias, convulsões e ataxia vestibular. No final da terapia celular, 3 animais apresentavam ausência total das sequelas previamente observadas, tendo recuperado completamente. Nos restantes 4 animais verificou-se uma recuperação parcial. As mioclonias, a ataxia cerebelar e a ataxia vestibular foram consideradas as sequelas neurológicas com menor possibilidade de recuperação, ou seja, com pior prognóstico. Os resultados obtidos apontam a terapia celular como uma opção promissora no tratamento ou na melhoria da qualidade de vida de animais com sequelas da esgana., Canine distemper is a cosmopolitan and enzootic disease caused by a virus of the family Paramyxoviridae and genre Morbillivirus. Currently, there are not specific antiviral drugs to combat the distemper virus, and therefore its treatment is only symptomatic. Frequently, this disease causes neurological sequelae resulting from a process of apoptosis and demyelination of oligodendrocytes, astrocytes and neurons. Stem cells have capacity for differentiation, self-renewal and proliferation. Among these, those with mesenchymal origin are distinguished and can be distinguished into mesodermal lineage cells and connective tissue precursor. The mesenchymal stem cells can be obtained from adipose tissue, allow the regeneration of damaged tissues, have immunomodulatory capacity and, for this reason, they have therapeutic interest. This study aimed to evaluate the potential of stem cells derived from adipose tissue in the treatment of distemper neurological sequelae. This work was carried out between January and June 2015, in a population composed by seven dogs with distemper neurological sequelae and submitted to cell therapy in Campinas (Brazil). The mesenchymal stem cells were obtained in the laboratory of Regenera Stem Cells from adipose tissue of young and healthy donours dogs. The cell therapy consisted in the intravenous administration of these cells, every thirty days, meaning a total of three applications in most of the studied clinical cases. From the seven studied dogs with different ages and races, four were males and three were females. The most frequently observed sequels were locomotion changes, myoclonus, seizures and vestibular ataxia. At the end of cell therapy, three animals didn’t show any of the sequelae previously observed, having completely recovered. The remaining four animals partially recovered. The myoclonus, cerebellar ataxia and vestibular ataxia were considered the neurological sequelae with lower chance of recovery, that is, with worse prognosis. - Abstract
Parkinson’s disease (PD) negatively affects patients’ Quality of Life (QoL) which depends on both objective criteria such as physical health and subjective ones such as worries and norms according to personal believes. Therefore, QoL could be also associated to personality dimensions in chronic neurological diseases such as PD. Our objective was thus to study the potential association between personality dimensions and QoL in PD patients with motor fluctuations before Deep Brain Stimulation of the Sub-Thalamic Nucleus (DBS-STN). Data were obtained from the French multicentric cohort study Predi-Stim. All PD patients awaiting DBS-STN and responding to the inclusion criteria at the time of the study were included. All participants answered the “Temperament and Character Inventory” (TCI) and the PDQ-39 before surgery. Analyses were made using adjusted univariate generalized linear regression models to evaluate a potential association between TCI dimensions and PDQ-39 scores. Three hundred thirty-three consecutive patients were included. The temperament Harm Avoidance was negatively associated with QoL (p = 1e-4, R2= 0.33), whereas the character Self-Directedness was positively associated with mental component of QoL (p = 2e-4, R2= 0.33) in PD patients with motor fluctuations awaiting DBS-STN. PD patients with motor fluctuations, with lower Harm Avoidance and higher Self-Directedness scores have the best QoL mainly at an emotional and social level. Therapeutic education of these PD patients focusing on their personal resources may thus be important to improve their well-being.

