Abstract

Background: General physiotherapy is a common means of rehabilitation after surgery for proximal humeral fracture (PHF). Better-targeted exercises seem worthy of investigation and the aim of this study was to assess the efficacy of a rehabilitation program including task-oriented exercises in improving disability, pain, and quality of life in patients after a PHF. Methods: By means of a randomized controlled trial with one-year follow-up, 70 working patients (mean age of 49 ± 11 years; 41 females), who were selected for open reduction and internal fixation with plates caused by PHF, were randomized to be included in an experimental (n = 35) or control group (n = 35). There was a permuted-block randomization plan, and a list of program codes was previously created; subsequently, an automatic assignment system was used to conceal the allocation. The first group underwent a supervised rehabilitation program of task-oriented exercises based on patients’ specific job activities, and occupational therapy. The second group underwent general physiotherapy, including supervised mobility, strengthening and stretching exercises. Both groups individually followed programs of 60-min session three times per week for 12 weeks in the outpatient setting. The Disability Arm Shoulder Hand questionnaire (DASH; scores range from 0 to 100; primary outcome), a Pain intensity Numerical Rating Scale (scores range 0 to 10; secondary outcomes), and the Short-Form Health Survey (scores range from 0 to 100; secondary outcomes) assessed the interventions. Participants were evaluated before surgery, before and after rehabilitation (primary endpoint), and at the one-year follow-up (secondary endpoint). A linear mixed model analysis for repeated measures was carried out for each outcome measure (p < 0.05). Abstract

To analyze risk factors for type 2 diabetes among women attending menopause clinics in Italy for counselling about the menopause.Women attending a network of first-level outpatient menopause clinics in Italy for general counselling about menopause or treatment of menopausal symptoms.Cross-sectional study with no exclusion criteria. Type 2 diabetes was defined according to National Diabetes Data Groups Indications and the fasting blood glucose at an oral glucose tolerance test within the previous year.Out of the 44 694 considered in this analysis, 808 had a diagnosis of diabetes type 2 (1.8%). In comparison with women aged50 years, the multivariate odds ratios (OR) of type 2 diabetes were 1.31 (95% confidence interval (CI), 0.99-1.74) for women aged 50-52 years, 1.66 (95% CI, 1.27-2.17) at 53-56 years and 2.84 (95% CI, 2.20-3.67) in women agedor = 57 years. Type 2 diabetes was less frequently reported in more educated women (OR high school/university vs. primary school = 0.44 (95% CI, 0.36-0.55)). Being overweight was associated with an increased risk of type 2 diabetes. In comparison with women reporting a low level of physical activity, the multivariate OR of type 2 diabetes was 0.67 (95% CI, 0.54-0.84) for women reporting regular physical activity. In comparison with premenopausal women, the multivariate OR of type 2 diabetes was 1.38 (95% CI, 1.03-1.84) in women with natural menopause. This finding was present also after allowing for the potential confounding effect of age. The multivariate OR of diabetes for users of hormonal replacement therapy was 0.58 (95% CI, 0.46-0.73).This large cross-sectional study suggests that postmenopausal women are at higher risk of type 2 diabetes after allowance for the effect of age. Other main determinants of risk of type 2 diabetes in women around menopause were low socioeconomic status and being overweight. Diabetes was found less frequently in those taking hormone replacement therapy.

Abstract

OBJECTIVE To obtain data on correlates of climacteric symptoms in women around menopause attending menopause clinics in Italy. METHODS Since 1997 a large cross sectional study has been conducted on the characteristics of women around menopause attending a network of first level menopause outpatient's clinics in Italy. A total of 66,501 (mean age 54.4 years) women are considered in the present paper. RESULTS The odds ratios of moderate and severe hot flashes/night sweats were lower in more educated women and (for severe symptoms only) in women reporting regular physical activity. Depression, difficulty to sleep, forgetfulness and irritability tended to be less frequent in more educated women and (depression only) in women reporting regular physical activity. Parous women reported more frequently these symptoms. CONCLUSIONS This large study confirms in Southern European population that low education, body mass index and low physical activity are associated with climacteric symptoms. Parous women are at greater risk of psychological symptoms.