- Abstract
Background: Whether reserve plays a role in Parkinson's disease (PD) patients has received less attention than in dementia and has been mainly examined in relation with cognitive function.Objective: To investigate whether reserve plays a role in the severity and progression of motor, cognitive, and nonmotor PD symptoms by examining whether education level (proxy of reserve) is associated with baseline performance and rate of progression.Methods: We used data from a longitudinal cohort of PD patients (≤5-year disease duration at baseline) annually followed up to 5 years (n = 393; 41% women; mean age = 62.3 years, standard deviation = 10.0; mean disease duration = 2.6 years, standard deviation = 1.5). We examined the relationship of education with time to reach Hoehn and Yahr stage ≥3 using Cox regression and with baseline severity and progression of motor (Movement Disorder Society-Unified Parkinson's Disease Rating Scale parts II and III, gait speed), cognitive (Mini-Mental State Examination), and nonmotor (depression, anxiety, nonmotor symptoms scale, quality of life) symptoms using mixed models.Results: Education level was not associated with age at onset or diagnosis. Compared with the low-education group, the incidence of Hoehn and Yahr ≥3.0 was 0.42 times lower (95% confidence interval, 0.22-0.82, P = 0.012) in the high-education group. Higher education was associated with better baseline motor function (P < 0.001), but not with the rate of motor decline (P > 0.15). Similar results were observed for cognition. Education was not associated with nonmotor symptoms.Conclusions: Higher education is associated with better baseline motor/cognitive function in PD, but not with rate of decline, and with a lower risk of reaching Hoehn and Yahr ≥3 during the follow-up. O presente estudo teve como tema Conhecer o Contributo das Unidades de Ensino Estruturado (UEE), para o Desenvolvimento Global da Criança com Perturbação do Espetro do Autismo (PEA) cujo objetivo foi Compreender como a Metodologia TEACCH contribui para o desenvolvimento global das crianças com PEA, nomeadamente a nível da autonomia, da comunicação e da socialização. A Dissertação apresentada encontra-se dividida em duas partes: Parte I – Enquadramento Teórico, onde foi feito uma pesquisa bibliográfica que serviu de suporte ao trabalho. Parte II – A Metodologia desenvolvida na prática. Apresentação, Análise e Discussão dos Resultados; Conclusões, Sugestões de Melhoria e Bibliografia. Neste contexto, estudámos e observámos numa UEE a metodologia TEACCH aplicada numa escola do 1º Ciclo do Ensino Básico. Foram observados e as rotinas e comportamentos dos alunos com PEA, as suas dificuldades, as suas evoluções e aquisições. Paralelamente, entrevistámos os professores de Educação Especial que com eles trabalharam. Os participantes neste estudo foram as seis crianças com PEA que frequentam a UEE e os professores de Educação Especial que nela trabalham. A recolha de dados foi feita através de observações na Sala TEACCH e da realização de entrevistas semi-estruturadas aos professores. Foram apresentados e analisados os dados obtidos e retiraram-se conclusões dos mesmos. Como resultado principal podemos afirmar que a metodologia TEACCH contribui para o desenvolvimento das crianças com PEA ajudando-as a ser mais autónomas e mais integradas no mundo que as rodeia. The theme of the present study was to Know the Contribution of the Structured Teaching Units (STU) to the Global Development of the Child with Autism Spectrum Disorder(ASD), whose main goal was to Understand how the TEACCH Methodology contributes to the global development of children with ASD, specifically at the autonomy, communication and socialization levels. The presented Thesis is divided into two parts: Part I – Theoretical Context, in which it was made a bibliographical research that was the foundation of the essay. Part II – The Methodology developed in practice. Presentation, Analysis and Discussion of the Results; Conclusions; Improvement Suggestions and Bibliography. In this context, we studied and observed in a STU the Methodology TEACCH applied in a 1st cycle School. The routines and the behaviours of the pupils with ASD were observed, their difficulties, their evolutions and acquisitions. At the same time, we interviewed the Special Education Teachers who worked there. The data gathering was done through observations in the TEACCH room and through the conduction of semi-structured interviews to the teachers. The obtained data were presented and analysed and conclusions were drawn from those data. As a main result, we can state that the TEACCH Methodology contributes to the development of the children with ASD, helping them to be more autonomous and more integrated in the world that surrounds them.