Abstract

Almost spherical CoFe2O4 nanocrystals with a narrow size distribution centered around 2.5(5)nm, uniformly dispersed in an amorphous SiO2 matrix have been prepared by high energy ball milling followed by thermal treatment at 900°C. Mössbauer spectra show the presence of CoFe2O4 nanoparticles in the superparamagnetic state at 293K and mainly in the blocked state at 77K. This is confirmed by ZFC/FC measurements giving evidence of a distribution of blocking temperatures, centered at 50K.

Abstract

Non-Stoichiometric CoFe2O4 nanoparticles dispersed in an silica matrix with a silica content of 87 wt% and Co/Fe molar ratio of 1:1, were prepared by the sol-gel method using an ethanolic solution of tetraethoxysilane and either iron(III) and cobalt(II) nitrates or iron(II) and cobalt(II) acetates. The influence of different metal precursors on the xerogels were examined by X-Ray Diffraction (XRD), Transmission Electron Microscopy (TEM) and N2 physisorption measurements at 77 K. Magnetic properties of the samples were investigated by field cooled FC and zero field cooled ZFC measurements. Depending on the metal precursor, different spinel oxides of a few nanometers were observed in the samples treated at 350°C. After heating at 900°C non-stoichiometric CoFe2O4 was formed in both samples, whose average particle size was only slightly larger than in samples treated at 350°C. [ABSTRACT FROM AUTHOR]

49. Iron−Cobalt Alloy Nanoparticles Embedded in an Alumina Xerogel Matrix

Abstract

Nanocrystalline γ-Al2O3 and FeCo−Al2O3 nanocomposite xerogels with high surface areas and pore volumes were prepared from alcogels obtained by a fast sol−gel procedure. The formation of γ-Al2O3 occurs via a sequence of stages starting from a disordered pseudo-bohemite phase which around 700 °C gives rise to amorphous allumina; this progressively cristallizes as γ-Al2O3, which is stable up to 1100 °C, when microcrystalline α-Al2O3 becomes the dominant phase; in the range 1000−1200 °C minor traces of δ-Al2O3 and θ-Al2O3 are present. Xerogels containing iron and cobalt are amorphous up to 700 °C; calcination at 800 °C gives rise to a spinel phase similar to γ-Al2O3 where metal ions partially fill the vacancies; at 1000 °C γ-Al2O3 progressively disappears to form α-Al2O3 and Co(Fe)Al2O4, which are the only phases present at 1200 °C. Reduction in hydrogen flow of the xerogel, previously calcined at 450 °C, leads to a nanocomposite constituted of FeCo alloy nanoparticles around 10 nm dispersed into α-Al2O3 nanocrystalline matrix. Zero-field-cooled (ZFC) and FC magnetic curves are typical of superparamagnetic materials and indicate the occurrence of high-strength particle interactions.

Abstract

(FexCo1-x)y(SiO2)1-y nanocomposites were prepared by a sol−gel method, using tetraethoxysilane and metal acetates as precursors. Two sets of xerogel samples were prepared:  in the first set x was kept equal to 0.5 and four y values were selected in the range 0.02−0.2; in the second set y was kept equal to 0.1 and three samples with x equal to 0.45, 0.50, and 0.70 were prepared. The samples were characterized by X-ray diffraction, TEM, and HREM to verify the formation of the FeCo alloy nanocrystals and to investigate their structure, morphology, and composition. All samples contain spherical FeCo alloy nanoparticles uniformly dispersed in the silica matrix with average size increasing with metal content. Magnetic measurements carried out on selected samples show a superparamagnetic behavior, with blocking temperature which depends on metal load, as a consequence of particle distance variation.