Dissertação de Mestrado em Matemática apresentada à Faculdade de Ciências e Tecnologia da Universidade de Coimbra Neste trabalho sobre a utilização de supernúcleos na estimação duma densidade de probabilidade, depois duma breve introdução aos estimadores do núcleo da densidade de probabilidade, estabelecemos a convergência em média quadrática integrada de tais estimadores e obtemos expressões assintóticas para o erro quadrático médio integrado nos casos em que o estimador do núcleo é baseado num núcleo de ordem finita k ≥ 2 ou num supernúcleo. Tendo em conta que os estimadores baseados num supernúcleo não são densidades de probabilidade pois podem tomar valores negativos, estudamos a seguir uma correcção proposta por Glad, Hjort e Ushakov (Scandinavian J. Statist. 30, 415–247, 2003) que permitirá transformá-los em estimadores próprios da densidade tendo o estimador corrigido um erro quadrático médio integrado não superior ao do estimador original. Finalmente, apresentamos um estudo de simulação com o objectivo de comparar os estimadores do núcleo baseados no núcleo normal (núcleo 2.a ordem) e no núcleo trapezoidal (supernúcleo). In this work on the use of superkernels in the estimation of a probability density function, after a brief introduction to the probability density kernel estimator we establish its mean integrated square error consistency and we obtain asymptotic expressions for the mean integrated square error of kernel estimators based on either finite order kernels or superkernels. Taking into account that superkernel based estimators are not proper density estimators because they may assume negative values, we study a correction proposed by Glad, Hjort and Ushakov (Scandina- vian J. Statist. 30, 415-–247, 2003), that turns any density estimator which integrates to 1 into one which is a proper density estimator with an inferior integrated square error. Finally, we present a simulation study in order to compare the estimators based on the normal kernel (second order kernel) and on the trapezoidal kernel (superkernel).

Dissertação para obtenção do Grau de Mestre em Engenharia Química e Bioquímica The core of this work is a piezoelectric biosensor in which acoustic waves are launched in very thin PVDF polymer films to produce an oscillatory resonant device. The essence of the device consists in a polymer film system made of a piezoelectric polymer, PVDF, responsible for the film oscillation and a porous membrane, Immobilon, a special type of porous PVDF with protein binding capacity which can act as a biosensitive area. The possibility of using a film system composed only by PVDF was studied. Due to its strong hydrophobic nature, surface modification was aimed to be performed by coating a functional layer on the membrane surface, in order to improve the hydrophilicity and biocompatibility of PVDF. An immersion method was preformed and applied to porous and non-porous PVDF membranes. Three distinct coating solutions were studied, namely, polyethylene glycol (PEG), 3,4-Dihydroxy-DL-phenylalanine (DL-DOPA) and Gum Arabic solutions. Hydrophilicity improvements of the membranes were characterized by water contact angle measurements and its elemental composition was studied by elemental microanalysis. BSA protein marked with FITC fluorescein was used to perform quantitative and qualitative assays in order to study its adsorption to coated and uncoated PVDF membranes. Globally, the best results were obtained when a solution of PEG was utilized with 0.2 μm microporous membrane. The sensor response was tested with polyethylene glycol (PEG) and bovine serum albumin (BSA), using a film system composed by PVDF/0.2 μm Immobilon/PVDF, which was able to respond to the presence of both compounds in liquid medium.

47. Caesarean Birth vs. Vaginal Delivery: Effects upon Maternal Satisfaction, Maternal Confidence and Maternal Perception of the Newborn Behaviour

In order to test the hypothesis that caesarean birth has negative consequences upon newly mothers’ satisfaction and perceptions, women delivering by caesarean birth (WCB) were compared with women delivering by vaginal birth (WVB). Subjects: 180 newly mothers; 93 WCB and 87 WVB. Instruments: A Socio-Demographic Questionnaire developed for this research, the Childbirth Perceptions Questionnaire and the Mother and Baby Scales. Results: WCB had significantly lower scores in perceptions of baby as alert/responsive and nearly significant lower scores for baby as alert during feeds. WVB showed a significantly higher level of satisfaction with delivery and conduct during labour, as also had significantly lower scores for perceptions of baby as irritable during feeds and for lack of confidence in feeds. After controlling for the kind of anesthesia received in labour, three conclusions must be taken into account: 1) between WCB with regional anaesthesia and WCB with general anaesthesia there is only one significant difference, with the former having higher scores for perception of baby as alert during feeds; 2) between WVB with regional anaesthesia and WVB with no anaesthesia there are only two significant differences, with the former having higher scores for lack of confidence in feeding and having lower scores for global confidence; 3) between WCB with regional anaesthesia and WVB with regional anaesthesia four significant differences emerge, with the former having a lower level of satisfaction with delivery and conduct in labour and having lower scores for perception of baby as alert responsive, and also having higher scores of perception of baby as irritable in feeds and higher scores for lack of confidence in feeding. Data seem compatible with the hypothesis that caesarean birth has some negative consequences upon mothers’ satisfaction and perceptions and, for this reason, psychological surveys should constitute a routine procedure in maternity hospitals, especially when newly mothers pertain to families affected by risks of psychological or social nature.

Mestrado em Química Analítica e Controlo de Qualidade Esta tese descreve a investigação realizada sobre vários métodos possíveis para a degradação do herbicida glifosato. Assim, foram feitas experiências de degradação fotocatalítica com TiO2 para este herbicida. Foi estudado o efeito da adição de iões metálicos como o Fe(III) e o Cu(II) e também de oxidantes como o H2O2. Foram também investigadas as concentrações óptimas destes adjuvantes. O estudo da fotodegradação deste herbicida com oxidantes como o H2O2, o K2S2O8 e KBrO3 também foi realizado. Foram realizadas experiências com o intuito de estudar a fotodegradação do glifosato com o sistema ferro-oxalato. Reacções de tipo Fenton foram realizadas em diferentes condições (com agitação, com radiação UV, e com ultrasons). Pelas experiências realizadas, a fotodegradação do glifosato com TiO2 degradou 44,8% da amostra numa hora. Ao adicionar-se pequenas quantidades de Fe(III) e Cu(II) este valor aumenta para 62% e 86% respectivamente, sendo este último, um dos métodos que se revelou mais eficaz na fotodegradação do glifosato. O sistema ferro-oxalato revelou resultados ligeiramente inferiores aos resultados da fotodegradação só com TiO2 (39% de fotodegradação numa hora). A fotodegradação do glifosato com oxidantes revelou ser o mais eficaz tendo-se obtido uma degradação do glifosato de 100% numa hora com o KBrO3 (10 mM), no entanto este método é mais prejudicial para o ambiente. As reacções de Fenton quando realizadas com radiação UV e Cu(II), foram quase tão eficazes como a fotodegradação do glifosato com TIO2 na presença de Cu(II) e radiação UV. ABSTRACT: This work describes the investigation about several possible methods for the gliphosate degradation in aqueous dispersion. With this purpose, TiO2 assisted photocatalysis experiments were made forthis herbicide. It was studied the effect of the adition of metallic ions like Fe(III) and Cu(II) and oxidants like H2O2. it was also investigated the optimum concentration for these adjuvants. The photodegradation study for this herbicide with oxidants like H2O2, K2S2O8 and KBrO3 was also made. Experiments were made with the purpose of studying the gliphosate photodegradation with ferrioxalate system. Fenton reactions alike were tested in different conditions (with agitation, UV radiation or ultrasound). By the results of the photodegradation of glyphosate with TiO2 it was accomplished 44,8% of degradation of the sample in 1 hour. Adding small amounts of metallic ions like Fe(III) and Cu(II) this value increases to 62% and 86% respectively, so this last method is one of the more efficient for the degradation of glyphosate. The ferrioxalate system showd slightly smaller results (39% of degradationin 1 hour) comparing to TiO2 assisted photocatalysis. In photodegradation with oxidants the results revealed the highest degradation rate. It was accomplished 100% of degradation in 1 hour for KBrO3 (10 mM), but this method leads to environmental contamination. Fenton reactions with UV radiation and Cu(II),were almost as efficient as TiO2 assisted photocatalysis with Cu(II).
